The impact of job redesign on employee job outcomes: the case of the implementation of a private-public partnership model at a hospital

It has been widely reported that despite high health expenditure and a myriad of policies in place, South Africa’s health outcomes are worse than those in many lower income countries. The adverse health outcomes are even more pronounced in a rural province such as the Eastern Cape Province. Consequently, the Eastern Cape Department of Health (ECDOH) had turned to Private Public Partnerships (PPP) with the hope of mitigating some of the challenges beleaguering the health system in the province. This study evaluated the impact of the implementation of one of the PPP models at an Eastern Cape Hospital on key employee job outcomes. This is crucial as there had not yet, been this type of scientific assessment of the impact of the PPP model since the inception of the PPP about half a decade ago. Theoretical guidance of the study was provided by the Job Characteristics Theory (JCT) of Oldham and Hackman (1975), which predicted that changes in five core characteristics of a job will affect three critical psychological states which will in turn impact on key employee job outcomes. Uniquely, this study veered away from the traditional quantitative approach to the application of the JCT but rather adopted a qualitative case study approach. There is historical evidence that cross cultural validation of a theory in a new setting (a South African PPP hospital in this instance) benefits from an initial qualitative study. Data collection and analysis were guided by the JCT. Primary data collection was by semi-structured, face to face, one on one interviews. The analyses of the data specifically employed pattern matching and explanation building techniques, all underpinned by the JCT. Validity of interview data was strongly contributed to by available relevant case study documents. This study found that indeed, the implementation of this PPP model brought about changes in all five (JCT) core job characteristics of clinical staff, but to varying degrees in the three unique shared service areas. Interestingly, the three psychological states were found to have been impacted upon by changes in the JCT core job characteristics but also by factors related to the context of the job, such as quality of supervision and co-worker relationship. In contrast to the predictions of the JCT, this study also found that the key job outcomes were impacted directly by such contextual factors as the recent availability of specialists and staff shortages, among others. It also emerged that the PPP implementation has directly evoked a perception of inequity and breach of psychological contract among clinical staff working at the shared service areas. Among the three shared areas, the accident and emergency unit was discovered to have had the worst overall impact. Due to the importance of contextual factors at this PPP setting, key recommendations were directed towards improved management of the jobs. It is also recommended that a follow-up quantitative study be commissioned to further explore the main themes that emerged from this study. This research report is presented in three sections. Section 1 is the evaluative report itself, structured as an academic paper. Section 2 expands on the literature that was briefly reviewed in Section 1 while Section 3 outlines, in greater details, the research methods followed during the conduct of the research and the justifications thereof.

O estudo se justifica face à escassez de trabalhos que apontem resultados de otimização de processos de trabalho em organizações do setor público, uma vez que a maior parte dos estudos existentes é voltada para a gestão de processos em empresas privadas e, aqueles que focalizam no setor público, normalmente apontam as diferenças com relação ao setor privado, ou seja, pouco se fala do resultado das transformações. Para alcançar esse objetivo, foi realizado um estudo de caso, com análise de conteúdo de dados e documentos, bem como modelagem dos processos selecionados, visando identificar as lacunas existentes antes do BPM, bem como de que maneira os processos foram impactados. Como contribuição prática, espera-se que o resultado sirva de incentivo aos demais órgãos públicos para implementação de uma gestão com foco em processos e aja como instrumento facilitador de uma melhor gestão pública. / This study aimed to identify the impact of BPM on Rioprevidência survivor’s pension process. To make this possible, we analyzed three business process executed by customer service sector of Rioprevidência, an independent self-governing public entity of Rio de Janeiro state, namely: Pension Concession, Pension Review and Pension Audit. The study is justified by the lack of studies that point results of business processes optimization in public sector organizations. It happens because most of the existing studies focus on business processes management in private companies and those that focus on public sector usually point out the differences from the private sector, in other word, hardly speaks about the outcome of transformations facing the public sector. To achieve this objective, we conducted a qualitative case study with data and documents analysis and redesigned the selected processes to identify gaps before BPM and to detect the impact suffered by processes. As a practical contribution, we expect that the results may encourage other public entities to implement a management focused on processes and work as a facilitator for better public management.

