Resistance training (RT) when associated with the caloric restriction diet (CR) has been shown to have important effects on blood pressure and the aortic artery, minimizing complications due to menopause, such as hypertension. Aim: In this study, we investigated the effects of Ovariectomy and the Resistance Training, Caloric Restriction, and the association of both intervention in systolic blood pressure, matrix metaproteinase-2 activity, aortic vascular reactivity and morphological alterations in ovariectomized Holtzman rats. Material and methods: Fifty female Holtzman rats were subjected to ovariectomy and Sham surgery and distributed into: Sham-sedentary; Ovariectomized-sedentary; Ovariectomized- Resistance Training; Ovariectomized-Caloric Restriction; and Ovariectomized-Resistance Training and Caloric Restriction groups. Resistance Training and 30% Caloric Restriction protocols were performed by 13-weeks. Systolic blood pressure, acetylcholine, and sodium nitroprusside induced relaxation of aortic rings; the hematoxylin-eosin stain used to highlight the aortas wall cross-sectional area and the matrix metaproteinase-2 activity, insulin tolerance test and aorta vessel remodeling were analyzed. Results: We observed that, Ovariectomy increased systolic blood pressure, decreased potency in dependent and independent endothelium relaxation and matrix metaproteinase-2 activity, promoted insulin resistance and aorta vessel remodeling in cross-sectional area, media to lumen ratio, collagen content and alter structure and elastic fibers of the vessel. However, the association of exercise and diet prevent the increase systolic blood pressure, improved the relaxation potency in dependent and independent endothelium relaxation, elevated the matrix metaproteinase-2 activity, increased the aorta cross-sectional area, media to lumen ratio, decreased collagen content and alter elastic fibers in aorta vessel wall. Conclusion: Our study reveled that the association of resistance training and caloric restriction can prevent the hypertensive condition, improve vascular function, activate matrix metaproteinase-2, and produce a beneficial hypertrophic remodeling with increased vascular compliance in ovariectomy in Holtzman rats. / Introdução: A hipertensão é um importante fator de risco para doenças cardiovasculares na menopausa. O treinamento resistido (TR) quando associado à dieta de restrição calórica (RC) têm demonstrado importantes efeitos sobre a pressão arterial e a artéria aorta, minimizando complicações decorrentes da menopausa, como a hipertensão arterial. Objetivo: O presente trabalho tem como objetivo investigar os efeitos da ovariectomia e das intervenções de TR, RC e da associação de ambos (TR+RC) sobre a pressão arterial sistólica, reatividade vascular aórtica, resposta insulínica, atividade da metaloproteinase de matriz 2 e alterações morfológicas em ratas Holtzman. Materiais e métodos: Cinquenta ratas Holtzman foram submetidas à cirurgia de ovariectomia e Sham; e distribuídas nos seguintes grupos (n=10): Sham-sedentária, Ovariectomizada-sedentária, treinamento resistido ovariectomizada, restrição calórica ovariectomizada e treinamento resistido associada à restrição calórica ovariectomizada. Os protocolos de TR e 30% de RC foram realizados por 13 semanas. A pressão arterial sistólica, teste de tolerância à insulina, reatividade vascular em anéis aórticos, atividade da metaloproteinase de matriz 2, análises morfológicas utilizando coloração de Hematoxilina e Eosina, Tricrômio de Masson e Pricrosírius-red foram realizadas. Resultados: Observamos que a ovariectomia aumentou a pressão arterial sistólica, promoveu resistência à insulina, diminuiu a potência do relaxamento dependente e independente do endotélio e atividade da metaloproteinase de matriz 2, além de promover o remodelamento do vaso aórtico sobre a área da secção transversal, relação túnica média/lúmen, alterando o conteúdo de colágeno e fibras elásticas da artéria aorta. No entanto, a associação entre exercício e dieta e ambos separados, preveniram o aumento da pressão arterial sistólica, melhoraram a potência do relaxamento dependente e independente do endotélio, promoveram ativação da metaloproteinase de matriz 