Redução do uso e do consumo de açúcar por merendeiras de escolas públicas : ensaio randomizado por conglomerado / Reducing the use and consumption of sugar by school lunch cooks in public schools: a cluster randomized trial

Conselho Nacional de Desenvolvimento Científico e Tecnológico / O Brasil é um dos maiores consumidores per capita de açúcar e estudos têm mostrado um papel específico do consumo excessivo de açúcar no ganho de peso. Com o aumento do ganho de peso observado em vários países, e também no Brasil, é importante testar quais mensagens, estratégias e propostas de intervenção seriam eficazes na prevenção dessa epidemia. Os dados reportados são referentes a um ensaio randomizado por conglomerado, controlado, conduzido em 20 escolas municipais na cidade metropolitana de Niterói no Estado de Rio de Janeiro, de março a dezembro de 2007, que testou a eficácia de orientações para merendeiras objetivando reduzir a disponibilidade de açúcar e de alimentos fontes de açúcar na alimentação escolar e no consumo delas. A intervenção consistiu em um programa de educação nutricional nas escolas usando mensagens, atividades e material educativo que encorajassem a redução da adição de açúcar na alimentação escolar pelas merendeiras e no consumo delas. A redução da disponibilidade per capita de açúcar pelas escolas foi analisada através de planilhas com dados da utilização dos itens do estoque. O consumo individual das merendeiras foi avaliado através de questionário de freqüência de consumo alimentar. As medidas antropométricas e bioquímicas foram realizadas de acordo com técnicas padronizadas. As escolas de intervenção apresentaram maior redução da disponibilidade per capita de açúcar quando comparadas às escolas controle (-6,0 kg vs. 3,4 kg), mas sem diferença estatisticamente significante. Houve redução no consumo de doces e bebidas açucaradas nas merendeiras dos dois grupos, mas o consumo de açúcar não apresentou diferenças estatisticamente significativas entre eles. Houve redução do consumo de energia total nos dois grupos, mas sem diferença entre eles, e sem modificação dos percentuais de adequação dos macronutrientes em relação ao consumo de energia. Ao final do estudo somente as merendeiras do grupo de intervenção conseguiram manter a perda de peso, porém sem diferença estatisticamente significante. A estratégia de redução da disponibilidade e do consumo de açúcar por merendeiras de escolas públicas não atingiu o principal objetivo de redução de adição de açúcar. Uma análise secundária dos dados avaliou a associação entre a auto-percepção da saúde e da qualidade da alimentação com o excesso de peso e concentração elevada de colesterol sérico das merendeiras na linha de base. As perguntas de auto-percepção foram coletadas por entrevista. Dentre as que consideraram a sua alimentação como saudável, 40% apresentavam colesterol elevado e 61% apresentavam excesso de peso vs. 68% e 74%, respectivamente, para as que consideraram a sua alimentação como não-saudável. Dentre as que consideraram a sua saúde como boa, 41% apresentavam colesterol elevado e 59% apresentavam excesso de peso vs. 71% e 81%, respectivamente, para as que consideraram a sua saúde como ruim. A maioria das mulheres que relatou ter alimentação saudável apresentou maior frequência de consumo de frutas, verduras e legumes, feijão, leite e derivados e menor freqüência de consumo de refrigerante. Conclui-se que perguntas únicas e simples como as utilizadas para a auto-avaliação da saúde podem também ter importância na avaliação da alimentação. / Brazil is one of the largest per capita consumers of sugar and several studies have shown a specific role of excessive consumption of sugar on weight gain. With the increased weight gain observed in several countries, including Brazil, it is important to test which messages, strategies and proposals for intervention would be effective in preventing this epidemic. The data reported are for an intervention study that tested the efficacy of guidelines for school lunch cooks aiming to reduce the added sugar in schools meals and their sugar intake. A cluster randomized controlled trial was carried out in twenty public schools in the metropolitan city of Niterói in Rio de Janeiro, Brazil, from March to December 2007, to assess the change in the availability and consumption of sugar. The intervention consisted of a nutrition educational program in schools using messages, activities and printed educational materials that encouraged the reduction of added sugar in the schools meals by the school lunch cooks and in their consumption. The reduction in per capita sugar availability by the schools was examined through spreadsheets with data from the use of inventory items. Individual food intake of the school lunch cooks was evaluated by a Food Frequency Questionnaire. Anthropometric and biochemical measurements were performed according to standard techniques and the variation in weight change was measured throughout the study. Per capita sugar availability reduced most markedly in the intervention schools compared to the control schools (-6,0 kg vs. 3,4 kg), however this difference was not statistically significant. Both groups of school lunch cooks showed a reduction in the consumption of sweets and sweetened beverages, but the difference in sugar intake was not statistically significant. A reduction in total energy consumption was observed in both groups, but there was no difference between them. Also, there was no difference in the percentage of adequacy of nutrients in relation to energy consumption. Sweetened beverages presented the most important consumption reduction. At the end of the study, only school lunch cooks in the intervention group were able to maintain weight loss, but not statistically significant. The strategy of reducing the availability and consumption of sugar by the school lunch cooks from public schools did not achieve the main goal of reducing added sugar. A secondary analysis examined the association between self-perceived health status and diet quality with overweight and high serum cholesterol concentration of the school lunch cooks at baseline. The self-perception questions were collected by interview. Among women who reported healthy diet, 40% presented high serum cholesterol and 61% were overweight. Among women who reported unhealthy diet, 68% presented high serum cholesterol and 74% presented overweight. Most women who reported healthy diet showed a higher frequency of consumption of sweets, fruits, vegetables, beans, dairy products and lower frequency of consumption of soft drink. In conclusion, single and simple questions, such as those used for self-perceived health status may also be important in assessing the diet.

