Desenho do Estudo: Avaliou-se a função pulmonar, mobilidade diafragmática (MD) e pressões respiratórias máximas antes e após o TMI. Avaliou-se o efeito do TMI sobre a força muscular respiratória, excursão diafragmática e função pulmonar em 31 indivíduos obesos mórbidos. Os voluntários foram randomizados em dois grupos: O grupo do treinamento muscular inspiratório (GTMI: n=16) que seguiu o protocolo de TMI com duração de doze semanas, realizado cinco vezes por semana, durante 30 minutos diários (15 minutos pela manhã e 15 minutos pela noite) e carga de 30% da pressão inspiratória máxima (PImax); O grupo controle (GC: n=15) que seguiu o mesmo protocolo de TMI sem carga inspiratória. Resultados: Após o TMI observou-se aumento significativo na PImax (-86.86 cmH2O ± -20.70 versus -106.43 cmH2O ± -32.97; p<0.05) e na ventilação voluntária máxima (VVM) (97.84 L/min ± 37.06 versus 115.17 L/min ± 34.17, p<0.05) no grupo TMI. No entanto, não se observou nenhuma mudança nas variáveis espirométricas, exceto pelo volume inspiratório forçado no primeiro segundo (VIF1) ao fim do protocolo, quando comparados o GTMI e o GC (3,35 L ± 0,96 versus 2,22 L ± 1,07; p<0,05). Não observadas diferenças da mobilidade diafragmática após a realização do protocolo de TMI em ambos os grupos. Conclusões: No presente estudo foi mostrado que é possível realizar a avaliação indireta do diafragma através da ultrassonografia modo-B em obesos mórbidos, sendo uma ferramenta útil para detectar possíveis disfunções relacionadas a esse músculo. O TMI aumentou as pressões respiratórias máximas, a ventilação voluntária máxima e promoveu alterações na VIF1. Esses achados sugerem que a eficiência muscular respiratória, alcançada com essa intervenção, foi insuficiente para mobilizar o diafragma e modificar a mecânica ventilatória, provavelmente pela presença do tecido adiposo que adicionou resistência ao sistema respiratório.

The aim of the present study is to value the effects of the inspiratory muscles training (IMT) on muscle force and on lung function in asthmatic infants. It is an analytic, experimental and random study. It were valued 50 infants by random choose, in two groups: Experimental Group (EG) composed of 25 infants who did IMT in a program of help an education on asthma and a Control Group (CG), composed of 25 infants who were submit only to monthly medical consults and education on asthma. The IMT was implement by using the Threshold IMT with load of 40% of Inspire Maximum Pressure. The analysis of the results was made by using the Student t Test paired and non-paired and the Qui-Square Test with p &#8804; 0,05. At the comparative analysis the variables muscle force and peak flow before and after intervention, was observed an increased of the Inspire Maximum Pressure on EG from -48,32 ± 5,706 to 109,92 ± 18,041 (p&#8804;0,0001); Expire Maximum Pressure from 50,64 ± 6,55 to 82,04 ± 17,006 (p&#8804;0,0001) and Peak flow from 173,6 ± 50,817 to 312 ± 54,848 (p&#8804;0,0001). There was no significant increased at the variables Inspire Maximum Pressure and Expire Maximum Pressure on CG. There was a significant increased of the Peak flow at the CG from 188 ± 43,97 to 208,80 ± 44,283 (p&#8804;0,0001). There was a significant increased at the variables gravity of the nocturnes symptoms, frequency of looking for emergency room and hospitalizations at the two groups (p&#8804;0,0001). In the variable gravity of attack frequency only the EG had significant increased (p&#8804;0,0001). Believe that the RMT can increase the mechanical efficiency at the respiratory muscles and at the lung function in asthmatic infants. / Os músculos respiratórios como os músculos esqueléticos, são sensíveis a um programa de treinamento físico adequado e podem ser treinados a fim de melhorarem sua força e endurance. O presente estudo tem como objetivo avaliar os efeitos do treinamento muscular inspiratório (TMI) com Threshold na força muscular e função pulmonar em crianças asmáticas. Trata-se de um estudo analítico, experimental e aleatório. Foram avaliadas 50 crianças alocadas aleatoriamente em dois grupos: Grupo Experimental (GE) composto de 25 