Infecção pelo vírus linfotrópico de células T humanas do tipo 1 (HTLV-1) em uma coorte acompanhada em São Paulo / Human T-cell lymphotropic virus type 1 (HTLV-1) infection in a cohort followed up in São Paulo

INTRODUÇÃO: O virus linfotrópico de células T humanas do tipo 1 (HTLV-1) é endêmico em várias partes do mundo e transmitido primariamente através de relações sexuais ou da mãe para o filho. MÉTODOS: Estes modos de transmissão foram investigados na coorte de pacientes com HTLV-1 acompanhados no Instituto de Infectologia Emílio Ribas de São Paulo comparando-se casais soroconcordantes e sorodiscordantes, para estudo da transmissão sexual, e binômios mãe-filho soroconcordantes e com filho soronegativo para estudo da transmissão vertical. Os dados foram consolidados e depois analisados utilizando o sistema RedCap (Research Electronic Data Capture) e o programa computacional estatístico Stata/IC 13.1. Resultados com valor de p < 0,05 foram considerados estatisticamente significativos. Variáveis com p < 0,2 na análise bivariada foram incluídos na análise multivariada. RESULTADOS: Entre janeiro de 2013 e maio de 2015, de 178 pacientes com HTLV-1 que se declararam casados, 107 (46 homens e 61 mulheres) tinham parceiro testado, resultando em 81 casais (26 homens e 26 mulheres já formavam pares entre si). Foram excluídos aqueles com HIV ou HTLV-2. A taxa de soroconcordância entre casais foi 46,9%. A carga proviral (PVL) de HTLV-1 foi comparada entre 19 casais soroconcordantes e 37 sorodiscordantes, e os casais soroconcordantes apresentaram cargas provirais mais elevadas (p = 0,03). Não houve diferença entre os grupos de acordo com idade, tempo de relacionamento, ter mãe ou irmão com HTLV-1, raça, local de nascimento, escolaridade, história de hemotransfusão, HAM/TSP, ATL ou soropositividade para hepatite C. Na análise multivariada, no entanto, o tempo de relacionamento (> 20 anos) manteve-se independentemente associado com a ocorrência de soroconcordância entre casais (p = 0,031). Por sua vez, no período de junho de 2006 a agosto de 2016 havia 192 mães com infecção pelo HTLV-1, resultando em 499 filhos expostos. Destes, 288 (57,7%) foram testados para HTLV-1, constituindo-se na amostra final para o estudo, juntamente com respectivas 134 mães. Entre os filhos testados, 41 foram positivos para HTLV-1, indicando taxa de transmissão vertical de 14,2%. Sete dos 134 núcleos familiares concentraram 20 (48,8%) do total de 41 filhos soropositivos e cinco apresentavam prole com três ou mais filhos soropositivos para HTLV-1. Estiveram associadas à soropositividade do filho: duração da amamentação >= 12 meses, PVL materna >= 100 cópias/104 PBMC, idade da mãe no parto > 26 anos, raça/etnia asiática, estado civil divorciada, filho com avó soropositiva para HTLV-1, ter irmão com HTLV-1. Na análise multivariada, amamentação >= 12 meses, PVL materna elevada e ter irmão com HTLV-1 mantiveram-se independentemente associados ao desfecho. CONCLUSÕES: Os resultados indicam que o HLTV-1 vem sendo transmitido ativamente na coorte, tanto por via sexual como materno infantil, com agregação familiar de casos e que o risco de transmissão entre casais persiste após décadas (mais de 20 anos) de sorodiscordância. Foi observada associação entre PVL e transmissão do vírus, tanto por via sexual como materno-infantil, mesmo depois de anos após o desfecho. PVL materna elevada e amamentação prolongada estiveram independentemente associados à transmissão vertical, sendo necessários outros estudos avaliando a influência de fatores genéticos. / INTRODUCTION: Human T-cell lymphotropic virus type 1 (HTLV-1) is transmitted primarily either through sexual intercourse or from mother to child. METHODS: The current study investigated sexual and vertical transmission among individuals diagnosed as HTLV-1-positive who have been followed up at the Institute of Infectious Diseases \"Emilio Ribas\". In order to study the sexual transmission and mother-to-child transmission, the selected individuals with their respective pairs (couples or mother and son, repectively) were classified into seroconcordant or serodiscordant groups according to serological findings. Data were collected and managed using Research Electronic Data Capture (REDCap) and Stata/IC 13.1 for Windows. p values < 0.05 were considered statistically significant. Variables with p < 0.2 in bivariate analysis were included in multivariate analysis. RESULTS: Between January 2013 and May 2015, 178 HTLV-1-positive patients had spouses, 107 of which (46 men and 61 women) had tested partners, thus forming the initial sample (81 couples). Individuals co-infected with HTLV-2 or human immunodeficiency virus were not included in the analysis. The rate of seroconcordance was 59.8%. The HTLV-1 proviral load was compared between 19 and 37 seroconcordant and serodiscondant couples, respectively, and the concordant couples showed higher proviral loads (p = 0.03). There were no differences between the groups according to age, relationship length, having a mother or sibling with HTLV-1, race, ethnicity, nationality, education, history of blood transfusion, HAM/TSP, ALT, or hepatitis C virus status. In multivariate analysis, relationship time (over 20 years) was shown associated with ocurrence of seroconcordance status (p = 0.031). In turn, between June 2006 and August 2016 there were 192 mothers with HTLV-1 infection, resulting in 499 exposed children. Of these, 288 (57.7%) were tested for HTLV-1, forming the final sample for the study with 134 respective mothers. Among the tested soons, 41 were positive for HTLV-1, indicating vertical transmission rate of 14.2%. Of 41 positive sons for HTLV-1, 40 (48.8%) were clustered in seven of 134 households, and five households had their offspring with three or more sons seropositive for HTLV-1. The following variables were associated with positive son for HTLV-1: breastfeeding duration >= 12 months, maternal PVL >= 100 copies/104 PBMC, mother\'s age at delivery > 26 years, asian race/ethnicity, divorced marital status, child with grandmother seropositive for HTLV-1, have brother with HTLV-1. In multivariate analysis, breastfeeding >= 12 months, higher maternal proviral load and have brother with HTLV-1 remained independently associated with the outcome. CONCLUSIONS: The results indicate that the HLTV-1 is actively transmitted in the cohort both sexually and vertically, with ocurrence of familial clustering. The sexual transmission risk persists among couples even after decades (over 20 years) of serodiscordance. PVL remained associated with the outcome even several years after the occurrence of the transmission both sexually and vertically. High maternal provirus load and breastfeeding beyond 12 months were independently associated with positive son for HTLV-1, but it is necessary further studies to evaluating the influence of genetic factors on the mother-to-child transmission.