2, aumento da área da secção transversal da aorta, da relação túnica média/lúmen, além de promover a diminuição do conteúdo de colágeno e alteração das fibras elásticas na parede do vaso aórtico. Conclusão: O presente estudo mostrou que os efeitos do TR e RC, associados ou realizados separadamente, foram eficientes para melhorar os parâmetros clínicos e moleculares, prevenindo o surgimento da condição de hipertensão, melhorando a função vascular, a resposta insulínica, a atividade da metaloproteinase de matriz 2 e o remodelamento da aorta com aumento da complacência vascular, na condição de hipertensão induzida pela ovariectomia em ratas Holtzman. / FAPESP: 2012/21087-4

Associação da disfunção diastólica de origem hipertensiva com a atividade simpática cardíaca e periférica / Association of diastolic dysfunction of hypertensive origin with cardiac and peripheral sympathetic activity

INTRODUÇÂO: A hipertensão arterial sistêmica (HAS) é uma condição clínica com alta prevalência, sendo considerada como o principal fator de risco modificável para o desenvolvimento de insuficiência cardíaca (IC). Dentre os mecanismos relacionados à progressão da HAS para a IC, a hiperatividade simpática e a disfunção endotelial devem ser consideradas. OBJETIVO: Avaliar a modulação do sistema nervoso autônomo (central e periférico), e a função endotelial em pacientes hipertensos com diferentes graus de disfunção diastólica (DD) do ventrículo esquerdo (VE). CASUÍSTICA E MÉTODO: Quarenta e cinco pacientes com HAS, sem outras co-morbidades foram submetidos ao exame de ecoDopplercardiograma tecidual, e foram alocados em três grupos: (GHT) sem alteração funcional ou estrutural cardíacas (n=15, 7 homens, 48±2 anos, IMC 28±1 Kg/m2), (GDD-ar) com diagnóstico prévio de IC diastólica e com DD padrão alteração de relaxamento do VE (n=15, 7 homens, 53±2 anos, IMC 29±1 Kg/m2) e (GDD-pr) com diagnóstico prévio de IC diastólica com padrão pseudonormal ou restritivo de DD do VE (n=15, 9 homens, 51±2 anos, IMC 27±1 Kg/m2). Voluntários saudáveis normotensos (n=14, grupo GNT) pareados para idade, sexo e IMC também foram avaliados. Curvas de pressão arterial (PA) foram registradas de modo contínuo e não invasivo (Finometer®) durante 15 minutos em repouso, na posição supina. Simultaneamente, a atividade nervosa simpática muscular (ANSM) foi registrada por meio da técnica de microneurografia. A variabilidade da freqüência cardíaca (VFC) e da pressão arterial sistólica (VPAS) foi estimada pelo método FFT. Em um segundo momento foi realizada a avaliação da função endotelial, por meio de ultrassonografia da artéria braquial associada à manobra de hiperemia reativa e após administração de trinitrato sublingual. As análises estatísticas foram realizadas pelo teste exato de Fisher e ANOVA, os resultados expressos em média ± erro padrão ou em mediana (valores mínimos e máximos). RESULTADOS: Não houve diferenças de gênero, idade e IMC entre os grupos, como também no uso das diferentes classes de drogas anti-hipertensivas entre os hipertensos. Os parâmetros estruturais cardíacos foram semelhante entre os grupos, com exceção da massa de VE do grupo GDD-pr [98 (66-162) g/m2] foi maior, p<0,05, quando comparada ao grupo GNT [85 (56-95) g/m2]. A PA sistólica (PAS) não foi diferente entre GHT, GDD-ar e GDD-pr [(138 (110-149), 133 (104-190) e 148 (118-171) mmHg, respectivamente]. Os grupos GDD-ar e GDD-pr apresentaram PAS maiores, p<0,05,quando comparados ao grupo GNT [121(108-133) mmHg]. A PA diastólica foi semelhante entre os grupos. Os grupos mostraram semelhantes valores para a modulação autonômica cardíaca avaliada pela VFC. A modulação simpática periférica representada pelo componente LF PAS da VPAS (mmHg2) foi aumentada nos grupos GDD-ar (12,2±1,3) e GDD-pr (11,7±1,2) quando comparados ao grupo GNT (6,7±0,6), p<0,05, mas não quando comparada ao grupo GHT (9,3±1,1). O prejuízo baroreflexo (índice alfa LF, ms/mmHg) foi observado nos grupos GDD-ar (4,6±0,6) e GDD-pr (5,07±0,7) quando comparados ao grupo GNT (8,2±1), p<0,05, mas não quando comparados ao grupo GHT (6,05±0,5). ANSM (espículas/min) foi maior significativamente nos grupos GDD-ar (33±1) e GDD-pr (32±1) quando comparada aos grupos GHT (26±1) e GNT (15±1) p<0,05. Ainda, o