Capsulotomia ventro-capsular e ventro-estriatal por raios gama no transtorno obsessivo-compulsivo: avaliação inicial da eficácia e perfil de eventos adversos / Ventral capsular and ventral striatal gamma capsulotomy in obsessive-compulsive disorder: initial assessment of efficacy and profile of adverse events

Antonio Carlos Lopes 29 August 2007 (has links)
Several studies have demonstrated that good treatment responses in obsessive-compulsive disorder (OCD) are often obtained in 60 to 80 % of patients, whether employing serotonin reuptake inhibitors or behavior therapy. However, a subgroup of these patients does not respond to the conventional therapeutic options. Severe psychosocial impairments are meanwhile observed. Stereotactic radiosurgeries, such as Gamma Knife anterior capsulotomy is a treatment option for these subjects. A smaller Gamma Knife capsulotomy technique, termed ventral capsular and ventral striatal gamma capsulotomy, has been recently developed at Brown University, with few adverse events. On the other hand, there is a lack of studies describing the efficacy and adverse events associated to this new technique. The aim of this study was to determine whether ventral capsular and ventral striatal gamma capsulotomy is efficacious in terms of obsessive-compulsive symptoms reductions and clinical global improvements, as well as to report the profile of adverse events, and neuropsychological and personality changes associated to this procedure. Fourteen refractory OCD patients were selected to be submitted to this new radiosurgical technique. Five subjects were operated in an open, pilot study. The other nine patients took part in a double-blind, randomized controlled trial, consisting of an active group (which received true radiosurgery) and a placebo group (with sham radiosurgery patients). All subjects were previously evaluated by a preliminary clinical/psychiatric interview, as well as by extensive specific instruments regarding psychiatric diagnosis, OCD evolution and severity, clinical global improvements, anxiety/depression symptoms, and neuropsychological and personality changes. All patients were offered periodical follow-up visits for a minimum of one year. Regarding the pilot study, two of five patients (40%) responded to treatment after one year of follow-up, while three of five (60%) responded after three years of follow-up. In the active group, one out of four patients (25%) responded to treatment until one year of follow-up, while three out of four subjects (75%) responded in the period between 12 and 24 months of follow-up. None of the five patients in the placebo group had responded to the false radiosurgery. The most serious adverse event was the development of a short-term, manic episode, in only one patient. Adverse neuropsychological and personality changes were not observed in this study. In conclusion, this study suggests that ventral capsular and ventral striatal gamma capsulotomy is a relatively efficacious procedure, with few adverse events.

Análise morfométrica voxel-a-voxel de estudos de ressonância magnética do crânio em transtornos psiquiátricos e neurológicos: implementação e otimização de métodos / Voxel-based morphometry analysis of brain magnetic resonance studies in psychiatry and neurologic disorders: implementation and optimization of methods

Fabio Luis de Souza Duran 03 September 2008 (has links)
The techniques most widely used for this purpose are manual volumetry using regions of interest (ROIs) and voxel-based morphometry (VBM). In this thesis, images from a databank of patients diagnosed with obsessive-compulsive disorder (OCD) (n=19) and healthy controls (n=15) were processed using the two VBM methods, namely standard and optimized VBM. The results obtained with those two automated methods were compared to the ROI approach, and the presence of artifacts of spatial normalization and image segmentation were also compared between standard and optimized VBM. METHODS: Image processing using VBM was conducted with the Statistical Parametric Mapping (SPM) program, and the ROIs were drawn over the brain regions adjacent to the lateral ventricles using the MRIcro program. The standard VBM processing included normalization of images to a pre-defined template, segmentation in gray matter, white matter and cerebrospinal fluid compartments, gaussian smoothing (12mm), and statistical comparisons using the general linear model. For optimized VBM, customized templates were built specifically for the study. Using these templates, images were segmented and spatially normalized for each brain compartment, generating parameters that allowed spatial normalization of images with greater precision. After such second normalization, images were segmented, modulated using the jacobian determinants, smoothed with a gaussian filter (12mm), and compared statistically. The ROIs were built using guidelines established in the previous international literature. RESULTS: For the brain regions in which abnormalities in association with OCD were predicted a priori, optimized VBM showed gray matter changes in the OCD group relative to controls, including reduced volume of the anterior cingulate gyrus, and increased volume of the orbitofrontal córtex and parahippcocampal gyrus. The same analysis using standard VBM detected only increased volume in OCD patients in the parahippocampal gyrus. When correlations between gray matter volumes and scale scores of obsessive-compulsive severity were investigated, the optimized VBM approach revealed a significant negative correlation in the adjacency of the dorso-medial thalamus, extending towards the lateral ventricle. Such ventricular extension was much greater when the same analysis was conducted using standard VBM. In regard to the white matter compartment, only the optimized VBM analysis showed a significant positive correlation with symptom severity in a region where OCD-related abnormalities had been predicted a priori, namely the corona radiata and internal capsula. Results regarding to the gray matter and cerebrospinal fluid xviii comparments were relative consistent between the optimized VBM and ROI analyses. CONCLUSIONS: In comparison to standard VBM, the optimized VBM method showed greater sensitivity to identify volumetric alterations in gray and white matter brain regions in which OCD-related changes had been predicted a priori. Also, the optimized VBM and ROI techniques were comparable for the evaluation of subcortical structures. Finally, both the standard and optimized VBM methods were prone to artifacts caused by ventricle enlargement, but with lesser intensity when the optimized VBM approach was employed

