The Effects of the Work Environment on California Community College Counselors' Professional Self-care

Healthy work environments are critical in fostering occupational self-care for helping professionals, but information on the self-care and the work environment is limited among community college counselors in California. Self-care is essential for the counseling profession in California community colleges (CCC), as it promotes wellness, which is needed in order to provide optimal services to students/clients. The lack of self-care results in counselor impairment, leading to undesirable factors (i.e., burnout, health conditions, and decrease in productivity). In educational institutions, counselors often find it difficult to maintain their own self-care. However, work environments have been shown to affect self-care at work (professional self-care). Past studies have suggested that the work environment can help counselors increase their ability to practice self-care. In this quantitative statewide study on 324 CCC counselors, it was found that the work environment was related to and predicted CCC counselors' professional self-care, even when considering demographic variables. Additional findings from this research study indicated that CCC counselors’ workload had the highest relation with counselors’ daily balance. Differences in groups (e.g., demographic/contextual factors) are found among CCC counselors' work environment and professional self-care, with the greatest group difference on CCC counselors’ status (e.g., tenured, adjunct). Results also suggest that CCC counselors who worked overload (if full-time) strengthened the relationship between their professional development and a sense of community in the workplace. However, overload hours worked per week showed a negative association with their sense of community. These results may be useful to community college counselors and administrators for organizational planning, policy, and advocacy of self-care-promoting work environments for CCC counselors. A limitation of the study was that the majority of participants were female, White, and tenured faculty, which may not represent those outside of this population.

Bakgrund: Hjärtsvikt är en diagnos som ökar i takt med att populationen i världen blir äldre. Patienter med hjärtsvikt kan via egenvård uppnå symtomlindring. Kommunikationen från sjukvården kring hur patienten ska uppnå en god egenvård är bristfällig.  Syfte: Syftet var att beskriva upplevelser av egenvård vid hjärtsvikt för vuxna patienter inom öppenvården. Metod: En kvalitativ litteraturöversikt med en induktiv ansats.  Resultat: I resultatet framkom fyra huvudkategorier, “Förståelse och kunskap kring egenvård”, “Behov av stöd för att utföra egenvård”, “Behov av att känna sig delaktig” och “Begränsningar i det vardagliga livet” med tillhörande subkategorier.  Slutsats: För att uppnå en god egenvård krävdes stöd från hälso-sjukvården samt anhöriga. Patienten behövde få känna sig delaktig i egenvården. Informationen kring egenvård från sjukvården ansågs bristande. / Background: Heart failure is a diagnosis that increases as the world's population ages. Patients with heart failure can achieve symptom relief through self-care. Communication from the healthcare system regarding how the patient should achieve good self-care is deficient.  Aim: The aim was to describe experiences of self-care in heart failure for adult patients in outpatient care.  Method: A qualitative literature review with an inductive approach.  Results: The result was four main categories, "Understanding and knowledge of self-care", “Need for support to carry out self-care", "Need to feel involved" and "Limitations in everyday life" with associated subheadings.  Conclusion: In order to achieve good self-care, support from health care and relatives was required. The patient needed to feel involved in self-care. Information about self-care from the healthcare system was considered insufficient.

Relationships among quality of life, self-care, and affiliated individuation in persons on chronic warfarin therapy

This descriptive, correlational, cross-sectional study explored the relationships among the variables self-care action, self-care knowledge, and affiliated individuation and quality of life for persons on chronic warfarin therapy. This study also explored the moderating effects of self-care knowledge and affiliated individuation on quality of life. This research was guided by a theoretical framework based on modeling and role-modeling theory (Erickson, Tomlin, & Swain, 1983). The sample consisted of 83 adults between the ages of 30 to 91 years. The majority of participants were Caucasian, educated, retired and almost evenly distributed between male and female. Each subject completed the following instruments: the Oral Anticoagulation Knowledge (OAK) test, the Duke Anticoagulation Satisfaction Scale (DASS), the Basic Needs Satisfaction Inventory (BNSI), and the generic quality of life survey (SF36v2). Data was analyzed using correlation and hierarchical multiple regression analysis. Results indicated significant correlations among most of the study variables. Self-care action significantly explained variances in all but two quality of life variables. Self-care knowledge and affiliated individuation had statistically significant moderating effects on the DASS negative impact and hassles/burdens subscales. Self-care knowledge also demonstrated a significant moderating effect on the SF36v2 physical function subscale. These findings support the concepts proposed by the study's theoretical framework. This research serves as validation of Acton's (1997) study findings for the concept of affiliated individuation and its value as a self-care resource in a specific clinical population. / text

Personal interviews were mainly used as research technique to identify problems, determine needs and address self-care deficiencies. Data was evaluated according to UNICEF' s GOBI FFFF, the "PKK" 's five concepts, the nursing process and OREM's Constructs. Examining the perceived nurses' support for self management among type 2 diabetes mellitus patients in Botswana

