Development, Characterization and Validation of Trastuzumab-Modified Gold Nanoparticles for Molecularly Targeted Radiosensitization of Breast Cance

The overexpression of the human epidermal growth factor receptor-2 (HER-2) in 20-25% of human breast cancers was investigated as a target for development of a gold nanoparticle (AuNP) based radiosensitizer for improving the efficacy of neoadjuvant X-radiation therapy of the disease. HER-2 targeted AuNPs were developed by covalently conjugating trastuzumab, a Health Canada approved monoclonal antibody for the treatment of HER-2-overexpressing breast cancer, to 30 nm AuNPs. Trastuzumab conjugated AuNPs were efficiently internalized by HER-2-overexpressing breast cancer cells (as assessed by darkfield microscopy and transmission electron microscopy) and increased DNA damage from X-radiation in these cells by more than 5-fold. To optimize delivery of AuNPs to HER-2-overexpressing tumors, high resolution microSPECT/CT imaging was used to track the in vivo fate of 111In-labelled non-targeted and HER-2 targeted AuNPs following intravenous (i.v.) or intratumoral (i.t.) injection. For i.v. injection, the effects of GdCl3 (for deactivation of macrophages) and non-specific (anti-CD20) antibody rituximab (for blocking of Fc mediated liver and spleen uptake) were studied. It was found that HER-2 targeting via attachment of trastuzumab paradoxically decreased tumor uptake as a result of faster elimination of the targeted AuNPs from the blood while improving internalization in HER-2-positive tumor cells as compared to non-targeted AuNPs. This phenomenon could be attributed to Fc-mediated recognition and subsequent sequestration of trastuzumab conjugated AuNP by the reticuloendothelial system (RES). Blocking of the RES did not increase tumor uptake of either HER-2 targeted or non-targeted AuNPs. Following i.t. injection, our results suggest that Au-NTs redistribute over time and traffick to the liver via the ipsilateral axillary lymph node leading to comparable exposure as seen with i.v. administration. In contrast, targeted AuNPs are bound and internalized by HER-2-overexpressing tumor cells following i.t. injection, with a lower proportion of AuNPs redistributing to normal tissues. In vivo, the combination of HER-2 targeted AuNPs injected i.t. and X-radiation (11 Gy) yielded a 46% decrease in tumor size over a 4 month period in contrast to an 11.5% increase in tumor size for X-radiation treatment alone. Toxicology studies (evaluated through complete blood cell counts, by serum transaminase and creatinine measurements and by monitoring the body weight) demonstrated no apparent normal organ toxicity from the combination of HER-2 targeted AuNPs and X-radiation. These results are promising for the clinical translation of HER-2-targeted AuNPs for radiosensitization of tumors to X-radiation.

La performance diagnostique des marqueurs tumoraux messagers dans le diagnostic et le suivi du cancer du sein

