Qualidade vocal em individuos asmáticos com e sem disfunção paradoxal de pregas vocais: correlatos perceptivo-auditivos, acusticos e fisiologicos / Vocal quality in asthmatics with and without paradoxal vocal fold dysfunction: perceptual, acoustic and physiologic correlates

Methods: recording of acoustic and electroglottographic speech data from 9 individuals with PVFD, 8 with asthma without PVFD and 8 without breathing problems (control group). The perceptual analysis was carried out by means of the phonetic vocal quality analysis protocol. The acoustic analysis comprised both long and short term spectra. The Lx wave of the electroglottographic signals were analyzed and measures relative to the phases of the glottal cycle were extracted. Results: Both PVFD and asthma groups presented supralaryngeal settings characterized by smaller extension and higher constriction of the vocal tract. Laryngeal settings of both PVFD and asthma groups were found to be characterized by hyperfunction, creaky voice, rough voice, breathy voice, sonority breaks, diplophonia and instability, with higher frequency of occurrence of rough voice and breathy voice adjustments and short term instability events in the asthma group. The analysis of the long term spectrum declination lines made it possible to differentiate among the groups and genders: higher degree of spectral intensity was found in the feminine asthma and the masculine control groups and higher degree of spectral intensity in the PVFD group, differentiation also brought out by intensity measures of short and long term spectra. The inspection of the Lx wave revealed the asthma and PVFD groups differed from the control group due to the presence of noise, low intensity and prolonged closed phases. The electroglottographic measures related to speed index and contact coefficient area differentiated the asthma and PVFD groups. Conclusions: the analysis of the data corroborates the hypothesis of the presence of vocal alteration in patients with PVFD and in patients with asthma and favors the description of vocal quality as a diagnosis method, delineating the important demand of speech therapy treatment for these patients / Objetivo: investigar a qualidade vocal de um grupo de indivíduos com disfunção paradoxal de pregas vocais (DPPV) com asma em relação a um grupo de indivíduos asmáticos sem DPPV e a um grupo de indivíduos sem problemas respiratórios, segundo aspectos fonéticos (perceptivo-auditivos, acústicos e fisiológicos). Métodos: gravação de amostras de fala dos sinais acústico e eletroglotográfico de 9 indivíduos com DPPV, 8 com asma sem DPPV e 8 sem problemas respiratórios (grupo controle). O sinal acústico foi analisado do ponto de vista perceptivo-auditivo com roteiro de descrição da qualidade vocal com motivação fonética. As modalidades de análise acústica utilizadas foram os espectros de longo e curto termo. O sinal eletroglotográfico foi analisado a partir da filtragem dos registros, da inspeção da onda Lx e da extração de medidas que refletem as fases do ciclo glótico. Resultados: foram identificados, nos grupos DPPV e asma, ajustes supralaríngeos caracterizados por menor extensão e maior constrição do trato vocal. Os ajustes laríngeos indicaram hiperfunção, voz crepitante, voz áspera, escape de ar, quebras de sonoridade, diplofonia e instabilidade nos grupos DPPV e asma com maior ocorrência de ajuste de voz áspera, escape de ar e ocorrência de curto termo de instabilidade no grupo asma. Na análise acústica de longo termo houve diferenciação entre os grupos e gêneros com maior declínio espectral no grupo asma feminino e controle masculino e maior intensidade espectral no grupo DPPV, diferenciação marcada também pelas medidas espectrais de curto e longo termo. Em termos fisiológicos, na inspeção da onda Lx os grupos asma e DPPV aproximaram-se e diferenciam-se do grupo controle porque apresentaram ruído, baixa intensidade e fase fechada alongada. As medidas eletroglotográficas índice de velocidade e área do coeficiente de contato diferenciaram os grupos asma e DPPV. Conclusões: a análise dos dados contempla a hipótese de alteração vocal em pacientes com DPPV e em pacientes com asma e possibilita a descrição da qualidade vocal como um método de diagnóstico, apontando para a importante demanda de tratamento fonoaudiológico junto a estes pacientes

Vibração das pregas vocais pré e pós aproximação cricotireóidea: estudo experimental em laringes humanas por videoquimografia. / Vibratory pattern of the vocal folds pre and post cricothyroid approximation: experimental study in human larynges by videokymography.

