本研究以城鄉差異觀點,評估台灣鄉村型社區心理衛生中心之服務需求,具體目的有(1)分析台灣縣市及雲林縣鄉鎮與心理衛生有關之各項社會指標,以驗證社區心理衛生相關問題的城鄉差異性;(2)瞭解雲林縣及其鄉鎮關鍵訊息提供者對高度農業地區各項社區心理衛生問題嚴重程度、問題型式、地理分佈與相關在地服務資源之看法;(3)瞭解雲林縣高度農業人口鄉鎮社區居民對其村落各項相關心理衛生問題嚴重程度、問題型式、社區壓力源、因應方式與求助情形之看法;(4)由社區居民調查結果,抽取鄉村心理衛生相關問題之共同因素,並歸納諸項調查研究與指標分析結果,初步擬定出一「台灣鄉村心理衛生指標系統目錄」。 研究方法採用多方法評估策略。社會指標分析方面,共計分析台灣地區23個縣市24項及雲林縣20個鄉鎮市16項心理衛生相關問題,以比較不同農業人口分群其發生率或盛行率之差異。關鍵訊息提供者調查方面,使用自編問卷分別調查雲林縣30位與二崙與水林兩鄉69位政府單位、民間機構或專業個人等。社區居民調查方面,二崙與水林兩鄉共發出2049份問卷,回收1074份(52.4%),有效問卷為967份(41村、97.6%)。統計方法有描述統計、集群分析、t考驗、單因子變異數分析、皮爾森積差相關分析、因素分析等。 研究主要發現如下:(1)高度農業人口地區之老年、低教育程度、喪偶、外籍配偶、身心障礙、意外事故死亡、醫事人員平均服務人口數等比例明顯較高,離婚、全般刑案、竊盜、強盜搶奪、暴力犯罪、強制性交等比例則較低。(2)台灣縣市心理衛生相關嚴重問題社會指標之地理分佈概況不明顯,但雲林縣心理衛生相關嚴重問題比例則以高度農業人口的二崙與麥寮兩鄉明顯較高。(3)雲林縣與二崙、水林兩鄉關鍵訊息提供者皆認為人口外移與老化、老人問題(獨居、貧窮、安養等)、電話詐騙等問題比例較高。雲林縣關鍵訊息提供者主觀覺得口湖、台西與四湖為最亟需心理衛生服務的農業鄉鎮。二崙鄉關鍵訊息提供者主觀覺得大庄、楊賢與港後為該鄉最亟需心理衛生服務的村落。水林鄉關鍵訊息提供者主觀覺得水北、塭底與大山為該鄉最亟需心理衛生服務的村落。(4)二崙與水林兩鄉社區居民認為電話詐騙、人口外移、農產經營影響、人口老化、家庭經濟壓力等問題比例較高,青少女母親(含未婚懷孕)、家庭人數眾多、親友與鄰居死亡頻傳、自殺、性侵害、精神疾病、家庭暴力、家庭虐待等比例較低。(5)二崙與水林兩鄉居民認為「社區孤立與無望感」來自人口老化、多孤獨貧窮老人、人口外流嚴重、生活無聊、缺乏休閒娛樂等因素較多,「犯罪被害擔憂與恐懼」來自竊盜、詐騙、嗑藥吸毒等問題較多,「社區憂鬱現象」來自個人與家庭經濟壓力、失業、農業損害或收益等因素較多。(6)二崙與水林兩鄉居民認為其感受社會壓力源以治安惡化為主,農業壓力源以農產收益與自然災害為主,社區壓力源以治安不佳、衛生環境不良、缺乏休閒娛樂等較多,學校壓力源以城鄉差距大、教育與學習資源不足、課業或升學壓力、教育政策多變等較多,家庭壓力源以經濟收入與子女教養為主,個人壓力源以經濟收入、身體健康、工作壓力等較多。(7)二崙與水林兩鄉居民指出習慣(或主要)抒解或因應壓力方法,以「找人聊天」居首,只有6.5%曾求助於親友之外的專業人員。(8)雲林縣現有7個單位或機構(衛生局─社區心理衛生中心與長期照護管理示範中心、社會局─各課、家庭教育中心、台大醫院雲林分院精神科、雲林區心理衛生諮詢服務中心與生命線協會)共提供37個鄉村心理衛生相關服務項目。 本研究由社區居民調查結果,共抽取出七個因素構面並分別命名為「一般社區心理衛生問題」、「農業社區心理衛生問題」、「環境污染」、「經濟壓力」、「居住生活風險」、「犯罪」與「家庭婚姻特性」,並歸納諸項調查研究與指標分析結果,初步擬定出一「台灣鄉村心理衛生指標系統目錄」(5大指標向度,14個指標項目)。 文末,研究者並根據研究結果與國內外相關文獻,分別提出鄉村居民心理健康政策、鄉村心理衛生指標系統、農業危機服務、老人心理衛生、社區孤立與無望感、犯罪被害恐懼、環境污染心理影響、鄉村性別與族群心理議題、鄉村學校之預防功能、鄉村心理衛生服務模式等10項建議。 / The purposes of this thesis were: (1) to analyze the differences of some social indicators related to mental health of Taiwan (23 cities/counties) and Yunlin county (20 townships). (2) to understand the opinions of the key informants about community mental health problems. (3) to survey community residents of agricultural areas about the subjective perceptions of their mental health. (4) to set up a summative index of Taiwan Rural Mental Health indicators System. This research used a multi-method strategy. Data collected in the spring of 2005 included 24 social indicators of 23 cities/counties in Taiwan, and 16 social indicators of 20 cities/township in Yunlin, and questionnaires of 99 key informants and 967 community members of Erh-lun and Shui-lin Township of Yunlin County. Major findings of this study were as follows: (1)Significant differences were found in the social indicators about the numbers of older population, the lower educational status, the widowed, the foreign spouse, and the disabled, the accidental injury-related deaths in agricultural counties. In contrast, urbanized areas had higher rates of the divorced, all criminal case, larceny, robbery and forceful taking, violent crime, rape, and the average number of people serviced by per medical personnel . (2)The geographic analysis in terms of the mental health status and service needs of residents revealed no significant differences among 23 cities/counties, but significant differences among 20 cities/township (Yunlin County), Erh-lun and Mailiao had much more problems. (3)Yunlin County’s key informants indicated that the areas of Kou-hu, Tai-si