crianças que realizaram TMI em um programa de assistência e educação em asma e um Grupo Controle (GC), composto de 25 crianças que foram submetidas apenas às consultas médicas mensais e educação em asma. O TMI foi realizado com o Threshold com a carga de 40% da pressão inspiratória máxima ( Pimáx). A análise dos resultados foi realizada através do Teste t de Student pareado e não pareado e Teste de &#967;2 (qui-quadrado) com p &#8804; 0,05. Na análise comparativa das variáveis força muscular, pressão inspiratória máxima (Pimáx) e pressão expiratória máxima (Pemáx) e pico de fluxo expiratório (PFE) pré e pós intervenção, foi observado aumento da Pimáx. em GE de -48,32 ± 5,706 para -109,92 ± 18,041 (p &#8804; 0,0001); Pemáx de 50,64 ± 6,55 para 82,04 ± 17,006(p &#8804; 0,0001) e PFE de 173,6 ± 50,817 para 312 ± 54,848 (p &#8804; 0,0001). Não houve aumento significativo das variáveis Pi máx e Pe máx. no GC. Houve aumento significativo do PFE no GC de 188 ± 43,97 para 208, 80 ± 44,283 (p &#8804; 0,0001). Houve melhora significativa das variáveis de gravidade como sintomas noturnos, frequência de procura ao pronto-socorro e hospitalização nos dois grupos (p<0,001). Na variável de gravidade frequência de crises apenas GE apresentou melhora significativa (p<0,001). Acredita-se que o TMI proporcione uma melhora na eficiência mecânica nos músculos respiratórios e na no pico de fluxo expiratório das crianças asmáticas.

O envelhecimento está associado à resistência a insulina, caracterizada pela diminuição da resposta insulínica aos estímulos, cujos efeitos progressivos podem levar a intolerância à glicose e diabetes tipo 2. Sendo assim, o objetivo proposto neste estudo foi investigar a influência do treinamento muscular inspiratório através do Threshold® sobre a resistência à insulina em pessoas idosas. O estudo envolveu a participação de 14 pacientes idosos voluntários, com idade variando de 61 a 82 anos, sedentários, com IMC entre 18,75 a 36,7, resistentes à insulina, de ambos os sexos, sem patologias pulmonares e cardiovasculares prévias do ambulatório do Hospital Geral de Taipas. Os pacientes selecionados foram submetidos à avaliação laboratorial, a avaliação física e avaliação cardiorrespiratória. Sendo posteriormente divididos em dois grupos: Grupo controle e Grupo experimental. O programa teve duração de 12 semanas e ambos os grupos realizaram o treinamento 07 vezes por semana, sendo 06 vezes domiciliar e 01 vez no ambulatório com duração de 30 minutos diários. Após o término de 12 semanas, todos os pacientes foram reavaliados, onde a comparação dos grupos após o período de estudo mostrou que o grupo que sofreu intervenção obteve melhora da força e desempenho muscular respiratório e diminuição dos valores metabólicos, sendo considerado estatisticamente significativo a diminuição dos valores de glicemia, insulina e do HOMA IR, dado este importante devido a correlação entre envelhecimento e resistência à insulina. Os dados sugerem que o treinamento muscular inspiratório melhora a sensibilidade à insulina em pacientes portadores de resistência à insulina. / The physiological degenerations in the aging process can cause physiological decline in the pulmonary and metabolic function, being able to affect in the elder quality of life. The aging is association with insulin resistance, characterized for the reduction of the insulin reply to the stimulations, whose gradual effect can take the glucose intolerance and diabetes type 2. Therefore, the objective in this study was to investigate the influence of the inspiratory muscular training through the Threshold® on the insulin resistance in aged people. The study involved the participation of 14 elder voluntaries, with age between 61 and 82 years old, sedentaries, with BMI between 18,75 - 36,7 and insulin resistants with lipids alterations, of both genders, without previous pulmonary and cardiovascular disease at the clinic of the Taipas Hospital. The selected patients have been submitted to the Laboratorial Test, the physical Test and Cardiopulmonary Test. After the test, the patients were divided in two groups: Control group and Experimental Group. The