Diante de uma suposta “epidemia do crack”, as políticas públicas de saúde estabelecem novas orientações a partir dos modelos existentes, a fim de que cada vez mais esses usuários sejam percebidos como doentes mentais. Com base na Antropologia da Saúde, pretende-se analisar os modelos de atenção vigentes, no âmbito da saúde mental, tendo como foco dois serviços de saúde no Rio Grande do Sul - Brasil. Esses modelos são constituídos por concepções e propostas terapêuticas que perpassam a composição da equipe profissional e a estrutura institucional. Foram contrastados os simbolismos presentes em cada modelo a partir da discussão sobre a desintoxicação, o uso de medicamentos, a reinserção social e a abordagem multidisciplinar na proposta terapêutica dos serviços de saúde. Considering this context, my intention was, through the presentation of ethnographic data about the formation process and the performance of the Indigenous Health Agents at the Upper Xingu, to question the role attributed to them, as well as the model of differentiated attention where this role was proposed. Exactly because they occupy a border position between different medicaltherapeutic systems, the agents performances are involved in several ambiguities and conflicts, which result from the imprecise definition of their functions, as well as from the effective existing conditions for their acting in the spaces attributed to them. I intend to show that the Indigenous Health Agents have their role redefined twice: by the non-indigenous professionals who think of them as transmitters of the biomedical knowledge to the attended populations as well as by the indigenous themselves, who see them as sources of goods and services from national society, in the general context of the political appropriation of this role and its acting spaces by the upper-xinguanos and local leaders themselves. / Dentro do contexto da reestruturação das políticas de saúde indígena ao longo dos últimos 20 anos no Brasil, e a conseqüente criação dos Distritos Sanitários Especiais Indígenas/DSEIs a partir de 1999, os Agentes Indígenas de Saúde/AISs aparecem como elementos centrais desse novo modelo de atenção à saúde indígena, na medida em que são entendidos como possíveis elos de ligação ou tradutores entre os sistemas tradicionais e a biomedicina. Tendo em vista este contexto, o objetivo desta dissertação foi, por meio da apresentação de dados etnográficos relativos à formação e à atuação dos AISs no Alto Xingu, problematizar o papel que lhes foi atribuído, bem como o modelo de atenção diferenciada no qual este papel foi proposto. Observa-se que, exatamente pelo fato de ocuparem uma posição de fronteira entre distintos sistemas médico-terapêuticos, a atuação dos AISs está envolta em ambigüidades e conflitos de diversas naturezas, decorrentes quer de certa imprecisão na formulação do seu papel, quer das condições efetivas das suas atuações nos espaços que lhes foram atribuídos. Procura-se mostrar que os Agentes Indígenas de Saúde têm sua atuação duplamente redefinida: tanto pelos profissionais não-indígenas que os pensam como transmissores dos conhecimentos biomédicos às populações atendidas quanto pelos próprios indígenas, que os vêem como fontes de acesso a bens e serviços provenientes da sociedade nacional, no contexto mais geral da apropriação política deste papel e dos seus espaços de atuação pelos próprios alto-xinguanos e lideranças locais.