and Sih-hu have high needs for mental health services. Erh-lun’s key informants indicated that 3 villages have high needs for mental health services. Shui-lin’s key informants indicated that 3 villages have high needs for mental health services. (4)Most respondents of resident sample ranked the following mental health problems as serious: fraudulent telephone calls, out-migrant, farm crisis, being elderly, and family economic hardship. (5)Most respondents of resident sample employed “chatting with others” as stress-relieving or coping method. Besides “family and friends”, only about 6.5% of respondents reported that they sought help from mental health professionals or specialists. (6)To set up the initial rural mental health indicator systems of Taiwan: a summative index consists of 14 items and is divided into five dimensions The recommendations for rural mental health policies, indicator systems, program strategies and rural mental health service delivery issues were also suggested.

長期心理治療創造新人際關係經驗之研究:從心理師觀點 / A study of how to create a new interpersonal relationships experience during long-term psychotherapy:From counselors viewpoint

人際經驗是個體發展他人形象與環境評估的關鍵因素。大部分尋求長期心理治療的個案,其困擾多與過去負面的人際經驗所帶來的關係議題有關。若個體在成長過程經常遭受拒絕,會發展出「世界不能控制、別人不可預測、不可信任」的他人觀點。因此,本研究旨在探討長期心理治療歷程個案他人表徵的差異,以及心理師如何協助個案改變原有的他人表徵,並創造新的人際關係經驗。 本研究以質性研究方式進行,總共以六位長期心理治療取向為主的資深心理師作為研究對象,以一對一的方式,與每位受訪者進行一次深度訪談。訪談內容著重探討個案他人表徵的改變,以及個案過去的重要他人形象如何重現在治療關係中;並深入了解長期心理治療關係對於個案發展新的人際關係經驗之影響。本研究透過現象學方式進行訪談資料的分析,呈現長期深度心理治療對改變個案人際關係經驗之成效。本研究結果發現如下: 一、他人表徵 治療前的他人表徵是模糊、沒有彈性、單一、負面的,因而無法辨認心理師真實的形象。經過長期心理治療,個案對他人形象的詮釋逐漸清晰、具體、有彈性、且納入更多正向的觀點。 二、長期心理治療歷程 先個案會將自己慣用的人際互動帶入與心理師的治療關係中。接著個案感受到心理師溫暖、支持的回應。由於治療關係不同於過去的人際經驗,使個案重新經驗穩定、安全的人際關係,進而開始辨認並建立新的他人形象。最後個案將學到的新人際互動方式帶到其他關係中,發展更適應的人際關係。 三、心理師的角色與功能 為了建立穩固的治療同盟,心理師需維持穩定的態度、提供溫暖支持的環境、尊重個案的自主性。同時心理師也可幫助個案覺察、理解自身的狀態,建立更彈性的想法、協助個案願意信任心理師及冒險。 / Interpersonal experience is the key factor for people to develop other- representation and to evaluate the environment. Most issues of long-term psychotherapy clients’ come from past negative experiences about interpersonal relationship. If people were frequently rejected by others as they grew, it is much easier for them to develop the viewpoints of other that the world is uncontrollable, and that people are unpredictable and untrustworthy. Hence, the main purpose of this research was to explore the difference of other-representation during the long-term psychotherapy period, and how counselors help the clients to change their old other-representation, in order to create new interpersonal relationship experience. This study used semi-structured interviews to collect data from 6 counselors who primarily conducted long-term psychotherapy for clients. Each psychologist was interviewed once, one by one. All interviews lasted for about one and half hour. The contents of the interviews focused on the change of other-representation, the reappearance of clients’ important interpersonal patterns in counseling relationships, and the influences of long-term counseling relationships on clients’ developing new interpersonal relationship experience. After data collection, verbatim transcription of each interviews were analyzed in a phenomenological approach. The study aims to obtain the effectiveness of long-term psychotherapy on the change of clients’ interpersonal relationship experience. The main findings of this study were as follows: 1. Other-representation The other-representation is indistinct, inflexible, single, and negative before long-term psychotherapy begins, and clients have difficulties to recognize the real image of counselor. After long-term psychotherapy, clients are able to interpret the image of others in a more distinct, specific and flexible way, and to include some positive view points in their other-representation. 