This thesis is focused on the analysis of experimental ECG records drawn up in isolated rabbit hearts and aims to describe changes in EKG caused by ischemia and left ventricular hypertrophy. It consists of a theoretical analysis of the problems in the evaluation of ECG during ischemia and hypertrophy, and describes an experimental ECG recording. Theoretical part is followed by a practical section which describes the method for calculating morphological parameters, followed by ROC analysis to evaluate their suitability for the classification of hypertrophy and at the end is focused on classification.

Echocardiography examination is an important and familiar method for heart`s examination. Echocardiography is used to assess the function of the heart during to check the heart disease. In an echocardiography examination, the heart´s flow rates, contractility (pumping capacity), wall thickness, and inner diameter can be examined. All these examinations are done with the help of evolution of the ultrasonic waves that the ultrasonic transducer sends out and receives. The transducer consists of piezoelectric crystals that can both transmit and receive ultrasonic waves with frequencies exceeding 20 kHz. The purpose of the study is to compare the measurement of the left ventricular ejection fraction (LVEF) between an experienced biomedical scientist (BMA) and a student. In addition to see how the image quality affects the result. The measurement was performed by using the Simpson method. The result was analyzed by using with a static method. The results were analyzed by using a paired t-test to see if there is any significant difference between the performance of a BMA and a student. The measurement was performed on apical 4-chamber and apical 2-chamber image. The study included 30 patients, both heart -healthy and cardiac patients of the genders. The result showed that there is a significant difference in the performance of LVEF- measurements between BMA and student, with lower values measured by the student. / Ekokardiografiundersökning är en viktig och vanlig metod vid undersökning av hjärtat. Ekokardiografi används för att bedöma hjärtats funktion vid utredning av hjärtsjukdomar. Vid en ekokardiografiundersökning kan hjärtats flödeshastigheter, kontraktilitet (pumpförmåga), väggtjocklek, och innerdiameter undersökas. Alla dessa undersökningar görs med hjälp av tolkning av ultraljudsvågorna som ultraljudsgivaren skickar ut och tar emot. Givaren består av piezoelektriska kristaller som kan både sända och tar emot ultraljudsvågor med frekvens på över 20 kHz. Syftet med denna studie är att jämföra mätningen av den vänstra ventrikulära ejektionsfraktion (LVEF) mellan en erfaren biomedicinsk analytiker (BMA) och en student samt att se hur bildkvalitén påverkar resultatet. Mätningen utfördes med Simpsons- metoden. Resultatet analyserades med hjälp av en statistisk metod. Resultatet analyserades med hjälp av parat t-test för att se om det finns någon signifikant skillnad mellan utförandet av en BMA och en student. Mätningen utfördes på apikala 4-kammarbilder och apikala 2-kammarblider. Studien inkluderade 30 patienter, både hjärtfriska och hjärtsjuka patienter av både könen. Resultatet visade att det finns en signifikant skillnad i utförande av LVEF- mätningar mellan BMA och student, med lägre uppmätta värden av studenten.

Simpsons biplan metod jämfört med Philips Heart Model vid bestämning av vänsterkammares ejektionsfraktion / Simpson’s biplane method compared to the Philips Heart Model when determining the left ventricular ejection fraction

Introduktion: Vänsterkammarens ejektionsfraktion (VKEF) är ett central mått på systolisk funktion i vänster kammare och är en av de mest betydelsefulla parametrar vid ekokardiografiska undersökningar. Idag är Simpson biplan metoden den mest använda metoden för bestämning av ejektionsfraktionen. Vid ekokardiografiska undersökningar sänder givaren med piezoelektriska kristaller ut ultraljudsvågor med en frekvens över 20 000 Hz. Ljudvågorna som skickas ut i kroppen reflekteras och sedan återvänder de till givaren för att skapa en bild. Denna studie jämför den tvådimensionella (2D) ultraljudsmetoden Simpsons biplan med Philips Heart Model som är en automatiserad tredimensionella (3D) funktion för bedömning av VKEF.  Material och metod: I studien inkluderades 31 hjärtfriska försökspersoner mellan åldrarna 21-64. Det samlades in bilder på apikala 4- och 2 kammarbilder från alla försökspersoner där Simpsons biplan metoden användes för att beräkna ejektionsfraktion. Apikala 4-kammarbilder samlades in för att beräkna ejektionsfraktionen med Philips Heart Model 3D funktion.  