The waxing and waning of mental disorders: Evaluating the stability of syndromes of mental disorders in the population

This article examines the stability of symptoms, syndromes, and diagnoses of specific anxiety and depressive disorders, as well as diagnostic shifts from one syndrome to another over time. Using retrospective and longitudinal prospective data from the baseline and first follow-up investigation (19.7 months later) of the Early Developmental Stages of Psychopathology Study (EDSP), we focus on establishing stability measures for early stages of mental disorders in a community sample of adolescents aged 14 to 17 years at baseline. The results are as follows: (1) Although only about 30% developed a full-blown DSM-IV disorder, psychopathological syndromes are widespread in adolescents: 15% of the population aged 14 to 17 at baseline were not affected by at least some clinically relevant symptoms of mental disorders either throughout their previous life or throughout the follow-up period. (2) The likelihood of staying free of symptoms and threshold disorders during follow-up was highest among subjects who were completely well at baseline. The probability of a positive outcome decreased as a function of severity of baseline diagnostic status. (3) There was a considerable degree of fluctuation not only in the diagnostic status and severity of specific disorders, but also in terms of complete remissions and shifts from one syndrome and disorder to another. (4) Anxiety disorders, overall, slightly differ with regard to the persistence and stability of the diagnostic status from depressive disorders. (5) However, there were remarkable differences between specific types of anxiety and depressive disorders. Consistent with other longitudinal epidemiological studies in the general population, this study finds that the syndromes and diagnoses of mental disorders have a strong tendency to wax and wane over time in this age group.

Implications of comorbidity: lessons from epidemiological studies

Objective: The paper discusses conceptual, methodological and clinical issues of comorbidity from the perspective of more recent epidemiological studies. Further the potential causal and pathogenic role of temporally primary disorders for the onset of secondary disorders is evaluated. Results: The available data suggest so far that comorbidity (a) is not an artefact of assessment strategies, sampling or design features, (b) is specific in different disorders, (c) is particularly frequent in anxiety and affective disorders, (d) affects systematically the course of the comorbid conditions and (0 might be related to symptom progression models. Conclusions: Furthermore, evidence is presented that specific forms of primary anxiety disorders affect the risk for secondary depressive disorders, increase the likelihood of non-remission as well as the number of subsequent depressive episodes. / Scopo: Lo studio affronta problemi concettuali, metodologici e clinici della comorbidità, alia luce dei piu recenti studi epidemiologici. Inoltre viene valutata la causa potenziale e il ruolo patogenetico dei disturbi temporaneamente primari per la comparsa dei disturbi secondari. Risultati: I risultati disponibili fino ad oggi suggeriscono che la comorbidità (a) non è un artefatto delle strategic di valutazione, del campionamento o del disegno, (b) è specifica nei differenti disturbi, (c) e particolarmente frequente nei disturbi affettivi e d'ansia, (d) influisce sistematicamente sull'andamento delle condizioni di comorbidita è (f) potrebbe essere correlata con modelli di evoluzione dei sintomi. Conclusioni: È inoltre evidente che forme specifiche di disturbi di ansia primaria comportano il rischio di sviluppare disturbi depressivi. secondari, aumentano la probability di non remissione cosi come il numero di successivi episodi depressivi.

The building industry makes up a considerable fraction of world&amp / #8217 / s energy consumption. The adverse effects of a growing energy demand such as depletion in fossil fuel reserves and natural resources hassled the building industry to a search for new technologies that result in less energy consumption together with the maximum utilization of natural resources. Energy- and ecology-conscious European countries incorporated the well-being of occupants while conducting research on innovative technologies. In view of the fact that double-skin fa&ccedil / ades offer a healthy and comfortable milieu for the occupants and use natural resources hence consume less energy they became a promising invention for all concerns. The analysis of the performance of the double-skin fa&ccedil / ades and energy consumption is inconclusive at this time. However, based upon thermal performance analysis have been done so far, a double-skin fa&ccedil / ade perform better and provide some energy reduction, particularly on the heating side cycle, from a standard double glazed unit wall. The aim of this study was to examine the relationship between double-skin fa&ccedil / ades and building management systems in intelligent office buildings as they relate to energy efficiency issues thus to find out whether or not the integration of these systems into intelligent buildings provides optimization in energy performance and comfort conditions. The building for the case study, which is an intelligent office building incorporating a double-skin fa&ccedil / ade was selected as one that promises high comfort conditions for the occupants with low energy consumption. The working principles of integrated fa&ccedil / ade systems, together with their advantages and disadvantages were investigated by means of the case study. It was concluded that due to their high initial costs, these systems offer no real advantages for today. However with the inevitable exhaustion of fossil fuels that is foreseen for the future, these systems would become an innovative solution in terms of energy conservation.

