Análise da mecânica ventilatória de indivíduos portadores de esclerose sistêmica através da Técnica de Oscilações Forçadas. / Analysis of ventilatory mechanics of subjects with systemic sclerosis by Forced Oscilation Technique

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Systemic sclerosis (SS) is a chronic and multisystemic disease characterized by fibrosis and vascular abnormalities in the skin and in visceral organs. Pulmonary complications are the most common cause of death and morbidity associated to the disease. The pulmonary function tests are abnormal in 67-95% of patients with SS. Some studies have demonstrated the potential of the forced oscillation technique (FOT) in the detection of respiratory mechanics modifications in several respiratory diseases, but there are no studies in the literature using this technique in patients with systemic sclerosis. Thus, the goals of the present study were: (1) to analyze changes in resistive and reactive properties of the respiratory system in patients with SS and (2) to investigate the diagnostic potential of this technique. This research consists of a controlled observational study. In the first part of the study, considering spirometry, 82 individuals were analyzed, 30 control subjects and 52 patients with diagnostic of SS, 22 with normal spirometry and 30 with restrictive alteration to spirometry. In a second study, based on evaluation of lung volumes, 50 individuals were evaluated, 21 control subjects and 29 with systemic sclerosis, 7 with normal result on volumes exam and 22 with restrictive ventilatory disturbance in this exam. All subjects performed FOT exam for the analysis of resistive and reactive properties of the respiratory system. In subsequent examinations, the individuals were submitted to spirometry and/or volumes exam. Considering the groups divided from the spirometry, the resistive and reactive parameters and the impedance module of the respiratory system at 4Hz (|Z4Hz|) showed significantly alterations (KW- ANOVA/ANOVA, p<0.004). In the comparison between groups, the restrictive group showed the highest changes in all study variables. Considering the division by lung volumes exam, the elastic parameters, |Z4Hz| and the slope of the resistance (S) showed significant changes (KW-ANOVA, p <0.006), while the others resistive variables (total and mean resistance, R0 and Rm, respectively) showed a tendency to increase. In this analysis, the restrictive group also showed greater impairment in oscillometric and volumetric parameters. The SS resulted in changes in resistive and reactive properties of the respiratory system. Analyzing the diagnosis power of FOT, with spirometry as reference, when comparing the normal exam group with restrictive group, the mean reactance (Xm) showed useful clinical value with an area under the curve (AUC) of 0.81. In the comparison between the control and restrictive groups, R0, dynamic compliance (Cdyn,rs) and |Z4Hz| showed the following AUC values, 0.81, 0.95 and 0.91, respectively, representing high accuracy for the last two variables. Using volumes exam as reference, comparing control and restrictive groups, the Xm and resonance frequency (fr) were noteworthy with values of 0.82 and 0.81 of AUC, respectively. Those results are consistent with physiopathological changes related to SS, which may include diffuse fibrosis that affects the lung, the bronchial wall and the alveolar septum with changes in parenchimal architecture, confirming the potential of FOT in the assessment of modifications in respiratory mechanics related to SS. / A Esclerose sistêmica (ES) é uma doença crônica, multissistêmica caracterizada por fibrose e anormalidades vasculares na pele e em órgãos viscerais. As complicações pulmonares são as principais causas de morte e morbidades associadas à doença. Os testes de função pulmonar apresentam-se anormais em 67-95% dos pacientes com ES. Alguns estudos têm demonstrado o elevado potencial da Técnica de Oscilações Forçadas (FOT) na detecção das alterações da mecânica respiratória de diversas doenças respiratórias, no entanto não existem trabalhos na literatura que utilizem esta técnica em pacientes com esclerose sistêmica. Desta forma, os objetivos desta pesquisa foram (1) analisar as alterações das propriedades resistivas e reativas do sistema respiratório em portadores de ES e (2) investigar o potencial de uso diagnóstico desta técnica. Trata-se de um estudo observacional controlado. Na primeira