Introduction: The acute postoperative pain management is recently a concern of many expert committees and professional associations. The evidence of an increased interest in this issue is the Practice Guidelines for Acute Pain Management released by American Pain Society. According to these recommendations, the pain should be monitored as a fifth vital function. As a logical consequence of modern trends of pain management, Czech Society of Anaesthesiology and Intensive Care Medicine issued guidelines for acute pain management in postoperative setting. This paper is an attempt to describe and evaluate the quality of postoperative analgetic care after total knee and hip replacements in a selected institution's department.………………………………..…………………………………… Methods: The aim of our empirical survey was to assess the level of quality of postoperative care and analgesia in the selected department. We focused mainly on the question of pain trivializing from the nurses side, their approach towards patients suffering from acute pain and, finally, the level of patient's awareness of pain intensity assessment systems and treatment options. Background: Robot-assisted laparoscopy (RAL) is an important surgical method that is expanding quickly. Despite its swift development, the research situation varies and from several perspectives there is a general lack of scientific evidence for RAL as a method. Aim: The aim of the study was to investigate the perioperative nursing care of the patient undergoing RAL, with focus on the intraoperative climate and nursing measures. Method: Data was collected through semi-structured interviews with perioperative registered nurses (RN). The interviews were analyzed using thematic analysis (TA) to identify and present patterns and themes. Results: The patients intraoperative care is affected by RAL in several ways. For the perioperative RN, there is a lot to keep track of and think about in order to maintain asepsis and to ensure the patient's physical safety during these procedures. The positioning of the patient is important in order to avoid pressure ulcers and press-related pain injuries, as well as over-stretching and nerve damage. It is a larger sterile area in a narrow environment where it can be difficult to get an overview and maintain the asepsis. Discussion: The study's results have contributed to new meaningful knowledge to the perioperative care in RAL. The difficulties with preparations and positioning were risk factors for the patient, in contrast with the advantages of the surgery for the same, which could however be improved and developed with the help of guidelines and follow-up. / Bakgrund: Robotassisterad laparoskopi (RAL) är en viktig kirurgisk metod som växer och utvecklas snabbt. Trots den snabba utvecklingen varierar forskningsläget och från flera perspektiv finns det en allmän brist på vetenskapliga bevis för RAL som en metod. Syfte: Syftet med denna studie var att undersöka den perioperativa omvårdnaden av patienten vid RAL, med fokus på den intraoperativa miljön och omvårdnadsåtgärderna. Metod: Data samlades in genom semistrukturerade intervjuer med operationssjuksköterskor. Intervjuerna analyserades med hjälp av tematisk analys (TA) för att identifiera och presentera mönster och teman. Resultat: Patientens intraoperativa vård påverkas av RAL på flera sätt. För operationssjuksköterskor finns det mycket att hålla reda på och tänka på för att upprätthålla aseptiken och för att säkerställa patientens fysiska säkerhet under dessa ingrepp. Placeringen av patienten är viktig för att undvika trycksår ​​och tryckrelaterade skador, liksom översträckning och nervskador. Educação pós-operatória: construção e validação de uma tecnologia educativa para pacientes submetidos à cirurgia ortognática / Post-operative education: construction and validation of an educational technology for patients undergoing orthognathic surgery