Patients’ perceptions of health care and support is a key determinant in self-efficacy and active participation in the management of chronic conditions. Nurses play a significant role in Diabetes Self-Management Education by providing clients with tools, empowerment and knowledge to self-manage their condition. The purpose of this study was to examine and describe diabetes patients’ perceptions of nurses’ support for self-management in Botswana. The aim was to improve clients’ skills in self-management and to strengthen diabetes health care management. This study was carried out at the Diabetes Clinic of Excellence, in the city of Francistown, the second largest city in Botswana. The study population comprised of Type 2 Diabetes patients registered at the Diabetes Clinic for their follow-up. All were aged 18 years and above. Three hundred and fifty-four (354) patients participated in the study. The study used a non-experimental, descriptive, quantitative design. Probability sampling method was used to recruit diabetes patients from the selected clinic. Data were were collected using a structured, researcher developed, questionnaire mostly in face-to-face interviews, a few participants completed the questionnaire. The Quantitative data analysis included descriptive and inferential statistics using SPSS (Statistical Package for Social Science Software (version 25). Spearman rho was used to determine statistical correlation between patients’ perceptions and their self-management practices. The findings showed that generally, patients’ perceptions of professional support was positive regarding most of the constructs measured. However, there were areas that showed less satisfaction with the support such as foot-care, risk control, and ability to identify signs of low and high blood sugar level and carrying of Identification Band (ID). Perceptions of nurses’ motivational behaviour showed varied responses. Patients’ self-care activities were sub-optimal and showed some variations which tended to correspond with their perceptions of professionals support. The Spearman's correlation results ranged from strong, moderate, and weak positive correlation. A few demographic variables showed some impact on self-care activities. Based on the findings, the study concludes that professional support through DSME and DSMS, self-management and patients’ perceptions of care play a significant role in diabetes management. / Health Studies / M.A. (Nursing Science)

La santé au travail des aides à domicile dans le secteur gérontologique prestataire breton La difficile prévention de soi dans l’exercice du care / Health at work

Le vieillissement de la population et les conséquences de la dépendance font évoluer les besoins d’accompagnement. Cette problématique est majeure en Bretagne. L’accompagnement de la personne âgée est réalisé par les aidants naturels et différents professionnels parmi lesquels les aides à domicile. Les enquêtes épidémiologiques présentent ces derniers comme exerçant un métier à risque : le métier relationnel, l’isolement professionnel et les conditions de travail pénibles et précaires fragiliseraient leur santé. A partir des approches écologiques et socio-environnementales définissant les déterminants du développement individuel, nous identifions une interdépendance forte entre les aides à domicile et la personne âgée. Cette recherche repose sur l’idée que la disposition de santé des aides à domicile participe au« bien-vieillir » dont ils sont des leviers majeurs.L’objectif de ce travail vise à proposer de nouveaux axes de prévention de la santé à destination de ces pourvoyeurs du care, dont les effets favorables contribueront à conforter l’accompagnement des personnes âgées. Nous avons réalisé une analyse approfondie sur 32 entretiens d’aides à domicile du secteur prestataire. Les résultats confirment une situation de santé sensible. Les valeurs associées au care et au travail social en contraignent la prévention,par un effacement de soi lié à un care perçu comme devant en priorité bénéficier à l’Autrui dépendant. Ce constat conduit à intégrer une nouvelle dimension dans les modèles de prévention de la santé, la capacité de« self-care », nécessitant un apprentissage et constituant le déterminant de la propension au comportement préventif, à ce jour peu mis en oeuvre parles aides à domicile. / The ageing of the population and the consequences of dependence are changing support needs. These problems are major in Bretagne. The accompaniment of the old person is carried out by the family and various professionals among whom the home-care worker. Epidemiological surveys have them as exercising a profession at risk: the relational work, professional isolation and painful and precarious working conditions weaken their health. From the ecological and socio-environmental approaches defining the determinants of individual development, we identify a strong interdependence between the home-careworker (caregivers) and the elderly. This research is based on the idea that the health disposition of caregivers participates in the "well - ageing" of which they are the major levers. The objective of this work is to propose new axes of health prevention destined for these providers of care, whose positive effects will contribute to consolidate support for the elderly. We conducted a thorough analysis on 32 interviews of caregivers in the service provider sector. The results confirm a sensitive health situation. The values associated with care and social work compel prevention, by erasing oneself linked to caring perceived as being in priority beneficial to others who are dependent. This observation led to integrate a new dimension in the prevention models of health, the ability to "self-care",requiring learning and forming the determinant of the propensity to preventive behavior, which to date is little implemented by the caregivers.

SELF-CARE: A CLARIFICATION OF MEANING AND EXAMINATION OF SUPPORTIVE STRATEGIES

Background Considering ~9 million Canadians have one or more chronic health conditions, and >3.3 million report some level of disability, the burden of care is substantive for individuals and health system. With such conditions, self-care is essential but may pose challenges to both individuals and providers of care. As a concept, self-care is poorly understood. Further, evidence for effective self-care support is diffuse and typically studied relative to specific conditions. Objectives To investigate the concept of self-care three objectives were undertaken: 1) explore and describe the construct of self-care as understood by individuals/families, health care professionals, researchers, policy-makers; and industry; 2) produce new knowledge for health care professionals about interventions for self-care across a range of population groups; 3) develop a provisional framework to inform practice and research. Method A multi-phase enquiry was undertaken. Phase 1 Concept clarification including: 1) synthesis of qualitative evidence on the experience of self-care reported by individuals/families; 2) content analysis and definitional study of the meaning of self-care; 3) concept analysis of self-care; 4) creation of a conceptual schema encompassing these perspectives. Phase 2 Self-care Interventions: Integrative study of systematic reviews, synthesizing evidence for self-care interventions from multiple disease/impairment groupings. Results Three modes of self-care were revealed : ‘Care of self’ self-care performed on one’s own behalf; ‘care by other’ acknowledging individuals with disabilities who guide and direct care provided by another person; and ‘care of other’- care of families and others at a community level. Analysis of 30 self-care interventions across 16 conditions demonstrated that educational sessions and self-care management plans are emerging as effective strategies to support and guide self-care. Conclusion Self-care is a complex care concept that is becoming an expected element in today’s health care environment. A full understanding of how it is viewed, including the individual’s perspective, is vital for enactment and beneficial support.