A aproximação cricotireóidea simula a contração do músculo cricotireóideo, aproximando as cartilagens cricóidea e tireóidea anteriormente. Distende e tensiona as pregas vocais, elevando a freqüência fundamental da voz. É indicada em pacientes que apresentam voz grave em conseqüência da paralisia do nervo laríngeo superior ou por doenças endócrinas e reumatológicas no sexo feminino. Com o objetivo de estudar os efeitos da aproximação cricotireóidea sobre a vibração das pregas vocais em ambos sexos, foram avaliadas 30 laringes excisadas de cadáveres humanos frescos pré e pós aproximação cricotireóidea. Esse trabalho experimental permitiu excluir as interferências dos mecanismos compensatórios da fonação presentes em um estudo 'in vivo'. As cartilagens aritenóideas foram aproximadas por sutura, fechando a glote. Um fluxo de ar comprimido umedecido foi injetado no interior da traquéia, e ao passar pela glote promoveu a vibração das pregas vocais. Foi então realizada a aproximação cricotireóidea através de suturas, aproximando as cartilagens tireóidea e cricóidea. O comprimento da porção membranosa das pregas vocais foi obtido através de um paquímetro. A vibração das pregas vocais foi gravada pré e pós a aproximação cricotireóidea utilizando-se uma câmera de videoquimografia. Após digitalização e análise quadro a quadro das imagens documentadas, foram realizadas medidas da freqüência fundamental, da amplitude de vibração das pregas vocais, do ciclo vibratório e suas fases (aberta, fechada, de abertura e fechamento) pré e pós aproximação cricotireóidea. Foram também calculados e analisados índices que correlacionam a duração das diferentes fases do ciclo vibratório, como o quociente de abertura, de fechamento e o quociente de velocidade. Estas variáveis foram analisadas estatisticamente pela técnica multivariada de análise de perfil e correlacionadas entre si. Observamos que houve alongamento significante das pregas vocais após a aproximação cricotireóidea, sendo mais evidente no sexo feminino. Ocorreu elevação significante da freqüência fundamental, mas esta não se correlacionou com o alongamento das pregas vocais. A amplitude de vibração diminuiu de forma significante, assim como a duração do ciclo vibratório e suas fases, correlacionando-se diretamente com o aumento da freqüência fundamental. A redução da fase de abertura foi significantemente maior no sexo masculino. Concluindo, a aproximação cricotireóidea provocou alongamento da prega vocal, redução da amplitude e duração do ciclo vibratório e suas fases, existindo correlação inversa com o aumento da freqüência de vibração. Embora as pregas vocais de laringes femininas tenham apresentado maior alongamento, e as masculinas maior redução da fase de abertura, não houve diferença significante do efeito da aproximação cricotireóidea sobre a freqüência vocal entre os sexos. / The cricothyroid approximation simulates the cricothyroid muscle contraction by approximating the cricoid and thyroid cartilage’s. By this surgery, the vocal folds are lengthened. Consequently, the fundamental frequency of the voice is increased. The main indications for this procedure are patients presenting voice hoarseness due to superior laryngeal nerve palsy or to rheumatoid and endocrinological diseases in women. Thirty fresh cadaver’s larynges were used for the purpose of studying the effects of the cricothyroid approximation on the vocal folds vibration in both genders. Ideally, this experiment would allow exclusion of the diverse phonation compensatory factors that can possible compromise the accuracy of an “in vivo" study. The surgical procedure was performed in the excised larynges. Initially, the arytenoids cartilages were approximated by suturing both together, closing the glottis. Humidified compressed air was injected inside of the tracheal tube, causing vocal folds vibration. The cricothyroid approximation was performed by sutures placed in the cricoid and thyroid cartilages. Calipers were used to measure the length of the membranous portion of the vocal folds. Pre and post cricothyroid approximation. Vocal folds vibration was recorded by a portable camera specific for videokymographic studies. After digitalizing the recorded images, each one of them was carefully evaluated. The fundamental voice frequency, the vocal folds vibratory amplitude, and the vibratory cycle with its phases (opened, closed, opening and closing phases) were measured pre and post operatively. Indexes related to the duration of the different phases of the vibratory cycle, such as open quotient, closed quotient and speed quotient were then calculated and analyzed. Multivariate statistical analysis and profile analysis were used for correlation and understanding of the results. There was a statistically significant lengthen of the vocal folds after cricothyroid approximation, more noticeable in female laryngeal specimens. The fundamental voice frequency was increased, but no relation with the increased length of the vocal cords could be observed. The vibratory amplitude was significantly reduced, as was the vibratory cycle duration. The latter was directly correlated with the increase of fundamental voice frequency. The male laryngeal specimens presented a significant higher reduction of the opening phase of the vibratory cycle. In this study, the cricothyroid approximation caused lengthening of the vocal folds. The vibratory cycle and its phases were also altered, presenting a reduced duration. An inverse correlation between these variables and the increase of the vibratory frequency was noted. Although the female laryngeal specimens had shown a longer length when compared with the male, and the male specimens had a higher decrease of the opening phase of the vibratory cycle, there was no statistically significant difference in the cricothyroid approximation effect on the vocal frequency according to gender.