Resultat: Resultatet från denna studie visade att det inte föreligger någon signifikant skillnad mellan Simpsons biplan metoden och Philips Heart Model metoden för bestämning av ejektionsfraktion. Båda metoderna visade likvärdiga mätresultat.  Diskussion: Philips Heart Model metoden är en relativ ny funktion som använder sig av artificiell intelligens för att analysera 3D bilder. Philips Heart Model metoden är en säker funktion att använda då de flesta studier bevisar likvärdiga och säkra mätresultat i jämförelse med andra metoder.  Konklusion: Enligt denna studie ger Philips Heart Model funktionen likvärdiga mätresultat av vänsterkammarens ejektionsfraktion i jämförelse med Simpsons biplan. / Introduction: Simpson’s biplane method is the most used method for determining the left ventricular ejection fraction (LVEF) in echocardiographic examinations. Ejection fraction is a central measurement of the heart's global systolic function. The probe with piezoelectric crystals emits ultrasound waves with a frequency above 20,000 Hz. The sound waves that are sent out into the body are reflected and then return to the probe to create an image. This study compares the two-dimensional (2D) ultrasound Simpson's biplane method with the Philips Heart Model method, which is an automated three-dimensional (3D) function for assessment of LVEF.                                                                                                                          Material and method: 31 subjects with no recorded heart pathologies between the ages of 21-64 were included in the study. Apical 4- and 2-chamber images were collected from the test subjects, where the Simpson's biplane method was applied to calculate the ejection fraction. 2D apical 4-chamber images were collected to convert to 3D and used to calculate the ejection fraction with the Philips Heart Model.    Results: The results of this study showed that there is no significant difference between the Simpson’s biplane method and the Philips Heart Model method for determining ejection fraction.    Discussion: The Philips Heart Model method is a relatively new feature that uses artificial intelligence to analyze 3D images. The Philips Heart Model method is a reliable feature to use as most studies have proven similar and reliable measurements when comparing it with other methods for determining LVEF.    Conclusion: According to this study, the Philips Heart Model feature provides equivalent measurements in comparison with the manual method Simpson's biplane.

Effet du resvératrol sur la dysfonction endothéliale des artères coronaires épicardiques associée à l’hypertrophie ventriculaire gauche dans un modèle porcin

Les propriétés antioxydantes du resvératrol sont d’un grand intérêt pour contrer la dysfonction endothéliale où la contribution du stress oxydant est majeure. Cette dysfonction endothéliale est d’ailleurs bien caractérisée en hypertrophie ventriculaire gauche (HVG). Cette étude vise à explorer les effets thérapeutiques du resvératrol sur la dysfonction endothéliale des artères coronaires épicardiques associée à l’HVG. L’HVG est induite suite à un cerclage aortique (CA) sur des porcelets de 2 mois. Le resvératrol est ensuite administré à 20 mg/kg/jour per os aux animaux répertoriés dans différents groupes. Le groupe 1 comprend les animaux contrôles sans CA alors que le groupe 2 représente les porcelets ayant subit le CA sans traitement. Le groupe 3 comprend les animaux traités pendant les 60 jours après la chirurgie alors que le groupe 4 est traité seulement à partir du 30ème jour jusqu’au jour 60. Des analyses échocardiographiques et histologiques ont été effectuées afin de déterminer le degré d’hypertrophie et l’impact du resvératrol sur la progression de l’HVG alors que la réactivité vasculaire a été évaluée par des expériences de chambres d’organes. La dysfonction endothéliale a été étudiée in vitro par quantification des métabolites nitriques et des niveaux de GMPc tandis que le stress oxydant est décrit par les niveaux d’angiotensine II (Ang II) et de protéines carbonylées. Les courbes dose-réponse à la sérotonine du groupe 3 traité pendant les 60 jours au resvératrol ont démontré une relaxation vasculaire significativement améliorée comparé au groupe 2 non-traité (p<0,05). Le pourcentage de changement du rapport de la masse du ventricule gauche sur la masse corporelle (LVmass/BW) a démontré une inhibition du développement de l’HVG dans le groupe 3 alors que le groupe 4 n’a pas eu d’effets bénéfiques des 30 jours de traitement au resvératrol. La biodisponibilité du NO représentée par la mesure de ses