<p>Acute myocardial infarction is a serious diagnosis. Both the patient and the loved ones experience an immediate uncertainty. To be able to emotionally support the patient´s spouse, the nurse in the coronary care unit must be prepared to also encounter the world of the spouse. The aim of this study was from a nursing perspective to illuminate how life was experienced during the first two months by the spouse of someone, who was stricken by a first time acute myocardial infarction. Interviews with six spouses were conducted. The methodological approach was phenomenological-hermeneutic, based on the French philosopher Paul Riceour. The result of the qualitative text analysis showed four themes: To lose one´s foothold, To be exposed to the medical and nursing staff, To get a changed relationship to one´s partner, To wish to make the most of one´s life. The first theme highlighted the spouse’s strong feelings of uncertainty concerning the outcome of the patient. The second theme showed the spouse’s confidence in the medical care. It also showed the spouse’s sense of being insulted by the nurses, who did not acknowledge the spouses´ need of obtaining information and emotional support. The third theme demonstrated how the partner cared for and felt responsible for observing the former patient´s health condition. The fourth theme showed that the spouse realized that life has to come to an end and therefore wanted to realize his/her plans for the future before it was too late. The findings from this study emphasize the importance of the medical and nursing staff to show interest in the spouses´ experiences. Thereby the spouses can experience a support in congruence with their needs, and their life situation might thereby be improved. It would be of great interest to study how nurses regard the spouses´ life situation during the patients´ stay in hospital. Further research could also highlight which factors could minimize the sufferings of partners to seriously sick patients, regardless of the patients´ diagnosis.</p>

Bakgrund Många mammor som fött barn prematurt önskar amning som uppfödning för sitt barn för bröstmjölkens immunologiska faktorer och som en kompensation för att graviditeten avslutats för tidigt. Amningens etablering kan vara komplicerad och kräva tålamod när barnet behöver vård. Beroende på barnets känslighet kan det ta olika lång tid tills barnet kan påbörja amningsträning. En av de viktigaste faktorerna för att amningen ska kunna etableras är att mamman får igång en tillräcklig mjölkproduktion motsvarande den mängd ett fullgånget barn skulle kräva. För att få igång en tillräcklig produktion är det bra att komma igång med urmjölkning så fort som möjligt efter barnets födelse. Mamman behöver stimulera mjölkproduktionen tills barnet ammar fullt. Syftet med studien var att belysa innebörden av urmjölkning för mammor vars barn fötts prematurt. Metod Det strategiska urvalet utgörs av sju mammor vars barn fötts före vecka 34 med erfarenhet av urmjölkning. Datainsamlingen består av öppna intervjuer och materialet har analyserades med en fenomenologisk hermeneutisk ansats. Resultatet visar att urmjölkning utförs i en oreflekterad ansträngning men att mammor är måna om de egna behoven samtidigt som behoven åsidosätts. Vårdkontexten kan kännas utlämnade och begränsande. Självtilliten får betydelse liksom strukturen. När utfallet inte blir som planerat kan sorgen innebära att förhålla sig till det som inte kan förändras. Känsla av hanterbarhet, begriplighet och meningsfullhet kan få betydelse för processen. Slutsats Ur ett vårdvetenskapligt perspektiv blir det betydelsefullt att utgå från den enskilda mammans upplevelser av urmjölkningen och stärka en god självtillit. / Background Many mothers who have given birth premature wish to breast feed their children for immunological reasons and as a compensation for a pregnancy that ended too early. The breast feeding may be complicated and in need of great patience as the child might need health care. Depending of the baby´s sensitivity it can vary in time for a child to start breast feeding. One of the important reasons for the mother to get her breast feeding started is to make milk production start, corresponding to the amount of milk that a fully mature baby would need. To get a sufficient production it is important to start emptying the breasts quick as possible after having given full extend. Aim with the study was to illuminate the meaning of emptying the breasts for mothers whos baby was born premature. Method Seven mothers who had given birth before week 34 and had experienced emptying breasts. The data collection come from interviews and the results have been analyzed by a fenomenological hermenutichal approach. Results shows that emptying the breasts is done without any effort but that mothers are eager to look at their own needs at the same time as their own needs are being put aside. The hospital care can make the women feel left out and controlled. Self-confidence gets important as well as the structure. When the result is not as you had planned sadness make you realize that things cannot be changed. The feeling of managing, understanding and meaningfulness fills you up. Conclusions looking at it from a scientific perspective it is very important to consider the individual mothers experience in how to empty the breasts from milk and how to strengthen her self-confidence.