A cirurgia ortognática consiste na correção de deformidades dentofaciais e visa à melhora do paciente no sentido estético, funcional e anatômico da estrutura da face. Dado o aumento da prevalência desta modalidade cirúrgica e a carência de informações destinadas ao paciente submetido a esse procedimento, este estudo teve por objetivo a construção e a validação de um material educativo. A trajetória metodológica envolveu cinco fases: revisão integrativa sobre as complicações cirúrgicas; busca na Internet sobre blogs e comunidades virtuais de pacientes submetidos a cirurgia ortognática para conhecer a população e sua necessidade de informação; realização de um grupo focal com pacientes submetidos a cirurgia ortognática para levantamento das necessidades de educação perioperatória; informação dos juízes sobre orientação feita ao paciente; construção do material educativo e validação do conteúdo técnico junto a juízes quanto a pertinência e categorias de avaliação de um material educativo aplicando a técnica Delphi; e posteriormente a clareza de conteúdo foi validada junto aos pacientes em pós-operatório de cirurgia ortognática. Os resultados referentes ao conhecimento da população demonstraram a necessidade de informação dos internautas quanto à: dúvidas e medos relativos ao procedimento, recuperação, alteração da estética facial, arrependimento após procedimento. No grupo focal, os pacientes expuseram dificuldades vivenciadas pelo edema facial e parestesia, discutiram tempo de recuperação da cirurgia, questões de autoimagem, resultados estéticos e propuseram melhor momento para entrega do material. Na primeira rodada da técnica Delphi, os juízes citaram orientações pertinentes ao período perioperatório, destacando-se: higiene oral, parestesia, edema facial, dificuldade respiratória e mastigatória, alteração da voz e movimentos musculares pós-operatórios e dieta liquida. A segunda rodada da técnica Delphi consistiu na avaliação do material educativo pelos juízes, que resultou 38,2% (84) concordo e 61,3% (135) concordo totalmente para as seis categorias do instrumento. Realizadas as correções sugeridas pelos juízes um novo material foi submetido à avaliação na terceira rodada da técnica Delphi e obtidos 30,9% (68) concordo e 68,6% (151) concordo totalmente, dando como finalizado esta fase. Submetido ao paciente para avaliar compreensão, obteve-se 33,8% (176) concordo e 59,6% (310) concordo totalmente, sem sugestões para alterações do texto e foi mantido o material apresentado. Conclui-se que a construção do material educativo é um processo que envolve profissionais e público alvo para alcançar níveis elevados de aceitação e aderência do material, sendo que este deve ser utilizado como forma complementar de orientação ao paciente. / The orthognathic surgery consists of correcting dental facial deformities and aims to improve esthetical, functional and anatomical features of the structure of the patients face. Given the increased prevalence of this surgical modality and a lack of information available to patients undergoing this procedure, this study presents the construction and validation of educational material. The methodological trajectory involved five phases: integrative review on surgical complications; search on the internet on blogs and virtual communities of patients undergoing orthognathic surgeries aiming to know the population and its need of information; a focal group with patients undergoing the procedure to identify the need of perioperative education; experts information concerning guidance provided to patients; construction of educational material and validation of technical content by experts concerning the relevance and categories of validation of education material using the Delphi technique; clarity of content was validated with patients in the postoperative period. The results concerning knowledge of the population show the need to inform Internet users about doubts and fear related to the procedure, recovery, changes in facial esthetics, regret after the procedure. Patients exposed in the focal group the difficulties experienced given the facial edema and paresthesia, discussed time of surgery recovery, self-image issues, aesthetical results and suggested the most opportune time to provide the material. In the first round of the Delphi technique, experts cited guidance related to the perioperative period and the following stood out: oral care, paresthesia, facial edema, difficulty breathing and chewing, changes in voice and muscle movements after the surgery and liquid diet. The second round of the Delphi technique consisted of the evaluation of the educational material by experts: 38.2% (84) agreed and 61.3% (135) totally agreed with the instruments six categories. The material with the corrections suggested by the experts was submitted to evaluation in the third round of the Delphi technique and 30.9% (68) agreed and 68.6% totally agreed with it and this phase was completed. When the instrument was submitted to patients to evaluate understanding, 33.8% (176) agreed and 59.6% (310) totally agreed; no suggestions to change the text were suggested and this was considered the final version. The conclusion is that the construction of educational material is a process that involves professionals and the target public to achieve high levels of acceptance and adherence to material, which should be used as an adjuvant to guidance provided to patients.

Proposição de protocolo de assistência perioperatória em um Pronto Socorro de Cirurgia de Trauma / Proposition of perioperative care protocol of Trauma Surgery Emergency Unit