parte do estudo, realizada com base no exame espirométrico, foram analisados 82 indivíduos, 30 indivíduos controles e 52 pacientes com diagnóstico de ES, sendo 22 normais ao exame espirométrico e 30 com distúrbio ventilatório restritivo à espirometria. Em um segundo estudo, baseado na avaliação de volumes pulmonares, 50 indivíduos foram avaliados, 21 indivíduos controles e 29 com ES, sendo 7 normais ao exame de volumes e 22 com distúrbio ventilatório restritivo detectado por esse exame. Todos os indivíduos realizaram exames da FOT para análise das propriedades resistivas e reativas do sistema respiratório. Posteriormente aos exames da FOT, os indivíduos foram submetidos à espirometria e/ou exame de volumes. Considerando os grupos divididos a partir da espirometria, os parâmetros resistivos, reativos e o módulo da impedância do sistema respiratório em 4Hz (|Z4Hz|) modificaram-se significativamente (KW- ANOVA/ANOVA, p<0,004). Na comparação entre grupos, o grupo restritivo apresentou as maiores alterações em todas as variáveis estudadas. Considerando o exame de volumes pulmonares, os parâmetros elásticos, |Z4Hz| e a inclinação da curva de resistência (S) apresentaram alterações significativas (KW-ANOVA, p<0,006), já as demais variáveis resistivas (resistências total e média, R0 e Rm, respectivamente) demonstraram uma tendência à aumento. Nessa análise, o grupo restritivo também apresentou maior prejuízo nos parâmetros oscilométricos e volumétricos. A ES resultou em alterações nas propriedades resistivas e reativas do sistema respiratório. Na análise do poder diagnóstico da FOT, sendo a espirometria referência, na comparação entre o grupo normal ao exame e o restritivo, a reatância média (Xm) apresentou valor útil para uso clínico com valor de área sob a curva (AUC) de 0,81. Já na comparação entre os grupos controle e restritivo, R0, complacência dinâmica (Cdin,sr) e |Z4Hz| apresentaram os seguintes valores de AUC, 0,81, 0,95 e 0,91, respectivamente, representando alta acurácia para as últimas duas variáveis. Com o exame de volumes como referência, comparando os grupos controle e restritivo, a Xm e a freqüência de ressonância (fr) destacaram-se com valores de 0,82 e 0,81 de AUC, respectivamente. Esses resultados são coerentes com as alterações fisiopatológicas relacionadas à esclerose sistêmica, que podem incluir fibrose difusa envolvendo o interstício pulmonar, a parede brônquica e o septo alveolar com alteração da sua arquitetura, confirmando o potencial da FOT na avaliação das modificações na mecânica respiratória relacionadas à ES.

Condições mecânicas do sistema respiratório em adultos com fibrose cística: estudo pela espirometria, pletismografia e técnica de oscilações forçadas / Mechanics conditions of respiratory system in adults with systic bibrosis: study by spirometry, plethysmography and forced oscillation technique

Cystic fibrosis (CF) is an autosomal recessive disease more common in the white population, which leads to reduction in life expectancy. Pulmonary disease is the major cause of morbidity and mortality. The relevance of this study takes on a some factors: the drastic increase in median survival in the last 60 years in CF; the pulmonary pathophysiology remains not well understood; up to now there is no reported study using forced oscillation technique (FOT) exclusively in adults with CF. Thus the aims of this study are: to analyze the changes in respiratory mechanics in adults with CF by spirometry, plethysmography and FOT; correlate the results of FOT to the spirometric and plethysmographic data and evaluate the sensitivity and specificity of FOT in these individuals. This is a descriptive cross-sectional study enrolling two groups of individuals: control (n=23) and CF (n=27). The results were expressed by mean standard deviation. The techniques applied to evaluate the lung function were performed as follows: FOT, spirometry and plethysmography. The following parameters were evaluated by plethysmography: total lung capacity (TLC); functional residual capacity (FRC); residual volume (RV); FRC/TLC; RV/TLC; resistance airway (Rva) and specific airway conductance (SGva). On spirometry: first second forced expiratory volume (FEV1); forced vital capacity (FVC); forced expiratory flow between 25% and 75% of FVC (FEF25% - 75%) and the relations FEV1/ FVC (%) and FEF/FVC (%). In FOT: the resistance properties of the respiratory system - R0 (resistance at the intercept); medium resistance (Rm); slope