Dyadic influences on awareness of condition in people with dementia: findings from the IDEAL cohort

Yes / The discrepancy between caregiver-ratings and self-ratings of abilities is commonly used to assess awareness in people with dementia. We investigated the contribution of caregiver and dyadic characteristics to the difference in perspective between caregiver-informants and people with dementia about difficulties experienced, when considering awareness of condition. Methods: We conducted exploratory cross-sectional analyses using data from the IDEAL cohort. Participants were 1,038 community-dwelling people with mild-to-moderate dementia, and coresident spouse/partner caregivers. The Representations and Adjustment to Dementia Index (RADIX) checklist reporting difficulties commonly experienced in dementia was completed by 960 caregiver-informants and 989 people with dementia. Difference in scores was calculated for 916 dyads. Demographic information, cognition, informant-rated functional ability and neuropsychiatric symptoms were recorded for the person with dementia. Self-reported data were collected on mood, comorbidity, religion, importance of religion, relationship quality, and caregiver stress. Results: For most dyads, caregivers reported more RADIX difficulties than people with dementia. Caregiver RADIX ratings were more closely associated with informant-rated functional ability and neuropsychiatric symptoms than with cognition. More RADIX difficulties and higher stress were reported by female caregivers. Greater RADIX difference was associated with more caregiver stress, and older age but less depression in people with dementia. Conclusion: Few dyadic characteristics were important, but caregiver stress was higher where caregivers reported more RADIX difficulties and/or the difference in perspective was greater, whereas partners with dementia reported better mood. The neighborhood is considered one of the most violent and problematic urban quarter of the city. The main hypothesis considered everyday fears and shame as central emotions and as the direct forces of the emotional culture of the neighborhood, as well as of the actions of the residents in the role of interactional units in relation to itself, to another resident, to the neighborhood and to the city. The symbolic-interactionist perspective guided an ethnographic methodology that was held from fieldwork that combined strategies of participant observation and direct interviews. The research results revealed a hierarchical and exclusionary social logic as a central element in the emotional culture of the city of João Pessoa over the peripheral areas of the city, of which the Varjão/Rangel is one of its parts. In this sense, the double neighborhood’s nomination examined here, the official Varjão and the unofficial Rangel as well, demonstrates the impact of moral disputes about the moralizing and modernization process in the city and reflects the everyday stigma, shame and fears as commonplace of an ambiguous and ambivalent identity, the one of the Varjão/Rangel neighborhood. A common resident of the neighborhood, socialized in a tense communicative environment characterized by a deteriorated identity, internalizes the perspective of a "generalized other" that perceives himself as a potentially dangerous character who contaminates the city as a whole. This classificatory logic and argumentation about the neighborhood of the city is taken over by the resident of Varjão/Rangel, who, in turn, hoping to safeguard his face, ends up falling in a play of moral disputes whose logic of excuses and accusations transforms every resident of the neighborhood, including himself, in a branded character. / Este trabalho tem por objetivo uma análise compreensiva do bairro do Varjão/Rangel da cidade de João Pessoa, Paraíba, como espaço de sociabilidade urbana de intensa pessoalidade sob a ótica da antropologia das emoções. O bairro é considerado como um dos mais violentos e problemáticos da malha urbana da cidade. Teve-se por hipótese a questão dos medos corriqueiros e da vergonha cotidiana como emoções centrais e organizadoras da cultura emotiva do bairro, bem como das ações dos moradores do mesmo enquanto unidades interacionais em relação a si mesmo, ao outro morador, ao bairro e à cidade. A proposta metodológica de cunho etnográfico, no interior de uma perspectiva simbólico-interacionista, foi realizada a partir de um trabalho de campo que combinou estratégias de observação participante e de entrevistas diretas. Os resultados da pesquisa revelaram uma lógica societária hierarquizante e excludente como elemento central na cultura emotiva da cidade de João Pessoa em relação aos bairros populares, dos quais o bairro trabalhado faz parte. Neste sentido, a dupla nominação do bairro aqui analisado, entre o oficial Varjão e o oficioso Rangel, demonstra o impacto das disputas morais em torno do processo de moralização e modernização do espaço urbano local, bem como reflete o estigma, a vergonha cotidiana e os medos corriqueiros de uma identidade ambígua e ambivalente, a do bairro do Varjão/Rangel. O morador do bairro, socializado em um ambiente comunicativo tenso e caracterizado por uma identidade deteriorada, assume a perspectiva de um “outro generalizado” que o percebe como um personagem potencialmente perigoso e que contamina a cidade como um todo. Esta lógica de classificação e argumentação da cidade sobre o bairro é assumida pelo morador do Varjão/Rangel, que, por sua vez, na intenção de salvaguardar a sua fachada, acaba caindo em um jogo de disputas morais cuja lógica de desculpas e acusações transforma todo morador do bairro, inclusive ele mesmo, em um personagem estigmatizado.

