Structural and Functional Studies of ATP7B, the Copper(I)-transporting P-type ATPase Implicated in Wilson Disease

Copper is an integral component of key metabolic enzymes. Numerous physiological processes depend on a fine balance between the biosynthetic incorporation of copper into proteins and the export of excess copper from the cell. The homeostatic control of copper requires the activity of the copper transporting ATPases (Cu-ATPases). In Wilson disease the disruption in the function of the Cu-ATPase ATP7B results in the accumulation of excess copper and a marked deficiency of copper-dependent enzymes. In this work, the structure of ATP7B has been modeled by homology using the Ca-ATPase X-ray structure, enabling a mechanism of copper transport by ATP7B to be proposed. The fourth transmembrane helix (TM4) of Ca-ATPase contains conserved residues critical to cation binding and is predicted to correspond to TM6 of the ATP7B homology model, containing the highly conserved CXXCPC motif. The interaction with Cu(I) and the importance of the 3 cysteines in TM6 of ATP7B has been shown using model peptides. ATP7B has a large cytoplasmic N-terminus comprised of six copper-binding domains (WCBD1-6), each capable of binding one Cu(I). Protein-protein interactions between WCBDs and the copper chaperone Atox1 has been shown, contrary to previous reports, to occur even in the absence of copper. 15N relaxation measurements on the apo and Cu(I)-bound WCBD4-6 show that there is minimal change in the dynamic properties and the relative orientation of the domains in the two states. The domain 4-5 linker remains flexible, and domain 5-6 is not a rigid dimer, with flexibility between the domains. Copper transfer to and between WCBD1-6 likely occurs via protein interactions facilitated by the flexibility of the domains with respect to each other. The flexible linkers connecting the domains are important in giving the domains motional freedom to interact with Atox1, to transfer copper to other domains, and finally to transfer copper to the transmembrane site for transport across the membrane.

Multidrug sedation for dental procedures in children younger than eight.

<p>In this case study research project I have determined that multidrug sedation in children younger than eight years are possible.<br /> Conscious sedation [or sedation where verbal contact with the patient is possible] can be used successfully to decrease anxiety and fear for unpleasant experiences, like dental procedures.</p> <p><br /> Behaviour therapy in conjunction with one or more drugs can be used to depress the central nervous system in order to decrease the patient&rsquo / s awareness of unpleasant stimuli. This enables treatment to be carried out without patient interference. Extensive literature surveys were done to determine the ideal drugs as well as the ideal route for conscious sedation in dental treatment for children. In this study project drugs like midazolam, propofol, alfentanyl and ketamine were titrated intravenously to achieve conscious sedation.</p>

To get a wider range of designs I decided to choose a modern staging by Robert Wilson and Luboš Hrůza. My goal was to accommodate the production as a source of inspiration and guidance on how to dispose the scene in my own design.

The chapters of this theses and the researches that were conducted seeks to prove that “siqueirismo” (siqueirism) is established, from the figure of former governor Siqueira Campos, reaching Tocantins society in socio-economic and political aspects over time. I sought to discuss the national and local level structured domination in order to show that the state creation discourse, with the “heroic” figure of Siqueira Campos, reproduce an established model and, at the same time, has been supported by the political capital of the first state governor. Given this situation, the central inquiry that underlies this this thesis concerns the maintainability of this model regarding other besides Siqueira Campos. Will siqueirism have enough strength, as it had over the last two decades, given the departure of the former governor from the political scene, given either his old age or the fact that he doesn’t hold any political office? To answer this question, research about Siqueira Campos’ actions over the eight years of his office as a federal deputy, the construction of Palmas as symbol of belongingness to the geographical space, the teaching materials extolling the figure of the State’s “creator”, the appropriation of a emancipation narrative from the nortegoiana (relative to the north of Goiás) region, and the creation of a political group of almost unconditional support under the command of Siqueira Campos . / A intencionalidade desta tese é lançar um olhar sobre uma forma de dominação que se estabelece no Tocantins, o mais novo estado da federação, e que antecede à própria criação do Estado. Os capítulos desta tese e as pesquisas realizadas procuram demonstrar que o “siqueirismo” se estabelece, a partir da figura do ex-governador Siqueira Campos, atingindo a sociedade do Tocantins nos aspectos socioeconômicos e políticos ao longo do tempo. Procurei discutir a dominação estruturada a nível nacional e local com o objetivo de demonstrar que o discurso de criação do Estado, com a figura “heróica” da luta de Siqueira Campos reproduz um modelo já estabelecido e, ao mesmo tempo, tem o seu suporte no capital político do primeiro governador do Estado. Diante deste quadro, a questão central que norteia essa tese diz respeito à capacidade de manutenção deste modelo para além do próprio Siqueira Campos. O “siqueirismo” terá forças para se manter no campo político de forma hegemônica, como foi ao longo de duas décadas, com a saída do ex-governador da cena política, seja pela idade avançada ou pelo fato de não ocupar nenhum cargo político? Para chegar a esta questão fiz as pesquisas a partir da atuação de Siqueira Campos enquanto deputado federal por dezoito anos, da construção de Palmas como símbolo de pertencimento ao espaço geográfico, dos materiais didáticos exaltando a figura do “construtor” do Estado, da apropriação de uma narrativa de emancipação da região norte goiana e da formação de um grupo político de apoio quase irrestrito ao comando de Siqueira Campos.