Moreover the use of industrial co-products reduces the amount of residue formation. Therefore the purpose of this study was to elaborate fermented lactose beverages with added red rice extract (RRE) and lactic serum, a dairy co-product, in order to enrich the developed lactose beverage. Not only this is a low cost drink, but it may also contribute to the health of people who have celiac disease or milk allergy, due to the use of rice and the low levels of milk. Pasteurized milk, fresh cheese serum and natural yogurt were used. To obtain the RRE, rice grits were blended with water addition, in a 5:1 ratio. It was heated at 85 °C and blended for 2 min. The RRE was used at the following percentage 6, 10, 20, 30, and 34 %, and the lactic serum in substitution of the lactose base (LS) at 26, 30, 40, 50, and 54 %. We added 10 % sacarose and 0.3 % GFG stabiliser to the mix. After these beverages reached the temperature of 45 °C, they received 2.5 % of inoculum, and were left in thermic isolation for 5 h. Then they were cooled at 8 °C for 10 h. After this period, the samples were homogenized and stored at a 5 °C environment. The Central Rotational Composite Layout was employed, and 11 lactose fermented drink treatments were elaborated, in other words, a 22 complete factorial, including four axial points and three repetitions in the central point. Physical and chemical tests were performed (pH and titratable acidity) in the raw material in order to verify milk and serum quality , as well as in the fermented lactose beverages, soon after its elaboration and after seven, 14, 21, 28, 35, 42, 49, and 56 days at 5 °C. We performed microbiologic analyses (qualitative determination of Salmonella sp., total coliforms, coliforms at 45 °C, and the coagulase positive Staphylococci count), and centesimal mineral composition determination and syneresis; besides the apparent viscosity and the sensorial analyses test (preference and acceptability). The beverages developed were characterised for having a protein level of > 1.98 g (100 g)-1 higher than the minimum recommended by the current legislation, since being partially skimmed < 1.77 g (100 g)-1 and for having levels of lactose that are tolerated by people who have bad absorption (< 6.19 g (100 g)-1). Considerable values of Zn, Mn and Cu were obtained, levels similar to other K and Fe lactose beverages, and low levels of Na, Mg and Ca. They showed great hygienic sanitary levels and a viable level of lactose bacteria superior to 109 UFC mL-1 during storage. The acceptability was higher than 80 % amongst potential consumers, with a higher acceptability rate of beverages that contained 6 to 20 % of RRE and between 26 to 40 % of LS. This result was compatible to the viscosity and syneresis. A 28 day life span is surmised, a satisfactory span, considering the fact that no conservatives were added to the product. / O desenvolvimento de bebidas lácteas adicionadas de extrato de arroz diversifica o mercado de lácteos. E a utilização de coprodutos da indústria minimiza a geração de resíduos. Por esse motivo, teve-se como objetivo neste estudo, elaborar bebidas lácteas fermentadas com adição de extrato de arroz vermelho (EAV) e soro lácteo, coproduto de laticínios, para enriquecer a bebida láctea desenvolvida. Esta bebida além do baixo custo, possivelmente irá contribuir com a saúde de portadores de doença celíaca ou alérgicas ao leite, devido a utilização de arroz e ao reduzido teor de leite utilizado. Utilizou-se leite integral pasteurizado, soro de queijo Minas Frescal e iogurte natural. Para a obtenção do EAV, triturou-se o arroz vermelho em liquidificador e adicionou-se água, na proporção de 5:1. Aqueceu-se a 85 °C e bateu-se por 2 min. O EAV foi utilizado nas proporções (6, 10, 20, 30 e 34 %), e o soro lácteo em substituição na base láctea (SL) (26, 30, 40, 50 e 54 %). Adicionaram-se 10 % de sacarose e 0,3 % de estabilizante GFG às misturas. Estas bebidas, após atingir a temperatura de 45 °C, receberam 2,5 % do inóculo, deixando-se em isolamento térmico por 5h. Em seguida, refrigerou-se a 8 °C por 10h. Após esse período, as amostras foram homogeneizadas e armazenadas sob refrigeração a 5 °C. O delineamento experimental foi o Delineamento Composto Central Rotacional, sendo elaborados 11 tratamentos de bebidas lácteas fermentadas, ou seja, um fatorial completo 22, incluindo 4 pontos axiais e 3 repetições no ponto central. Foram realizados ensaios físico-químicos (pH e acidez titulável) na matéria prima, com a finalidade de verificar a qualidade do leite e soro utilizados, assim como nas bebidas lácteas fermentadas, logo após sua elaboração e após 7, 14, 21, 28, 35, 42, 49 e 56 dias de armazenamento a 5 °C. Foram feitas análises microbiológicas (determinação qualitativa de Salmonella sp., coliformes totais, coliformes a 45 °C, e contagem de Estafilococos coagulase positiva), determinação de composição centesimal, de minerais e sinérese; além dos testes