métabolites en circulation (398,71±21,74 ; 151,04±14,95 ; 241,86±15,53 ; 208,17±26,52 uM pour les groupes 1, 2, 3 et 4 respectivement, p<0,001 pour groupe 1 et 3 vs groupe 2) et par le niveau de GMPc a été augmentée avec l’administration du polyphénol (2,54±0,63 ; 0,67±0,45 ; 1,71±0,25 ; 1,98±0,29 pmol/ml pour les groupes 1, 2, 3 et 4 respectivement, p<0,05 pour groupe 1, 3 et 4 vs groupe 2). Le rôle antioxydant du resvératrol a été confirmé avec une réduction des niveaux de protéines carbonylées chez le groupe 3 comparé aux valeurs du groupe 2 (0,14±0,05 vs 0,33±0,03 nmol/mg respectivement) sans diminution des niveaux d’Ang II. Le resvératrol a aussi réduit l’hypertrophie des cardiomyocytes et la fibrose interstitielle. Ainsi, le resvératrol peut effectivement réduire la dysfonction endothéliale des artères coronaires épicardiques et limiter l’occurrence de remodelage myocardique associé à l’HVG, principalement à travers l’activation de la signalisation dépendante du NO. / Antioxidant properties of resveratrol are of great interest regarding endothelial dysfunction in which oxidative stress has a major causative role. This dysfunction is well characterized in left ventricular hypertrophy (LVH). The current study was designed to explore the effects of resveratrol as a treatment for the coronary endothelial dysfunction associated with LVH. An aortic banding (AB) was performed on swine to induce LVH on two-month-old swine for 60 days. Resveratrol was administered at 20 mg/kg/d per os to different groups. Group 1 represented healthy controls while group 2 was not treated. After AB, group 3 received resveratrol from day 1 to 60 post-surgery whereas group 4 was treated from day 30 to 60 only. Echocardiographic and histological studies were performed to evaluate the degree of LVH. Vascular reactivity of epicardial coronary arteries was assessed in organ chamber experiments. Endothelial dysfunction was evaluated in vitro by nitric oxide (NO) metabolites and cyclic guanosine-3,5-monophosphate (cGMP) while angiotensin II (Ang II) levels and protein carbonylation evaluated Oxidative stress. After treatment, the LVmass/body weight ratio was improved in group 3 associated with reduced cardiomyocyte hypertrophy and interstitial fibrosis (p<0.05). Dose-response curves to serotonin for the resveratrol treated group 3 demonstrated a greater vascular relaxation compared to LVH untreated group 2 (p<0.05). Bioavailability of NO measured by its circulating metabolites (398,71±21,74; 151,04±14,95 ; 241,86±15,53 ; 208,17±26,52 uM for groups 1, 2, 3 and 4 respectively, p<0,001 for groups 1 and 3 vs group 2) and by vascular cGMP (2,54±0,63 ; 0,67±0,45 ; 1,71±0,25 ; 1,98±0,29 pmol/ml for groups 1, 2, 3 and 4 respectively, p<0,05 for group 1,3 and 4 vs group 2) was increased. The antioxidant effect of the polyphenol was confirmed by decreased protein carbonylation with the 60 days treatment (0,14±0,05 vs 0,33±0,03 nmol/mg for group 3 vs 2) associated to unchanged Ang II levels. Therefore, resveratrol effectively reduces epicardial coronary endothelial dysfunction and limits the occurrence of cardiac remodelling associated with LVH, mainly through an NO-dependent pathway.

Leaflet Material Selection for Aortic Valve Repair

Leaflet replacement in aortic valve repair (AVr) is associated with increased long-term repair failure. Hemodynamic performance and mechanical stress levels were investigated after porcine AVr with 5 types of clinically relevant replacement materials to ascertain which material(s) would be best suited for repair. Porcine aortic roots with intact aortic valves were placed in a left-heart simulator mounted with a high-speed camera for baseline valve assessment. Then, the non-coronary leaflet was excised and replaced with autologous porcine pericardium (APP), glutaraldehyde-fixed bovine pericardial patch (BPP; Synovis™), extracellular matrix scaffold (CorMatrix™), or collagen-impregnated Dacron (HEMASHIELD™). Hemodynamic parameters were measured over a range of cardiac outputs (2.5–6.5L/min) post-repair. Material properties of the above materials along with St. Jude Medical™ Pericardial Patch with EnCapTM Technology (SJM) were determined using pressurization experiments. Finite element models of the aortic valve and root complex were then constructed to verify the hemodynamic characteristics and determine leaflet stress levels. This study demonstrates that APP and SJM have the closest profiles to normal aortic valves; therefore, use of either replacement material may be best suited. Increased stresses found in BPP, HEMASHIELD™, and CorMatrix™ groups may be associated with late repair failure.