program had duration of 12 weeks and both the groups did the inspiratory muscle training 07 times per week, being 06 times domiciliary and 01 time in the clinic with duration of 30 minutes daily. All tests were repeated after 12 weeks of inspiratory muscle training, where the comparison between the groups after the period of study showed that the group that suffered intervention got an improvement of the respiratory muscular performance and force, reduction of metabolic values. It being considered statistical significant the reduction of the values in glicemic, insulin and HOMA IR, this is important because the correlation between aging and insulin resistance. Functional status and dyspnea as performance of activities of daily living after physical and inspiratory muscle training in patients with chronic obstructive pulmonary disease

Less is known on ventilatory and metabolic responses ( E, O2) during the execution activities of daily life (ADL) in patients going through physical training with additional or non-additional inspiratory muscle training (IMT). Aim: This study aims to compare changes on E and O2 responses, dyspnea, oxygen saturation and ADL time during a standard set of ADL tasks after two different training programs. Methods: A set of 5 ADL using a metabolic system (making bed, taking shower, brushing teeth and combing hair, lifting and lowering containers on a shelf above eye level and below the pelvic waist) was performed by 28 patients with COPD before and after the whole protocol. Patients were divided into two groups. One performed aerobic training, lower limbs (LL) resistive exercise and respiratory exercise (GPT) and the other aerobic training, resistive exercise of LL and an additional IMT (GPT+IMT). 13 composing the GPT (age 67.1±7.3yrs, FEV1 43.0±4.0%pred) and 15 patients composing the GPT+IMT (age 67.4±11.7yrs, FEV1 51.0±3.0%pred). Results: Both groups had a significant reduction (p<0.05) of E, O2 and Borg and a significant improvement of SpO2 during the ADL set within them showing benefits of physical training on performance of ADL. However, adding IMT did not show additional benefits in those variables between groups. There was a significant difference (p<0.05) in PImax in both groups and between them. On the other hand, although there are no differences between groups, both of them improved their performance in the cardiopulmonary test and six minute walking distance. Conclusion: The pulmonary rehabilitation reduces metabolic cost of common tasks of daily life as both groups improved their performance in the ADL set and, even with IMT, additional benefits were not perceived.

O treinamento muscular inspiratório não altera a arritmia sinusal respiratória de pacientes com infarto agudo do miocárdio

The RSA is lower in patients with acute myocardial infarction. Objective: To evaluate the effect of inspiratory muscle training (IMT) on the RSA magnitude and maximal inspiratory pressure (PImax) in patients with AMI after 6 days of physiotherapeutic intervention during hospitalized period. Methodology: Thirty three patients were divided in two groups: 19 patients of treatment group (TG) (14 males, 5 females, aged 50±9 years) who underwent the cardiovascular physiotherapy and 14 patients of control group (CG) (11 males, 3 females, aged 52±11 years) without physiotherapy. The intervals RR of electrocardiogram were obtained by Polar S810i during the RSA test on the 1st and 6th days. It was used to calculate the heart rate variability in the time (TD) and in the frequency domain and the RSA index. The PImax was obtained by manovacuometer on the 2nd and 6th days. The IMT (3x, 10 forced inspiration, 40% PImax, for 3 seconds each) was done from the 2nd to 6th days. Two-way ANOVA, Paired-t Test, Chi-square test and Pearson s correlation (p<0.05) were used. Results: In the TD, the TG showed higher values than the CG for the SDNN (1st day 58±37 and 33±23ms; 6th day 49±20 and 32±17ms, respectively) and for the RMSSD (1st day 40±32 e 21±18 ms; 6th day 33±18 and 22±14ms, respectively). The &#8710;IE decreased significantly for the TG (12,8±7,5 to 9,9±4,8 bpm) and for the CG (10,8±5,3 to7,5±3,4 bpm). For the TG, the PImax increased (p<0,05) from 78±25 to 101±25. Conclusion: The IMT increased the PImax, although it did not modify the ASR magnitude in patients with acute myocardial infarction. / Introdução: A arritmia sinusal respiratória (ASR) tem sido utilizada para estudar a modulação autonômica cardíaca sendo avaliada pelos índices expiração/inspiração (E/I) e variação da freqüência cardíaca inspiração-expiração (&#8710;IE). A ASR encontra-se reduzida em pacientes com infarto agudo do miocárdio (IAM). Objetivo: Avaliar o efeito do treinamento muscular inspiratório (TMI) na magnitude da ASR e da pressão inspiratória máxima (PImax) de pacientes com IAM após 6 dias de intervenção fisioterapêutica no período intra-hospitalar. Metodologia: Trinta e três pacientes foram divididos em: grupo tratado (GT) com 19 pacientes (14 homens, 5 mulheres, idade 50±9 anos) que foram submetidos a fisioterapia cardiovascular associado com o TMI e o grupo controle (GC) com 14 pacientes (11 homens, 3 mulheres, idade 52±11 anos) sem fisioterapia. Os intervalos RR do eletrocardiograma coletados por meio de um polar S810i, durante uma manobra para acentuar a ASR, realizada na posição supina no 1° e 6° dias. Foram calculados a variabilidade da freqüência cardíaca no domínio do tempo (DT) por meio dos índices RMSM e RMSD dos intervalos R-R em ms e da freqüência (DF) e os índices da ASR. A PImax foi obtida por meio de um manuvacuômetro no 2° e 6° dia e o TMI (3x de 10 inspirações forçadas a 40% da PImax por 3 segundos cada) realizado do 2° ao 6° dia. Análise estatística: Foram usados ANOVA two way de medidas repetidas, teste t pareado, teste qui-quadrado e correlação de Pearson (p<0,05). Resultados: No DT, o GT apresentou comparativamente ao GC, valores superiores de RMSM (1º dia 58±37 e 33±23 ms; 6º dia 49±20 e 32±17ms) e de RMSSD (1º dia 40±32 e 21±18ms; 6º dia 33±18 e 22±14ms), respectivamente. O &#8710;IE diminuiu (p<0,05) do 1º para o 6º dia no GT (12,8±7,5 para 9,9±4,8 bpm) e no GC (10,8±5,3 para 7,5±3,4 bpm). No GT, a PImax aumentou (p<0,05) de 78±25 para 101±25. Conclusão: O TMI aumentou a PImax, porém, não modificou a magnitude da ASR no IAM.

Influência do treinamento muscular inspiratório na capacidade funcional e pulmonar pré e pós-operatória de cirurgia de revascularização do miocárdio / Influence of inspiratory muscle training in functional and pulmonary capacity on pre and post CABG surgery

Objective: To analyse the effects of a preoperative inspiratory muscle training (IMT) program on functional and pulmonary capacities in pre-and post-operative coronary artery bypass graft. Material and Methods: The study is a controlled clinical trial. The sample of this research was composed of 32 individuals admitted to the Imperial Hospital de Caridade (Florianópolis) undergoing elective coronary artery bypass grafting with cardiopulmonary bypass through a median thoracotomy (sternotomy). The study included individuals at high risk for developing pulmonary complications after surgery. The subjects were divided into control and intervention groups. The intervention group received inspiratory muscle training (IMT) with Threshold-loading device. The patients started breathing at a resistence equal of 30% of their maximal inspiratory mouth pressure. The patients trained 7 days a week, 2 times a day (3 sets, 10 repetitions) at least 2 weeks before surgery. Data collection was obtained by: individual assessments records, information about surgical procedures, spirometry, maximal mouth pressures, 6-minute walk test and pulmonary complications after surgery range. The data were analyzed using descriptive statistic and compared by specific statistical tests. Results: The demographic, clinical and surgical procedures were similar in both groups. In the assessment of lung volumes and flows was found that the FVC (p = 0.783), FEV1 (p = 0.668), PEF (p = 0.94) and FEV1/FVC (p =0.745) did not differ significantly between the intervention and control groups in different conditions before and after surgery, however, both showed a significant decrease after surgery. The maximal inspiratory pressure (MIP) differed significantly between groups (p <0.001). Before surgery, it was observed that there was a significant increase in MIP in the intervention group of 70.0 ± 19.7 cmH2O to 92.7 ± 26.8 cmH2O. In contrast, the control group showed a significant reduction in MIP from 75.9 ± 25.6 cmH2O to 66.6 ± 23.6 cmH2O. MIP in the intervention group had a better recovery, returning to baseline (57.5 ± 11.5 and 64.1 ± 14.1 cmH2O, respectively), but the control group remained decreased (43.4 ± 14.1 and 47.1 ± 15.0 cmH2O, respectively), after surgery. The MEP did not obtain significant difference between control and intervention group (p = 0.286), both groups showed a decrease after surgery. There was a significant increase in functional capacity in the intervention group (361.9 ± 92.6 to 434.4 ± 89.5) preoperatively, with smaller drop after surgery. The control group had a decrease in distance walked in 6 minutes (361.9 ±92.6 to 434.4 ±89.5m) in the preoperative period, not returning to baseline in the postoperative period. The length of stay in ICU (p = 0.564) and hospital stay (p = 0.892) did not differ between the two groups. The intervention group had a lower incidence of pulmonary complications (p = 0.046). Conclusion: An inspiratory muscle training program before surgery was able to increase the functional and pulmonary capacities in preoperative, and improve clinical outcomes in patients at high risk for developing pulmonary complications undergoing surgery coronary artery bypass graft. / Introdução: A realização da cirurgia de revascularização do miocárdio está associada com efeitos deletérios sobre a função pulmonar e capacidade funcional no pós-peratório imediato. Objetivo: Analisar os efeitos de um programa de treinamento muscular inspiratório (TMI) pré-operatório sobre a capacidade funcional e pulmonar pré e pós-operatória de cirurgia de Revascularização do Miocárdio (RM). Material e Métodos: O estudo caracteriza-se por ser um ensaio clínico controlado. A amostra desta pesquisa foi constituída por 32 indivíduos internados no Imperial Hospital de Caridade de Florianópolis que foram submetidos à cirurgia eletiva de revascularização do miocárdio com circulação extracorpórea através de toracotomia mediana (esternotomia). Foram incluídos no estudo, indivíduos de alto risco para o desenvolvimento de complicações pulmonares pós-operatórias. Os indivíduos foram alocados em grupo controle e intervenção. O grupo intervenção foi submetido a um treinamento muscular respiratório com auxílio do aparelho Threshold IMT. A carga utilizada para o fortalecimento respiratório foi de 30% do valor registrado na PIMáx. Os pacientes realizaram o treinamento 7 dias por semana, 2 vezes ao dia (3 séries de 10 repetições), pelo menos 2 semanas que antecedem a cirurgia. Foram utilizados como instrumentos de coleta de dados: fichas de avaliações do indivíduo, ficha de procedimentos cirúrgicos, espirometria, manovacuometria, teste de caminhada de 6 minutos e escala de complicações pulmonares pós-operatórias. Os dados foram analisados através da estatística descritiva e comparados por meio de testes estatísticos específicos. Resultados: Os dados demográficos, clínicos e cirúrgicos foram similares nos dois grupos. Na avaliação dos volumes e fluxos pulmonares foi verificado que a CVF (p=0.783), o VEF1 (p= 0.668), o PFE (p= 0.94) e o VEF1/CVF (p=0.745) não se diferenciaram significativamente entre os grupos controle e intervenção, nas diferentes condições pré e pós-operatórias, ambos apresentaram uma queda significativa após a cirurgia. A força muscular inspiratória diferenciou-se significativamente entre os grupos (p<0.001). No pré-operatório, observou-se que ocorreu um aumento significativo de força muscular inspiratória no grupo intervenção de 70.0 ±19.7cmH2O para 92.7 ±26.8 cmH2O. Em contrapartida, o grupo controle apresentou uma redução significativa da PImáx de 75.9 ±25.6 cmH2O para 66.6 ±23.6 cmH2O. A PIMáx do grupo intervenção teve uma melhor recuperação no pós-operatório, retornando ao valor basal (57.5 ±11.5 e 64.1 ±14.1 cmH2O, respectivamente), porém, a do grupo controle continuou diminuída (43.4 ±14.1 e 47.1 ±15.0 cmH2O, respectivamente), após a cirurgia. A PEMáx não obteve diferença significativa entre o grupo controle e intervenção (p=0.286), apresentando uma redução após a cirurgia. Houve um aumento significativo da capacidade funcional no grupo intervenção (361.9 ±92.6 para 434.4 ±89.5m) no pré-operatório, com menor queda após a cirurgia. O grupo controle teve uma diminuição da distância percorrida (384.8 ±136.3 para 333.7 ±116.3 m) no pré-operatório, não retornando aos valores basais, no pós-operatório. O tempo de internação em UTI (p=0.564) e permanência hospitalar (p=0.892) não apresentou diferença entre os dois grupos. O grupo intervenção teve menor incidência de complicações pulmonares (p=0.046). Conclusão: A realização de um programa de treinamento muscular inspiratório no pré-operatório foi capaz de incrementar a capacidade funcional e pulmonar pré-operatória, e melhorar os desfechos clínicos, em indivíduos com alto risco para o desenvolvimento de complicações pulmonares pós-operatórias submetidos à cirurgia de revascularização do miocárdio.

Therefore, the objective of this study was to compare body composition between hemodialysis patients and healthy subjects, to evaluate cardiorespiratory fitness by oxygen uptake ( peak VO2  ) and the effects of inspiratory muscle training (IMT) on respiratory function and quality of life of male patients on hemodialysis. A clinical trial was conducted with 62 patients, aged 22-71 years. Body composition was estimated by vectorial bioelectrical impedance and anthropometric measures. Respiratory muscle strength was assessed by manometer to obtain the maximal inspiratory (Pimax) and expiratory pressures (Pemax). Pulmonary function and classification of respiratory disorders were examined by spirometry. Cardiorespiratory fitness was assessed by cardiopulmonary exercise test performed on a treadmill using the Bruce protocol which was adapted to estimate peak oxygen uptake ( peak OV 2  ). The aspects of quality of life were assessed by questionnaire KDQOL - SF (Kidney Disease Quality of Life - Short Form). The IMT was held for six months during the sessions and always at the first two hours of hemodialysis. Two training devices were used: a specific trainer and a respiratory incentive associated with two breathing exercises. The respiratory function of patients was evaluated before, during and after the inspiratory muscle training. The results were presented in three studies. In the first study, the comparison of body composition between hemodialysis patients and healthy subjects indicated differences for triceps skin fold (p<0.001) and arm circumference (p<0.001), but not for arm muscle circumference and area. The vectorial bioelectrical impedance indicated patients in dehydration and reduced body cell mass when compared with healthy subjects. In the second study was analyzed the usefulness of respiratory parameters to detect patients with poor functional capacity ( peak OV 2  below 70 % of predicted). The Pimax (β=-0.037, p=0.014) and percentage of predicted forced vital capacity - FVC % (β=-0.056, p=0.025) were significant to identify patients with poor functional capacity predictors and the ROC curve established cutoff points for MIP (<74 cmH2O) and % FVC (<79%). In the third study were verified the effects of IMT on respiratory muscle strength, lung function and quality of life of hemodialysis patients. After training muscle strength was found significant increase in respiratory strength in both groups. The group that used incentive spirometry had greater increment to inspiratory strength (39.80%) than the group that used the specific trainer (28.30%). The aspects related to quality of life, found significant differences for the dimensions cognitive function (p=0.03), sexual function (p=0.009) and social function (p=0.04) in the group that trained with specific trainer, and to physical function (p=0.03) in the group that trained with incentive spirometry. The results of this study show that male patients in hemodialysis exhibit differences in body composition when compared with healthy subjects; inspiratory muscle strength and percent of vital