de viscosidade aparente e também análise sensorial (preferência e aceitação). As bebidas desenvolvidas caracterizam-se por ter teores de proteínas > 1,98 g (100 g)-1 superiores ao mínimo preconizado pela legislação vigente, por serem parcialmente desnatadas < 1,77 g (100 g)-1 e por terem teores de lactose tolerados por pessoas que possuem má absorção (< 6,19 g (100 g)-1). Forneceu valores consideráveis de Zn, Mn e Cu, teores similares a outras bebidas lácteas de K e Fe, e baixo teor de Na, Mg e Ca. Mostraram-se ótima qualidade higiênicosanitária e presença de bactérias lácteas viáveis superiores a 109 UFC mL-1 durante o armazenamento. A aceitação foi superior a 80 % entre potenciais consumidores, com melhor aceitabilidade nas bebidas que continham entre 6 e 20 % de EAV e entre 26 a 40 % de SL. Este resultado foi compatível com a viscosidade e a sinérese. Supõem-se uma vida útil de até 28 dias, tempo satisfatório, considerando o fato de não ter adicionado conservante ao produto.

Produção e purificação da glicocerebrosidase humana recombinante expressa por célula CHO em meio livre de soro fetal bovino / Production and purification of recombinant human glucocerebrosidase expressed by CHO cells in serum free medium

A produção de proteínas recombinantes, principalmente para uso farmacêutico, tem sido intensamente estudada, juntamente com suas propriedades físico-químicas, o que possibilita uma melhor escolha da técnica de purificação, e assim, evitar perdas no rendimento e custos elevados. A glicocerebrosidase (GCR) é uma enzima lisossomal, e sua deficiência ocasiona um distúrbio autossômico recessivo denominado doença de Gaucher. Atualmente o tratamento para essa patologia é por meio da Terapia de Reposição Enzimática (TRE), a qual tem sido realizada com grande êxito. Tendo em vista fornecer dados que possam auxiliar na redução do número de etapas cromatográficas no processo de downstream, este trabalho teve como objetivo, a purificação da GCR expressa por células de Ovário de Hamster Chinês (CHO), em meio livre de soro fetal bovino (SFB). Para se alcançar tais resultados, realizou-se o cultivo das células CHO-GCR em meio quimicamente definido livre de SFB (CHO-S-SFM II). Foram realizadas três técnicas cromatográficas (troca iônica, interação hidrofóbica e afinidade), com base em suas propriedades físico-químicas. E para o ensaio da atividade enzimática, foi utilizado o substrato fluorogênico 4-Methylumbeliferyl &#946;-D-glucopyranoside (4MU-G). As células CHO-GCR cultivadas em meio CHO-S-SFM II, apresentando viabilidade maior que 95%, e produção da GCR ativa, durante todo o período de cultivo. Os protocolos aplicados para a purificação da GCR, não apresentaram resultados significativos. Com o volume não retido após cromatografia por interação hidrofóbica, se estimou os valores de KM 2,13 e VMAX 0,0295 UFR/h para as constantes cinéticas da GCR. A diálise no processo de purificação mostrou ser uma etapa necessária para a atividade enzimática da GCR. No cultivo das células CHO-GCR para a formação do banco de trabalho, nos meios RPMI 1640 e &#945;-MEM, ambos com a adição de 10% SFB, não houve diferença significativa no crescimento entre eles, e apresentaram 100% de viabilidade durante todo o período de cultivo. Porém, ao analisar de forma isolada a fase exponencial de cada curva, notou-se que às células cultivadas no meio RPMI 1640, apresentaram taxa de crescimento superior, às cultivadas em meio &#945;-MEM. Concluiu-se que a expressão da GCR em meio livre de SFB, proporciona amostras menos complexas, em relação aos meios de cultura que necessitam de suplementação com SFB, o que pode reduzir o número de etapas cromatográficas, melhorando o rendimento e a redução da perda da atividade da GCR. O meio basal RPMI 1640 com a adição de SFB foi uma alternativa satisfatória para o cultivo das células CHO-GCR. Este estudo forneceu dados que podem contribuir para a melhoria do processo de purificação da GCR. Novas pesquisas podem ser desenvolvidas a fim de melhorar o processo de purificação da GCR. / The production of recombinant proteins, mainly for pharmaceutical use, has been intensively studied along with its physico-chemical properties, which allows a better choice of the purification technique, and thus avoid losses in yield and high costs. Glucocerebrosidase (GCR) is a lysosomal enzyme, and its deficiency causes an autosomal recessive disorder called Gaucher\'s disease. Currently the treatment for this pathology is through Enzymatic Replacement Therapy (ERT), which has been successfully performed. In order to provide data that may help reducing the number of chromatographic steps in the downstream process, this work aimed to purify GCR expressed by Chinese Hamster Ovary (CHO) cells in fetal bovine serum free (FBS). To achieve such results, the CHO-GCR cells were cultured in chemically defined Serum-free medium (CHO-S-SFM II). Three chromatographic techniques (ion exchange, hydrophobic