To this, the work uses the Barajas and Steiner (2002) methodology but with a larger country sample (Brazil among nine Latin American countries and other forty outside Latin America). The results suggest that bank credit has not grown as expected and is still as volatile as the Latin America average. The size of the Brazilian banking system is not small as expected, but is less prone to lending to the private sector. Credit to the public sector still occupies a very important share of the Brazilian banking system balance sheet. About this matter the work shows that an increase in securitization could increase significantly the credit to the private sector. / O objetivo deste trabalho é estudar o nível de crédito bancário no Brasil no período pós implantação do Plano Real. Para isso, o trabalho utiliza a metodologia de Barajas e Steiner (2002) mas utilizando uma amostra maior de países (além da economia brasileira, nove países da América Latina e quarenta não América Latina). Os resultados encontrados mostram que o crédito bancário historicamente não cresceu como o esperado e ainda é tão volátil quanto a média América latina. O sistema bancário não mostrou-se menor do que o esperado para a economia brasileira, mas é menos propenso ao crédito ao setor privado. O crédito ao setor público ainda ocupa uma fatia muito relevante no balanço do agregado bancário e sobre esse respeito o trabalho mostra que um aumento na securitização e cessão de crédito aumenta significativamente o crédito ao setor privado.

The development and validation of an assessment framework for measuring the organisational effectiveness of a metropolitan municipality in South Africa

The aim of this quantitative study was to develop and validate a model to measure the organisational effectiveness of a metropolitan municipality in South Africa. The literature review phase explored the concept of organisational effectiveness and the assessment thereof in both the Public and Private Sectors. The literature review indicated that there is a clear distinction between business performance (operational and financial performance) and the larger concept of organisational effectiveness, and also that the measurement of organisational effectiveness in the Public Sector differed from the measurement thereof in the Private Sector. The literature review also indicated that measures of Public Sector effectiveness could not be directly applied to measure the effectiveness of Private Sector organisations. From the literature review a proposed theoretical model for measuring the organisational effectiveness of a metropolitan municipality in South Africa was proposed. This proposed model included organisational and behavioural variables contained in traditional approaches to organisational effectiveness, variables that were identified in previous organisational effectiveness studies, as well as variables contained in existing assessment models of organisational effectiveness. This model was then validated during the empirical phase by conducting a survey of an existing metropolitan municipality in South Africa (n = 6514) and exposing the results of the survey to Structural Equation Modelling (SEM). The confirmatory factor analysis conducted as part of SEM subsequently identified three main and 10 secondary statistically significant organisational and behavioural variables that could be used to measure the effectiveness of a metropolitan municipality in South Africa. The three main variables identified were (1) Healthy Systems, (2) Goal Achievement and (3) Service Delivery, while the 10 secondary variables identified were (1) Diversity, (2) Training & Development, (3) Rewards & Recognition, (4) Management Practices, (5) Internal Functioning, (6) Work Environment, (7) Interpersonal Relations, (8) Workforce Equity, (9) Customer Satisfaction and (10) Vision & Mission. It was thus recommended that metropolitan municipalities in South Africa could use this validated model as an assessment framework to measure their current organisational effectiveness, to identify aspects which need to be rectified to improve effectiveness, and to compare and benchmark their municipality in order to learn from other metropolitan municipalities to improve their effectiveness. / Industrial and Organisational Psychology / D. Caracterização de fisioterapeutas no sistema de saúde brasileiro

