Reversibility of airway remodeling in equine asthma : contribution of anti-inflammatory and bronchodilator therapies

L’asthme bronchique est caractérisé par un remodelage et une inflammation des voies aériennes. La masse du muscle lisse ainsi que la déposition de matrice extracellulaire sont augmentées dans la paroi des bronches asthmatiques, ce qui contribue à l’obstruction respiratoire. Peu d’études ont évalué les effets des traitements utilisés dans l’asthme sur le remodelage bronchique, et surtout peu de données sont disponibles concernant les effets sur le muscle lisse. La combinaison de corticostéroïdes et de β2-agonistes à longue durée d’action administrée par inhalation permet de mieux contrôler les crises d’asthme par rapport à la monothérapie avec des médicaments corticostéroïdes. Cependant, l’action spécifique de la combinaison sur le remodelage et sur l’inflammation des bronches périphériques n’est pas décrite. Surtout, il reste à clarifier si l’administration de la combinaison est avantageuse par rapport à la monothérapie corticostéroïde. La plupart des études réalisées chez l’homme utilisent des tissus bronchiques obtenus par biopsie endobronchique, qui ne sont pas représentatifs du processus pathologique affectant les voies respiratoires périphériques. Leur inaccessibilité par des méthodes non invasives est la raison pour laquelle si peu de données existent sur la pathophysiologie des voies périphériques chez les patients asthmatiques. L’asthme équin, aussi connu comme « le souffle », est une pathologie obstructive des chevaux adultes considérée comme un modèle animal d’asthme humain. Elle est caractérisée par un remodelage des bronches périphériques et par une inflammation bronchoalvéolaire de type neutrophilique. En étudiant le modèle équin, cette thèse a évalué la contribution des médicaments corticostéroïdes et de β2-agonistes à longue durée d’action, administrée comme monothérapies ou en combinaison, sur la réversibilité du remodelage et de l’inflammation de voies aériennes dans l’asthme bronchique. A cette fin, nous avons d’abord optimisé et validé l’application de la biopsie endobronchique et de l’échographie endobronchique chez le cheval adulte. Nos résultats indiquent que les échantillons obtenus par biopsie endobronchique sont inadéquats pour l’évaluation quantitative de la masse du muscle lisse chez le cheval. Cependant, ils permettent d’étudier les changements quantitatifs des structures épithéliales et de la lamina propria, ainsi que les aspects qualitatifs du muscle lisse. L’échographie endobronchique, quant à elle, permet d'estimer la masse du muscle lisse bronchique, et ce, chez des chevaux sains et chez des chevaux asthmatiques. Cette thèse démontre aussi que qu’un traitement de 12 semaines avec des corticostéroïdes induit une diminution significative de la masse du muscle lisse périphérique, qui n’est pas amélioré davantage par l’administration concomitante d’un β2-agoniste à longue durée d’action. Cette diminution est toutefois incomplète. Un effet positif et synergique de la combinaison a également été observé au niveau de la déposition de matrice extracellulaire. La combinaison a produit une diminution significative de la quantité de matrices déposées dans la lamina propria et dans la couche du muscle lisse dans les bronches centrales, alors que l’effet été limité à la couche du muscle lisse dans les bronches périphériques. La combinaison n’améliore pas le contrôle de l’inflammation bronchique ni bronchiolaire par rapport aux monothérapies ; cependant, elle diminue la neutrophilie bronchoalvéolaire de façon synergique. / Airway remodeling and inflammation are the hallmarks of asthma. Both airway smooth muscle (ASM) mass and extracellular matrix (ECM) deposition are increased in the central and peripheral airways of asthmatic patients, which contribute to airway obstruction. Few studies have investigated the ability of current asthma medications to reverse airway remodeling, especially the increased ASM mass. Inhaled corticosteroids (ICS) and long-acting β2-agonist combinations (ICS/LABA) are more effective than ICS monotherapy to control asthma exacerbations. However, their efficacy at modifying bronchial inflammation and remodeling at the peripheral level of the lung is not well-described. In fact, most work has been performed using endobronchial biopsy samples obtained from asthmatic subjects, which completely disregard the alterations occurring in peripheral airways. Ethical considerations limit the possibility of biopsying the peripheral airways in humans due to the invasiveness of the procedure. Equine asthma, or heaves, is a naturally-occurring disease of adult horses and a recognized animal model of human asthma characterized by neutrophilic inflammation as well as ASM and ECM remodeling of peripheral airways. This thesis has assessed the contribution of ICS and LABA, alone or combined, to the reversal of remodeling and inflammation in central and peripheral airways using the equine asthma model. To attain this goal, we have first optimized and validated the application of endobronchial biopsy and endobronchial ultrasound (EBUS) in the equine species. EBUS reliably estimates the bronchial ASM. Subsequently, asthmatic horses with ongoing airway remodeling and inflammation were treated with ICS, LABA, ICS/LABA, or antigen avoidance. Lung function, airway remodeling and inflammation were then assessed weekly for 3 months. Our results demonstrated a 30% decrease of peripheral ASM remodeling attained with ICS and ICS/LABA pharmacological treatment. A decrease of a similar magnitude of peripheral ASM was previously reported after 6 and 12 months of ICS monotherapy and antigen avoidance, respectively. A synergistic effect of ICS/LABA was observed on ECM deposition and airway lumen neutrophils. ICS/LABA decreased the ECM fraction of the ASM layer both peripherally and centrally, while the same effect on the lamina propria was observed only in central airways. Both ICS/LABA and ICS monotherapy decreased submucosal inflammation in central airways, while only ICS/LABA and antigen avoidance decreased bronchoalveolar neutrophilia. In conclusion, our results suggest that the enhanced therapeutic effect of ICS/LABA over ICS monotherapy in asthmatic horses was associated with a reduction of ECM deposition, mainly observed within the large airways, and possibly also with a decreased airway neutrophilia. However, ICS/LABA did not provide additional benefit to ICS monotherapy in terms of peripheral ASM remodeling as both induce a 30% decrease of the ASM mass in 3 months.

