Utvärdering av munvårdsutbildning för sjuksköterskestudenter : En enkätstudie / Evaluation of oral health care education for nursing students. : A survey study.

Bakgrund: Den orala hälsan har påverkan på individers välbefinnande, livskvalitet och allmän hälsa. Hälso- och sjukvårdspersonal bör ha kunskaper om orala hälsans risk och friskfaktorer samt hantering av symtom. Munvårdsutbildning är avgörande för att utföra åtgärder som krävs vid ohälsa, för att förebygga munhälsa och för att skapa förståelse om vikten av samarbete mellan tandvård och hälso- och sjukvården. Syftet: Syftet med studien var att studera om kunskap inom oral hälsa och munvård ökar genom att genomföra munvårdsutbildningen som görs i samverkan mellan sjuksköterske- och tandhygienistutbildningen.  Material och metod: En interventionsstudie genomfördes med pappersenkäter. Urvalet var sjuksköterskestudenter i termin två på Jönköping University. Metodanalys genomfördes med fisher exact test samt en kvalitativ innehållsanalys. Resultat: Respondenternas kunskap inom oral hälsa och munvård ökade efter utförd munvårdsutbildning och det visades en statistisk signifikant skillnad på de flesta frågor före respektive efter. Utifrån respondenternas reflektioner framkom tre kategorier, erhöll ny kunskap, Fick förståelse för munvård och sambandet mellan oral-allmän hälsa och Behöver praktisk träning. Slutsats: Sjuksköterskestudenters kunskap om munhälsa och munvård ökade efter utförd munvårdsutbildning. Utbildningen bidrog till framtida kliniska färdigheter och gav en inblick i samverkan mellan professioner. / Title: Evaluation of oral health care education for nursing students. A survey study. Background: Oral health care has an impact on individuals' well-being, quality of life and general health. Healthcare professionals should have knowledge of oral health risks and health factors as well as treatment of symptoms. Oral health education is crucial to carry out measures required in case of bad oral health and to create an understanding of the importance of cooperation between dental care and health care. Aim: The purpose of this study was to study whether knowledge in oral health and oral care increases by implementing oral health education in collaboration between the nursing program and the dental hygienist program. Method: An intervention study was conducted with paper questionnaires. The selection was nursing students in semester two at Jönköping University. Method analysis was conducted with fisher exact test and a qualitative content analysis. Results: The respondents' knowledge of oral health and oral care increased after oral health education and there was a statistically significant difference in most questions before and after. Based on the respondents' reflections, three categories emerged, Gained new knowledge, Gained an understanding of oral care and the link between oral-general health, Need practical training. Conclusion: Nursing students' knowledge of oral health and oral care increased after oral health education. The education contributed to future clinical skills and provided insight into interprofessional collaboration.

Building a Foundation for Interprofessional-Education (IPE) Between Dietetic Students and Dental Hygiene Students at East Tennessee State University (ETSU)

Nutrition education is an integral part of dental education as well as a variety of other healthcare professions1, but interprofessional education (IPE) between the fields of dietetics and dental hygiene is limited. The purpose of this pilot study was to define areas of opportunity to establish a foundation for the implementation of complimentary curriculum between the dietetics and dental hygiene programs at ETSU. A 76-question survey was developed and administered to dietetic interns (n=26), dental hygiene students (n=49), dietetic faculty (n=23), and dental hygiene faculty (n=19) at ETSU and Baylor College of Dentistry at Texas A&M Health Science Center. Data analysis reveals a knowledge proficiency deficit in dental hygiene students related to nutrition and oral health as well as significant findings in perceived roles of the ‘other’ profession. The potential for interdisciplinary education and training between dietetic and dental hygiene students at ETSU is promising, potentially leading to improved patient care.

