Interventioneller Verschluss von VorhofdefektenEwert, Peter 17 July 2003 (has links)
Einleitung Der interventionelle Verschluß eines einfachen Vorhofseptumdefekts (ASD) vom Sekundumtyp und eines persistierenden Foramen ovale (PFO) ist zur Routinemethode gereift (1). Die Intervention wird unter Röntgendurchleuchtung (Strahlenexposition) durchgeführt. Sie ist wegen der geringen Invasivität auch bis ins hohe Alter mit deutlich niedrigerem Risiko als eine Operation durchführbar (2). Die hier vorgestellten Arbeiten haben systematisch untersucht, welche Möglichkeiten bestehen, bei der Intervention auf eine Strahlenexposition zu verzichten (3-6), welche interventionellen Möglichkeiten bei multiplen Defekten und Vorhofseptumaneurysmen bestehen (7,8) und welche Auswirkungen ein restriktiver linker Ventrikel auf die hämodynamische Adaptation nach Defektverschluß haben kann (9-11). Methodik Alle Untersuchungen wurden im Rahmen der klinischen Routine im Herzkatheterlabor am sedierten Patienten mit Vorhofseptumdefekt vom Sekundumtyp, persistierendem Foramen ovale, perforiertem Vorhofseptumaneurysma oder multiperforiertem Vorhofseptum durchgeführt. 1. Es wurde eine Methode zum Verschluß von Vorhofseptumdefekten unter alleiniger Ultraschallkontrolle entwickelt, d.h. unter vollständigem Verzicht auf die sonst notwendige Röntgenstrahlung. 2. Die Morphologie von Vorhofseptumaneurysmen und multiperforierten Vorhofsepten wurde analysiert und im Hinblick auf die interventionellen Verschlußmöglichkeiten klassifiziert. Dabei wurde auch die Möglichkeit der simultanen Implantation mehrerer Okkluder mit einbezogen. 3. Zur Erkennung von Patienten mit einem restriktiven linken Ventrikel, der unmittelbar nach ASD-Verschluß insuffizient werden könnte, wurde eine Methode der präinterventionellen hämodynamischen Evaluation etabliert. Dazu wird die Vorlast und die diastolische Funktion des linken Ventrikels unter temporärem Verschluß des ASD mit einem Okklusionsballon untersucht. Demaskiert sich eine linksventrikuläre Restriktion, so wird als Therapiekonzept der Ventrikel auf den interventionellen Verschluß durch eine prophylaktische 'Konditionierung' mittels Diuretika und Inotropika vorbereitet. Resultate 1. Interventioneller ASD-Verschluß ohne Strahlenexposition Wir konnten zeigen, daß der interventionelle ASD-Verschluß ohne Einsatz von Röntgenstrahlung durchführbar ist (3). Dies gilt für die präinterventionelle Diagnostik, die invasive Größenmessung (Ballonsizing) (6) und den interventionelle Verschluß selbst (4). Als einziges bildgebendes Verfahren für die Intervention dient die Echokardiographie. Im Vergleich zum Standardprocedere waren beim Verzicht auf eine Strahlenexposition gleich gute Ergebnisse zu erzielen, die Prozedurdauer war vergleichbar. Beim spontan atmenden Patienten sind für diese Methode höhere Dosen an Sedierung erforderlich, um die längere Verweilzeit der transösophagealen Echokardiographiesonde zu ermöglichen (5). Der Amplatzer Occluder ist wegen seiner guten Sichtbarkeit im transösophagealen Ultraschall, seiner Rotationssymmetrie und seiner einfachen Plazierung für diese neue Methode des ASD-Verschlusses ohne Röntgenstrahlung besonders geeignet. 2. Verschluß morphologisch komplexer Vorhofseptumdefekte Auch multiperforierte Vorhofsepten können interventionell erfolgreich verschlossen werden. Bei dicht nebeneinander liegenden Defekten ist dies mit einem Occluder, der alle Defekte abdeckt, möglich, bei weiter auseinanderliegenden Defekten ist die simultane Implantation zweier Occluder sinnvoll. Zwei Occluder führen mit größerer Sicherheit zu einem Verschluß ohne Restshunt (7). Multiple Defekte sind häufig mit einem Vorhofseptumaneurysma vergesellschaftet. Im Hinblick auf die Interventionsmöglichkeiten läßt sich diese Anomalie in vier Gruppen unterteilen: Aneurysma mit PFO (Typ A), mit ASD (Typ B), mit mehreren dicht nebeneinander liegenden Defekten (Typ C) und große Aneurysmen mit einer Vielzahl irregulär verteilter Perforationen (Typ D). Die ersten drei Formen lassen sich interventionell verschließen. Dabei gelingt zumindest eine Teilstabilisierung der Aneurysmen (8). 3. Vorhofseptumdefekte und restriktiver linker Ventrikel Wir konnten zeigen, daß insbesondere bei älteren Patienten mit ASD eine verdeckte linksventrikuläre Restriktion vorliegen kann. Ein interventioneller ASD-Verschluß kann bei diesen Patienten zur akuten kardialen Dekompensation führen (9). Als Hinweis auf eine gestörte linksventrikuläre Compliance fanden wir bei temporärer Okklusion des Defekts einen deutlichen Anstieg des linksatrialen Drucks und einen gestörten Mitralklappeneinstrom (10). Nach einer prophylaktischen 'Konditionierung' des linken Ventrikels mittels vorlastsenkenden und inotropiesteigernden Medikamenten (Diuretika, Phosphodiesterasehemmer, Katecholamine) gelang bei fast allen Patienten der interventionelle ASD-Verschluß mit guter Adaptation des Ventrikels, ohne daß es zur kardialen Dekompensation kam (11). Schlußfolgerungen Die in dieser Habilitationsschrift ausgeführten Arbeiten haben weiterführende Fragestellungen und Grenzbereiche des interventionellen Verschlusses von ASD und PFO aufgezeigt und neue interventionelle Therapiestrategien dargestellt. Dadurch ist es möglich, im klinischen Alltag weniger Röntgenstrahlung und Röntgenkontrastmittel zu verwenden, auch morphologisch komplexe Defekte standardisiert zu behandeln und selbst im hohen Alter bei Vorliegen einer linksventrikulären restriktiven Dysfunktion Defekte mit geringem Risiko zu verschließen. / Introduction Interventional closure of the simple secundum type atrial septal defect (ASD) and of persistent foramen ovale (PFO) has developed into a routine procedure (1). The intervention is carried out under X-ray monitoring (X-ray exposure). Since it is minimally invasive it can be carried out even in patients of advanced age with significantly less risk than an operation (2). The work presented here investigates systematically the possibilities of carrying out the intervention without X-ray exposure (3-6) and in the case of multiple defects and atrial aneurysms (7, 8). It also looks at the effects of a restrictive left ventricle on hemodynamic adaptation after closure of the defect (9-11). Methods All examinations were carried out as part of the clinical routine of the heart catheter laboratory in sedated patients with a secundum type atrial septal defect, persistent foramen ovale, perforated atrial aneurysms or multiply perforated atrial septum. 