Bakgrund Demens är ett samlingsnamn för flera kognitiva sjukdomar. Det är en obotlig sjukdom som blir allt vanligare i hela världen. Vårdandet består främst av olika omvårdnadsåtgärder som bör anpassas efter individens behov. Ett vanligt problem är att personen med demenssjukdomen blir allt mer begränsad i det senare sjukdomsförloppet. I samband med sjukdomsprogressionen är det vanligt att deras autonomi begränsas till följd av bristande kommunikation. Personer med demenssjukdomar trivs som bäst inom hemmiljön. Vård inom akutsjukvården är även vanligt förekommande, trots att den sällan är anpassad för patientgruppens behov. Syfte Syftet var att beskriva sjuksköterskors erfarenheter av att vårda personer med demenssjukdomar inom akutsjukvård. Metod En litteraturöversikt utformades utifrån information hämtat från kvalitativa och kvantitativa artiklar från databaserna Cinahl Complete och PubMed. Dataanalysen utfördes enligt Fribergs fyra steg. Resultat Författarna fann fyra teman i resultatet; en otrygg vårdmiljö, kommunikation mellan patient och sjuksköterska, ett gott bemötande samt organisationens påverkan. Ur dessa teman framkom det att miljön inom akutsjukvården inte var anpassad för personer med demenssjukdomar. Utöver det beskrev deltagarna vikten av att kunna föra god kommunikation och att bemöta patienten med respekt och förståelse. Slutsats Vårdmiljön inom akutsjukvården ansågs vara svåranpassad. Bristfällig kommunikation mellan sjuksköterska och patient kunde leda till att patientens autonomi begränsades. Ett gott bemötande genomsyrades av ett personcentrerat förhållningsätt. Slutligen framkom det att samarbetet inom arbetsteamet ansågs vara bristande och att adekvat kunskap saknades. / Background Dementia is the collective name of several cognitive disorders. Dementia is an incurable disease that is becomming increasingly more common throughout the world. Care mainly consists of various nursing measures adapted to the needs of the individual. A common issue is that the person with the disease becomes increasingly limited in the later stages of the disease. With the progression of the disease, their autonomy is commonly limited as a result of a lack of communication. People with dementia feel the best in a homelike environment, care within an acute care setting is also common, despite the fact that it is seldom a care setting adapted to the patient groups needs. Aim The aim of this study was to describe nurses experiences of caring for people with dementia in an acute care setting. Method A literary review was designed based upon information from qualitative and quantitative articles from the databases Cinahl Complete and PubMed. The data analysis was performed according to Friberg's four steps. Results The authors found four themes in the results; an unsafe care environment, communication between the patient and nurse, good treatment, organizational impact. From these themes it was concluded that the environment within a emergency healthcare setting was not adapted for people with dementia. In addition, the study participants described the importance of being able to communicate with and to treat the patient with respect and understanding. Conclusions The emergency care environment was considered difficult to adapt. Poor communication between nurses and patients could lead to restrictions of the patients autonomy. Good treatment was characterised by a person-centred approach. Lastly, it was considered to be inadequate co-operation within the workforce and that there was a lack of adequate knowledge.

A Narrative Study of the Older Patient and Family Caregiver Experience of Alternate Level of CareCressman, Greg 10 1900 (has links)
<p>Alternate Level of Care, or ALC, is a designation assigned to a patient during an acute care hospital stay when that patient is occupying a bed but no longer requires that intensity of service. A high proportion of beds occupied by patients requiring ALC results in problems in the healthcare system related to patient flow, access, and efficiency. The purpose of this study was to better understand the experience of older patients and family caregivers while the patient is designated as requiring ALC. This study employed the Three-Dimensional Narrative Inquiry Space method of Clandinin and Connelly (2000) to examine the experiences of five patients and four family caregivers. The primary data source for this study was a series of semi-structured interviews with individual participants over time while they were in the midst of the experience of ALC. Data analysis involved a twofold process of restorying all interview transcriptions for a given participant into a single story summarizing his or her experience, and analyzing the content across stories to identify common themes. The major finding of this study was that uncertainty was a significant part of the experience of ALC. This uncertainty was manifest in relation to self-concept, waiting for placement, not knowing about information, not being included in planning, lack of mobility, and lack of meaningful activity. The study findings have implications for strategies to manage uncertainty in the areas of practice, policy, education, and research in order to improve the experience of older patients and their family caregivers.</p> / Master of Science (MSc)