Therefore the research followed with the aim to understand how women who work outside the home - a group of public servants that works in a public Health Institution in Goias - Brazil (Secretaria de Estado da Saúde de Goiás) - understand the sense of health food considering the multivocality of the subject. The methodology included conducting an observation-based ethnography and individual interviews with the servants that allowed to understand the representations of eating healthy structured into two main categories: space and time. In this sense, I came to the understanding that the meals eaten at home are linked to the notion of health for much more complex reasons than those involving nutritional matters. The house takes an idealized character, since most women are expanding their areas of expertise and thus acting more intensely out of the private sphere. So the healthy eating concept ends up being perceived more as a desire, a dream to be fulfilled, but given the constraints given by the demands of contemporary society ends up becoming a more guided practice in speech and with poor adherence in everyday life. / A pesquisa desenvolvida esteve fundamentada no argumento que, por mais que haja um o discurso médico/nutricional, a rede complexa de relações envolvida no tema alimentação faz com que ele seja transferido para o cotidiano das pessoas de uma forma diferenciada. É nesta direção que a pesquisa seguiu, ou seja, com o objetivo de compreender como as mulheres que trabalham fora de casa, no caso as servidoras públicas da Superintendência de Vigilância em Saúde da Secretaria de Estado da Saúde de Goiás, entendem a noção de alimentação saudável, tendo em vista a multivocalidade que o tema proporciona. A metodologia compreendeu a realização de uma etnografia baseada em observação e entrevistas individualizadas com as servidoras que permitiram entender as representações do comer saudável estruturadas em duas categorias principais: o espaço e o tempo. Neste sentido, cheguei ao entendimento de que as refeições realizadas em casa estão atreladas à noção de saudável por motivos muito mais complexos do que aqueles envolvendo a questão nutricional. Vislumbrou-se que a casa assume um caráter idealizado, uma vez que a grande parte das mulheres está ampliando os seus espaços de atuação e, assim, atuando mais intensamente fora do âmbito privado. Assim a noção de alimentação saudável acaba sendo entendida mais como um desejo, um ideal a ser cumprido, mas que diante das imposições dadas pela demanda da sociedade contemporânea acaba se tornando uma prática mais pautada no discurso e com pouca adesão no cotidiano.

L’interdisciplinarité au pays des protocoles : ethnographie du travail d’équipe dans une unité d’hémato-oncologie pédiatrique

Ce mémoire de maîtrise se penche sur la façon dont l’interdisciplinarité peut s’exprimer dans le quotidien d’une équipe de soin, en l’occurrence l’unité d’hémato-oncologie du Centre hospitalier universitaire Ste-Justine. Par une approche ethnographique soutenue par des observations au sein de l’unité et dans des réunions, par du shadowing auprès de divers professionnels et par des entrevues avec les professionnelles, j’ai tenté de sortir du champ gravitationnel de la dyade médecin-infirmière pour m’intéresser aux professions les plus en « marge » : les intervenantes psychosociales et les préposés aux bénéficiaires. J’ai cherché à mieux comprendre la façon dont l’unité se positionne dans la multiplicité de définitions de l’interdisciplinarité, puis cela a mis en lumière l’importance du quotidien et d’une approche de care. C’est quand les soignants tâtonnent, quand ils cherchent des solutions dans la perspective de la vie qui continue, plutôt que le « protocole », qu’ils semblent avoir le plus de facilité à travailler en équipe. Dans ce contexte, les infirmières se retrouvent à jouer un rôle clé, étant les principales interlocutrices à la fois des mondes médicaux et psychosociaux. Parallèlement, le contexte de travail des préposés montre la persistance d’une hiérarchie dans l’unité, malgré un discours faisant la promotion d’une collaboration plus égalitaire. / This Master's thesis examines how interdisciplinarity can be expressed in the daily practice of a healthcare team, in this case the Hematology-Oncology Unit at CHU Ste-Justine. I looked at the daily practice within the unit through an ethnographic lens sustained by observing, shadowing and interviewing various professionals. In an attempt to further understand which type of interdisciplinarity is found in the unit, I tried to escape the doctor-nurse dyad's gravitational field to look at the most "marginal" professions instead – psychosocial workers and healthcare assistants. I sought to better understand which type of interdisciplinarity is practiced in the unit. My work highlights the importance of everyday life and of a logic of care. It is when caregivers are trying and guessing, when they are looking for solutions from a perspective where "life goes on" rather than one that follows the "protocol", that they seem to be the most at ease working as a team. In this context, nurses find themselves playing a key role, being the main interlocutors of both the medical and psychosocial worlds. At the same time, healthcare assistants show us the persistence of hierarchy in the unit, despite a discourse promoting more egalitarian collaboration.