A voz é a principal ferramenta de comunicação da espécie humana e quase 70% da população economicamente ativa dos países desenvolvidos dependem direta ou indiretamente dela em sua profissão. Sua produção deve-se ao funcionamento harmônico de sistemas fisiológicos distintos, nos quais a laringe desempenha um importante papel. É nela que as funções de deglutição, respiração e fonação se encontram e também onde o pulso glotal é formado durante a passagem do ar pelas pregas vocais. Se os nervos e músculos da região por alguma razão são lesionados, o funcionamento dessas funções é prejudicado, causando sérios danos à qualidade de vida do indivíduo. Em virtude disso, diversas pesquisas tem sido realizadas visando adquirir informações que auxiliem as tomadas de decisões clínicas e cirúrgicas. Embora diversos avanços tenham sido realizados no campo de modelagens das pregas vocais e nos estudos da laringe, modelos baseados em geometrias de pacientes específicos que possam colaborar mais ativamente no planejamento cirúrgico, permanecem um desafio. Nesse sentido, este trabalho apresenta o desenvolvimento de um modelo computacional tridimensional, com base em imagens de tomografia computacional. Tendo por objetivo impulsionar a modelagem das características fisiológicas de pacientes reais e assim proporcionar maiores informações para tomadas de decisões. Esse modelo foi utilizado em simulações de escoamento de fluido solucionadas por elementos finitos, apresentando possibilidades satisfatórias de contribuir para avanços na modelagem de pacientes com patologias e em abordagens interativas, tal como interferências nos modelos virtuais por interfaces hápticas e simulações virtuais de cirurgia da laringe. / The voice is the main instrument for communication of human beings and almost 70% of the economically active population in the developed countries depends, directly or indirectly, on it for their profession. Its production is due to the harmonious interaction of different physiological systems, in which the larynx plays an important role. The larynx is involved in the deglutition, breathing and phonation functions and it is where the glottal pulse is formed during airflow through the vocal folds. If the nerves and muscles in this region for some reason are injured, this functions are adversely affected, causing serious damages to the individuals quality of life. As a result, several researches have been carried out, aiming at acquiring information that help in the clinical and surgical decision making. Although many progresses had been reached in the field of vocal folds modeling and in larynx studies, patientspecific geometry modeling that may take an active part in the surgical planning are still a challenge. In this regard, this work presents the development of a threedimensional computational model, based on images from computed tomography (CT) scans. This model was used in fluid flow simulations, solved by finite element analysis, showing satisfactory possibilities for contributions to progresses in the modeling of patients with lesions and in interactive approaches, such as interferences in the models with haptic interface and virtual surgery of the larynx.