grupo GHT apresentou aumento da ANSM quando comparado ao grupo GNT, p<0,05. Os grupo GDD-ar e GDD-pr apresentaram valores semelhantes de ANSM. Com relação à avaliação da função endotelial, os grupos hipertensos apresentaram menor dilatação dependente do endotélio, sendo que somente no grupo GDD-ar [0,67 (0,0-8,7)%] houve significância estatística quando comparado ao GNT [6,3 (2,6-8,2)%]. Na avaliação da vasodilatação independente do endotélio os grupos apresentaram respostas semelhantes. CONCLUSÃO: A presença de disfunção diastólica, em qualquer grau, está associada à maior ANSM e modulação simpática periférica (LF PAS) e a menor sensibilidade do baroreflexo. A modulação simpática cardíaca não apresentou diferença entre os grupos em repouso. METHOD: Forty-five hypertensive patients without comorbities were submitted to tissue Doppler echocardiography and allocated into three groups: (GHT) without cardiac functional or structural abnormalities (n=15, 7 men, 48±2 years, BMI 28±1 Kg/m2); (GDD-ar) with prior diastolic HF and impaired relaxation pattern of DD of LV (n=15, 7 men, 53±2 years, BMI 29±1 Kg/m2), and (GDD-pr) with prior diastolic HF and pseudonormal and restrictive patterns of DD of LV (n=15, 9 men, 51±2 years, BMI 27±1 Kg/m2). Normotensive healthy volunteers matched for age, sex and body mass index were also evaluated. Curves of blood pressure (BP) were recorded non-invasively and continuously (Finometer®) for 15 minutes at rest in the supine position. Simultaneously, muscle nerve sympathetic activity (MNSA) was recorded by microneurography technique. The heart rate and systolic blood pressure variability (HRV and SPBV) was estimated by FFT method. Afterwards, an evaluation of endothelial function through brachial artery ultrasound maneuver associated with reactive hyperemia and after sublingual administration of trinitrate was conducted. Statistical analysis was performed by Fishers exact test and ANOVA, the results are expressed as mean±standard deviation or median (minimum and maximum values). RESULTS: There were no differences in gender, age and BMI between the groups, as well as in the use of different classes of antihypertensive drugs among hypertensive patients. Cardiac structural parameters were similar between groups, except for LV mass in GDD-pr group [98 (66-162) g/m2] which was higher, p<0.05, when compared to the GNT group [85 (56-95) g/m2]. The systolic blood pressure (SBP) was similar between GHT, GDD-ar and GDD-pr groups [(138 (110-149), 133 (104-190) e 148 (118-171) mmHg, respectively]. The impairment of the baroreflex (LF alpha índex, ms/mmHg) was observed in the GDD-ar (4,6±0,6) e GDD-pr (5,07±0,7) groups compared to the GNT group (8,2±1), p<0.05, but not when compared to GHT group (6,05±0,5). MNSA (burst/min) was significantly higher in GDD-ar (33±1) e GDD-pr (32±1) groups compared to GHT group (26±1) and GNT group (15±1) p<0.05. Also the GHT group showed increased MNSA when compared to GNT group, p<0.05. The GDD-ar and GDD-pr groups showed similar values of MNSA. Regarding the assessment of endothelial function, hypertensive groups had lower endothelium-dependent dilatation, but only in GDD-ar group [0,67 (0,0-8,7)%] was statistically significant when compared to GNT group [6,3 (2,6-8,2)%]. In the evaluation of endothelium-independent vasodilatation all groups showed similar responses. This Master's thesis deals with the influence of breathe to baroreceptor reflex. The first chapter is focused on measuring of blood pressure, in reference to later used method. In the second part is detailed analysis and explanation of term baroreflex, relationship between frequency spectra of systolic blood pressure, heart rate and breathing, mechanisms of regulation of blood pressure and heart rate variability. Third chapter deals with possible ways, how to analyze data, which are needed for estimation of baroreflex sensitivity, analyze of heart rate variability and estimation of power spectral density. Next parts of this thesis is paid attention to technical equipment and methods of acqusition of data, using this equipmnent. Part of this work is also software application, which makes possible from the recorded data estimate baroreflex sensitivity.

