Separation anxiety presents a negative influence on the total field of experience, his relations to it, encompassing scholastic functioning and social relationships. The research is focused on the junior primary pupil (Sub A up to Stan de rd 1). There is no differentiation regarding sex. A list of diagnosis, the idiographic research and diagnostic model, and the pedotherapy programme (Jacobs: 1980, · 1981) are all used, to diagnose separation anxiety and for the treatment of this condition. The purpose of this study was to investigate the effect of a new intervention method, Technology Assisted Therapy for Separation Anxiety (TTSA), on the psychosocial functioning and quality of life of persons with intellectual and visual disability who experience separation anxiety and exhibit challenging behaviour. A pre-experimental within group design was used with randomised multiple baselines and staggered intervention start points. The data was collected at an institution in the Netherlands that provides long-term care and residence for persons with intellectual disability and visual impairment. Six participants took part in this study. This research was informed by Bowlby‟s attachment theory, which conceptualises the tendency of human beings to forge strong emotional bonds with others and explains how the disturbance of this bond might lead to various forms of emotional distress and personality disturbances. The interplay of separation anxiety and challenging behaviour with respect to the formation of attachment relationships provides the specific theoretical context in which this study is grounded. The results indicate that both the separation anxiety experienced and the challenging behaviour exhibited by the participants decreased significantly after the implementation of TTSA. Their psychosocial functioning and quality of life also increased significantly. This study demonstrates the first successful application of TTSA to treat separation anxiety and challenging behaviour in persons with intellectual and visual disability. Évaluation de l’efficacité d’un programme d’entraînement parental pour les parents d’enfants souffrant d’anxiété de séparation

