Militariseringen av EU : Varför valde Sverige att ingå i Pesco?

Med anledning av den förändrade säkerhetsmiljön i Europa startade en process som syftade till att öka samarbetet inom säkerhets- och försvarsområdet inom EU. Detta försvarssamarbete kallas det Permanenta strukturerade samarbetet (Pesco) och innebär ett mer upptrappat och konkret militärt samarbete än tidigare inom EU. Genom ett medlemskap i Pesco förbinder sig medlemmarna till att mer intensivt utveckla sin egen försvarskapacitet inom forskning och anskaffning av försvarsmateriel, men även att uppbringa och bibehålla en stark försvarsbudget. Vidare ska även medlemstater bidra med stridsgrupper i beredskap för insatser inom EU:s ram. Hur kan vi förstå logiken i att Sverige ingår med i ett mer bindande försvarssamarbete som Pesco. Syftet med denna studie är att identifiera bakomliggande drivkrafter för att förstå varför Sverige beslutade att ingå i Pesco. Denna fallstudie har nyttjat ett teoretiskt perspektiv som utgått ifrån Graham Allisons konceptuella modeller för att identifiera dessa bakomliggande drivkrafter. Genom en kvalitativ textanalys studeras materialet i denna fallstudie bestående av regeringens proposition gällande deltagande i Pesco, Försvarsutskottets betänkande gällande Pesco och förvarspolitisk inriktning 2016-2020. För att komplettera textmaterialet har även mailintervjuer genomförts med strategiskt utvalda informanter i form av riksdagsledamöter och stabsofficerare i Försvarsmakten. Studiens slutsatser påvisar att de bakomliggande drivkrafterna för beslutet var flera. Den främsta drivkraften var att Sverige sedan tidigare ratificerat EU:s solidaritetsklausul och uttalat en solidaritetsförklaring gentemot övriga medlemstater i EU. Genom att ingå i Pesco förväntas det öka Sveriges trovärdighet som medlemsstat i EU. En ytterligare drivkraft var att ingå i Pesco i ett tidigt skede i syfte att forma samarbetet och ha inflytande i den riktning som Sverige anser var förenliga med militär alliansfrihet, samt att fortsatt driva den mellanstatliga prägel som samarbetet nu innehar. En drivkraft var också att bygga upp det nationella försvaret, öka den operativa förmågan och stärka totalförsvaret genom försvarssamarbetet. En majoritet av Riksdagens partier var överens om Sveriges ingående i Pesco, detta på grund av tidigare beslutad Försvarsinriktningsperiod 2016-2020 som också var en bärande drivkraft till varför Sverige valde att ingå i Pesco. / In response to the changing security environment in Europe a process started aimed at increasing cooperation in the security and defence area within the EU. This defence cooperation is called the Permanent structured cooperation (Pesco) and means a more gradual and substantial military cooperation than before in the EU. Through a membership in Pesco, the members commit to more intensively develop of its own defence capabilities in research and acquisition of defence equipment, but also to obtain and maintain a strong defence budget. In addition, the member states should also contribute with battle groups ready for military missions within the framework of the EU. How can we understand the logic of Sweden joining more binding defence cooperation like Pesco. The purpose of this study is to identify the underlying driving forces to understand why Sweden decided to join Pesco. This case study uses a theoretical perspective based on Graham Allison's conceptual models to achieve this purpose. Through a qualitative text analysis, the data that is studied in this case study consisting of Swedish government proposition and Defence committee report regarding Pesco and Defence bill 2016-2020. In order to widen the study, mail interviews were conducted with strategically selected informants. Those were members of the Swedish parliament and staff officers in the Swedish Armed Forces. The study concludes that the underlying driving forces for the decision were several. The main driving force was that Sweden previously ratified the EU solidarity clause and stated a declaration of solidarity in relation to other member states of the EU. By joining Pesco is also expected to boost Sweden's credibility as a member state of the EU. An additional driving force was to join Pesco at an early stage in order to forge cooperation and have influence in the direction in which Sweden considers compatible with own military nonalignment, and also to continue to drive the intergovernmental nature that Pesco now holds. An additional driving force was also building up the national defence, increase the operational capacity and strengthen the armed forces through the defence cooperation. A majority of the parliamentary parties agreed on joining Pesco because of the previously decided defence bill 2016-2020, which also considers as a driving force for why Sweden chose to join Pesco.

