From the known to the unknown future of a newly graduated nurse : a qualitative minor field study in India

Bakground : A new kind of birthing center, providing a new concept of maternal care is about to open in a city located in the eastern part of India. Being part of a new concept in maternal care and having the opportunity to work at a new advanced multispecialty hospital as well as being a newly graduated nurse can create a lot of expectations in which we aim to explore. Aim : The aim was to explore newly graduated nurse's expectations before the opening of a natural birthing center in the eastern part of India. Method : Qualitative interviews were chosen as a data collecting method. The participants were five newly graduated nurses. A qualitative content analysis with an inductive approach and a manifest content was selected for analyzing the collected data. Findings: Four categories with sub-categories were found. They were Unknown future, In relation with patients and relatives, Opportunities to develop as a nurse and to become a good nurse. Conclusion : In this study it was seen that the nurses felt unsure in their role and about their future. To be able to care for patients and relatives as a newly graduated nurse in a professional manner (and make them feel safe), it is important to have the confidence to feel safe in the nursing role. It is also of importance that the organization allows the nurse to develop by creating attuned climates for this, all to become a good nurse.

Work Readiness of Newly Graduated Nurses with Implications for Academia and Employers

Background: The transition and retention of newly graduated nurses are worldwide problems. With the nursing shortage and 33-61% of newly graduated nurses leaving their job within the first year, newly graduated nurses need to be work ready. Work readiness of new nurses is a new concept developed in Australia. Significance: New nurses are a vulnerable population that is dependent upon experienced nurses for knowledge, skills, and socialization into the profession. However, new nurses often experience rudeness, humiliation and conflict influencing professional success, patient care, and retention. Purpose: To apply the Work Readiness Scale – Graduate Nurses (WRS-GN) to a population of Baccalaureate (BSN) and Master’s Entry into the Profession of Nursing (MEPN) graduates from a southwestern university and determine if there is a relationship between the variables of work readiness, individual experiences of graduates, and the two groups. Research questions included: 1) What is the relationship between work readiness (social intelligence, personal work characteristics, work competence, and organizational acumen) and individual experiences? 2) Do newly graduated BSN and MEPN degree nurses differ on the WRS-GN constructs of social intelligence, personal work characteristics, work competence, and organizational acumen? Method: Descriptive correlational study with a convenience sample of graduates from a southwestern university. Participants received a survey through their school email account and a message was placed on the Alumni Facebook page. Results: Thirty participants (9.2% response rate), 93.3% were female, and 76.7% work in Arizona. None of the participants were planning to leave the profession of nursing in the next year. A statistically significant relationship was detected between work competency and length of nurse residency (r=.44, p=0.02) and a negative relationship was detected between personal work characteristics and nurse residency (r=-.41, p=0.02). No relationship was detected between the two groups and constructs of work readiness. Implications/conclusions: Work readiness is complex. Longer nurse residency is associated with greater work competence. Academia and employers should collaborate and provide courses that enhance the work readiness of newly graduated nurses. The WRS-GN has been tested once in a population of Australian graduate nurses therefore further research is needed to validate the WRS-GN.

Att stå på egna ben : Upplevelser av att vara nyutbildad sjuksköterska

Syftet med studien var att undersöka nyutbildade sjuksköterskors gemensamma upplevelser av att börja arbeta efter examen. Detta undersöktes genom åtta intervjupersoner varav två män och sex kvinnor, valdes ur ett handplockat urval och intervjuades utifrån en semistrukturerad intervju. Deltagarna intervjuades om sina unika upplevelser av att vara nyutbildade sjuksköterskor. Resultaten av en tematisk analys påvisade fyra kategorier, att känna osäkerhet, att känna stress, självständighet och att få stöd. Första kategorin bestod av subkategorierna ensamhet, att begå misstag, och kunskapsbrist. Andra kategorin hade subkategorierna oförmåga, tidsbrist, brist på stöd och att hantera svåra lägen. Tredje kategorin hade subkategorierna att känna självtillit, att bli stark och att få rutiner. Sista kategorin hade subkategorierna att få bekräftelse och feedback, att få uppskattning och bli vägledd. Studien kan ge bredare perspektiv på framtida studier gällande nyutbildade sjuksköterskors upplevelse av stress. Slutsatsen av studien är att nyutbildade sjuksköterskors upplevelser är individuella och kan vara både positiva och negativa.

