Patienters upplevelse av vården inom rättspsykiatrin : En allmän litteraturöversikt / Patients’ experience of care in forensic psychiatry : A general litterature review

Background: A person with a serious mental disorder who commits a crime can be sentenced to forensic psychiatric care. Forensic psychiatric care is permeated by a high sense of security, while patients are expected to follow a predetermined treatment plan. This restrictive environment, where patients are forcibly cared for during indeterminate time leads to a complex ethical dilemma. Patients are treated for their mental illness whilst the restrictive nature of the care environment deprives them of the opportunity for full autonomy, which can affect their overall experience of the care period. Aim: The aim was to describe patients' experience of their care in forensic psychiatry. Method: A general literature review based on ten scientific articles with a qualitative approach that were analyzed with a content analysis. Results: Three main categories were identified: Being locked up and supervised, The importance of interpersonal encounters and Rehabilitation process in forensic psychiatry. Mentioned main categories were divided into six subcategories. Conclusion: The strict requirements of the forensic psychiatric care form require an adaptation of the admitted patients. Patients' experience the lack of meaningful activities and autonomy. Central are the relationships between care recipients and care providers to evoke a sense of safety and meaningfulness for the patients. A therapeutic approach is important and seeing the human behind the crime. Suggestions for further research: The results show the absence of therapeutic roles in forensic psychiatry. The need for guidelines regarding the therapeutic treatment of forensic psychiatric patients should thus be considered to be developed. This to create a safe and empathetic existence that contributes to a health-promoting environment for forensic psychiatric patients. / Bakgrund: Personer med en allvarlig psykisk störning som begår ett brott kan dömas till rättspsykiatrisk vård. Rättspsykiatrisk slutenvård genomsyras av strikta riktlinjer kring säkerhet samtidigt som patienterna förväntas följa en förutbestämd behandlingsplan. Denna restriktiva miljö där patienterna tvångsvårdas under en obestämd tid bidrar till ett komplext etiskt dilemma, då personer behandlas för sin psykiska ohälsa samtidigt som vårdmiljöns restriktiva natur fråntar deras möjlighet till full autonomi, vilket kan påverka deras helhetsupplevelse av vårdtiden. Syfte: Syftet var att beskriva patienters upplevelse av vården inom rättspsykiatrin. Metod: En allmän litteraturöversikt baserat på tio vetenskapliga artiklar med kvalitativ ansats som analyserades med en innehållsanalys. Resultat: Tre huvudkategorier identifierades: Att vara inlåst och övervakad, Betydelsen av mellanmänskliga möten och Rehabiliteringsprocessen inom rättspsykiatrin. Nämnda huvudkategorier delades in i sex underkategorier. Slutsats: Den rättspsykiatriska vårdformens strikta krav kräver en anpassning av de inlagda patienterna. Patienterna upplever avsaknad av meningsfulla aktiviteter och autonomi. Centralt är relationerna mellan vårdtagare och vårdgivare för att framkalla en känsla av trygghet samt meningsfullhet hos patienterna. Att vara patient med HIV: erfarenheter av möten med sjukvårdspersonal : En systematisk litteraturstudie

Bakgrund: Humant Immunbrist Virus (HIV) är en infektionssjukdom som angriper kroppens immunförsvar vilket leder till att det stegvis försämras och slutligen helt avtar. Historiskt sett har patienter med HIV blivit diskriminerade i samhället och orättvist behandlade. Joyce Travelbee’s omvårdnadsteori tar avstånd från en generaliserande människosyn och förespråkar att se individens unika egenskaper, och sjukvårdspersonal ska enligt lag möta patienter med lika värde och lika vård oavsätt diagnos. Syftet är att beskriva erfarenheter som vuxna patienter med HIV har av möten med sjukvårdspersonal. Metod: En systematisk litteraturstudie. Arbetet utfördes genom en systematisk sökning  efter vetenskapliga artiklar i tre databaser där elva artiklar sedan kvalitetsbedömdes och analyserades. Resultat: Patienter med HIV hade övervägande positiva erfarenheter av mötet med sjukvårdspersonal, men negativa erfarenheter av till exempel obekväma blickar, överdrivna säkerhetsåtgärder, anklagelser och respektlöshet förekom i olika vårdsammanhang. Slutsats: Patienter med HIV ska aldrig behöva erfara diskriminerande möten med sjukvårdspersonal på grund av sin diagnos. Uppmärksammande av erfarenheter av möten med sjukvårdspersonal hos patienter med HIV kan öka medvetenheten angående stereotypa negativa beteenden och därmed förbättra deras vård och erfarenheter av mötet. Existentiellt lidande i sen palliativ fas : En systematisk litteraturstudie utifrån vårdpersonals erfarenheter

