Ciulla, Leandro 26 February 2013 (has links)
Made available in DSpace on 2015-04-14T13:53:56Z (GMT). No. of bitstreams: 1 447890.pdf: 8825285 bytes, checksum: 2f13c7fbfd2c35d22260ef80079c2bf9 (MD5) Previous issue date: 2013-02-26 / Population aging is a global phenomenon that occurs more rapidly in developing countries like Brazil. As a consequence, there is the increased frequency of health problems characteristic of the elderly, including mental disorders. Depression is the most common psychiatric illness among the elderly. This pathology can achieve different levels of severity, from mild to the most severe and associated with suicide. Older individuals are at higher risk of suicide than any other population. The aim of this study is to evaluate ideation, attempts and risks of suicide in a random sample of the elderly attended by the Family Health Program of Porto Alegre. A prospective cross-sectional study was conducted. The sample consisted of 530 randomly selected older people from 27 Family Health Teams in the city of Porto Alegre (FHT/POA) drawn in a stratified manner by the "Health Districs". Diagnosis was made by psychiatrists experienced in evaluating the elderly and using the Brazilian version of the Mini International Neuropsychiatric Interview (M.I.N.I 5.0.0 plus). The results showed a total of 15.7% (n=83) of the elderly sample presented a risk of suicide. Of this total 10% (n = 53) had low risk, 0.8% (n = 4) moderate risk, and 4.9% (n = 26) high risk. Among the subjects who were at risk for suicide a few significant associations can be highlighted: female (19.5%, P <0.001), not having income (34.1%, P = 0.022) and loss of one or more children (22.6%, P = 0.007). Regarding the mood it was found that 59.2% (n=45) of those individuals with current unipolar depression presented a suicide risk, whilst 62.5% (n=5) with current bipolar disorder and 33.3% (n=8) with a past history of this disease also presented a risk of suicide. A high rate of suicide risk was found by this study in the elderly population living in the community of Porto Alegre. Increased mental distress was noted in the evaluated individuals and it was possible to identify predictive factors for suicide risk through the analyses performed. The findings of this study are worrying, being necessary to be replicated in other localities and samples. Even so, the results cited clamor for urgency in promoting the treatment of mental disorders and suicide risk, especially those specializing in the elderly.

The causes of this mood disorder are multi factorial, and recent research points to a decrease in oxytocin, a neuropeptide, which acts as a neurotransmitter and neuromodulator, plays an important role in social behavior. The gene for its receptor (OXTR) is located on the short arm of the human chromosome 3p25, and a single nucleotide variation (rs2254298), have been linked with the emergence of mood disorders. Due to a point mutation, the guanine (G) was replaced by adenine (A), causing the G allele carriers to be favored by the behavioral relations. On the other hand, absence or a poor or ineffective social network, low self-esteem, associated with aging, OXTR polymorphism and depression may increase cardiovascular risk factors (CRF), making the elderly more susceptible to a negative outcome. PURPOSE: to analyze the association of oxytocin receptor gene polymorphism (2254298) with depression, social support, self-esteem and cardiovascular risk factor (CVRF). METHODS: A cross-sectional study carried out with a population of elderly people from the basic care of Porto Alegre, with and without depression, diagnosed by the Brazilian version of Mini International Neuropsychiatric Interview (MINI Plus), DNA extraction was performed from the leukocyte layer, and Genotyping by the Polymerase Chain Reaction and Restriction Fragment Length Polymorphism (PCR-RFLP) technique, and the digestion fragments were run with agarose gel stained with ethidium bromide. Self-esteem and social emotional / affective support were verified through the Rosemberg and Griep Social Support scales respectively. Hypertension, diabetes mellitus (DM), dyslipidemia, obesity, sedentary lifestyle and smoking were the CVRF investigated, and the metabolic variables investigated were triglycerides and postprandial glucose concentration by capillary puncture. Body mass index (BMI) and waist circumference and blood pressure were also measured. The hormones oxytocin and cortisol were obtained through the serum and measured by the ELISA technique. RESULTS: The sample consisted of 177 elderly people of basic care in Porto Alegre, mean age 72.6 ? 6.9, of which 129 (72.9%) were female. 66 (37.3%) had a diagnosis of depression, there was no association between OXTR rs 2254298 with depression, social support, self-esteem and CVRF. In the Poisson regression analysis, it was observed that the self-esteem (p = 0.031), and the social interaction domain (p = 0.037). A negative correlation was observed between serum oxytocin and BMI (p = 0.019) and postprandial triglycerides, (p = 0.035). The elderly classified as lean by Lipschitz criterion had significantly higher oxytocin averages (p = 0.005) and this relationship was maintained by regression analysis (p = 0.010). CONCLUSIONS: The results suggest that the OXTR polymorphism (rs 2254298) is not associated with depression, social support, self-esteem, and cardiovascular risk factors in this sample. The final multivariate analysis showed a higher frequency of depression in individuals with lower self-esteem and social interaction; and the elderly with higher levels of serum oxytocin had lower BMI, and those with lower concentration of this hormone, higher postprandial TGC.