Defesa: Curitiba, 31/10/2016 / Inclui referências : f. 165-168 / Resumo: A montagem teatral realizada por Robert Wilson a partir do texto-fonte A dama do mar (1888), de Henrik Ibsen, e de sua reescritura por Susan Sontag (1997) coloca em foco elementos como a voz, a luz, o cenário, a movimentação, a sonoplastia, ou seja, dá ênfase à visualidade e sonoridade. Este estudo analisa os mecanismos utilizados na transposição da obra original para a obra final, passando pela adaptação textual por Sontag, no contexto dos estudos da adaptação. É analisado o processo como a peça é transformada a partir das escolhas de Sontag, como os monólogos de frente para a plateia e a inclusão de textos folclóricos, e da estética de Wilson, que potencializa o substrato simbólico da peça com seu cenário e iluminação repletos de poesia, atuações que simulam o movimento de marionetes, gestual preciso e diálogos e movimentos propositadamente artificiais. Dentre os recursos utilizados para a análise, destaca-se a gravação em DVD do espetáculo apresentado em São Paulo em 2013 e a memória do espetáculo assistido ao vivo pela autora no mesmo ano; a peça de Susan Sontag publicada no Brasil pela editora n-1; entrevistas com atores da montagem, com o tradutor do texto de Sontag e o biógrafo da escritora, entre outras, e críticas sobre o espetáculo publicadas na mídia geral e especializada. Teatro contemporâneo. / Abstract: The staging by Robert Wilson using the source text The Lady from the Sea, by Henrik Ibsen (1888), and its rewrite by Susan Sontag (1997) highligths elements such as voice, scenery, sound, lights and movement, which means it emphasizes visuality and sonorical aspects. This study analyzes the mechanisms used in the transposition of the original play to the final work, by means of the textual adaptation by Sontag, in the context of the studies of adaptation. The play is transformed because of Sontag's choices, such as the monologues facing the public and the insertion of folk texts. It is furthermore transformed by Wilson's aesthetics, which empowers the simbolic substrate of the original play with his scenery and lights full of poetry, acting that reminds the movement of puppets, precise gesture and artificial dialogue. Among the resources used for this analysis are the recording of the play presented in São Paulo in 2013 as well as the memory of the live show to wich the author attended in the same year; Susan Sontag's Lady from the Sea, published in Brazil by n-1 editions; interviews with actors from the play, its translator to Portuguese and Sontag's biographer, among others, and critic texts about the staging published in the general and specialized media. Para tanto, procuramos primeiramente conhecer este antigo gênero literário, que nasce na Idade Antiga e atravessa os séculos, até chegar à atualidade. Dessa forma, coube à primeira parte do trabalho definir o conceito de fábula, elencar as suas características e compreender a sua estrutura, partindo dos modelos apresentados pelos fabulistas canônicos Esopo, Fedro e La Fontaine, para depois passarmos à produção brasileira, representada pelos escritores Monteiro Lobato e Millôr Fernandes. Após termos estabelecido o quadro de requisitos para entender como esse gênero literário se constitui, passamos a tratar das fábulas atuais de Bueno e contrastá-las com a fábula tradicional, visando mostrar, com isso, as diferenças que despontam: quais características conservam, quais são modificadas e até renovadas. Com efeito, como resultado da pesquisa pudemos obter, a partir das comparações e das análises feitas, a compreensão das transformações da fábula através dos tempos, e de como... / In this work we propose study the 25 fables that compose the Wilson Bueno’s book Cachorros do Céu (2005). First of all, we expose this literary genre, that begins at the Ancient Age, through the centuries, until the present. Therefore, the work begins, at its first part, defining the fable concept, listing its properties and understand its structure, based on the models presented by canonical fabulists Aesop, Phaedrus and La Fontaine, focalizing, after that, in the Brazilian’s fable production represented by Monteiro Lobato and Millôr Fernandes. After defined the bases to understand how this literary genre is composed and works, we treat Bueno’s current fables and contrast them with the traditional fable in order to show, therefore, the differences that emerge: what properties remain, which are modified and even renewed. Avaliação neuropsicológica de pacientes com doença de Wilson e correlação das alterações cognitivas com exames de neuroimagem estrutural e perfusão cerebral / Neuropsychological assessment of Wilson\'s disease patients and correlation of cognitive changes with structural neuroimaging and cerebral perfusion