Uticaj programa unapređenja prevencije i kontrole krvnoprenosivih infekcija na smanjenje profesionalnog rizika radnika u zdravstvu / Influence of program of strengtheningprevention and control of blood-borneinfections to reduction of professionalrisk of health personnel

<p>Infekcije krvnoprenosivim virusima (HIV, hepatitis B virus, hepatitis C virus)<br />predstavljaju veoma aktuelan problem. Iako se primenom op&scaron;tih i specifičnih<br />mera prevencije i postekspozicione profilakse ove infekcije među radnicima u<br />zdravstvu mogu gotovo u potpunosti sprečiti, &scaron;to je i slučaj u razvijenim<br />zemljama, u AP Vojvodini su infekcije krvnoprenosivim virusima među<br />radnicima u zdravstvu i dalje prisutne.<br />Ciljevi istraživanja bili su da se utvrdi učestalost rizičnih i protektivnih faktora za<br />profesionalnu izloženost krvnoprenosivim infekcijama, učestalost profesionalne<br />izloženosti krvnoprenosivim infekcijama, uticaj sprovođenja Programa<br />unapređenja prevencije i kontrole krvnoprenosivih infekcija u zdravstvenim<br />ustanovama na smanjenje izloženosti zdravstvenih radnika potencijalno<br />infektivnom bolesničkom materijalu i unapređenje mera prevencije i kontrole<br />krvnoprenosivih infekcija u zdravstvenim ustanovama i da se identifikuju<br />kritične tačke u sistemu prevencije i kontrole krvnoprenosivih infekcija u<br />zdravstvenim ustanovama.<br />Za potrebe istraživanja je formirana kohorta koja je obuhvatala populaciju<br />radnika u zdravstvu sa područja AP Vojvodine, koji povremeno ili stalno dolaze<br />u kontakt sa potencijalno infektivnim bolesničkim materijalom. Deo kohorte je<br />obuhvaćen Programom unapređenja prevencije i kontrole krvnoprenosivih<br />infekcija, čija je implementacija započeta u septembru 2007. godine (zaposleni u<br />državnom sektoru), a deo nije obuhvaćen ovim Programom (zaposleni u<br />privatnom sektoru). Nakon perioda praćenja od &scaron;est meseci, izvr&scaron;ena je<br />evaluacija rezultata sprovođenja ovog Programa.<br />Preko 80% radnika zaposlenih u državnim i privatnim zdravstvenim<br />ustanovama, povremeno ili stalno dolazi u kontakt sa krvlju ili drugim telesnim<br />tečnostima i u proseku imaju 15,5 izlaganja u toku dana. Svega dve trećine<br />5<br />zaposlenih u zdravstvenim ustanovama, koji dolaze u kontakt sa krvlju i drugim<br />telesnim tečnostima, redovno koriste za&scaron;titne rukavice, manje od 15% za&scaron;titnu<br />masku, dok za&scaron;titne naočare koristi manje od 3% ispitanika u državnom i oko<br />22% ispitanika u privatnom sektoru. Manje od 40% zaposlenih u državnom<br />sektoru i manje od 30% zaposlenih u privatnom sektoru su potpuno vakcinisani<br />protiv hepatitisa B. Manje od dve trećine zaposlenih u zdravstvu pravilno uklanja<br />opasan medicinski otpad. Preko 30% ispitanika imalo je povredu na iglu, 16%<br />povredu na o&scaron;tar predmet, 38% kontakt kože sa krvlju, a skoro 14% imalo je<br />kontakt sluzokoža sa krvlju. Najče&scaron;će dolazi do izlaganja medicinskih<br />sestara/tehničara, nemedicinskog osoblja i učenika srednjih medicinskih &scaron;kola. U<br />većini slučajeva do izlaganja dolazi perkutano, nakon zavr&scaron;etka intervencije,<br />nepravilnim uklanjanjem opasnog medicisnkog otpada. Propusti koji su u<br />potpunosti preventivni (bezbedno uklanjanje medicinskog otpada i nevraćanje<br />za&scaron;titne kapice na iglu) su odgovorni za 59% slučajeva izlaganja.