1. A method was developed whereby the closure of atrial septal defects can be carried out solely under echocardiographic monitoring, i.e. completely without X-ray exposure. 2. The morphology of atrial septal aneurysms and multiply perforated atrial septum was analyzed and classified with regard to the possibilities of interventional closure. The possibility of simultaneous implantation of several occluders was also considered. 3. To recognize patients with a restrictive left ventricle, which might become insufficient directly after ASD closure, a method of preinterventional hemodynamic evaluation was established. This involves examining preload and the diastolic function of the left ventricle during temporary closure of the ASD with an occlusion balloon. If this procedure reveals left ventricular restriction, the ventricle is prepared for interventional closure by prophylactic conditioning by means of diuretics and inotropes. Results 1. Interventional ASD Closure without X-Ray Exposure We were able to show that interventional ASD closure is possible without the use of X-rays (3). This applies to the preinterventional diagnostic procedures, invasive size measurement (balloon sizing) (6) and the interventional closure itself (4). The sole imaging procedure used for the intervention is echocardiography. In comparison with the standard procedure, the results are equally good and the duration of the procedure is comparable. In the spontaneously breathing patient higher sedative doses are necessary so that the transesophageal echocardiography tube can remain in place throughout (5). The Amplatzer occluder is particularly suitable for this new method because it is easily viewed in transesophageal echocardiographic imaging, rotationally symmetrical and easily positioned. 2. Closure of Morphologically Complex Atrial Septal Defects Multiple perforations of the atrial septum can also be successfully closed by intervention. If the defects are close together, one occluder can be used to cover all the defects; if they are further apart, the simultaneous implantation of two occluders is indicated. Two occluders are more likely to achieve occlusion without residual shunt (7). Multiple defects are often associated with an atrial septal aneurysm. With regard to the interventional possibilities these anomalies can be divided into four groups: aneurysm with PFO (type A), with ASD (type B), with several defects situated close together (type C) and large aneurysms with a number of irregularly distributed perforations (type D). The first three types may be closed by intervention, which mostly achieves partial stabilization of the aneurysms (8). 3. Atrial Septal Defects and Restrictive Left Ventricle We showed that, in particular in older patients with ASD, left ventricular restriction may be concealed. In these patients interventional ASD closure can lead to acute cardiac decompensation (9). A sign of disruption of left ventricular compliance was a marked rise in the left atrial pressure and disturbance of the mitral valve inflow during temporary occlusion of the defect (10). Following prophylactic 'conditioning' of the left ventricle by drugs that reduce the preload and increase inotropism (diuretics, phosphodiesterase inhibitors, catecholamines), interventional ASD closure succeeded in almost all patients with good adaptation of the ventricle and without cardiac decompensation occurring (11). Conclusion The work reported here addresses complex questions and frontier areas of the interventional closure of ASDs and PFO and presents new interventional strategies. It enables less X-ray exposure and less X-ray contrast medium to be used in clinical practice. Morphologically complex defects can be treated by standard procedures and with a small risk, even in patients with advanced age and left ventricular restrictive dysfunction.

Emprego de técnicas de neurocirurgia minimamente invasiva para o tratamento de aneurismas incidentais de circulação anterior / Employment of minimally invasive neurosurgical techniques for treatment of unruptured brain aneurysms of the anterior circulationBrigido, Maurício Mandel 22 February 2018 (has links)
Introdução: A neurocirurgia minimamente invasiva já é uma realidade em muitos centros em todo o mundo. A aplicação de conceitos antigos com a incorporação de novas tecnologias permite o emprego de medidas menos invasivas, mas com a mesma eficácia e segurança. O real papel destas técnicas e o seu efeito sobre a evolução dos doentes ainda é nebuloso. Objetivos: avaliar a segurança e resultados da técnica minimamente invasiva na clipagem de aneurismas cerebrais de circulação anterior e determinar o momento seguro para alta hospitalar. Materiais: Cento e onze doentes adultos com diagnóstico de aneurismas não rotos de circulação anterior foram randomicamente distribuídos e submetidos a cirurgia por um acesso minimamente invasivo (grupo de estudo - 36 pelo acesso transpalpebral e 34 através de craniotomias minipterional reduzida) ou acesso pterional clássico (grupo controle - 41 doentes). O endoscópio acoplado a um telefone celular foi utilizado juntamente com o microscópio durante as cirurgias do grupo de estudo. Os doentes do grupo de estudo foram submetidos a um protocolo específico para avaliação da segurança da alta hospitalar precoce. Foram avaliados desfechos cirúrgicos, clínicos/funcionais, estéticos e sobre qualidade de vida. Resultados: Em ambos os grupos, os dados demográficos e as características dos aneurismas foram similares. O tempo médio das cirurgias foi menor no grupo de estudo (214 min. vs 292 min, p = 0,0008). A necessidade de transfusão sanguínea foi menor no grupo de estudo (1 doente vs 7 doentes, p = 0,018). O número de eventos isquêmicos foi menor no grupo de estudo (4 doentes vs 8 doentes, p = 0,07), mas os eventos com repercussão clínica foram semelhantes (2 doentes vs 3 doentes, p = 0,53). A presença de colo residual na angiografia controle foi menor no grupo de estudo (6 doentes vs 11, p = 0,021), mas foram todos colos pequenos, 1,75 ± 0,68 mm, sendo que apenas um doente do grupo controle foi reoperado. A paralisia do ramo