Talking about sexuality -Medical staffs´ experiences of conversations about sexuality with patients at a cardiology unit The background of this essay is the taboo to talk about sexuality among people who suffer from some kind of illness. When a person gets ill and need medical attention, his or her life changes dramatically. The patients get treatment for their symptoms. The illness and treatment have often an influence on the patient’s sexuality. This study has taken place on a special medical unit for patients with cardiovascular diseases. The patients get treatment for eight weeks at the unit. It is very unusual that a patient is treated by a small team of medical staff during such a long time. This situation gives patients the opportunity to develop a trust and confidence that could lead to discussions about sexuality. But how do the doctors and nurses deal with patients regarding sexual problems? The purpose of this study was to examine the medical staffs´ experience of conversations about sexuality with patients at a cardiovascular unit. A total of 13 medical staff was interviewed about their experiences concerning sexual problems in patients with cardiac disorders. The data was analyzed using latent content analysis. A number of categories were identified. Some staff have conversations with the patients about sexuality and some staff mean they do not have enough knowledge in the matter. Sometimes the staff is well prepared and sometimes they feel there is a lack of knowledge in dealing with problems concerning sexuality among patients. The staff had a wish for guidelines and more in-depth knowledge about sexuality, in order to be better prepared to deal with patient questions concerning sexuality.

Designing and developing an intervention to maximise the coping resources of doctors working with trauma patients at Johannesburg General Hospital

M.A. / This research aimed to design and develop an intervention that would maximise the coping resources of doctors working with trauma patients at Johannesburg General Hospital. Intervention Research methodology was used in order to achieve this objective. The study was divided in to three phases, namely, Diagnosis, Feedback and Discussion and Evaluation. The initial intervention comprised the Diagnostic as well as the Feedback and Discussion phase. The findings from the Diagnostic phase suggested that the doctors who had been working in the unit for less than three months, as well as the doctors who had less than four years medical practice experience, were most likely to show areas of vulnerability. This vulnerability related to their under-use of coping resources accompanied by elevated negative mood states, as identified by the Coping Resource Inventory and the Profile of Mood States questionnaire respectively. Other groups of doctors that shared this vulnerability included interns, medical officers, females and single doctors. The Feedback and Discussion phase brought to light themes relating to the stressors that the doctors’ experience in their work context. These themes along with the results of the Evaluation phase where used in order to re-design the Intervention for future use with doctors working with trauma patients. The findings from the Evaluation phase also confirmed that the doctors found the intervention to be beneficial.

Sveikatos priežiūros paslaugų kokybės gerinimo galimybės ligoninėje medicinos personalo požiūriu / Health care quality improvement in ukmerge hospital: the perspectives of medical staff