The Acute Toxic Effects of the Synthetic Cannabinoid, JWH-018 on the Cardiovascular and Neuroendocrine Systems in Ictalurus punctatus (Channel Catfish)

Cannabinoid (CB) receptors have been found in most vertebrates that have been studied. The location of various CB receptors in the body and brain are known, but their physiological functions are not fully understood. The effects CBs have on the cardiovascular system have been of growing interest in recent years. Increasing reports from emergency departments and law enforcement agencies detail acute cardiovascular and psychological effects from synthetic CB intoxication, such as JWH-018. This major health concern is substantiated by governmental agencies like the CDC and NIDA. This pilot study investigates the acute toxic effects of the synthetic CB, JWH-018, on the cardiovascular and neuroendocrine systems in Ictalurus punctatus (channel catfish). Research in organisms besides the traditional mammal models can provide new insights into CB function and physiology. Ictalurus punctatus lend multiple benefits as a model organism that permits researchers to investigate in vivo effects of both cardiovascular and neuroendocrine systems without much influence from traditional sampling methods, and further more provide ample size and tissue to perform specific cardiovascular experiments. Multiple methods were used to assess cardiovascular function and sympathetic nervous system activation. Two different doses, low (500 µg/kg) and high 1,500 µg/kg, of JWH-018 were evaluated in the study. Delivery of JWH-018, via dorsal aorta cannulation, was administered to channel catfish in order to measure cardiovascular functions and sample blood. Plasma levels of the hypothalamus-pituitary-adrenal/interrenal (HPA/I) biomarkers; ACTH, cortisol, epinephrine, and norepinephrine, were measured using ELISAs. Myocardial and neural tissue was collected after the exposures for rt-PCR analysis on β2 adrenergic and glucocorticoid receptor density change. Acute exposure of JWH-018 in undisturbed channel catfish yielded several findings: (1) High dose of JWH-018 was responsible for cardio depressor effects in catfish with a tendency to produce tachycardia, (2) rt-PCR results showed a 2.7 fold increase of glucocorticoid receptor mRNA density in catfish cardiomyocytes when exposed to each dose of JWH-018, (3) Catfish plasma ACTH levels were increased with high doses of JWH-018, while plasma cortisol was increased by low doses. Channel catfish is an excellent animal model to examine the effects of synthetic cannabinoids and cardiovascular function. Acute exposures to high levels of JWH-018 appear to produce cardiovascular dysfunction providing evidence that substantiates emergency department reports, in addition yields novel information about the interaction of CBs exposure and the increase of glucocorticoid receptors levels on cardiomyocytes. The channel catfish is a new animal model that can aid in further investigations of CB exposure and multiple physiological functions for health and toxicology studies. With relatively easy adjustments from this pilot study, the effects on CBs can be monitored on Ictalurus punctatus with confident results concerning human health.