Background: Work-related stress among newly graduated nurses is a problem that results in ill health for the nurses and negatively impacts patient care, risking patient safety. For newly graduated nurses to provide quality and safe nursing care despite work-related stress, it is necessary to understand their experience regarding how work-related stress affects their nursingpractice. Purpose: The aim was to shed light on newly graduated nurses' experiences of work-related stress. Method: A general literature review, including ten original articles, was analysed using Graneheim & Lundmans method of qualitative content analysis. Results: The results of the literature study identified two categories and five subcategories: The transition from theory to practice with the subcategories of Managing unexpected workload, Being clinically ignorant, and Fear of making mistakes. New to the team with subcategories, Adapting to the healthcare team and Meeting expectations. Conclusions: The experience of work-related stress among newly graduated nurses affects their nursing practice and jeopardizes patient safety. With support from experienced nurses, colleagues, and adequate preparation, the transition process can be facilitated, enabling newly graduated nurses to provide quality and safe nursing care more easily. Further research: A longitudinal study would enable investigation of possible patterns and trends regarding stress levels, coping strategies and their impact on nurses' well-being, with a specific focus onnursing work and patient safety.

It is being built at an ever-increasing pace at the same time as there is a shortage of skills in theconstruction industry. As construction companies have difficulty finding competent staff,attraction, recruitment and the introduction of new employees will be a crucial issue forcompanies. This report, which is carried out together with JM AB, examines crucial factors fora good introduction of a newly hired, newly graduated supervisor. In addition, it is examinedwhat three different categories of employees consider to be important in a good introduction andwhether their opinions agree or not.The empirical part of the report consists of an interview and a survey. During the interview, itappeared that JM works to develop its leaders and create a sustainable working life and that therespondent has seen the results of a good introduction in one of JM's regions. During the survey,supervisors, site managers and business unit managers were asked to rank what is important inan introduction of a newly graduated, newly hired supervisor. It emerged that there is a relativelylarge spread, both within each group and in a comparison of the groups, regarding what isconsidered important in a good introduction.Together with the empirical part, a literature study has been carried out where two theories,Learning and Self-Determination Theory, have been studied and linked to how they cancontribute to improving an introduction.The study showed that a good introduction has a positive impact on the new employee's wellbeingand performance and contributes to the new employee becoming part of the communitymore quickly. All three groups of employees considered it important that the workplace isestablished on arrival and with reoccurring supervision meetings.

The aim was to find out the experiences of mentoring and its introduction in social services. The object was achieved by interviewing experienced social worker who had a few years of work experience in social services. Our study was an attempt to understand how mentoring can be seen during the induction period for newly qualified social workers in social services. To get an understanding of the interviewees' perceptions of mentoring, we tooke help of a Human Resources perspective. In the study, we used a qualitative research with data collection based on semi-structured interviews. The sample consisted of a convenience sample of five experienced social workers in department’s financial assistance and child and youth care. According to the study result it is important to have a mentoring and a comprehensive introduction, since it benefits the entire organization. All respondents in the study felt that a mentoring provides security and a check for recent graduates. Based on the results we have seen that even if an organization has an elaborate mentoring program/introduction program it’s not guaranteed to be completed. / Syftet med studien var att undersöka erfarna socialsekreterares uppfattningar kring mentorskap samt den introduktion nyutexaminerade får inom socialtjänsten. Syftet uppnådes genom intervjuer med erfarna socialsekreterare som har arbetslivserfarenheter inom socialtjänsten. Studien kan ses som ett försök till att förstå hur mentorskap kan uppfattas under introduktionsperioden för nyutexaminerade socionomer inom socialtjänsten. För att få en förståelse för intervjupersonernas uppfattningar kring mentorskap har ett Human Resource (HR) perspektiv använts. Studien är kvalitativ och baserad på semistrukturerade intervjuer. Urvalet bestod av ett fem erfarna socialsekreterare inom avdelningarna ekonomiskt bistånd och barn- och ungdomsvården. Resultat påvisade vikten av att tillämpa ett mentorskap och en innehållsrik introduktion. Detta var något som gynnade hela organisationen. Mentorskapet upplevdes bidra till trygghet och kontroll för de nyutexaminerade. Utifrån resultatet har vi kunnat konstatera att även om en organisation har ett utarbetat mentorskapsprogram/introduktionsprogram är det inte garanterat att det fullföljs.