Bakgrund: När patienter vårdas i sen palliativ fas är existentiellt lidande något som kan drabba patienterna oavsett om de vårdas på vårdavdelningar, genom hemsjukvård eller på specialiserad palliativ vårdavdelning. Detta kan innebära att de behöver få stöd och lindring i sitt existentiella lidande av vårdpersonalen då existentiella behov ska bli mötta för att dessa patienter ska erhålla en god livskvalitet. Syfte: Att beskriva vårdpersonals erfarenheter av att vårda patienter med existentiellt lidande i sen palliativ fas. Metod: Kvalitativ litteraturstudie med beskrivande syntes.Resultat: Efter analysen framkom två teman och sex subteman. Temat Meningsfullt vårdande inkluderar Att få patienterna att öppna upp genom förtroendefulla relationer och Att hitta sätt att stödja patienterna. Temat Utmaningar i vårdandet inkluderar Att finna en balans i sin professionella roll, Att identifiera existentiellt lidande, Att hantera svåra samtal och Att begränsas av yttre förutsättningar. Slutsats: Vårdpersonalen använder enkla medel i omvårdnaden såsom att enbart dela tystanden vilket ger ett meningsfullt vårdande men möjligheterna att identifiera och lindra patienternas existentiella lidande hämmas av tidsbrist och patienternas mentala och fysiska hälsa. Även vårdpersonalens egen dödsrädsla och tron att deras kunskaper brister eller att samtal om döden ökar lidandet minskar möjligheterna för att lindra existentiellt lidande. / Background: Existential suffering can afflict patients in late palliative phase, regardless whether they are cared for in a ward, receive home healthcare or in a specialized palliative care unite. This may mean that health professionals need to support the patients and provide relief in their existential suffering since meeting their existential needs is important in order for the patients to receive a good quality of life. Aim: To describe health professionals' experiences of caring for patients with existential suffering in late palliative phase. Method: A qualitative approach, through a literature study with a descriptive synthesis. Results: After the analysis two themes and six subthemes emerged. The theme Meaningful caring includes Getting the patients to open up through trustful relationships and Finding ways to support the patients. The theme Challenges in caring includes Finding a work-life balance, Identifying existential suffering, Managing difficult conversations and Being limited by external conditions. Conclusion: Nursing actions such as solely sharing the silence resulting in meaningful caring are often used but time restraints and the patients’ overall health conditions limits the possibilities to identify and ease their existential suffering. Additionally, health professionals' death anxiety and experienced knowledge gaps or beliefs that death conversations increase suffering limits the possibilities to ease existential suffering.