Spiritual Life Review With Older Adults: Finding Meaning in Late Life DevelopmentStinson, Alicia Margaret 01 January 2013 (has links)
ABSTRACT
Spirituality has been recognized as a positive factor in the lives of older adults, especially as it influences their emotional, mental, and physical well-being. This convenience sample study included 17 older adults residing at a faith based continuing care retirement community in Florida. The sample was represented by Caucasian older adults with an average age of 84 years, highly educated, majority Protestant and mostly female. Spiritual life reviews were conducted using spiritual life maps (Hodge, 2005) and semi-structured interview questions. Erikson's epigenetic stage of ego-integrity was used along with Butler's life review process and Tornstam's gerotranscendence as a conceptual framework for understanding late life development and spirituality in older adults. This mostly qualitative study used a hermeneutic phenomenological approach to analyze the responses to the open-ended interview questions about spirituality across the life-time. Ego-integrity was measured at the beginning and end of the spiritual life review study.
Paired t-tests found that participation in the spiritual life review did not influence the ego integrity scores of participants. Specifically, there were no statistically significant difference between the pre ego integrity score (M=82.94, SD= 8.235) and the post ego integrity score (M=84.47, SD= 7.551); t (16) = -.769 p= .453. However, in comparison, the qualitative analysis revealed that the spiritual life review does influence ego-integrity in some participants. Additionally, the spiritual life review confirms gerotranscendence and contributes to information about spiritual development in the lives of older adults. The conclusion offers a discussion about the study's limitations, strengths, implications for future research, and suggestions for clinical practice.

Virtue and inquiry, knowledge and ignorance: Lessons from the TheaetetusIngle, Jennifer F 01 June 2007 (has links)
Plato's dialogues are set in fifth century Athens but they are performed for a fourth century audience. The context of his dialogues, then is wider perhaps than other philosophers and because of the difference in periods, it is clear that it is necessary for an audience member to possess knowledge of the events of the previous generation, viz., the fifth century BCE. When its cultural context is taken into account, the Theaetetus can not be read as an attempt by Plato to establish an epistemology in the modern sense of the term. While the characters of the dialogue are searching for the 'essence' of knowledge, Plato is teaching the audience of the dialogue to consider the knowledge that different practices of paideia produce and to evaluate that knowledge in light of its implications on the individual and the polis. The answer that emerges is that philosophy is the paideia that will produce the best individual and the best polis, because it is only the practice of philosophy that teaches intellectual virtue. The Theaetetus is an account of the practice of philosophy and the practitioner of philosophy.

The Meaning of Discontent: A Multi-method Qualitative Investigation of Women's Lived Experiences with Body DissatisfactionRoss, Erin 14 January 2014 (has links)
This study explored adult women’s lived experiences with body dissatisfaction. Using a multi-methods qualitative approach incorporating in-depth semi-structured interviews and arts-based projects, women between the ages of 20-39 engaged in a critical exploration of their body experiences in order to deepen understanding of the psychological construct of body dissatisfaction and its ongoing influence in their lives. Ten women from diverse social and ethnocultural backgrounds took part in the study, completing 1-2 interviews, an in-session drawing exercise, and a creative project. Interview transcripts, drawings, and creative projects were analyzed for themes using an hermeneutic phenomenological approach. Four core categories emerged from the data. The first category contained the women’s understanding of the experience and meaning of body dissatisfaction. The second category captured the external reinforcement of body dissatisfaction and related body beliefs. The third emergent category delineated the impact of body dissatisfaction on daily life, including body-self relationships and interpersonal relationships. The final category captured the difficulties the women encountered as they attempted to overcome their feelings of body dissatisfaction and their negative body beliefs. This research highlighted the complex and multidimensional meaning of body dissatisfaction in adult women’s lives.

"LOST IN TRANSLATION" : Hur personer med ohälsa och lidande orsakat av sektmedlemskap upplever mötet med den psykiatriska vården.Klubb Skoglund, Sarita January 2014 (has links)
Studiens syfte var att undersöka hur personer med ohälsa och lidande orsakat av sektmedlemskap upplever mötet med den psykiatriska vården. Studien hade en fenomenologisk hermeneutisk ansats och byggde på fem intervjuer med personer från fyra olika sekter som i samband med processen att lämna sekten kommit i kontakt med den psykiatriska vården. Resultatet beskrivs på tre tolkningsnivåer; naiv förståelse, strukturanalys och tolkad helhet. Den naiva analysen visade att personer med ohälsa och lidande orsakat av sektmedlemskap upplevde att mötet med den psykiatriska vården präglades av missförstånd och brist på kunskap om problematiken och vilka konsekvenserna blev när en person valde att lämna en sekt. Strukturanalysen resulterade i elva subteman, tre teman och ett huvudtema. Huvudtemat fick namnet Lost in Translation och sammanfattade kärnan i studiens resultat vilket var att personer med ohälsa och lidande orsakat av sektmedlemskap kände sig vilse och utanför och hade svårt att förstå och få förståelse för sin situation vilket ledde till missförstånd i mötet med den psykiatriska vården. Den tolkade helheten visade att de personer som kände sig sedda och bekräftade i sitt lidande hade en mer positiv upplevelse av mötet med den psykiatriska vården. / The aim of the study was to investigate how persons with illness and distress caused by cult membership experienced the encounter with psychiatric care. The study had a phenomenological hermeneutic approach and was based on five interviews with people from four different cults who in the process of leaving the cult had contact with psychiatric care. The result are described in three levels of interpretation; naive understanding, structural analysis and interpreted whole. The naive analysis showed that persons with illness and distress caused by cult membership experienced that the contact with psychiatric care was characterized by misunderstanding and lack of knowledge of the problems and the consequences that were a result of a person leaving a cult. The structural analysis resulted in eleven subthemes, three themes and an overall theme. The main theme was called Lost in Translation and summarized the essence of the results of the study which was that persons with illness and distress caused by cult membership felt lost and outside and had trouble to understand and to find understanding for their situation and that this caused misunderstandings in the encounter with psychiatric care. The interpreted whole showed that persons that felt that they were seen and confirmed in their suffering had a more positive experience of the encounter with psychiatric care.