In many occupations, exposure to hand-transmitted vibration (HTV) over a prolonged period causes various disorders involving the vascular, neural and musculoskeletal systems, collectively known as the hand-arm vibration syndrome (HAVS). It is a complex and potentially disabling chronic disorder of the upper extremities, especially of the hands. Numbness, tingling, reduced tactile discrimination, and impaired manipulative dexterity are often reported by workers exposed to HTV. The precise pathophysiological mechanism responsible for vascular injuries in HAVS has not yet been fully clarified; it seems to be multifactorial and highly complex. Interaction of neural signals, hormones, mediators and changes in the blood vessel itself appear to contribute to the development of such vascular injuries. This study aims to assess the risk of the hand-transmitted vibration exposure during pneumatic impact wrench operation in a rock drill repair and maintenance workshop at a South African platinum mine. A total of 8 workers working on a day to day basis with impact wrenches were available for this study. For each of the workers a control (not exposed to vibration) was selected on the basis of gender, ethnic group, smoking habits, age and body mass index (BMI). Grip force, dexterity and hand-eye co-ordination were tested on the workers and control group before and after work. Finger systolic blood pressure (FSBP) was also measured after cold provocation of the worker and control groups. Results have shown astatistically significant difference between the two groups with respect coordination, dexterity and FSBP after cold provocation. Vibration measurements shows three workers had values above the suggested ELV of 5 m/s² for an eight hour A (8) workday. With regards to dexterity, workers were capable to manipulate small objects better with their dominant right (vibration exposed) hand after work than before work. In contrast, it seems that the number of pegs correctly inserted by the controls is not uniformly affected by their 8 hour workday. The worker group showed a greater grip force than that of the control group, both before and after work.There was a statistically significant difference between the control and worker group with respect to the number of mistakes during the mirror trace and the time to complete this test only for the right hand. The difference in FSBP after cold provocation between the control and worker group observed is of medium importance when compared with effect sizes, however, there was no statistical significant difference. In this study, it was very difficult to make valid conclusions due to the limitations of a small sample size. Genetic determinants of cardiovascular disease : heritability and genetic risk score / Les déterminants génétiques des maladies cardiovasculaires : l’héritabilité et les scores de risque génétique

Les maladies complexes telles que les maladies cardio-Vasculaires (MCV) sont influencées par des facteurs génétiques et environnementaux. L’estimation du risque cardio-Vasculaire chez un individu est généralement évaluée par la sommation des facteurs de risque reconnu des MCV (p. ex. l’âge, le sexe, le tabac, la pression artérielle et le cholestérol). Dernièrement, plusieurs bio-Marqueurs ont été examiné pour leur aptitude à améliorer la prédiction des maladies cardio-Vasculaires au-Delà des facteurs de risques traditionnels. L’intérêt de découvrir de nouveaux loci est incité notamment par les découvertes qui émergent des études d'association pangénomique (GWAS) qui permettent de tester l’association de variation génétique au risque de contracter une maladie commune. Les GWAS ont considérablement amélioré notre connaissance de l'architecture génétique des maladies cardio-Vasculaires, à ce jour plus de 50 variations génétiques sont formellement associées à des maladies cardio-Vasculaires, de même plus de 200 marqueurs génétiques seraient associés à des facteurs de risque cardiovasculaire traditionnels (p. ex. le taux sanguin des lipides, la pression artérielle, l’indice de masse corporelle et le diabète de type 2). Le succès remarquable de ces études d’association, qui a permis l’identification de nombreux bio-Marqueurs, a conduit à une réévaluation des données génétiques dans le but de définir des informations cliniquement utiles pour limiter et mieux prédire les risques de maladies, grâce à une application plus efficace des stratégies de prévention. Dans cette thèse, nous examinons tout d'abord une nouvelle approche pour étudier l'architecture génétique de l'hypertension artérielle (HTA; facteur de risque majeur des maladies cardiovasculaires prématurées), puis nous avons constitué plusieurs modèles pour prédire le risque de développer une maladie coronarienne (MC; type le plus commun de MCV), enfin nous avons déterminé une base génétique commune du principal prédicteur de complications cliniques des maladies coronariennes – l'athérosclérose subclinique - afin d'ajouter une valeur pronostique supplémentaire en plus des scores de risque traditionnels à différents âges. Nous avons estimé l'héritabilité de la première mesure de la pression artérielle systolique (PAS) à ~25%/~45% et à ~30%/~37% pour la pression artérielle diastolique (PAD) chez les sujets d’origine Européenne (N = 