Munnen är ett viktigt organ vad gäller äldres fysiska, psykiska och socialavälbefinnande. Syftet med studien är att sammanställa sjuksköterskans syn påvilka faktorer och förhållande som spelar in på hur den orala vården utförs påäldre. Metoden för studien är en litteraturstudie. Resultatet visade attkunskapsbrist, rutinbrist, tidsbrist, utvärderingsredskap, oral hälsa i syfte attförebygga sjukdomar, etiska dilemma och samarbete med tandvården hade en storbetydelse för hur den orala vården utfördes. Kunskapsbristen ansågs vara kärnantill varför de äldres orala vård försummades av vårdpersonalen. Det framkomockså att munvården var ett bortprioriterat område i sjukvården och att det fannsett bristfälligt samarbete mellan olika yrkeskategorier och olika instanser. / The mouth is an essential organ to older people concerning their physical, mentaland social well-being. The aim of this study was to compile nurse’s view onwhich factors and conditions that matters the performance of oral care. Themethod was a literature study. The result showed that there was a lack ofknowledge, failing of routines, lack of time, lack of assessment tools, oral healthto prevent diseases, ethical dilemmas and collaboration with dental care have aimportant meaning to oral care performance. The lack of knowledge considered tobe the core why the oral care of aged were neglected. It also emerged that the oralcare was neglected by nursing staff and that an insufficient cooperation betweendifferent professions and instances.

Gender Differences in the Association of Periodontitis and Type 2 Diabetes

Aims: The objective was to investigate if gender differences exist in the associations between periodontitis and type 2 diabetes. Disproportionate disparities by gender were found to exist in rates of both periodontitis and diabetes with respect to demographics and behavioural predictors that cannot be explained solely by the well-established association between these two diseases. Materials and methods: Multiple datasets were extracted from the National Health and Nutrition Examination Survey (NHANES) 2009–2014, which used a stratified multistage probability sampling to obtain samples from all civilian non-institutionalised people in the USA. Bivariate relationships between each explanatory variable and periodontitis level were assessed with odds ratios (OR) and their 95% confidence intervals (CI). A set of weighted logistic regression models was used to investigate the association differentiations between periodontitis and diabetes by gender. C-statistics measured the goodness-of-fit of weighted logistic regression models. Results: The prevalence of moderate–severe periodontitis was 36.39% and 22.71% among participants with type 2 diabetes and without diabetes, respectively. Type 2 diabetes was significantly associated with moderate–severe periodontitis OR (OR = 1.47, 95% CI: 1.18–1.82) among males even after adjusting for demographics, socioeconomic status and oral health behaviours. The aforementioned relationship was not found in females. Furthermore, different relationships of moderate–severe periodontitis with body mass index and the use of mouthwash were found between the males and females. Conclusions: The current findings suggest that important improvements in the development of gender-specific strategies in prevention, such as oral home-care, to reduce the high prevalence of periodontal disease and maintain good oral health are vital, and are especially important for male diabetic patients and those who are at high risk of developing diabetes, such as those who are obese.

Oral health and dental services utilization of children with learning disabilities

OBJECTIVE: The purpose of this dissertation is to examine the oral health, caregivers’ perception of oral health, and dental services utilization among children with learning disabilities (LD). METHODS: We used the National Health And Nutrition Examination Survey 1999-2004 data to investigate the oral health and caregivers’ perception of oral health among children with LD alone, ADHD alone, and LD with ADHD. We used the National Survey of Children’s Health 2011-2012 data to examine dental services utilization and unmet dental needs among children with LD alone, non-LD CSHCN, and LD CSHCN. RESULTS: Children with LD alone have greater likelihood of having dental caries in permanent dentition compared to non-LD, non-ADHD children (OR: 1.6, 95% CI: 1.1-2.2), while the likelihood of dental caries in permanent dentition among children with combined LD and ADHD is much greater (OR: 1.9, 95%CI: 1.3-2.7). Caregivers of children with LD, ADHD, and LD with ADHD perceived their oral health to be poorer when compared to non-LD, non-ADHD (OR: 1.8, 95% CI: 1.2-2.8, OR: 1.9, 95% CI: 1.1-3.0, OR: 2.0, 95% CI: 1.3-3.1, respectively). The accuracy of caregivers’ assessments of their children’s oral health was lower among those with LD, ADHD, and LD with ADHD children. Children with ADHD alone had the least accurate caregivers’ perception (OR: 0.5, 95% CI: 0.3-0.8). Children with LD alone were less likely to have a dental visit within the past year, whether for preventive visit (OR: 0.6, 95% CI: 0.5- 0.9) or any other dental visits (OR: 0.7, 95% CI: 0.5- 0.9). While LD severity did not impact dental visit receipt, children with moderate to severe LD have higher unmet dental needs than non-LD and mild-LD (OR: 1.8, 95% CI: 1.3- 2.5). CONCLUSION: Children with learning disabilities have significant oral health needs and are at a greater risk for dental disease. Despite that, children with LD are less likely to utilize preventive and other dental services. Future interventions need to target this vulnerable population to improve their oral health and reduce these disparities.