The severity of this injury is directly related to factors associated with the therapy itself and the individual characteristics of each patient. To date, there is no standard treatment protocol for this condition. Therefore, the present study aimed to evaluate clinically and histologically the effect of granulocyte colony-stimulating factor (G-CSF) on the healing of traumatic ulcers produced in the tongue of Wistar rats undergoing RT. The animals were randomly divided into 4 groups: 1) RT + traumatic ulcer + Filgrastim; 2) RT + traumatic ulcer + saline; 3) no RT + traumatic ulcer + Filgrastim; 4) no RT + traumatic ulcer. Filgrastim (r-metHuG-CSF), a recombinant G-CSF, was injected subcutaneously into the animals once daily, beginning 1 day after a single-dose radiation protocol (30 Gy), immediately after the traumatic ulcer was made. At the end of a 7-day experimental period, animals were euthanized and had their tongue surgically removed, processed, and stained with hematoxylin and eosin (HE). Clinically, all groups differed significantly in presence and size of traumatic ulcers, and the irradiated groups also differed significantly in severity of radiation-induced OM. Filgrastim-treated groups, submitted or not to RT, showed a better performance in this aspect. Histologically, there was an increased inflammatory response in the nonirradiated groups, with no statistically significant difference between the study drug and the respective control groups. Based on the present clinical results, we conclude that the use of Filgrastim reduced the manifestation and severity of trauma-induced ulcers and radiation-induced OM. This condition has a complex pathogenesis, which requires further detailed investigations on the actual role of G-CSF in its management before a formal recommendation regarding its use for this purpose can be made. / A mucosite oral (MO) radioinduzida ? um efeito colateral altamente debilitante, que afeta a quase totalidade dos indiv?duos portadores de neoplasias malignas na regi?o de cabe?a e pesco?o, submetidos ? radioterapia (RT). A gravidade desta inj?ria est? diretamente relacionada a fatores pertinentes ? terapia em si e ?s caracter?sticas individuais de cada paciente. N?o h?, at? o momento, um protocolo padr?o para o tratamento desta enfermidade. Sendo assim, o presente estudo objetivou analisar clinica e histologicamente a a??o do fator estimulador de col?nias de granul?citos (G-CSF) em ?lceras traum?ticas confeccionadas na l?ngua de ratos Wistar submetidos ? RT. Os animais foram alocados randomicamente em quatro grupos distintos: 1) RT + ?lcera traum?tica + Filgrastim; 2) RT + ?lcera traum?tica + soro fisiol?gico; 3) sem RT + ?lcera traum?tica + Filgrastim; 4) sem RT + ?lcera traum?tica. O Filgrastim (r-metHuG-CSF), um G-CSF recombinante, foi injetado diariamente nos animais pela via subcut?nea, iniciando um dia depois do protocolo ?nico de irradia??o (30 Gy), imediatamente ap?s a confec??o da ?lcera traum?tica. Uma vez conclu?do o tempo experimental de sete dias, os animais foram eutanasiados e tiveram a l?ngua excisada, processada e corada com hematoxilina e eosina (HE). A avalia??o cl?nica diferiu significativamente entre todos os grupos quanto ? presen?a e ao tamanho da ?lcera traum?tica, al?m do grau de severidade da MO radioinduzida, entre os grupos irradiados. Os grupos tratados com Filgrastim, submetidos ou n?o ? RT, apresentaram uma melhor performance neste quesito. Histologicamente, houve maior resposta inflamat?ria nos grupos n?o irradiados, sem diferen?a estatisticamente significante entre a medica??o estudada e os respectivos grupos controle. Baseado nos resultados cl?nicos encontrados, pode-se concluir que o uso do Filgrastim reduziu a manifesta??o e o grau de severidade da ?lcera confeccionada e da MO radioinduzida. Esta ? uma doen?a de patogenia complexa, a qual necessita investiga??es mais detalhadas sobre o real papel do G-CSF no seu manejo, para que uma indica??o formal do seu uso possa ser recomendada com esta finalidade.

