Mandatory Disease Notification and Underascertainment: A Geographical Perspective

Mandatory notification of disease forms the backbone of disease surveillance in New Zealand and overseas. Notification data is used by public health professionals and academics to identify cases requiring public health control, monitor disease incidence and distribution, and in epidemiological research. However, there is emerging evidence that notification rates do not accurately reflect the true extent of notifiable diseases within the community, resulting in the underascertainment of many notifiable cases. While adequate surveillance does not necessarily require that all cases of notifiable disease be captured, the systematic underascertainment of disease can have significant implications for perceived spatial and demographic trends in disease prevalence; potentially threatening the credibility of spatial epidemiological research by under or overestimating the burden of disease in different populations. There is evidence that systematic underascertainment occurs as a result of the differential actions of laboratories and general practitioners. It has also been recognised that that underascertainment can be influenced by a patient's willingness to seek medical attention and participate in laboratory tests. However, few studies have investigated whether these factors systematically influence notification either in New Zealand or overseas. Furthermore, the discipline of health geography has been slow to engage with this topic of public health importance, despite the inherently spatial nature of the processes involved, and the close ties to the geographic literature on health service utilization and healthcare provision. This thesis explores the spatial and temporal variation in notification rates in New Zealand for the period 1997-2005 and the potential relationships between notification rates and different variables. Unlike many underascertainment studies, which have used individual data and capture-recapture methods, data constraints inspired a unique ecological approach to investigating the factors which may be associated with notification in New Zealand. Variables were divided into categories based on Anderson's behavioural model for healthcare utilization and the influence of these variables on notification was determined through multiple regression analyses. The main findings of this research indicate that in New Zealand notification rates have increased during the period 1997-2005 and that there is a north-south gradient in notifications, with substantially lower rates in the North Island than in the South Island. Furthermore, it is also evident that the variables associated with notification vary according to disease, spatial aggregation and spatial scale. Notification rates are significantly associated with a range of predisposing and enabling factors which might influence patient choice to consult for many frequently underascertained diseases. More variation in enteric diseases is explained by the independent variables analysed than the variation in non-enteric diseases. However, further research into these relationships, and underascertainment in general, is required before firm conclusions can be drawn.

在民國七十六年十一月二日政府正式開放大陸探親後,兩岸人民往來逐漸頻繁,隨著兩岸人民的交流,對一群隨國民政府來臺的老榮民而言,此再回故鄉距離國民政府撤退大陸已經近四十餘年。兩岸從一九七0年代末期以來,在經濟與社會各方面交流日趨密切與頻繁,在開放探親後,雖屬同文同種,但在兩岸間文化、生活、語言上之差異,兩岸通婚人數逐年增加,從以往的偷渡犯罪,漸變為合法入境而產生另一種犯罪模式的轉變,所衍生各種犯罪問題亦日趨增加,其對國內安全議題影響亦逐漸擴大。 當前影響國家安全之因素,主要有國際環境、兩岸關係與國內形勢等三個面向,除傳統性安全的武力威脅外,許多非傳統性威脅亦逐漸增加,例如:大陸地區人民偷渡來臺、假結婚真賣淫、人口販運、毒品、走私、兩岸組織犯罪、傳染病等問題,均實值得做深入探討與研究,這些問題均對國內社會環境造成變化,增加維持社會治安與社會秩序的內政成本。 全球化的趨勢,在不影響國家安全的前提下,建構兩岸合作打擊犯罪之機制,實刻不容緩,臺灣應儘速尋求兩岸合作之可行模式,本文共分為幾部分,先經由政府對大陸政策實施迄今與執行層面做檢視與探討為出發點,並輔以對探親、結婚及近來討論開放觀光等議題和相關法令做全盤、統整性之分析。再來就相關犯罪所衍生之問題,在移民機關及警察機關間之協調聯繫和在執法上遇到之困境做分析,最後,本研究就合法來臺之大陸人民犯罪問題為研究主軸,並就相關問題提出個人之看法與建議,希能提供就國家安全機制上更完整之參考。 / The Republic of China launched an open policy for people to go abroad to China and visit with their relatives on November 2, 1987, thus the relationship between cross-straits becomes closer and closer since then. To those veterans who followed the Kuomintang government’s move to Taiwan in 1949, it has been almost 40-year- separation for them to make a return voyage. Moreover, since the end of 1970s, the interactions between Taiwan and China have become closer and more frequent in all aspects of economics and society. After the said policy, although the cultural, living, and language difference still exists, the numbers of intermarriage mountain up year by year. As a result, the crime pattern has gradually changed from previous illegal immigration to legal entering but crime enhancing. The crime pattern transformation derives more and more homeland security problems. Nowadays, the factors of influence upon homeland security are mainly related to international environment, cross-strait relationship and domestic situation. Except China’s traditional threat of force to Taiwan, gradually, much other kinds of untraditional threat arise, such as human smuggling, prostitution under the disguise of marriage, drug trafficking, cross-strait organization crime, infectious diseases and etc. Not only do the aforementioned issues influence local social environment but also increase the cost both of social security and public order. Under the trend of globalization, accordingly, there is an eager need for Taiwan to establish possible crime-fighting corporate system with China without influencing Taiwan’s homeland security. The purpose of this study is three-fold. First, based on Taiwan’s “China policy”, this study examines and reviews the policy implementation and effects, then further discusses and analyzes comprehensive issues and related regulations, like relatives visiting, intermarriage, and opening Taiwan to mainland tourism which topic has recently been discussed enthusiastically. Second, aiming at criminal derivatives, this study discusses the difficult situation for said policy communication and implementation between the Competent Authority of immigration, National Immigration Agency and Police Department when dealing with related crime. Finally, focusing on the crime issues of people who enter Taiwan legally from China, this study tends to launch a policy suggestion for reference as setting up a well-designed homeland security system.