Introdução: Trata-se de um estudo metodológico. Objetivo: Analisar as evidências científicas sobre a assistência perioperatória aos pacientes com trauma para estabelecimento dos aspectos fundamentais para elaboração de um protocolo de atendimento; identificar o perfil sociodemográfico, clínico e terapêutico de pacientes com traumas, atendidos em um serviço de pronto socorro cirúrgico, em uma instituição pública; e propor um protocolo de assistência de enfermagem perioperatória com base neste perfil e na demanda de necessidades desta clientela. Método: Estudo metodológico realizado em duas etapas. A primeira etapa do estudo foi realizada mediante a Revisão Integrativa (RI), fundamentada na Prática Baseada em Evidências, com delimitação temática com a estratégia PICO, para responder a questão: \"Quais as evidências científicas sobre a assistência de enfermagem perioperatória para pacientes com traumas nas unidades de pronto atendimento?\" para elaboração de um protocolo perioperatório com busca nas bases de dados Scopus, Web of Science e Medline (Index Medicus Eletrônico da National Library of Medicine). Resultados: A amostra final de 13 artigos científicos evidenciou a necessidade de padronização dos cuidados específicos no pré, intra e pós-operatórios para o atendimento de pacientes com traumas nas unidades de atendimento hospitalar, que subsidiaram a elaboração do protocolo de assistência perioperatória para pacientes com traumas em um Pronto Socorro de Cirurgia. Na segunda etapa, com o estudo descritivo transversal foi obtida a caracterização da clientela (Parecer No. 115/2018 CEP/EERP-USP - CAAE 87824418.8.0000.5393), que definiu a especificidade sociodemográfico, clínico e terapêutico de pacientes com traumas e direcionamento para a elaboração final do protocolo. Conclusões: O protocolo proposto contempla os componentes Caracterização da Clientela, Definição das opções terapêuticas, Implementação do protocolo e Implantação do protocolo. Acreditamos que este protocolo poderá contribuir efetivamente na padronização das ações para a melhoria da assistência aos pacientes com traumas em um serviço de pronto socorro / Introduction: This is a methodological study. Objective: To analyze the scientific evidence on the perioperative assistance to patients with trauma to establish the fundamental aspects for the elaboration of a care protocol; to identify the sociodemographic, clinical and therapeutic profile of patients with trauma, attended at a surgical emergency service, in a public institution; and to propose a perioperative nursing care protocol based on this profile and on the needs of this clientele. Method: A two-step methodological study. The first stage of the study with the Integrative Review (IR), was based on the Evidence Based Practice, with thematic delimitation with the PICO strategy, to answer the question: \"What scientific evidence on perioperative nursing care for patients with trauma in emergency care units?\" for the elaboration of a perioperative protocol with a search in the Scopus, Web of Science and Medline (Index Medicus Electronic of the National Library of Medicine) databases. Results: The final sample of 13 scientific articles evidenced the need for standardization of specific preoperative, intraoperative and postoperative care for the treatment of trauma patients in the hospital care units, which supported the elaboration of the perioperative care protocol for patients with trauma in emergency unit. In the second stage, with the descriptive cross-sectional study was the characterization of the clientele (CEP/EERP-USP No. 115/2018 - CAAE 87824418.8.0000.5393), which defined the sociodemographic, clinical and therapeutic specificity of patients with traumas and directed to the final elaboration of the protocol. Conclusions: The proposed protocol includes the components Characterization of the Client, Definition of the therapeutic options, Implementation of the protocol and Implementation of Customers, Definition of therapeutic options, Protocol implementation and Protocol deployment. We believe that this protocol can effectively contribute to the standardization of actions to improve the care of patients with trauma in a first aid service

Patientens upplevelse av den perioperativa vården / The patient's experience of the perioperative care

Background: Patients who waiting for a surgery can feel fear, which increases the risk of more concern and anxiety. Patients need a clear information and communication that includes openness and hope in the preoperative care. Aim: The aims of this study are to investigate patients' experiences of the perioperative care in connection with an operation and with questions about the patients experience regarding information, communication, satisfaction and anxiety. Method: A quantitative descriptive study in the form of questionnaires conducted. The study participants were all patients who had undergone elective surgery at a hospital in central Sweden during week 10-11 in 2015. It was attended 50 patients, 30 men and 20 women between 20-86 years from five different surgical divisions. Results: The patient has high confidence of nurses and doctors professional competence, however more than half of the patients reported that the experienced anxiety before surgery. The preoperative meeting with the anesthesiologist experienced positive without reducing their anxiety. Patients felt that communication with a nurse anesthetist is easier compared to an anesthesia doctor, according to descriptive data. The majority of patients that the experienced unexpected situations in the operating room, and 28% wanted more information on how the surgery would go to. Conclusion; Lack of information is extensive in the preoperative care of patients who waiting for an operation. Most patients felt unexpected situations during the hospital stay, spite that the 100% of patients had high confidence in the nurse anesthesia and anesthesia doctor’s professional competence. / Bakgrund: Patienter som väntar på en operation kan uppleva rädsla vilket ökar risken för oro och ångest. Det önskas av patienter tydlig information och kommunikation som innehåller öppenhet och hopp i den preoperativa vården. Syfte: Syften med denna studie är att undersöka patienters upplevelse av den perioperativa vården i samband med en operation med frågeställningar om patienters upplevelse angående information, kommunikation, tillfredställelse och oro/ångest. Metod: En kvantitativ deskriptiv studie i form av enkäter utfördes. Studiedeltagarna var alla patienter som hade genomgått en elektiv operation vid ett sjukhus i Mellansverige under vecka 10-11 år 2015. Det deltog 50 patienter, 30 män och 20 kvinnor mellan 20-86 år från fem olika kirurgiska verksamheter. Resultat: Patienter har högt förtroende på sjuksköterskan och läkarens yrkeskompetens. Däremot angav mer än hälften av patienterna att de upplevde oro eller ångest inför operationen. Den preoperativa mötet med anestesiologen upplevdes positivt utan att minska deras oro och ångest. Patienter upplever att kommunikation med en anestesisjuksköterska är lättare jämfört med en anestesiolog enligt beskrivande data. En Majoritet av patienterna angav att de upplevde oförutsedda situationer i operationssalen och 28 % önskade mer information om hur operationen skulle gå till. Slutsats: Brist på information är omfattande i den preoperativa vården för patienter som väntar på en operation. De flesta patienter kände oberäknade situationer under sin sjukhusvistelse trots det hade 100 % av patienter högt förtroende för anestesisjuksköterskan och anestesiologens yrkeskompetens.