interaction and affinity) were performed, based on their physicochemical properties. And for the enzymatic activity assay, the fluorogenic substrate 4-Methylumbeliferyl &#946;-D-glucopyranoside (4MU-G) was used. CHO-GCR cells cultured in CHO-S-SFM II medium, presenting viability greater than 95% and GCR production active, throughout the culture period. The protocols applied for GCR purification did not present significant results. With the volume not retained after chromatography by hydrophobic interaction, KM values of 2.13 and VMAX 0.0295 UFR/h were estimated for GCR kinetic constants. Dialysis in the purification process was shown to be a step necessary for the enzymatic activity of GCR. In the culture of the CHO-GCR cells for the formation of the working bank, in the media RPMI 1640 and &#945;-MEM, both with the addition of 10% FBS, there was no significant growth difference between them, and they showed 100% viability during all the growing period. However, when analyzing in isolation, the exponential phase of each curve, it was observed that the cells grown in RPMI 1640 medium showed higher growth rates, tham grown in &#945;-MEM medium. It was concluded that the expression of GCR in serum-free medium provides less complex samples, relative to the culture media requiring FBS supplementation, which may reduce the number of chromatographic steps, improving yield and loss reduction of GCR activity. The basal medium RPMI 1640 with the addition of FBS was a satisfactory alternative for culturing the CHO-GCR cells. This study provided data that may contribute to the improvement of the GCR purification process. New research can be developed to improve the GCR purification process.

Obesity leads to a reduction in life expectancy through its association with chronic diseases of lifestyle. The prevalence of obesity is rapidly increasing throughout the world. It is now accepted that most cases of obesity result from an interaction between genetic and environmental factors. This rapid increase in obesity generally leads to an increase in morbidity and mortality from chronic diseases such as cardiovascular disease, type 2 diabetes, osteoarthritis and cancer of which obesity is a risk factor. There is a lack of information in molecular research to explain how obesity predisposes individuals to these diseases. Proteomics is a molecular tool and a set of techniques used to identify changes at protein level from a diseased state. This study aims to identify differentially expressed proteins in serum of obese rats fed different isocaloric diets using proteomics.

Desenvolvimento de metodologia para produção de soro AB humano para suplementação de meio de cultura destinado ao cultivo de células mesenquimais / Development of methodology for human AB serum production for culture medium supplementation for the cultivation of mesenchymal cells

O crescente número de aplicações clínicas envolvendo células mesenquimais estromais multipotentes (CMMs) gera a necessidade da produção em larga escala destas células com adequada qualidade terapêutica. As CMMs são geradas atualmente através de culturas aderentes na presença de soro fetal bovino (SFB). Entretanto, apesar da eficiência na promoção do crescimento celular, o uso de SFB não é isento de desvantagens e riscos. Além do alto custo, sua utilização pode gerar risco de contaminação do produto final com agentes adventícios como vírus e príons. Por outro lado, a sua variabilidade em diferentes lotes e fornecedores dificulta a padronização do meio e reprodutibilidade do cultivo. Uma alternativa promissora para a cultura de células destinadas a terapia celular é a substituição de SFB por soro AB humano obtido a partir de plasma humano. Neste cenário, o objetivo deste trabalho foi produzir soro AB humano e avaliar a sua qualidade e eficácia como substituto do SFB na produção de células mesenquimais destinadas à terapia celular. Para a produção do soro AB humano foram avaliados tanto Plasma comum (PC>24h) (n=3) quanto Plasma isento de crioprecipitado (PCIC) (n=3). Após a produção, foi realizado o controle de qualidade dos lotes produzidos sendo verificado que os mesmos atenderam as exigências necessárias para a utilização na terapia celular. As duas fontes de plasma utilizadas para a produção do soro AB humano apresentaram características semelhantes quanto aos constituintes bioquímicos e demais parâmetros analíticos e foram eficazes na suplementação do cultivo e expansão das CMMs. A suplementação do meio com 10% de soro AB humano foi eficaz tanto no cultivo em culturas estáticas quanto em microcarregadores. Esta característica alinhase com a possibilidade de produção em larga escala, uma vez que, permitiu a expansão das CMMs de maneira similar à condição controle (suplementada com 10% de SFB) e preservou as características imunofenotípicas, funcionais e perfil citogenético pós cultivo. Além disso, o soro AB humano produzido pode ser utilizado por no