Introdução: A doença de Wilson (DW) é uma doença genética rara, sistêmica, causada por déficit no metabolismo do cobre, levando a acúmulo em diversos órgãos, incluindo o cérebro, especialmente os núcleos da base. Apesar de distonia e disartria serem as alterações neurológicas mais comuns, alterações cognitivas, incluindo demência, podem surgir em casos não tratados. Objetivos: Avaliar as funções cognitivas de um grupo de pacientes com DW e correlacionar com os achados de RM e SPECT. Métodos: Todos os pacientes com DW atendidos consecutivamente em uma clínica de distúrbios do movimento, entre outubro de 2006 e agosto de 2007, foram submetidos ao Miniexame do Estado Mental (MEEM), Bateria Cognitiva Breve (BCB), teste de fluência verbal (animais, FAS e verbos), Bateria de Avaliação Frontal (BAF), STROOP, Extensão de Dígitos (ED) ordem direta e ordem inversa, Escala de Demência de Mattis (DRS), Teste de Seleção de Cartas de Wisconsin (WCST), Hooper e Blocos do WAIS. Depois dos testes, todos os pacientes realizaram RM e SPECT. Alterações de RM (hipersinal, hiposinal e atrofia) foram quantificadas em uma escala (0-17). O SPECT foi analisado por meio de SPM. Pacientes com depressão ou com anartria foram excluídos. Os resultados foram comparados com o desempenho de um grupo de voluntários sadios. Resultados: 20 pacientes com DW (11 homens) e 20 controles (9 homens) foram avaliados. A média de idade no grupo de pacientes com DW e controles foi 30,05±7,25 anos e 32,15±5,37 anos, respectivamente. A escolaridade média foi 11,15±3,73 anos nos pacientes com DW e 10,08 ± 2,62 anos nos controles. Pacientes com DW tiveram comprometimento cognitivo nos seguintes testes: MEEM (26,70±2,45 x 28,75±1,29), DRS (132,45 ±10,77 x 140,55±3,72), Fluência verbal: FAS (22,40±12,40 x 38,75±9,11) e Verbos (8,50±6,63 x 15,40±6,22); ED ordem direta (4,95±0,82 x 6,15±1,42), STROOP (4,40±4,87 x 0,50±0,68), BAF (12,95±2,85 x 16,25±1,25) e BCB: M2 (9,20±0,69 x 9,75±0,44), M5 (8,45±1,35 x 9,50 ±0,76). O diagnóstico de demência foi realizado em dois pacientes. Houve correlação entre o comprometimento cognitivo e a escala de RM (r=0,5348). Essa correlação foi mais forte com as alterações de hipersinal e atrofia (r=0,718). A avaliação por SPM mostrou uma hipoperfusão no caudado de forma simétrica, no cerebelo à esquerda e insula à esquerda. Conclusões: Pacientes com DW apresentam um comprometimento cognitivo, especialmente nas funções executivas, com boa correlação entre cognição e RM / Background: Wilsons disease (WD) is a rare, genetic and systemic disease, caused by a deficit in the copper metabolism, leading to its accumulation in different organs, including the brain, especially the basal ganglia. Although dystonia and dysarthria are the most common neurological signals, cognitive changes, including dementia, may emerge in untreated cases. Objective: To assess cognitive functioning of a group of WD patients and correlate with MRI and SPECT findings. Methods: All WD patients consecutively attended in a Movement Disorders Clinic between October 2006 and August 2007 were submitted to the Mini-Mental State Examination (MMSE), Brief Cognitive Screening Battery (BCSB), verbal fluency tests (animals, FAS and verbs), the Frontal Assessment Battery (FAB), STROOP, Digit Span forward and backward (DS), Dementia Ranting Scale (DRS), Wisconsin Card Sorting Test (WCST), Hooper and WAIS block design. After the tests, a MRI and SPECT were done in all patients. MRI abnormalities (high intensity signal, low intensity signal and atrophy) were ratted in a scale (0-17). SPECT was analyzed with SPM. Patients with depression or with anarthria were excluded. Results were compared with the performance of a group of healthy controls. Results: 20 WD patients (11 men) and 20 controls (9 men) were evaluated. Mean age in the WD and control groups were 30.05±7.25 years and 32.15±5.37, respectively. Mean educational level was 11.15±3.73 years among WD cases and 10.08 ± 2.62 years among controls. The WD patients had a cognitive impairment in the: MMSE (26.70±2.45 x 28.75±1.29), DRS (132.45 ±10.77 x 140.55±3.72), verbal fluency: FAS (22.40±12.40 x 38.75±9.11), Verbs (8.50±6.63 x 15.40±6.22); DS forward (4.95±0.82 x 6.15±1.42), STROOP (4.40±4.87 x 0.50±0.68), FAB (12.95±2.85 x 16.25±1.25) and BCSB: M2 (9.20±0.69 x 9.75±0.44), M5 (8.45±1.35 x 9.50 ±0.76). Dementia diagnostic was made in tow patients. There was a correlation between cognitive impairment and MRI scale (r=0.5348), this correlation was better with high intensity signal plus atrophy (r=0.718). The SPM showed a symmetrical caudate, left cerebellar and left insular hypoperfusion. Conclusion: WD patients presented cognitive impairment, especially in executive functions, with good correlation between cognition and MRI
