Reliability and Cost-Benefit Analysis of the Battery Energy Storage System / Tillförlitlighet och Kostnadsnyttoanalys av Batterienergilagringssystemet

The battery energy storage system (BESS) is crucial for the energy transition and decarbonisation of the energy sector. However, reliability assessment and capital cost challenges can hinder their widespread deployment. Reliability and cost-benefit analysis help address these challenges and assess BESS adoption's feasibility and viability, which is the aim of this project. A BESS contains various components such as battery packs, inverters, a DC/DC converter, a Battery Thermal Management System (BTMS), electrical protection devices, a transformer, and an Energy Management System (EMS). All these fundamental components must be considered to obtain a complete reliability prediction. Most previous studies focused on the reliability analysis of individual components, but few consider all the abovementioned components in collective reliability analysis. In this thesis, each component is mathematically modelled to estimate failure rates and then used to predict the reliability of the overall BESS system. The model accuracy is verified by comparing the computed reliability indices with the values from standards/references, showing that the proposed reliability prediction methods provide reasonable outcomes. Different scenarios to enhance BESS reliability through component redundancy are explored in this project. It is proved that applying component redundancy can boost the overall BESS reliability at the price of an increased capital cost. However, the enhancement in reliability and lifespan due to component redundancy can also curtail maintenance costs. A cost-benefit analysis assesses each scenario's profitability, considering manufacturers' and owners' perspectives. It helps determine the optimal balance between reliability and profitability. Redundancy applied to components with higher failure rates and lower costs improves the reliability and profitability of the BESS. The finding highlights the importance of strategic component selection for enhancing BESS reliability. Careful reliability and cost analysis should be performed simultaneously to find the most optimised BESS scenario. / Batterienergilagringssystemet (BESS) är avgörande för energiomställningen och avkarboniseringen av energisektorn. Tillförlitlighetsbedömning och utmaningar med kapitalkostnader kan dock hindra deras utbredda användning. Tillförlitlighet och kostnads-nyttoanalys hjälper till att hantera dessa utmaningar och utvärdera BESS-antagandets genomförbarhet och genomförbarhet, vilket är syftet med detta projekt. Ett BESS innehåller olika komponenter som batteripaket, växelriktare, en DC/DC-omvandlare, ett Battery Thermal Management System (BTMS), elektriska skyddsanordningar, en transformator och ett energiledningssystem (EMS). Olika scenarier för att förbättra BESS-tillförlitligheten genom komponentredundans utforskas i detta projekt. Det är bevisat att tillämpning av komponentredundans kan öka den övergripande BESS-tillförlitligheten till priset av en ökad kapitalkostnad. Förbättringen av tillförlitlighet och livslängd på grund av komponentredundans kan dock också minska underhållskostnaderna. En kostnads-nyttoanalys bedömer varje scenarios lönsamhet, med hänsyn till tillverkarnas och ägarnas perspektiv. Det hjälper till att bestämma den optimala balansen mellan tillförlitlighet och lönsamhet. Redundans som tillämpas på komponenter med högre felfrekvens och lägre kostnader förbättrar tillförlitligheten och lönsamheten för BESS. Resultatet belyser vikten av strategiskt komponentval för att förbättra BESS-tillförlitligheten. Noggrann tillförlitlighets- och kostnadsanalys bör utföras samtidigt för att hitta det mest optimerade BESS-scenariot.

Att utforma detaljplaner för ett kostnadseffektivt byggande / To create a zoning plan for a cost-efficient construction