Dimensões de sintomas associados à resposta às cirurgias límbicas para o tratamento  do transtorno obsessivo-compulsivo / Limbic neurosurgery for obsessive-compulsive disorder: relations between symptom dymensions and outcome

Gentil, André Felix 30 October 2013 (has links)
Concluindo, a presença da dimensão de colecionismo no momento pré-operatório associou-se à redução da melhora clínica decorrente da intervenção neurocirúrgica / Research on obsessive-compulsive disorder (OCD) using symptom dymension strategies to identify more homogeneous patient subgroups, coupled with genetic, neuroimaging, and treatment outcome studies, has produced results of heuristic value. In particular, the hoarding dimension has more distinct neurobiological characteristics and has been associated with worse response to pharmachological and psychoterapeutic treatments. At the same time, the most severe and treatment refractory cases of OCD have been treated with novel neurosurgical techniques, with better efficacy and safety profiles. However, the association between symptom dimensions prior to surgery and the treatment outcome after the limbic procedure has not been sistematically investigated in the literature so far. The objective of this study was to investigate if symptom dymensions, in particular hoarding, could influence treatment outcome of limbic neurosurgeries for OCD. Information on 77 patients that underwent limbic ablative procedures for OCD from three research centers at Sao Paulo (Brazil, n=17), Boston (USA, n=37), and Stockholm (Sweden, n=23) were collected and analyzed. Symptom stratification was obtained using the Dimensional Yale-Brown Obsessive-Compulsive Scale (DYBOCS; Sao Paulo) or the Yale-Brown xix Obsessive-Compulsive Scale Symptom Checklist (YBOCS-SC; Boston and Stockholm) and treatment outcome was defined using YBOCS scores. Mean YBOCS scores reduced 34.2% after surgery (CI 95% = 27.2% to 41.3%) with a mean follow-up of 34.8 months. There was no significant difference among centers or in relation to the method of surgical intervention (capsulotomy, Sao Paulo and Stockholm; cingulotomy, Boston). Patients with hoarding symptoms had worse response to treatment (mean YBOCS reduction of 22.70% [SD = 32.23] for hoarding patients vs. 41.60% [SD = 25.99] for patients without hoarding symptoms, p=0,006). Patients with forbidden thoughts symptoms apparently also had worse response to treatment (mean YBOCS reduction of 30.10% [SD = 29.61] for patients with forbidden thoughts vs. 51.33% [SD = 32.74] for patients without this symptom dymension, p=0,033), but this effect proved dependent on the co-occurence of forbidden thoughts with hoarding dymensions. Indeed, using an analisys of variance model (ANOVA) only the negative influence of the hoarding dymension remained: patients without hoarding had a mean YBOCS redution of 13 points, while in patients with hoarding symptoms the mean reduction was of 7 points (p=0.002). In conclusion, the pre-operative presence of the hoarding dymension was associated with worst clinical outcome after the neurosurgical procedures

Alterações na personalidade após procedimento de capsulotomia ventral anterior por raios gama em pacientes com transtorno obsessivo-compulsivo (TOC) refratário / Personalty change after gamma ventral capsulotomy in refractory obsessive-compulsive disorder (OCD) patients