frontal do nervo facial foi menor no grupo de estudo, tanto a temporária (3 vs 14, p = 0,008) quanto a definitiva (0 vs 4, p = 0,032). A atrofia do músculo temporal foi menos frequente e mais leve no grupo de estudo (9 vs 14, p = 0,012). No grupo de estudo, 91,4% dos doentes receberam alta precoce no dia seguinte da cirurgia e nenhum doente apresentou evento adverso por este motivo. Os doentes do grupo de estudo ficaram assintomáticos mais rapidamente no pós-operatório (pela avaliação da escala de Rankin, p = 0,0026), mas não houve diferença entre os grupos dentre as pontuações acima de 1 na escala de Rankin modificada. Um doente do grupo controle faleceu no pós-operatório (0,9%). Conclusões: Os resultados demonstraram que as alternativas minimamente invasivas propostas são seguras e tem resultados clínicos e cirúrgicos iguais ou superiores ao tratamento clássico em vários quesitos. A alta precoce nestes doentes é possível e segura. O acesso nanopterional ou transpalpebral é uma alternativa melhor em relação à craniotomia pterional clássica para tratar aneurismas não rotos da circulação anterior / Introduction: Minimally invasive neurosurgery is already a reality in many centers across the world. The application of old concepts with the incorporation of new technologies allows the use of less invasive measures with the same effectiveness and safety. However, the real role of these techniques and their effect on the outcome of patients is still obscure. Objectives: To evaluate the safety and results of minimally invasive techniques in brain aneurysm clipping and determine the possibility of early hospital discharge. Methods: 111 adult patients with unruptured anterior circulation aneurysms were randomized and underwent a minimally invasive surgical approach, (36 by transpalpebral approach and 34 through a reduced minipterional craniotomy) or classical pterional approach (41 patients). The endoscope coupled to a smart phone was used along with the microscope during surgery (study group only). Patients in the study group were subjected to a specific protocol for assessment of early hospital discharge. Surgical, clinical/functional and aesthetic outcomes were evaluated along with long term quality of life. Results: In both groups, the demographics and characteristics of aneurysms were similar. The average time of surgery was lower in the study group (214 min. vs. 292 min, p = 0.0008). The need for blood transfusion was lower in the study group (1 patient vs 7 patients, p = 0.018). The number of ischemic events was lower in the study group (patients 4 patients vs. 8, p = 0.07), but events with clinical significance were similar (3 patients vs. 2 patients, p = 0.53). The presence of residual neck on control angiography was lower in the study group (6 patients vs 11, p = 0.021), but only small ones were found, 1.75 ± 0.68 mm, and only one control group patient required reoperation for this reason. The paralysis of the frontal branch of the facial nerve was lower in the study group, both temporary (3 vs 14, p = 0.008) and definitive (0 vs. 4, p = 0.032). The atrophy of the temporal muscle was less frequent and less severe in the study group (9 vs 14, p = 0.012). Most patients in the study group (91.4%), were discharged on the next day of the surgery and no patients had any related adverse events. Patients in the study group got asymptomatic faster (assessed by the Rankin scale, p = 0.26), but there was no difference between the groups among scores above 1 on the modified Rankin scale. One control group patient died postoperatively (0,9%). Conclusions: The results showed that the proposed minimally invasive alternatives are safe. Clinical and surgical results are equal or superior to conventional treatment in several topics. Early discharge in these patients is possible and safe. The described approaches (nanopterional or transpalpebral) are better alternatives to the classical pterional craniotomy to treat unruptured aneurysms of the anterior circulation

Indução de aneurisma em aorta abdominal de porcos: um modelo endovascular / Endovascular model of abdominal aortic aneurysm induction in swineLederman, Alex 29 September 2015 (has links)
A ruptura do aneurisma da aorta abdominal está entre as principais causas de óbito. A alta morbi-mortalidade associada à ruptura e tratamento dos aneurismas representa um grande desafio aos médicos e um alto risco aos pacientes. Apesar dos modelos experimentais serem úteis para compreendermos, treinarmos, testar novos métodos diagnósticos e terapêuticos para esta doença, os modelos existentes até o momento ainda não são os ideais. Nos modelos existentes, os animais são muito pequenos e não representam a doença nos humanos, ou o procedimento envolve laparotomia, ou o comportamento do aneurisma criado não é semelhante ao de um aneurisma verdadeiro. Desenvolvemos, a partir de uma abordagem minimamente invasiva, um método eficiente de induzirmos a formação de um aneurisma verdadeiro na aorta abdominal infrarrenal de porcos Large White. Os animais foram submetidos a indução química a partir de uma aplicação por via endovascular de cloreto de cálcio a 25% ou elastase pancreática suína. Os animais controles foram submetidos a tratamento com soro fisiológico (NaCl 0,9%). Todos os animais foram submetidos à mesma técnica operatória, sob anestesia geral. Os animais foram acompanhados com exames ultrassonográficos com Doppler semanalmente, e as aortas colhidas para testes biomecânicos e análise histológica após 4 semanas. Apesar das aortas tratadas com elastase apresentarem apenas dilatação, estudos de imagens, histológicos e biomecânicos mostraram que as aortas tratadas com cloreto de cálcio evoluíram para aneurismas verdadeiros, com comportamento biomecânico semelhante ao dos aneurismas de humanos. Estes resultados/achados indicam que a abordagem endovascular para a indução de aneurisma é factível e não ocasiona uma fibrose retroperitoneal / Abdominal aortic aneurysms rupture are among the main causes of death. The high morbidity and mortality associated with aneurysm rupture and repair represents a challenge for surgeons and high risk for patients. Although experimental models are useful to understand, train, and develop new treatment and diagnostic methods for this disease, animal models developed