The present thesis reports an empirical study of how ten remedial teachers or special remedial teachers, all women, encountered reality as supervisors or tutors in schools during the 1990s. The study sought to define the conditions for supervision and tutoring in the remedial-teaching function. Data was collected in two steps. The preparatory step consisted of a scrutiny of Report DsU 1986:13 on the modified remedial-teacher function, and perusal of the referees’ comments thereon. The second step consisted ofinterviews with all ten participants three years after their training as supervisors/tutors at the Stockholm Institute of Education. Neither in the above Report nor in the referees’ comments was supervision defined or discussed. Despite this resistance, supervision is required by the macro level for the modified remedial-teacher function. Regarding the micro-level situation for supervising in school, two themes were distinguished: an organisational theme and a legitimacy theme. The informants saw school management, the organisation’s pre-understanding of supervision and the profession’s continual adaptation to areas of knowledge not possessed by teachers as important aspects. The legitimacy theme expresses how differentiation of the supervisory task and the design of teaching posts influence the implementation. The supervision/tutoring examined here is described through six experiential themes: the dialogical here-and-now-theme, the relational theme, the didactic ”technical” theme, the reflective and self-fostering theme, the change theme and the leadership theme. Remedial-educational supervision as described in the study appears as vertical but does not extend to the depth of therapeutic intervention. It is seen as an ethical act where consideration or recognition, rather than care, is a distinctive feature. Care may represent ”tacit” gender-loaded knowledge and needs to be discussed in educational contexts where supervision or tutoring is included in the special-remedial-teacher function.

La présente thèse a d'abord pour sujet la fonction participative reconnue à
l'interprète de la nonne, déniée par la doctrine classique au profit de la fiction qui
fonde l'absolutisation de l'intention de l'auteur de la nonne.
Faisant état des phases de l'avènement du sens de la loi dans le déploiement
du cercle hennéneutique, nous analysons la doctrine classique et, abordant
l'interprétation selon l'approche hennéneutique, nous dégageons et explicitons les
conditions propres à l'avènement de la juridicité tant au niveau de l'autonomie du
texte que de sa réception.
La présente thèse a également pour fin d'évaluer le poids des contraintes sur
l'interprétation et l'application à l'oeuvre en droit. Comment, dans le processus de
l'avènement du sens de la nonne, l'interprétation juridique s'édifie-t-elle et s'enrichitelle
des valeurs et des règles qui régissent la production du droit?
Pour analyser et caractériser le rôle du juge dans l'avènement du sens de la
nonne juridique selon une démarche dont la spécificité relève de la phénoménologie
et de 1'hennéneutique philosophique, nous choisissons le domaine des droits
linguistiques. L'analyse des données empiriques recueillies pennet de voir sur
quelles voies, au-delà de la prédétennination et de la co-détennination du sens de la
nonne, s'engage la «surdétennination» (concept propre à la théorie de «l'analyse
systémale» et selon lequel le sens de la nonne émerge aussi en référence au champ
de valeurs qui traverse la représentation de la réalité de la nonne) et dès lors d'en
illustrer concrètement les manifestations propres.
D'abord envisagée sous le règne de l'histoire, des principes fondateurs et des
enjeux liés à la dualité linguistique, l'étude du contentieux linguistique est
spécifiquement traitée sous l'angle des droits des minorités linguistiques et de la
représentation des valeurs privilégiées. L'analyse des mémoires déposés par les