capacity predicted for age can be used as predictors of poor functional capacity in this group of patients; and finally, inspiratory muscle training improved inspiratory muscle strength and aspects related to the quality of life of the two trained groups. / A doença renal crônica (DRC) e o seu tratamento, hemodiálise, estão associados com o surgimento de comorbidades, tais como, alterações hormonais, aumento da resposta inflamatória, as doenças cardiovasculares e a desnutrição energético-protéica, com consequente perda de tecido muscular. Essa perda reduz a força muscular respiratória e a função pulmonar e culmina na reduzida capacidade funcional, aptidão cardiorrespiratória e qualidade de vida destes pacientes. Considerando o exposto acima, o objetivo deste estudo foi avaliar a composição corporal, a capacidade aeróbia e os efeitos do treinamento muscular inspiratório (TMI) sobre a função respiratória e a qualidade de vida de pacientes do sexo masculino, submetidos à hemodiálise. Foi realizado um ensaio clínico com 62 pacientes, com idade entre 22-71 anos. A composição corporal foi estimada pelas medidas antropométricas e bioimpedância vetorial. A força muscular respiratória foi avaliada a partir da manovacuometria para obtenção das pressões inspiratória (Pimax) e expiratória máximas (Pemax). A função pulmonar e a classificação dos distúrbios ventilatórios foram examinadas pela espirometria. A aptidão cardiorrespiratória foi avaliada pelo teste de esforço cardiopulmonar, realizado em esteira e com a utilização do protocolo de Bruce que foi adaptado para estimar o consumo de oxigênio de pico ( pico OV 2  ). Os aspectos relacionados à qualidade de vida foram avaliados pelo questionário KDQOL-SF (Kidney Disease Quality of life – Short Form). O treinamento inspiratório foi realizado por seis meses durante as sessões e sempre nas primeiras duas horas da hemodiálise. Dois dispositivos de treinamento foram utilizados: um treinador específico e um incentivador respiratório associado a dois exercícios respiratórios. Os pacientes foram examinados quanto à função respiratória antes, durante e após o treinamento muscular inspiratório. Os resultados foram apresentados em três estudos. No primeiro estudo a comparação da composição corporal de pacientes em hemodiálise com a de sujeitos saudáveis indicaram diferenças para dobra cutânea tricipital (p<0,001) e circunferência de braço (p<0,001), mas, nenhuma para circunferência e área muscular de braço. A análise da bioimpedância vetorial indicou pacientes em estado de desidratação e redução de massa celular. No segundo estudo foi analisada a utilidade de parâmetros respiratórios para detectar pacientes com pobre capacidade funcional ( pico OV 2  abaixo de 70% do predito). A Pimax (β=-0,037; p=0,014) e a porcentagem predita da capacidade vital forçada - %CVF (β=-0,056; p=0,025) foram considerados preditores significativos para identificar os pacientes com pobre capacidade funcional e a curva ROC estabeleceu os pontos de corte para Pimax (<74 cmH2O) e %CVF (<79%). No terceiro estudo foram verificados os efeitos do TMI sobre função respiratória e a qualidade de vida dos pacientes em hemodiálise. Após o treinamento a força inspiratória aumentou, significativamente, nos dois grupos. O grupo que usou o incentivador respiratório teve maior incremento de força inspiratória (39,80%) que o grupo que utilizou o treinador específico (28,30%). Quanto à qualidade de vida, foram encontradas diferenças significativas para as dimensões função cognitivas (p=0,03), função sexual (p=0,009) e função social (p=0,04) no grupo que treinou com dispositivo específico; e para a dimensão função física (p=0,03) no grupo que treinou com incentivador respiratório. Conclui-se que os pacientes, do sexo masculino, em hemodiálise apresentam diferenças na composição corporal quando comparados com sujeitos saudáveis; a Pimax e a %CVF predita para idade podem ser utilizadas como preditores de pobre capacidade funcional nesse grupo de pacientes; e por último, o TMI melhorou de forma significativa a força inspiratória e os aspectos relacionados à qualidade de vida dos dois grupos treinados.