8901) et d’origine Africaine (N = 2860) faisant respectivement partie de la cohorte Atherosclerosis Risk in Communities (ARIC), en accord avec les études antérieures. Par ailleurs, nous avons développé un moyen de combiner un score de risque génétique (SRC) – somme des effets génétiques parmi un ensemble de marqueurs – avec une évaluation indépendante du risque clinique, en utilisant un système d'équations log-Linéaire. Nous avons employé cet outil à la prédiction de la maladie coronarienne (MC) dans la cohorte ARIC. L'ajout d'un score de risque génétique (SRG) à un score de risque clinique (SRC) améliore à la fois la discrimination et l'étalonnage des maladies coronariennes dans la cohorte ARIC, et révèle par la même comment cette information génétique influence l'évaluation des risques ainsi que l’approche clinique. Enfin, parmi 1561 cas et 5068 contrôles (de la présence ou non de calcifications coronaires), faisant partie de plusieurs ensembles de données cliniques et génétiques disponibles via la base de données NCBI de Génotypes et Phénotypes (dbGAP), nous avons constaté qu’une augmentation d'un écart-Type dans le score de risque génétique de 49 bio-Marqueurs de MC est associée à 28 % d’augmentation de risque de développer une athérosclérose coronarienne subclinique diagnostiquée à un stade avancé (p=1.43x10-16). (...) / Complex diseases such as cardiovascular disease (CVD) are influenced by both genetic and environmental factors. Estimation of an individual’s cardiovascular risk usually involves measurement of risk factors correlated with risk of CVD (e.g. age, sex, smoking, blood pressure, and total cholesterol). Lately, several biomarkers have been evaluated for their ability to improve prediction of cardiovascular disease beyond traditional risk factors. The interest in novel loci is propelled notably by emerging discoveries from the advent of genome-Wide association studies (GWAS) of genetic variants associated with risk for common diseases. GWAS has greatly enhanced our knowledge of the genetic architecture of cardiovascular disease, yielding over 50 variants confirmed to be associated with CVD to date, as well as over 200 associated with traditional cardiovascular risk factors (e.g. lipids, blood pressure, body mass index, and type 2 diabetes mellitus). This recent and continuing success in discovering increasing numbers of robustly associated genetic markers has led to reassessment of whether genetic data can provide clinically useful information by refining risk prediction and moderating disease risk through a more efficient application of prevention strategies. In this thesis, we first address novel approach to survey the genetic architecture of hypertension (i.e. major risk factor for premature CVD), then construct risk prediction models for coronary artery disease (CAD; i.e. most common type of CVD) and finally establish a common genetic basis of the strongest predictor of clinical complications of CAD, subclinical atherosclerosis, to add incremental prognostic value above traditional risk scores across a range of ages. We show that, for first visit measurements, the heritability is ~25%/~45% and ~30%/~37% for systolic (SBP) and diastolic blood pressure (DBP) in European (N=8,901) and African (N=2,860) ancestry individuals from the Atherosclerosis Risk in Communities (ARIC) cohort, respectively, in accord with prior studies. Then we present a means to combine a polygenic risk score - genetic effects among an ensemble of markers - with an independent assessment of clinical risk using a log-Link function. We apply the method to the prediction of coronary heart disease (CHD) in the ARIC cohort. The addition of a genetic risk score (GRS) to a clinical risk score (CRS) improves both discrimination and calibration for CHD in ARIC and subsequently reveal how this genetic information influences risk assessment and thus potentially clinical management. Finally, Among 1561 cases and 5068 controls, from several clinical and genetic datasets available through the NCBI's database of Genotypes and Phenotypes (dbGAP), we found a one SD increase in the genetic risk score of 49 CAD SNPs was associated with a 28% increased risk of having advanced subclinical coronary atherosclerosis (p = 1.43 x 10-16). This increase in risk was significant in every 15-Year age stratum (.01 > p > 9.4 x 10-7) and was remarkably similar across all age strata (p test of heterogeneity = 0.98). We obtained near identical results and levels of significance when we restricted the genetic risk score to 32 SNPs not associated with traditional risk factors. Accordingly, common variation largely recapitulates the known heritability of blood pressure traits. The vast majority of this heritability varies by chromosome, depending on its length, and is largely concentrated in intronic and intergenic regions of the genome but widely distributed across the common allele frequency spectrum. Respectively, our proposed method to combine genetic information at established susceptibility loci with a nongenetic risk prediction tool facilitates the standardized incorporation of a GRS in risk assessment. Is the Association of Diabetes With Uncontrolled Blood Pressure Stronger in Mexican Americans and Blacks Than in Whites Among Diagnosed Hypertensive Patients?