Detaljplaner sätter ramarna och förutsättningarna för vad som legalt kan byggas på en fastighet. Hur de förutsättningarna ser ut har en enorm inverkan på hur kostnadseffektivt det framtida bygget kommer att bli. Att i tidigt skede identifiera och påverka de faktorer som driver byggkostnaderna utan att skapa motsvarande mervärde för de framtida kunderna är en viktig uppgift. I den här studien kartläggs de planbestämmelserna och utformningarna, det ska dessutom inte vara självklart att det finns tillräckligt samhällsnytta för att väga upp kostnaderna. Litteraturen inom området är mycket bristande. Den huvudsakliga metoden för den här studien är intervjuer, totalt genomförs sjutton intervjuer med privata fastighetsutvecklare och kommunalt anställda inom markexploatering och detaljplanering. Det identifieras en rad detaljplansutformningar och fenomen kopplat till detaljplaneprocessen. Allt ifrån det optimala byggnadsdjupet och underjordiska garage till felaktiga betraktelsevinklar för arkitekturen och nya typer av kommunala särkrav som blir allt vanligare. Dessutom hanterar studien samarbetet inom detaljplaneprocessen mellan privata fastighetsutvecklare och kommunen med fokus på byggkostnader, lönsamhet och avvägningarna mellan det enskilda intresset hos fastighetsutvecklaren och allmänna intressen. Fastighetsutvecklare har inflytande under detaljplaneprocessen och kan bidra med mycket. Inom kommuner finns tankar kring vad som är den mest samhällsekonomiskt lönsamma avvägningen för planutformningar där projektets lönsamhet får väga tyngre, men än så länge gäller det främst fram tills en detaljplan är genomförbar och ibland för kommunens egna fastigheter eller bostadsprojekt.

Analysis of Information Security Risks and Protection Management Requirements for Enterprise Networks.

With widespread of harmful attacks against enterprises¿ electronic services, information security readiness of these enterprises is becoming of increasing importance for establishing the required safe environment for such services. Various approaches are proposed to manage enterprise information security risks and to assess its information security readiness. These approaches are, however, not adequate to manage information security risks, as all required information security components of its structural and procedural dimensions have not considered. In addition, current assessment approaches lack numerical indicators in assessing enterprise information security readiness. Furthermore, there is no standard approach for analysing cost versus benefit in selecting recommended protection measures. This thesis aims at contributing to the knowledge by developing comprehensive Enterprise Information Security Risk Management (EISRM) framework that integrates typical approaches for information security risk management, and incorporates main components of key risk management methodologies. In addition, for supporting phases of the proposed EISRM framework, analytical models for enterprise information security readiness assessment and cost-benefit analysis are developed. The practical evaluation, using the proposed enterprise information security readiness assessment model has been performed depending on a developed investigation form that used to investigate nine enterprises inside Saudi Arabia. The results demonstrate the effectiveness of the model in assessing and comparing enterprises information security readiness at all levels of the model, using numerical indicators and graphical representations. The EISRM framework and the analytical models presented in this research can be used by enterprises as single point of reference for assessing and cost effectively improving their information security readiness.

Towards positive energy districts: assessing the contribution of virtual power plants and energy communities

The concept of positive energy districts (PED) encompasses a range of policies and strategies in response to climate protection targets in urban areas. Due to the limited potential of renewable energy in urban neighborhoods, broader definitions of PED are proposed that allow for energy exchange through the grid infrastructure. This study evaluates demand side management in combination with a virtual power plant (VPP) to assess the impact on the design of PED. In particular, the optimal customer behavior in response to flexible electricity tariffs is analyzed. A techno-economic energy system model is proposed for an urban area in Germany that optimizes the customer cost and the VPP’s margin. This includes electrical energy generation, storage, demand, and access to the short-term electricity market. Based on economic analysis, a dynamic market-based tariff allows the VPP to maximize profit margins. Consumers benefit when the local balances of renewable energy supply and demand are integrated into the dynamic tariff.

Characterisation, biophysical modelling and monetary valuation of urban nature-based solutions as a support tool for urban planning and landscape design