mínimo doze meses após produção, quando armazenado em temperatura inferior a 20ºC negativos / The growing number of clinical applications involving multipotent mesenchymal stromal cells (MSCs) generates the need for large-scale production of these cells with appropriate treatment quality. The MSCs are now generated through adherent cultures in the presence of fetal bovine serum (FBS). However, despite the efficiency of this method, the use of FBS is not exempt of drawbacks and hazards. Besides the high cost, their use can lead to the risk of final product contamination with adventitious agents such as viruses and prions. In addition, due to its high variability of reproducibility, this methodology requires high level of standardization. A promising alternative would be to replace FBS by human AB serum from human plasma. In this scenario, our objective was to produce human AB serum and evaluate their quality and effectiveness as a FBS substitute on production of mesenchymal cells for therapy. For the production of human AB serum were evaluated both common plasma (CP > 24h; n=3) and plasma cryoprecipitate depleted (PCD; n=3). Quality controls were carried out to stablish minimal requirements of quality to be used in a cell therapy scenario. Both plasma sources of human AB serum presented similar biochemical characteristics and other analytical parameters, being effective in supplementation of the culture and expansion of MSCs. Supplementing the culture medium with 10% human AB serum was effective both in cultivation of static cultures as for microcarriers. This condition might indicate that this method would be useful for large scale production, allowing the expansion of MSCs similarly to the control condition and maintaining immunophenotypic, functional and cytogenetic characteristics. Furthermore, the human AB serum produced can be used for at least twelve months after production when stored at temperature below 20ºC negative

O objetivo deste trabalho foi comparar os efeitos sorológicos e clínicos de dois protocolos de terapias periodontais em indivíduos com diabetes tipo 2 (DMT2) e periodontite crônica. Foram analisados 36 pacientes, randomizados em dois grupos: um grupo recebeu terapia intensiva de raspagem e alisamento radicular (INT; n=18) e outro recebeu apenas raspagem supragengival (SUP; n=18). Os grupos foram avaliados quanto aos parâmetros clínicos periodontais e marcadores inflamatórios séricos, antes e após 6 meses do tratamento periodontal. O exame clínico periodontal avaliou: placa visível (IP), índice gengival (IG), supuração (SUPUR), profundidade clínica de sondagem (PCS) e nível clínico de inserção (NCI). Amostras sanguíneas foram obtidas para análise de marcadores inflamatórios e hemoglobina glicada (HbA1c). Os marcadores de inflamação avaliados foram: interleucina (IL)-1, IL-2, IL-4, IL-5, IL-6, IL-7, IL-8, IL-10, IL-12, IL-13, IL-17, fator estimulador de colônias granulocitárias (G-CSF), fator estimulador de colônias de granulócitos-macrófagos (GM-CSF), interferon- (IFN-), proteína quimiotática de monócito-1 (MCP-1), proteína inflamatória de macrófago-1 (MIP-1) e fator de necrose tumoral (TNF-), através do imunoensaio multiplex (Bioplex). Ambas as terapias resultaram na melhora de quase todos os parâmetros clínicos periodontais (p<0,05), com exceção do NCI (p=0,09) no grupo SUP. Não houve diferença significativa para os níveis de IL-1, IL-4, IL-5, IL-10, IL-13, MIP-1 e TNF- (p>0,05), após tratamento, em ambas as terapias. Houve redução significativa de IL-6 (p=0,01), IL-12 (p=0,04) e MCP-1 (p=0,02) no grupo INT e de GCS-F nos grupos SUP (p=0,04) e INT (p=0,01). Os níveis de IL-2, IL-7, IL-8, IL-17, GM-CSF e IFN- não foram detectados. A terapia INT tem um efeito benéfico na redução dos níveis séricos de IL-6, IL-12 e MCP-1, na redução de PCS em sítios profundos e no ganho de inserção quando comparado à terapia supragengival em um período de 6 meses. / The objective of this study was to compare the serological and clinical effects of two periodontal therapies in individuals with type 2 diabetes (T2DM) and chronic periodontitis. 36 patients were analyzed, randomized into two groups: one group received intensive t of scaling and root planing (INT, n=18) and another received only supragingival scaling (SUP, n=18). The groups were evaluated for periodontal parameters and serum inflammatory markers before and after 6 months of periodontal treatment. The periodontal parameters assessed were: visible plaque (PI), gingival index (GI), suppuration (SUP), probing pocket depth (PPD) and clinical attachment level (CAL). Blood samples were obtained for analysis of inflammatory markers and glycated hemoglobin (HbA1c). The inflammatory markers evaluated were: interleukin (IL)- 1 , IL- 2, IL -4, IL -5, IL -6, IL -7 , IL-8 , IL-10 , IL-12 , IL-13 , IL -17, granulocyte colony-stimulated factor (G -CSF), colony stimulating factor granulocyte-macrophage (GM -CSF), interferon - (IFN - ), monocyte chemotactic protein-1 (MCP-1) , macrophage inflammatory protein-1 (MIP-1 ) and tumor necrosis factor (TNF- ) through a multiplex immunoassay (Bioplex). Both therapies resulted in improvement of almost all periodontal clinical parameters (p<0.05), with the exception of the NCI in SUP group. There was no significant difference for (IL )- 1 , IL- 4, IL -5, IL-10 , IL-13 , MIP- 1 and TNF- after treatment for both therapies (p>0.05). A significant reduction was observed in IL-6 (p=0.01), IL-12 (p=0.04) and MCP-1 (p=0.02) levels for INT group and in GCS- F levels for SUP (p=0.04) and INT (p=0.01) groups. The levels of IL-2, IL -7, IL-8 , IL-17, GM- CSF and IFN- were not detectable. The INT therapy has a beneficial effect in reducing serum levels of IL-6, IL-12 and MCP-1, in reducing PPD of deep sites and in attachment gain when compared to SUP therapy considering a period of 6 months.

INTRODUÇÃO: a albumina sérica reduzida em pacientes em diálise peritoneal (DP) é associada à aterosclerose, causa de morte mais comum entre esses pacientes. Semelhantemente à síndrome nefrótica, supõe-se que a perda de proteínas conjuntamente a de fatores de regulação da hemostasia leva ao estímulo da síntese hepática de fatores pró-coagulantes, como o fibrinogênio, deslocando o equilíbrio hemostático em direção ao estado pró-trombótico. Pacientes em DP apresentam valores séricos elevados de marcadores da ativação endotelial e fatores pró-coagulantes, quando comparados a pacientes em hemodiálise (HD). A tromboelastografia (TEG) é um método que avalia propriedades do sangue global e dinâmica da coagulação, fornecendo, por meio de um traçado, valores absolutos do tempo de formação de fibrina (K), a agregação plaquetária (amplitude máxima - AM), a firmeza do coágulo (G), entre outros dados. Por final, classifica a coagulação em normal, hipocoagulante ou hipercoagulante segundo o índice de coagulação (IC) apresentado, sendo assim útil no diagnóstico precoce de coagulopatias. Por ser pouco utilizado em pacientes com DRC, utilizou-se o TEG na avaliação da hemostasia dos pacientes em diálise peritoneal automatizada (DPA) e investigou-se a relação com a perda de proteínas, bem como outras condições clínicas inerentes ao tratamento dialítico. MÉTODOS: este estudo foi do tipo transversal que incluiu pacientes estáveis em DPA. Foram obtidos dados demográficos, clínicos e bioquímicos de rotina do prontuário médico eletrônico. Adicionalmente, foram avaliados a coagulometria, a hemostasia primária [antitrombina (AT), proteína S, fator VIII (FVIII), fator IX (FIX), fator V (FV), fibrinogênio e dímero-D] e o TEG. Os pacientes foram submetidos ao teste de equilíbrio peritoneal (PET), a avaliação da perda de proteínas para a solução de diálise (PSD) e a absorção de glicose. O estado nutricional dos pacientes foi avaliado por meio de métodos objetivos e subjetivos. RESULTADOS: vinte pacientes (38±16 anos de idade, 55% mulheres, 22,4±14,8 meses em DPA, 40% de glomerulopatia, 70% transportadores médio lento/lento e em bom estado nutricional) foram incluídos no estudo. O FVIII e FIX elevados em 85% e 50% da amostra, respectivamente. O fibrinogênio (553,8±100,5 mg/dL) e o dímero-D (720 (520-1940) ug/L) foram elevados em mais da metade dos pacientes. O TEG revelou 55% dos pacientes hipercoagulantes, 45%, normais, e nenhum era hipocoagulante. Os pacientes hipercoagulantes foram caracterizados por um tempo K menor (1,3±0,4 vs. 1,8±0,3 minutos, p=0,007); AM (72,1±2,4 vs. 64,7±3,6 mm, p=0,000) e G (13,1±1,6 vs. 9,3±1,5 K, p= p=0,000) elevados, também alterados em 78% e 33%, respectivamente, nos pacientes com coagulação normal. Pacientes hipercoagulantes também apresentaram maiores valores de plaquetas (251±28 vs. 214±51 mil/mm³, p=0,038), que se correlacionou positivamente com AM/G (r=0,594, p=0,006), enquanto a proteína C foi menor (108±12 vs. 117±20 %, p=0,034) e a AT se correlacionou positivamente com o tempo K (r=0,635, p=0,011). Não houve diferença de albumina sérica, PSD, cinética de creatinina e estado nutricional entre os grupos normal e hipercoagulante. Os pacientes hipercoagulantes, entretanto, apresentaram menores valores de hemoglobina (10,3±1,4 g/dL vs. 12,0±1,1 g/dL; p=0,007), que se correlacionou negativamente com AM/G (r=-0,673, p=0,001), bem como o hematócrito (31±4 % vs. 36±3 %; p=0,010), que também se correlacionou negativamente com AM/G (r=-0,640; p=0,002). CONCLUSÃO: demonstramos que pacientes em DPA estáveis apresentaram uma tendência pró-trombótica caracterizada pela hiperfunção plaquetária e maior força de coágulo. Mesmo não havendo linearidade na relação com a hemostasia a perda de proteínas pode ter contribuído para a hipercoagulabilidade nesses pacientes. Entretanto, os eritrócitos reduzidos representaram um fator de confusão para o resultado do / INTRODUCTION: reduced serum albumin in patients on peritoneal dialysis (PD) is associated to atherosclerosis that is the leading cause of