Assessing the quality of mobile apps for oral health: content analysis and usability

AIM: To review and characterize dental mobile apps in the peer-reviewed literature and two popular mobile app stores, and assess their quality. METHODS: A scoping review methodology was used to identify different types of mobile dental apps in seven medical and technical databases. The data of the identified research studies on dental apps were extracted (coded) by two researchers in a systematic process and the results were analyzed. Dental apps’ characteristics, publication pattern, development methodology, efficacy, and usability were reported. Then, a systematic review was conducted in Google Play and iOS app stores to identify patient-facing dental apps. Popular keywords were identified from multiple sources such as using Google Trends. Two researchers extracted the information of the identified dental apps and descriptive, and correlation analyses were conducted. Apps were evaluated for the presence of behavior change techniques (BCTs), evidence-based guidelines, technological features, privacy and security, credibility and health literacy. Lastly, a quality assessment using MARS scale was conducted for a selected sample of dental patient-facing apps from the published literature and the popular app stores. RESULTS: The search resulted in 38 studies on dental apps. The identified dental apps (37) targeted multiple users and the common topic was oral pathology such as dental caries. The majority of dental apps was developed for diagnostics and screening purposes (40.5%). Apps included varied features and educational strategies. Only two papers used theories for developing dental apps, and five studies followed user-centered design principles. Fourteen papers only reported the use of clinical guidelines for developing dental apps (36.8%). Mobile app search resulted in 52 patient-facing dental apps (37 Android apps, and 15 iOS apps). The majority of apps focused on Oral Hygiene Behavior (31%). Apps targeted multiple users and contained varied features and BCTs including instructions (80.8%) and credible source (67.3%). Eleven apps were identified for the third project and the highest total MARS score was Philips Sonicare. The app contained the largest number of features compared to other apps (24). CONCLUSION: The dissertation will warrant the selection and prescription of high-quality dental apps, and will advance the research work in mobile apps in dentistry.

Insufficient Oral Behaviour and the High Need for Periodontal Treatment in Patients with Heart Insufficiency and after Heart Transplantation: A Need for Special Care Programs?

Background: The aim of this cross-sectional study was the assessment of dental behaviour and oral health condition of heart transplant recipients (HTx) in comparison to patients with heart insufficiency (HI). Methods: Patients attending the Department for Cardiac Surgery, Leipzig Heart Center, Germany were recruited. Standardized questionnaires regarding dental behaviour and periodontal complaints were applied. A dental (decayed-, missing- and filled-teeth index) and periodontal examination (periodontal probing depth (PPD) and clinical attachment loss (CAL)) was performed. Based on the oral findings, dental and periodontal treatment need was determined. Statistics: T-test, Mann-Whitney U test, Chi-square test, and Fisher-test (p < 0.05). Results: A total of 201 patients (HTx: 112, HI: 89) were included. HTx patients were significantly more often allocated to dentists (p < 0.01). Furthermore, the HTx patients rated feeling informed appropriately about oral health more often (p < 0.01). HTx patients used interdental cleaning (p < 0.01) and mouth rinse (p = 0.02) more often than HI patients. No differences between groups were present regarding dental status and periodontitis severity (p > 0.05). Periodontal treatment need was high, showing prevalence of 79.5% (HTx) and 87.6% (HI, p = 0.14), respectively. Conclusions: Both groups show insufficient oral behaviour and a high need for periodontal treatment. Special care programs for HTx candidates and recipients appear recommendable.