Structural evolution of the Warwick Hills, Marathon Basin, West Texas

A detailed structural analysis was conducted of the Warwick Hills at the northeast tip of the doubly-plunging Dagger Flat anticlinorium, Marathon Basin, west Texas. Field work delineated a folded duplex structure composed of three horses. Thrust transport was towards the northwest and resulted in a hinterland-dipping duplex. Initial thrusting In the Warwick Hills shortened the area by 2.2:1 (54%). Post-thrusting, the duplex underwent nearly isoclinal folding creating two anticlines and a syncline, second-order folds to the Dagger Flat anticlinoium. Folding combined with thrusting brought the total shortening of the rock package to 6.5:1 (85%). Earlier estimates gave a shortening for the Warwick Hills of 3:1. Finally, the folded duplex was extended by oblique tear faulting that offset the folded thrusts accommodating extension of the major folds in a northeast direction. These tear faults occurred post-plunging of the folds and were the last deformational movements that affected the Warwick Hills. The Ordovician Maravillas and Devonian Caballos Formations acted in the Warwick Hills as a structurally competent couplet. Addition or subtraction of this couplet, or units in this couplet, controlled the location of the major and minor thrusts, the style and shape of folds, and the location of the fold hinges. Bounding the couplet are incompetent shales of the Ordovician Woods Hollow and the Mississippian Tesnus Formations. Thrusts in the Warwick Hills duplex have a basal décollement in the Woods Hollow shale and ramp up through the Maravillas/Caballos couplet with an upper décollement in the Tesnus shale. The entire duplex was primarily folded by flexural slip (i.e. concentric folds) as evidenced by slickensides oriented parallel to bedding and perpendicular to fold axes, the constant thickness of the competent layers and the change in fold shape with depth. Fold wavelength, as determined from the couplet in the lowest thrust sheet, averages ~1,300 m and the average fold axis for the Warwick Hills, as determined stereographically, plunges ~54° N90°E. Shale in the Woods Hollow and Tesnus Formations bounding the couplet, flowed passively during folding into the cavities that were created by the bending of the more competent units. Lower and upper boundaries of disharmonic folding developed in the Woods Hollow and Tesnus Formations respectively. Unique to this area when compared to the rest of the anticlinorium are the presence of tightly folded thrusts and steep east-trending fold axes. The anticlinorium plunges in the Warwick Hills because it drapes off a down-to-the-northeast basement fault. Folds were "dragged" or diverted to the east during thrusting of the duplex over this transversely-oriented paleotopographic fault scarp, or were diverted subsequent to thrusting of the duplex by strike-slip movements at depth along the basement fault. / text

An?lise estrutural da deforma??o cenoz?ica na Bacia de Cumuruxatiba (BA)

This basin was formed in distensional context, with rifting and subsequent thermal phase during Neocomian to late Cretaceous. At Cenozoic ages, the Abrolhos magmatism occurs in the basin with peaks during the Paleocene and Eocene. In this period, there was a kinematic inversion in the basin represented by folds related to reverse faults. Structural restoration of regional 2D seismic sections revealed that most of the deformation was concentrated at the beginning of the Cenozoic time with the peak at the Lower Eocene. The post-Eocene is marked by a decrease of strain rate to the present. The 3D structural modeling revealed a fold belt (trending EW to NE-SW) accommodating the deformation between the Royal Charlotte and Sulphur Minerva volcanic highs. The volcanic eruptions have caused a differential overburden on the borders of the basin. This acted as the trigger for halokinesis, as demonstrated by physical modeling in literature. Consequently, the deformation tends to be higher in the edges of the basin. The volcanic rocks occur mainly as concordant structures (sills) in the syn-tectonic sediment deposition showing a concomitant deformation. The isopach maps and diagrams of axis orientation of deformation revealed that most of the folds were activated and reactivated at different times during the Cenozoic. The folds exhibit diverse kinematic patterns over time as response to behavior of adjacent volcanic highs. These interpretations allied with information on the petroleum system of the basin are important in mapping the prospects for hydrocarbons / A Bacia de Cumuruxatiba est? localizada no extremo sul da costa do Estado da Bahia, no Nordeste do Brasil. Esta bacia foi formada em ambiente distensional, com rifteamento e posterior fase termal durante o Neocomiano at? final do Cret?ceo. Durante o