<br />Uvođenjem Programa unapređenja prevencije i kontrole KPI u zdravstvenim<br />ustanovama, broj profesionalnih izloženosti značajno je smanjen &ndash; za 30-40% je<br />bilo manje uboda na iglu i povreda o&scaron;trim predmetom. Tokom 6 meseci praćenja<br />procenjeno je da se u zdravstvenim ustanovama u AP Vojvodini koje su bile<br />obuhvaćene programom sprečilo 2500 uboda na iglu, 3000 povreda o&scaron;trim<br />predmetom, 2000 kontakata krvi sa sluzokožama i 2500 izlaganja kože krvi<br />pacijenta. Dok se pre realizacije programa tek svaka 200-ta izloženost<br />registrovala, nakon 6 meseci realizacije programa registrovana je svaka deseta<br />profesionalna izloženost.</p> / <p>Blood-borne infections (HIV, hepatitis B, hepatitis) are an important health<br />problem. Despite those infections are preventable by application of general and<br />specific measures of prevention and control, which is the fact in developed<br />9<br />countries, in AP Vojvodina, those professionally acquired infections are still<br />present,<br />The aims of research were to identify incidence of risky and protective factors for<br />professional exposure to BBI, incidence of professional exposures, influence of<br />program for prevention and control of BBI in health care settings to reduction of<br />exposures of health personnel and to strengthen implementation of measures of<br />prevention and control, as well as to identify critical points in prevention and<br />control.<br />The cohort of population of health personnel from AP Vojvodina was formed,<br />including those who are in contact with potentially infectious fluids. Part of the<br />cohort was covered by program for strengthen prevention and control of BBI,<br />which implementation started in September 2007 (public health sector), and was<br />not (private health sector). After six months since the program started, we<br />evaluated its results.<br />More then 80% of health personnel in private and public sector is in contact with<br />potentially infectious body fluids with average 15.5 exposures daily. Only two<br />thirds of them use protection gloves regularly, less than 15% protection mask,<br />and less than 3% in public and around 22% in private sector use protection<br />glasses. Less than 40% in public and 30% in private sector are vaccinated against<br />hepatitis B. Less than 2/3 disposes dangerous medical waste properly. More than<br />30% had needle stick injury, 16% other sharp injuries, 28% skin contact with<br />blood, and almost 14% mucosal contact with blood. Nurses and non medical staff<br />are exposed mostly, as well as medical students. The most of the exposures are<br />percutaneous, after medical intervention, due to wrong practices of waste<br />disposal. Completely preventable practices (waste disposal, recapping of needles)<br />are responsible for 59% exposures.<br />Due to program of strengthening prevention and control of BBI in health care<br />institutions, number of professional exposures declined - 30-40% less needle<br />stick and sharp object injuries. During 6 months of follow up, it is estimated that<br />10<br />2500 needle stick injuries, 3000 sharp object injuries, 8000 skin exposures and<br />2500 mucosal exposures were prevented. Comparing with every 200th exposure<br />reported before start of the program, during 6 months every 10th exposure was<br />reported.</p>