of the linear resistance (S) and reactive properties: dynamic compliance of the respiratory system (Cdyn,sr); mean reactance (Xm); resonance frequency (fr) and impedance module at 4 Hz (|Zrs4Hz|). In the spirometry parameters, obstructive respiratory disease (ORD) with reduced FVC prevailed, with marked reduction in FEF25% - 75% in CF group (p <0.0001) compared to control. It was observed in plethysmography an elevation of RV, in the presence of normal TLC and elevated Rva and decreased SGva in the group with CF. Changes in the FOT occurred in the CF group compared to control: increased R0, Rm (p <0.0001) and fr (p <0.0002) related to obstruction of the airways; reduction of S (p <0.0006) and Xm (P <0.0001) associated with non-homogeneity of the respiratory system and reduction in the Cdyn, sr (p <0.0001) related to the pulmonary compliance. Increased module of the impedance at 4 Hz (| Zrs4Hz |) (representing total mechanical load of the respiratory system) resulting from the interaction of other changes in the FOT cited. The FOT parameters showed very good correlation with spirometry and moderated correlation with plethysmography. Rm was the only parameter that was not related to any of these techniques. Sensitivity and specificity of FOT in adults with CF was elevated, mainly for the reactive parameters, particularly, Xm (85.2% and 73.9%, respectively and area under the curve 0.86). / A fibrose cística (FC) é a doença autossômica recessiva mais comum na população branca que leva à redução na expectativa de vida. A doença pulmonar é a maior causa de morbidade e mortalidade. A relevância do presente estudo se dá diante de alguns fatores: aumento drástico da sobrevida média nos últimos 60 anos na FC, a fisiopatologia pulmonar não é bem compreendida, ausência de estudos reportados na literatura, até o momento, utilizando a técnica de oscilações forçadas (TOF) exclusivamente em adultos com FC. Assim sendo os objetivos deste estudo são: analisar as alterações da mecânica respiratória em adultos com FC através da espirometria, pletismografia e TOF; correlacionar os resultados da TOF aos espirométricos e pletismográficos e avaliar a sensibilidade e especificidade da TOF nestes indivíduos. É um estudo de corte transversal descritivo, no qual foram analisados dois grupos de indivíduos: controle (n=23) e FC (n=27). Os resultados foram expressos através média desvio-padrão. As técnicas funcionais respiratórias foram realizadas na seguinte sequência: TOF, espirometria, pletismografia. Na pletismografia foram avaliados os parâmetros: CPT (capacidade pulmonar total), CRF (capacidade residual funcional) e VR (volume residual), CRF/CPT e VR/CPT, resistência (Rva) e condutância específica das vias aéreas (SGva). Na espirometria: volume expiratório forçado no primeiro segundo (VEF1), capacidade vital forçada (CVF), fluxo expiratório entre 25% e 75% (FEF25%-75%) da CVF (FEF25%-75%) e razões VEF1/CVF (%) e FEF/CVF (%). Na TOF: propriedades resistivas do sistema respiratório- R0 (resistência no intercepto), Rm (resistência média) e S (inclinação da reta de resistência) e propriedades reativas: Cdin,sr (complacência dinâmica do sistema respiratório), Xm (reatância média), frequência de ressonância (fr); e o módulo da impedância em 4 Hz (&#1472;Zrs4Hz&#1472;). Na espirometria o distúrbio ventilatório obstrutivo (DVO) com CVF reduzida foi predominante, com marcante redução do FEF25%-75% no grupo FC (p<0,0001) em relação ao controle. Na pletismografia: destacou-se a elevação de VR, na presença de CPT normal e elevação da Rva e redução da SGva no grupo FC. Alterações da TOF ocorridas no grupo FC em relação ao controle: aumento de R0 e Rm (p<0,0001) e fr (p<0,0002), relacionados à obstrução das vias aéreas; redução de S (p<0,0006), Xm (p<0,0001) associadas à não-homogeneidade do sistema respiratório e Cdin,sr (p<0,0001), relacionada à redução da complacência pulmonar; aumento do módulo da impedância em 4 Hz (&#1472;Zrs4Hz&#1472;) (representando a carga mecânica total do sistema respiratório) resultante da interação das demais alterações da TOF citadas. Os parâmetros da TOF apresentaram correlações muito boas com a espirometria e moderadas com a pletismografia. Rm foi o único parâmetro que não se relacionou com nenhuma destas técnicas. A sensibilidade e especificidade da TOF em adultos com FC apresentaram valores elevados, sobretudo nos parâmetros reativos, em especial, Xm (85,2% e 73,9% respectivamente e área sob a curva de 0,86).

