Bakgrund: Att vårda barn perioperativt ställer höga krav på personalens kompetens. Barn skall inom vården inte betraktas som små vuxna. Anestesisjuksköterskan behöver ha särskilda kunskaper om andning, cirkulation och farmakokinetik vid olika åldrar. Syfte: Undersöka vilka faktorer anestesisjuksköterskan betraktar som mest utmanande inför generell anestesi på mindre barn på en allmän operationsavdelning, samt undersöka sambandet mellan de betraktade utmaningarna och bakgrundsfaktorer. Metod: Kvantitativ tvärsnittsstudie med enkäter som distribuerades till anestesisjuksköterskor. Deskriptiv och statistisk analys har genomförts. Resultat: 97% av respondenterna känner någon grad av oro i samband med generell anestesi på barn. De faktorer som upplevs som mest utmanande är laryngospasm och hypoxi. Kvinnor upplever oro i högre utsträckning än män, yngre upplever mer oro än äldre och de med kortare arbetserfarenhet upplever oro i större utsträckning än dem med längre arbetserfarenhet. Konklusion: Laryngospasm och hypoxi är allvarliga tillstånd som kan förekomma vid generell anestesi på barn. Den unga och/eller oerfarne ser detta som mer utmanande än den äldre och/eller erfarne. Det är av betydelse att den erfarne medarbetaren sprider sin kunskap genom teamarbete för att den oerfarne kollegan ska kunna ge en god och säker vård / Background: Caring for children perioperatively places high demands on the staff's competence. Children should not be regarded as small adults. The nurse anesthetist needs special knowledge of breathing, circulation and pharmacokinetics at different ages. The aim: Investigate which factors the nurse anesthetist considers most challenging before general anesthesia in smaller children in a general operating department and investigate the connection between the consider challenger and background factors. Method: Quantitative cross-sectional study with questionnaires that was distributed to nurse anesthetists. Descriptive and statistical analysis has been carried out. Results: 97% of the respondents feel some degree of anxiety in connection with general anesthesia in children. The factors perceived as most challenging are laryngospasm and hypoxia. Women experience anxiety to a greater extent than men, younger experience more anxiety than elder and those with shorter work experience feel anxiety to a greater extent than those with longer experience. Conclusion: Laryngospasm and hypoxia are serious conditions that can occur during general anesthesia in children. The young and/ or inexperienced see this as more challenging than the older and/ or experienced. It is important that the experienced employee spreads their knowledge through teamwork so that the inexperienced colleague can provide good and safe care.

Perioperativa åtgärder för att lindra och förebygga postoperativ halsont och heshet hos patienter som genomgått generell anestesi

Bakgrund: Halsont och heshet är några av de vanligaste postoperativa komplikationerna hos patienter som erhållit endotrakealtub eller larynxmask under generell anestesi. Syfte: Att kartlägga anestesisjuksköterskans perioperativa åtgärder för att förebygga och lindra patientens postoperativa halsont och heshet. Metod: En litteraturöversikt med deskriptiv design. Resultat sammanställdes utifrån 18 granskade artiklar av kvantitativ ansats, eftersökta i databaserna PudMed och Cinahl. Resultat: Litteraturöversikten visade flertalet åtgärder som kunde minska både incidens och svårighetsgrad av postoperativ halsont och heshet. En mindre endotrakealtub, användning av kontinuerlig tryckregulator, administrering av glukokortikoider och användning av larynxmask gav minskad incidens av halsont och heshet. Även videolaryngoskop, minskad kraft vid extraktion av ledare och vissa naturläkemedel visade sig minska incidensen. Åtgärderna visade på goda effekter i nära anslutning till operationen och inom första postoperativa dygnet, emellertid gav få artiklar resultat med bibehållen effekt över tid. Slutsats: Studien gav goda indikationer för åtgärder som kunde minska patienters förekomst av halsont och heshet och därigenom deras lidande. Resultatet visade på stor spridning vilket gjorde det svårt bedöma vilka åtgärder som var lämpliga att applicera i kliniken. Att minska storleken på endotrakealtuben och använda larynxmask när operationen tillåter var åtgärder som ansågs användbara. Även användning av glukokortikoider/antiinflammatoriska läkemedel samt att i större utsträckning utnyttja videolaryngoskopstekniken ansågs som adekvata åtgärder för att minska patientens lidande orsakat av postoperativ halsont och heshet. / Background: Sore throat and hoarseness are some of the most common postoperative complications in patients receiving endotracheal tube or laryngeal mask during general anesthesia. Aim: To investigate the anesthetic nurse's perioperative measures to prevent and alleviate the patient's suffering from postoperative sore throat and hoarseness. Method: A literature review with descriptive design. Results were compiled based on 18 articles of quantitative approaches, requested in the databases PudMed and Cinahl. Result: The literature review showed several measures that could reduce both incidence and severity of postoperative sore throat and hoarseness. A minor endotracheal tube, the use of continuous cuff pressure regulator, administration of glucocorticoids and use of laryngeal mask reduced the incidence of sore throat and hoarseness. Also, video laryngoscopes, reduced force when extracting stylet removal and some herbal medications seemed to give good results in reducing the incidence. The measures showed good effects close to surgery and within the first postoperative day, however, few studies gave results with retained effect over time. Conclusions: The study provided good indications for measures that could reduce patients' incidence of sore throat and hoarseness and thereby their suffering. The results showed a large spread, which made it difficult to assess which measures were suitable for application in the clinic. Reducing the size of the endotracheal tube and using the laryngeal mask when the operation allows was implications that considered useful. Also, the use of glucocorticoids / anti-inflammatory drugs and to a greater extent utilizing the video laryngoscope technique was considered adequate measures to reduce the suffering of patients caused by postoperative sore throat and hoarseness.