Darbo tikslas: nustatyti sveikatos priežiūros paslaugų kokybės gerinimo perspektyvas Ukmergės ligoninėje medicinos personalo požiūriu. Tyrimo metodika. Anonimine anketa apklausti visi ligoninėje dirbantys gydytojai ir slaugytojai (n=231). Užpildytas anketas grąžino 187 respondentai (atsako dažnis 76,2 proc.). Anketinių duomenų analizei naudota 10 versijos SPSS for Windows statistinė programa. Ryšiai tarp požymių vertinti Chi kvadrato kriterijumi. Rezultatai. 63,2 proc. medikų žino padalinio kokybės sistemos tikslus, 41,6 proc. medikų susipažinę su ligoninės kokybės tikslais ir tik 11,8 proc. yra iš esmės susipažinę su nacionaliniais sveikatos priežiūros kokybės dokumentais. Pacientų padėkos ir darbo sąlygų pagerinimas, vadovų ir kolegų pripažinimas yra svarbi skatinimo priemonė 76,3 – 61,8 proc. respondentų. Profesionalumas ir nuolatinis tobulėjimas (67,4 proc.) bei asmeninė darbuotojo atsakomybė (67,9 proc.) yra labiausiai puoselėjamos vertybės padaliniuose. 40,9 proc. respondentų mano, jog jų pacientai gali visiškai pasitikėti paslaugomis. Kas antras medikų jaučia aiškų vadovų skatinimą dirbti geriau. Išvados. Medicinos personalo informuotumas apie sveikatos priežiūros paslaugų kokybės gerinimo sistemą yra didžiausias skyriuje. Svarbiausia audito grupės funkcija siejama su planiniais patikrinimais padaliniuose. Didesnė dalis gydytojų nei slaugytojų žino apie audito grupės funkcijas nagrinėti pacientų skundus, rengti dokumentaciją, atlikti tikslinius patikrinimus... [toliau žr. visą tekstą] / The purpose of the study - to determine the perspectives of medical staff in relation to health care quality improvement in Ukmerge Hospital. Methods. Analysis of hospital organizational activity was performed. All physicians and nurses were questioned by an anonymous questionnaire (n=231). 187 of questionnaires were completed and returned by the respondents. The response rate was 76,2 percent. The data was analyzed using statistical package SPSS 10.0 for Windows. The associations between the variables were measured using the Chi-squared test. Results: 63.2 percent of medical staff were informed about the quality tasks in the unit, 46.6 percent of them know the quality tasks in the hospital and 11.8 percent are familiar with the national health care quality legislation. The acknowledgments from the patients and improvement of work conditions, recognition from leaders and colleagues give physicians and nurses (76.3 – 61.8 percent) an important incentive to work better. Professional competency with knowledge improvement (67.4 percent) and personal employee’s responsibility (67.9 percent) were the main value enshrined in the organization. By the opinion of 40.9 percent of respondents, patients can trust absolutely in the services or hospital. Work motivation from leaders is appreciable for every second of respondents. Conclusions. Darbo tikslas: Įvertinti ligoninės medicinos personalo požiūrį į kokybės vadybos sistemą ir jos tobulinimo galimybes įstaigoje. Uždaviniai: 1. Įvertinti ligoninės medicinos personalo žinias apie kokybės vadybos sistemą ir jos tobulinimo galimybes. 2. Nustatyti medicinos personalo požiūrį į dalyvavimą kokybės vadybos dokumentų rengimo procese. 3. Palyginti gydytojų ir slaugytojų žinias apie kokybės vadybos sistemą ir pasirengimą dalyvauti jos sertifikavime. Tyrimo metodika. Tyrimas vykdytas 2011 metais rajono ligoninėje, kurioje dirbo 220 medicinos darbuotojų. Atlikta ligoninės medicinos personalo anoniminė anketinė apklausa. Buvo išdalintos 204 anketos, gauta užpildytų 150 anketų. Atsako dažnis 73,5 proc. Statistinė duomenų analizė atlikta SPSS 19.0 versijos statistinių duomenų analizės paketu. Rezultatai. 59,3 % respondentų nežino, kas yra kokybės vadybos sistema, 63,3 % – nežino, ar jų sveikatos priežiūros įstaigoje tokia sistema veikia. 58,0 % medicinos personalo prisipažino, kad jiems trūksta žinių apie įstaigoje esančią kokybės vadybos sistemą. 42,1 % įstaigos kokybės vadybos sistemos tobulinime nedalyvauja, o svarbiausia problema tobulinant kokybės vadybos sistemą nurodo informacijos apie kokybės vadybos sistemą įstaigoje stoką. Didžioji dalis ligoninės medicininio personalo mano, kad kokybės vadybos dokumentų rengimo procesu turėtų užsiimti medicinos audito grupės vadovas ir audito grupė. To assess the knowledge of hospital medical staff about the system of quality management and the possibilities of its development in the institution. 2. To identify the attitude of the staff to its participation in the preparatory process of quality management documents. 3. To compare the knowledge of the doctors and nurses about the system of quality management and their readiness to participate in its certification. Methods. The research was carried out in 2011 in a district hospital which had 220 medical employees. A questionnaire-form anonymous survey of the hospital medical staff was conducted. 204 questionnaire forms were distributed, 150 questionnaires were returned with the answers. The response rate was 73.5 %. The statistical analysis of the data was conducted using SPSS 19.0 version of the packet for statistical data analysis. Consumer behavior analysis through nudging : A study on nudging of single-used hospital garments in healthcare sector of Region Östergötland