Bakgrund: Kliniskt omdöme kan beskrivas som förmågan att på ett insiktsfullt sätt identifiera och agera på en förändring eller ett behov relaterat till patientens hälsotillstånd. Kliniskt omdöme kommer till sin spets i komplexa situationer som exempelvis när en patient plötsligt försämras. Det har skett en akademisering av sjuksköterskeutbildningen som gett sjuksköterskor en stadigare teoretisk grund men det kliniska omdömet behöver utvecklas efter examen. Syfte: Syftet var att beskriva faktorer som påverkar utvecklingen av nyexaminerade sjuksköterskans (NS) kliniska omdöme. Metod: Metoden som användes var litteraturöversikt, baserad på tio vetenskapliga artiklar varav åtta kvalitativ metod och två mixad metod. Artiklarna hämtades ur databaserna CINAHL Complete och PubMed. Resultat: Faktorer som påverkar utvecklingen av NS kliniska omdöme sammanfattas under tre huvudteman: Förvärvandet av kliniska förmågor, Ansvarsfull kommunikation och Integrerat stöd. NS behöver utveckla förmågan att identifiera relevanta förändringar, tolka dem i relation till en helhetsbild av patienten, prioritera åtgärder samt kommunicera med kollegor. Utvecklande av kliniskt omdöme underlättas i ett positivt arbetsklimat med stöd av erfarna sjuksköterskor (ES). Vårderfarenheter ger teoretiska kunskaper en ny mening, vilket också bidrar till utvecklandet av kliniskt omdöme. Diskussion: Resultatet diskuterades med utgångspunkt i Benners teori för hur sjuksköterskans kliniska omdöme utvecklas från novisens till expertens. NS är inte mogen att hantera komplexa kliniska situationer och organisationen måste utformas för att ge NS det stöd som behövs för att utveckla det kliniska omdömet. Tillgången till ES, reflektion och ett rimligt arbetstempo är viktiga stöttande faktorer, men sällan en självklarhet i vården. / Background: Clinical judgement can be described as the ability to wisely identify and act upon changes in - or needs of a patient’s - health status. Clinical judgement is most applicable to complex situations such as caring for deteriorating patients. As nursing was included in higher education nurses have acquired a more solid theoretical knowledge base, however their clinical judgement needs to be further developed post graduation. Aim: The aim of was to describe factors influencing the development of newly graduated nurse’s (NN) clinical judgment. Method: The method consists of a literature review based upon ten scientific articles, of which eight were qualitative and two followed a mixed method. The articles were retrieved from CINAHL Complete and PubMed databases. Results: Contributing factors to the development of NN clinical judgment were grouped into three main themes: Development of clinical abilities, Responsible communication and Integrated support. NN needs to develop the ability to recognise relevant changes, relate them to a holistic assessment of the patient and to communicate findings. The development of clinical judgment is supported by a positive work climate and by access to experienced nurses. Caring for patients adds a new dimension to theoretical knowledge and contributes to the development of clinical judgment. Discussion: The results were discussed using Benner´s theory of how the clinical judgement of nurses develops from novice to expert. NN is not able to safely handle complex clinical situations without the support of experienced staff. It is also essential for the development of clinical judgment that NN is given a reasonable workload and opportunities for reflection. However, there may often be a lack of these supporting factors in nursing organisations today.