Le trouble d’anxiété de séparation (TAS) est le trouble anxieux le plus prévalent chez les enfants. Il apparaît tôt et entraîne plusieurs conséquences négatives. La thérapie cognitivo-comportementale (TCC) a été reconnue efficace pour traiter les troubles anxieux. Toutefois, peu d’études ont vérifié son efficacité pour le traitement spécifique du TAS et très peu en ont examiné l’effet auprès d’enfants de moins de 7 ans. Les quelques interventions étudiées visant les moins de 7 ans ont en commun d’inclure le parent dans le traitement ou de l’offrir directement à celui-ci. L’objectif principal de cette thèse est de vérifier l’efficacité d’un programme d’entraînement parental de type TCC, adapté pour les parents d’enfants de 4 à 7 ans souffrant de TAS. Cette étude vise également deux objectifs spécifiques : observer la fluctuation des symptômes de TAS de l’enfant pendant le traitement et examiner l’impact du programme sur les variables parentales. Un devis à cas unique à niveaux de base multiples a été utilisé. Six familles ont pris part à l’étude. Des entrevues semi-structurées, des questionnaires auto-administrés et des calepins d’auto-observations quotidiennes ont été utilisés auprès des parents pour mesurer les symptômes anxieux des enfants, leurs pratiques parentales, le stress parental et leurs symptômes anxieux et dépressifs. Des questionnaires sur les difficultés de l’enfant incluant l’anxiété ont aussi été envoyés à l’éducatrice ou à l’enseignante. Tous les questionnaires ont été administrés aux trois temps de mesure (prétraitement, post-traitement et relance 3 mois). Les calepins d’auto-observations ont été remplis quotidiennement durant le niveau de base, pour toute la durée de l’intervention et pendant une à deux semaines à la relance. Les résultats de l’étude indiquent que cinq enfants sur six ne répondent plus au diagnostic de TAS suite au traitement ainsi que trois mois plus tard. Les résultats des calepins d’auto-observations montrent une amélioration claire des manifestations principales de TAS pour la moitié des enfants et plus mitigée pour l’autre moitié, de même qu’une amélioration systématique de la fréquence hebdomadaire totale de manifestations de TAS suite à l’intervention pour quatre enfants. Les résultats aux questionnaires remplis par les parents montrent une amélioration des symptômes d’anxiété et de TAS chez quatre enfants au post-test et/ou à la relance, tandis que les questionnaires de l’éducatrice (ou enseignante) suggèrent que les symptômes anxieux des enfants se manifestaient peu dans leur milieu scolaire ou de garde. L’impact du programme sur le stress parental et les pratiques parentales est également mitigé. Ces résultats suggèrent que le Programme d’entraînement parental pour les enfants souffrant d’anxiété de séparation (PEP-AS) est efficace pour réduire les symptômes de TAS chez les enfants d’âge préscolaire ou en début de parcours scolaire et appuient la pertinence d’offrir le traitement aux parents et d’inclure un volet relationnel. D’autres études seront cependant nécessaires pour répliquer ces résultats auprès d’un plus vaste échantillon. Il serait également intéressant de vérifier les effets indépendants des différentes composantes du traitement et d’évaluer les effets du programme sur davantage de pratiques parentales associées spécifiquement à l’anxiété. / Separation anxiety disorder (SAD) is the most prevalent anxiety disorder among children. It appears early in development and has multiple negative consequences. Cognitive-behavioral therapy (CBT) has been shown to be an effective treatment for anxiety disorders. However, few studies have examined the efficacy of CBT to treat SAD in particular, and even fewer have examined the impact of this form of therapy on children younger than 7 years old. The main objective of the present doctoral thesis was to evaluate the efficacy of a CBT parent-training program, that was adapted specifically for parents of children aged 4 to 7 years old suffering from SAD. This study had two specific objectives: to observe any fluctuations in the child’s SAD symptoms during the treatment and to examine the impact of the program on parental variables. A single-case multiple baseline across-subjects design was used. Six families with a child aged 4-7 years old and with a diagnosis of SAD participated. Semi-structured interviews, self-reported questionnaires and daily diaries were used with the parents to assess the child’s anxiety symptoms, parental practices, parenting stress, and the parents anxious and/or depressive symptoms. Questionnaires on child problems were also sent to the children’ teacher or educator. All questionnaires were administered at three times of measurement (pre-treatment, post-treatment and 3 months follow-up). Daily diaries were also completed by the parents at baseline, throughout the treatment, and during one to two weeks at follow-up. Results revealed that five of the six children no longer met the criteria of a SAD diagnosis after treatment and three months later. Findings from the daily diaries showed a clear reduction of the principal SAD symptoms for half of the children but mixed results for the other half of the children and that four of six children presented a systematic favourable change of the total weekly frequency of SAD symptoms after the intervention. The results of parent questionnaires showed an improvement of SAD symptoms for four children at post-treatment and/or follow-up. The teacher/educator questionnaires indicated low impact of anxiety symptoms. The impact of the program on parenting stress and parental practices is mixed with some parents showing improvement and others less so. The results support the efficacy of the Programme d’entraînement parental pour les enfants souffrant d’anxiété de séparation (PEP-AS) to reduce SAD symptoms in preschool age children and support the relevance of directing the treatment towards parents and including a relational component in the intervention. However, more research is needed to replicate these findings with larger samples and randomized control trials. It would also be interesting to dismantle the program and to examine the various components of the treatment in different combinations, and to explore more specifically the program effects on parental variables.

Contribution à la question du sens de la chute du sujet âgé : les raisons de la chute, la chute de la raison / Reflection upon the question of the meaning of the fall of the elderly : the reasons for the fall, the fall of reason