Hepatitis B vaccination in end-stage pulmonary disease patients evaluated for lung transplantation: a retrospective single-center evaluation

Background: In times of limited organs for transplantation, anti-HBc positive organs can be accepted for lung transplantation to increase the number of donors. Transplant recipients should be vaccinated against hepatitis B to prevent HBV infection. However, response after HBV vaccination has only been poorly evaluated in patients with end-stage pulmonary disease. Material/Methods: Anti-HBs titers of 40 anti-HBc negative patients with end-stage pulmonary disease evaluated for lung transplantation were analyzed with the Architect® system (Abbott, Germany). Responders, partial responders, or non-responders after HBV vaccination were defined by anti-HBs titers >100 IU/L, 10–100 IU/L, and <10 IU/L, respectively. Results: There were 34/40 individuals (85%) vaccinated against hepatitis B, and 6 were not vaccinated. Response, partial response, and non-response after vaccination were observed in 10/34 (29.4%), 11/34 (32.4%), and 13/34 (38.2%) of patients, respectively. Response to vaccination did not correlate with sex, pulmonary disease, comorbidities, immunosuppressive therapy, or smoking status. Conclusions: Although 85% of patients evaluated for lung transplantation were vaccinated against hepatitis B, 38.2% did not show an anti-HBs titer >10 IU/L. Thus, anti-HBs titers should be regularly monitored. Nonresponders should be considered for booster vaccinations, alternative vaccination schedules, or prophylactic treatment with a nucleos(t)ide analogue in case of transplantation of an anti-HBc–positive organ.

Molecular and Functional Properties of Transmitted HIV-1 Envelope Variants: A Dissertation