parties et les intervenants dans les causes entendues en matière de droits linguistiques
révèle que les valeurs identitaires dominent les interventions analysées. L'analyse de
la réceptivité de la Cour et l'interprétation des juges confirme ensuite que la
surdétennination est à l' oeuvre.
Ce parcours théorique pennet que nous disposions d'une représentation
raisonnée du rôle du juge dans la construction du sens en droit selon laquelle les trois
moments du cercle hennéneutique - compréhension, interprétation, application s'unissent
dans l'avènement du sens de la loi en une dynamique circulaire et
anticipatoire propre à révéler les constituants du texte juridique.
L'analyse empmque confirme quant à elle la participation du récepteur et
interprète de la nonne sur le sens qui préside à son accession à la juridicité. / This thesis deals primarily with the participative function of the interpreter
of the law as it is acknowledged except by the c1assical doctrine that is more inc1ined
to see the intention of the lawmaker as the absolute detenninant.
Referring to the various stages in the meaning-making process through the
creation of the henneneutical circ1e, we analyze the c1assical doctrine and, while
dealing with interpretation of the law under the henneneutical approach, we put
forward and explain the very conditions where the legal phenomenon appears at the
level of the autonomy of the text and of its reception.
This thesis also intends to assess the impact of the construction and
enforcement constraints regarding the law. How, in the course of the meaningmaking
process, is legal interpretation built and enriched by the values and rules
which govem law-making?
To analyze and characterize the role of the judge in the making of the
meaning of the law with a specifie approach inspired by phenomenology and
philosophical henneneutics, we have chosen the area of language rights. The analysis
of the empirical data that was gathered enables us to see, beyond the predetermination
and the co-determination of the meaning of the law, where overdetermination
is headed (a specifie concept of the "systemal analysis" theory under
which the meaning of the law also emerges by reference to the value system
permeating the representation of the reality of the law) and therefore to concretely
illustrate its very expressions.
First approached from the perspectives of history, of the founding principles
and of the issues related to the linguistic duality, the linguistic case law is specifically

reviewed in the context of linguistic minority rights and of the representation of
preferred values. An analysis of submissions filed by the parties and the interveners
in linguistic rights cases shows the major role played by identity-related values in the
reviewed interventions. The analysis of the Court's responsiveness and the
interpretation of judges then confirm the existence of an over-determination.
This theoretical exploration allows us to have a reasoned representation of
the judge's role in the making of meaning in law through the three stages of the
hermeneutical circle - understanding, interpretation, application - that are combined
in a circular and anticipatory process aiming at revealing the constitutive elements of
the legal text.
The empirical analysis confirms the influence of the recipient and interpreter

Le cercle du comprendre et la conception heideggérienne de la philosophie dans Sein und ZeitVaillancourt, Richard 08 1900 (has links)
La conception heideggérienne du « cercle de la compréhension » exposée aux §§ 31 et 32 de Sein und Zeit est bien connue et a souvent été analysée, notamment par Hans-Georg Gadamer dans Vérité et méthode (1960) et Jean Greisch dans son commentaire de Sein und Zeit (1994). Mais on a trop peu vu que la théorie du cercle herméneutique déterminait aussi la conception heideggérienne de la philosophie. Notre recherche s’efforcera de le montrer et d’expliquer pourquoi il en est ainsi. Au plan de la méthodologie, nous proposerons, en nous inspirant des commentateurs les plus autorisés (Greisch, Gadamer, Courtine, et al.), un commentaire serré des §§ 31 et 32 de Sein und Zeit, sans oublier le § 63 où Heidegger souligne lui-même la « situation herméneutique » de sa propre enquête, ni le § 68 où se trouve mise en évidence la dimension temporelle du cercle de la compréhension. Nous tâcherons ensuite de montrer en quoi ce cercle affecte la conception heideggérienne de la philosophie telle que présentée au § 7 de Sein und Zeit et dans d’autres textes de Heidegger tels que Les problèmes fondamentaux de la phénoménologie (GA 24). / Heidegger’s understanding of the « hermeneutic circle » presented in §§ 31 et 32 of Sein und Zeit is well known and was analysed, among others, by Hans-Georg Gadamer in Truth and Method (1960) and Jean Greisch in his commentary of Sein und Zeit (1994). However, what has received too little attention is the extent to which the hermeneutic circle theory has a determining influence on Heidegger’s idea and practice of philosophy. Our research will attempt to explain why this is so. In our methodology, guided by the most relevant commentators (Greisch, Gadamer, Courtine, et al.), we elaborate a close analysis of §§ 31 and 32 in Sein und Zeit. This commentary also takes into account § 63, where Heidegger himself underlines the « hermeneutic situation » of his own investigation and § 68 where the temporal dimension of the hermeneutic circle is pointed out. We conclude by trying to demonstrate in what respect the circle influences Heidegger’s interpretation of philosophy, as shown in § 7 of Sein und Zeit and in other texts of Heidegger, such as The Fundamental Problems of Phenomenology (GA 24).