Selon plusieurs évidences, la présence de cellules tumorales occultes dans la circulation sanguine aux premières étapes du cancer du sein pourrait être à l’origine des lésions métastasiques. Plusieurs études de recherche ont montré que l’utilisation de la RT-PCR en temps réel pour la détection des cellules tumorales circulantes CTC offre la meilleure sensibilité dans la quantification des marqueurs tumoraux. Présentement de routine, le suivi du cancer du sein est réalisé par le dosage immunologique des marqueurs sériques CA15-3 et CEA. Cependant, la faible sensibilité de ces marqueurs aux stades précoces de la maladie et leur manque de spécificité tissulaire ne permet pas leur utilisation pour le diagnostic et le pronostic du cancer du sein. Le diagnostic de la maladie est plutôt basé sur l’analyse d’une biopsie de la tumeur ou des ganglions lymphatiques, des méthodes invasives, coûteuses et peu adaptées pour un suivi de routine dans l’évaluation du risque de rechute et de la réponse au traitement. Malgré les études, la détection de ces cellules dans les laboratoires hospitaliers est rare. Nous avons envisagé de mettre en place un nouveau test RT-PCR pour la détection de cellules malignes du cancer du sein dans la circulation. La spécificité et la sensibilité de plusieurs marqueurs potentiels ont été comparées. Le but ultime de ce projet est d’offrir la détection d’un ou d’une combinaison de ces marqueurs de routine aux patientes. Nos résultats montrent une corrélation positive entre l’expression des ARNm des marqueurs CK19 et de HER2 avec les données cliniques des patientes. De plus, la sensibilité et la spécificité des tests RT-PCR sont comparables à la littérature récente. Finalement, la comparaison de notre test avec le dosage immunologique des marqueurs tumoraux sériques CA15.3 et CEA a montré que la détection de la CK19 et de HER2 par RT-PCR est plus sensible chez les patientes de cancer du sein métastatique. / According to several evidences, the presence of occult tumor cells in blood circulation in the early stages of breast cancer could be the origin of metastatic lesions. Recent research studies have shown that the use of qRT-PCR for the detection of circulating tumor cells CTC offers the best sensitivity in the quantification of tumor markers. Currently routine monitoring of breast cancer is performed by immunoassay of serum markers CA15.3 and CEA. However, the low sensitivity of these markers in the early stages of the disease and the lack of tissue specificity does not allow their use in diagnosis and prognosis of breast cancer. The diagnosis of the disease is rather based on the analysis of biopsies of the tumor or lymph nodes. This is an invasive procedure, expensive and not suitable for routine monitoring of the risk of relapse and response to treatment. Despite these studies, detection of these cells in hospital laboratories is rare. Our objective is to set up a new RT-PCR assay for the detection of malignant breast cancer cells in the circulation. The specificity and sensitivity of some potential markers will be compared. The ultimate goal of this project is to routinely offer the detection of one or of a combination of these markers routinely to patients with breast cancer. Our results show a positive correlation between the expression of CK19 and HER2 mRNA with clinical data of patients. Furthermore, the sensitivity and specificity of our RT-PCR tests are similar to the recent literature. Finally, the comparison of our test with the immunoassay of serum tumor markers CEA and CA15.3 showed that the detection of HER2 and CK19 by RT-PCR is more sensitive in patients with metastatic breast cancer

Recently HER3, member of the epidermal growth factor receptor family (EGFR), has been found to play a crucial role in the development of resistance towards inhibitors that are given to patients with HER1- and HER2-driven cancers. As HER3 is up-regulated or over-activated in several types of human cancers, it is of outmost importance that new innovative drugs target its oncologic activity. The Affibody® Molecule Z08698 inhibits the heregulin induced signalling of HER3 with high affinity (KD~50 pM). As the Affibody® Molecule is small, has high solubility and outstanding folding kinetics, an effective penetration of tumour tissue is suggested together with a rationalized manufacturing process. Further coupling to an albumin binding domain (ABD) expands the plasma half-life of the molecule, hence increasing the molecule's potential of serving as a therapeutic. A process development for production of Z08698-VDGS-ABD094 has been established, where the molecule is efficiently produced in the E. coli host strain BL21(DE3), through a T7 based expression system. Cultivations were performed with a fed-batch fermentation process and the conditions were further optimized in order to obtain highest expression, while avoiding undesirable modifications like gluconoylations. By employing Design of experiments in combination with multivariate data analysis, a production process resulting in ~3.5 g product/ l culture could be verified. Moreover, thermolysis was evaluated as a suitable method for cell disruption, enabling an easy and cost-effective manufacturing process of the ABD fused Affibody® Molecule.