Avec l’allongement de la durée de vie, les écrits sur la psychologie du vieillissement se sont développés à une moindre échelle que ceux concernant la clinique de l’adulte et de l’enfant. Il nous est donc apparu intéressant, passionnant et fécond de réfléchir à cette période de senescence en reprenant la dualité somat/psyché à travers l’événement chute. L’utilisation d’une méthodologie quantitative et qualitative permet d’aborder l’évènement chute comme une expérience traumatogène selon les données corporelles (sujet dit fragile ou valide, conséquences somatiques et psychomotrices…), temporelles (délai de la dernière chute, moment de la chute, âge objectif et subjectif de la chute…) et psychiques (défenses mises en place, réminiscence, travail du vieillir…). Nous nous sommes donc plus particulièrement penchés sur le point de bascule de cet événement dans un contexte de traitement coûteux des conflits issus de cette période pour y découvrir des défenses de type obsessionnel et une régression maternelle primaire. Par conséquent, le développement des réflexions psychologiques et psychanalytiques sur la clinique de la chute du sujet âgé ne peut être appréciable qu’à la condition de ne pas « tomber » dans le mythe d’un lien de causalité unique, dans une classe démographique indifférenciée dont l’âge serait vu comme un facteur étiologique. L’utilisation de l’analyse quantitative et qualitative du discours donne accès aux données réactionnelles post chute mais également aux modalités de traitement psychique des affects et conflits éveillés par cet événement. Nous avons associé le discours en libre association avec la technique du dessin pour avoir un aperçu des images mentales fixées dans le récit historien de la mémoire. La chute est alors exposée comme un agir, pour pallier la défaillance du système pare-excitation, des capacités d’élaboration et de liaison de la blessure narcissique de la sénescence. Cependant, elle peut également être appréciée comme un retour au corps pour une reprise élaborative des liens d’attachement en vue d’une fin inéluctable. C’est alors que « ce qu’on appelle une raison de vivre est en même temps une excellente raison de mourir » (Albert Camus). En effet, les communications sur la question de la psychologie de la chute du sujet âgé sont principalement étudiées dans le cadre de processus déficitaires ou psychopathologiques du sujet âgé fragile, hospitalisé, sans réellement prendre en compte les remaniements complexes individuels et groupaux inhérents au travail du vieillir. Nous nous sommes donc intéressés au mouvement psychique chez le sujet âgé valide afin d’en tirer de nouvelles réflexions cliniques sur le type d’angoisse et de relation d’objet. Le TAT/SAT proposé au sujet de notre étude a établi la constitution psychique d’un modèle interne de relation insécure du chuteur, en lien avec des expériences d’empiètement relationnel avec un objet maternel défaillant et décevant. / With the increase of life expectancy, psychological writings about ageing have not developed as much as those concerning adults and children. It seemed to us interesting, fascinating and fruitful to think about this senescence period by restating the « somat/psyche » duality throughout the deterioration event. The use of a quantitative and qualitative methodology allows us to tackle the fall event as a traumatic experience according to physical data (fragile ou valid subject, somatic or psychomotor consequences…), temporal (since the last fall, moment of the fall, objective and subjective age of the fall…) and psychic (defenses set up, reminiscence, the work of getting old…) So we specifically looked at the turning point of this event in the context of expensive treatment of conflicts emerging at that time to discover obsessive defense mechanisms and primary maternal regression. In consequence, the development of psychological and psychoanalytical reasoning on the observation of the fall of the ageing subject can only be appreciated by not « falling » in the belief of a unique link of casuality, within an indistinguishable demographic class where age would be seen as an etiological factor. The use of quantitative and qualitative analysis of the discourse gives access to postfall reactive data but also to psychic treatment methods of the conflicts and affects which emerged from this event. We combined verbal communication with the drawing technique to get an insight of the mental images set in the historical narrative of memory. The fall is then exposed as an action, to mitigate the failure of the protective shield system and the capacities of development and connection to the narcissistic injury of senescence. However, it can also be appreciated as a coming back an elaborative resumption of attachment that leads to an inevitable end. It was then that "what is called a reason for living is also an excellent reason for dying" (Albert Camus). Indeed writings on the subject of the psychological deterioration of the ageing person are mainly studied in the context of the psychopathological or deficient process of the frail person in question, hospitalized without really taking into account the individual and collective complex alterations common to the process of ageing. We therefore concentrated on the psychic process of a valid old subject in order to glean more clinical reflections on the type of anxiety and object interaction. The TAT/SAT proposed concerning our study has established the psychic constitution of an internal model of an insecure relationship of the ageing, linked with an infringing relational experience with a failing and disappointing maternal object.