Entretanto, a exposição ao tempo prolongado na UTI, ao uso de ventilação mecânica (VM) e a medicamentos podem levar à perda da Função Pulmonar (FP) desses indivíduos. Nesse contexto, a análise da FP é importante no desenvolvimento de terapêuticas intervencionistas. Objetivo: Analisar a FP de pacientes críticos durante a internação em UTI, após sete dias da alta da UTI e estabelecer equações de predição da Pimáx e PEmáx em pacientes críticos no despertar e na alta de UTI. Materiais e Métodos: Estudo de coorte prospectiva na UTI no Hospital Regional de Ceilândia (HRC) entre maio de 2015 e julho de 2016. Foi realizada entrevista com os familiares dos pacientes e leitura dos prontuários eletrônicos para caracterização da amostra. Os Testes de Função Pulmonar (TFP) conglomeraram a avaliação das Pressões Respiratórias Máximas (PRMs) e Espirometria, realizados no despertar, na alta e sete dias após a alta da UTI. Resultados: Foram elegíveis 35 pacientes para avaliação das PRMs no despertar e alta, desses, 25 foram elegíveis para avaliação da espirometria na alta. No follow-up de sete dias após a alta da UTI, 21 realizaram teste PRMs e 16 avaliações espirometrica. Foi possível observar aumento da FP ao longo do tempo avaliado, porém os valores obtidos foram significativamente menores (p≤0,05) em todos os momentos avaliados, quando comparados com os valores previstos presentes na literatura. Por meio da análise da FP com base no modelo de regressão linear múltipla, foram estabelecidas equações de predição para PRMs no despertar e na alta. Conclusão: Os pacientes críticos apresentaram diminuição da função pulmonar mantida até sete dias após a alta de UTI, quando comparados aos valores preditos para pacientes saudáveis. / Introduction: Currently the number of patients submitted to hospitalization in the Intensive Care Unit (ICU) is increasing. However, prolonged ICU exposure, the use of mechanical ventilation (MV), and medications that can be used in the loss of pulmonary function (PF). In this context, a PF analysis is important in the development of interventional therapies. Objective: To analyze a critical patient's FP during an ICU stay after seven days of ICU discharge and to establish prediction equations for MIP and MEP in critically ill patients without awakening and at ICU discharge. Materials and Methods: Prospective cohort study at the ICU not Hospital Regional de Ceilândia (HRC) between May 2015 and July 2016. An interview was carried out with the relatives of the patients and the electronic records were read to characterize the sample. The Pulmonary Function Tests (TFP) conglomerated the evaluation of the Maximum Respiratory Pressure (PRMs) and Spirometry, performed without awakening, on discharge and seven days after discharge from the ICU. Results: Thirty-five patients were eligible for the evaluation of MRPs on awakening and discharge, of which 25 were eligible for evaluation of spirometry at discharge. Without follow-up seven days after discharge from the ICU, 21 performed PRMs and 16 spirometric tests. It was found at all times, when compared with the predicted values in the literature. By means of FP analysis based on no linear multiple regression model, prediction equations were established for PRMs on awakening and discharge. Conclusion: Critical patients presented a decrease in lung function maintained up to seven days after an ICU discharge, when compared to predicted values for healthy patients.

Comparação dos testes de escada, caminhada e espirometria preditos com os obtidos no pós-operatório de ressecções pulmonares /

Resumo: A ressecção de tecido pulmonar funcionante leva a uma redução definitiva dos volumes e capacidades pulmonares, podendo seus valores serem preditos para o pós-operatório (ppo) através de uma regra de três simples. Como os valores espirométricos só avaliam os pulmões em repouso e não a real capacidade física do indivíduo, partiu-se da hipótese de que os testes de escada (TE) e de caminhada de 6 minutos (TC6) poderiam também se alterar proporcionalmente ao pulmão funcionante ressecado. Verificar se os testes usados como preditores de risco cirúrgico (VEF1, TC6, TE) se alteram proporcionalmente ao pulmão funcionante ressecado. O estudo incluiu pacientes candidatos a toracotomia para ressecção pulmonar que preenchiam os critérios de inclusão e concordaram em participar da pesquisa. No pré-operatório (pré) e no mínimo 3 meses após a cirurgia (pós), realizaram espirometria, teste de escada e teste de caminhada, no mesmo dia, com intervalo mínimo de 20 minutos entre os testes. A curva de capacidade vital foi realizada com o paciente sentado, pelo menos por três vezes, escolhendo-se aquela em que o VEF1 foi maior. O TC6 foi realizado segundo as normas da American Thoracic Society, em corredor plano de 120 metros à sombra. O TE foi realizado em escada com uma inclinação de 30°, à sombra, composta por 6 lances, com 12 degraus por lance (72 degraus), cada degrau medindo 16,9cm, num total de 12,16m de altura. O paciente foi orientado a subir todos os degraus no menor tempo possível, com incentivo verbal, padronizado a cada lance. O tempo em segundos percorrido na subida da altura total foi denominado tempo de escada (tTE). Os cálculos dos valores preditos para o pós-operatório (ppo) para o VEF1 e TC6 foram realizados por regra de três simples direta, a partir dos resultados... (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Resection of the functioning lung