Études de réseaux d’expression génique : utilité pour l’élucidation des déterminants génétiques des traits complexes

Les traits quantitatifs complexes sont des caractéristiques mesurables d’organismes vivants qui résultent de l’interaction entre plusieurs gènes et facteurs environnementaux. Les locus génétiques liés à un caractère complexe sont appelés «locus de traits quantitatifs » (QTL). Récemment, en considérant les niveaux d’expression tissulaire de milliers de gènes comme des traits quantitatifs, il est devenu possible de détecter des «QTLs d’expression» (eQTL). Alors que ces derniers ont été considérés comme des phénotypes intermédiaires permettant de mieux comprendre l’architecture biologique des traits complexes, la majorité des études visent encore à identifier une mutation causale dans un seul gène. Cette approche ne peut remporter du succès que dans les situations où le gène incriminé a un effet majeur sur le trait complexe, et ne permet donc pas d’élucider les situations où les traits complexes résultent d’interactions entre divers gènes. Cette thèse propose une approche plus globale pour : 1) tenir compte des multiples interactions possibles entre gènes pour la détection de eQTLs et 2) considérer comment des polymorphismes affectant l’expression de plusieurs gènes au sein de groupes de co-expression pourraient contribuer à des caractères quantitatifs complexes. Nos contributions sont les suivantes : Nous avons développé un outil informatique utilisant des méthodes d’analyse multivariées pour détecter des eQTLs et avons montré que cet outil augmente la sensibilité de détection d’une classe particulière de eQTLs. Sur la base d’analyses de données d’expression de gènes dans des tissus de souris recombinantes consanguines, nous avons montré que certains polymorphismes peuvent affecter l’expression de plusieurs gènes au sein de domaines géniques de co-expression. En combinant des études de détection de eQTLs avec des techniques d’analyse de réseaux de co-expression de gènes dans des souches de souris recombinantes consanguines, nous avons montré qu’un locus génétique pouvait être lié à la fois à l’expression de plusieurs gènes au niveau d’un domaine génique de co-expression et à un trait complexe particulier (c.-à-d. la masse du ventricule cardiaque gauche). Au total, nos études nous ont permis de détecter plusieurs mécanismes par lesquels des polymorphismes génétiques peuvent être liés à l’expression de plusieurs gènes, ces derniers pouvant eux-mêmes être liés à des traits quantitatifs complexes. / Complex quantitative traits are measurable characteristics of living organisms resulting from the interaction between multiple genes and environmental factors. Genetic loci associated with complex trait are called "quantitative trait loci" (QTL). Recently, considering the expression levels of thousands of genes as quantitative traits, it has become possible to detect "expression QTLs " (eQTL). These eQTL are considered intermediate phenotypes and are used to better understand the biological architecture of complex traits. However the majority of studies still try to identify a causal mutation in a single gene. This approach can only meet success in situations where the gene incriminate as a major effect on the complex trait, and therefore can not elucidate the situations where complex traits result from interactions between various genes. This thesis proposes a more comprehensive approach to: 1) take into account the possible interactions between multiple genes for the detection of eQTLs and 2) consider how polymorphisms affecting the expression of several genes in a module of co-expression may contribute to quantitative complex traits. Our contributions are as follows: We have developed a tool using multivariate analysis techniques to detect eQTLs, and have shown that this tool increases the sensitivity of detection of a particular class of eQTLs. Based on the data analysis of gene expression in recombinant inbred strains mice tissues, we have shown that some polymorphisms may affect the expression of several genes in domain of co-expression. Combining eQTLs detection studies with network of co-expression genes analysis in recombinant inbred strains mice, we showed that a genetic locus could be linked to both the expression of multiple genes at a domain of gene co-expression and a specific complex trait (i.e. left ventricular mass). Our studies have detected several mechanisms by which genetic polymorphisms may be associated with the expression of several genes, and may themselves be linked to quantitative complex traits.