death. Similarly to nephrotic syndrome they lose protein; it is assumed that together with regulating factors of haemostasis this loss leads to liver synthesizes of procoagulants factors, such as fibrinogen, shifting the hemostatic equilibrium to a prothrombotic state. Patients on PD present elevated serum markers of endothelial activation and coagulant factors when compared with hemodialysis (HD) patients. Thromboelastography (TEG) is a method that evaluate blood properties through coagulation\'s global and dynamic perspectives, providing through a trace absolute values of fibrin\'s time formation (K), platelet aggregation (maximum amplitude - MA), clot strength (G), among other data. Finally it classifies the coagulation in normal, hypocoagulant or hypercoagulant according to the coagulation index (CI), so that it is useful in the early diagnosis of coagulopathies. TEG is not often used in patients with CKD, because of this we chose to use TEG for hemostatic evaluation in patients on APD and investigated its relation with protein loss as well as other clinical conditions intrinsic to the dialytic therapy. METHODS: this was a cross-sectional study that included stable patients on automated peritoneal dialysis (APD). Demographic, clinical and routine biochemical data were obtained from electronic medical chart. Additionally the coagulometry, primary hemostasis [antithrombin (AT), protein S, factor VIII (FVIII), factor IX (FIX), factor V (FV), fibrinogen and D-dimer] and TEG were evaluated. Patients were submitted to peritoneal equilibrium test (PET), protein loss to dialysis solution (PDS) and glucose absorption evaluations. Patients nutritional status was evaluated by objective and subjective methods. RESULTS: twenty patients (38±16 years old, 55% women, 22.4±14.8 months on APD, 40% of glomerulopathy, 70% slow average/slow transporters and well nourished) were included in this study. FVIII and FIX were elevated in 85% and 50% of the sample, respectively. Fibrinogen (553.8±100.5 mg/dL) and D-dimer (720 (520-1940) ug/L) were elevated in over half of the patients. TEG showed 55% of the patients hypercoagulant, 45% were normal and nobody was hypocoagulant. Hypercoagulant patients were characterized by a lower K-time (1.3±0.4 vs. 1.8±0.3 minutes; p=0.007); elevated MA (72.1±2.4 vs. 64.7±3.6 mm; p=0.000) and G (13.1±1.6 vs. 9.3±1.5 K; p= p=0.000); altered too in 78% and 33%, respectively, in normal coagulation patients. Hypercoagulant patients presented too higher values of platelet count (251±28 vs. 214±51 mil/mm³, p=0,038), but within the normal range, that correlated positively with MA/G (r=0.594; p=0.006) while protein C was lower (108±12 vs. 117±20 %; p=0,034) and AT correlated positively with K-time (r=0.635; p=0.011). There was no difference to serum albumin, PDS, creatinin kinetic and nutritional status between hypercoagulant and normal groups. However hypercoagulant patients presented lower values of hemoglobin (10.3±1.4 g/dL vs. 12.0±1.1 g/dL; p=0.007); that correlated negatively with MA/G (r=-0.673; p=0.001), as well as hematocrit (31±4 % vs. 36±3 %; p=0,010), which also correlated negatively with MA/G (r=-0.640; p=0.002). CONCLUSION: we demonstrated that stable patients on APD presented a prothrombotic tendency characterized by platelet hyperfuntion and clot strength. Even though there was no linearity in relation to hemostasis; protein loss may have contributed to the hypercoagulability in these patients. However reduced erythrocytes were a confounding factor in the TEG analysis

Vitamin D Status and Demographic and Lifestyle Determinants Among Adults in the United States (NHANES 2001-2006)

This study looked at risk factors associated with vitamin D levels in the body among a representative sample of adults in the U.S., NHANES III (2001-2006) data were used to assess the relationship between several demographic and health risk factors and vitamin D levels in the body. The Baseline-Category Logit Model was used to test the association between vitamin D level and the potential risk factors age, education, ethnicity, poverty status, physical activity, smoking, alcohol, obesity, diabetes and total cholesterol with both genders. Vitamin D insufficiency and deficiency were significantly associated with age, race, education, physical activity, obesity, diabetes and total cholesterol level for both genders. Almost half of the adults sampled in these data had vitamin D levels lower than the recommended limits, with the highest frequency among the younger groups. Determining an individual’s vitamin D level is very difficult without proper clinical testing. Many of those who have low vitamin D levels are unaware. With such a high prevalence of individuals with low vitamin D levels in the U.S. and a better understanding of characteristics associated with these lower levels, increased education and prevention efforts should be focused toward those with higher risk characteristics.