TRANSMISSION AND PATHOGENESIS OF HANTAVIRUS / HANTAVIRUS ÖVERFÖRING OCH PATOGENES

Hantaviruses are the causative agents of hemorrhagic fever with renal syndrome (HFRS) in Eurasia, and of hantavirus cardiopulmonary syndrome (HCPS) in the Americas. Transmission to humans usually occurs by inhalation of aerosolized virus-contaminated rodent excreta. To date, human-to-human transmission has only been described for the Andes hantavirus. The mode of transmission of Andes hantavirus is not yet known, but transmission through saliva has been suggested. In Sweden, we have one hantavirus that is pathogenic to humans, Puumala virus (PUUV), which is endemic in Central and Northern Europe. It induces a relatively mild form of HFRS, also called nephropathia epidemica (NE). The rodent reservoir is the bank vole (Myodes glareolus). The mechanism behind the pathogenesis of hantavirus is complex and probably involves both virus-mediated and host-mediated mechanisms. The aim of this project was to investigate the transmission mechanisms and pathogenesis of hantavirus disease in humans. In our first study, we described the largest outbreak of PUUV so far in Sweden. We investigated factors that might be important for causing the outbreak, and suggested that a peak in the bank vole population together with concurrent extreme weather conditions most probably contributed to the outbreak. Our next studies concentrated on human-to-human transmission of hantaviruses. We found PUUV RNA in saliva from PUUV-infected patients, suggesting that there is PUUV in the saliva of infected humans, although no person-to person transmission appears to occur with PUUV.  In the studies that followed, we showed that human saliva and human salivary components could inhibit hantavirus replication. We also found PUUV-specific IgA in the saliva of PUUV-infected patients, which might prevent person-to-person transmission of the virus.  In the final study, we focused on the pathogenesis of NE. One hundred five patients were included in a prospective study.  They were divided into a group with mild disease and a group with moderate or severe disease. We found that the immune response had a dual role in disease development. It was partly responsible for development of severe disease, with significantly higher amounts of neutrophils in severely ill patients, but it was also protective against severe disease, because patients with mild disease had higher levels of PUUV-specific IgG. In conclusion, a peak in the bank vole population in combination with extreme weather will increase the risk of human infection, PUUV RNA is present in saliva, PUUV-specific IgA and salivary components inhibit person-to-person transmission of PUUV, and the immune response is important for the pathogenesis of PUUV and the severity of the disease. / Hantavirus är en grupp av virus som finns hos gnagare som bär på viruset utan att själva bli märkbart sjuka. Varje hantavirus har anpassat sig till sin egen art av gnagare som de infekterar (kallas virusets reservoar). Hantaviruset kan överföras till människor från gnagare och kallas då för en zoonos eftersom detsprids från djur till människa. I människa orsakar hantavirus blödarfeber med njurpåverkan i Eurasien och blödarfeber med med hjärt och lungpåverkan i Nord- och Sydamerika. I Sverige har vi bara ett hantavirus som är sjukdomsframkallande hos människor, Puumala-viruset som även finns i delar av övriga Europa. Det framkallar en relativt mild form av blödarfeber, som kallas sorkfeber eller Nephropathia epidemica. Puumala-virusets reservoar är skogssorken (Myodes glareolus). Människor smittas oftast av hantavirus när de andas in infekterat damm som innehåller utsöndringar (avföring, urin eller saliv) från gnagare som har torkat in och sedan blivit luftburet. Vad man vet hittills så finns det bara ett hantavirus som smittar från person till person, för övriga hantavirus är människan en ”dead end”. Det virus som kan smitta från person till person heter Andes hantavirus och finns i Sydamerika. Andes hantavirus har en mus som reservoar från vilken människor kan smittas, sedan har smittan i vissa fall förts vidare från människa till människa, som tur är har dessa utbrott gått att stoppa. Fastän utbrotten har varit små har många personer dött, eftersom dödligheten är så hög, ungefär 30-40% av de diagnostiserade fallen dör. Hur Andes hantavirus överförs från människa till människa är inte känt men överföring genom saliv har föreslagits. Hur viruset ger upphov till sjukdom hos människa är inte klarlagt. Studier talar för att mekanismen bakom sjukdomsutvecklingen (den så kallade patogenesen) hos hantavirusorsakade blödarfebrar är komplex. Sannolikt beror patogenesen både på egenskaper hos viruset och värden d.v.s. människan som är smittad av viruset. Vårt mål med detta projekt var att undersöka vad som hindrar överföring av Puumala hantavirus från människa till människa och att undersöka hur virusinfektionen påverkar sjukdomsutvecklingen hos människan. I vår första studie beskrev vi det största utbrottet av sorkfeber hittills i Sverige och vi undersökte faktorer som kan ha orsakat utbrottet. Vi föreslog att en topp i skogssorkpopulationen samtidigt med extremt varmt väder troligen bidrog till utbrottet. Utbrottet skedde i december och det extremt varma vädret medförde att snön smälte bort. Sorkarna bor vanligtvis under snön på vintern, vi tror att frånvaro av snötäcke fick sorkarna att söka sig till byggnader för att söka skydd och där kom i kontakt med människor. Våra efterföljande studier fokuserade på överföring av hantavirus från människa till människa. Vi hittade Puumala-virusets arvsmassa (RNA) i saliv från sorkfeberpatienter, vilket tyder på att det finns Puumala-virus i saliven hos infekterade människor, även om ingen överföring från person till person verkar inträffa. I efterföljande studier visade vi att mänsklig saliv och mänskliga salivkomponenter minskar hantavirus smittsamhet. Vi fann också Puumala-virusspecifika IgA-antikroppar i saliven från sorkfeberpatienter, vilket kan förhindra överföring från person till person. I den sista studien fokuserade vi på patogenesen hos människor efter hantavirusinfektion. 105 patienter ingick i en prospektiv studie och delades in i en grupp med mild sjukdom och en grupp med måttlig/svår sjukdom. Vi hittade en dubbel roll hos immunsvaret för sjukdomsutvecklingen. Immunsvaret var delvis ansvarig för utveckling av svår sjukdom med betydligt högre mängd neutrofiler hos svårt sjuka patienter, men det var också skyddande mot allvarlig sjukdom, eftersom patienter med en mild sjukdom hade högre nivåer av Puumalavirusspecifika IgG-antikroppar. Detta talar för att behandling med IgG-antikroppar specifikt riktade mot hantavirus skulle kunna vara effektiv hos hantavirusinfekterade patienter. Sammanfattningsvis; en topp i skogssorkspopulationen i kombination med extremt väder ökar risken för infektion hos människor; Puumala-virus arvsmassa (RNA) finns i saliv; Puumala-virusspecifika IgA-antikroppar och salivkomponenter hämmar överföring av Puumalavirus från person till person; immunsvaret är viktigt för Puumala-virus patogenes och sjukdomens svårighetsgrad.