Orientador: Erika Veruska Paiva Ortolan / Resumo: Introdução: A classificação não eletiva de cirurgias em emergências e urgências é subjetiva e altamente variável entre os cirurgiões. Na tentativa de padronizar essa classificação e diminuir a subjetividade, o Grupo de Estudos da Sociedade Mundial de Cirurgias de Emergência, baseado na experiência adquirida em catástrofes, propôs a classificação para cirurgias de urgência em uma escala de cores que pudesse ser mais facilmente reproduzível. Este estudo avaliou a aplicabilidade do sistema Timing of Acute Care Surgery (TACS - Classificação para atendimento de cirurgias de urgência) em um hospital público terciário de um país em desenvolvimento. Métodos: Este foi um estudo longitudinal, ambispectivo, analítico, de centro único, de março a agosto de 2016 e 2017. Foram selecionadas quatro especialidades cirúrgicas; seus chefes classificaram previamente os seus procedimentos cirúrgicos de urgências e emergências de acordo com o sistema TACS. Para comparação, emergências foram consideradas como vermelhas e laranjas e urgências como amarelas, com tempo ideal para a cirurgia de 1 hora e 6 horas, respectivamente. Resultados: Os procedimentos não eletivos foram 61% do total de cirurgias. Houve predomínio das cores, vermelho, laranja e amarelo. Ocorreram várias alterações nas cores de forma ascendente. In an attempt to standardize this classification and diminish the subjectivity, the World Emergency Surgery Study Group, based on the experience gained from catastrophes, proposed the classification for emergency surgeries in a colour scale that could be more easily reproducible. This study evaluated the applicability of the Timing of Acute Care Surgery (TACS) system in a public tertiary hospital in a developing country. Methods: This was a longitudinal, ambispective, single-centre study from March to August of 2016 and 2017. Four surgical specialties were selected; their chiefs previously classified all the acute surgical procedures based on TACS. For comparison, emergencies were considered as red and orange and urgencies as yellow, with ideal time to surgery of 1 hour and 6 hours, respectively. Results: The non-elective procedures were 61 % of the total number of surgeries. Red, orange, and yellow were predominant. There were several changes in colours in an ascending manner. There was no difference in the ratio between ideal time to surgery and actual time to surgery before and after the TACS system. There was an improvement of the time to surgeries in the yellow colour after the TACS system. Night and day times influenced the time to surgery, with better results during nights. Improving Fall Prevention Strategies in an Acute-Care SettingBoye-Doe, Sylvia B. 01 January 2017 (has links)
Falls with or without injuries among the elderly have become a public health concern, with falls among adults age 65 years and older increasing every year. Nurses play a role in ensuring patient safety by following fall prevention guidelines. The purpose of this evidence-based study was to implement the RE-AIM evaluation tool to determine the impact of the Safe Five program on staff compliance with the program; patients' awareness of the need for falls prevention; and falls among older adults, ages 65 years and older, admitted to an acute care nursing unit. The literature supports implementing a falls prevention program with multifactorial and interdisciplinary components, and an evaluation plan to help decrease falls in acute care settings. The Safe Five falls prevention program was implemented on the acute care nursing unit in an effort to decrease the inpatient falls rate on the unit. The inpatient falls data were collected retrospectively, 2 years pre implementation of the Safe Five program, and 8 to 10 months post implementation. Data were collected from the Safe Five checklists, recorded inpatient fall rates, and high fall risk chart audits provided by staff and nurse manager; they were then analyzed using the RE-AIM evaluation tool. The long-term effects of the Safe Five program include an 18% increase in patients' awareness of the importance of preventing falls, an 18% increase in staff compliance with the program, and a 14% decrease in inpatient fall rate on the unit. It is projected that the decrease in inpatient falls will result in decreased healthcare costs and improved patient satisfaction with the healthcare system, communication among the interdisciplinary team, and health outcomes for the patients.