BACKGROUND: Clinical evidence shows that diabetes may provoke uncontrolled blood pressure (BP) in hypertensive patients. However, racial differences in the associations of diabetes with uncontrolled BP outcomes among diagnosed hypertensive patients have not been evaluated. METHODS: A total of 6,134 diagnosed hypertensive subjects aged ≥ 20 years were collected from the National Health and Nutrition Examination Survey 1999-2008 with a stratified multistage design. Odds ratios (ORs) and relative ORs of uncontrolled BP and effect differences in continuous BP for diabetes over race/ethnicity were derived using weighted logistic regression and linear regression models. RESULTS: Compared with participants who did not have diabetes, non-Hispanic black participants with diabetes had a 138% higher chance of having uncontrolled BP, Mexican participants with diabetes had a 60% higher chance of having uncontrolled BP, and non-Hispanic white participants with diabetes had a 161% higher chances of having uncontrolled BP. The association of diabetes with uncontrolled BP was lower in Mexican Americans than in non-Hispanic blacks and whites (Mexican Americans vs. non-Hispanic blacks: relative OR = 0.55, 95% confidence interval (CI) = 0.37-0.82; Mexican Americans vs. non-Hispanic whites: relative OR = 0.53, 95% CI = 0.35-0.80) and the association of diabetes with isolated uncontrolled systolic BP was lower in Mexican Americans than in non-Hispanic whites (Mexican Americans vs. non-Hispanic whites: relative OR = 0.62, 95% CI = 0.40-0.96). Mexican Americans have a stronger association of diabetes with decreased systolic BP and diastolic BP than non-Hispanic whites, and a stronger association of diabetes with decreased diastolic BP than non-Hispanic blacks. CONCLUSIONS: The association of diabetes with uncontrolled BP outcomes is lower despite higher prevalence of diabetes in Mexican Americans than in non-Hispanic whites. The stronger association of diabetes with BP outcomes in whites should be of clinical concern, considering they account for the majority of the hypertensive population in the United States.