Susceptibility and vulnerability of Indian women to the impact of HIV/AIDS

The objective of this thesis is to examine which socio-economic, structural and cultural factors may influence Indian women’s propensity to contract HIV and later their ability to access the relevant healthcare services for their condition. The research draws on two theoretical frameworks, the first being Barnett and Whiteside’s (2002) concept of social structural factors of disease transmission. Second, Anderson and Aday’s (1981) model of access examines how a variety of structural and resource-based factors, e.g. area of residence, can influence usage of healthcare facilities. Two stages of data analysis were undertaken, the first being secondary statistical analysis of the National Family Health Survey III. The survey provided state level estimates on the HIV sero-status of the general population in India and data on demographic and socio-economic determinants for family planning, nutrition, utilization of healthcare and emerging health issues. The second stage of analysis consisted of a set of qualitative interviews conducted in Andhra Pradesh, India. Thirty-three interviews were conducted with female sero-positive patients and ten with HIV-infected women who were providing social services to others with the same condition. Statistical results on social structural determinants of HIV transmission illustrated that Indian women who were formerly married (OR=5.27, CI=3.07-9.04), lived in higher prevalence states (OR=3.48, CI=2.19-5.54), had a low level of education (OR=2.27, CI=1.40-3.68) and were employed (OR=1.45, CI=0.96-2.18) had significantly (<.05) higher odds of being HIV-positive in comparison to those who were not. Findings in the qualitative phase of analysis were similar but participants’ narratives illustrated that their risk of contracting HIV begun before they even had the opportunity to seek a match as they seemed to live in communities with a high level of HIV prevalence. Many of the participants commented that there were factors outside of their sphere of control, e.g. lack of education, which resulted in them having a narrow choice of potential partners. Additionally, statistical results on female participants’ access to healthcare services indicated the vast majority of HIV-positive respondents were almost certainly not aware of their sero-status as they had not undertaken an HIV test prior to the survey. As the sample of female HIV infected respondents was relatively small, it was difficult to ascertain which social factors had an impact on these participants utilisation of HIV testing services. On the other hand, respondents’ narratives from the qualitative stage of research highlighted on social structural factors which could potentially influence WLHA’s continual utilisation of HIV-related healthcare services. It was found that participants experienced the most barriers to accessing healthcare facilities in the initial phases of their treatment. These barriers were mediated by the structure of healthcare services, culturally sanctioned medical practices (e.g. physicians refusal to inform the patient of their sero-status) and quality of services.

La question de l’observance non-médicamenteuse est primordiale dans la prise en chargedes maladies cardiovasculaires chroniques. Des programmes d’éducation thérapeutique dupatient (ETP) sont proposés aux patients afin de les aider à « mieux vivre avec leur maladie »(OMS, 1998). Leurs évaluations sont toutefois décevantes : ils touchent peu de patients etsont peu efficaces.Cette étude porte sur deux populations : des patients (N=19) et des médecins généralistes(N=14). Elle se base sur des entretiens menés auprès des uns et des autres ainsi que desobservations de consultation et de séquences de vie quotidienne. Nous en avons extrait uneanalyse qualitative approfondie ainsi que des éléments quantitatifs afin de mener à bien desanalyses factorielles. Le croisement de ces données permet dans un premier temps demettre en évidence la diversité des Arrière-plans (Searle, 1982, 1985) susceptiblesd’influencer la transformation des conduites des patients et d’en construire une typologie.Elle établit ensuite trois styles thérapeutiques, pratiqués par les médecins généralisteslorsqu’ils sont confrontés à ce type de patients.L’étude de la rencontre de cette diversité anthropologique avec ces différents styles montreque, malgré une configuration plus propice que celles des programmes hospitaliers, larencontre éducative entre médecine de proximité et patients cardiovasculaires ne se faitpas, ou ne se fait que sous certaines conditions bien fragiles. Son analyse sous l’angleanthropodidactique permet toutefois d’ébaucher de nouvelles formes d’ingénieriesdidactiques pour l’ETP et de donner de nouvelles pistes pour la formation des soignants. / Non-medication compliance is essential when taking care of chronic cardiovascular diseases.Therapeutic patient educational (TPE) programs are proposed to patients in order to helpthem to « have a better life with their disease » (WHO, 1998). However, those programs’evaluations are disappointing: they reach very few patients and are not as efficient asplanned.Drawing on semi-structured interviews with general practitioners (GPs, N=14) and patientsdealing with a cardiovascular chronic disease (N=19), and quantitative analysis of datarelated to a potential link between social factors and types of behaviour, this study firstemphasizes the diversity of Backgrounds (Searle, 1982, 1985) which may influence patients’life-style modifications. This work then defines four patient-types and three therapeuticstyles applied by GPs when confronted to those patients. Those styles vary according to GPsrepresentations and attitudes related to therapeutic education and cardiovascular diseases.This study shows that even though GPs are more likely to help with this transformation thanmandatory educational programs, the encounter between them and chronic patients doesnot happen, or only does in very specific and fragile conditions. Nevertheless, this anthropodidacticalapproach allows us to draft new forms of didactic engineering for TPE and to givenew tracks for healthcare professionals training.