In 2008 the Nobel Prize in Physiology or Medicine was awarded to the co-discoverers of the Human Immunodeficiency Virus Type 1 (HIV-1), the causative agent of Acquired Immunodeficiency Syndrome (AIDS). This award acknowledged the enormous worldwide impact of the HIV-1/AIDS pandemic and the importance of research aimed at halting its spread. Since the syndrome was first recognized, 25 million people have succumbed to AIDS and over 33 million are currently infected with HIV-1 (www.unaids.org). The most effective strategy for ending the pandemic is the creation of a prophylactic vaccine. Yet, to date, all efforts at HIV-1 vaccine design have met with very limited success. The consistent failures of vaccine candidates stem in large part from the unprecedented diversity of HIV-1. Among the novel theories of vaccine design put forward to address this diversity is the targeted vaccine approach. This proposal is based on the finding that mucosal transmission of HIV-1, the most prevalent form, occurs across a selective bottleneck such that typically only a single (or a few) variants of the viral swarm present in a donor are passed to the recipient. While the mechanisms controlling the selection are largely unknown, the targeted vaccine approach postulates that once they are identified, we can utilize this understanding to design vaccines specifically targeted to the characteristics shared by the rare, mucosally transmissible HIV-1 variants. The studies described in this work were conducted to improve our understanding of the factors influencing viral variant selection during mother-to-child-transmission of HIV-1, a route of mucosal transmission which has globally become the leading cause of child infection. A unique panel was generated, consisting of nearly 300 HIV-1 envelope genes cloned from infected mother-infant pairs. Extensive characterization of the genotypes, phenotypes and phylogeny of these clones was then done to identify attributes differentiating early infant from maternal variants. Low genetic diversity of HIV-1 envelope variants was detected in early infant samples, suggesting a bottleneck and active selection of variants for transmission. Transmitted variants did not differ from non-transmitted variants in CD4 and CCR5 use. Infant isolates replicated poorly in macrophages; a cell subtype hypothesized to be important in the establishment of infection. The sensitivity of infant envelope variants to neutralization by a panel of monoclonal antibodies, heterologous and autologous plasmas and HIV-1 entry inhibitors varied. Most intriguingly, envelopes cloned from infants infected during delivery exhibited a faster entry phenotype than maternal isolates. Together, these findings provide further insight into viral variant selection during mother-to-child transmission. Identification of properties shared by mucosally transmitted viral variants may allow them to be selectively targeted, resulting in improved methods for preventing HIV-1 transmission.

Ultrastructure of Cimex lectularius L. (Hemiptera: Cimicidae) salivary glands after a blood meal infected with Bartonella henselae (Hyphomicrobiales: Bartonellaceae)

Bed bugs (Hemiptera:Cimicidae) are a common, hematophagous ectoparasite of humans and other animals, and are experiencing an international resurgence. Cimicids have been implicated in the transmission many disease agents, including various Bartonella species, however disease transmission has not yet been confirmed. Bartonella spp. are transmitted by a variety of arthropods, including fleas, lice and sand flies, and it is speculated that bed bugs may also serve as a potential vector. In this study, we used an artificial membrane to feed two groups of adult Cimex lectularius rabbit blood, either infected or uninfected with Bartonella henselae. After two weeks, the presence of Bartonella henselae was assessed via PCR, and salivary glands from infected and uninfected bed bugs were dissected and processed for transmission electron microscopy. We were unable to visually identify Bartonella henselae in the images, and therefore unable to confirm the role of bed bugs in B. henselae transmission.

The Challenges and Opportunities in Monitoring and Modeling Waterborne Pathogens in Water- and Resource-Restricted Africa: Highlighting the critical need for multidisciplinary research and tool advancement

Water is a primary shared resource that connects all species across the landscape and can facilitate shared exposure to a community of waterborne pathogens. Despite remarkable global progress in sanitation and hygiene development in the past two decades, infectious diarrhea remains a prominent public health threat in sub-Saharan Africa. This thesis identifies and discusses persistent challenges limiting the success of current waterborne disease management strategies and several existing research hurdles that continue to impede characterization of microbial transmission and transport. In this work, the Chobe River watershed in Northern Botswana serves as a target study site for the application of hydrological modeling tools to quantify emergent water quality and health challenges in Southern Africa. A watershed model with extensive data requirements, the Hydrological Simulation Program – Fortran (HSPF), is used to identify primary data gaps and model assumptions that limit the progress of model development, and guide opportunities for data collection, tool development, and research direction. Environmental pathogen exposure risk and epidemiological outbreak dynamics are best described by interactions between the coupled human and environmental processes within a system. The challenge of reducing diarrheal disease incidence strengthens a call for research studies and management plans that join multiple disciplines and consider a range of spatiotemporal scales. / Master of Science

Molecular epidemiology of mother-to-child transmission of HIV-1 in children at Tygerberg Hospital