2. Long-term psychotherapy process First, clients bring their habitual interpersonal interactive mode into counseling relationships. Second, they receive kind and supportive response from the counselor. Because of counseling relationship is different from their past interpersonal experience, clients re-experience a stable and, secure interpersonal relationship and start to recognize and establish a new image of other people. Third, clients learn new interpersonal interactive skill, and develop more suitable interpersonal relationship . 3. The role and function of a counselor In order to establish stable working alliance, counselors should keep stable attitude, provide a holding environment, and respect the autonomy of clients. At the same time, counselors also help clients to comprehend their predicament, expand their cognition with elasticity, and trust counselors.

教育與心理健康之關聯:大學水平分化的效果 / The Relationship between Education And Mental Health: The Effect of Horizontal Differentiation within Higher Education

社會學研究心理健康關注焦點在了解社會因素如何造成心理困擾的發生,其中教育是重要的一環,教育程度的提升有助於身心健康。在社會變遷的脈絡下台灣的教育結構產生變化,高等教育擴張帶來諸多影響,教育對健康的意義可能有所改變,因此高等教育擴張後不同的教育類別是否會產生各種期望上落差的因素出現心理健康的差異是本文的研究課題。 本文使用台灣教育長期追蹤資料庫後續調查 (TEPS-B) 2010年以及台灣教育長期追蹤資料庫 (TEPS) 2003年高中職五專學生與家長問卷資料,嘗試了解教育擴張後大學水平分化的結果是否會透過經濟資源(如學以致用、工作收入)、主觀地位與社會資本造成不同教育類別在這些因素上的差異進而產生對心理健康的效應,並使用一系列迴歸分析與Bootstrapping檢驗這些因素之間的關聯性。 研究結果發現,相對於專科以下,國立一般大學與私立一般大學憂鬱程度較低,教育對心理健康的直接效果得到部分支持。國立一般大學、國立科技大學愈能在工作上發揮所學進而憂鬱情緒較低,國立一般大學與私立一般大學主觀地位較高,進而降低憂鬱情緒,因此教育會透過學以致用與主觀地位對心理健康產生效果,工作收入無顯著差異,社會資本與過去研究結果相異,教育的間接效果得到部分支持。希冀透過檢視上述的各種因素能對於其中的樣貌有更清楚的掌握。 / To figure out how does social causes lead mental illness to be occurred is what the mental health of sociological study focus on, and the education plays an important role in this statement, which means the higher education will make the better physical and mental status. Under the expansion of higher education in Taiwan, it might have changed the meaning in education to health. The purpose of this research is to realize the direct and indirect effect on mental health whether different education category have different kinds of causes leads to mental health discrepancy after the expansion of higher education. Based on the both data of Taiwan Education Panel Survey and Beyond (TEPS-B) 2010, Taiwan Education Panel Survey wave2 (2003) Senior (Vocational) High School and Junior College Students and parents, this study attempt to examine the pathway of which education category have direct and indirect (job match, income, subjective social status and social capital) effects on mental health after the expansion of higher education which produces horizontal within higher education. The major findings, which by using OLS regression and bootstrapping analyses, were as follows: (1) Comparing to junior college, the general universities have the lower degree of depression, but it’s not