Characterisation of nicotine binding sites on human blood lymphocytes

Nicotine exerts a therapeutic effect in ulcerative colitis (UC) but the mechanism underlying this effect, is not clear. However, this effect may imply that nicotine has some, as yet to be discovered, effect on the immune system. The aim of the work described in this thesis was to characterise the nicotinic acetylcholine receptors (nAChRs) on human peripheral blood lymphocytes in term of receptor subtype. To achieve this, a combination of radioligand binding assays, pharmacological and molecular biological techniques were used. The data obtained from the binding studies suggested that the presence of one binding site for (-)- nicotine on human peripheral blood lymphocytes with a Kd 15 ± 5.759 nM (1.5 ± 5.759 x 10-8 M) and Bmax 2253 ± 409 sites/cell. The competition studies showed that ligands competing with [3H]-(-)-nicotine were (-)-nicotine, epibatidine and α-bungarotoxin, while others ligands for nAChRs displaced radiolabelled nicotine in insignificant quantities. Thus, radioligand-binding experiments suggest that the binding site for nicotine on human peripheral blood lymphocytes is a nAChR containing α7 and possibly α4 or/and b2 containing nAChR subunits. No evidence was obtained to suggest the presence of a non-cholinergic nicotine receptor. Furthermore, considerable subject to subject variation in the specific binding of radiolabelled nicotine was observed. Because of this only tentative conclusions could be drawn from radioligand binding data. Polymerase chain reaction (RT-PCR) was then used to demonstrate mRNA for the subunits of nAChRs suggested by radioligand binding studies. Data obtained show that the human peripheral blood lymphocytes tested, expressed mRNAs for α4, α5, α7, β2 neuronal nAChRs subunits and β1 muscle nAChR subunit. Expression of the α5 mRNA subunit of nAChR was observed in the lymphocytes in each sample of lymphocytes tested. In contrast, the expression pattern of mRNAs for α4, α7, β1, and β2 mRNAs subunits of nAChRs, varied between individuals. Finally, Western blot analysis was used to confirm that mRNA expression resulted in the expression of protein for nAChR subunits in human peripheral lymphocytes using monoclonal antibodies against α4, α5, α7, and β2 nAChR subunits, which had been detected by RT-PCR. The results obtained from the Western blot analysis show that protein for α4, α5, and α7 nAChR subunits was expressed in most, but not all of the human peripheral blood lymphocyte samples tested and some of the bands obtained were faint. In contrast, protein for the β2 nAChR subunit was observed in a few samples tested and the bands were faint. From the results obtained in this study, it is possible to conclude that human peripheral blood lymphocytes may contain nAChRs with subunit compositions of α4β2, α4β2α5, and/or α7. However, further studies are necessary to show whether or not the single binding site for nicotine demonstrated by radioligand binding experiments is due to one or all of these nAChRs. Thus, the findings of the present study suggest the presence of nAChR on human peripheral blood lymphocytes. Nicotine and its effect may occur through these non- neuronal nAChRs mechanisms. Such a mechanism of action could account for the beneficial of nicotine in ulcerative colitis. Furthermore, a compound that acts on these receptors, but not on nAChRs found on other cells may have therapeutic utility in the treatment of inflammation.

Impacto do reprocessamento de dialisadores de alto fluxo e alta eficiência sobre o transporte de solutos em sessões de hemodiafiltração online curta diária / Reprocessing high-flux, high-efficiency polysulfone dialyzers on solutes removal in short daily online hemodiafiltration