tissue leads to a definite reduction in the lung volumes and capacities, and postoperative (ppo) values can be predicted through a simple rule of three. As spirometric values only evaluate lungs at rest and not the individual's real physical capacity, stair-climbing test (SCT) and 6-minute walk test (6MW) were suggested to change proportionally to the resected functioning lung. To verify whether tests used as predictors of surgical risk (FEV1, 6MW, SCT) change proportionally to the resected functioning lung. The study included patients candidate for thoracotomy for lung resection and who fulfilled the inclusion criteria, agreeing to participate in the investigation. In the preoperative period (pre) and at least three months after surgery (pos), the patients were subjected to spirometry, stair-climbing and walk tests on the same day, with a minimal interval of 20 minutes between tests. The vital capacity curve was done with the patient sit and repeated at least three times; the curve with the highest FEV1 was chosen. 6MW was carried out, according to the American Thoracic Society standards, in a 120m plain alley in the shade. SCT was also done in the shade in a 30°-inclination staircase of 12 steps in each of the six sets (72 steps) and each step measured 16.9cm in a total of 12.16m height. The patient was instructed to climb up all steps as fast as possible, being verbally stimulated, which was standardized for each set of steps. The time in seconds for total climb-up was named stair-climbing time (tSCT). The predicted postoperative (ppo) values for FEV1 and 6MW were calculated through direct simple rule of three, based on the results obtained in the preoperative tests and according to the number of lost functioning segments; for tSCT, values were determined through inverse rule of three. Avaliação do desenvolvimento pulmonar em crianças prematuras do primeiro ano de vida à idade escolar

There is extensive material on the effects of prematurity on lung function in early life, but little has been studied over the long term outcomes. This study therefore has the objective to describe lung development in premature infants, using data collected in the first year of life and correlating these data with functional tests obtained years later. To address this question we conducted a cohort study, prospective, where examinations were performed pulmonary function in infants in a group of children born prematurely, before the first year of life, and after some years the children were again subjected to spirometry to assess and performing development lung compared to the results of the first test. 40 children underwent lung function testing of infants (first test) and 37 children performed the second test. Of the 40 subjects, 17 (42%) were male, and 25 (63%) were white, the variation of Z scores for FVC and FEF2575 between the two tests showed statistically significant increase (p from 0. 028 to 0. 016). The predictor variables that influenced these changes were gender (male sex) and infection pre-and postnatal (chorioamnionitis and early sepsis).In conclusion, our data suggest that there is a trend of accelerated growth of lung function between the first and fifth years of life for children born premature, both in volume and in pulmonary flow, suggesting that, in part, this growth is due to the regression to the mean. Exposure to infection in the neonatal period has a limited effect on lung function with reduction in expiratory flows during the first years of life. These abnormalities are not detected later in life. / A função pulmonar de crianças nascidas prematuras é reduzida e existem vários fatores pré e pós-natais associados a fluxos baixos. Existe vasto material sobre efeitos da prematuridade na função pulmonar nos primeiros meses de vida, mas pouco se estudou sobre os desfechos em longo prazo. Este estudo tem, portanto, o objetivo de descrever o desenvolvimento pulmonar em prematuros, usando dados coletados no primeiro ano de vida e correlacionando estes dados com os testes funcionais obtidos anos mais tarde. Para responder a esta questão foi realizado um estudo de coorte, prospectivo, onde foi realizado exame de função pulmonar de lactentes em um grupo de crianças nascidas prematuras, antes do primeiro ano de vida, e após alguns anos as crianças foram novamente submetidas à espirometria para avaliar o seu desenvolvimento pulmonar e realizar comparação com os resultados do primeiro exame. 40 crianças realizaram o teste de função pulmonar de lactente (primeiro exame) e 37 crianças executaram o segundo teste. Dos 40 sujeitos, 17 (42%) eram do sexo masculino, e 25 (63%) eram da raça branca, a variação dos escores Z da CVF e do FEF2575 entre os dois testes realizados apresentaram aumento estatisticamente significativa (p de 0,028 e 0,016). As variáveis preditoras que influenciaram nestas alterações foram sexo (masculino) e infecção pré e pós-natal (corioamnionite e sepse precoce).Concluindo, os dados deste estudo sugerem que existe uma tendência de crescimento acelerado da função pulmonar entre o primeiro e o quinto anos de vida de crianças nascidos prematuras, tanto em volume quanto em fluxo pulmonares, sugerindo que, em parte, esse crescimento deva-se a regressão à média. A exposição à infecção no período neonatal tem efeito limitado no pulmão, com redução de fluxos expiratórios durante os primeiros anos de vida. Estas alterações espirométricas não são detectadas quatro a seis anos mais tarde.