Region Östergötland has encountered issue in ecological and economical overconsumption of single-used garments in healthcare sector among medical staff in Universitetssjukhuset Linköping. Proper approaches to reach solutions towards decrease of usage of single-used garments were targeted to be implemented for employees of healthcare sector. Two goals were established for this study. First of all, it was aimed to analyse consumer behaviour of employees in healthcare sector of Östergötland region to find out reasons that lay behind increase of single-time garments by employees. Secondly, the aim was targeted to examine what nudging techniques, if any, are feasible to use in positively changing current consumer behavior. The findings showed that nudging has a strong potential to be regarded as an effective tool in influencing healthcare workers of Linköping University Hospital disposable garments usage. Main behavioural reasons that triggered increase of disposable garments usage were detected as lack of information and old habits of medical staff. Application of information disclosure, framing, feedback and social norms nudging techniques were identified to have promising possibilities in impacting current behaviour and raise healthcare workers awareness about their consumption patterns.

These days human being can find itself swamped with tons of information. It is neccessary to think over provided information given by a health care system and its efficiency. The thesis named Types of patient´s information needs is divided into a theoretical part and a practical part. Materials about nursing, medicine, psychology, sociology and a law issued in the Czech Republic and abroad were used to get necessary information about current condition of problematics. Two goals were set based on acquired knowledge. The first goal was to get to know about what type of information were requested and needed by patients and which they missed from nurses and doctors in ambulances and hospitals. The second goal was to double check if a personal perceiving of a given information corresponds with the real diagnosis. With respect to thesis goals, 9 research questions were formulated. Information regarding examined topic were acquired through research and investigation. As the means of collecting the data an interview was chosen. 23 interviews with patiens were done and the analyzed according to the Strausse theory. It was found, that patiens involved in this interview, take care about their health condition and that the information, which they perceive as important, are given to them by the medical staff. The information they have makes them more comfortable and they view it as a very important. It was as certained, that patients are very sensitive about the way the medical staff communicates with them and that their personality plays part in this process. The purpose of this thesis is to give an impulse to reinforce informing the patiens in a written form, but mainly to reinforce their personal contact with a doctor. Is is obvious, that the information and the way they get it is very important for patients.

This thesis deals with the position of men and women on the position of non-medical staff, namely on the position of general nurse. The aim of this thesis is, through questionnaire survey, to compare the position of men and women working as general nurses. Then, on the basis of gained results and determined hypothesis evaluate, if men are perceived same as women, whether their low number is caused by gender prejudices and if healthcare personnel and the general public want more men on this position. Theoretical part is devoted to gender, history of nursing, education and specification of general nurse's work and position of men and women in health care in the Czech Republic and the world. The research part deals with the analysis and interpretation of results from the questionnaire surveys designed not only for general nurses, but also for the public.

The diploma thesis deals with a concept of a new system of anesthetic administration in the Czech Republic. The attitude of anesthesiological nurses was inquired in a form of quantitative research questionnaire. It suggests higher/wilder competence for non - medical staff during carring out an anesthesia. In the particular chapters of the theoretical part are described different contexts of the topic (history of anethesia, change implementation theory, description of a current situation in anestesiology, comparison with other countries, legislative) and their mutual connection. Research results dealing with anesthesiological nurses opinions on suggested model of changes in carring out an anesthesia are described in an Empirical section. In a section Results of quantitative research I describe many facts, give explanations, confirmations or disproves of hypothesis arising from the research. According to the results, there exists sufficient support among anesthesilogical nurses for carring out a new system of work into an anesthesiological department. Based on facts and information from the first part of the thesis and data research I described a plan of change implementation supervision.