Efficacité d'un programme d'intervention auprès d'un parent d'enfant ayant une déficience intellectuelle et une anxiété de séparation

Parmi les diagnostics de troubles anxieux auprès de la population ayant une déficience intellectuelle (DI), le trouble d’anxiété de séparation figure parmi les plus fréquents. Près de 30 à 50 % des personnes avec une déficience intellectuelle (DI) présentent également un trouble de santé mentale en comorbidité, dont les troubles anxieux. Sachant que la prévalence de la DI est de 1,0 % à 2,6 % de la population du Québec, il semble pertinent d’offrir des services adaptés à leurs besoins particuliers. Pourtant, la grande majorité des recherches portant sur des interventions pour réduire les symptômes anxieux auprès d’une population ayant un trouble neurodéveloppemental, tel que le trouble du spectre de l’autisme (TSA), exclut les participants ayant une DI. Pour répondre à ce besoin, ce projet de recherche visait à évaluer l’efficacité d’un programme d’intervention auprès d’un parent d’un enfant âgé de 8 ans ayant une DI et un trouble d’anxiété de séparation. Dans le cadre d’un devis à cas unique de type AB, nous avons mesuré les comportements anxieux tout au long du programme d’intervention mis en place. Les résultats montrent que certains symptômes anxieux ont diminué suivant l’implantation par le parent de stratégies cognitivo-comportementales. D’autres études devront être effectuées afin d’explorer davantage l’efficacité de ce programme. / Among the diagnoses of anxiety disorders in the population with intellectual disabilities (ID), separation anxiety disorder is the most common. About 30 to 50% of people with intellectual disabilities (ID) also have a comorbid mental health problem, including anxiety disorders. Knowing that the prevalence of the disease is from 1.0% to 2.6% of the population of Quebec, it seems relevant to offer services tailored to their specific needs. Yet, the vast majority of research has been done on interventions to reduce the symptoms of the population with a neurodevelopmental disorder, such as the autism spectrum disorder (ASD), excludes participants with an ID. To address this need, this research project was designed to evaluate the effectiveness of an intervention program with a parent of an 8-year-old child with ID and separation anxiety disorder. As part of a single-case AB type design, we measured anxiety behaviors throughout the intervention program set up. The results show that some anxiety symptoms have decreased following the parent's implementation of cognitive-behavioral strategies. Other studies should be done to explore the effectiveness of the program.

This study was undertaken as a qualitative needs assessment of emotional support to the adolescent with a terminally ill parent. Five participants in their adolescent life phase (14 to 18 years) with a terminally ill parent, participated in the study. Individual unstructured interviews were conducted with each participant, in order to assess their need for emotional support. With the consent of the parents and adolescents, the interviews were video-recorded and transcribed. Eight recurring themes were selected from the case studies and verified with the existing literature. Factors related to school refusal of black adolescents in the Impendle area

The specific aim of this research study was to identify, explore, describe and interpret the factors related to school refusal of black adolescents. An overview of literature was used to arrive at a broader appreciation of issues pertaining to school refusal phenomenon and also to black adolescents. Ten subjects participated in this study. Data were gathered qualitatively using ethnographic interviews. The following factors were found to be contributory to the problem of school refusal: School refusers who stay with grandparents instead of staying with biological parents; fear of something specific at school; lack of insight into the refusal problem; lover plays a role in refusal behaviour; protective parents; poor social relationships; fathers that play a marginal role in the family; parents' worldview; emotional problems and weak or no attempts to stop behaviour.