Some patients may present structural changes in the airway wall, with an irreversible loss of lung function (LF) parameters. The aim of this study was to describe the phenotype and LF characteristics of children with persistent asthma. Methods: information was collected from 96 children (5-17 years) using ISAAC questionnaire, spirometry and skin prick test (SPT) for aeroallergens. Irreversible loss of LF was defined as FEV1/FVC that persists < 0. 85. Atopy was characterized according to standard ISAAC protocol. Atopic index was used to define non/mild-reactive (response to < 4 allergens) or multi-reactive patients (response to b 4 allergens). Results: Among the 96 children analyzed, 92. 7% were atopic and 8. 3% presented persistently reduced lung function, irreversible even after treatment with steroids. Among asthma severity groups, it was observed significant differences in FEV1 between moderate and severe asthma (p=0. 006). Regarding atopy, multi-reactive asthmatics presented lower LF than non/mild-reactive patients (FEV1/FVC, p=0. 032 and FEF25-75%, p=0. 026). Conclusions: Most of the children with persistent asthma presented normal or reversible LF. The atopic index of b 4 allergens and severity of the disease are predictors to reduced LF parameters. / Introdução: A asma é uma doença inflamatória crônica das vias aéreas caracterizada pela limitação reversível ao fluxo aéreo. Alguns pacientes podem apresentar modificações estruturais nas vias aéreas, com uma perda irreversível dos índices de função pulmonar (FP). O objetivo deste estudo foi descrever as características fenotípicas e de FP de crianças com asma persistente. Métodos: Foram coletadas informações de 96 crianças (5-17 anos), através do questionário ISAAC, e realizada espirometria e teste cutâneo para aeroalergenos. Perda irreversível de FP foi definida como VEF1/CVF persistentemente < 0,85. Atopia foi caracterizada conforme o protocolo ISAAC. O índice de atopia foi utilizado para definir pacientes pouco-reatores (resposta para < 4 alérgenos) e multi-reatores (resposta para b 4 alérgenos). Resultados: Dentre as 96 crianças estudadas, 92,7% eram atópicas e 8,3% apresentaram FP persistentemente reduzida, sendo irreversível mesmo após ciclo de tratamento com corticosteróide. Entre os grupos de gravidade da doença, observou-se diferença significativa entre moderados e graves no parâmetro de VEF1 (p=0,006). Com relação à atopia, pacientes multi-reatores apresentaram índices de FP inferiores aos pouco-reatores (VEF1/CVF, p=0,032 e FEF25-75%, p=0,026). Conclusões: A maioria das crianças com asma persistente apresenta FP normal ou reversível. Cerca de 8% dos pacientes demonstraram índices de FP persistentemente reduzidos, sugerindo que este pequeno grupo já possui alterações compatíveis com remodelamento pulmonar. O índice de atopia b 4 alérgenos e gravidade da doença foram preditores para redução dos níveis de FP.

Methods: Multicenter cross-sectional study of Brazilian children in 3 regions of Brazil. Data were collected between 2010 and 2012. We use the calculation of Z-scores of each individual software by Global Lung Initiative. Results: Spirometry was obtained satisfactorily in 456 children aged between three and 12 years (45. 8% male) and 70% were white, with the collaboration of 10 Brazilian centers. Of the 5 variables, only FEV1 had a mean above 0. 5 suggesting a slight disagreement with the GLI 2012.Conclusion: In summary, the data collected in 10 collaborating centers suggest that equation GLI 2012 can be used in Brazilian children aged 3 to 12 years of age. / Objetivos: Obter espirometrias em uma amostra representativa de crianças brasileiras sadias do ponto de vista respiratório de três a 12 anos de idade e testar a validade dos valores de referência multi-étnicos para espirometria na faixa etária dos três anos 95 anos propostas por Global Lung Iniciative (GLI) 2012 em nossa população. Material e Métodos: Estudo transversal e multicêntrico de crianças brasileiras em três regiões do Brasil. Os dados foram coletados entre os anos de 2010 e 2012. Utilizamos o cálculo do escores Z de cada indivíduo pelo software Global Lung Initiative. Resultados: Espirometria foi obtida satisfatoriamente em 456 crianças com idade entre três e 12 anos de idade (45,8% sexo masculino) sendo 70% da raça branca, com a colaboração de 10 centros brasileiros. Das cinco variáveis analisadas, apenas o VEF1 apresentou média acima de 0,5 sugerindo uma discordância leve com a GLI 2012.Conclusão: Em resumo, os dados coletados em 10 centros colaboradores sugerem que a equação GLI 2012 pode ser usada em crianças brasileiras de três a 12 anos de idade.