Att leva på gränsen - en litteraturstudie om kroniskt njursjuka patienters upplevelse av att vara beroende av hemodialys

Bakgrund: Hemodialys är en livslång behandling av njursvikt som är en allvarlig kronisk sjukdom. Hemodialyspatienter har frekventa möten med sjuksköterskor och utvecklar ofta en nära kontakt som kan sträcka sig över flera år. Sjuksköterskan behöver arbeta personcentrerat för att främja patienternas hälsa. Syfte: Syftet med studien var att belysa kroniskt njursjuka patienters upplevelser av att leva med hemodialys.Metod: Litteraturstudie med kvalitativ ansats. Olika sökblock byggdes upp baserade på forskningsfrågan och sökningar utfördes i databaserna CINAHL och PubMed. Efter att ha relevans- och kvalitetsgranskats befanns 12 artiklar vara relevanta för analys. Forsberg och Wengströms förenklade innehållsanalys användes för att analysera materialet.Resultat: Fem teman trädde fram: Tiden då, nu och framåt, Uppfattningen om jaget och kroppen, Relationer, Beroenden och begränsningar samt Förlust, acceptans och tacksamhet.Konklusion: Livet med hemodialys påverkar patienterna både socialt, mentalt och fysiskt. Patienterna upplevde sig begränsade, i beroendeposition och deras sociala relationer förändrades. Sjuksköterskan behöver finnas tillgänglig för patienten för att denne ska uppleva hälsa.Nyckelord: Hemodialys, mellanmänskliga relationer, patient, personcentrerad vård upplevelse. / Background: Hemodialysis is a lifelong treatment of chronic kidney failure - a severe and demanding disease. Patients going through hemodialysis have frequent interaction with nurses and often develop a personal relationship which can continue for several years. The nurse has to apply patient centered care to promote health for the patient.Aim: The aim of this study was to illuminate the lived experience of patients with chronic kidney failure receiving hemodialysis.Method: This study was conducted as a literature review with a qualitative approach. The database searches were conducted in CINAHL and PubMed. 12 articles passed the relevance- and quality review. A content analysis method of five steps was used to analyse the material.Result: Five themes emerged; The past, the now and the future, The perception of self and the body, Relationships, Dependency and limitations and Loss, acceptance and gratitude.Conclusion: Life on hemodialysis affects the patients both socially, mentally and physically. The patients felt restricted, in dependency and that their social relationships were changed. This master thesis The magic and power of red lipsticks represents theoretical-empirical analysis that argues how red lipsticks influence our perception and how we communicate through them with our surrounding. The thesis stems from the basic assumption that red lipstick is truly important and bold communication mean that is able to talk to our surrounding in a completely different ways than other colors or color shades. The aim of the thesis was to find out whether the aforementioned assumption matches the reality. Key questions for this master thesis were as follows: How do women communicate with their surrounding through red color (red lipstick)? What do they expect from these communication means? How do they feel when they communicate through these means (when they wear red lipstick)? How to they perceive reactions for this mean of communication? What are these reactions like (men- and women- wise)? The progress in the field of technology is often described as massive and its development has had a great impact on the social development (Vetenskapsrådet, 2012). As a result of the digital evolution, there has been a change in the way customer and company interact (Normann, 2011). The traditional way for players to interact with each other required an interaction human to human (ibid), a mode of interaction that now can be replaced or supplemented by todays technology and often Internet-based solutions (Salomonson et al., 2013). These technological solutions, the technical interaction, are often referred to as self-service systems. These systems enables the customer to carry out the tasks previously performed by the company (Hilton and Hughes, 2013). A growing number of Swedish companies are implementing these self-service solutions, particularly in matters relating to telephony and mobility, in order to complement or replace existing support (Gustafsson, 2012). Earlier research shows that both companies and customers can benefit from the implementation of self-service systems, however there are other contributions to the field of research that paradoxically advocates that the meeting in person, the human interaction is important for the competitiveness of a company (Normann, 2011). The purpose of this thesis is to create an understanding about how customer value is created in the interaction between customers and companies in both human and technological interaction. As data collection method, this study has applied qualitative interviews in order to enhance understanding of existing phenomena. In order to help analyse the study’s results the thesis theoretical framework consists of theories concerning services, interaction, value creation, service logic and service quality linked to the discipline service management. The results of the study indicate that the creation of customer value in the interaction between customer and company occurs in the same way in human interaction and technological interaction. The factors that create customer value are expressed in different ways by human and technological interaction.

Exploratory Study of Nurse-Patient Encounters in Home Healthcare: A Dissertation

The purpose of this study was to explore nurse-patient encounters from the perspective of the Home Healthcare Registered Nurse. A qualitative descriptive design was used to collect data from a purposive sample of 20 home healthcare registered nurses from Connecticut, Massachusetts, and Rhode Island currently or previously employed as a home healthcare nurse. Four themes and one interconnecting theme emerged from the data: Objective Language; Navigating the Unknown; Mitigating Risk; Looking for Reciprocality in the Encounter; and the interconnecting theme of Acknowledging Not All Nurse-Patient Encounters Go Well. One goal of the study was to propose an empirically informed definition of what constituted a difficult encounter. An important early finding was that the terms difficult patient and difficult encounter were not generally used by study participants. HHC RNs voiced a preference for objective and nonjudgmental language to communicate outcomes of nurse-patient encounters. Three types of HHC RN-patient interactions emerged from the data, with constructive encounters the norm and non-constructive or destructive encounters less frequent. A constructive encounter is when two or more human beings, the nurse on the one side, and the patient, caregiver, or both on the other, interact to achieve a mutually agreed upon outcome. A nonconstructive encounter is when one or more human beings obstruct efforts to achieve at least one positive outcome. A destructive encounter is when one or more human beings direct anger at or physically aggress toward another human being. Strategies to promote reciprocality are routinely employed during HHC RN-patient encounters, but HHC RNs who miss cues that a strategy is ineffective or failed may be at risk in the home. Study data lend support to key concepts, assumptions, and propositions of Travelbee’s (1971) Human-to-Human Relationship Model. Study results provide a foundation for further research to increase the understanding, recognition, and development of empirically derived responses to non-constructive or destructive encounters such that HHC RNs are safe and best able to meet patients’ healthcare needs.