to date are far from ideal. Animals are either too small and do not represent the pathology of humans, or the procedures employ laparotomy, or the aortic behavior does not resemble that of a true aneurysm. We developed a novel, less invasive and effective method to induce true aortic aneurysms in Large White pigs. Animals were submitted to an endovascular chemical induction using either calcium chloride (25%) or swine pancreatic elastase. Controls were exposed to saline solution. All animals were operated on using the same surgical technique under general anesthesia. They were followed weekly with ultrasound examinations and at 4 weeks the aorta was harvested. Although elastase induced only arterial dilation, imaging, histological, and biomechanical studies of the aorta revealed the formation of true aneurysms in animals exposed to calcium chloride. Aneurysms in the latter group had biomechanical failure properties similar to those of human aneurysms. These findings indicate that the endovascular approach is viable and does not cause retroperitoneal fibrosis

INTRODUÇÃO: O diâmetro transverso máximo (DTM) de um aneurisma da aorta abdominal (AAA), a medida mais comumente utilizada para determinar quando uma intervenção cirúrgica está indicada nos portadores desta afecção, tem limitações e não reflete o exato risco de rotura. Parâmetros biomecânicos e morfológicos detectados pelo ultrassom (US) com propriedades tridimensionais (3D), podem ajudar a melhor entender o comportamento dos AAA e a determinar o melhor momento para o tratamento cirúrgico deste grupo de pacientes. OBJETIVOS: Verificar a capacidade e a viabilidade do ultrassom bidimensional (US-2D) acoplado com algoritmos de speckletracking em avaliar as propriedades biomecânicas da aorta em pacientes com e sem AAA. Mensurar o volume parcial dos AAA através do ultrassom tridimensional (US- 3D) e compará-lo com o volume estimado pela tomografia computadorizada (TC). MÉTODOS: Este foi um estudo prospectivo. Trinta e um pacientes com aortas normais (grupo 1), 46 pacientes com AAA de diâmetro máximo entre 3,0 - 5,5 cm (grupo 2) e 31 pacientes com diâmetro dos AAA >- 5,5 cm (grupo 3) foram submetidos ao exame de US 2D/3D da aorta infrarrenal, e as imagens foram pós-processadas antes de serem analisadas. No diâmetro máximo, o strain (deformação) circunferencial global e a rotação global máxima acessados pelo algoritmo de speckle-tracking 2D foram comparados entre os três grupos. O strain regional da parede posterior foi também comparado com o da parede anterior em todos os grupos. Os dados de volumetria obtidos usando o US-3D de 40 pacientes foram comparados com os dados de volumetria obtidos por uma TC contemporânea. RESULTADOS: A mediana do strain circunferencial global foi 2,0% (interquartile range (IR): 1,0 - 3,0), 1,0% (IR: 1,0 - 2,0) e 1,0% (IR: 1,0 - 1,75) nos grupos 1, 2 e 3 respectivamente (p < 0,001). A mediana da rotação global máxima diminuiu progressivamente dos grupos 1 ao 3 (1,38º (IR: 0,77 - 2,13), 0,80º (IR: 0,57 - 1,0) e 0,50º (IR: 0,31 - 0,75) (p < 0,001)). Na análise regiãoespecífica, o pico de strain na parede posterior foi significativamente maior que na parede anterior apenas no grupo 3 (p = 0,003). Os volumes dos AAA estimados pelo US-3D tiveram boa correlação com a TC (R-square = 0,76). CONCLUSÕES: O US é capaz de detectar parâmetros biomecânicos distintos entre aortas normais, aneurismas pequenos e aneurismas grandes. A propriedade 3D do US é capaz de determinar o volume dos AAA e apresenta boa correlação com o método padrão ouro (TC). Estudos prospectivos e com seguimento longo são necessários para aprofundar a compreensão não invasiva do comportamento biomecânico e morfológico dos AAA e correlacionar esses parâmetros com o risco de rotura / INTRODUCTION: The maximum transverse diameter of an abdominal aortic aneurysm (AAA), the most common measurement utilized to determine whether surgical intervention is indicated, has limitations and does not reflect the exact risk of rupture. Biomechanical and morphological parameters detected by ultrasound (US), including three-dimensional (3D) properties, can help to better understand the behavior of AAA and to determine the optimal approach to treatment in this group of patients. OBJECTIVES: To demonstrate the feasibility and the ability of the US with speckletracking algorithms to evaluate biomechanical parameters of the aorta in patients with and without abdominal aortic aneurysm (AAA). To measure the partial aneurysm volume by 3D-US and to compare it with the aneurysm volume measured by computed tomography (CT). METHODS: It was a prospective study. Thirty-one patients with normal aortas (group 1), 46 patients with AAA measuring 3.0 - 5.5 cm (group 2) and 31 patients with AAA >- 5.5 cm (group 3) underwent a 2D/3D US examination of the infrarenal aorta, and the images were post-processed prior to being analyzed. In the maximum diameter, the global circumferential strain and the global maximum rotation assessed by 2D speckle-tracking algorithms were compared among the three groups. The regional strain on posterior wall was also compared to that in the anterior wall for all groups. The volumetry data obtained using 3D-US from forty AAA patients was compared with the volumetry data obtained by a contemporary CT. RESULTS: The median global circumferential strain was 2.0% (interquartile range (IR): 1.0 - 3.0), 1.0% (IR: 1.0 - 2.0) and 1.0% (IR: 1.0 - 1.75) in groups 1, 2 and 3, respectively (p < 0.001). The median global maximum rotation decreased progressively from group 1 to 3 (1.38º (IR: 0.77 - 2.13), 0.80º (IR: 0.57 - 1.0) and 0.50º (IR: 0.31 - 0.75) (p < 0.001)). In the region-specific analysis, the strain in the posterior wall was significantly higher than anterior wall only in group 3 (p = 0.003). AAA volume estimations by 3D-US correlated well with CT (R-square = 0.76). CONCLUSIONS: The US can detect distinct biomechanical parameters between normal aorta, small aneurysms and big aneurysms. The 3D property of US is able to determine AAA volume and correlates well with the gold standard technique (CT). Prospective studies and with long-term follow-up are necessary in order to deepen the non invasive understanding of AAA biomechanical and morphological behavior and to correlate those parameters with rupture risk