Composite International Diagnostic Interview screening scales for DSM-IV anxiety and mood disorders

Background Lack of coordination between screening studies for common mental disorders in primary care and community epidemiological samples impedes progress in clinical epidemiology. Short screening scales based on the World Health Organization (WHO) Composite International Diagnostic Interview (CIDI), the diagnostic interview used in community epidemiological surveys throughout the world, were developed to address this problem. Method Expert reviews and cognitive interviews generated CIDI screening scale (CIDI-SC) item pools for 30-day DSM-IV-TR major depressive episode (MDE), generalized anxiety disorder (GAD), panic disorder (PD) and bipolar disorder (BPD). These items were administered to 3058 unselected patients in 29 US primary care offices. Blinded SCID clinical reinterviews were administered to 206 of these patients, oversampling screened positives. Results Stepwise regression selected optimal screening items to predict clinical diagnoses. Excellent concordance [area under the receiver operating characteristic curve (AUC)] was found between continuous CIDI-SC and DSM-IV/SCID diagnoses of 30-day MDE (0.93), GAD (0.88), PD (0.90) and BPD (0.97), with only 9–38 questions needed to administer all scales. CIDI-SC versus SCID prevalence differences are insignificant at the optimal CIDI-SC diagnostic thresholds (χ2 1 = 0.0–2.9, p = 0.09–0.94). Individual-level diagnostic concordance at these thresholds is substantial (AUC 0.81–0.86, sensitivity 68.0–80.2%, specificity 90.1–98.8%). Likelihood ratio positive (LR+) exceeds 10 and LR− is 0.1 or less at informative thresholds for all diagnoses. Conclusions CIDI-SC operating characteristics are equivalent (MDE, GAD) or superior (PD, BPD) to those of the best alternative screening scales. CIDI-SC results can be compared directly to general population CIDI survey results or used to target and streamline second-stage CIDIs.

Quantum Dissipative Dynamics and Decoherence of Dimers on Helium Droplets

In this thesis, quantum dynamical simulations are performed in order to describe the vibrational motion of diatomic molecules in a highly quantum environment, so-called helium droplets. We aim to reproduce and explain experimental findings which were obtained from dimers on helium droplets. Nanometer-sized helium droplets contain several thousands of 4-He atoms. They serve as a host for embedded atoms or molecules and provide an ultracold “refrigerator” for them. Spectroscopy of molecules in or on these droplets reveals information on both the molecule and the helium environment. The droplets are known to be in the superfluid He II phase. Superfluidity in nanoscale systems is a steadily growing field of research. Spectra obtained from full quantum simulations for the unperturbed dimer show deviations from measurements with dimers on helium droplets. These deviations result from the influence of the helium environment on the dimer dynamics. In this work, a well-established quantum optical master equation is used in order to describe the dimer dynamics effectively. The master equation allows to describe damping fully quantum mechanically. By employing that equation in the quantum dynamical simulation, one can study the role of dissipation and decoherence in dimers on helium droplets. The effective description allows to explain experiments with Rb-2 dimers on helium droplets. Here, we identify vibrational damping and associated decoherence as the main explanation for the experimental results. The relation between decoherence and dissipation in Morse-like systems at zero temperature is studied in more detail. The dissipative model is also used to investigate experiments with K-2 dimers on helium droplets. However, by comparing numerical simulations with experimental data, one finds that further mechanisms are active. Here, a good agreement is obtained through accounting for rapid desorption of dimers. We find that decoherence occurs in the electronic manifold of the molecule. Finally, we are able to examine whether superfluidity of the host does play a role in these experiments. / In dieser Dissertation werden quantendynamische Simulationen durchgeführt, um die Schwingungsbewegung zweiatomiger Moleküle in einer hochgradig quantenmechanischen Umgebung, sogenannten Heliumtröpfchen, zu beschreiben. Unser Ziel ist es, experimentelle Befunde zu reproduzieren und zu erklären, die von Dimeren auf Heliumtröpfchen erhalten wurden. Nanometergroße Heliumtröpfchen enthalten einige tausend 4-He Atome. Sie dienen als Wirt für eingebettete Atome oder Moleküle und stellen für dieseeinen ultrakalten „Kühlschrank“ bereit. Durch Spektroskopie mit Molekülen in oder auf diesen Tröpfchen erhält man Informationen sowohl über das Molekül selbst als auch über die Heliumumgebung. Man weiß, dass sich die Tröpfchen in der suprafluiden He II Phase befinden. Suprafluidität in Nanosystemen ist ein stetig wachsendes Forschungsgebiet. Spektren, die für das ungestörte Dimer durch voll quantenmechanische Simulationen erhalten werden, weichen von Messungen mit Dimeren auf Heliumtröpfchen ab. Diese Abweichungen lassen sich auf den Einfluss der Heliumumgebung auf die Dynamik des Dimers zurückführen. In dieser Arbeit wird eine etablierte quantenoptische Mastergleichung verwendet, um die Dynamik des Dimers effektiv zu beschreiben. Die Mastergleichung erlaubt es, Dämpfung voll quantenmechanisch zu beschreiben. Durch Verwendung dieser Gleichung in der Quantendynamik-Simulation lässt sich die Rolle von Dissipation und Dekohärenz in Dimeren auf Heliumtröpfchen untersuchen. Die effektive Beschreibung erlaubt es, Experimente mit Rb-2 Dimeren zu erklären. In diesen Untersuchungen wird Dissipation und die damit verbundene Dekohärenz im Schwingungsfreiheitsgrad als maßgebliche Erklärung für die experimentellen Resultate identifiziert. Die Beziehung zwischen Dekohärenz und Dissipation in Morse-artigen Systemen bei Temperatur Null wird genauer untersucht. Das Dissipationsmodell wird auch verwendet, um Experimente mit K-2 Dimeren auf Heliumtröpfchen zu untersuchen. Wie sich beim Vergleich von numerischen Simulationen mit experimentellen Daten allerdings herausstellt, treten weitere Mechanismen auf. Eine gute Übereinstimmung wird erzielt, wenn man eine schnelle Desorption der Dimere berücksichtigt. Wir stellen fest, dass ein Dekohärenzprozess im elektronischen Freiheitsgrad des Moleküls auftritt. Schlussendlich sind wir in der Lage herauszufinden, ob Suprafluidität des Wirts in diesen Experimenten eine Rolle spielt.