In 1969, the profession is regulated by Decree-Law No. 938 and the following year the number of physiotherapists totaled 700 with six undergraduate courses. In 2011, the total number of physiotherapists registered in the Federal Council of Physical Therapy and Occupational Therapy (COFFITO) has reached 154,563 professionals, there are 542 undergraduate courses. The growing number of professionals, however, is not necessarily linked to the expansion of physical therapy for the population equitably. The present study aimed to investigate the distribution and role of physiotherapists in the health system, in order to contribute to policies and interventions according needs of the population, consisting of four articles. The first three articles aimed to describe the distribution of physiotherapists in the health system according to data provided by National Register of Health Service Providers (CNES), considering the different regions, federal units, and population size of cities, public and private sectors, and the different levels complexity of health care. The studies were characterized as descriptive cross-sectional studies, developed from data collected on the bank of CNES in Brasilia, referring to February 2010, and Census for 2010, and analyzed using descriptive statistics. Results: The study identified 53,181 registries of physiotherapists distributed of 22,238 establishments, arranged in 76% of the cities. Approximately 50% of registries were concentrated in the Southeast, 21% Northeast 18% South, Central-West 7% and 4% North. South and Southeast had the best ratio physiotherapists/1000 inhabitants (0.33 and 0.35, respectively), as opposed to the North (0.13), and the highest percentage of municipalities with registries (91% Southeast and 83% South), being observed in the North only 40% of cities with this type of professional. Metropolises and big cities concentrated 64% of the identified positions. Approximately 60% of registries occurred in the private sector, and specialized private establishments in the Southeast of the main actuation sites of physiotherapists, 57% of registries were linked to Specialized Ambulatory Care and 30% to Hospital Care. The Primary Health Care (PHC), which represents the priority level of attention of the health system, focused only 13% of registries, divided into 46.7% of the cities - the highest percentage in small cities (38.2% of registries in the APS). From this fact, the fourth article aimed to investigate the role of physiotherapists in APS, developed through a questionnaire online; 34 physiotherapists participated in the study, with aged between 22 and 47 years (mean 32.5 ± 6.36). Despite the intense demand for therapeutic actions, the initiatives in health promotion and disease prevention have been demonstrated. Main difficulties cited: demand spontaneous excessive, insufficient number of physiotherapists, inadequate physical space, lack of transportation to home care activities, insufficient support of medium and high complexity, ignorance of the attributions of professional PHC, difficulties in teamwork and low population adherence interventions. / A fisioterapia surge mundialmente no final do século XIX, sendo instalado no Brasil o primeiro serviço de fisioterapia em 1884. Em 1969, a profissão é regulamentada pelo Decreto- Lei nº 938 e no ano seguinte o número de fisioterapeutas totalizava 700, existindo seis cursos de graduação. Em 2011, o total de fisioterapeutas inscritos no Conselho Federal de Fisioterapia e Terapia Ocupacional já atingia 154.563 profissionais, existindo 542 cursos de graduação. O crescimento do número de profissionais, contudo, não está necessariamente vinculado à expansão da oferta de assistência fisioterapêutica à população de forma equânime. O presente estudo pretendeu investigar a distribuição e atuação de fisioterapeutas no sistema de saúde, buscando contribuir com políticas e intervenções que possam transformar a expansão de profissionais em uma oferta voltada às necessidades de saúde da população, sendo constituído por quatro artigos. Os três primeiros artigos objetivaram descrever a distribuição dos fisioterapeutas no sistema de saúde de acordo com o Cadastro Nacional de Estabelecimentos de Saúde (CNES), considerando as diferentes regiões, unidades federativas e porte populacional dos municípios; os setores público e privado; e os diferentes níveis de complexidade de atenção à saúde. Todos se caracterizaram como estudos transversais descritivos, desenvolvidos a partir de dados coletados no banco do CNES em Brasília, referentes a fevereiro de 2010, e dados do Censo Demográfico de 2010, sendo analisados por técnicas estatísticas descritivas. Resultados: foram identificados 53.181 cadastros de fisioterapeutas distribuídos entre 22.238 estabelecimentos, dispostos em 76% dos municípios. Aproximadamente 50% dos cadastros estiveram concentrados na região Sudeste, 21% Nordeste, 18% Sul, 7% Centro-Oeste e 4% Norte. Sudeste e Sul apresentaram as melhores razões de fisioterapeutas/1000 habitantes (0,33 e 0,35, respectivamente), em contraposição ao Norte (0,13), e as maiores proporções de municípios com cadastros (91% Sudeste e 83% Sul), sendo no Norte observado apenas 40% dos municípios com este profissional. Metrópoles e municípios de grande porte concentraram 64% dos postos de trabalho. Aproximadamente 60% dos cadastros ocorreram no setor privado, sendo estabelecimentos privados especializados do Sudeste os principais locais de atuação deste profissional; 57% dos cadastros estiveram vinculados à Atenção Ambulatorial Especializada e 30% à Atenção Hospitalar. A Atenção Primária à Saúde (APS), que representa o nível de atenção prioritário do sistema de saúde, concentrou apenas 13% dos cadastros, distribuídos em 46,7% dos municípios - maior percentual em municípios de pequeno porte (38,2% dos cadastros na APS). A partir desta realidade, o quarto artigo objetivou investigar a atuação de fisioterapeutas na APS, por meio da aplicação de questionário on line. Participaram do estudo 34 fisioterapeutas da APS com idade entre 22 e 47 anos (média 32,5 ± 6,36), que, apesar da intensa demanda por ações terapêuticas, demonstram que iniciativas de promoção da saúde e de prevenção de enfermidades estão sendo desenvolvidas pelo país. Entre as dificuldades enfrentadas, destacam-se: Demanda espontânea excessiva; Número insuficiente de fisioterapeutas; Espaço físico inadequado; Falta de transporte para ações domiciliares; Suporte insuficiente da média e alta complexidade; Desconhecimento sobre atribuições de profissionais da APS; Dificuldades no trabalho em equipe e Baixa adesão da população as intervenções.