Nyutexaminerade sjuksköterskors upplevelser och hantering av arbetsrelaterad stress under det första året i yrket : En intervjustudie

Bakgrund: Sjuksköterskeyrket innebär en hög arbetsbelastning och kan därmed leda till upplevelse av arbetsrelaterad stress. Nyutexaminerade sjuksköterskor är särskilt mottagliga för denna typ av stress då de saknar erfarenhet i sin yrkesroll. Att utarbeta strategier för hantering av stress är därför betydelsefullt för sjuksköterskans välbefinnande. Syfte: Syftet med föreliggande studie var att beskriva hur nyutexaminerade sjuksköterskor upplever och hanterar arbetsrelaterad stress under det första året i yrket. Metod: Studien har en kvalitativ ansats med deskriptiv design. I studien deltog åtta stycken sjuksköterskor som tagit examen januari alternativt juni 2014, anställda på fyra olika somatiska avdelningar belägna i Mellansverige. Data samlades in med semistrukturerade intervjuer och analyserades med manifest kvalitativinnehållsanalys. Huvudresultat: Resultatet visade att den arbetsrelaterade stressen orsakades av kombinationen mellan de nyutexaminerade sjuksköterskornas inre krav och känslor samt omgivningens yttre påverkan. Hur sjuksköterskorna hanterade den arbetsrelaterade stressen var individuellt och varierande beroende på situation. Att finna stöd hos och ta hjälp av kollegorna, att sänka de egna kraven samt att utveckla ett arbetssätt som passar den enskilda individen visade sig vara effektiva strategier för stresshanteringen. Slutsats: Slutsatsen var att de nyutexaminerade sjuksköterskorna upplevde flera typer av stress under det första året i yrket. Möjligheten att utvecklas, lära sig att vara snäll mot sig själv samt att kollegorna fanns tillgängliga för att ge hjälp och stöd var förutsättningar som krävdes för att stressen skulle kunna hanteras. / Background: The nursing profession involves a high workload and can thus lead to a perceived work related stress. Newly graduated nurses are particularly susceptible to this type of stress because of their lack of experience in their professional capacity. To develop strategies for managing stress is therefore important for the nurse’s wellbeing. Aim: The aim of the present study was to describe how recently graduated nurses experience and cope with work related stress during the first year in the profession. Method: The study had a qualitative approach with descriptive design. In this study eight nurses participated who graduated in January, alternatively, in June 2014, and were employed at four different somatic wards situated in central Sweden. Data were collected using semistructured interviews and were analyzed with manifest qualitative content analysis. Main Findings: The results showed that work related stress was caused by a combination of the recently graduated nurse’s internal demands, feelings and ambient external influences. How nurses handled work related stress was individual and varied depending on the situation. To find the support and enlist the help of colleagues, to reduce their own requirements and to develop an approach that suits the individual was found to be effective strategies for stress management. Conclusion: The conclusion was that the recently graduated nurses experienced several types of stress during the first year in the profession. The ability to evolve, learn to be kind to yourself and that the colleagues were available to provide assistance and support were prerequisites for managing the stress.

<p>Syftet med denna studie var att belysa olika aspekter av nyutexaminerade allmänsjuksköterskors erfarenheter av att möta närstående i sorg. Studien hade en kvalitativ ansats och data insamlades genom sex stycken bandade intervjuer på tre medicinavdelningar vid ett medelstort sjukhus i mellersta Sverige. Datamaterialet analyserades enligt en manifest metod och ur analysen skapades två huvudteman – relationer och kunskap. Det största temat är relationer som byggs upp av tre underteman och beskriver sjuksköterskans förhållande till närstående, sig själv och övrig vårdpersonal. Temat kunskap innehåller två underteman och beskriver sjuksköterskans kunskapsutveckling och behov av kunskap.</p><p>Slutsatserna från studien är att nyexaminerade sjuksköterskor anser att möta närstående i sorg är en av deras svåraste uppgifter på grund svårigheterna i att tillgodose närståendes individuella behov. Det visade sig även att sjuksköterskorna ansåg att sjuksköterskeutbildningen inte förberett dem tillräckligt för detta. Att lära sig att möta närstående är något som sker i praktiken. Sjuksköterskorna ansåg att det viktigaste i mötet med närstående är att vara närvarande och visa att de är tillängliga snarare än att säga eller göra rätt saker.</p><p>Vidare forskning inom området skulle kunna fokusera på skillnader mellan erfarna och nyexaminerades erfarenheter. En vidare forskning skulle också kunna inbegripa patientens perspektiv.</p> / <p>The aim of this study was to shed light on different aspects of newly graduated nurses’ experiences from interacting with relatives to patients who were dead or dying. The study has a qualitative approach and data was collected from six tape recorded interviews conducted at three medicine wards in a medium sized hospital in the middle of Sweden. The data was analyzed by a manifest content method and two main themes – relationships and knowledge, were derived from it. The largest main theme is relationships, which also contains three sub themes describing the nurse’s relationship to relatives, self and other members of staff. The main theme knowledge contains two sub themes which describes the nurse’s need for and development in knowledge. The study concludes that interacting with grieving relatives is one of the most complicated tasks for the newly graduated nurse. The difficulties were grounded in satisfying the complex needs of the relatives. The nurses stated that the nursing education program didn’t sufficiently prepare them for this, it was something they learnt from real life experience.</p><p>The findings of this study could be beneficial for the nurse education program as well as hospitals to increase their awareness of the difficulties newly graduated nurses experiences when interacting with grieving relatives.</p><p>Further research could involve describing differences between experienced and newly graduated nurses and including the patient’s perspective.</p>