Introdução: Não há estudos que avaliem o impacto da reutilização do dialisador na remoção de solutos em sessões de hemodiafiltração online curta diária (HDF-OL-D). Objetivo: Avaliar o impacto do reuso do dialisador na dose do tratamento, na cinética de beta2-microglobulina e na extração de solutos em sessões de HDF-OL-D e comparar com sessões de hemodiálise curta diária de alto fluxo (HD-D). Métodos: Foram incluídos 14 pacientes do programa de HD-D. Foram coletadas amostras de sangue pré, no meio e pós-diálise e amostras do dialisato no 1º, 7º e 13º usos do dialisador em sessões de HD-D e de HDF-OL-D. Resultados: A massa total extraída (MTDQ) e a depuração (KDQ) diretamente quantificada dos solutos pequenos (ureia, fósforo, creatinina e ácido úrico), bem como a dose de diálise ofertada, foram semelhantes quando o 1º, 7º e 13º usos do dialisador em sessões de HD-D foram comparados, respectivamente, ao 1º, 7º e 13º usos em sessões de HDF-OL-D. A MTDQ e a KDQ dos solutos pequenos e a dose de diálise foram semelhantes entre os usos do dialisador tanto em sessões de HD-D e quanto em sessões de HDF-OL-D. A MTDQ, a KDQ e o Kt/V diretamente quantificado (Kt/VDQ) de beta2-microglobulina foram maiores em sessões de HDF-OL-D do que nos respectivos usos do dialisador em sessões de HD-D. Não houve diferença quanto à cinética de beta2-microglobulina, avaliada pela MTDQ, KDQ ou Kt/VDQ, entre o 1º, 7º e 13º usos do dialisador em sessões de HD-D nem em sessões de HDF-OL-D. Na HDF-OL-D, a perda intradialítica de albumina foi significativamente diferente entre os usos do dialisador (p < 0,001), estando reduzida no 7º e 13º usos, quando comparados ao 1º uso do dialisador. Não houve correlação entre o volume de enchimento do dialisador e a MTDQ e a KDQ de moléculas pequenas em nenhum dos métodos dialíticos estudados. Em sessões de HDF-OL-D, foi observada uma correlação fraca entre o volume de enchimento do dialisador e a KDQ de beta2-microglobulina, não percebida com a MTDQ ou o Kt/VDQ de beta2-microglobulina. Conclusão: O reprocessamento de dialisadores de alto fluxo e alta eficiência não resulta em comprometimento da dose do tratamento, da cinética de beta2-microglobulina nem da extração de solutos em sessões de HDF-OL-D. A extração de beta2-microglobulina foi maior em sessões de HDF-OL-D do que em sessões de HD-D, sem diferenças significativas na remoção dos demais solutos. O reprocessamento do filtro, em sessões de HDF-OL-D, resultou numa redução significativa da perda intradialítica de albumina. Não parece haver influência do volume de enchimento do filtro na extração de solutos em nenhum dos métodos de diálise estudados / Introduction: There no studies evaluating the impact of dialyzer reutilization on solute removal in daily online hemodiafiltration (D-OL-HDF) sessions. Objectives: Our aim was to evaluate the impact of dialyzer reuse on solute extraction, beta2-microglobulin kinetics and dialysis dose in D-OL-HDF and compare to those in high-flux short daily hemodialysis (D-HD). Methods: 14 patients undergoing a D-HD program were included. Pre, middle and post-dialysis blood samples and effluent dialysate were collected in the 1st, 7th and 13th dialyzer uses in D-HD sessions and in D-OL-HDF sessions. Results: Directly quantified small solute (urea, phosphorus, creatinine and uric acid) total mass removal (TMDQ) and clearance (KDQ), as well as dialysis dosis, were similar when the 1st, 7th and 13th dialyzer D-HD uses were compared to the 1st, 7th and 13th D-OL-HDF uses. TMDQ and KDQ of small solutes and dialysis dose were similar among dialyzer uses in D-HD sessions and also in D-OL-HDF sessions. beta2-microglobulin TMDQ, KDQ and directly quantified Kt/V (Kt/VDQ) were statistically higher in D-OL-HDF dialyzer uses than in the respective D-HD uses. There was no difference in beta2-microglobulin kinetics, evaluated by TMDQ, KDQ or Kt/VDQ, among 1st, 7th and 13th uses in D-OL-HDF sessions or in D-HD sessions. In D-OL-HDF, albumin loss was significantly different among studied dialyzer uses (p < 0.001), being reduced in the 7th and 13th dialyzer uses, when compared to the first use. There was no correlation between dialyzer priming volume and small molecules TMDQ, KDQ in neither analyzed dialytic method. In D-OL-HDF sessions, it was observed a week correlation between dialyzer priming volume and beta2-microglobulin KDQ, not observed with beta2-microglobulin MTDQ or Kt/VDQ. Conclusion: High flux, high efficiency dialyzer reprocessing did not did result in a reduction of the offered dialysis dose, beta2-microglobulin kinetics or solute extraction in D-OL-HDF. beta2-microglobulin removal was greater in D-OL-HDF than in D-HD sessions, without difference in other solutes extraction. There was a significant reduction in intradialytic albumin loss with dialyzer reprocessing in D-OL-HDF sessions. Dialyzer priming volume does not appear to influence solute removal in neither analyzed dialytic method