Cenoz?ico ocorre o magmatismo de Abrolhos na bacia com picos durante o Paleoceno e Eoceno. Neste per?odo ocorre uma invers?o cinem?tica na bacia representada por dobras relacionadas a falhas reversas. Restaura??es estruturais regionais de se??es s?smicas 2D, revelaram que a maior parte da deforma??o est? concentrada no inicio do Cenoz?ico com o pico no Eoceno Inferior. O per?odo p?s-Eoceno ? marcado pela diminui??o da taxa de deforma??o at? o presente. A modelagem estrutural 3D revelou uma frente de dobras (de orienta??o E-W variando para NE-SW) acomodando a deforma??o entre os altos vulc?nicos de Royal Charlotte e Sulphur Minerva. Os derrames vulc?nicos causaram uma sobrecarga diferencial nas bordas da bacia que serviu de gatilho para atua??o da halocinese, como j? demonstrado em modelagens f?sicas na literatura. A disfunção das pregas vocais (DPV) é caracterizada por movimento paradoxal de fechamento das pregas vocais, ocorrendo de forma episódica e involuntária durante a inspiração, levando à obstrução das vias aéreas. Os pacientes que apresentam DPV costumam não responder ao tratamento indicado para asma, levando ao uso inapropriado de corticóides sistêmicos com efeitos colaterais, freqüentes passagens em setores de emergência, hospitalizações por vezes desnecessárias e, com menos freqüência, intubação e traqueostomia. O objetivo principal deste estudo foi o de investigar a relação desta problemática com a economia psíquica destes pacientes, no que diz respeito à psicogênese e as possibilidades de tratamento por meio do método clínico de investigação e tratamento psicanalíticos. Para isto, foram incluídos 5 pacientes do sexo feminino, com idades entre 30 e 55 anos, que foram acompanhadas em tratamento psicanalítico no ambulatório do Serviço de Imunologia Clínica e Alergia do Instituto Central do HCFMUSP. Constatamos que a DPV aparece de maneira singular na vida de cada paciente investigada, porém com características peculiares que indicam uma localização psíquica capaz de operar como uma satisfação inconsciente frente a um conflito anterior. A princípio, abordar a paciente como se esta não possuísse asma pode levar a uma reação negativa, interpretada por elas como um fingimento ou uma estratégia consciente de obter ganhos primários. A interlocução entre o psicanalista e os demais profissionais da equipe mostra-se de significativa importância / Vocal fold dysfunction (VFD) is characterized by paradoxical movement of closing of the vocal folds, occurring of irregularly and involuntary form during the inspiration, leading to the blockage of the aerial ways. The patients who present VFD do not answer to the treatment indicated for asthma, leading to the unnecessary use of systemic steroids with collateral effect, frequent visits in emergency sectors, hospitalizations and, sometimes, intubations and tracheotomies. The main objective of this study was to investigate the relation of this problematic and the psychic economy of these patients, in whom it says respect to psychogenic explanation and the possibilities of psychoanalytical treatment. For this, 5 patients of the feminine sex had been enclosed, with ages between 30 and 55 years, that had been followed in psychoanalytical treatment in the clinic of the Service of Clinical Imunologia and Allergy of the Central Institute of the HCFMUSP. We evidence that the VFD appears in singular way in the life of each investigated patient, however with peculiar characteristics that indicate a psychic localization capable to operate as an unconscious satisfaction to a previous conflict. To approach the patient as if this did not possess asthma can lead to a negative reaction, interpreted for them as a deceit or a conscientious strategy to get primary profits. Este trabalho propõe direcionar o estudo de involuções para dois ramos de pesquisa dentro da teoria das Singularidades e de Sistemas Dinâmicos. Mais precisamente, tratamos sua interligação com diagramas divergentes de dobras e seu aparecimento nos sistemas dinâmicos discretos reversíveis. No primeiro, tratamos da importante relação entre a classificação de diagramas divergentes de dobras, digamos de s dobras, e a classificação de s-uplas de involuções associadas a estes diagramas. No segundo contexto, o estudo se volta para a questão sobre condições para a linearização simultânea de uma classe de pares de involuções e a obtenção de formas normais desses pares / This work proposes to address the study of involutions for two branches of research into the theory of Singularities and Dynamical Systems. More precisely, we treat its interconnection with divergent diagrams of folds and their appearance in discrete reversible dynamical systems. First, we treat the important relationship between the classification of divergent diagrams of folds, say s folds, and the classification of s-tuples of involutions associated with these diagrams. In the second context, the study turns to the question of conditions for simultaneous linearization of a class of pairs of involutions and the deduction of the normal forms of these pairs