A fim de determinar a incidência de insuficiência renal aguda (IRA) e os fatores de risco para seu desenvolvimento no pós-operatório de cirurgia para correção de aneurismas da aorta tóraco-abdominal e abdominal, foi realizado estudo retrospectivo das operações eletivas no ano de 2000. Foi encontrada alta incidência de IRA, tanto IRA leve, descrita como elevação de 25% da creatinina basal, em 57% dos pacientes, como uma forma mais grave, descrita como elevação de 50% e encontrada em 33% dos pacientes. Os fatores de risco independentes para IRA leve foram a presença de clampeamento acima do óstio das artérias renais (OR 6,9, IC 95% 1,32-36,12) e o tempo total de anestesia maior que 339 min (OR 1,00 para cada min acima de 339min, IC 95% 1,003-1,016). Para lesão mais grave, a instabilidade hemodinâmica, representada pela necessidade do uso de droga vasoativa no pós-operatório, também foi fator independente para o desenvolvimento de IRA (OR 7,4, IC 95% 1,84-30,16). A mortalidade hospitalar encontrada no estudo foi de 42,8% nos pacientes que tiveram clampeamento supra-renal, e 21,4% nos pacientes com clampeamento apenas infra-renal. Neste estudo, os fatores de risco independentes para óbito foram a filtração glomerular (FG) menor que 49 mL/min (OR 17,07, IC 95% 2,00-145,23), a necessidade de clampeamento acima das artérias renais (OR 9,6, IC 95% 1,37-67,88), desenvolvimento de IRA considerada como aumento da creatinina de 50% em relação a basal (OR 8,8, IC 95% 1,31-59,39) e a hiperglicemia no pós-operatório (OR 19,99, IC 2,32-172,28). Conhecendo a incidência de IRA, os fatores de risco para o seu desenvolvimento e a morbi-mortalidade associada ao seu desenvolvimento, foi testada N-acetilcisteína (NAC) como medida protetora, através de estudo randomizado, placebo-controlado e duplo-cego com os pacientes elegíveis para correção cirúrgica eletiva de aneurismas da aorta tóraco-abdominal e abdominal. NAC foi administrado por via oral 1200mg duas vezes ao dia 24 horas antes da operação, e após esta por 48 horas de forma endovenosa, 600mg de 12 em 12horas. Quarenta e dois pacientes foram incluídos no estudo, 18 no grupo NAC e 24 no grupo controle. A creatinina e FG basal foram semelhantes entre os grupos (1,19±0,33 vs 1,37±0,49 mg/dL; e 64,6±26,22 vs 65,7±28,32 ml/min, NAC vs controle, respectivamente, p=0,17 e p=0,90). A incidência de IRA leve, definida como aumento de 25% da creatinina basal, foi de 36% (13/36), mas não foi estatisticamente diferente entre os dois grupos (7/14, 50% no grupo NAC e 6/22, 27,3% no grupo controle, p=0,16). A mortalidade hospitalar foi de 23% (10/42) e também não foi diferente (p=0,209) entre os grupos, 33,3% no NAC e 16,7% no controle, o mesmo ocorrendo com o tempo de internação na UTI (2,93±1,53 vs 2,52±1,36 dias, p=0,40). Dessa forma, esse estudo sugere que embora possa haver algum efeito biológico da NAC, o seu uso na prevenção da IRA associada à operação eletiva de aneurisma da aorta abdominal não é justificável / Small alterations of the renal function during hospital stay and more specifically in the postoperative period determine significant increase in hospital mortality. In order to determine the incidence of AKI (acute kidney injury) and the risk factors for its development in the postoperative period of elective surgical open repair of thoracoabdominal and abdominal aortic aneurisms, it was carried out a retrospective study through the year of 2000. It was found high incidence of AKI, defined as a 25% increase in baseline serum creatinine (SCr), which occurred in 57% of the patients, and in a more strict definition, 50% increase in SCr, which occurred in 33%. The independent risk factors for AKI 25% was the presence of supra-renal clamping (OR 6,9, IC 95% 1,32-36,12) and the total anesthesia duration greater than 339 min (OR 1,00 for each min above 339 min, IC 95% 1,003-1,016). For more serious AKI (50%), the hemodynamic instability, represented for the need of vasoactive drugs in the postoperative period, also was an independent risk factor (OR 7,4, IC 95% 1,84-30,16). The mortality rate found in the study was 42,8% in patients who had supra-renal aortic clamping, and 21,4% in those with only infra-renal aortic clamping. In this study, the independent risk factors for death was baseline glomerular filtration rate (GFR) less than 49mL/min (OR 17,07, IC 95% 2,00-145,23), the need of supra-renal aortic clamping (OR 9,6, IC 95% 1,37-67,88), AKI defined as an increase of 50% in baseline SCr (OR 8,8, IC 95% 1.31-59,39) and the hyperglycemia in the postoperative period (OR 19,99, IC 2,32-172,28). Thereafter, we tested the protective effect of NAC through a randomized, placebo-controlled, double-blind study with patients eligible for elective open surgical repair of thoracoabdominal e abdominal aortic aneurisms. NAC was give po 1200mg twice daily 24 hours before the operation, and for 48 hours after surgery 600mg twice daily iv. Forty-two patients were enrolled in the study, 18 in group NAC and 24 in the control group. The baseline SCr and baseline GFR did not differ between groups (1,19±0,33 vs 1,37±0,49 mg/dL; and 64,6±26,22 vs 65,7±28,32 ml/min), NAC vs control, respectively, p=0,17 e p=0,90. The incidence of AKI defined as 25% increase in baseline SCr was 36% (13/36), but it was not statistically different between groups (7/14, 50% in NAC and 6/22, 27.3% in the control group, p=0,16). Hospital mortality was 23% (10/42) and was not different (p=0,209) in NAC group (33,3%) when compared with control (16,7% ). Likewise, the length of ICU stay did not differ (2,93±1,53 vs 2,52±1,36 days, p=0,40). This study suggests that although a biological effect of NAC cannot be excluded, its use in the prevention of AKI associated with elective aortic aneurysms cannot be justified

Specific features of the aortic endothelium in a murine model of Marfan Syndrome / Spécificités de l’endothélium aortique dans un modèle de souris MarfanEgana, Isabel 31 October 2013 (has links)