Lack of oral health awareness and interdisciplinary dental care: a survey in patients prior to endoprosthesis and orthopaedic centres in Germany

Objectives: This cross-sectional survey aimed to evaluate the oral health behaviour of patients prior to endoprosthesis (EP), as well as the handling of oral health topics by German orthopaedic surgeons. Materials and methods: Consecutive patients prior to EP answered a questionnaire regarding oral health behaviour, oral hygiene, oral complaints and information on the relationship between EP and oral health. Another questionnaire was digitally mailed to orthopaedic centres throughout Germany. This questionnaire included the importance of oral health for EP and issues on dental referrals/consultations prior to EP. Results: A total of 172 patients were included in the study, of whom 35.5% of patients reported that they were informed about oral health and EP. Half of the individuals reported regular professional tooth cleaning, and less than one-third (29.1%) reported of the performance of interdental cleaning. Information on oral health and EP was associated with regular professional tooth cleaning (yes: 59.8% vs. no: 35.6%, p = 0.01). A total of 221 orthopaedic clinics were included in the study, of which only a few had dental contact (14%), although the majority (92.8%) of the clinics were familiar with causal relationships between oral health and EP infections. Less than half of the centres reported of either verbal (48%) or written (43.9%) referrals for their patients to the dentist. University Medical Centres reported of more frequent dental contacts (p < 0.01). Conclusion: Prior to EP, patients exhibited deficits in oral health behaviour, and orthopaedic clinics exhibited a lack of dental collaboration. Improvements in interdisciplinary care, especially regarding practical concepts for patient referral and education on oral health, appear to be necessary.

Bakgrund En god munhälsa är en förutsättning för att bibehålla välbefinnande och livskvalitet. Äldre personer med nedsatt egenvårdsförmåga behöver stöd för att bibehålla god munhygien. Sjuksköterskan ansvarar för att kartlägga den äldres munstatus samt risker för att förebygga ohälsa. Genom ett personcentrerat perspektiv kan hela personen synliggöras. Personcentrerad vård innefattar att se helheten, respektera och bekräfta individen och dennes behov för att främja hälsa. Syfte Syftet med litteraturstudien är att undersöka äldre personers upplevelser och inställningar till munhälsa och munvård. Metod Examensarbetet har genomförts som en strukturerad litteraturstudie med inslag av den metodologi som används vid systematiska översikter genom sammanställning av data från13 artiklar. Sökningarna gjordes i databaserna CINAHL och PubMed. Urvalet följde Fribergs (2017) metod. Dataanalys gjordes som en integrerad analys; en förenklad form av innehållsanalys. Resultat Äldres upplevelser och inställningar kan delas in i två huvudkategorier; “äldres syn på munhälsa och munvård” och “omständigheter som påverkar äldres munhälsa”. Inom respektive kategori identifierades underkategorier. Slutsats Faktorer som socialt umgänge, värdighet, födointag, tidigare erfarenheter, prioriteringar, ekonomi, hinder i tillgänglighet, brister i kunskap och utförande påverkade äldre personers erfarenheter och inställningar till munhälsa och munvård. / Background Good oral health is a prerequisite of maintaining well-being and quality of life. Elderly people with a reduced ability of selfcare need support with oral hygiene. Nurses are responsible for mapping out the oral status of elderly and risks to prevent illness. The entire person can be made visible through a person-centred perspective. Person-centred care involves seeing the entirety of a person, respecting, and confirming the individual and their needs to promote health. Aim The aim of the literature study is to examine elderlies’ experiences and attitudes towards oral health and oral care. Method This thesis was made as a structured literature study with elements of the methodology used when conducting systematic reviews, through a compilation of data from 13 articles. Searches were made in the databases CINAHL and PubMed. The selection followed Friberg’s (2017) method. Data analysis was made as an integrated analysis, a simplified content analysis. Results Elderlies’ experiences and attitudes can be divided into two main categories: “elderlies’ views on oral health and oral care” and “circumstances that impact elderlies’ oral health”. Within each category, subcategories were identified. Conclusions Factors such as social interaction, dignity, nutritional intake, previous experiences, priorities, economy, lack of accessibility, poor knowledge and management influenced elderlies’ experiences and attitudes towards oral health and oral care.