Performance diagnóstica do PET-CT com rubídio-82 na avaliação da perfusão e da função ventricular esquerda em pacientes submetidos à cinecoronarioangiografia e SPECT com 99mTc-SESTAMIBI / Diagnostic performance of rubidium-82 PET-CT in evaluation of perfusion and left ventricular function in patients submitted to coronary angiography and 99mTc-SESTAMIBI SPECT

Introdução: A avaliação e detecção precoce dos defeitos de perfusão miocárdica permite estratificar o risco cardiovascular e auxiliar a tomada de decisão terapêutica nos pacientes com DAC estabelecida ou suspeita. O PET/CT com Rb-82 na avaliação da perfusão e função miocárdica não invasiva é uma ferramenta de elevada acurácia diagnóstica. O presente trabalho procura estabelecer uma avaliação de performance de diagnóstica, entre dois métodos na medicina nuclear - SPECT com 99mTc-Sestamibi e PET/CT com Rb-82. Métodos: Avaliação da perfusão e da função do ventrículo esquerdo em 42 pacientes submetidos ao PET-CT com Rb-82 e SPECT com MIBI com mesmo estresse farmacológico (Dipiridamol), utilizando-se das variáveis de perfusão (SSS, SDS, SRS, fluxo sanguíneo e reserva coronariana) e de função (FEVE, DeltaFEVE, VDF, VSF - no repouso e no estresse), considerando o CATE como o método de referência. Resultados: No escore SDS, o Rb-82 apresentou valores médios próximos ao dobro do MIBI no grupo dos alterados (7,6 vs 3,9). Obteve-se um valor de sensibilidade do Rb-82 de 90,9%, com VPP de 88,2% (p=0,012) para o SSS e de 87,8%, com VPP de 87,8% (p= 0,012) para o SDS. As áreas sobre curva demonstraram superioridade do Rb-82 com maior exatidão e acurácia do método em relação ao MIBI na amostra avaliada. No escore de reversibilidade (SDS), houve maior sensibilidade em cut-off de menor valor e maior especificidade em cut-off de maior valor. FEVE em repouso e estresse, assim como, VDF e VSF em repouso e estresse apresentaram valores de concordância altos. Os valores de SSS, SRS e SDS apresentaram concordâncias moderadas. Houve aparente queda do DeltaFEVE no Rb-82, contudo similar tendência não foi observada no MIBI, considerando que as diferenças não foram significativas com relação ao número de vasos com estenose em ambos os radioisótopos (p > 0,05). Em outra análise, entre uma variável de função ventricular e a CFR global foi evidenciada diferença significativa (p=0,024) entre os grupos normal e alterado. No grupo alterado com CFR global < 2,0 houve redução do DeltaFEVE em 2,7%.Conclusão: O PET-CT com 82Rb apresenta maior exatidão e acurácia diagnóstica na avaliação perfusional miocárdica em relação ao SPECT com MIBI, inclusive com reclassificação dos grupos avaliados que passaram de defeito perfusional de pequena para moderada extensão. A FEVE estresse na população geral sugere valor mais alto no 82Rb (aquisição do exame no pico do estresse).Haveria uma sugestão de que no grupo com CATE alterado, a CFR global seria reduzida; que em maiores graus de estenose coronariana ao CATE, a CFR global decresceria; que existiria queda da DeltaFEVE no 82Rb quanto maior fosse o número de vasos obstruídos ao CATE. A relação entre a FEVE reserva e a CFR global no 82Rb foi significativa com queda da DeltaFEVE no grupo com CFR global alterada / Background: The evaluation and early detection of myocardial perfusion defects allows to stratify cardiovascular risk and it helps therapeutic decision-making in patients with established or suspected CAD. Rb-82 PET/CT in perfusion evaluation and noninvasive myocardial function is a tool of high diagnostic accuracy. The present study seeks to establish a diagnostic performance evaluation between two methods in nuclear medicine - 99mTc-Sestamibi SPECT and Rb-82 PET/CT. Methods: Evaluation of perfusion and left ventricular function in 42 patients submitted to Rb-82 PET-CT and MIBI SPECT with the same pharmacological stress (Dipyridamole), using perfusion variables (SSS, SDS, SRS, blood flow and coronary reserve) and function (LVEF, DeltaLVEF, EDV, ESV - in rest and stress), considering CAG as the reference method. Results: In the SDS score, Rb-82 presented mean values close to double the MIBI in the altered group (7.6 vs 3.9). A Rb-82 sensitivity of 90.9% was obtained, with PPV of 88.2% (p = 0.012) for SSS and 87.8% for PPV of 87.8% (p = 0.012) for or SDS. The areas on the curve indicated superiority of Rb-82 with greater accuracy and accuracy of the method in relation to the MIBI in the sample evaluated. In the reversibility score (SDS), there was greater sensitivity in the lower cut-off value and greater specificity in the higher cut-off value. LVEF at rest and stress, as well as VDF and VSF at rest and stress had high agreement values. The values of SSS, SRS and SDS presented moderate agreement. There was an apparent decrease of DeltaLVEF in Rb-82, however a similar trend was not observed in the MIBI, considering that the differences were not significant in relation to the number of vessels with