Avaliação dos biomarcadores urinários no controle do tratamento de estenose de junção ureteropélvica em adultos / The role of urinary biomarkers in the assessment of ureteropelvic junction obstruction in adults

INTRODUÇÃO E OBJETIVO: A estenose de junção ureteropélvica (EJUP) é importante causa de obstrução do trato urinário e pode levar a deterioração progressiva da função renal. Há espaço para o aprimoramento de novos métodos diagnósticos capazes de discriminar hidronefrose e uropatia obstrutiva. Acredita-se que os biomarcadores urinários podem fornecer indícios de lesão renal precoce na obstrução urinária. Neste contexto, KIM-1 pode elevar-se na urina por lesão tubular proximal, NGAL por lesão no túbulo proximal, distal ou alça de Henle, CA19-9 por produção excessiva no túbulo obstruído e ?2-microglobulina (beta2M) por injúria ao glomérulo ou ao túbulo proximal. O objetivo do presente estudo foi avaliar as propriedades diagnósticas dos biomarcadores urinários citados em adultos com EJUP, sendo o primeiro estudo na literatura a avaliar tais moléculas nesta população. MÉTODOS: Foram estudados de modo prospectivo pacientes consecutivos acima de 18 anos com diagnóstico de EJUP submetidos a pieloplastia videolaparoscópica de dezembro de 2013 a fevereiro de 2015. Foram excluídos do estudo pacientes com EJUP bilateral, rim contralateral patológico, EJUP em rim único, antecedentes de tratamento cirúrgico para estenose de JUP ou taxa de filtração glomerular inferior a 60 ml/min/1,73m2. Cada paciente forneceu quatro amostras de urina para medição de biomarcadores, uma no pré-operatório e outras com 1, 3 e 6 meses de seguimento pós-operatório. O grupo controle foi constituído por voluntários saudáveis sem hidronefrose à ultrassonografia. RESULTADOS: Foram incluídos 47 pacientes com idade média de 38,6 ± 12,7 anos (intervalo 19 a 64 anos), sendo 17 (36,2%) do sexo masculino e 30 (62,8%) do sexo feminino. O grupo controle foi composto por 40 indivíduos semelhantes ao grupo com EJUP no que concerne idade (p = 0,95) e sexo (p = 0,82). KIM-1 foi o marcador com melhores propriedades diagnósticas, apresentando área sob a curva (AUC) de 0,79 (95% CI 0,70 a 0,89). O NGAL, por sua vez, teve AUC de 0,71 (95% CI 0,61 a 0,83), CA19- 9 teve AUC de 0,70 (95% CI 0,60 a 0,81) e (beta2M) apresentou AUC de 0,61 (95% CI 0,50 a 0,73), sendo o único biomarcador com propriedades inadequadas neste cenário. O KIM-1 foi o marcador mais sensível com o ponto de corte 170,4 pg/mg de creatinina (sensibilidade 91,4%, especificidade 59,1%) e o CA 19-9 o mais específico para o ponto de corte de 51,3 U/mg de creatinina (sensibilidade 48,9%, especificidade 88,0%), enquanto o NGAL foi o que apresentou maior queda após desobstrução, com 90,0% dos pacientes apresentando clareamento superior a 50%. CONCLUSÕES: A avaliação dos biomarcadores urinários é útil no diagnóstico de obstrução em adultos com EJUP submetidos a pieloplastia videolaparoscópica. O KIM-1 foi o marcador mais sensível e o CA 19-9 o mais específico, enquanto o NGAL foi o que apresentou maior que com a desobstrução. Houve queda das concentrações dos marcadores após pieloplastia no período estudado. O papel exato dos biomarcadores urinários no cenário de obstrução em adultos deve ser mais amplamente investigado / INTRODUCTION AND OBJECTIVE: Ureteropelvic junction obstruction (UPJO) is an important cause of urinary tract obstruction and can lead to progressive deterioration of renal function. Thus the development of novel non-invasive methods capable of discriminating obstruction and hydronephrosis may be useful. Elevation of urinary biomarkers may provide early evidence of kidney damage in urinary obstruction. In this scenario, urinary concentrations of KIM-1 may be elevated following proximal tubular injury, while NGAL may increase as result of injury to proximal or distal tubule as well as to loop of Henle, CA19-9 after overproduction in the obstructed tubule and ?2 microglobulin (beta2M) after injury to the glomerulus or the proximal tubule. The aim of this study was to evaluate the diagnostic properties of these urinary biomarkers in adults with UPJO. METHODS: We prospectively studied consecutive patients older than 18 years diagnosed with UPJO undergoing laparoscopic pyeloplasty from December 2013 to February 2015 in our institution. Exclusion criteria included patients with bilateral UPJO, unilateral UPJO with contralateral pathologic kidney, solitary kidney, history of previous surgical treatment for UPJO or glomerular filtration rate below 60 ml/min/1,73m2. Each patient provided four voided urine samples for biomarker measurement, one at preoperative consultation and the others at 1, 3 and 6 months of postoperative follow-up. Healthy individuals with no hydronephrosis on ultrasound evaluation constituted our control group. RESULTS: We included 47 patients with a mean age of 38.6 ± 12.7 years (range 19-64 years), from which 17 (36.2%) were males and 30 (62.8%) were females. The control group consisted of 40 subjects with no statistical difference to the study group regarding age (p = 0.95) and gender (p = 0.82). KIM-1 had an area under the curve (AUC) of 0.79 (95% CI 0.70 to 0.89) and was the biomarker with the best diagnostic properties. CA19-9 had an AUC of 0.70 (95% CI 0.60 to 0.81), NGAL had an AUC of 0.71 (95% CI 0.61 to 0.83) and beta2M had an AUC of 0.61 (95% CI 0.50 to 0.73). KIM-1 was the most sensitive marker with a cutoff of 170.4 pg/mg creatinine (sensitivity 91.4%, specificity 59.1%) whereas CA 19-9 as the most specific one, displaying a cutoff of 51.3 U/mg creatinine (sensitivity 48.9%, specificity 88.0%). NGAL showed the greatest decrease in urinary concentrations after pyeloplasty, in which 90.0% of patients had a clearance greater than 50% in comparison to preoperative values. CONCLUSIONS: The evaluation of urinary biomarkers is useful in the assessment of UPJO in adults undergoing laparoscopic pyeloplasty. Urinary concentrations of CA 19-9, NGAL and KIM-1 were elevated in patients with UPJO and significantly decreased after pyeloplasty. The exact role of those biomarkers in the setting of obstruction in adults should be further evaluated

Avaliação dos biomarcadores urinários no controle do tratamento de estenose de junção ureteropélvica em adultos / The role of urinary biomarkers in the assessment of ureteropelvic junction obstruction in adults