Analysis of HER2 testing in breast cancer: disparities, cost-effectiveness, and patterns of care

HER2 breast cancer is an aggressive disease that occurs in 20 - 30% of the breast cancer population. Treatment for HER2 breast cancer includes use of an anti-HER2 monoclonal antibody, trastuzumab. Testing for HER2 is of critical importance due to the adverse side effects and substantial costs associated with this anti-HER2 treatment. Currently, two kinds of tests, Fluorescence In Situ Hybridization (FISH) and Immunohistochemistry (IHC), are FDA approved for determination of HER2 status in breast cancers. Clinical and non clinical factors that affect the choice HER2 test and the use of anti-HER2 therapy in breast cancer were analyzed using a data set containing information from six outpatient oncology clinics in the United States. The analysis showed that geographic location, cancer stage, and diagnosis date (pre- or post-publication of testing guidelines) have significant effects on choice of test. With regard to trastuzumab prescription, geographic location and HER2 status have significant effects on the prescription of trastuzumab. In addition, there was a non-significant trend for certain Medicare patients not to receive trastuzumab therapy. These findings indicate that disparities are present in breast cancer care based on geography and cancer stage, and highlight the importance of testing guidelines. The cost effectiveness of FISH vs. IHC was determined, by considering the financial and health-related costs associated with testing and subsequent treatment as well as the accuracy of each test. The results show that FISH is the optimal choice for HER2 testing and is more cost-effective than IHC.

O receptor epidérmico humano 2 (Her-2) e a topoisomerase-IIα (T2A) são dois marcadores biológicos importantes, ambos tendo um valor prognóstico e preditivo potencial em pacientes com tumores sólidos. A amplificação dos genes Her- 2 e T2A são eventos independentes, embora o último seja mais frequente em tumores com amplificação do Her-2 (34-90%), do que em tumores sem amplificação do Her-2 (5-10%). Existe uma melhor correlação entre amplificação e superexpressão do Her-2 no câncer de mama (CM) do que em outros tumores. No entanto, no CM, a correlação entre amplificação e superexpressão da T2A tem sido inconsistente, e existe uma carência de tais dados em outros tipos de tumores. A expressão da proteína T2A tem mostrado uma boa correlação com o índice de proliferação tumoral, particularmente no CM. Objetivos. Artigo 1: Sintetizar o conhecimento atual sobre a importância dos marcadores Her-2 e T2A nos tumores sólidos. Artigo 2: Investigar a prevalência de amplificação e superexpressão do Her-2 e da T2A, a correlação entre estas variáveis e a correlação entre as variáveis e estágio clínico, em amostras de câncer de ovário (CO) fixadas em parafina. Artigo 3: Investigar a prevalência de amplificação da T2A, assim como a correlação entre esta variável e a expressão da proteína T2A e do marcador de proliferação celular Ki-67, em amostras de CM fixadas em parafina, mostrando uma amplificação do Her-2. Métodos. Artigo 1: Os dados foram identificados através de busca em bases de dados eletrônicas (medline), livros de resumos de congressos e referências de artigos de revisão e originais. Artigo 2: 73 amostras de CO foram testadas para amplificação e superexpressão do Her-2 e T2A, por hibridização in situ fluorescente (FISH) e imuno-histoquímica (IHC), respectivamente. Artigo 3: 103 amostras de CM, com amplificação do Her-2, foram testadas para amplificação do gene T2A (por FISH) e superexpressão das proteínas T2A e Ki-67 (por IHC). Resultados. Artigo 2: Com base nos pontos de corte >1.5 e >2 (relação cópias/CEP17), as taxas de amplificação do Her-2 foram 15/64(23.4%) e 8/64(12.5%), versus 16/64(25%) e 5/64(7.8 %) para a T2A. Encontramos somente 3/72(4.2%) casos de superexpressão do Her-2(3+), contra 15/70(21.4%) para a T2A (marcagem em >10% das células). Foi observada uma modesta correlação entre amplificação e superexpressão da T2A (p= 0.01) e uma forte correlação entre amplificação da T2A e do Her-2, quando analisados como variáveis contínuas (p<0.001). A amplificação da T2A correlacionou-se com estágio FIGO avançado (p= 0.02). Artigo 3: Uma amplificação do gene T2A foi observada em 36.9%(38/103) dos casos. Os níveis de amplificação do Her-2 (número de cópias) não se correlacionaram com a amplificação da T2A. There is a better correlation between Her-2 amplification and protein overexpression in breast cancer (BC) than in other tumour types. Nevertheless, there is a doubtful correlation between T2A amplification and overexpression in BC, with virtually no data available in other tumour types. In BC, the expression of the T2A protein has shown a good correlation with tumour proliferation rate. Objectives. Article 1: To summarise the available literature on Her-2 and T2A in solid tumours. Article 2: To investigate the prevalence of Her-2 and T2A amplification and overexpression, the correlation between these variables and with clinical stage, in paraffin-embedded samples of ovarian cancer (OC). Article 3: To investigate the prevalence of T2A amplification, as well as the correlation between this variable and the expression of T2A protein and the proliferation marker Ki-67, in paraffinembedded samples of Her-2 amplified BC. Methods. Article 1: The data were identified through search in electronic databases (medline), abstract books and references from review and original articles. Article 2: 73 samples of OC were tested for Her-2 and T2A amplification and overexpression, by fluorescence in situ hybridisation (FISH) and immunohistochemistry (IHC), respectively. Article 3: 103 samples of Her-2 amplified BC were tested for T2A amplification (by FISH) and overexpression (by IHC), and Ki-67 expression (by IHC). Results. Article 2: Based on cut-offs of ≥1.5 and ≥2 (ratio copies/CEP17), amplification rates for Her-2 were 15/64(23.4%) and 8/64(12.5%) versus 16/64(25%) and 5/64(7.8%) for T2A. We found only 3/72(4.2%) cases of Her-2 overexpression(3+) versus 15/70(21.4%) for T2A (staining in >10% of the cells). There was a modest correlation between T2A amplification and overexpression (p=0.01) and a strong correlation between T2A and Her-2 amplification when these markers were analysed as continuous variables (p<0.001). T2A amplification significantly correlated with advanced FIGO stage (p=0.02). Article 3: T2A gene amplification was observed in 36.9%(38/103) of the Her-2 amplified samples. Her-2 amplification level (i.e. copy number) was not predictive of T2A amplification. The median percentage of T2A positive cells for T2A non-amplified and amplified cases were 5% and 10%, respectively. A weak but still significant correlation was observed between T2A gene amplification level and percentage of positively stained cells (Spearman=0.23, p=0.02), the observed correlation being higher in patients with positive staining for Ki-67. Conclusions. Article 2: The assessment of Her-2 and T2A amplification and overexpression by FISH and IHC, respectively, is feasible in OC samples. There was a good correlation between Her-2 and T2A gene amplification, but the correlation between gene amplification and protein overexpression was poor for both markers. Amplification rates were higher in the absence of correction for the number of copies of the CEP17. Finally, we found a good correlation between T2A amplification and advanced disease stage. Further studies should aim to determine the optimal cut-offs for these markers. Article 3: Contrary to Her-2, T2A gene amplification does not always lead to protein overexpression in BC. Other factors, especially tumour proliferation rate, may interfere with the T2A protein status. Although the majority of the cases of T2A gene aberrations are seen in Her-2 positive tumours, the level of Her-2 amplification does not predict for T2A amplification.