The sample was comprised of 301 elderly people, with an average age of 71,1 years, predominantly women (71,8%). This paper is attached to the Aging and Mental Health Program line of research of the Post-Graduate Program of the Geriatrics and Gerontology Institute (IGG) of the PUC-RS University. The data was gathered through structured interviews (sociodemographic data collected by questionnaire forms), and by application of the GDS-15 instrument (depressive symptom tracking). RESULTS: Possessing an average income of over one minimum wage is a protective factor, decreasing the occurence of depressive symptoms (42%) when compared to those with no income. Elderly with individual income of less than one minimum wage also presented a reduction in the occurrence of depressive symptoms of 35% when compared to those with no income. Besides individual income, elderly with daily use of reading and writing also presented a reduction in the occurrence of depressive symptoms of 21%. The biggest significance variable that distinguishes both genders was marital status. In men, the lack of companionship increase depression by 82% (RP=1,82; IC 95%: 1,20 ? 2,76; p=0,005). In women, marital status wasn't a significant indicator of depression (RP=1,01; IC 95%: 0,79 ? 1,29; p=0,948).

Pesquisa do Estado da Bahia - FAPEB / Introduction: Lichen Planus is a chronic inflammatory disease of uncertain etiology that may affect skin or mucosa, especially the oral mucosa. Objective: Evaluate the clinical epidemiological profile, and psychological and salivary factors of patients with oral lichen planus. Materials and Methods: The clinical epidemiological profile was assessed by descriptive analysis of 38 dental records of individuals with clinical/pathological diagnosis of oral lichen planus, treated at an Oral Lesions Reference Center in the period between 2005 and 2015 (Article 1). A case control study was conducted to evaluate the levels of anxiety, depression and stress, and both flow and pattern of secretion of salivary cortisol in 42 adults. The experimental group was composed of 21 individuals with a clinical/histopathological diagnosis of oral lichen planus. The control group originated from the same population of cases, consisted of 21 individuals without the disease, randomly admitted and matched to the cases by sex and age. Both groups underwent clinical examination of the oral cavity, structured interview, psychological tests (Beck Anxiety and Depression Inventory and Perceived Stress Scale) and saliva collection at three different times of the day (Article 2). Data was initially analyzed by means of descriptive statistics, by means of absolute and relative frequency and centralization and dispersion steps. Bivariate analysis was performed between the study variables and the presence of oral lichen planus, estimating the odds ratio (OR) as a measure of association, and using confidence intervals at 95%. The Mann-Whitney test was used to compare the groups with regard to scores of anxiety, depression and stress, and the pattern of salivary cortisol secretion. The Friedman test was used to evaluate the pattern of salivary cortisol secretion throughout the day. We used the t-Student test to compare the mean salivary flow between the groups, as well as the area under the curve of the cortisol response to awakening. The correlation between cortisol concentrations with the scores of depression, anxiety, stress and salivary flow was measured through the Spearman correlation coefficient. The adopted significance level was 5%. Results: In Article 1, we observed that 68.42% of the subjects were aged greater than or equal to 40 years; 65.80% were women and 78.90%, had black or brown skin. Most had no systemic disease (68.42%). As for lifestyle, 42.10% were drinkers and 26.30% were smokers. Reticular lichen planus was the most prevalent clinical form (78.13%), mainly affecting the oral mucosa (68.40%). In Article 2, a significant association was found between anxiety scores (p = 0.001), depression (p = 0.005) and perceived stress (p = 0.026), but not when compared to the salivary flow (p = 0.29) or to the pattern of salivary cortisol secretion when waking up (p = 0.98), 30 minutes after waking up (p = 0.95) and at nighttime (p = 0.97). Conclusion: Most cases of lichen planus of the reticular type, usually located in the oral mucosa, occurred in aged over 40, white women. Through psychological tests, it was observed an association between oral lichen planus and anxiety, depression and stress, but not in relation to the analyzed salivary factors. A maioria n?o apresentava doen?a sist?mica (68,42%).