The recognition of nature in the resolution of societal challenges has been growing in relevance. This recognition has been associated with the development of new concepts from science and policy such as natural capital, ecosystem services, green infrastructure, and more recently Nature-Based Solutions (NBS). NBS intends to address societal challenges in an effective and adaptive form providing economic, social, and environmental benefits. The overall aim of this PhD thesis is to develop an environmental and economic assessment of NBS for highly urbanised territories based on rationales and models underpinning ecosystem services, urban/landscape ecology, and life cycle thinking approaches. This combined evaluation approach would help to better understand if NBS are cost-effective or not. The aim is developed according to four specific objectives. The first objective corresponds to the characterisation of NBS in relation to urban contexts and the problematics that they can help to address or mitigate. To achieve this objective a critical review on the study of the relationship between NBS, ecosystem services (ES) and urban challenges (UC) was developed. As a main output, a graph of plausible cause-effect relationships between NBS, ES and UC is obtained. The graph represents a first step to support sustainable urban planning, moving from problems (i.e. urban challenges) to actions (i.e. NBS) to resolutions (i.e. ES). The second objective corresponds to the definition of an adequate set of biophysical and monetary assessment methods and indicators to evaluate the value of NBS in urbanised contexts. To achieve this objective, a review of existing methods on ecosystem services valuation, life cycle cost analysis and life-cycle assessment are developed. The review takes into account specific constraints such as easiness to use and availability of data. At the end, potential methods and indicators were selected, which will be later integrated in the combined assessment framework. The third objective corresponds to the design of a combined assessment framework integrating methods from life cycle assessment, landscape/urban ecology and ecosystem services that quantifies the environmental and economic value of NBS informing about the cost-effectiveness of its entire life cycle. To achieve this objective, a conceptual framework is developed. From it, a system dynamics model of ecosystem (dis)services is developed and coupled with a life cycle assessment method. The combined evaluation is tested with a relevant NBS type (i.e. urban forest) in a case study in the metropolitan area of Madrid. The fourth objective is the development of a decision support (DSS) tool that integrates the assessment framework as part of iterative design processes in urban planning and landscape design. The DSS intends to enhance the interrelation between science, policy and planning/design. To achieve this objective a user-friendly web-based prototype DSS on NBS, called NBenefit$®, is developed. The prototype DSS provides the user a simple form of quantifying the provision of multiple ES and costs over the entire life cycle (implementation, operational life, and end-of-life) of NBS. This thesis contributed to the characterisation of NBS and its environmental and economic assessment to inform urban planning and landscape design processes, allowing decisions that are more informed. / Il riconoscimento della natura nella risoluzione delle sfide sociali è diventato sempre più importante. Questo riconoscimento è stato associato allo sviluppo di nuovi concetti provenienti dalla scienza e dalla politica, come il capitale naturale, i servizi ecosistemici, le infrastrutture verdi e, più recentemente, le soluzioni basate sulla natura (NBS). NBS intende affrontare le sfide della società in una forma efficace e adattabile fornendo benefici economici, sociali e ambientali. Lo scopo di ricerca di questa tesi di dottorato è quello di sviluppare una valutazione ambientale ed economica delle NBS per territori altamente urbanizzati basata su logiche e modelli che hanno alla base i servizi ecosistemici, l'ecologia urbana e paesaggistica e degli approcci di approcio life cycle. Questo quadro di valutazione combinato aiuterebbe a capire meglio se le NBS sono costo effetive e se contribuiscono a uno sviluppo resiliente e sostenibile. Questo scopo di ricerca è sviluppato secondo quattro obiettivi specifici. Il primo obiettivo corrisponde alla caratterizzazione delle NBS in relazione ai contesti urbani e alle problematiche che possono aiutare ad affrontare o mitigare. Per raggiungere questo obiettivo è stata sviluppata una revisione critica dell letteratura sullo studio della relazione tra NBS, servizi ecosistemici (ES) e sfide urbane (UC). Come risultato principale, si ottiene un grafico delle relazioni causa-effetto plausibili tra NBS, ES ed UC. Il grafico rappresenta un primo passo per supportare la pianificazione urbana sostenibile, passando dai problemi (es. UC) alle azioni (es. NBS) alle risoluzioni (es. ES). Il secondo obiettivo corrisponde alla definizione di un set di metodi e indicatori di valutazione biofisica e monetaria adeguate per valutare il valore della NBS in contesti urbanizzati. Per raggiungere questo obiettivo, viene sviluppata una revisione dei metodi esistenti sulla valutazione dei servizi ecosistemici, l'analisi dei costi del ciclo di vita e la valutazione del ciclo di vita. La revisione tiene conto di vincoli specifici come la facilità d'uso e la disponibilità dei dati. Alla fine, sono stati selezionati potenziali metodi e indicatori, che saranno successivamente integrati nel quadro di valutazione combinato. Il terzo obiettivo corrisponde alla progettazione del quadro di valutazione combinato, integrando metodi di valutazione del ciclo di vita, ecologia paesaggistica / urbana e servizi ecosistemici che quantifica il valore ambientale ed economico della NBS informando sull'efficacia in termini di costi del suo intero ciclo di vita. Per raggiungere questo obiettivo, prima viene sviluppato un quadro concettuale. Da esso, viene sviluppato un modello di dinamica di sistemi per calcolare i servizi (e disservici) ecosistemici, il quale è interrelazionato con un metodo di valutazione life cycle. Questa valutazione combinata viene testata con un tipo di NBS pertinente (foresta urbana) in un caso di studio nell'area metropolitana di Madrid. Il quarto obiettivo è lo sviluppo di uno strumento di supporto decisionale (DSS) che integri il quadro di valutazione come parte dei processi di progettazione iterativa nella pianificazione urbana e nella progettazione del paesaggio. Il DSS intende migliorare l'interrelazione tra scienza, politica e pianificazione / progettazione. Per raggiungere questo obiettivo è stato sviluppato Nbenefit$® un prototipo di DSS online per la valutazzione NBS di facile uso. Il prototipo DSS fornisce all'utente una forma semplice per quantificare multipli ES e costi (internalizatti o no) durante l'intero ciclo di vita (implementazione, vita operativa e fine vita) del NBS. In conclusione, questa tesi ha contribuito alla caratterizzazione di NBS e alla sua valutazione ambientale ed economica per informare i processi di pianificazione urbana e progettazione del paesaggio, consentendo decisioni più informate.