Nyutexaminerade sjuksköterskors upplevelser av arbetsrelaterad stress samt stressens konsekvenser : En kvalitativ intervjustudie

Syftet med studien var att beskriva nyutexaminerade sjuksköterskors upplevelser av arbetsrelaterad stress samt att beskriva deras upplevelser av vad stress kan leda till för konsekvenser. En deskriptiv intervjustudie med kvalitativ ansats genomfördes. Datainsamlingen utfördes genom åtta semistrukturerade intervjuer. Det insamlade materialet analyserades med kvalitativ manifest innehållsanalys. Huvudresultatet påvisade att nyutexaminerade sjuksköterskor har upplevt stress under sin första tid i yrket, bland annat till följd av att det är en tid av osäkerhet. Nyutexaminerade sjuksköterskor har beskrivit att upplevelsen av stress har minskat med tilltagna kunskaper och erfarenheter samt minskade krav på sig själv i yrkesrollen. Sjuksköterskeyrket har beskrivits vara stressigt på grund av en mängd olika orsaker men det finns åtgärder som kan vidtas för att minska stressen. Stressen har beskrivits kunna leda till konsekvenser både för sjuksköterskan som individ och för omvårdnaden av patienterna, vilket har beskrivits kunna resultera i känslor som frustration och otillräcklighet. I linje med Benners teori kan konstateras att nyutexaminerade sjuksköterskor genomgår en yrkesutveckling vilken leder till en upplevelse av minskad stress. Stress inom sjuksköterskeyrket är ett komplext problem som måste angripas från flera håll. För den nyutexaminerade sjuksköterskan är det av stor vikt att vara snäll mot sig själv i yrkesrollen genom att inte ha för höga krav på sig själv, detta för att kunna utöva sin yrkesroll som innebär att bedriva optimal patientfokuserad omvårdnad. / The aim of the study was to describe newly graduated nurses' experiences of work-related stress and to describe their experiences of what consequences stress can lead to. A descriptive interview study with qualitative approach was used. Data collection was performed by eight semi-structured interviews. The collected data was analyzed using qualitative manifest content analysis. The main result demonstrates that newly graduated nurses experienced stress in the beginning of their profession, partly because it is a time of uncertainty. Newly graduated nurses have described a decreased experience of stress with increased knowledge and experience, and reduced demands on herself in the professional role. Nursing is described to be stressful due to a variety of causes but there are measures that can be used to reduce stress. Stress has been described to cause consequences both for the nurse as an individual and for the care of patients, which has been described to result in feelings such as frustration and inadequacy. In line with Benner's theory it can be stated that newly graduated nurses undergo a professional development which lead to a experience of decreased stress. Stress in the nursing profession is a complex problem that must be dealt with on several fronts. For the newly graduated nurse it is very important that she is gentle with herself in the professional role by not having too high demands on herself to be able to practice her profession, that is carry out optimal patient-focused nursing.