Developing methods for understanding the nature of voting patterns and party competition in Britain

This research both develops new methods and expands upon existing methodologies in order to improve our understanding of voting patterns and party competition in Britain. The thesis comprises five sections, each of which relates to a particular research focus. The first and principal section describes the process of determining a new method for decomposing electoral bias for three-party competition under simple plurality rules of voting. The study of electoral bias is important for voting systems that requires periodic boundary reviews intended to equalise electorate and to remove malapportionment. These papers describe both the process for developing the threeparty bias method and later its application to UK general elections from 1983 onwards. The second section uses aggregate data gathered for the elections to the Greater London Authority in order to understand the patterns of electoral support across the capital, particularly support for minor parties. A considerable amount of research effort has been expended upon providing reliable models for electoral forecasting both in the UK and elsewhere. The third section includes a paper that develops a forecast model that utilises aggregate local election data to estimate national vote shares for the three main parties in the UK. A fourth section brings together a series of papers that are linked by the themes of voter behaviour, either in terms of geographical or ballot context. A study of voter turnout in a London borough describes the relationship between proximity to polling station and electoral turnout at different types of election. A 8 number of papers included in this section also detail the effects of candidate ballot order on electoral support. The fifth and final section groups together two papers that using individual-level survey data to describe the pattern of candidate recruitment for local elections in Britain and, specifically, the under-recruitment of both women and Black, Asian and other minority ethnic candidates.

På grund av bristande erfarenheter inom byggtekniken gällande fukt har många skolor byggda under 1970-talet fuktproblem idag. I uppdrag av Vöfab ska tre olika fuktskadade objekt som har renoverats jämföras med varandra. Objekten består av tre skolor i Växjö som är byggda under 1970-talet. Objektens fuktskador och åtgärder ska undersökas. Detta görs för att analysera om samma typ av åtgärder vidtagits samt om samma typ av fuktproblem uppkommit i alla tre objekten. I studien ska det med hjälp av det teoretiska ramverket även undersökas om ytterligare fuktproblem kan uppkomma i framtiden trots renoveringarna.