Economic evaluation of preventive methods used in dentistry – a systematic review

Syfte: Syftet med studien var att undersöka ekonomiska utvärderingar av preventiva metoder som används inom tandvården genom att göra en systematisk litteraturöversikt och besvara frågor om vilka preventiva metoder som analyserats avseende kostnad och effekt samt vilka typer av ekonomiska utvärderingar som analyserats. Material och metod: En sökningsstrategi genomfördes och PubMed, The Cochrane Library och Web of Science utgjorde databaser för litteratursökningen. Inklusions- och exklusionskriterier fastställdes och ett protokoll utgjorde grunden för läsningen av de inkluderade studierna i fulltext. En kritisk granskning av de inkluderade studierna genomfördes genom att använda ett andra protokoll. Resultat: Totalt 38 publikationer om ekonomiska utvärderingar av preventiva metoder som används inom tandvården inkluderades. Studier om kariespreventiva metoder var av övervägande majoritet (n=35) och endast ett fåtal studier utvärderade preventiva metoder för andra sjukdomar (n=3). Den vanligaste preventiva metoden som utvärderades var fissurförsegling (n=8) följt av fluorlackning (n=6) och vattenfluoridering (n=3). Många studier utvärderade kombinerade preventiva metoder (n=12). Typerna av ekonomiska utvärderingar var CEA (n=16) följt av CA/Cost-minimization analysis (n=10), CBA (n=3), en kombination av CEA och CBA (n=5), CUA (n=1) och en kombination av CEA och CA (n=1). Konklusion: Även om studierna om ekonomiska utvärderingar av preventiva metoder som används inom tandvården är betydligt fler än när SBU-rapporterna publicerades så kan ingen generell slutsats beträffande kostnadseffektiviteten dras på grund heterogenitet i design och sätt att analysera. Fler välgjorda studier rekommenderas. / Aims: The aim of this study was to examine economic evaluations of preventive methods used in dentistry by using a systematic review approach and answering questions about which preventive methods used in dentistry that have been analysed regarding costs and effects and what types of economic evaluations that have been conducted. Material and method: A search strategy were conducted and PubMed, The Cochrane Library and Web of Science formed the basis of the literature search. Inclusion- and exclusion criteria were defined and a protocol was made for full-text reading of the included publications. A critical appraisal of the included publications was made using a second protocol. Results: A total of 38 publications on economic evaluations of preventive methods used in dentistry were included. Publications on caries preventive methods were of overwhelming majority (n=35) and only a few publications evaluated preventive methods for other diseases (n=3). The most common evaluated preventive method was pit and fissure sealant (n=8) followed by fluoride varnish (n=6) and water fluoridation (n=3). Most publications evaluated combined preventive methods (n=11). Types of economic evaluations were CEA (n=16) followed by CA/Cost-minimization analysis (n=10), CBA (n=3), a combination of CEA and CBA (n=5), CUA (n=1) and a combination of CEA and CA (n=1). Conclusions: Publications on economic evaluations of preventive methods used in dentistry were considerably more than when the SBU reports were published. Yet, no general conclusions could be drawn regarding cost-effectiveness due to heterogeneity in design and way of analyses. Future well-designed studies are recommended.