O controle da reprodução : estudo etnográfico da prática contraceptiva de implantes subcutâneos na cidade de Porto Alegre/RS

Este trabalho se inscreve no campo dos estudos sobre sexualidade e práticas reprodutivas e contraceptivas, no contexto do Município de Porto Alegre onde muito recentemente passou a ser disponibilizado, na rede pública de saúde, um novo método contraceptivo, o implante subcutâneo Implanon. Partindo de um caso específico de implementação de uma ação governamental na área de saúde reprodutiva, este estudo tem como objetivo compreender, a partir de uma perspectiva antropológica, os significados da prática contraceptiva de implantes subcutâneos para as jovens residentes na região geográfica da cidade, a Restinga e seu entorno, que foi alvo privilegiado desta intervenção pública. Como pano de fundo descreve-se e apresenta-se o processo de idealização, implementação e debate em torno desta política de saúde, assim como o conjunto de atores e agencia que participaram do mesmo. Com a finalidade de conhecer o universo cultural das mulheres que optaram pela tecnologia contraceptiva oferecida pela política municipal, analisam-se as práticas sexuais, reprodutivas e contraceptivas das mulheres entrevistadas, evidenciando algumas categorias e valores imputados a estas práticas e experiências. Discutem-se, a partir do contexto específico pesquisado, os significados e o universo de relações onde se dá a prática contraceptiva de implantes subcutâneos. Enfatiza-se, nesta parte final, como é que tal prática contraceptiva se conecta com as relações afetivo-sexuais, contraceptivas e reprodutivas. A partir desta contextualização foi possível perceber que os eventos envolvidos no processo de gestar e evitar gravidez pode significar e gerar diferentes consequências para os sujeitos neles envolvidos. Igualmente, a abordagem centrada nas especificidades do grupo social "alvo" da ação governamental, evidenciou as diferentes perspectivas e apropriações desta política municipal. / Situated within the field of studies on sexuality and reproductive and contraceptive practices, this study takes as its context the municipality of Porto Alegre, where a new contraceptive method - the subcutaneous implant, Implanon - was recently made available through the public health system. Departing from a specific case of the implementation of a governmental action in reproductive health, the objective of this study is to understand, from an anthropological perspective, what the contraceptive implants mean for young female residents of a particular area of the city - Restinga and its surroundings - that was a target of this public intervention. As background, the process of the health policy's formulation and implementation, and the subsequent debates regarding it, is described, and the collection of participating actors and agencies is presented. With the aim of becoming familiar with the cultural universe of the women who opted for the contraceptive technology proffered by the municipal policy, the sexual, reproductive, and contraceptive practices of interviewed women are analyzed; these data bring to light some of the categories and values ascribed to aforesaid practices and experiences. The meanings and the universe of relationships in which the contraceptive practice of the implants takes place is discussed with specific reference to the research context. The last section emphasizes how such contraceptive practices connect with affective-sexual, contraceptive and reproductive relationships. Through contextualization, it is possible to perceive that the process of both becoming pregnant and avoiding becoming pregnant can mean different things and generate diverse consequences for the subjects involved. Likewise, an approach focused on the specificities of the target population of this governmental action makes plain the different perspectives on and appropriations of municipal policy.