Thesis (MMed (Medical Microbiology))--University of Stellenbosch, 2006. / One of the major routes of transmission of human immunodeficiency virus (HIV) in the developing world is vertical transmission from mother to infant – pre-, intra-, or post-partum. In the Western Cape, HIV-1 subtype C is the predominant subtype in the heterosexual population, and this trend was expected to be seen amongst cases of mother-to-child transmission of HIV. The aim of this study was to perform genetic characterisation and phylogenetic analysis of the HIV-1 genome in positive serum/plasma samples obtained from children (age 0 to 18 months) from 2000-2002, and temporally related specimens from their mothers. We obtained 27 suitable pairs of samples taken within 6 months of delivery. From this pool, we obtained 21 infant DNA sequences and 17 maternal sequences, resulting in 16 mother-infant pairs. All patient sequences were identified as HIV-1 subtype C, and, as expected, mother and infant viral sequences clustered together. In some cases where a mother was suspected to have two dominant quasispecies based on the electropherogram, only one sequence was detectable in the infant. Single or multiple amino acid deletions were consistent between mothers and infants, and some pairs showed the same amino acid deletions seen in other pairs.

The Western Cape government, through the Premier, has committed itself to providing access for all HIV positive women in the province attending antenatal clinics to MTCT prevention programmes by the end of 2003. The MTCT prevention programme is aimed at intervening transmission during pregnancy, during the birth of the baby and post delivery during breast-feeding of the newborn baby. The HIV virus attacks and destroys the cells of the immune system by being incorporated into the reproductive cycle of the cell. Antiretroviral drugs in the fight against HIV target two aspects of this replication cycle of the virus and as such do not halt the infection but slow it down by preventing the replication of the virus. MTCT internationally has been reduced dramatically by the use of antiretroviral drugs (AZT) antenatally, during birth and post-natally for mother and baby. However this regimen was found to be not suitable for resource-poor countries due to the complexity in administration, compliance and costs. Poor countries have developed alternative MTCT prevention programmes based on shorter course regimens and on alternative antiretroviral drugs. MTCT prevention In South Africa initially did not have the support of the national government. However, the -Western Cape Administration had developed a regional programme phasing in MTCT prevention at all state clinics within the province. The cost drivers associated with the MTCT prevention programme include costs associated with the HIV tests, costs for pre- and post-test counselling, costs for the antiretroviral drugs and costs for substitute feeding to replace breast-feeding to prevent viral transmission during breast-feeding. Depending on the option exercised, costs can be calculated per HIV positive baby averted. As regards substitute feeding, which is a substantial cost driver and a major disadvantage to the newborn baby as regards protection from common childhood diseases such as diarrhea, research from Tanzania showed that ARV therapy during childbirth could suppress HIV transmission through breast-feeding during the first few weeks after childbirth, thus enabling an optimal combination of breast-feeding in the first few crucial weeks of the newborn's life for the protective benefits of breast-milk and formula feeding thereafter to ensure protection for the infant from HIV transmission through breastfeeding. The data were subjected to a sensitivity or a "what-if' analysis usmg an Excel spreadsheet. Costs per HIV averted were calculated for each assumed parameter in the model that was developed. As regards prevalence, the MTCT prevention programme is most cost effective at the lower levels of the seroprevalence of HIV in the population served. If the seroprevalence is above the "cross-over" or "optimal" point for the regimen an alternative regimen should be assessed. As regards levels of identification and number accepting intervention the principle of an economy of scale applies and therefore, a level of 100 percent in both cases should be targeted. This will be dependent on wide scale advertisements and support for the programme with active efforts to destigmatise the infection. Innovative programmes such as the mothers-to-mothers-to-be (M2M2B) programme must be encouraged. / AFRIKAANSE OPSOMMING: Na seksuele oordraging van HIV is moeder tot kind (MTK) oordraging die algemeenste rede van HIV infeksie in Suid Afrika. Die Wes-Kaapse regering, deur die Premier, het hulself verbind om toegang vir alle HIV positiewe vroue in die provinsie, wat voorgeboorte klinieke bywoon, tot MTK voorkomingsprogramme voor die einde van 2003 moontlik te maak. Die MTK voorkomingsprogram is gerig op die tussenkoms van transmissie gedurende swangerskap, gedurende die geboorte en die nasorg tydens die borsvoeding periode van die pasgebore baba. Die HIV virus val die selle van die immuun sisteem aan en' vernietig dit. Die virus word geïnkorporeer tot die reproduksie siklus van die sel. Jovens vivendo com HIV desde a infância: características clínicas e preditores de controle da infecção na transição de cuidado pediátrico para o de adultos / HIV-infected youths transitioning from pediatric to adult care: clinical characteristics and predictors of infection control