Objective: To evaluate psychological (anxiety, depression and stress) and salivary (flow and levels of salivary alpha amylase) factors in individuals with oral lichen planus. Materials and Methods: An exploratory control case study was conducted. The case group consisted of 23 individuals with clinical and histopathological diagnosis of oral lichen planus, matched by sex and age to a control group, with an equal number of healthy individuals, from the same population that originated the cases. The population underwent clinical examination of the oral cavity, psychological tests (perceived stress scale and Beck depression and anxiety inventories) and saliva collection to determine the salivary flow and alpha-amylase levels at three moments along the day (on waking, thirty minutes after waking and before bed). The data were analyzed statistically. The Odds Ratio and its 95% confidence interval were computed. The McNemar ?2 test was used, considering a significance level of 5%. For quantitative variables, the Mann-Whitney test was used to compare the medians of salivary flow, alpha amylase response on waking, and salivary alpha amylase levels at each of the collection times, between cases and controls. The Friedman test was applied to evaluate the concentration of the alpha amylase from each of the three moments of saliva collection throughout the day. Spearman's correlation coefficient was computed to determine correlation between alpha amylase concentration and the test scores of stress, anxiety, depression, and salivary flow. Results: There was a significant correlation between anxiety, depression, stress and LPB, and between stress and ?-AS decline. Higher levels of salivary alpha amylase were found in the case group in the second collection time when compared to controls. Conclusion: The applied psychological tests indicated that factors such as stress, anxiety and depression were correlated to oral lichen planus. Salivary analyses demonstrated a higher concentration of alpha amylase thirty minutes after waking on individuals with the disease, suggesting the role of alpha amylase as a promising biomarker for further longitudinal studies on the subject.

Several studies have reported impaired quality of life in epilepsy associated with different clinical and psicossocial aspects. However, it is still little studied in adult patients with epilepsy (PWE) the ability to deal with adverse events and whether there is relationship between resilience and aspects of epilepsy. Objetive: Assess the resilience and its relationship with sociodemographic, clinical and the perception of life in PWE. Materials and procedures: 80 consecutive adults PWE were included, treated in the clinic Clinical Neurology Hospital Celso Pierro (PUC-Campinas). The PWE were assessed: sociodemographic questionnaire, medical history, economic classification criteria Brazil, Wagnild & Young resilience scale, life orientation test (LOT-R) and Inventory of depression in neurological disorders epilepsy (NDDI-E) . Statistical tests were used with significance level of p <0.05. Results: The average age of PWE was 45.4 (? 14.4) years, 46 (57.5%) were females; 38 (47.5%) and 42 are unemployed (52.5%) and 41 (51.2%) patients were married. The average age of the first patients seizure was 21.1 (? 17.1) years; 49 (61.2%) were seizure for less than a year and the average duration of epilepsy was 27.7 (? 16.2) years. The seizure type was focal in 67 (83.75%) cases and generalized only in 13 (16.25%). Fifty-one PWE were using one antiepileptic drug (AED), and more than two drugs, 29 (36.2%) cases. The epileptic syndrome was, idiopathic generalized epilepsy in 13 cases (16.25%), symptomatic focal in 45 (56.25%) and probably symptomatic focal in 22 cases (27.50%). In symptomatic epilepsies etiology was temporal lobe epilepsy with hippocampal sclerosis in 27 (33.7%) cases. The average score of the resilience scale was 139.2 (? 15.6). In classification level resilience was observed that 15 (18.7%) patients had low resilience (?125), middle (> 125 and ?145) and high resilience (> 145) were respectively 32 (40%) and 33 (41.2%) patients. The average score TOV-R was 20.5 (? 3.4) and NDDI-E 12.6 (? 2.6). The PWE that used two or more AED had significantly lower scores on scale resilience when compared to those using single drug (142.3 ? 14.7 133.9 ? 16.1 x; t-test, p = 0.025). It was observed that the PWE presenting depressive episode (values above 15) in NDDI-E show significantly lower scores on scale resilience (141.7 ? 14.1 x 119.8 ? 14.2; T-test; p = 0.000) and lower scores on TOV -R (21.0 ? 2.6 x 16.5 ? 6.1, t-test, p = 0.000). There was a significant positive correlation between the scores of resilience scale with the TOV-R. There was a negative correlation between the scores of resilience and range scores NDDI-E. Discussion and conclusion: Most PWE showed medium and high resilience, suggesting that persons with high resilience can develop skills to overcome challenges and adversity and unpredictability of epilepsy. In screening for depressive episodes in NDDI-E was observed average values do not suggestive of depressive symptoms. Low resilience was significantly associated with the use of more than one AED. There was no association with other clinical and socio-demographic aspects of epilepsy. PWE with greater optimism in the perception of their lives (LOT-R) have more resilience. There was an association between depressive symptoms (NDDI-E) and lower resilience. In conclusion, less resilients PWE use more than one AED, show less optimism to perceive their lives and more depressive symptoms.