Best Practices for Glucose Management Using a Computer-Based Glucose ManagementJackson-Cenales, Oteka 01 January 2017 (has links)
The prevalence of diabetes mellitus (DM) continues to be a global concern among health care practitioners. Without collaboration and interventions, this chronic disease, which poses a significant financial burden for health care institutions, will continue to be problematic. Promoting the use of glycemic control measures among diabetic patients is an intervention, which has the potential to reduce diabetic complications and improve outcomes. The purpose of this doctoral project was to explore available evidence through a systematic review of the best practices for glucose management. The chronic care model served as the theoretical framework. The evidence based practice question was, What is the current evidence supporting the utilization of a computer-based glucose management system (CBGMS) for inpatient diabetic adults in acute and critical care settings? A systematic review was conducted, yielding 532 studies in which 3 of the studies related to CBGMSs published from 2008 to 2017 were critically appraised. The John Hopkins Nursing Evidence Appraisal Tool with specific inclusion and exclusion criteria was utilized. Participants were adult patients (aged 18 and over) with DM in inpatient care settings who were English speaking. Interventions included the traditional paper-based sliding scale regimen versus the utilization of a CBGMS. Outcome measures included decreased length of stay, reduced cost, and glucose optimization. A conclusion was the implementation of a CBGMS has the potential to improve patient outcomes with additional research that exhibits overall benefits and implement into practice. Thus, implementation of a CBGMS can lead to positive social change by aiding in a change in practice that will ultimately ameliorate patient health outcomes.

How privatization and corporatization affect healthcare employees’ work climate, work attitudes and ill-health : Implications of social statusFalkenberg, Helena January 2010 (has links)
Political liberalization and increased public costs have placed new demands on the Swedish public sector. Two ways of meeting these novel requirements have been to corporatize and privatize organizations. With these two organizational changes, however, comes a risk of increased insecurity and higher demands on employees; the ability to handle these changes is likely dependent on their social status within an organization. The general aim of the thesis is to contribute to the understanding of how corporatization and privatization might affect employees’ work climate, work attitudes and ill-health. Special importance is placed on whether outcomes may differ depending on the employees’ social status in the form of hierarchic level and gender. Questionnaire data from Swedish acute care hospitals were used in three empirical studies. Study I showed that physicians at corporatized and privatized hospitals reported more positive experiences of their work climate compared with physicians at a public administration hospital. Study II showed that privatization had more negative ramifications for a middle hierarchic level (i.e., registered nurses) who reported deterioration of work attitudes, while there were no major consequences for employees at high (physicians) or low (assistant nurses) hierarchic levels. Study III found that although the work situation for women and men physicians were somewhat comparable (i.e., the same occupation, the same organization), all of the differences that remained between the genders were to the detriment of women. The results of this thesis suggest that corporatizations and privatizations do not necessarily imply negative consequence for employees. However, the consequences appear to differ between groups with different social status. Employees whose immediate work situation is affected but who do not have sufficient resources to handle the requirements associated with an organizational change may perceive the most negative consequences. / At the time of the doctoral defense, the following papers were unpublished and had a status as follows: Paper 3: Manuscript.

Die Folgen sind für die Betroffenen, das Personal und die Kliniken schwerwiegend. Seitens der Patienten sind besonders die Einbußen des kognitiven und physischen Status sowie ein Verlust an Selbstständigkeit zu nennen. Für das Pflegepersonal ist vor allem die hohe Arbeitsbelastung
aufzuführen. Lösungsansätze einer besseren Versorgung schenken einer demenzsensiblen Gestaltung der gebauten Umwelt in Deutschland bislang zu wenig Beachtung. Das Ziel dieser Arbeit besteht daher in der Entwicklung eines Katalogs demenzfreundlicher Planungskriterien für Akutkrankenhäuser.

Dies stellt einen wesentlichen Unterschied zum Raumprogramm stationärer Altenpflegeeinrichtungen
dar. Für demenzerkrankte Patienten könnte jedoch so dem Verlust der Selbstständigkeit und kognitiver Fähigkeiten vorgebeugt werden. Die Übertragung einer wohnlichen Atmosphäre muss in Bezug auf Machbarkeit und Sinnhaftigkeit kritisch diskutiert werden.