stenosis in both radioisotopes (p > 0.05). In another analysis, a significant difference (p = 0.024) between the normal and altered groups was found of ventricular function variable and global CFR. In the altered group with global CFR < 2.0 there was a reduction of DeltaLVEF in 2.7%.Conclusion: 82Rb PET-CT has a greater accuracy and diagnostic accuracy of myocardial perfusion assessment in relation to MIBI SPECT, including reclassification of the evaluated groups that went from small to moderate perfusion defect extension. The LVEF stress in the general population suggests a higher value in 82Rb (acquisition of the exam at peak stress). There would be a suggestion that if the CAG group changed, the overall CFR would be reduced; in which greater degrees of coronary stenosis to CAG, the global CFR would decrease; and there would be a decrease of DeltaLVEF in 82Rb, the greater the number of vessels obstructed to CAG. The relation to reserve LVEF and overall CFR in 82Rb was significant with decrease in DeltaLVEF of the group with altered global CFR

Associação dos achados morfofuncionais cardíacos, renais e vasculares com as alterações do índice tornozelo-braço em pacientes hipertensos diabéticos / Association of cardiac, renal and vascular morphological and functional findings with changes in ankle brachial index in diabetic hypertensive patients

Introdução: Inúmeros estudos estabeleceram correlações entre o índice tornozelo-braço (ITB), um marcador de aterosclerose subclínica, e o prognóstico cardiovascular em diferentes populações. No entanto, poucos estudos avaliaram a correlação entre os valores do ITB e lesões cardiovasculares e renais, exclusivamente, em pacientes com hipertensão arterial e diabetes. Objetivo: Estudar a prevalência de alterações morfofuncionais cardíacas, carotídeas, retinianas e renais de acordo com a presença ou não de valores de ITB alterados (ITB <= 0,9 ou ITB > 1,4) em pacientes hipertensos com diabetes tipo 2. Métodos: Foram incluídos no estudo 99 pacientes hipertensos diabéticos com idade entre 50 e 80 anos. A aferição do ITB foi realizada em todos os pacientes por método validado e estes foram classificados em Grupo 1 (ITB normal, n = 49) ou Grupo 2 (ITB alterado, n =50). Todos os pacientes foram submetidos, em até 06 meses, à realização de ecodopplercardiograma, ultrassonografia de carótidas, retinografia colorida, aferição da taxa de filtração glomerular (TFG) e da albuminúria de 24h. Os pacientes foram analisados para a ocorrência ou não de um desfecho-composto ecocardiográfico que incluiu alterações morfológicas e funcionais cardíacas relevantes para a prática clínica. Os pacientes dos grupos 1 e 2 foram também comparados quanto à prevalência de placas carotídeas com ou sem repercussão hemodinâmica, TFG < 60 ml/mim/m2, albuminúria de 24h > 30mg e presença ou não de retinopatia. Por fim, foram comparadas as frequências médias das seguintes lesões de órgãos-alvo de ambos os grupos, considerando-se valor unitário para a presença de cada uma delas: hipertrofia do ventrículo esquerdo, retinopatia hipertensiva, TFG < 60 ml/min/m2 e estenose da artéria carótida interna > 50% do seu diâmetro. Resultados: A média de idade dos pacientes foi 65,4 ± 7 anos, sendo 61,6% deles do sexo feminino. A presença de níveis elevados de pressão arterial sistólica (153,4 ± 18 versus 170 ± 26 mmHg), de albuminúria de 24h > 30mg (55,3% versus 82,6%) e de TFG < 60 ml/min/m2 (12,8% versus 33,3%) foi significativamente maior (p < 0.05) entre os pacientes do Grupo 2. O desfecho-composto ecocardiográfico foi mais prevalente no grupo 2 (84,0% versus 59,2%; p = 0,006) e a frequência média de lesões de órgãos-alvo também foi maior nos pacientes do grupo 2 (0,36 ± 0,31 versus 0,19 ± 0,19; p = 0,001). Análise por regressão logística binária revelou que o ITB foi uma das variáveis preditoras independentes para o desfecho-composto ecocardiográfico (OR = 3,43; IC 95% = 1,07 - 11,0; p = 0,04). A partir da análise por regressão linear obteve-se um modelo final no qual o ITB foi uma das três variáveis preditoras independentes para a estimativa da frequência média de lesões de órgãos-alvo com coeficiente beta = 13,22 (1,81 - 24,63), ao lado da idade e do infarto prévio. Conclusão: Nossos dados mostram que valores de ITB alterados estão associados à maior prevalência de lesões em órgãos-alvo, principalmente alterações ecocardiográficas, em pacientes com hipertensão arterial e diabetes / Introduction: A lot of studies have established strong correlations between the ankle-brachial index (ABI), a marker of subclinical atherosclerosis and cardiovascular prognosis in different populations. However, few studies have assessed the correlation between the values of the ABI and cardiovascular and renal lesions in patients with hypertension and diabetes. Objective: To study the prevalence of cardiac, carotid, renal and retinal morphological and functional changes according to