Conclus?o: Indiv?duos com CP acima de 42 cm apresentaram maior preval?ncia de redu??o da fun??o pulmonar e a CP foi a medida que apresentou maior associa??o com a fun??o pulmonar reduzida nos obesos. / Introduction: Obesity shows changes in pulmonary function and respiratory mechanics, however, little is known regarding the prevalence of worsening respiratory function when considering the increase in central or peripheral adiposity or general obesity. Objectives: To analyze the association between anthropometric adiposity and decreased lung function in obese. Materials and Methods: Patients eligible for this study obese individuals (IMC?30kg/m2) in pre-bariatric surgery and referred for Treatment Clinic of Obesity and Related Diseases, located at the University Hospital Onofre Lopes (HUOL), from October 2005 and July 2014. The evaluation included clinical information and measurement of anthropometric measures (body mass index (BMI), body fat index (BFI) and waist circumference (WC) and neck (NC)) and spirometric. The prevalence and analysis by Poisson regression was performed considering the following outcome variables: forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and Maximum Voluntary Ventilation (MVV) and as predictor variables were considered: BMI, IAC, WC and NC and as control variables: age, gender, smoking history and comorbidities (diabetes mellitus, dyslipidemia and hypertension). Statistical analysis was performed using Statistical Package for Social Sciences software (SPSS - version 20.0). Results: We analyzed 384 individuals, 75% women, mean BMI: 46.6 (? 8.7) kg/m2, IAC: 49.26 (? 9.48)%, WC: 130.84 (? 16.23) cm and NC: 42.3 (? 4.6) cm. The higher prevalence of FVC and FEV1 <80% was observed in individuals with NC above 42 cm, followed those with a BMI above 45 kg/m2. Multivariate analysis using Poisson regression showed as risk factors associated with FVC <80%, the variables: NC above 42 cm (odds ratio (OR) 2.41) and BMI over 45Kg/m2 (OR 1.71 ). As for FEV1 <80% predicted, all predictor variables were associated, with the largest odds presented by the NC (3.40). MVVV was not associated with any studied varaible. Conclusion: Individuals with NC above 42 cm had higher prevalence of reduced lung function and the NC was the measure with the highest association with reduced lung function in obese.

Análise da mecânica ventilatória de indivíduos portadores de esclerose sistêmica através da Técnica de Oscilações Forçadas. / Analysis of ventilatory mechanics of subjects with systemic sclerosis by Forced Oscilation Technique

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Systemic sclerosis (SS) is a chronic and multisystemic disease characterized by fibrosis and vascular abnormalities in the skin and in visceral organs. Pulmonary complications are the most common cause of death and morbidity associated to the disease. The pulmonary function tests are abnormal in 67-95% of patients with SS. Some studies have demonstrated the potential of the forced oscillation technique (FOT) in the detection of respiratory mechanics modifications in several respiratory diseases, but there are no studies in the literature using this technique in patients with systemic sclerosis. Thus, the goals of the present study were: (1) to analyze changes in resistive and reactive properties of the respiratory system in patients with SS and (2) to investigate the diagnostic potential of this technique. This research consists of a controlled observational study. In the first part of the study, considering spirometry, 82 individuals were analyzed, 30 control subjects and 52 patients with diagnostic of SS, 22 with normal spirometry and 30 with restrictive alteration to spirometry. In a second study, based on evaluation of lung volumes, 50 individuals were evaluated, 21 control subjects and 29 with systemic sclerosis, 7 with normal result on volumes exam and 22 with restrictive ventilatory disturbance in this exam. All subjects performed FOT exam for the analysis of resistive and reactive properties of the respiratory system. In subsequent examinations, the individuals were submitted to spirometry and/or volumes exam. Considering the groups divided from the spirometry, the resistive and reactive parameters and the impedance module of the respiratory system at 4Hz (|Z4Hz|) showed significantly alterations (KW- ANOVA/ANOVA, p<0.004). In the comparison between groups, the restrictive group showed the highest changes in all study variables. Considering the division by lung volumes exam, the elastic parameters, |Z4Hz| and the slope of the resistance (S) showed significant changes (KW-ANOVA, p <0.006), while the others resistive variables (total