Her-2 and T2A gene amplification are separate events, although the latter is more frequently seen in Her-2 amplified (34-90%) than in Her-2 non-amplified (5-10%) tumours. There is a better correlation between Her-2 amplification and protein overexpression in breast cancer (BC) than in other tumour types. Nevertheless, there is a doubtful correlation between T2A amplification and overexpression in BC, with virtually no data available in other tumour types. In BC, the expression of the T2A protein has shown a good correlation with tumour proliferation rate. Objectives. Article 1: To summarise the available literature on Her-2 and T2A in solid tumours. Article 2: To investigate the prevalence of Her-2 and T2A amplification and overexpression, the correlation between these variables and with clinical stage, in paraffin-embedded samples of ovarian cancer (OC). Article 3: To investigate the prevalence of T2A amplification, as well as the correlation between this variable and the expression of T2A protein and the proliferation marker Ki-67, in paraffinembedded samples of Her-2 amplified BC. Methods. Article 1: The data were identified through search in electronic databases (medline), abstract books and references from review and original articles. Article 2: 73 samples of OC were tested for Her-2 and T2A amplification and overexpression, by fluorescence in situ hybridisation (FISH) and immunohistochemistry (IHC), respectively. Article 3: 103 samples of Her-2 amplified BC were tested for T2A amplification (by FISH) and overexpression (by IHC), and Ki-67 expression (by IHC). Results. Article 2: Based on cut-offs of ≥1.5 and ≥2 (ratio copies/CEP17), amplification rates for Her-2 were 15/64(23.4%) and 8/64(12.5%) versus 16/64(25%) and 5/64(7.8%) for T2A. We found only 3/72(4.2%) cases of Her-2 overexpression(3+) versus 15/70(21.4%) for T2A (staining in >10% of the cells). There was a modest correlation between T2A amplification and overexpression (p=0.01) and a strong correlation between T2A and Her-2 amplification when these markers were analysed as continuous variables (p<0.001). T2A amplification significantly correlated with advanced FIGO stage (p=0.02). Article 3: T2A gene amplification was observed in 36.9%(38/103) of the Her-2 amplified samples. Her-2 amplification level (i.e. copy number) was not predictive of T2A amplification. The median percentage of T2A positive cells for T2A non-amplified and amplified cases were 5% and 10%, respectively. A weak but still significant correlation was observed between T2A gene amplification level and percentage of positively stained cells (Spearman=0.23, p=0.02), the observed correlation being higher in patients with positive staining for Ki-67. Conclusions. Article 2: The assessment of Her-2 and T2A amplification and overexpression by FISH and IHC, respectively, is feasible in OC samples. There was a good correlation between Her-2 and T2A gene amplification, but the correlation between gene amplification and protein overexpression was poor for both markers. Amplification rates were higher in the absence of correction for the number of copies of the CEP17. Finally, we found a good correlation between T2A amplification and advanced disease stage. Further studies should aim to determine the optimal cut-offs for these markers. Article 3: Contrary to Her-2, T2A gene amplification does not always lead to protein overexpression in BC. Other factors, especially tumour proliferation rate, may interfere with the T2A protein status. Although the majority of the cases of T2A gene aberrations are seen in Her-2 positive tumours, the level of Her-2 amplification does not predict for T2A amplification.