the presence or not of altered ABI values (ABI <= 0.9 or ABI > 1.4) in hypertensive patients with type 2 diabetes. Methods: It was included 99 diabetic hypertensive patients aged between 50 and 80 years. The measurement of the ABI was performed in all patients by validated method and they were classified in Group 1 (normal ABI, n = 49) or group 2 (altered ABI, n = 50). All patients were submitted, up to 6 months, to Doppler echocardiography, carotid ultrasound, color retinography, assessment of glomerular filtration rate (GFR) and 24h albuminuria. Patients were analyzed for the occurrence or not of a composite echocardiographic outcome which included morphological and functional cardiac alterations relevant to clinical practice. Patients in groups 1 and 2 were compared regarding the prevalence of carotid plaques with or without hemodynamic repercussion, TFG < 60 ml/min/m2, 24h albuminuria > 30 mg and the presence or not of retinopathy. Finally, we compared the prevalence of mean frequency of the following end-organ lesions of both groups, considering unit value for each one: left ventricular hypertrophy, hypertensive retinopathy, TFG < 60 ml/min/m2 and internal carotid artery stenosis > 50%. Results: The mean age of the patients was 65.4 ± 7 years, with 61.6% of them female. The presences of elevated levels of systolic blood pressure (153.4 ± 18 versus 170.0 ± 26 mmHg), of 24h albuminuria > 30 mg (55.3% versus 82.6%) and TFG < 60 ml/min/m2 (12.8% vs. 33.3%) were significantly greater (p < 0.05) among the patients of Group 2. The composite echocardiographic outcome was more prevalent in Group 2 (84.0% versus 59.2%, p = 0.006) and the average frequency of subclinical injury of target organs was also greater in patients of Group 2 (0.36 ± 0.31 versus 0.19 ± 0.19; p = 0.001). Binary logistic regression analysis revealed that the ABI was one of the independent predictors of composite echocardiographic outcome (OR = 3.43; IC 95% = 1.07 - 11.0; p = 0.04). From the linear regression analysis it was obtained a final model in which the ABI was one of three independent predictors for the estimation of the average frequency of end-organ damage with ? coefficient = 13.22 (1.81-24.63), besides age and previous myocardial infarction. Conclusion: Our data demonstrates that changed ABI values are associated with higher prevalence of subclinical end-organ lesions, principally changes in echocardiographic parameters, in patients with hypertension and diabetes

A hipertensão arterial (HA) é uma doença multifatorial na qual há a interação de vários mecanismos, e está relacionada com alterações funcionais e/ou estruturais dos órgãos-alvo. Alterações funcionais dos mecanismos regulatórios da pressão arterial (PA) a curto prazo, como os barorreceptores e os quimiorreceptores, vem sendo bastante exploradas com o objetivo de entender os possíveis mecanismos que podem estar relacionadas à gênese da HA. Diante disso, utilizamos o modelo experimental de desnervação sinoaórtica (DSA) e desnervação seletiva desses aferentes (aórtica DA) e/ou carotídea (DC) e a ligadura da artéria do corpúsculo carotídeo (LA) para avaliarmos a importância relativa dos barorreceptores e quimioreceptores no controle neurogênico da circulação mediando respostas cardíacas e músculo-esqueléticas na HA. Para tanto, utilizamos ratos Wistar (CTR) e espontaneamente hipertensos (SHR) submetidos às diferentes desnervações (SHRDSA/ SHRDA / SHRDC), bem como, a ligadura da artéria do corpúsculo carotídeo (SHRLA). Os animais foram acompanhados durante 10 semanas após as desnervações seletivas, e em seguida foram realizadas as avaliações ecocardiográficas, gasometria arterial, hemodinâmicas, autonômicas e de fluxo sanguíneo regional. Posteriormente, os animais foram eutanasiados para a coleta dos tecidos para as avaliações gênicas e histológicas. Resultados: Os animais SHR apresentaram disfunções hemodinâmicas, autonômicas e gasométricas (alcalose respiratória) quando comparado ao grupo CTR, assim como nas análises de hipertrofia, fluxo e histologia do músculo esquelético, como transição no fenótipo para um perfil mais glicolítico no sóleo e aumento da área de secção transversa das fibras do tipo I e redução das fibras do tipo IIB no músculo diafragma. Nos grupos experimentais hipertensos, os animais com prejuízo do quimiorreflexo (SHRDC, SHRLA e SHRDSA), apresentaram valores maiores de pCO2 em relação ao grupo SHR e SHRDA. Todos os grupos com as diferentes desnervações apresentaram alterações autonômicas, de fluxo sanguíneo e de capilarização. Entretanto, nossos maiores achados foram em relação ao grupo SHRDA, que apresentou valores significativamente maiores que o grupo SHR nos parâmetros PAS (212±2 vs 200±3), PAD (156±4 vs 144±3) PAM (185±9 vs 172±3) e FC (377±4 vs 350±7). Além disso, apresentou aumento da variabilidade da PA (VPAS), bem como o simpático periférico (BFPAS), contrariamente ao observado nos grupos SHRDC e