Logistik för textilavfall inom Röda Korset : En analys av ekonomiska och miljömässiga aspekter / Logistics of textile waste in The Swedish Red Cross : An analysis of economic and environmental aspects

Varje år slänger svenskar nära 6 kg textil direkt i sitt restavfall, varav 60 procent ofta är i skick att fortsätta användas. I ett resursintensivt samhälle med alltmer sinande resurser är åtgärder för att minska textilkonsumtionen avgörande. Produktionen står för den största andelen av textilers klimatpåverkan i och med stor användning av vatten, energi och kemikalier. Därför är återanvändning av textil är en viktig åtgärd för att minska textilers klimatpåverkan.  Välgörenhetsorganisationer med second hand-verksamhet har en viktig roll i återanvändningen i Sverige, samtidigt återanvänds bara 20 procent av det som samlas in hos välgörenhetsorganisationer och resterande textil går främst till export och en liten andel till förbränning i Sverige. Att hantera den textil som inte säljs i Sverige kostar dock pengar som hade kunnat gå till välgörenhetsorganisationernas egentliga syfte. För Röda Korset, som denna studie är gjord för, är detta syfte att lindra och hindra katastrofer och kriser. Röda Korsets hantering av textilerna som inte kan säljas i Sverige behöver därmed vara kostnadseffektiv men också fortfarande ge en miljönytta, eftersom det är den främsta anledningen till att ta hand om textilavfallet.  Denna studie utgår från fem av Röda Korsets second hand-butiker och undersöker tre scenarier för hantering av detta textilavfall. Målet med studien är att undersöka vilket scenario som innebär den lägsta kostnaden för Röda Korset, störst miljönytta samt störst samhällsekonomisk nytta. Referensscenariot innebär att textilerna sorteras i butik där textilavfallet transporteras till närmaste återvinningscentral för förbränning. Scenario 1 innebär att textilerna sorteras i butik för att transporteras till depåer, för att sedan exporteras till Tyskland för återanvändning, materialåtervinning samt förbränning. Scenario 2 undersöker regional sortering av textilerna där textilavfallet transporteras till Tyskland för återanvändning, materialåtervinning samt förbränning.  Studien görs utifrån metoden för kostnads-nyttoanalyser där kostnader och nyttor för de olika scenarierna identifieras för att sedan monetäriseras. Data som används har erhållits av Röda Korset och har kompletterats med en litteraturstudie kring miljöpåverkan för de olika processerna samt monetäriseringen av dessa.  Resultatet av kostnads-nyttoanalysen var att regional sortering (Scenario 2) bidrar till störst samhällsekonomisk nytta och är samtidigt mest kostnadseffektivt för Röda Korset, medan Scenario 1 är mest miljömässigt fördelaktig på grund av de kortare transporter som scenariot innebär. En känslighetsanalys gjordes för felkällorna i rapporten, de som hade störst påverkan på resultatet var uppskattningen av arbetstid vid sortering i butik, exkluderandet av arbetstiden som görs av frivilliga samt vattenanvändningens miljöpåverkan i produktionen av textil. Trots deras stora påverkan på resultatet bedömdes de inte ha nog påverkan för att ändra det övergripande resultatet, att scenariot för regional sortering var mest lönsamt.  Eftersom Röda Korsets butiker har en stor variation i hur de drivs, geografisk placering och textilflöde skulle resultatet kunna skilja sig mycket beroende på vilka butiker som kopplas till en regional sortering. Detta gäller främst om butikerna har större avstånd, effektivare sorteringstakt eller extrema flöden jämfört med de som studerades i denna rapport. Utöver de kostnader och nyttor som tas med i denna rapport finns det även ytterligare fördelar med regional sortering för Röda Korset, dessa tas upp i diskussionen och är till exempel minskad sårbarhet för butikerna, mer specialiserad sortering som kan bidra till ökade intäkter samt minskade flöden till export.  Slutligen diskuteras även det producentansvar för textil som regeringen utreder som åtgärd för att minska andelen textil som slängs i restavfallet i Sverige. Slutsatsen från denna diskussion var att det skulle behövas ytterligare bredd i utredningen för producentansvaret. Dels kring de rekyleffekter som skulle kunna uppstå, som ökad konsumtion i vetande om att textiler man gör sig av med inte bara förbränns, dels kring hur man praktiskt ska genomföra producentansvaret för att textilflödena ska gå till återanvändning och inte direkt till materialåtervinning. På grund av den låga återanvändningsgraden i Sverige skulle också ytterligare studier behövas kring de hinder som finns för att svenskar ska öka sin återanvändning. / Every year, a Swede throws close to 6 kg textile directly into their residual waste, even though 60 percent of this textile waste often is in good condition and could be re-used. In a resource-intensive society with increasingly scarce resources, measures to reduce textile consumption are crucial. The production accounts for the largest part of textiles' climate impact due to the extensive use of water, energy, and chemicals. This is why re-use of textiles is an important measure to reduce the climate impact of textiles.  