Studietiden är som en del av individens vardag i ständig förändring, till följd av att nya möjligheter till undervisning på distans utvecklas och tas i bruk. Denna enkätstudie bygger på data från 71 elever och avser att reflektera deras uppfattningar om kemiämnets möjligheter till inspiration och som källa till kunskaper i vardag och samhälle. Elevers upplevelser av digital distansundervisning ställs mot upplevelser av undervisning i skolmiljö. Eleverna som grupp, ger i denna studie uttryck för ett lägre intresse och minskade möjligheter att förklara vardagliga observationer när undervisningen sker på distans.  I studien reflekteras även den sociala aspekten av lärande genom elevers uppfattningar om möjligheter att stödja deras lärprocesser vid undervisning i skolmiljö och på distans. Elevgruppens svar speglar sämre förutsättningar att stödja lärprocesser då undervisningen i huvudsak sker på distans eftersom möjligheterna till sociala och pedagogiska interaktioner då begränsas. Elevers skattningar av den egna kemilärarens didaktiska förmåga i den digitala undervisningens arena vägs mot betydelsen av den pedagogiska relationen och undervisningens förmedlade kunskapskrav. Elevsvaren speglar i detta avseende en kluvenhet där betydelsen av didaktiska val sammanvägs med betydelsen av den pedagogiska omsorgsrelationen och möjligheter till vägledning. / Study time is constantly changing, as a result of new opportunities for distance learning being developed and put into use. This questionnaire study is based on data from 71 students and intends to reflect their perceptions of the chemistry subject's opportunities for inspiration, as a source of knowledge in everyday life and society. Students' experiences of digital distance education are contrasted with experiences of teaching in a school environment. In this study, the students as a group express a lower interest and reduced opportunities to explain everyday observations when teaching takes place at a distance. The study also reflects the social aspect of learning through students' perceptions of opportunities to support their learning processes when teaching in a school environment and at a distance. The student group's responses reflect poorer conditions for supporting learning processes as teaching mainly takes place at a distance, as the opportunities for social and pedagogical interactions are then limited. Students' estimates of their own chemistry teacher's didactic ability in the digital teaching arena are weighed against the importance of the pedagogical relationship and and mediated contents of curriculum. In this respect, the students' responses reflect a division where the importance of didactic choices is weighed together with the importance of the pedagogical care relationship and opportunities for guidance.

Bakgrund: I takt med att antalet personer som lever med demenssjukdom väntas öka blir behovet av kunskap inom ämnet av vikt för att möjliggöra en god och evident vård. Därför behövs mer forskning utifrån ett patientperspektiv om vad dessa individer anser vara meningsfullt i vardagen. Syfte: Att beskriva upplevelsen av en meningsfull vardag hos personer med mild till måttlig demenssjukdom. Metod: En systematisk litteraturstudie med kvalitativ ansats utifrån Evans analysmetod tillämpades. Resultat: Resultatet presenteras utifrån tre teman med två subteman vardera. I dessa framkommer att personer med demenssjukdom värderade relationer med familj och anhöriga samt en allmän gemenskap med andra. Aktiviteter var en viktig faktor för att uppleva meningsfullhet, både för att stärka individens identitet och för att hålla sig aktiv och sysselsatt. Slutligen identifierades vikten av självständighet och självbestämmande hos personer med demenssjukdom. Slutsats: Det framkom att flera faktorer spelar stor roll för att personer med demenssjukdom ska uppleva en meningsfull vardag. Dessa är viktiga att ha i åtanke vid vård av individer ur denna patientgrupp för att möjliggöra bästa möjliga upplevelse av hälsa. / Background: As the number of people with dementia are expected to increase globally, the need for knowledge relevant to the subject is important to enable good and evident care. Therefore, an increased amount of research is needed from a patient perspective of what these individuals consider to be meaningful in their everyday life. Aim: To describe experiences of a meaningful everyday life for individuals suffering from mild to moderate dementia Method: Systematic literature review with qualitative approach based on Evans analytical method was applied. Results: Three main themes emerged with two subthemes each. Individuals suffering from dementia valued relationships with family and relatives, as well as a connection with people in general. Activities were seen as an important factor for experiencing meaningfulness, both to strengthen their own identity and to stay active. Finally, the importance of freedom was identified by a feeling of self-determination and independence. Conclusion: Several factors have shown to play an important role in how individuals suffering from dementia value as meaningful in their everyday lives. These key factors are important to consider when caring for this group of patients to allow for optimal health.