Ungdomars tankar inför operation och upplevelser av den perioperativa vården utifrån ett livsvärldsperspektiv : - En kvalitativ intervjustudie

Bakgrund: Den perioperativa vården syftar till en individuellt anpassad vård som tillgodoser patientens unika behov. Kvalitativ forskning om ungdomars upplevelser av detta är begränsad.  Syfte: Syftet var att undersöka ungdomars tankar inför operation och upplevelser av den perioperativa vården.  Metod: Studien genomfördes som en kvalitativ intervjustudie med semistrukturerade frågor. Datan analyserades med hjälp av systematisk textkondensering (STC). Tio ungdomar i åldrarna 12 till 17 år som hade genomgått operation under generell anestesi intervjuades på ett universitetssjukhus i Mellansverige under våren 2018.  Resultat: Analysen resulterade i tre kategorier “tankar kring anestesi, kirurgi och livet”, “erfarenheter av den preoperativa informationen” och “faktorer som påverkar välbefinnandet”. Ungdomarna fokuserade på hur operationen skulle påverka det sociala livet. De hade frågor och tankar som berörde såväl anestesin och kirurgin vilket hanterades på olika sätt. De besked och den information ungdomarna fick under det preoperativa vårdförloppet upplevdes som bristfällig och oanpassad, vilket bidrog till stress och oro. Delaktighet och personcentrerad omvårdnad bidrog till trygghet och en bra relation mellan patient och vårdare.  Slutsats: Resultatet visade på att personalen som är involverad i den perioperativa vården behöver kunskap om ungdomars upplevelser och erfarenheter av den perioperativa vården och operation för att på så sätt kunna förstå vilka omvårdnadshandlingar som är viktiga att ta hänsyn till utifrån ett ungdomsperspektiv. Kunskapen kan användas för att utveckla riktlinjer och den perioperativa vården för att främja patientbemötandet utifrån ungdomars unika förutsättningar. / Introduction: The perioperative care aims at an individually adapted care that meets the patient's unique needs. Qualitative research on adolescents experiences in this subject is limited.  Aim: The purpose was to investigate the thoughts of adolescents on the operation and experiences of perioperative care.  Methodology: The study was conducted as a qualitative interview study with semistructured questions. The data was analyzed using systematic text condensation (STC). Adolescents aged 12 to 17 years who had undergone surgery under general anesthesia were interviewed at a university hospital in central Sweden in the spring of 2018.  Results: The analysis resulted in three categories "thoughts about anesthesia, surgery and life", "experiences of preoperative information" and "factors that affect well-being". The adolescents focused on how the operation would affect the social life. They had questions and thoughts that concerned both anesthesia and surgery, which was handled in different ways. The information the adolescents received during the preoperative stage were perceived to be inadequate and inappropriate, which contributed to stress and concern. Participation and person-centered care contributed to a feeling of safety and a good relationship between patients and caregivers.  Conclusion: The result showed that the personnel involved in the perioperative care need knowledge about the experiences of young people in perioperative care and surgery in order to understand which caring actions are important to take into account from an adolescent perspective. This knowledge can be used to develop guidelines and the perioperative care based on adolescents unique conditions.