Particle and macromolecular fouling in submerged membrane

Particles and macromolecular components, including biopolymers (protein and carbohydrate), are viewed as the main foulants in the complex feed submerged membrane filtration systems such as membrane bioreactor (MBR). This work focused on two aspects of fouling in complex fluids: 1- Assessing fouling propensity and mechanisms for various model solutions. 2- Using of two specific solutions modelling biomass found in MBR for a better understanding of the fouling mechanisms in submerged MBR processes. Filtrations were carried out with 0.22 ??m PVDF hollow fibre membrane. Alginate was used as a model for polysaccharide, bovine serum albumin (BSA) as a model for protein, (un)washed yeast and bentonite were representing suspended solid contents. According to the data obtained during this study the fouling propensity of each model solution was classified as follow in a decreasing order: Alginate &gt unwashed yeast &gt washed yeast &gt BSA &gt bentonite for one-component solutions; and Alginate-washed yeast &gt Alginate-BSA &gt Alginate-bentonite &gt Alginate-unwashed yeast for two-component solutions. Introducing the alginate increased the reversible fouling (except BSA). Passive adsorption had a significant effect on fouling of alginate even before the beginning of the filtration. Washed yeast and a mixture of washed yeast + BSA were then used as model solutions to simulate the activated sludge found in MBR. The concentration of washed yeast and BSA used in this study were calculated in order for the characterisations of the two model solution to match (in terms of biopolymer contents) those of MBR biomasses reported in the literature. By rinsing, backwashing and chemical cleaning of the membrane, three fouling layers of upper, intermediate and lower were defined respectively. Results obtained from the analysis of the biopolymers found in the cleaning solutions allow a better understanding of the fouling mechanisms occurring for the two model solutions used in this study: for washed yeast, the lower layer and for washed yeast + BSA , the upper and intermediate layers were found to have relatively high biopolymeric composition. This was explained by higher concentration of solids on the membrane surface and by higher biopolymer interactions when washed yeast was mixed with BSA.

<p>In this thesis, the structure and stability of globular proteins adsorbed onto nanometer-sized hydrophilic silica particles were investigated using differential scanning calorimetry (DSC), hydrogen/deuterium exchange (HDX), and mass spectrometry (MS). The adsorption process itself was characterized with fluorescence and absorption spectroscopy and surface plasmon resonance (SPR). The combination of these methods offered a unique insight into adsorption-induced changes within proteins related to their adsorption characteristics. DSC contributed with thermodynamic information on the overall structural stability within the protein population. HDX in combination with MS contributed information on the structure and stability of adsorbed proteins with focus on changes within the secondary structure elements. In order to increase the structural resolution in this part of the investigation, proteolysis was performed prior to the MS analyzing step. Knowledge on the protein adsorption process was utilized in a practical approach called ligand fishing. In this approach, SPR was used to monitor the chip-based affinity purification of a protein with MS used for protein identification.</p><p>Adsorption isotherms revealed that electrostatic interactions play an important role in the adsorption of proteins to hydrophilic surfaces. DSC investigation revealed that the thermal stability of proteins reduces with increasing electrostatic attraction between the protein and the surface and that this effect diminishes at higher surface coverage. The mass-increase due to exchange between protein hydrogen atoms and deuterium atoms in solution was investigated as a function of time. This gave insight into adsorption-induced changes in the structural stability of proteins. By combining DSC and HDX-MS, it was possible to differentiate between adsorption-induced changes in the secondary and tertiary structure. Additionally, if limited proteolysis was performed, the investigations gave insight into the orientation and protein segment specific changes in the stability of proteins adsorbed to silica surfaces. The adsorption of proteins to silica particles also provided the basis for a new experimental design that allows handling of minute amounts of proteins in a ligand fishing application, as used in the field of functional proteomics.</p>