The overall aim of this research was to understand the essential elements/factors that contribute to the effectiveness of the content of codes of conduct in the public service in addressing corruption. The ultimate objective was to compare the South African public service code of conduct with practical guidelines to determine whether it complies with international practice. The international guidelines used in this research were that of the Organisation for Economic Co-operation and Development on codes of conduct, the Technical Guide to the United Nations Convention against Corruption, the United Nation’s International Code of Conduct for public officials and the Council of Europe’s Model Code of Conduct for public officials. Assessment of capacity development through public-private partnership: a case study of the Department of Economic Development and Environmental Affairs in the Eastern Cape

The aim of this study was to examine the effectiveness of a capacity development programme initiated by the Department of Economic Development and Environmental Affairs (DEDEA) as a mechanism to enhance the performance of the employees in order to improve service delivery to the people of the Eastern Cape. The public service is enjoined by the Constitution to provide services that citizens need, and for this reason it is in need of educated, trained and professional public servants, especially in the technical, professional and managerial categories. The department also recognized the potential of public–private partnerships (PPPs) to improve performance in the areas of transformation and service delivery, and that it required investments aimed at developing and strengthening public management capacity. However, the effectiveness of the PPP as a mechanism for capacity development, remained unknown. The literature reviewed overwhelmingly supported the assertion that human capital development is critical for performance in the public service. The review also highlighted the importance of partnerships, especially the role of the private sector in the area of capacity development, advancing the argument that PPPs should be pursued in order to leverage ideas, resources, and capabilities to achieve public service goals. Various research tools, namely interviews, questionnaires and document analysis were used to conduct the study. Data collected and analysed provided empirical evidence of improved performance of both the individual managers and the department in the functional areas of financial governance and management; programme and project management; as well as ethics and professionalism. It suggested a strong correlation between training, capacity development and service delivery. This research found the DEDEA/National Business Initiative (NBI) PPP initiative, as a capacity development mechanism to enhance the performance of the department‟s employees, effective. On strength of the overwhelming evidence gathered through this research, it strongly recommends that the programme be rolled out to other employees.

Specifically, the focus of the research is directed toward those people who were condemned to imprisonment through long length sentences, due to have been accused and convicted of serious crimes against individuals, the social order and its security. The hypothesis raised here based upon careful empirical observations is that the State, alone, is weak and unprepared to cope with programs of re-socialization of these people; there is no serious academic and educational strategies directed to help them to reach dignity and professional and social respectability; nor have been introduced effective policies toward a professional training, or re-training, in order to integrate them in industrial or commercial activities. By contrast, what has happened is that prisons have frequently be transformed in a web of delinquence, tying, inside and outside the prisons, criminal leaders and their gangs. So, once they leave prison, (when and if they do) they are again engaged in different types of criminal actions. Objectively, this research tries to conceive a new and revolutionary project, on the basis of establishing agreements between state partners and private ones, concentrating both money, social services, and technologies, able to provide educational and professional means and efforts, not only in order to low the level of criminality, but mainly to create new and more effective alternatives to the prisoners to reach moral, social and professional reintegration within the community, as well as regain their citizenship / Esta dissertação dedica-se ao estudo das consequências da aplicabilidade da pena privativa de liberdade e das condições de possibilidade de legitimá-la, através de propósitos ressocializadores. Parte-se do princípio de que a pena carcerária é um mal que ainda se apresenta necessário, e para tanto não merece total extinção, mas sim significativas reformas. Toma-se como base dessa hipótese a desvinculação dos crimes que atingem bens jurídicos de maior relevância e propostas alternativas de encarceramento. Como fundamento, analisa-se os reflexos do encarceramento e suas diferentes propostas, enquanto medida indutora à reincidência, pretendendo sugerir propostas de modificação dos caminhos que provocam o sempre crescente índice de reincidência, embasando-se na participação da sociedade nos processos de reconstrução da dignidade profissional do apenado e do egresso. Nesse contexto, sugere-se que as parcerias entre o poder público e a iniciativa privada haveriam por emprestar ferramentas hábeis a esse desiderato, sugerindo o estudo e a implementação de estratégias que minimizem os efeitos perniciosos da pena convencional.