Paiva, Raquel Reis de 16 August 2017 (has links)
In general, about 20% of OCD patients are considered refractory for not responding to multiple first-line treatment interventions. Performing surgical procedures such as gamma ventral capsulotomy (GVC) is an option to be considered for the treatment of refractory OCD patients. Clinical and neuropsychological studies with GVC in OCD patients have demonstrated the efficacy and safety oh this procedure efficacy and safety. However, personality studies with this treatment in OCD are not found in literature. The objective of this study was to investigate personality changes after one year of GVC in refractory OCD patients. Personality assessment was performed using the Revised NEO Personality Inventory (NEO PI-R) and the Cloninger\'s Temperament and Character Inventory (TCI) in fourteen refractory OCD patients before and one year after GVC. Personality data comparisons between treatment responders (n = 5) and non-responders (n = 9), and also linear regressions using a stepwise bidirectional elimination approach for variables selection were performed. As the main result, no impairment in personality was found after GVC. On the contrary, there was a decrease tendency in Neuroticism and Persistence scores and in general, patients\' data approached to controls\'. Patients responders presented a reduction in Neuroticism scores and an increase in Extraversion scores, whereas non-responders did not change personality. Persistence, Selfdirectedness and Cooperativeness were significant in predicting percentage of change in YBOCS scores. Novelty Seeking, Reward Dependence, Selfdirectedness and Cooperativeness were significant in predicting the percentage of improvement in global functioning. This improvement in global functioning after GVC could be thought of as relating to better motivation to begin tasks, pursue goals, complete tasks, and engage with others. In conclusion, GVC procedure for OCD treatment is not only safe in terms of personality changes, but it can also improve some of its dimensions

Use of the Power Card Strategy as an Intervention with an Elementary School Student with Asperger Syndrome: Increasing On-Task Behavior in the General Education Setting

Devenport, Jane M 11 June 2004 (has links)
It has been slightly more than a decade since Asperger syndrome was recognized as a distinct sub-category of autism disorder and was first given a diagnostic category in the DSM-IV. An abundance of suggestions, ideas, and recommendations for treatment have been offered, yet there is only a limited amount of research that empirically evaluates these interventions. This study explores an intervention, the Power Card Strategy (PCS), previously demonstrated to be effective with improving social behaviors with a young girl with autism, by employing the student's area of special interest. An advantage to this intervention is it is relatively easy to implement, requires minimal time, and the cost is virtually nil. This study used a reversal design to investigate the utility of the Power Card Strategy to increase on-task behavior during teacher-directed math instruction in a general education class. The results of this study suggest that the PCS was effective for increasing on-task behavior with this student. An upward trend was observed in the student's on-task behavior during the intervention condition. Upon return to the baseline condition, the student's on-task behavior stabilized at levels observed during intervention, suggesting that skills acquired during the intervention phase maintained.

The Genetic and Functional Analysis of the Obsessive-Compulsive Disorder Spectrum

Ozomaro, Uzoezi 22 June 2011 (has links)
Obsessive-compulsive disorder (OCD) and the spectrum of associated conditions, affect 2-4% of the population worldwide. Although heritability studies in OCD have shown a 3 - 12 times increased risk for first degree relatives, the identification of the underlying risk-conferring genetic variation using classic genetic association studies has proven to be difficult. The possibility of a larger contribution of rare genetic variants to the risk of psychiatric disorder has been suggested by several successful studies. We expect that a spectrum of risk allele frequencies exists, which includes not only common variation but also a substantial amount of rare genetic variants that contribute to OCD. This thesis is aimed at identifying and functionally characterizing rare genetic variation in the OCD spectrum. Identified statistically significant variants were scrutinized for changes related to synaptic function using high content screening and subsequent functional analyses. Identifying the genetic profile of rare variants found in the OCD spectrum cohort combined with the functional impact that these variants have has provided insight into the etiology of the OCD spectrum. With these approaches a foundation can be laid for the development of a predictive model of the OCD spectrum.

How to Enhance Employees’ Work Passion?”: The Perspective of Supervisor Behaviors

Unknown Date (has links)
Work passion can have noticeable beneficial effects on both employees and employers. Therefore, raising the employee’s passion for work is one of the key points which enterprises need to concern. There are some researches indicate that the behaviors of direct supervisors can impact on work passion of employee but the relationship remains unexplained. Here we’d like to investigate that from supervisor’s perspective, what kind of behavior from managers can enhance the work passion of employee. In the first part of this research, we have gone through reviews from both references and interviews of 5 managers from industry to conclude the specific behaviors which influence the work passion. Then we have induced a questionnaire according to the factors. Later, we have measured quantitatively from the survey and we have verified the relationships between supervisor’s behavior and employee’s work passion. We have collected 259 effective samples from 8 companies including IT, electronics, and financial industries. The subjects need to be a full-time employee for at least 6 months. This research takes hierarchical regressions to analyze the relationships between supervisors’ behaviors and employee’s work passion, and we found the “aggressive instruction and care” have productive influence on the harmonious passion. Besides, “employee-oriented encourage” also have affirmative effect on both harmonious passion and obsessive passion of workers.