and mean resistance, R0 and Rm, respectively) showed a tendency to increase. In this analysis, the restrictive group also showed greater impairment in oscillometric and volumetric parameters. The SS resulted in changes in resistive and reactive properties of the respiratory system. Analyzing the diagnosis power of FOT, with spirometry as reference, when comparing the normal exam group with restrictive group, the mean reactance (Xm) showed useful clinical value with an area under the curve (AUC) of 0.81. In the comparison between the control and restrictive groups, R0, dynamic compliance (Cdyn,rs) and |Z4Hz| showed the following AUC values, 0.81, 0.95 and 0.91, respectively, representing high accuracy for the last two variables. Using volumes exam as reference, comparing control and restrictive groups, the Xm and resonance frequency (fr) were noteworthy with values of 0.82 and 0.81 of AUC, respectively. Those results are consistent with physiopathological changes related to SS, which may include diffuse fibrosis that affects the lung, the bronchial wall and the alveolar septum with changes in parenchimal architecture, confirming the potential of FOT in the assessment of modifications in respiratory mechanics related to SS. / A Esclerose sistêmica (ES) é uma doença crônica, multissistêmica caracterizada por fibrose e anormalidades vasculares na pele e em órgãos viscerais. As complicações pulmonares são as principais causas de morte e morbidades associadas à doença. Os testes de função pulmonar apresentam-se anormais em 67-95% dos pacientes com ES. Alguns estudos têm demonstrado o elevado potencial da Técnica de Oscilações Forçadas (FOT) na detecção das alterações da mecânica respiratória de diversas doenças respiratórias, no entanto não existem trabalhos na literatura que utilizem esta técnica em pacientes com esclerose sistêmica. Desta forma, os objetivos desta pesquisa foram (1) analisar as alterações das propriedades resistivas e reativas do sistema respiratório em portadores de ES e (2) investigar o potencial de uso diagnóstico desta técnica. Trata-se de um estudo observacional controlado. Na primeira parte do estudo, realizada com base no exame espirométrico, foram analisados 82 indivíduos, 30 indivíduos controles e 52 pacientes com diagnóstico de ES, sendo 22 normais ao exame espirométrico e 30 com distúrbio ventilatório restritivo à espirometria. Em um segundo estudo, baseado na avaliação de volumes pulmonares, 50 indivíduos foram avaliados, 21 indivíduos controles e 29 com ES, sendo 7 normais ao exame de volumes e 22 com distúrbio ventilatório restritivo detectado por esse exame. Todos os indivíduos realizaram exames da FOT para análise das propriedades resistivas e reativas do sistema respiratório. Posteriormente aos exames da FOT, os indivíduos foram submetidos à espirometria e/ou exame de volumes. Considerando os grupos divididos a partir da espirometria, os parâmetros resistivos, reativos e o módulo da impedância do sistema respiratório em 4Hz (|Z4Hz|) modificaram-se significativamente (KW- ANOVA/ANOVA, p<0,004). Na comparação entre grupos, o grupo restritivo apresentou as maiores alterações em todas as variáveis estudadas. Considerando o exame de volumes pulmonares, os parâmetros elásticos, |Z4Hz| e a inclinação da curva de resistência (S) apresentaram alterações significativas (KW-ANOVA, p<0,006), já as demais variáveis resistivas (resistências total e média, R0 e Rm, respectivamente) demonstraram uma tendência à aumento. Nessa análise, o grupo restritivo também apresentou maior prejuízo nos parâmetros oscilométricos e volumétricos. A ES resultou em alterações nas propriedades resistivas e reativas do sistema respiratório. Na análise do poder diagnóstico da FOT, sendo a espirometria referência, na comparação entre o grupo normal ao exame e o restritivo, a reatância média (Xm) apresentou valor útil para uso clínico com valor de área sob a curva (AUC) de 0,81. Já na comparação entre os grupos controle e restritivo, R0, complacência dinâmica (Cdin,sr) e |Z4Hz| apresentaram os seguintes valores de AUC, 0,81, 0,95 e 0,91, respectivamente, representando alta acurácia para as últimas duas variáveis. Com o exame de volumes como referência, comparando os grupos controle e restritivo, a Xm e a freqüência de ressonância (fr) destacaram-se com valores de 0,82 e 0,81 de AUC, respectivamente. Esses resultados são coerentes com as alterações fisiopatológicas relacionadas à esclerose sistêmica, que podem incluir fibrose difusa envolvendo o interstício pulmonar, a parede brônquica e o septo alveolar com alteração da sua arquitetura, confirmando o potencial da FOT na avaliação das modificações na mecânica respiratória relacionadas à ES.