Para a coleta de dados foi utilizado o Question?rio sociodemogr?fico, o Question?rio de avalia??o da Universidade de Washington, a Escala de Investimento Corporal, a escala de Autoestima de Rosenberg e a Escala de Ansiedade e Depress?o Hospitalar. A amostra foi constitu?da por 10 pacientes (06 homens e 04 mulheres), com idade entre 35 e 82 anos. Os resultados obtidos mostraram que as melhores pontua??es na escala UW-QoL foram no dom?nio Ombro. As piores pontua??es foram no dom?nio Mastiga??o para os homens e dom?nio Fala para as mulheres. Dentre os dom?nios da Escala de Investimento Corporal, o dom?nio Cuidado corporal apresentou diferen?a significativa entre os homens e mulheres, apontando que as mulheres ap?s o tratamento do CCP exercem um maior cuidado sobre o pr?prio corpo. A autoestima dos participantes da pesquisa foi classificada entre m?dia e alta, indicando a exist?ncia de autovalor positivo, n?o havendo diferen?a significativa entre homens e mulheres. Houve correla??o significativa entre a Qualidade de vida geral e a Imagem corporal, apontando que quanto melhor a qualidade de vida, melhor a rela??o com a imagem do corpo. Atrav?s desse estudo foi poss?vel observar que para os pacientes com c?ncer de cabe?a e pesco?o os aspectos funcionais s?o os mais significativos para a obten??o de uma melhor QV, bem como que existe rela??o entre a imagem corporal e a qualidade de vida nestes pacientes. / The head and neck cancer, as well as its treatment, cause major aesthetic and basic functions changes for the patient and may cause changes in Quality of Life, Body Image and Self- Esteem. The aim of this study was to evaluate the Quality of Life, Body Image and Self- Esteem in patients with head and neck cancer in the city of Natal-RN candidates for prosthetic rehabilitation. We conducted a cross-sectional study. For data collection was used a sociodemographic questionnaire, the University of Washington?s evaluation questionnaire, the Body Investment Scale, the Rosenberg?s Self-esteem scale and the Hospital Anxiety and Depression Scale. The sample consisted of 10 patients (06 men and 04 women), aged between 35 and 82 years old. The results showed that the best scores at UW-QoL scale were in the shoulder area. The worst scores were in the chewing area for men and speech area for women. Among the areas of Investment Body Scale, body care domain showed a significant difference between men and women, noting that after treatment of CCP women exercise greater care over their own bodies. The self-esteem of survey participants ranked among medium and high, indicating the existence of positive self-worth, with no significant difference between men and women. There was a significant correlation between the overall quality of life and body image, indicating that the better the quality of life, the better the relationship with body image. Through this study it was observed that for patients with head and neck cancer, the functional aspects are the most significant to achieve a better QoL, and that there is a relationship between body image and quality of life in these patients.

PESCO – ett hot mot svensk alliansfrihet? : En kritisk studie av svensk utrikespolitiskpolicy och utökade militära samarbeten

Hur uppfattas den svenska militära alliansfrihetens ställning i takt med utökade militära samarbeten? Sveriges ingående av PESCO (Permanent Structured Cooperation), ett försvars-och säkerhetspolitiskt samarbete inom EU, har presenterats som ett steg i mot ett militariserat EU. Syftet med den här uppsatsen har varit att redogöra för och förstå underliggande problemframställningar till Sveriges säkerhets-och försvarspolicy samt vilken innebörd svenska policyskapare anseratt ökade samarbeten har för alliansfriheten. För att besvara syftet har regeringens utrikesdeklaration 2018, statens offentliga utredningar om säkerhetssamarbeten och propositionen om Sveriges ingående av PESCO analyserats med Carol Lee Bacchis kritiska policyanalys, What’s the problem represented to be?, och tolkats utifrån det teoretiska ramverket om nationell säkerhet och regionala säkerhetskomplex. Dominerande problemframställningar har presenterats utgöras av ryska aggressioner i östersjöområdet och Ukraina där både den svenska nationella säkerheten och EU:s regionala säkerhet upplevs hotad. Studiens slutsatser sammanfattas till att den nationella och regionala säkerheten upplevs förbättras med PESCO. Policyskapare anser även att alliansfriheten stärks och är beroende av ökade militära samarbeten, något som presenterats som ett nära steg till en allians, men studien kan inte avgöra hur allians friheten skulle påverkas av en direkt konflikt där en involverad stat undertecknat PESCO.