La formation de podosomes dans les cellules endothéliales (Ces) aortiques en réponse au TGFβ est bien décrite in vitro (dans des boîtes de culture) et ex vivo (dans des segments de vaisseaux aortiques vivants). Le projet de cette thèse était de franchir l’étape suivante en démontrant la présence de podosomes in vivo en réponse à du TGFβ d’origine endogène. Pour ce faire, nous avons choisi d’utiliser un modèle de souris génétiquement modifiées présentant spontanément des niveaux de TGFβ élevés dans la paroi aortique, raisonnant que cet environnement devrait être propice à l’apparition des structures. La souris Marfan (FbnC1039G/+) constitue le modèle murin de la maladie humaine appelée « syndrome de Marfan ». Chez la souris comme chez l’homme, une mutation dans le gène codant pour la fibrilline-1 conduit à une augmentation des niveaux de TGFβ dans la paroi aortique et dans le sang circulant. Nous avons utilisé ce modèle pour rechercher la présence de podosomes dans l'endothélium aortique. La visualisation « en face » de l'endothélium marqué pour l’actine fibrillaire, la cortactine, la protéine adaptatrice Tks5 et la metalloprotéase MT1-MMP, a révélé des rosettes de podosomes semblables à celles détectées ex vivo en réponse à du TGFβ d’origine exogène. Ces rosettes peuvent être trouvées dans l'endothélium de l'aorte ascendante ou descendante. L’analyse du tissu sous-jacent révèle une détérioration de la lame basale qui apparait parsemée de zones dépourvues d’un de ses constituants majeurs, le collagène IV. Nous émettons l'hypothèse que les rosettes de podosomes sont impliquées dans la formation de ces zones de dégradation du collagène IV. Pour examiner les conséquences du déficit en fibrilline-1 pour les CEs et confirmer les données in vivo concernant la souris Marfan, nous avons utilisé deux approches in vitro. D’abord, nous avons mis au point un protocole pour isoler les CEs aortiques à partir des souris Marfan. Ensuite, nous avons, par la stratégie des siRNA, procédé à la déplétion en fibrilline-1 de CEs aortiques d’origine bovine (BAEc). Les cellules endothéliales aortiques isolées des souris Marfan conservent in vitro le phénotype « TGFβ » et forment podosomes capables de dégrader la matrice extracellulaire sans aucune stimulation exogène. Dans le modèle des cellules BAE, le dosage du TGFβ indique que, in vitro, la déplétion de ces cellules en fibrilline-1 provoque une augmentation de TGFβ biologiquement actif associé à la fraction cellulaire. Ce TGFβ conduit alors à la formation de podosomes dans les CEs. Nous avons observé d’autres conséquences du déficit en fibriline-1 dans l’endothélium in situ. L’analyse topologique de la surface de l’endothélium aortique de la souris Marfan révèle des phénomènes de « blebbing », des filopodes, des anomalies des jonctions cellules-cellules. L’analyse ultrastructurale en microscopie électronique à transmission révèle que l’endothélium de la souris Marfan a une apparence radicalement distincte de celui observé chez les témoins sauvages. La lame élastique, fragilisée par le déficit en fibrilline-1, disparaît comme digérée, par endroits. On observe une sécrétion anormale de matrice extracellulaire. Les cellules présentent un phénotype activé ou des signes d’apoptose. Ces études fournissent la première démonstration de l'apparition de podosomes endothéliaux in vivo et suggèrent leur implication en physiopathologie vasculaire. Elles fournissent également la première démonstration que l’endothelium aortique est profondément modifié dans le modèle murin de la maladie de Marfan. / Podosome formation in aortic endothelial cells exposed to TGFβ has been well described in vitro (in tissue culture dishes) and ex vivo (in living aortic vessel segments). The aim of the project was to go to the next step and demonstrate the occurrence of podosomes in vivo in response to endogenous TGFβ. For this purpose, we chose to use a genetically engineered mouse model, which spontaneously presents high TGFβ levels in the aorta, reasoning that this would be a favorable environment for these structures to appear. Marfan mice represent the murine model of the human disease Marfan syndrome. Similar to the human disease, a mutation in the fibrillin-1 gene (C1039G/+) leads to enhanced TGFβ levels in the aortic wall as well as in circulating blood. We therefore used the Marfan mouse model in search of podosomes in the aortic endothelium. “En face” viewing of the endothelium stained for filamentous actin, cortactin, Tks5 adaptator protein and MT1-MMP metalloprotease detected podosome rosettes with features similar to those detected in the ex vivo situation. Podosome rosettes were found in both descending and ascending aorta. Analysis of the underlying tissue with collagen IV staining revealed a basement membrane scattered with staining-free patches, most likely corresponding to collagen IV degradation. We propose that podosome rosettes are involved in basement membrane degradation in this mouse. To examine the consequences of fibrillin-1 deficiency in endothelial cells and confirm the data obtained in vivo in Marfan mouse aorta, we used two in vitro approaches. First, we set up a protocol to isolate aortic endothelial cells from Marfan aortas, second, we depleted fibrillin-1 from BAE cells by siRNA silencing. Isolated Marfan aortic endothelial cells retained in vitro the TGFβ activated phenotype and formed functional podosomes without any exogenous stimulation. TGFβ levels measurements in fibrillin-1 depleted aortic endothelial cells confirmed that fibrillin-1 deficiency triggers an increase in active, cell associated TGFβ, which in turns, leads to podosome formation in endothelial cells. Finally we studied other alterations caused by the fibrillin-1 defect at the endothelial cell level in situ. Topological analysis of the Marfan mouse aortic endothelium monolayer revealed cell blebbing, numerous filopodia and showed altered cell-cell junctions. At the ultrastructural level, transmission electronic microscopy revealed that the Marfan mouse endothelium had an appearance dramatically distinct from that observed in control littermates. The elastic lamina, weakened by fibrillin-1 deficit, disappeared in some places. The vessel wall also showed abundant extracellular matrix proteins. Endothelial cells presented an activated or apoptotic phenotype. These studies provide the first demonstration for the occurrence of endothelial podosomes in vivo and suggest their involvement in vascular physiopathology. In addition, they provide evidence that the aortic endothelium is profoundly altered in the murine model of Marfan syndrome.