Charities with second-hand activities have a significant role for increasing re-use of textiles in Sweden. However, only 20 percent of what is collected by charities is reused and the remaining textile goes mainly to exports, and a small proportion to incineration in Sweden. On the other hand, managing the textiles that remain unsold in Sweden costs money that could have gone to the main purpose of the charities. For the Swedish Red Cross, which this study was conducted for, their main purpose is to alleviate and prevent disasters and crises. Thus, the Swedish Red Cross' handling of their unsold textiles needs to be cost-effective but also provide an environmental benefit, as this is the main reason for taking care of textile waste.  This study is based on five of the Swedish Red Cross' second-hand stores and examines three scenarios of logistics for managing their textile waste. The aim of the study is to investigate which scenario entails the lowest cost for the Swedish Red Cross, the greatest environmental benefit, and the greatest socio- economic benefit. In the reference scenario, the textiles are sorted in the stores where the textile waste later is transported to the nearest recycling center for incineration. Just as in the reference scenario, the textiles are sorted in the stores in Scenario 1, but the textile waste is instead transported to depots, and finally exported to Germany for reuse, material recycling and incineration. Scenario 2 examines regional sorting of textiles where the textile waste, as in Scenario 1, is transported to Germany for reuse, material recycling and incineration.  The study is based on the method for cost-benefit analyzes where costs and benefits for the different scenarios are identified and monetized. The data have been obtained from the Swedish Red Cross and have been supplemented with a literature study on the environmental impact of the various processes and their monetization.  The result of the cost-benefit analysis was that regional sorting (Scenario 2) both contributes to the greatest socio-economic benefit and is the most cost-effective scenario for the Swedish Red Cross, while Scenario 1 is most environmentally beneficial due to the shorter transports that the scenario entails. A sensitivity analysis was conducted for the sources of error in the report. The sources of errors that were assessed to have the greatest impact on the result were the estimate of working hours for sorting, the exclusion of working hours done by volunteers and the environmental impact of water use in textile production. Despite their substantial impact on the result, these sources of error were assessed not to have enough impact to change the overall result, that the scenario for regional sorting was most profitable.  Since the Swedish Red Cross' stores have a large variation in how they are administered, geographical location and textile flow, the result could differ greatly depending on which stores are linked to a regional sorting. These factors are particularly relevant if the stores have greater distances, more efficient sorting rates or extreme textile flows compared to the stores studied in this report. In addition to the costs and benefits included in this report, there are other benefits of regional sorting for the Swedish Red Cross. Some are discussed in this report, such as reduced vulnerability for the stores, more specialized sorting that can contribute to increased revenues and reduced flows to export.  Finally, the Swedish government is investigating a producer responsibility for textiles as a measure to reduce the proportion of textiles that are thrown away in residual waste in Sweden. The conclusion from this discussion was that further expanding of scope in this investigation is needed. For example, with an inclusion of the rebound effects that could occur, such as increased consumption with the knowledge that textiles you dispose are not just incinerated. Furthermore, how to practically implement producer responsibility so that textile flows go to re-use and not directly to material recycling. Due to the low degree of re-use in Sweden, further studies would also be needed on the obstacles that exist for Swedes to increase their re-use.