Among these are technological innovations our students are familiar with. In human anatomy teaching, these innovations are increasingly important, since the need for ?mechanizing? many structures may make learning tiresome and nonstimulating for students. The search for new technologies may make this process simpler. Another important issue is the need to constantly emphasize applying the knowledge in question, in order for students to better understand the meaning of learning. Accordingly, this project aims to develop an interactive tablet application for neck anatomy. This application will function as a guide to dissection, containing text, photographs, drawings and videos, distributed systematically, with a script emphasizing the clinical and surgical importance of neck anatomy.

Autonomi eller inflytande? : En innehållsanalys av Sveriges ambivalenta inställning till PESCO

Syftet med denna uppsats är att undersöka Sveriges inställning till det permanenta strukturerade samarbetet (PESCO) utifrån teorierna om småstaters säkerhetsdilemman med betoning på dikotomin mellan autonomi och inflytande. Undersökningen genomfördes med en textanalys av framförallt propositioner, motioner, partiprogram, debattartiklar, samt med samtalsintervjuer av Socialdemokraternas, Moderaternas, Vänsterpartiets och Liberalernas försvarspolitiska företrädare. Resultatet indikerar en dominans av parti-strategiska argument för samarbetet och kulturbaserade argument emot det. Slutsatsen är att Sveriges inställning till PESCO är i stort sett positiv, men att den präglas av en ambivalens som beror på pragmatiska argument kontra kulturbaserade argument.