INTRODUÇÃO E OBJETIVO: A estenose de junção ureteropélvica (EJUP) é importante causa de obstrução do trato urinário e pode levar a deterioração progressiva da função renal. Há espaço para o aprimoramento de novos métodos diagnósticos capazes de discriminar hidronefrose e uropatia obstrutiva. Acredita-se que os biomarcadores urinários podem fornecer indícios de lesão renal precoce na obstrução urinária. Neste contexto, KIM-1 pode elevar-se na urina por lesão tubular proximal, NGAL por lesão no túbulo proximal, distal ou alça de Henle, CA19-9 por produção excessiva no túbulo obstruído e ?2-microglobulina (beta2M) por injúria ao glomérulo ou ao túbulo proximal. O objetivo do presente estudo foi avaliar as propriedades diagnósticas dos biomarcadores urinários citados em adultos com EJUP, sendo o primeiro estudo na literatura a avaliar tais moléculas nesta população. MÉTODOS: Foram estudados de modo prospectivo pacientes consecutivos acima de 18 anos com diagnóstico de EJUP submetidos a pieloplastia videolaparoscópica de dezembro de 2013 a fevereiro de 2015. Foram excluídos do estudo pacientes com EJUP bilateral, rim contralateral patológico, EJUP em rim único, antecedentes de tratamento cirúrgico para estenose de JUP ou taxa de filtração glomerular inferior a 60 ml/min/1,73m2. Cada paciente forneceu quatro amostras de urina para medição de biomarcadores, uma no pré-operatório e outras com 1, 3 e 6 meses de seguimento pós-operatório. O grupo controle foi constituído por voluntários saudáveis sem hidronefrose à ultrassonografia. RESULTADOS: Foram incluídos 47 pacientes com idade média de 38,6 ± 12,7 anos (intervalo 19 a 64 anos), sendo 17 (36,2%) do sexo masculino e 30 (62,8%) do sexo feminino. O grupo controle foi composto por 40 indivíduos semelhantes ao grupo com EJUP no que concerne idade (p = 0,95) e sexo (p = 0,82). KIM-1 foi o marcador com melhores propriedades diagnósticas, apresentando área sob a curva (AUC) de 0,79 (95% CI 0,70 a 0,89). O NGAL, por sua vez, teve AUC de 0,71 (95% CI 0,61 a 0,83), CA19- 9 teve AUC de 0,70 (95% CI 0,60 a 0,81) e (beta2M) apresentou AUC de 0,61 (95% CI 0,50 a 0,73), sendo o único biomarcador com propriedades inadequadas neste cenário. O KIM-1 foi o marcador mais sensível com o ponto de corte 170,4 pg/mg de creatinina (sensibilidade 91,4%, especificidade 59,1%) e o CA 19-9 o mais específico para o ponto de corte de 51,3 U/mg de creatinina (sensibilidade 48,9%, especificidade 88,0%), enquanto o NGAL foi o que apresentou maior queda após desobstrução, com 90,0% dos pacientes apresentando clareamento superior a 50%. CONCLUSÕES: A avaliação dos biomarcadores urinários é útil no diagnóstico de obstrução em adultos com EJUP submetidos a pieloplastia videolaparoscópica. O KIM-1 foi o marcador mais sensível e o CA 19-9 o mais específico, enquanto o NGAL foi o que apresentou maior que com a desobstrução. Houve queda das concentrações dos marcadores após pieloplastia no período estudado. O papel exato dos biomarcadores urinários no cenário de obstrução em adultos deve ser mais amplamente investigado / INTRODUCTION AND OBJECTIVE: Ureteropelvic junction obstruction (UPJO) is an important cause of urinary tract obstruction and can lead to progressive deterioration of renal function. Thus the development of novel non-invasive methods capable of discriminating obstruction and hydronephrosis may be useful. Elevation of urinary biomarkers may provide early evidence of kidney damage in urinary obstruction. In this scenario, urinary concentrations of KIM-1 may be elevated following proximal tubular injury, while NGAL may increase as result of injury to proximal or distal tubule as well as to loop of Henle, CA19-9 after overproduction in the obstructed tubule and ?2 microglobulin (beta2M) after injury to the glomerulus or the proximal tubule. The aim of this study was to evaluate the diagnostic properties of these urinary biomarkers in adults with UPJO. METHODS: We prospectively studied consecutive patients older than 18 years diagnosed with UPJO undergoing laparoscopic pyeloplasty from December 2013 to February 2015 in our institution. Exclusion criteria included patients with bilateral UPJO, unilateral UPJO with contralateral pathologic kidney, solitary kidney, history of previous surgical treatment for UPJO or glomerular filtration rate below 60 ml/min/1,73m2. Each patient provided four voided urine samples for biomarker measurement, one at preoperative consultation and the others at 1, 3 and 6 months of postoperative follow-up. Healthy individuals with no hydronephrosis on ultrasound evaluation constituted our control group. RESULTS: We included 47 patients with a mean age of 38.6 ± 12.7 years (range 19-64 years), from which 17 (36.2%) were males and 30 (62.8%) were females. The control group consisted of 40 subjects with no statistical difference to the study group regarding age (p = 0.95) and gender (p = 0.82). KIM-1 had an area under the curve (AUC) of 0.79 (95% CI 0.70 to 0.89) and was the biomarker with the best diagnostic properties. CA19-9 had an AUC of 0.70 (95% CI 0.60 to 0.81), NGAL had an AUC of 0.71 (95% CI 0.61 to 0.83) and beta2M had an AUC of 0.61 (95% CI 0.50 to 0.73). KIM-1 was the most sensitive marker with a cutoff of 170.4 pg/mg creatinine (sensitivity 91.4%, specificity 59.1%) whereas CA 19-9 as the most specific one, displaying a cutoff of 51.3 U/mg creatinine (sensitivity 48.9%, specificity 88.0%). NGAL showed the greatest decrease in urinary concentrations after pyeloplasty, in which 90.0% of patients had a clearance greater than 50% in comparison to preoperative values. CONCLUSIONS: The evaluation of urinary biomarkers is useful in the assessment of UPJO in adults undergoing laparoscopic pyeloplasty. Urinary concentrations of CA 19-9, NGAL and KIM-1 were elevated in patients with UPJO and significantly decreased after pyeloplasty. The exact role of those biomarkers in the setting of obstruction in adults should be further evaluated