INTRODUÇÃO: Crianças com infecção por HIV por transmissão perinatal ou adquirida nos anos 1980 e 90 começaram a ser transferidas da pediatria para ambulatórios especializados no cuidado de adultos ao final da primeira década dos anos 2000. A transição entre esses serviços para jovens com outras doenças crônicas é um momento crítico que pode estar associado a maior número de intercorrências clínicas, mas pouco há descrito sobre desfechos clínicos desfavoráveis nos pacientes infectados pelo HIV. MÉTODOS: Estudo de coorte retrospectiva, com inclusão de todos os jovens transferidos do Ambulatório de Infectologia Pediátrica para o Serviço de Extensão ao Atendimento de Pacientes com HIV/aids, ambulatório responsável pelo atendimento de jovens e adultos com HIV/aids, ambos do Hospital das Clínicas da Faculdade de Medicina da USP, até dezembro de 2012. Foi realizado acompanhamento durante o período de transição, desde os 2 anos anteriores até 2 anos após a transferência entre serviços. Foram descritos o perfil sócio-demográfico e as características clínico-laboratoriais dos participantes no período do estudo. Além disso, foram realizados estudos comparativos dos parâmetros clínicos e laboratoriais dos jovens durante o seguimento nos dois serviços, análise de associações com a viremia do HIV e o status imunológico no momento de chegada ao serviço adulto e avaliação de possíveis fatores preditores dos desfechos clínico-laboratoriais durante o período da transição, com ajuste para possíveis confundidores. RESULTADOS: Durante o período estudado foram transferidos 41 jovens, com mediana de idade de 19 anos, 95% dos quais adquiriram HIV por transmissão vertical, 54% de mulheres, 73% de cor branca e escolaridade mediana de 12 anos. 59% dos jovens já havia iniciado a vida sexual e 4 (10%) estavam gestantes ou tinham parceiras em gestação. O nadir mediano de células CD4+ foi de 117/mm3, com mediana de 15 anos de uso de antirretrovirais e 89% com classificação de aids pediátrica (critério CDC) entre B2 e C3. Na chegada ao SEAP a mediana de células CD4+ foi de 250/mm3 e viremia por HIV foi >= 400cp/ml em 51% dos jovens. Registros de baixa adesão ao tratamento foram relatados no prontuário de 54% dos jovens e 34% apresentou adoecimento relacionado à aids nos últimos 2 anos no ICr. Durante o período da transição a adesão inadequada (aferida por registro de prontuário, retirada de medicamentos antirretrovirais na farmácia ou falta em consultas) foi superior a 70% em ambos os serviços. Contudo, a carga viral mediana teve redução progressiva (3,72 para 1,95 log10 cópias/ml) e a mediana do número de células CD4+ elevou-se ao final do seguimento (289 para 376/mm3). A incidência de adoecimentos relacionados à aids e de hospitalizações foi semelhante em ambos os serviços. No que se refere a fatores de risco associados a desfechos desfavoráveis, não foram encontrados fatores associados à pior adesão. Entretanto, identificou-se associação entre viremia por HIV no período de transição e menor nadir de células CD4+, uso de maior número de esquemas ARV na pediatria e adesão inadequada ao tratamento. Menor número de células CD4+ foi associado a menor nadir de células CD4+, adesão inadequada, maior carga viral do HIV, uso de efavirenz e a não ter o estudo como ocupação exclusiva durante o período de transição pediátrica. Quanto aos adoecimentos relacionados à aids, houve maior incidência entre os jovens com menor nadir de células CD4+ e menor número de células CD4+. Por fim, as hospitalizações foram associadas com adoecimentos relacionados à aids e com baixo nadir de células CD4+. Conclusão: Os jovens avaliados possuem histórico de imunodepressão grave e adoecimentos relacionados à aids, além de longo período de exposição aos antirretrovirais. Chegaram no serviço adulto com dificuldade de adesão ao tratamento, com controle insatisfatório da infecção pelo HIV, em plena vivência da sexualidade e com baixa inserção no mercado de trabalho ou no ensino superior. A transição entre os serviços não teve como impacto a piora na adesão ou nas características clínico-laboratoriais. No entanto, houve manutenção de alto percentual de jovens com viremia detectada e sem a restauração adequada