O controle da reprodução : estudo etnográfico da prática contraceptiva de implantes subcutâneos na cidade de Porto Alegre/RS

Este trabalho se inscreve no campo dos estudos sobre sexualidade e práticas reprodutivas e contraceptivas, no contexto do Município de Porto Alegre onde muito recentemente passou a ser disponibilizado, na rede pública de saúde, um novo método contraceptivo, o implante subcutâneo Implanon. Partindo de um caso específico de implementação de uma ação governamental na área de saúde reprodutiva, este estudo tem como objetivo compreender, a partir de uma perspectiva antropológica, os significados da prática contraceptiva de implantes subcutâneos para as jovens residentes na região geográfica da cidade, a Restinga e seu entorno, que foi alvo privilegiado desta intervenção pública. Como pano de fundo descreve-se e apresenta-se o processo de idealização, implementação e debate em torno desta política de saúde, assim como o conjunto de atores e agencia que participaram do mesmo. Com a finalidade de conhecer o universo cultural das mulheres que optaram pela tecnologia contraceptiva oferecida pela política municipal, analisam-se as práticas sexuais, reprodutivas e contraceptivas das mulheres entrevistadas, evidenciando algumas categorias e valores imputados a estas práticas e experiências. Discutem-se, a partir do contexto específico pesquisado, os significados e o universo de relações onde se dá a prática contraceptiva de implantes subcutâneos. Enfatiza-se, nesta parte final, como é que tal prática contraceptiva se conecta com as relações afetivo-sexuais, contraceptivas e reprodutivas. A partir desta contextualização foi possível perceber que os eventos envolvidos no processo de gestar e evitar gravidez pode significar e gerar diferentes consequências para os sujeitos neles envolvidos. Igualmente, a abordagem centrada nas especificidades do grupo social "alvo" da ação governamental, evidenciou as diferentes perspectivas e apropriações desta política municipal. / Situated within the field of studies on sexuality and reproductive and contraceptive practices, this study takes as its context the municipality of Porto Alegre, where a new contraceptive method - the subcutaneous implant, Implanon - was recently made available through the public health system. Departing from a specific case of the implementation of a governmental action in reproductive health, the objective of this study is to understand, from an anthropological perspective, what the contraceptive implants mean for young female residents of a particular area of the city - Restinga and its surroundings - that was a target of this public intervention. As background, the process of the health policy's formulation and implementation, and the subsequent debates regarding it, is described, and the collection of participating actors and agencies is presented. With the aim of becoming familiar with the cultural universe of the women who opted for the contraceptive technology proffered by the municipal policy, the sexual, reproductive, and contraceptive practices of interviewed women are analyzed; these data bring to light some of the categories and values ascribed to aforesaid practices and experiences. The meanings and the universe of relationships in which the contraceptive practice of the implants takes place is discussed with specific reference to the research context. The last section emphasizes how such contraceptive practices connect with affective-sexual, contraceptive and reproductive relationships. Through contextualization, it is possible to perceive that the process of both becoming pregnant and avoiding becoming pregnant can mean different things and generate diverse consequences for the subjects involved. Likewise, an approach focused on the specificities of the target population of this governmental action makes plain the different perspectives on and appropriations of municipal policy.
