Rezidivierte und refraktäre Hodentumoren waren bis zu Beginn der 80er Jahre nur selten kurativ behandelbar. Mit Einführung der Hochdosischemotherapie in Verbindung mit autologer Stammzellreinfusion, konnte eine kurative Behandlungsoption auch in dieser prognostisch ungünstigen Situation in der Klinik etabliert werden. Die vorliegende Arbeit beschreibt die Ergebnisse der ersten Phase I/II Studie zur klinischen Etablierung dieses Therapieverfahrens ebenso wie verschiedene nachfolgende Untersuchungen zur Optimierung der Hochdosischemotherapie. Eine "matched-pair" Analyse konnte zumindest im retrospektiven Vergleich, den Nutzen dieses neuen Therapieverfahrens im Vergleich zu einer konventionell-dosierten Behandlung belegen. / Until the beginning of the 1980ies relapsed and refractory germ-cell tumors were rarely cured. With the introduction of high-dose chemotherapy in combination with autologous stem cell reinfusion, a curative treatment option could be established in this prognostically unfavorable situation. The present work describes the results from the initial phase I/II studies that established this new treatment as well as the results of several subsequent trials to optimize this new procedure. Finally, the results of a "matched-pair" analysis is presented that demonstrates the superiority of this new treatment as compared to conventional-dose chemotherapy.

The familial aggregation of Major Depressive Disorder (MDD) has been repeatedly demonstrated. Several studies have investigated associations between various clinical characteristics of MDD in probands and overall rates of MDD in relatives. Few studies, however, have considered the familial aggregation of clinical characteristics of MDD. The aim of the present report is to examine mother–offspring associations of a variety of clinical characteristics of MDD in a general population sample. Data were derived from baseline and 4-year-follow-up data of 933 adolescents and their biological mothers of the Early Developmental Stages of Psychopathology (EDSP) study, a prospective-longitudinal community study. MDD and its characteristics were assessed with the Munich-Composite International Diagnostic Interview. We found that children of mothers who had a lifetime history of severe MDD and high number of symptoms, high impairment and/or melancholia, revealed elevated odds of MDD regarding the same characteristics as their mothers (ORs between 5.2 and 13.9). The observed associations did not differ by the children’s sex. DSM-IV melancholia and severity as well as impairment were found to aggregate within families. This finding can be interpreted as a validation of the DSM-IV MDD severity subtypes as well as of the melancholic specifier. Severe and melancholic MDD reveal a considerable high degree of familiar aggregation making the search for mechanisms involved in the familiar transmission of these forms of MDD particularly promising.

Introduction: This paper does not aim to predict the future of clinical psychology in Germany. The future of psychology depends on the complex interaction between political, sociological, economic and health-care related factors as well as on the scientific progress in the discipline itself and in neighbour disciplines. However, it is fair to say that clinical psychology continues to gain even stronger influences in health care and will face a number of new challenges over the next years of its expansion.
Our paper will present some of these potential fields of development and change based on a brief description of the status quo. The focus of the article will be specific developments in Germany, although there will be an overlap with general tendencies that describe the situation of clinical psychology in the new millenium in general. Furthermore, for research as well as practice, the specific relationship between clinical psychology and psyciatry will be highlighted.

Patients with bulimia nervosa are at high risk for dental erosion. However, not all bulimic patients suffer from erosion, irrespective of the severity of their eating disorder. It is often speculated that differences in the saliva are important, however, little is known about salivary parameters in bulimic patients, particularly directly after vomiting. The aim of the clinical trial was to compare different salivary parameters of subjects suffering from bulimia with those of healthy controls. Twenty-eight subjects participated (14 patients with bulimia nervosa, 7 of them with erosion; 14 matched healthy controls). Resting and stimulated saliva of all participants was analysed as well as saliva collected from bulimic patients directly and 30 min after vomiting. Parameters under investigation were flow rate, pH, buffering capacity and the enzyme activities of proteases in general, collagenase, pepsin, trypsin, amylase, peroxidase, and lysozyme. Regarding flow rate, pH and buffering capacity only small differences were found between groups; buffering capacity directly after vomiting was significantly lower in bulimic subjects with erosion than in subjects without erosion. Differences in enzymatic activities were more pronounced. Activities of proteases, collagenase and pepsin in resting and proteases in stimulated saliva were significantly higher in bulimic participants with erosion than in controls. Peroxidase activity was significantly decreased by regular vomiting. Malignant brain tumors, especially malignant gliomas, have a poor prognosis, a fact which has remained unchanged over the last decades despite the employment of multimodal therapeutic approaches. Malignant gliomas are among the most vascularized tumors known and the amount of vascularization has been correlated to their prognosis. Since tumor growth is dependent on concomitant vascularization, recent experimental studies have focused on the use of anti-angiogenic molecules as a novel strategy in brain tumor therapy. Angiogenesis inhibitors target at proliferating endothelial cells and suppress the formation of a sufficient vascular bed. Inhibitors such as TNP-470, suramin and angiostatin have shown their therapeutic potential in experimental studies. In a clinical setting, they could be applied for the treatment of multiple tumors or postsurgically as an adjuvant therapy to prevent recurrence. This article discusses presently available anti-angiogenic agents, emphasizing on substances already in clinical trials. / Maligne Hirntumoren, insbesondere die malignen Gliome, haben trotz multimodaler Therapieansätze eine unverändert schlechte Prognose. Diese Aggressivität korreliert mit der Tatsache, daß maligne Gliome zu den gefäßreichsten Tumoren zählen, die wir kennen. Die Quantifizierung der Gefäßdichte in diesen Tumoren erlaubte die Korrelation zur Überlebenszeit der Patienten. Da das Tumorwachstum von einer begleitenden Neovaskularisierung abhängt, wurden erste experimentelle Therapieansätze durchgeführt, um das Tumorwachstum durch Inhibierung der Neovaskularisierung zu verhindern. Inhibitoren der Angiogenese, z.B. TNP-470, Suramin und Angiostatin hemmen die Proliferation von Endothelzellen und die Ausbildung eines funktionsfähigen Gefäßbettes. Erste experimentelle Ansätze haben ihre tumorstatische Wirksamkeit in vivo bewiesen. Zur klinischen Behandlung wären diese Substanzen in Verbindung mit bestehenden Therapien einsetzbar, insbesondere für die Behandlung multipler Tumoren und zur postoperativen Therapie.