Impacto do reprocessamento de dialisadores de alto fluxo e alta eficiência sobre o transporte de solutos em sessões de hemodiafiltração online curta diária / Reprocessing high-flux, high-efficiency polysulfone dialyzers on solutes removal in short daily online hemodiafiltration

Introdução: Não há estudos que avaliem o impacto da reutilização do dialisador na remoção de solutos em sessões de hemodiafiltração online curta diária (HDF-OL-D). Objetivo: Avaliar o impacto do reuso do dialisador na dose do tratamento, na cinética de beta2-microglobulina e na extração de solutos em sessões de HDF-OL-D e comparar com sessões de hemodiálise curta diária de alto fluxo (HD-D). Métodos: Foram incluídos 14 pacientes do programa de HD-D. Foram coletadas amostras de sangue pré, no meio e pós-diálise e amostras do dialisato no 1º, 7º e 13º usos do dialisador em sessões de HD-D e de HDF-OL-D. Resultados: A massa total extraída (MTDQ) e a depuração (KDQ) diretamente quantificada dos solutos pequenos (ureia, fósforo, creatinina e ácido úrico), bem como a dose de diálise ofertada, foram semelhantes quando o 1º, 7º e 13º usos do dialisador em sessões de HD-D foram comparados, respectivamente, ao 1º, 7º e 13º usos em sessões de HDF-OL-D. A MTDQ e a KDQ dos solutos pequenos e a dose de diálise foram semelhantes entre os usos do dialisador tanto em sessões de HD-D e quanto em sessões de HDF-OL-D. A MTDQ, a KDQ e o Kt/V diretamente quantificado (Kt/VDQ) de beta2-microglobulina foram maiores em sessões de HDF-OL-D do que nos respectivos usos do dialisador em sessões de HD-D. Não houve diferença quanto à cinética de beta2-microglobulina, avaliada pela MTDQ, KDQ ou Kt/VDQ, entre o 1º, 7º e 13º usos do dialisador em sessões de HD-D nem em sessões de HDF-OL-D. Na HDF-OL-D, a perda intradialítica de albumina foi significativamente diferente entre os usos do dialisador (p < 0,001), estando reduzida no 7º e 13º usos, quando comparados ao 1º uso do dialisador. Não houve correlação entre o volume de enchimento do dialisador e a MTDQ e a KDQ de moléculas pequenas em nenhum dos métodos dialíticos estudados. Em sessões de HDF-OL-D, foi observada uma correlação fraca entre o volume de enchimento do dialisador e a KDQ de beta2-microglobulina, não percebida com a MTDQ ou o Kt/VDQ de beta2-microglobulina. Conclusão: O reprocessamento de dialisadores de alto fluxo e alta eficiência não resulta em comprometimento da dose do tratamento, da cinética de beta2-microglobulina nem da extração de solutos em sessões de HDF-OL-D. A extração de beta2-microglobulina foi maior em sessões de HDF-OL-D do que em sessões de HD-D, sem diferenças significativas na remoção dos demais solutos. O reprocessamento do filtro, em sessões de HDF-OL-D, resultou numa redução significativa da perda intradialítica de albumina. Não parece haver influência do volume de enchimento do filtro na extração de solutos em nenhum dos métodos de diálise estudados / Introduction: There no studies evaluating the impact of dialyzer reutilization on solute removal in daily online hemodiafiltration (D-OL-HDF) sessions. Objectives: Our aim was to evaluate the impact of dialyzer reuse on solute extraction, beta2-microglobulin kinetics and dialysis dose in D-OL-HDF and compare to those in high-flux short daily hemodialysis (D-HD). Methods: 14 patients undergoing a D-HD program were included. Pre, middle and post-dialysis blood samples and effluent dialysate were collected in the 1st, 7th and 13th dialyzer uses in D-HD sessions and in D-OL-HDF sessions. Results: Directly quantified small solute (urea, phosphorus, creatinine and uric acid) total mass removal (TMDQ) and clearance (KDQ), as well as dialysis dosis, were similar when the 1st, 7th and 13th dialyzer D-HD uses were compared to the 1st, 7th and 13th D-OL-HDF uses. TMDQ and KDQ of small solutes and dialysis dose were similar among dialyzer uses in D-HD sessions and also in D-OL-HDF sessions. beta2-microglobulin TMDQ, KDQ and directly quantified Kt/V (Kt/VDQ) were statistically higher in D-OL-HDF dialyzer uses than in the respective D-HD uses. There was no difference in beta2-microglobulin kinetics, evaluated by TMDQ, KDQ or Kt/VDQ, among 1st, 7th and 13th uses in D-OL-HDF sessions or in D-HD sessions. In D-OL-HDF, albumin loss was significantly different among studied dialyzer uses (p < 0.001), being reduced in the 7th and 13th dialyzer uses, when compared to the first use. There was no correlation between dialyzer priming volume and small molecules TMDQ, KDQ in neither analyzed dialytic method. In D-OL-HDF sessions, it was observed a week correlation between dialyzer priming volume and beta2-microglobulin KDQ, not observed with beta2-microglobulin MTDQ or Kt/VDQ. Conclusion: High flux, high efficiency dialyzer reprocessing did not did result in a reduction of the offered dialysis dose, beta2-microglobulin kinetics or solute extraction in D-OL-HDF. beta2-microglobulin removal was greater in D-OL-HDF than in D-HD sessions, without difference in other solutes extraction. There was a significant reduction in intradialytic albumin loss with dialyzer reprocessing in D-OL-HDF sessions. Dialyzer priming volume does not appear to influence solute removal in neither analyzed dialytic method