Health and poverty : the issue of health inequalities in Ethiopia

The objectives of this study are to provide a comprehensive assessment of inequalities in infant and under-five years' child survival, access to and utilisations of child health services among different socio-economic groups in Ethiopia; and identify issues for policies and programmes at national and sub-national levels. This thesis examines the effect of parental socioeconomic status, maternal and delivery care services, mothers' bio-demographic and background characteristics on the level of differences in infant and under-five years' child survival and access to and utilisation of child health services. Descriptive and multivariate analyses were carried out for selected variables in the literature which were consider as the major determinants of infant mortality rate (IMR) and under-five years' child mortality rate (U5MR); access to and utilisations of child health services based on data from Ethiopian demographic and health survey (EDHS), covering the years 2000-2005. In the multivariate analysis a logit regression model was used to estimates inequalities in infant and under-five years' child survival, and inequalities in access to and utilisation of child health services. In Ethiopia, little was known about inequalities in IMR and U5MR, and inequalities in access to and utilisation of child health services. Besides, there is no systematic analysis of health inequalities and into its determinants using logistic regression. According to the available literature, this is the first comprehensive and systematic analysis of inequality of health in Ethiopia. The findings show that compared to under-five years' children of mothers' partners with no work, mothers' partners in professional, technical and managerial occupations had 13 times more chance of under-five years child survival for 2000 weighted observations. In addition, compared to infants of mothers who were gave birth to one child in last 5 years preceding the survey, infants of mothers who were gave birth to 2 children in last 5 years preceding the survey had 70% less chance of infant survival while infants of mothers who were gave birth to 3 or more children had 89% less chance of infant survival for 2000 weighted observations. Moreover, this study finding also indicates that inequalities increased significantly in the five years period between 2000 and 2005 among mothers with different birth interval. Most of the relations between birth interval and receiving childhood immunisation for vaccine-preventable diseases were statistically significant. Moreover compared to non-educated mothers, mothers who completed secondary and higher education were nearly 10 times more likely to receive DPT3 immunisation for their young children. This study concludes that policy measures that tackle health inequalities will have a positive impact in the implementation of health sector strategy of Ethiopia. Health inequalities studies in Ethiopia and Sub-Saharan Africa (SSA) countries should focus on systematic analysis of different socio-economic groups. The finding of this study support investing in the Ethiopia's health extension package (HEP) is a necessary but not sufficient condition for addressing rural poor health problem. HEP is successful in increasing primary health care coverage in rural Ethiopia to 89.6% (FMOH, 2009) but unable to reduce Ethiopia's higher level of IMR and U5MR. HEP is one of the success stories that address the rural poor health problem and can also be adapted to developing countries of SSA. The finding also shows that the success stories such as health insurance programs like Rwanda (World Bank, 2008a) and Ethiopia (FMOH, 2009/10) will play a key role in achieving country's health care financing goal of universal coverage. This can also be replicated in the developing SSA countries.

Beslutstagande under risk inom svenska bostadsaktiebolag : En kvantitativ studie före och efter implementeringen av Lag (2010:879) om allmännyttiga bostadsaktiebolag

Svensk bostadsmarknad har länge präglats av en snedvriden konkurrens med en markant fördel till Sveriges kommunala allmännyttiga bostadsaktiebolag jämte de privata bostadsaktiebolagen. I syfte att utjämna existerande sektoriella diskrepanser och skapa en konkurrensneutral marknad med jämlika villkor för privata respektive kommunala bostadsaktiebolag, infördes per den 1:a januari år 2011, Lag (2010:879) om allmännyttiga bostadsaktiebolag. Lagen innebär för de kommunala bostadsaktiebolagen ett avsteg från den tidigare självkostnadsprincipen mot ett i högre grad affärsmässig agerande enligt vinstdrivande syfte. Denna studie avser utifrån sambandet mellan risk och avkastning inom svenska bostadsaktiebolag, undersöka huruvida svenska kommunala allmännyttiga bostadsaktiebolag efter införandet av lagen uppvisar ett mer affärsmässigt agerande i termer om risk och avkastning på totalt kapital. Studiens teoretiska utgångspunkter tar huvudsakligen ansats i prospektteorin samt den förväntade nyttoteorin, vilka utgör två välrenommerade modeller i syfte att förklara beslutstagande under risk. Den förväntade nyttoteorin antar att individer är rationella nyttomaximerare och därefter agerar antingen riskaversivt, risksökande eller riskneutralt. Prospektteorin hävdar i motsats till den förväntade nyttoteorin att individen kan vara en kombination av riskaversiv, risksökande och riskneutral. Varav individen således inte alltid antas agera rationellt. Författarna har funnit flertalet tidigare forskare vilka genom perspektivet av den strategiska företagsledningen, bevisat stöd för prospektteorin som förklarande modell av beslutstagande under risk på företagsnivå, inom och mellan olika branscher. Därmed ställer författarna följande frågeställning: Uppvisar Sveriges kommunala allmännyttiga bostadsaktiebolag ett i högre grad affärsmässigt riskbeteende efter införandet av Lag (2010:879) om allmännyttiga bostadsaktiebolag? Utifrån sekundärdata insamlad via databasen Retriever Business har ett kvantitativt metodangrepp tillämpats i syfte att besvara studiens framställda hypoteser. Insamlad data består av de svenska bostadsaktiebolagens årliga avkastning på totalt kapital för tidsperioden 2006-2010 samt 2011-2014. Det empiriska materialet har vidare analyserats genom korstabeller, rangkorrelationer samt deskriptiv statistik. Resultatet visade att prospektteorin utgör ett bra alternativ som deskriptiv modell av beslutstagande under risk inom svenska bostadsaktiebolag. Enligt prospektteorin påvisades att svenska bostadsaktiebolags riskbeteende varierar beroende på bolagets branschallokering i förhållande till branschens genomsnittliga prestation, varav den strategiska företagsledningen inom svenska bostadsaktiebolag kan antas vara en sammanslagning av både risksökande och riskaversiva. Därmed motsäger resultatet den förväntade nyttoteorins antaganden om att individen alltid agerar rationellt. Vidare påvisade jämförelse av de kommunala bostadsaktiebolagens riskbeteende före och efter reformen att de kommunala bostadsaktiebolagens riskbeteende inte påverkats i större utsträckning, varför indikationer ges att allmännyttiga bostadsaktiebolag inte agerar i högre grad affärsmässigt efter Lag (2010:879) om allmännyttiga bostadsaktiebolag.