BIRTHWEIGHT AND SUSCEPTIBILITY TO CHRONIC DISEASE

The thesis examines the relationship of birthweight to risk factors and markers, such as proteinuria and glomerular filtration rate, for chronic disease in postnatal life. It made use of the Australian Diabetes, Obesity and Lifestyle Study (AusDiab). The AusDiab study is a cross sectional study where baseline data on 11,247 participants were collected in 1999-2000. Participants were recruited from a stratified sample of Australians aged ≥ 25 years, residing in 42 randomly selected urban and non-urban areas (Census Collector Districts) of the six states of Australia and the Northern Territory. The AusDiab study collected an enormous amount of clinical and laboratory data. During the 2004-05 follow-up AusDiab survey, questions about birthweight were included. Participants were asked to state their birthweight, the likely accuracy of the stated birthweight and the source of their stated birthweight. Four hundred and twelve chronic kidney disease (CKD) patients were approached, and 339 agreed to participate in the study. The patients completed the same questionnaire. Medical records were reviewed to check the diagnoses, causes of kidney trouble and SCr levels. Two control subjects, matched for gender and age, were selected for each CKD patient from participants in the AusDiab study who reported their birthweight. Among 7,157 AusDiab participants who responded to the questionnaire, 4,502 reported their birthweights, with a mean (standard deviation) of 3.4 (0.7) kg. The benefit and disadvantages of these data are discussed in chapter three. The data were analysed for the relationship between birthweight and adult body size and composition, disorders of glucose regulation, blood pressure, lipid abnormalities, cardiovascular diseases and glomerular filtration rate. Low birthweight was associated with smaller body build and lower lean mass and total body water in both females and males. In addition low birthweight was associated with central obesity and higher body fat percentage in females, even after taking into account current physical activity and socioeconomic status. Fasting plasma glucose, post load glucose and glycosylated haemoglobin were strongly and inversely correlated with birthweight. In those with low birthweight (< 2.5 kg), the risks for having impaired fasting glucose, impaired glucose tolerance, diabetes and all abnormalities combined were increased by 1.75, 2.22, 2.76 and 2.28 for females and by 1.40, 1.32, 1.98 and 1.49 for males compared to those with normal birthweight (≥ 2.5 kg), respectively. Low birthweight individuals were at higher risk for having high blood pressure ≥ 140/90 mmHg and ≥ 130/85 mmHg compared to those with normal birthweight. People with low birthweight showed a trend towards increased risk for high cholesterol (≥ 5.5 mmol/l) compared to those of normal birthweight. Females with low birthweight had increased risk for high low density lipoprotein cholesterol (≥ 3.5 mmol/l) and triglyceride levels (≥ 1.7 mmol/l) when compared to those with normal birthweight. Males with low birthweight exhibited increased risk for low levels of high density lipoprotein cholesterol (<0.9 mmol/l) than those with normal birthweight. Females with low birthweight were at least 1.39, 1.40, 2.30 and 1.47 times more likely to have angina, coronary artery disease, stroke and overall cardiovascular diseases respectively, compared to those ≥ 2.5 kg. Similarly, males with low birthweight were 1.76, 1.48, 3.34 and 1.70 times more likely to have angina, coronary artery disease, stroke and overall cardiovascular diseases compared to those ≥ 2.5 kg, respectively. The estimated glomerular filtration rate was strongly and positively associated with birthweight, with a predicted increase of 2.6 ml/min (CI 2.1, 3.2) and 3.8 (3.0, 4.5) for each kg of birthweight for females and males, respectively. The odd ratio (95% confidence interval) for low glomerular filtration rate (<61.0 ml/min for female and < 87.4 male) in people of low birthweight compared with those of normal birthweight was 2.04 (1.45, 2.88) for female and 3.4 (2.11, 5.36) for male. One hundred and eighty-nineCKD patients reported their birthweight; 106 were male. Their age was 60.3(15) years. Their birthweight was 3.27 (0.62) kg, vs 3.46 (0.6) kg for their AusDiab controls, p<0.001 and the proportions with birthweight<2.5 kg were 12.17% and 4.44%, p<0.001. Among CKD patients, 22.8%, 21.7%, 18% and 37.6% were in CKD stages 2, 3, 4 and 5 respectively. Birthweights by CKD stage and their AusDiab controls were as follows: 3.38 (0.52) vs 3.49 (0.52), p=0.251 for CKD2; 3.28 (0.54) vs 3.44 (0.54), p=0.121 for CKD3; 3.19 (0.72) vs 3.43 (0.56), p= 0.112 for CKD4 and 3.09 (0.65) vs 3.47 (0.67), p<0.001 for CKD5. The results demonstrate that in an affluent Western country with a good adult health profile, low birthweight people were predisposed to higher rates of glycaemic dysregulation, high blood pressure, dyslipidaemia, cardiovascular diseases and lower glomerular filtration rate in adult life. In all instances it would be prudent to adopt policies of intensified whole of life surveillance of lower birthweight people, anticipating this risk. The general public awareness of the effect of low birthweight on development of chronic diseases in later life is of vital importance. The general public, in addition to the awareness of people in medical practice of the role of low birthweight, will lead to a better management of this group of our population that is increasingly surviving into adulthood.