This thesis focuses on the issue of nursing care for adolescent boys who have undergone phimosis surgery. The nursing care is then evaluated according to the NANDA, NIC and NOC classification systems. The theoretical part deals with topics such as adolescence, phimosisa disease of the external genitalia of a man, preoperative and postoperative care for boys with phimosis, and the classification taxonomy of the NANDA, NIC and NOC classification systems, which represent the comprehensive standardized, but still evolving, nursing language. Available Czech and foreign literary sources were used for the compilation of the theoretical part of the stated subject. The empirical part of the thesis was processed using qualitative and quantitative research methods: content analyzes, modelling, thought experiment, structured and semi-structured interviews, and qualitative data analysis. Four objectives were defined. The way adolescent boys perceive and experience the bio-psycho-social aspects of the problems that the pre-op and post-op period brings, has been assessed using the NANDA, NIC and NOC classification systems. There were 46.66% of respondents with higher education, 53.34% of respondents with secondary education, and eight adolescent boys who had received phimosis surgery, all of whom contributed to the assessment. It was found that adolescent boys initially addressed their physician regarding the phimosis problem, because they had a sense of trust and anonymity, but also to some degree because of necessity. It is surprising that young men with this type of personal problem rather speak to their mothers. When it comes to peer relationships, the boys confide the problem to their peers, but they do not ask them for help in this area. The boys named the internet as the main source of information. The following assessments were made using the Fehring methods for determining the weighted scores. Out of 13 nursing diagnoses within the NNN classification systems there were 112 major and minor characteristics (41.18%) selected by the university educated respondents, 80 major and minor characteristics (29,41%) selected by by the secondary school educated respondents, and nine major and minor characteristics (3.31%) were selected by the adolescent boys. Out of the 15 nursing interventions published in the Nursing Interventions Classification (NIC), 203 major and minor characteristics (55.31%) were chosen by the university educated respondents and 235 major and minor characteristics (64.03%) were chosen by the secondary school educated respondents. Out of the 11 expected nursing outcomes published in the Nursing Outcomes Classification (NOC), 39 major and minor characteristics (15.42%) were selected by the university educated respondents, while 34 major and minor characteristics (13.44%) were selected by the secondary school educated respondents. By implementing the NANDA, NIC and NOC classification systems in nursing care one can expect to find solutions that help the professionals in nursing care to apply the nursing process effectively, and that allow nursing care to focus more on the individual needs of patients and to be improved in all areas.

Följsamhet till handhygien- och klädrutiner på operationsavdelningar

Introduktion: En god följsamhet till rätt handhygien- och klädrutiner minskar risken för smittspridning. En av operationspersonalens viktigaste uppgifter är att förebygga infektioner. Följsamhet till handhygien- och klädrutiner skyddar både patient och personal från infektioner.   Syfte: Att undersöka och sammanställa operationspersonalens följsamhet till kläd– och handhygienrutiner på operationsavdelning.                                                                             Metod: En deskriptiv systematisk litteraturstudie utfördes, där databaserna PubMed, Cinahl och ScienceDirect genomsöktes. Efter granskning och analys valdes tio vetenskapliga artiklar ut att inkluderas i resultatet, fem med hög kvalitet och fem med medelhög kvalitet.                                         Resultat: Fungerande följsamhet till handhygien- och klädrutiner kunde bero på god information angående rutinerna. Personalen hade god följsamhet till att avlägsna smycken innan de gick in på operationsavdelningen. Bristande följsamhet till handhygien- och klädrutiner på operationssal var exempelvis otillräckligt utförd handdesinfektion samt brister i användandet handskar. Bristerna berodde dels på en negativ inställning och attityd, men även på dålig kunskap om handhygien- och klädrutiner.              Slutsats: Totalt tio vetenskapliga studier togs med gällande kläd- och handhygienrutiner. I dessa upptäcktes brister i handhygien- och klädrutiner men också det som fungerade i rutinerna. Det framkom även skillnader bland operationspersonalens följsamhet till handhygien- och klädrutiner. / Introduction: Good follow-up to proper hand hygiene and clothing routines reduces the risk of infection spread. One of the operational personnel's most important tasks is to prevent infections. Compliance with hand hygiene and clothing routines protects patients and personnel from infections. Aim: To investigate and compile operational personnel compliance with hand hygiene- and clothing routines in the operations department. Method: A descriptive systematic literature study was conducted using the databases PubMed, Cinahl and Science Direct. After examination and analysis, ten scientific studies were selected to be included in the results, five with high quality and five with medium quality.   Results: Functional compliance with hand hygiene and clothing routines could be due to good information about the routines. The staff had good adherence to removing jewelry before entering the operations department. Inadequate compliance with hand hygiene and clothing routines in the operating room were, for example, insufficient hand disinfection and shortcomings in the use of gloves. The shortcomings were due to a negative attitude and attitude, but also to poor knowledge of hand hygiene and clothing routines. Conclusion: A total of ten scientific studies were carried out with current hand hygiene- and clothing procedures. In these, defects were discovered in hand hygiene- and clothing routines, but also in the hygiene routines. There were also differences among operating personnel in compliance with hand hygiene- and clothing routines.