This study examines how smartphones, laptops and tablets are used to create a sense of security, place, and time in the everyday lives of their consumers. Mark Deuze (2012) and his take on the modern society as a zombie apocalypse has been an inspiration for my work, there of the title. ”We’re all fucking zombies” is a metaphor for how highly-connected people of today are by living in and through the media, instead of living with it. To implement this I’ve chosen a phenomenological perspective, as it is the media-users’ subjective experiences of their everyday lives that I’m mainly interested in studying. I decided to focus on the mobile use of media since most of the research in this area focus on domestic media use. The theoretical framework that has set the foundation for the study is a combination of the time- space-dimensions of mobility, media as practice, symbolic interactionism, relational artifacts and phenomenological sociology. The purpose of using these theories is to get insight on how the media creates new opportunities for our social life, and to get an overview of how the new technological media leads to entirely new types of practices. The empirical data has been collected through a qualitative focus group interview with four respondents. They were selected to participate as they perceive themselves to be above average in comparison with the statistics of Mediebarometern (2014). The results showed that being connected through mobile media is considered to be of high importance. Although the ”connectedness” is only vital when being present in the locations directly related to the everyday life. Based on Silverstones (1994) explanation of phenomenology, and his studies of how television contributes to the ontological security, I conclude that the personalized mobile media has the same effect on it’s users.

Body Dysmorphic Disorder, BDD, is a psychiatric diagnosis that is characterized by self- perceived ugliness. The knowledge about BDD is low and people experience that they have the diagnosis, but haven’t been diagnosed with BDD due to the lack of knowledge among professionals. Previous research has mostly consisted of quantitative studies regarding the prevalence of BDD. The purpose of this study is to describe and analyze how people, who feel that they have BDD, describe living with it in their everyday life. The study is based on four narrative interviews, and the analysis of the results is based on a narrative approach. The results show that many areas in life are negatively affected by BDD, such as, school or work, relationship with others and quality of life. There is also a lack of knowledge regarding BDD among professionals which affects the treatment of people with BDD. In conclusion, BDD is a complex diagnosis to live with and many aspects in life are limited because of the diagnosis. Therefor the knowledge of BDD must increase in order for people to be able to get the right treatment.

The everyday dinner usually involves a number of different and sometimes conflicting ambitions that may include striving for self-fulfillment and striving to care for one’s family and society at large. To understand the consumption that occurs in connection with these ambitions, consumer researchers must understand the context surrounding the everyday dinner. In this dissertation theories of practice are utilized as a conceptual framework to emphasize the importance of context. Theories of practice have gained renewed interest within the field of consumption. Yet, Consumer Culture Theory (CCT) has neglected practice theories’ ability to operationalize the consumption context. The aim of this dissertation is to develop further CCT’s practice perspective to increase the understanding of the consumption context and thereby better understand consumption as a social and cultural phenomenon. An ethnographic approach is employed to identify what practices operate within a complex consumption situation such as the everyday dinner among single mothers; how these practices incorporate consumption in their strivings and how the different practices operating within the consumption situation interact with one another. This new approach comes to the conclusion that mothering, defined as a meta-practice, dominated the consumption situation and organized the other practices involved. A meta-practice is one with major influence over consumption and thus a type of practice consumption researchers should look for. Furthermore in Western society consumption situations, like the everyday dinner, seem to be especially important when it comes to anchoring meta-practices and thereby the social order. A preliminary characterization of the meta-practice is proposed as consisting of four different traits: I) its impact on the social order; II) its generalizability, density and superiority; III) its regulation and IV) its stability or slow change. However, more studies are necessary to explore these characteristics further.