A degradação da matriz extracelular e de proteínas da parede aórtica associada à inflamação é uma das principais características dos aneurismas da aorta abdominal (AAA). O objetivo deste trabalho foi investigar a participação das metaloproteinases 2 e 9 na formação de AAAs, através de um modelo experimental inédito de indução de AAA em ratos Wistar, desencadeado por duas potenciais causas de secreção e ativação de MMP-2 e 9: alteração do fluxo sanguíneo e lesão vascular externa na aorta. A formação de aneurismas foi observada em 60%-70% dos animais, apresentando diâmetro de 7 a 8 vezes maior que o diâmetro normal da aorta. Histologicamente observou-se remodelamento, intensa resposta inflamatória, destruição maciça de fibras elásticas e aumento da síntese de colágeno na parede aórtica. A expressão de ambas as formas de MMP-2 foram observadas tanto nos AAAs como nos grupos controles, neste com menor atividade, entretanto a expressão da pró e da MMP-9 ativa foram encontradas apenas nos AAAs. Conclusão: Nossos resultados sugerem que tanto as MMP-2 quanto as MMP-9 apresentam importante papel no desenvolvimento de AAA e este novo modelo de indução de AAA, pode ajudar a elucidar os mecanismos que desencadeiam a secreção e ativação das MMP-2 e MMP-9 na formação de aneurismas. / Degradation of extracellular matrix and proteins associated with inflammation of the aortic wall is the main characteristics of the abdominal aortic aneurysms (AAA). The aim of this study was investigate the participation of Metalloproteinase 2 and 9 in AAA formation in Wistar rats. A novel experimental model of AAA was developed, providing two potential causes of MMPs secretion and activation, turbulent flow (caused by surgically induced extrinsic stenosis) and outside vascular injury is detailed described. The days analyzed were the 3rd and the 7th post surgery. Aneurysms were observed to occur in 60-70% of the Group AAA, exhibiting a major transversal diameter to 7 from 8 times larger than controls and sham groups. Histologically, the aneurysms wall showed extensive structural remodeling, intense inflammatory response, massive elastic fibers destruction and abundant collagen deposition. Increased pro- and active MMP-2 was demonstrated in the AAA and controls groups, whereas pro- and active MMP-9 were found to be expressed only in the AAA group. Conclusions: MMP-2 and MMP-9 may have a pivotal role in the development of experimental AAA. This model can help to elucidate the mechanisms which trigger off MMP-2 and MMP-9 secretion and activation causing aneurysms.

Modelo suino de aneurisma de aorta abdominal com bolsa de pericárdio bovino / The aortic pericardium sac: new model of abdominal aortic aneurysm in pigsPerini, Sílvio César January 2008 (has links)
Introdução: A correção do aneurisma pela técnica endovascular ganha mais espaço com o aprimoramento do material utilizado, buscando-se um sistema ideal. Os estudos com esses dispositivos necessitam de modelos experimentais que devem apresentar características anatômicas e fisiopatológicas similares aos aneurismas em humanos. Apresentamos um novo modelo com bolsa de pericárdio bovino em suínos, com abordagem cirúrgica por via retroperitonial. Materiais e métodos: Estudo prospectivo, de coorte, avaliado pelo comitê de bioética, utilizando 11 suínos de raça Large White e do sexo feminino, com idade de 12 semanas, dividido em duas fases. A fase I consistiu na confecção do aneurisma, abordando a aorta retroperitonial com anestesia geral; na fase II, após 15 dias, realizou-se a arteriografia, eutanásia e retirada da peça para estudo anatomopatológico. Resultados: Em todos os animais, observou-se formação de trombos murais, como ocorre em aneurismas verdadeiros que comprometem de 30 a 90% da luz. À microscopia foram observadas calcificações na região do trombo em 82% dos animais. Havia infiltrado linfoplasmocitário no enxerto e região perienxerto, com fibrose em nove animais. Em três suínos havia espessamento miointimal considerável, e em oito, microcalcificações. A mortalidade foi nula, sem complicações cirúrgicas locais, isquêmicas e de ruptura. Conclusão: O modelo corresponde ao objetivo inicial de manter a aorta, seus ramos colaterais e terminais pérvios, com achados estruturais semelhantes aos de aneurismas encontrados em humanos: reação inflamatória parietal, periadvencial, trombo e calcificações da parede. As técnicas anestésica e operatória desenvolvidas foram satisfatórias e de morbimortalidade nula no período avaliado. / Background: The correction of an aneurysm using endovascular techniques has gained importance with the improvement of materials. The study of endovascular prostheses requires experimental models that reproduce anatomic and physiopathologic characteristics of human aneurysms. This study describes a new swine model that used bovine pericardium and a retroperitoneal surgical approach. Material and methods: This prospective cohort study used eleven Large White female pigs with a mean age of 12 weeks in the two study phases. In phase I, the aneurysm was produced and surgery using a retroperitoneal approach to the aorta under general anesthesia was performed; and in phase II, 15 days later, the animals underwent arteriography and were later killed, and specimens were removed for histological analysis Results: The formation of parietal thrombus, which is also found in true aneurysms that involve 30 to 90% of the lumen, was observed in all animals. Microscopic analysis showed calcifications in the area of thrombus in 82% of the animals. There was lymphoplasmocytic infiltration in the graft and adjacent area, with fibrosis in nine animals. Three pigs had substantial myointimal thickening, and eight had microcalcifications. Mortality was zero, and there were no ruptures, ischemia or surgery site. Conclusion: The aorta and its collateral and terminal branches remained healthy, and structural findings were similar to those observed in human aneurysms: wall inflammation, periadventitial thrombus and wall calcification. The anesthetic and operatory techniques were satisfactory, and there was no morbidity or mortality during the study.