An economic analysis of digitalized and standardized workflows within the operating room

Tesis por compendio / [ES] La digitalización en la sanidad sigue poco desarrollada en comparación con muchos otros sectores, pero ofrece la posibilidad de hacer frente a retos cada vez mayores. A partir del caso de un sistema digital de gestión del flujo de trabajo implantado en el quirófano, esta tesis examina cuál podría ser el impacto de la digitalización en la calidad, la eficiencia y la economía de la gestión hospitalaria. La metodología de investigación se basa en un enfoque cuantitativo. En primer lugar, se ha desarrollado una visión general y un análisis del mercado sanitario europeo, así como una amplia y sistemática revisión bibliográfica. Esta se ha enfocado en la eficiencia y el impacto económico de la estandarización y digitalización de los procesos intraoperatorios en el quirófano. Los resultados proporcionaron la base para derivar las preguntas de investigación en torno a los sistemas digitales de gestión del flujo de trabajo. Para cubrir las lagunas de investigación identificadas, se han obtenido y analizado de forma retrospectiva datos de un hospital, como estudio de caso, sobre un sistema digital de gestión del flujo de trabajo, Surgical Process Manager (SPM), en ortopedia y cirugía general. Los datos adicionales proporcionados por los pacientes, se han utilizado de forma retrospectiva para realizar cálculos económicos con el fin de responder a la pregunta principal de la investigación en cirugía general, centrándose en la cirugía de la obesidad. Las principales conclusiones de la tesis muestran que la implantación de sistemas digitales de gestión del flujo de trabajo, como el SPM, mejora la eficiencia y la economía en la cirugía de la obesidad. El análisis de odds ratio (razón de oportunidades/probabilidades) para evaluar el impacto en la calidad no permitió extraer conclusiones ni en ortopedia ni en cirugía de la obesidad. El cálculo coste-beneficio en cirugía de la obesidad mostró un ahorro de costes de 318 euros por paciente, lo que supuso un total de 10.073 euros. Este beneficio económico se consiguió gracias a la disminución de la duración de la estancia (-1,2 días). Por primera vez, esta investigación aporta pruebas sobre el valor económico de los procesos digitalizados y estandarizados en el quirófano en cirugía general y ambulatoria. Los resultados facilitarán las decisiones de inversión en digitalización de la gestión hospitalaria y ofrecerán opciones para superar la carga económica en el mercado sanitario actual. La metodologia d'aquesta investigació es basa en un enfocament quantitatiu. En primer lloc, es va fer una visió general i anàlisi del mercat sanitari i una extensa revisió sistemàtica de la literatura. Es va centrar en l'eficiència i l'impacte econòmic de l'estandardització i la digitalització dels processos intraoperatoris al quiròfan. This thesis examines the example of digital workflow management implemented in the surgical Operating Room (OR) and what the impact of digitalization on quality, efficiency and economics for hospital management could be. The methodology for this research is based on a quantitative approach. First, an overview and analysis of the healthcare market and an extensive systematic literature review were carried out. It was focused on the efficiency and economic impact of standardization and digitalization of intraoperative processes in the OR. The findings provided the basis for the research questions around digital workflow-management-systems. Single-center hospital data on a digital workflow-management-system, Surgical Process Manager (SPM), has been retrospectively extracted and analyzed in orthopedics and general & visceral surgery (G&V) to fill identified research gaps. Additionally, provided patient data has been retrospectively used for economic calculations to conclude the main research question in G&V, focusing on obesity surgery. The main findings of the thesis show that implementing digital workflow management systems, in the example of the SPM, improves efficiency and economics in obesity surgery. The odds ratio analysis to assess the impact on quality did not allow conclusions in orthopedics or obesity surgery. The cost-benefit calculation in obesity surgery showed cost savings of 318 € per patient, totaling 10,073€. This economic benefit was achieved by decreasing the length of stay (-1.2 days). For the first time, research provides evidence of the economic value of digitized and standardized processes in the OR in G&V. The results will facilitate investment decisions in digitization of hospital management and offer options to overcome the financial burden in the current healthcare market.