Internet non-use : a comparative study of Great Britain and Sweden

Digital inequalities continue to concern researchers across highly technologised countries. In recent years, however, most research on digital inequalities has focused on distinguishing different patterns of Internet use rather than examining Internet non-users. The few quantitative studies of non-use and reasons for non-use often stay on a descriptive level, while qualitative studies mostly investigate subpopulations, such as those living in specific neighbourhoods. Although there are a number of studies of digital inequalities among young people and the elderly, a systematic literature search finds no research focusing on middle-aged individuals, who might face specific problems with being offline, as they are of working age and more likely than other age groups to have children living in the household. On the basis of a mixed-methods research design that compares non-users in Great Britain and Sweden, the overarching research question for this study sheds light on the mechanisms contributing to Internet non-use, as it examines the socio-economic backgrounds, attitudes, perceptions, and everyday life experiences of non-users with a focus on 25 to 55 year-olds. It applies advanced statistical analyses of secondary nationally representative survey data as well as in-depth analysis of qualitative interviews with 10 middle-aged non-users from each country. The analysis of these combined data shows that while socio-economic variables still play an important role in influencing who is on- and offline, general attitudes towards technologies are even more influential. The study finds that there is a lack of life-fit (Selwyn 2006) of ICTs for a number of non-users and a potentially negative impact of warm experts (Bakardjeva 2005) who have previously been regarded as a positive influence. Moreover, vulnerable groups (e.g. immigrants) are facing more substantial problems, such as issues with literacy and language, which need to be tackled first. The deliberation of similarities and differences between Britain and Sweden against the backdrop of policy interventions and cultural values shows that being offline is not problematic for all Internet non-users; some of them thrive in highly technologised societies. The conclusion offers proposals for both future research and policy interventions for those, currently offline mainly due to socio-economic reasons, who would like to go online in the future.

Sociology of small things : Olive Schreiner, Eleanor Marx, Amy Levy and the intertextualities of feminist cultural politics in 1880s London

This thesis investigates the cultural politics of a small group of women through their writing and other activities in 1880s London. Focussed on Olive Schreiner, Eleanor Marx and Amy Levy and the connections they had to one another and to other women, such as Henrietta Frances Lord, Clementina Black and Henrietta Müller, it explores key events in their everyday lives, the writings and texts they produced. It analyses a wide selection of textual sources, re-reading these for small details, intertextual connections and points of disjuncture, to allow for different ways of understanding the mechanics of feminist cultural politics as produced and performed by these interconnected women. Small things in texts can be revealing about such women’s everyday lives and connectedly the cultural politics which underpinned their actions, thus contributing to knowledge about how writing was used strategically and imaginatively to challenge, side-step and overcome oppression and inequality, in these years in London and after. Using the term ‘writing’ in a broad sense to include letters and diaries and other archival sources such as newspaper articles, reviews and manuscript drafts, as well as some selected published work and biographies, the thesis is anchored around four event-driven investigations: Olive Schreiner being accosted by a policeman; the first public performance of Ibsen’s A Doll’s House; the writing of a letter mentioning Eleanor Marx; and, the death of Amy Levy. Relatedly, there are discussions concerning working with historical documents, documenting and archiving the past, researching and representing the past in the present. These investigations allow for the operationalization of a research approach framed by ideas concerning micro, small-scale, everyday life and its qualitative aspects, which together contribute to a re-conceptualisation of a ‘sociology of small things.’ Specifically, it is argued that close and small-scale studies of women’s writing, whether undertaken alone or connected to others, sheds light on the importance of relationship dynamics in connection with writing output, on what writing was produced and what role each text played in larger scale political agendas. Concepts such as palimpsest, liminality and bricolage are interrogated with respect to researching and representing the spatial and temporal interconnectedness of the selected authors and textual sources. And contributions are made to contemporary thinking about epistolarity and social networks, focussing on reciprocity, gift-giving and receiving and notions of ‘letterness,’ along with the defining of boundaries, and the value of determining the nature of ties between women. The thesis also argues that the relationships between intimacy and distance, interiority and exteriority, public and private, are frayed with complicated overlaps.