La collision entre les plis vocaux lors de la phonation, phénomène physiologique essentiel pour la synchronisation de la vibration des plis, est également une composante du phonotraumatisme. Le rôle du phonotraumatisme dans la physiopathologie des dysphonies est communément admis mais sa nature reste mal définie et sa quantification difficile. Ce travail propose d’aborder cette question au moyen d’approches in vivo sur des sujets sains et ex vivo sur des larynx humains excisés. L’objet de ce travail est l’étude du comportement laryngé lorsque le larynx est soumis à des conditions de pression sous-glottique extrêmement élevées. In vivo, l’étude de la voix criée reposait sur la mesure constante de la pression sous-glottique. Ex vivo, cette dernière était manipulée en dehors de toute modification des autres paramètres de contrôle. Le volet ex vivo a nécessité un développement méthodologique (modernisation et adaptation au larynx humain) du banc expérimental utilisé pour des larynx animaux. Les résultats ont montré l’existence chez les locuteurs sains d’un « plafonnement » dans l’intensité vocale et le signal électroglottographique pour les très hauts niveaux de pression sous-glottique. Les études sur larynx humains excisés mesuraient la force de collision entre les plis vocaux sous l’effet de la pression sous-glottique et ont mis en évidence l’existence, ici aussi, d’un « plafonnement » de cette force lorsque la pression sous-glottique est très élevée. Les observations rapportées par ce travail ont un intérêt clinique important même si les phénomènes physiques sous-jacents restent à préciser. / The collision between the vocal folds during phonation is a physiological phenomenon with a powerful role in the synchronization of their vibration. This collision is also a component of the phonotrauma. The role of phonotrauma in the pathophysiology of dysphonia is commonly accepted but its nature still unclear and its quantification remains difficult. This work proposes, an in vivo study in healthy subjects, and an ex vivo one on excised human larynges. The purpose of this work was to study of the laryngeal behavior when the larynx is subjected to extremely high subglottic pressure. In vivo, it is studying the shouted voice with constant measure of subglottal pressure. Ex vivo, the subglottic pressure is directly manipulated. The ex vivo study required a methodological development to modernize the experimental bench used for animal larynges studies and to adapt it to the human larynx. The results showed, in healthy speakers the existence of a "plateau" in the vocal intensity and in the electroglottographic signal for very high levels of subglottic pressure. The excised larynx studies measured the collision force between the vocal folds and its changes depending on other acoustic and electroglottographic parameters. They highlighted the existence, here too, of a "plateau" in the level of the collision force when the subglottic pressure was very high. The observations presented in this work are, to our knowledge, unprecedented. They have important clinical interest for understanding the concept of phonotrauma. Their interpretation in physical terms remains very incomplete.

[pt] O célebre teorema de Ambrosetti-Prodi estuda perturbações do Laplaciano sob condições de Dirichlet por funções não lineares que saltam sobre o autovalor principal do operador. Diversas extensões desse resultado foram obtidos para operadores auto-adjuntos, em particular por Berger-Podolak em 1975, que deram uma descrição geométrica do conjunto solução. Nós empregamos técnicas baseadas no princípio do máximo que nos permite obter novos resultados inclusive para o cenário auto-adjunto. Em particular, nós mostramos que o operador semi-linear é uma dobra global. Obtemos também uma contagem exata de soluções para esses operadores ainda quando a perturbação não é suave. / [en] The celebrated Ambrosetti-Prodi theorem studies perturbations of the Dirichlet Laplacian by a nonlinear function jumping over the principal eigenvalue of the operator. Various extensions of this landmark result were obtained for self-adjoint operators, in particular by Berger-Podolak in 1975, who gave a geometrical description of the solution set. In this thesis we show that similar theorems are valid for non self-adjoint operators. We employ techniques based on the maximum principle, which even let us obtain new results in the self-adjoint setting. In particular, we show that the semilinear operator is a fold. As a consequence, we obtain exact count of solutions for these operators even when the perturbation is non-smooth.