Die vorliegende Dissertation untersucht die Relevanz von Kontrollüberzeugungen in der intergenerationalen Transmission von sozialem Status. Motiviert ist die Arbeit von der Frage nach der Möglichkeit Chancengerechtigkeit durch die Verringerung herkunftsbedingter Unterschiede in Kontrollüberzeugungen zu erhöhen. Nach einer Einführung des zentralen Konstruktes werden im theoretischen Teil durch die Zusammenführung soziologischer und sozialpsychologischer Theorien potentielle Transmissionsmechanismen erörtert. Der empirische Teil prüft den postulierten Transmissionskanal anhand längsschnittlicher Daten. Um die Möglichkeit der Verringerung herkunftsbedingter Unterschiede in Kontrollüberzeugungen abzuschätzen wurde mit Hilfe von Zwillingsdaten und einer randomisiert kontrollierten Interventionsstudie untersucht wie stark Kontrollüberzeugung genetisch determiniert sind beziehungsweise von sozialen Faktoren innerhalb und außerhalb des familiären Kontextes beeinflusst werden. Die Ergebnisse deuten darauf hin, dass Kontrollüberzeugungen den Einfluss des Herkunftsstatus auf den eigenen Status teilweise mediieren. In Übereinstimmung mit bisherigen Studien weisen die Ergebnisse der Zwillingsstudien darauf hin, dass Kontrollüberzeugungen in hohem sozial determiniert sind. Die Interventionsstudie indiziert ,dass Kontrollüberzeugungen gezielt verändert werden können. Im konkreten Fall konnten externe, fatalistische Kontrollüberzeugungen durch ein Mentoringprogramm verringert werden. Abschließend wird die gesellschaftliche Norm zur Internalität kritisch betrachtet. Sowohl internale als auch externale Kontrollüberzeugungen können adaptiv sein, sofern sie auf realistischen Einschätzungen der tatsächlich vorhandenen Kontrollmöglichkeiten basieren. Die Grenzen der tatsächlich vorhanden Kontrollmöglichkeiten zu erforschen und zu benennen wird als wichtiges Ziel soziologischer Forschung in diesem Bereich herausgestellt. / This dissertation aims to assess the role of locus of control (LoC) in the intergenerational transmission of social status as well as the potential to raise fair equality of opportunity by reducing the social gradient in LoC. After the core concept of locus of control is introduced, the theoretical part explicates the mechanisms through which social status is likely to be reproduced via LoC across generations. Structural equation modeling is used to assess the substantive importance of LoC in the intergenerational reproduction of social status using data from the British Cohort Study 1970. The evidence suggests that LoC partly mediates the influence of parents’ status on own status attainment: Children from low-SES households are less likely to be endowed with the type of LoC that benefits status attainment later in life. Hence LoC is a potential lever for reducing intergenerational status persistence. As the potency of this lever depends on the degree to which LoC is socially formed rather than genetically determined another aim was to provide evidence on the contribution of genetic inheritance and different social actors at different points in the life-span. Towards this end, information from a multi-cohort twin study and a randomized controlled intervention study were analyzed. The evidence suggests that extra-familial influences become more important in determining locus of control across age, and that a low-intensity mentoring program can reduce fatalistic LoC in low-status children, but not their overall locus of control. The overall discussion reflects upon a general social norm of internality. It is argued that internality and externality are functional if they are based on a realistic assessment of the boundaries of control. Providing empirical evidence for these boundaries is endorsed as an important goal for social scientific research.