Hedge funds : fees, return revisions, and asset disclosure

This thesis is a collection of three essays on hedge funds with contributions to the empirical understanding of their fees, and their voluntary disclosure of returns and assets under management, using a large consolidation of widely-employed publicly available hedge fund databases. First, time-series variation in reported fees is analysed using fund launches within hedge fund management companies, and conditioning fees at launch on fund family characteristics. Larger and better performing fund families launch high fee funds. Funds with high management fees at launch do not perform any differently from low fee funds, though funds with high incentive fees marginally outperform. An interval regression technique is proposed to overcome the discrete nature of reported fees. Secondly, the reliability of voluntary disclosures of financial information is analysed with a different measure of time-variation --- tracking changes to statements of historical performance recorded at different points in time. This uncovers evidence that historical returns are routinely revised. These revisions are not merely random or corrections of earlier mistakes; they are partly forecastable by fund characteristics. Moreover, funds that revise their performance histories, significantly and predictably underperform those that have never revised. Finally, the availability, and timing, of the selective disclosure of assets under management by funds is examined. More than a third of funds have asset records falling short of returns published. There is evidence of strategic disclosure by funds --- asset reporting drying up after times of fund stress, such as poor performance or outflows. Furthermore, investors should take heed of the greater propensity for shortfall funds to trigger fraud performance flags. These results suggest that unreliable disclosures: constitute a valuable source of information for current and potential investors; have implications for researchers; and, exhort market regulators to include assets, not just returns, in the debate around mandatory disclosure by financial institutions.

On the genetic and environmental associations between body composition, depression symptoms and smoking behavior.

Obesity is a serious public health crisis and recent estimates of its incidence are the highest in United States history, with 35% and 17% of American adults and children affected, respectively. The clinical definition of adult obesity is operationalized as a body mass index (BMI) greater than 30 kg/m2. Although the prevalence of common obesity has increased dramatically over the past 30 years–largely thought to be due to changes in the environment, such as high calorie diets and sedentary lifestyles—twin and family studies have shown consistently that relative body weight is under considerable genetic influence in both children and adults, with heritability estimates ranging from 40% to 90%. Elucidating the genetic and environmental liability to relative body weight is an important public health endeavor. To further our understanding of the genetics of BMI and common complex obesity, several studies are described that integrate clinical, twin, and genome-wide association (GWAS) methodology in the context of genetic risk scores, clinical risk prediction, development across adolescence into adulthood, and comorbidity with depression symptoms and smoking behavior. First, in two cross-sectional genetic association studies, the utility of genetic risk sum scores (GRSS) were assessed, which summarize the total number of risk alleles, as an alternative form of replication and for potential clinical utility for obesity risk prediction. Next, since there has been only limited research on when during development BMI-associated variants begin influencing BMI, a longitudinal twin study was utilized to assess the effects of adult-validated BMI-SNPs across adolescence into adulthood. In addition, obesity is comorbid with numerous medical conditions including cardiovascular disease, insulin-resistance and some forms of cancer, as well as, various psychiatric disorders including eating disorders, mood disorders, and substance use. The next series of studies aimed to understand phenotypic and genetic associations between BMI/obesity and binge eating disorder (BED), depression symptoms and smoking behavior. Using a clinical sample of overweight and obese women with and without BED, the relationship of BED, food intake and internalizing symptoms of depression and anxiety was examined. Next, twin study methodology was used to investigate if shared genetic and/or environmental liability was responsible for phenotypic associations found between BMI, depression symptoms, and impulsivity. Finally, a genetic association study aimed at investigating whether genetic variants were associated with multiple behaviors, body composition and smoking behavior, or were trait-specific is presented. By utilizing several samples and methodologies and by pursuing methods development, a comprehensive approach is presented that is hoped to represent a more powerful evidence-based strategy to understanding the genetic and environmental determinants of BMI and common complex obesity, along with associated depression symptoms and smoking behavior.