SHRLA em relação ao grupo SHR. No controle autonômico da FC, o grupo SHRDA apresentou menor efeito parassimpático e maior efeito simpático em relação ao grupo SHR. Já os grupos SHRDC e SHRLA apresentaram um menor efeito parassimpático, sem alterações no efeito simpático, embora a resistência vascular periférica estivesse aumentada no grupo SHRLA. As adaptações morfofuncionais cardíacas (ecocardiografia e marcadores de hipertrofia cardíaca) foram mais evidentes no grupo que apresentava disfunção total de ambos receptores (SHRDSA). Conclusão: A ausência do controle reflexo exercido predominantemente pelos barorreceptores arteriais demonstrou uma maior influência do componente aórtico do que o carotídeo sobre a PA. Em adição, a função sistólica parece maior nos grupos SHRDA e SHRDSA, sugerindo que a desnervação aórtica esteja associada com ativação simpática. Não se observou alteração cardíaca na desnervação carotídea. Em relação ao músculo esquelético, todas as desnervações mostraram aumento de capilarização, enquanto somente o grupo SHRDSA mostrou redução de fibras intermediárias. O aumento de capilarização pode estar associado com a liberação do simpático periférico nas desnervações que incluem a retirada dos barorreceptores aórticos, levando ao aumento de VPA e à ausência dos quimiorreceptores nos grupos com desaferentação dos receptores carotídeos / Arterial hypertension (AH) is a multifactorial disease on which there is the interaction of several mechanisms , therefore is related to functional and / or structural changes of the target organ . Functional changes of the regulatory mechanisms of blood pressure (BP) in the short term , as pressoreceptor and chemoreceptors, has been extensively explored in order to understand the possible mechanisms that may be related to the genesis of hypertension. Thus, we used the experimental model of sinoaortic denervation (DSA) and selective denervation of those afferent aortic (DA) and / or carotid (DC) and the ligature of the carotid body artery (LA) to evaluate the relative importance of baroreceptors and chemoreceptors on control of neurogenic circulation mediating cardiac and musculoskeletal responses in HA. There by, we used Wistar rats (CTR) and spontaneously hypertensive rats (SHR) subjected to different denervation (SHRDSA / SHRDA / SHRDC) and the ligature of the carotid body artery (SHRLA). The animals were followed for 10 weeks after the selective denervation it was performed echocardiographic evaluations, blood gas, hemodynamic, autonomic and regional blood flow. Subsequently, the animals were euthanized for tissue collection for genetic and histological evaluations. Results: SHR animals showed hemodynamic, autonomic dysfunction and gas exchange (respiratory alkalosis) compared to CTR group as well as the analysis of hypertrophy, flow and histology of skeletal muscle, such as the transition to a more glycolytic phenotype profile in soleus and increased cross-sectional area of type I fibers and reduction of type IIB fibers in the diaphragm . In hypertensive experimental groups, animals with prejudice chemoreflex (SHRDC, SHRLA and SHRDSA) , showed higher pCO2 compared to SHR and SHRDA group. All groups with different denervation showed autonomic changes in blood flow and capillarization. However, our major findings were compared to SHRDA group, which was significantly higher than the SHR in SBP (212 ± 2 vs 200 ± 3), DBP (156 ± 4 vs 144 ± 3), MAP (185 ± 9 vs 172 ± 3) and HR (377 ± 4 vs 350 ± 7) parameters. Furthermore, we showed an increase in BP variability (BPV) and in the peripheral sympathetic (BFPAS), otherwise showed in the groups SHRDC and SHRLA compared to SHR . Autonomic control of HR, the SHRDA group showed lower sympathetic and higher parasympathetic effect effect in relation to SHR. Already SHRDC and SHRLA groups had a lower parasympathetic effect without changes in sympathetic, although peripheral vascular resistance was increased in SHRLA group. The cardiac morphofunctional adaptations (echocardiography and cardiac hypertrophy markers) were more evident in the group that had total dysfunction of both receptors (SHRDSA). Conclusion: The absence of reflex control exerted by arterial baroreceptors predominantly showed a greater influence of the aortic component of the carotid on BP. In addition, systolic function appears higher in groups SHRDA and SHRDSA, suggesting that aortic denervation is associated with sympathetic activation. No cardiac abnormality was observed in the carotid denervation. Regarding skeletal muscle, all denervations showed an increased of capillarization, while only SHRDSA group showed reduction of intermediate fibers. Increased capillarization may be associated with the release of the peripheral sympathetic denervation, including removal of the aortic baroreceptors, leading to increased VPA and the absence of chemoreceptors in groups with deafferentation of the carotid receptors