Radiotherapy, whether associated or not with chemotherapy, is the therapeutic modality most often used for cancer of the head and neck. However, it can trigger reactions at the oral level, such as oral mucositis, the most limiting side effect, with intense pain and difficulty eating, which may require the suspension of oncologic treatment for the patient. Polyhexanide has been used effectively and safely in the treatment of acute and chronic wounds. Objectives: To describe the clinical-epidemiologic profile and investigate the efficacy of polyhexanide in the treatment of oral mucositis and pain relief, in the period of 8-15 days, among patients with cancer of the head and neck undergoing treatment with radiotherapy (whether associated or not with chemotherapy). Methods: A randomized clinical trial was conducted, parallel and double-blind, among patients with cancer of the head and neck undergoing radiotherapy treatment with or without chemotherapy, in the High Complexity Oncology Unit of Feira de Santana, Bahia, who developed oral mucositis, were 18 years of age or older and consented to participation and signed informed consent forms in the period of July 2015 to May 2016. Two equal groups were formed by random allocation?an intervention group (polyhexanide gel) and a control group (gel without polyhexanide). These groups were reevaluated after 8 and 15 days of use of the gels. Clinical exams, evaluations of the medical records, and interviews with the application of a survey instrument used in the study were approved by the Committee for Ethics in Human Subjects Research of the State University of Feira de Santana, under the number 1.074.479/2015. Statistical analysis consisted of relative and absolute frequencies, averages, medians, and standard deviation, and the p-value?considered statistically signification when <0.05?were obtained by non-parametric Friedman tests, Wilcoxon the posts with signs and Wilcoxon-Mann-Whitney. Results: The findings were presented as an article. The mean age was 59.68 years; 58.8% men; 67.6% Brown/Black, and 79.4% with an elementary school education or less. The majority of tumors (64.7%) already presented in advanced stage, and the most frequent were those of the pharynx (23.5%) and the oral cavity (20.4%). Radiotherapy associated with chemotherapy was the most used therapy. Grades III and IV of oral mucositis were the most frequent in the third and fourth weeks of radiotherapy treatment. A total of 34 individuals were accompanied, 17 in the intervention group and 17 in the control group. After a period of 8 to 15 days, statistically significant differences were observed (p<0.05) in relation to the reduction in severity of the oral mucositis and pain, between the time points of gel use in the intervention group. Conclusions: Knowledge of the clinical-epidemiologic profile of patients undergoing oncology treatment with oral mucositis is important to establish potentially effective treatments. In this study polyhexanide represented an effective therapy in the treatment of oral mucositis and relief of pain, with positive results in the first 8 days of use. / As neoplasias malignas representam uma das principais causas de morbimortalidade no Brasil e no mundo. M?todos: Realizou-se um ensaio cl?nico randomizado, paralelo e duplo cego, com todos os pacientes com c?ncer de cabe?a e pesco?o sob tratamento com radioterapia associada ou n?o a quimioterapia, na Unidade de Alta Complexidade em Oncologia de Feira de Santana, Bahia, que desenvolveram mucosite oral, tinham 18 anos ou mais de idade e aceitaram participar assinando o termo de consentimento livre e esclarecido, no per?odo de julho de 2015 a maio de 2016. Dois grupos foram igualmente formados por aloca??o aleat?ria, o de interven??o (gel de polihexanida) e o grupo controle (gel sem polihexanida), e reavaliados nos intervalos de 8 e 15 dias de uso dos g?is. Avalia??es cl?nicas, de prontu?rios e entrevistas com aplica??o de formul?rio foram aplicadas neste estudo aprovado pelo Comit? de ?tica em Pesquisa com Seres Humanos da Universidade Estadual de Feira de Santana sob o n?mero 1.074.479/2015. Para a an?lise estat?stica empregaram-se as frequ?ncias relativas e absolutas, m?dias, medianas e desvio-padr?o, e o valor de p, considerado estatisticamente significativo quando ? 0,05 foi obtido por meio dos testes n?o param?tricos de Friedman, Wilcoxon dos postos com sinais e Wilcoxon-Mann-Whitney. Resultados: Os achados foram apresentados em forma de artigo. A m?dia de idade foi de 59,68 anos; 58,8% homens; 67,6% de cor parda e 79,4% analfabetos ou com baixo grau de escolaridade. A maioria (64,7%) dos tumores j? se apresentava em est?dio avan?ado, sendo os mais frequentes os da faringe (23,5%) e cavidade oral (20,4%). A Radioterapia associada ? quimioterapia foi a terap?utica mais empregada. Graus III e IV de mucosite oral foram mais frequentes nas 3? e 4? semanas de tratamento radioter?pico. Um total de 34 indiv?duos foi acompanhado, sendo 17 do grupo de interven??o e 17 do grupo controle. Ap?s um per?odo de 8 e 15 dias, observaram-se diferen?as estatisticamente significativas (p<0,05) em rela??o a redu??o da gravidade da mucosite oral e da dor, entre os tempos de uso do gel no grupo de interven??o. Conclus?es: O conhecimento do perfil cl?nico-epidemiol?gico de pacientes em tratamento oncol?gico com mucosite oral faz-se importante para se estabelecer terap?uticas potencialmente eficazes. Neste estudo a polihexanida apresentou-se como uma terap?utica eficaz no tratamento da mucosite oral e controle da dor, com resultados positivos ainda nos primeiros 8 dias de uso.

PESCO: A New Era in EU’s Security Rhetoric?

The aim of this paper is to uncover what has changed in the EU security field that caused the EU to decide that their new military treaty, PESCO, should be the first treaty that excludes NATO from its constitution. The purpose for this aim is to understand if a paradigm shift has occurred in EU’s security rhetoric in order to understand the general picture of the EU security research field on the day PESCO was officially welcomed. The European Council will be the main actor and subject of the thesis, where the press statements of select EU members along with the Council President will be analysed as data. The context behind their statements will be brought forth based on the content that they deliver in the statements, which will by default deepen the comprehension of the EU security situation on the day that those statements were made.