The vascular variability of the iliac system and clinical diagnosis in radiology and neurologyAl Talalwah, Waseem January 2013 (has links)
The sciatic nerve is the largest nerve in the human body giving both motor and sensory innervations to the lower limb. It can be affected in chronic diseases, such as diabetes, or compressed anatomically by structures such as piriformis and aneurysms leading to sciatica or paralysis of the lower limb. The current study therefore focuses on the arterial supply of the sciatic nerve as well as its course. Embryologically, the sciatic nerve is supplied via the axial artery during the first trimester. As the axial artery regresses, the iliac system develops. A failure of sciatic artery regression leads to several variations of pelvic and femoral arteries, with a risk of iatrogenic injury/trauma for those patients undergoing pelvic, gluteal and thigh surgical procedures. An understanding of the variability of the pelvic arteries in relation to a coexistent sciatic artery will provide an appropriate background for clinicians. The present study proposes a new theory of sciatic artery development and persistence, as well as new theories for the superior and inferior gluteal, internal pudendal and obturator arteries. The thesis is in two parts: first an anatomical study on the dissection of 171 cadavers including the pelvic, gluteal and thigh regions to observe (i) the patterns of the arteries these regions, and (ii) the course of the sciatic nerve. With variable course of sciatic nerve, there is a variability of its blood supply. Moreover, it includes a new classification of sciatic nerve with respect to clinical implications. The thesis clarifies the origins of the sciatic artery and its course. The second part is a literature review of sciatic artery aneurysm cases in 171 patients, which clarifies the risk of aneurysm, together with its incidence with respect to pathologic finding and associated disorders. Radiologists have to be aware of the internal iliac artery classifications to be able to alert general surgeons, orthopaedic surgeons, obstetricians, gynecologists, and urologists so that they can improve patient management.

Biomarcadores genéticos na hemorragia subaracnoidea aneurismática em pacientes da Amazônia / Genetic biomarkers in patients with aneurysmal subarachnoid hemorrhage in the Amazon patientsPaschoal, Eric Homero Albuquerque 24 August 2017 (has links)
Hemorragia subaracnoidea aneurismática (HSAa) é considerada causa importante de morte e de sequelas neurológicas. A taxa de mortalidade desta doença pode alcançar 50% nos primeiros dois meses após sangramento de aneurisma encefálico. Apesar dos avanços científicos da modernidade, o resultado do tratamento da HSAa não mudou nos últimos anos. O presente estudo avaliou o papel de 14 biomarcadores genéticos, incluindo o polimorfismo (SNP) do gene eNOS, em pacientes da Amazônia com HSAa, para verificar as alterações alélicas associadas ao risco de vasoespasmo encefálico e déficit neurológico tardio. Avaliou-se a ancestralidade desta amostra de pacientes em que se utilizou 48 marcadores para identificar possível etnia associada à predisposição ao VE. Investigou-se 14 biomarcadores genéticos no tocante à resposta inflamatória encefálica na HSAa. Foram avaliados 265 doentes que foram divididos em dois grupos: grupo I (pacientes com vasoespasmo encefálico) e grupo 2 (pacientes sem vasoespasmo). A média das idades foi 51 anos, havia 224 mulheres (84%) e 124 pacientes (46,79%) apresentaram vasoespasmo encefálico (VE). A maior incidência de VE ocorreu na idade entre 50 e 59 anos. Tabagismo e hipertensão arterial sistêmica foram os fatores de risco mais associados à VE. Aneurismas encefálicos de tamanho pequeno e médio predominaram nesta casuística. As escalas amarela e vermelha do VASOGRADE associaram-se ao risco de VE (p < 0,001). Não houve variação na distribuição ancestral entre os grupos estudados e o que ocorre na população brasileira saudável na região Amazônica. O gene da eNOS com seus respectivos polimorfismos T-786C e 27VNTR4 correlacionaram-se com VE. Outros marcadores observados foram TP53, CASP8, ACE2, IL4 e XRCC1. O gene TP53 (modelo recessivo alelo 1) mostrou-se ser um fator protetor de VE, enquanto que genes com mutações INDEL CASP8 (modelo recessivo alelo 2) e o XRCC1 (modelo recessivo alelo 1) mostraram tendência ao desenvolvimento de VE com risco 2 vezes maior e 1,4 vezes maior que o grupo II (p < 0,001). Conclui-se que SNPs da eNOS se correlacionam com desenvolvimento de VE sintomático pós-HSAa. Este estudo também mostrou o papel dos marcadores inflamatórios na HSAa, o que auxiliaria na condução da terapia clínica. / Aneurysmal subarachnoid hemorrhage (aSAH) is a leading cause of premature death and neurological disability. It is considered as a devastating condition that accounts to 50% of mortality during the first two months after a hemorrhagic event. Despite foremost advances in the clinical management of post-aSAH patients, the rates of mortality and morbidity have not changed in recent years. This study appraised the role of 14 genetic biomarkers, including the eNOS polymorphism (SNP) between Amazon\'s patients with aSAH, as means to document how variant alleles are related to a higher disposition to cerebral vasospasm (CV) and delayed cerebral ischemia (DCI). 265 patients were evaluated and then divided into two clusters: Group I (with symptomatic CV) and group II (presenting no symptomatic CV). The median ages of patients were 51.61 years of age, 224 (84.52%) were women and 124 patients (46.97%) had symptoms of cerebral vasospasm (CV). Tobacco smoking and systemic arterial hypertension are the risk factors most associated to CV. In the course of this research, most aneurysms found were small and medium-sized. The score VASOGRADE yellow and VASOGRADE red presented a high risk of CV (p < 0.001). We established a panel of 48 ancestry informative markers for estimating which ethnicity could present a predisposition to CV. There was no variation in the ancestral distribution between study groups and healthy brazilian folk over the Amazon region. The eNOS gene with its polymorphisms T-786C and 27 VNTR4 were correlated to CV. Other markers were accomplished: TP53, CASP8, ACE2, IL4, and XRCC1. The TP53 gene (recessive genetic model allele 1) supporting evidence of the protective role to CV. Whilst other genes with INDEL mutation like as CASP8 (recessive model allele 2) and the XRCC1 (recessive model allele 1) indicated a propensity to spread out CV with odds 2-fold higher, and 1.414 times greater than group II (p < 0.001). It follows that eNOS SNPs correlate to a positive association with a syntomatic CV post-aSAH. Also, this study showed up the role of inflammatory markers at aSAH to a further educated therapeutic choice for a better clinical response