O receptor epidérmico humano 2 (Her-2) e a topoisomerase-IIα (T2A) são dois marcadores biológicos importantes, ambos tendo um valor prognóstico e preditivo potencial em pacientes com tumores sólidos. A amplificação dos genes Her- 2 e T2A são eventos independentes, embora o último seja mais frequente em tumores com amplificação do Her-2 (34-90%), do que em tumores sem amplificação do Her-2 (5-10%). Existe uma melhor correlação entre amplificação e superexpressão do Her-2 no câncer de mama (CM) do que em outros tumores. No entanto, no CM, a correlação entre amplificação e superexpressão da T2A tem sido inconsistente, e existe uma carência de tais dados em outros tipos de tumores. (EN) Tumours are heterogeneous and consist of multiple populations of cells. The population of cells with tumour-initiating capability is known as cancer stem cells (CSC). Cells with increased stemness properties and elevated resistance to anti-cancer treatment have been shown to be highly affected upon decline of mitochondrial respiration, linking the concept of CSCs to deregulated bioenergetics. Consistently, functional electron transport chain (ETC) is crucial in tumorigenesis. Expression of HER2 oncogene, associated with resistance to treatment in breast cancer, has been connected with regulation of mitochondrial function. We therefore investigated the possibility that manipulation of mitochondrial bioenergetics via disruption of ETC eliminates the conventional therapy-resistant populations of tumour, such as CSCs and HER2high cells. We demonstrate that HER2high cells and tumours have increased complex I-driven respiration and increased assembly of respiratory supercomplexes (SC). These cells are highly sensitive to MitoTam, a novel mitochondria-targeted derivative of tamoxifen, acting as a CI inhibitor and SC disruptor.