significant in technical universities. These findings partially support the education directly affect to mental health. (2) Both of public general and technical universities have the better sense of job match and the lower distress. (3) Public and private general universities have higher subjective social status and lower distress. Income is not significant and the results of social capital contradict to the previous research. These findings support the education indirect effects on mental health partially

正向心理介入方案對促進大學生 幸福感影響之研究 / The study of positive psychology intervention effects for promoting college students’ well-being

本研究旨在探討正向心理介入方案對大學生正向心理的影響,以及正向心理介入方案、正向因應策略與主觀幸福感的關係。受試者為國立政治大學的大學生,有效樣本為130人。本研究採問卷調查法,施以情緒溫度計量表、主觀幸福感量表、臺灣憂鬱情緒量表、臺灣正向比量表、臺灣巔峰幸福感量表、靈性幸福感量表、正向因應策略量表及心理資本量表,並蒐集前測與後測資料。資料分析方法包含:成對樣本t檢定、獨立樣本t檢定、單因子變異數分析、內容分析法及結構方程式模型。本研究主要發現如下: 一、在前、後測方面 (一)大學生在「主觀幸福感」、「主觀幸福感」分量表之「社會幸福感」及「情緒幸福感」、「情緒刻度」、「快樂時間」、「持平時間」、「正向因應策略」、「巔峰幸福感」、「巔峰幸福感」分量表之「正向機能」及「正向情緒」、「靈性幸福感」、「靈性幸福感」分量表之「團體靈性幸福感」、「環境靈性幸福感」及「超然靈性幸福感」、「正向比」、「心理資本」、「心理資本」分量表之「復原力」、「希望」及「樂觀」上有顯著差異。 二、在背景變項方面 (一)不同性別大學生在「正向心理介入方案」上有顯著差異。 (二)不同性別大學生在「負向情緒」上有顯著差異。 (三)不同年級大學生在各背景變項上皆無顯著差異。 三、在結構模式方面 (一)正向心理介入方案對正向因應策略有直接正向效果。 (二)正向心理介入方案對主觀幸福感有直接正向效果。 (三)正向因應策略對主觀幸福感有直接正向效果。   最後,研究者根據研究結果與討論,針對高等教育及未來研究提出若干建議。 / The main purpose of this study was to explore the positive psychology intervention effects on college students, and also the relationships among positive psychology interventions, positive coping strategies and subjective well-being. The participants included 130 college students sampled from National Chengchi University. The pretest and posttest data were both collected by questionnaires, including the Emotional Thermometer Scale, the Subjective Well-Being Scale, the Taiwan Depression Scale, the Taiwan Positivity Scale, the Taiwan Flourishing Scale, the Spirituality Well-Being Scale, the Positive Coping Scale, and the Psychological Capital Scale. Moreover, the data analysis was based on paired-samples t-test, independent-sample t-test, one-way ANOVA, content analysis method, and SEM. The main results were summarized as follows: About the pretest and posttest: 1.Students were significantly different in the scores of subjective well-being, social well-being, emotional well-being, emotional degree, happy time, balanced time, positive coping strategy, flourishing well-being, positive function, positive emotion, spirituality well-being, group-spirituality well-being, environment-spirituality well-being, super spirituality well-being, positivity, psychological capital, resilience, hope, and optimism. About the background variables: 1. Students with different gender were significantly different in the scores of positive psychology interventions. 2. Students with different gender were significantly different in the scores of negative emotion. 3. Students with different grade were not significantly different in the scores of background variables. About the structural model: 1. Positive psychology interventions had positive influence on positive coping strategy directly. 2. Positive psychology interventions had positive influence on subjective well-being. 3. Positive coping had positive influence on subjective well-being. Based on the results and discussion of this study, directions of higher education and future research were suggested.