Introdução. A morbi-mortalidade durante e após anestesia para aneurismectomia de aorta abdominal é alta, pois esta doença acomete pacientes após a sétima década de vida e que possuem várias doenças concomitantes. Objetivos. Analisar e discutir as condutas anestésicas utilizadas nos períodos pré e intra-operatório no Serviço de Anestesiologia do HCFMRP-USP. Casuística e Método. Foram analisados os prontuários de 104 pacientes submetidos à aneurismectomia de aorta no tocante às condutas utilizadas pelos anestesiologistas para a condução destes casos. Resultados. Apenas um paciente possuía menos de 40 anos de idade, 76,80% estavam na sétima ou oitava década de vida e 88,46% eram do sexo masculino. A hipertensão arterial acometeu 70,19% dos pacientes e 26,92% possuíam coronariopatia. Pacientes com obesidade foram a minoria (26,92%). O ecocardiograma pré-operatório demonstrou que a grande maioria dos pacientes apresentava função ventricular normal. A cirurgia foi realizada em regime de urgência em 7,69% dos casos. A anestesia geral exclusiva foi realizada em 17 pacientes e associada com a peridural em 57 pacientes, com a raquianestesia em 11 e com a raqui-peri combinadas em 19. O tempo cirúrgico variou de 120 a 510 minutos enquanto que o tempo de clampeamento aórtico variou de 30 a 165 minutos. Houve um óbito no período intra-operatório e a causa foi choque hipovolêmico e 10 óbitos até o vigésimo dia pós-operatório. Sessenta e seis pacientes receberam concentrado de papa de hemácias durante o período intra-operatório, mas só em 43,27% desses casos a indicação esteve suportada por exame laboratorial. Oitenta pacientes foram extubados ainda na sala de cirurgia, enquanto que os demais (23) permaneceram intubados no período pós-operatório e 19 necessitaram de suporte ventilatório que teve tempo que variou de 3 a 96 horas com média de 42,31 horas. Apenas quatro pacientes fizeram pós-operatório imediato no Centro de Terapia Intensiva enquanto que os demais permaneceram na Sala de Recuperação Pós-Anestésica. Conclusão. Não existe um protocolo único para a realização de anestesia para aneurismectomia de aorta no HCFMRP-USP e a técnica anestésica utilizada não influenciou o morbi-mortalidade. / Introduction. The morbidity and mortality during and after anesthesia for aneurysmectomy of the abdominal aorta are high since this disease affects patients after the seventh decade of life who have several concomitant diseases. Objectives. To analyze and discuss the anesthetic conducts used during the preoperative and intra-operative periods at the Service of Anesthesiology of HCFMRP-USP. Cases and Method. The medical records of 104 patients submitted to aneurysmectomy of the aorta were analyzed regarding the conducts used by the anesthesiologists for the management of these cases. Results. Only one patient was less than 40 years old, 76.80% were in he seventh or eighth decade of life, and 88.46% were male. Arterial hypertension was present in 70.19% of the patients and 26.92% had coronary artery disease. Obese patients were a minority (26.92%). The preoperative echocardiogram demonstrated that most patients had normal ventricular function. Surgery was performed on an emergency basis in 7.69% of cases. Seventeen patients received exclusive general anesthesia, while general anesthesia was associated with peridural anesthesia in 57, with rachi-anesthesia in 11 and with combined rachi-peridural anesthesia in 19. Surgical time ranged from 120 to 510 minutes and time of aortic clamping ranged from 30 to 165 minutes. One death occurred intra-operatively due to hypovolemic shock and 10 patients died up to the 20th postoperative day. Sixty-six patients received a red blood cell concentrate intra-operatively, but this indication was supported by a laboratory exam in only 43.27% of these cases. Eighty patients were extubated while still in the operating room while the remaining 23 continued to be intubated during the postoperative period and 19 required ventilatory support lasting 3 to 96 hours (mean duration: 42.31 hours). Only four patients spent the immediate postoperative period in the Intensive Care Unit, while the remaining ones stayed in the Post-Anesthesia Recovery Room. Conclusion. There is no single protocol for the application of anesthesia for aneurysmectomy of the aorta at HCFMRP-USP and the anesthetic technique used did not influence morbidity-mortality.

Pacientes com câncer e idade menor ou igual a 21 anos, em tratamento no Serviço de Oncologia Pediátrica do Hospital de Clínicas de Porto Alegre, submetidos a procedimentos de curta duração, diagnósticos e/ou terapêuticos, sob anestesia geral, no Centro Cirúrgico Ambulatorial. Resultados: Cento e trinta e sete pacientes foram submetidos a 423 procedimentos sob anestesia geral. Eram do sexo masculino 61%, com média de idade de 7,5 anos (0,2 a 21) e ASA II 98%. Oitenta e sete por cento dos procedimentos foram realizados em pacientes com leucemia ou linfoma. A maioria dos procedimentos não determinou eventos adversos durante os períodos intra-operatório e pós-operatório. Nenhum procedimento foi suspenso após iniciado.