It is considerated one of the great reasons that take people to search health care assistance in the world. Its symptoms are sad humor, melancholy, cry, apathy, tedium, chronic dislike, angry increase, anxiety, despair, fatigue, tiredness, less wish, beyond other symptoms. From the point of view of behavior analysis, depression must be understood by analyzing the contingencies that sustain the behavior because the main reasons that explain the behavior must be searched in people relations with the external environment. Looking by the perspective that a behavior must be studied in terms of its function, depression could be understood as responses classifications selected and developed during the person?s life considering the interaction with the environment that the person was exposed. The purpose of this present study was to identify functional relations among the verbal report of five people with depression diagnosis. It was chosen to do the analysis of the most frequent behavior in each participant. The methodologies used for the selection of the behavior were Beck?s Depression Inventory (BDI), clinic interviews and self-observation. After the selection of the behavior, additional interviews were included in order to collect information about the function of the behaviors and about the life history of each participant. The results showed similar functional processes in all participants. The contingences involved in the most frequent behavior in each one were about negative reinforcement and as a result, it was observed avoidance behavior under an aversive stimulation. In the investigation of their life history related with the most frequent behavior, were detected learning aspects of the behavior and negative reinforcement such as frequent situations of avoidance, low incidence of positive reinforcement, frequent punishment situations in childhood and adolescence and lack of positive reinforcements. It was concluded that the contingences involved in the most frequent behavior and the investigation of the relationships during the life history of these participants were adequate methods to obtain data regarding to depression. In addition, it gives information about the interaction of these people with their environment. Therefore, investigations like this study could support a more specific assessment in each case, contributing for a better treatment.

It has been carried out the factorial analysis and the scale remained with 24 items in 4 factors: Depressed Mood, Social Relationship, Self-Esteem and Autonomic-Physiologic Response. The Cronbach Coefficient has shown good precision (alfa=0,82). Table in percentile and T score were elaborated for: type of school, sex, age, school grade, age range and school grade range. Statistical analysis (Mann-Whitney and Kruskal-Wallis) have shown the necessity of specific standards for: type of school (p<0,001), sex (p<0,001) and public school x sex (p<0,001). In the analysis of the children who presented percentile above 75, the Qui-Square showed significant difference for: type of school (x?=11,30 GL=1; p<0,001), sex (x?=9,79; GL=1; p=0,002) and public school x sex (x?=11,13; GL=1; p<0,001). The Children Depression Evaluation Scale has proven to be valid and trustworthy for the use in the Brazilian population, being of great value for mental health research, prevention and treatment.