da imunidade. Nesse sentido, há necessidade de atenção redobrada para ocorrência de eventos clínicos desfavoráveis durante esse período, principalmente entre os jovens com menor nadir de células CD4+ e número baixo de células CD4+ / INTRODUCTION: Children with perinatal HIV infection or infected in the 1980\'s and 1990\'s have been transitioning to adult care since late 2000´s. Among young adults with other chronic conditions, transition to adult care is a critical moment that can be associated with worsening disease control, but little is described on unfavorable outcomes among HIV-infected youths. METHODS: Retrospective cohort study, including all consecutive youths referred from pediatric to adult HIV care at Hospital das Clinicas da Faculdade de Medicina da Universidade de Sao Paulo up to December 2012. Follow-up comprised the transition period between 2 years prior to 2 years after transition to adult care. We described socio-demographic and clinical characteristics of study participants during the study period. Furthermore, we compared clinical and laboratory variables observed during follow-up in pediatric and adult care, investigated factors associated with viral suppression and immune status at the first visit in the adult care setting, and studied predictors of clinical and laboratory outcomes over the transition period with adjustment for potential confounders. RESULTS: 41 youths were referred to adult HIV care during the study period, with median age of 19 years, 95% of whom acquired HIV infection through mother-to-child transmission; 54% were female, 73% were caucasian, with median schooling of 12 years. 59% reported having had sexual debut and 4 (10%) were either pregnant or had partners expecting a child. Median CD4+ cell nadir was 117/mm3, 15 years was the median time of antiretroviral use and 89% had pediatric CDC AIDS classification between B2 and C3. At the first laboratory assessment at the adult care service, median CD4+ count was 250/mm3 and HIV viremia was detectable for 51% of participants. Poor adherence was recorded in medical charts for 54% of youths and 34% had developed AIDS-related conditions during the last two years of follow-up at pediatric care. During the transition period, inadequate adherence (determined by either medical chart report, inadequate pharmacy withdrawal of antiretrovirals or missed medical appointments) was identified in 70% of the cohort, in both pediatric and adult services. Nonetheless, HIV viral load lowered progressively (median 3.72 to 1.95 log10 copies/ml) and median CD4+ count increased at the end of follow-up (289 to 376 cells/mm3). The incidence of AIDS-related conditions and hospitalizations was similar in pediatric and adult care services. Regarding predictors of unfavorable outcomes, we failed to find factors associated with poor adherence. However, we found association between HIV viral load during the transition period and lower CD4+ nadir, higher number of antiretroviral regimens during pediatric care and inadequate treatment adherence. Lower CD4+ count was associated with lower CD4+ nadir, poor adherence, higher HIV viral load, efavirenz use and with occupation other than being only a student during pediatric transition period. AIDS-related conditions were more frequent among youths with lower CD4+ nadir and lower CD4+ count. Finally, hospitalizations were associated with AIDS-related conditions and lower CD4+ nadir. CONCLUSIONS: Youths enrolled in this study had a history of poor immune status and AIDS-related illnesses, with prolonged exposure to antiretrovirals. At transition to adult care, they presented with important challenges in adherence to treatment, poor control of HIV infection, fully experiencing sexual life and with poor participation in the workforce or high school. The transition from pediatric to adult care services did not worsen adherence or clinical and laboratory outcomes. However, there was still a high percentage of youths with detectable HIV viral load and inadequate immune recovery. Therefore, providers should have increased attention to unfavorable outcomes during the transition period, particularly for youths with lower CD4+ nadirs and lower CD4+ counts