Relaxation of Isolated Human Myometrial Muscle by beta2-Adrenergic Receptors but Not beta1-Adrenergic Receptors

OBJECTIVE: Human myometrium contains both beta1-adrenergic and beta2-adrenergic receptors. This study was designed to assess the importance of each beta-adrenergic receptor subtype in relaxation of human myometrial muscle strips. STUDY DESIGN: Radioligand binding studies were used to establish the presence of each beta-adrenergic receptor subtype, whereas highly selective beta1-antagonists and beta2-antagonists were used to assess the contribution of beta-adrenergic receptor subtypes to myometrial relaxation after exposure to (-)-isoproterenol. RESULTS: Membranes prepared from myometrium contained 82% +/- 4% beta2-adrenergic receptors. After contraction produced by exposure to potassium chloride (35 mmol/L), isoproterenol produced relaxation with half maximal effect at 0.02 micromol/L and a maximal relaxation of 52% +/- 3%. Beta1-antagonist CGP-20712A had no significant effect, whereas beta2-antagonist ICI-118551 produced a characteristic rightward shift of the isoproterenol concentration-relaxation relationship. CONCLUSIONS: Although both beta1-adrenergic receptors and beta2-adrenergic receptors are present in human myometrial tissue at term, relaxation by nonselective beta-agonist isoproterenol is mediated exclusively by beta2-adrenergic receptors.

Modellierung und Visualisierung von Systemen zur Beschreibung der intra- und intermolekularen Wechselwirkungen in hydrophoben Peptiden

Die vorliegende Arbeit beschäftigt sich mit der Untersuchung und Beschreibung der Eigenschaften der synthetischen Dekapeptide Cetrorelix und Ozarelix durch analytische Methoden und computergestützte Modellierung. Diese Moleküle sind hydrophobe, aggregierende Antagonisten des Gonadotropin-Releasing-Hormons (GnRH). Zusätzlich wurden amyloidbildende Peptidstrukturen als Modelle für die Assoziationsprozesse in hydrophoben Peptiden untersucht und visualisiert. Die intrinsische Fluoreszenz der GnRH-Antagonisten und zusätzlich der Peptide Teverelix und D-Phe6-GnRH sowie von verkürzten Fragmenten des Cetrorelix wurde untersucht. Ein Strukturmodell für die Beschreibung der Aggregation der Dekapeptide wurde erarbeitet. Der Aufbau eines Rechenclusters durch das Einbinden der Computer am Lehrstuhl in ein Linux-System zur Verteilung von Rechenprozessen über das Netzwerk ermöglichte die Bereitstellung der notwendigen Leistung zur Realisierung der Berechnungen. Es wurden Werkzeuge zur Modellierung der solvatisierten Aggregate von Peptiden ohne eindeutige Vorzugsstruktur programmiert und in ein Docking-System für beliebige Moleküle eingebunden. Verwendet wurde das Kraftfeld MMFF94 mit einer Erweiterung durch ein Verfahren zur dynamischen Berechnung von Partialladungen in Molekülstrukturen. Solvatisierte Aggregate der Dekapeptide und von bekannten amyloidbildenden Strukturen wurden modelliert (Docking). Berechnet wurden als aggregierend beschriebene Sequenzen und entsprechende Vergleichsstrukturen des Calcitonins, des Insel-Amyloid-Polypeptides, des beta2-Mikroglobulins, des Amyloid-beta-Proteins, des Lactoferrins und weitere Modellpeptide. Die wesentlichen Wechselwirkungen während der Aggregation konnten schließlich anhand von Dynamik-Simulationen der faltblattartigen Dimere des Cetrorelix und Ozarelix beschrieben werden. So wurden die Prozesse der hydrophoben Assoziation und Stabilisierung durch Wasserstoffbrücken von Peptiden veranschaulicht und auf molekularer Ebene erfolgreich analysiert. Die Visualisierung der erhaltenen Modellierungsergebnisse erfolgt durch die Darstellung der Strukturen und Dynamik-Simulationen als interaktive 3D-Modelle in einem für diese Arbeit aufgebauten Internetauftritt. / This work discusses the analysis of the aggregation properties of the gonadotropin releasing hormone antagonists Cetrorelix, Teverelix, Ozarelix and of small amyloid forming model peptides by analytical fluorescence spectroscopy and molecular modelling. A high performance linux compute cluster was developed for calculation of molecular structures. Solvated aggregate clusters of peptides without defined secondary structure were modelled by molecular mechanics methods (forcefield mmff94) in combination with an advanced charge equilibration and docking technique. Molecular dynamics of solvated peptide dimers were implemented and the role of hydrophic association and hydrogen bond formation in hydrophobic peptide aggregates was explained. Finally, an aggregation model for the directed association of hydrophobic peptides is presented. The modelling results, 3d structures and dynamic simulations are visualized in an interactive web material.
