The Use of Gesture in Self-Initiated Self-Repair Sequences by Persons with Non-Fluent AphasiaFeltner, Eleanor M. 01 January 2016 (has links)
This study examines the relationship between types of gestures and instances of self-initiated self-repair (SISR) used by persons with non-fluent aphasia (NFA), which is a type of aphasia characterized by stilted speech or signing (Papathanasiou et al., 2013), in interactions with clinicians. Conversation repairs in this study are assessed using the framework of Conversation Analysis (CA), which is an approach for describing, analyzing, and understanding social interaction (Sidnell, 2010). Previous linguistic studies have demonstrated a distinct preference for the use of gesture during a repair by persons with aphasia (Goodwin, 1995; Klippi, 2015; Wilkinson, 2013). This study draws more conclusive generalizations than previous studies about the types of gesture used in successful and unsuccessful SISR by persons with NFA through the use of the AphasiaBank corpus. Results show that there does not appear to be a connection between the overall frequencies of gesture used by persons with NFA during a phase of the repair mechanism as compared to other phases in the repair mechanism. Additionally, there is a slight tendency in this dataset for persons with NFA to have more successful repairs when they use gesture during the initiation and reparable portions of the repair mechanism.

A nomeação constitui uma das tarefas mais importantes no processamento da linguagem. A nomeação de diferentes categorias semânticas e gramaticais diverge em suas propriedades lexicais e possui substratos neuroanatômicos distintos. Esta pesquisa tem como objetivos caracterizar as alterações de nomeação de sujeitos com afasia, comparando-os aos sujeitos sem alteração de linguagem e estabelecer relações da tipologia de resposta na tarefa de nomeação de verbos e de substantivos em afásicos. A amostra foi composta por indivíduos divididos em dois grupos: GC e GP. O GC foi constituído de 95 sujeitos sem alteração de linguagem, com média de idade de 56,7 anos (15,4) e escolaridade de 9,3 anos (4,4). O GP foi constituído por 33 afásicos, subdividido em não fluentes (GNF) e fluentes (GF). No GNF foram avaliados 17 sujeitos com média de idade de 65,5 anos (14) e escolaridade de 8,2 anos (6,4). No GF foram avaliados 16 sujeitos com média de idade de 60,5 anos (16,7) e escolaridade de 11,5 anos (7,6). O teste utilizado foi a bateria inglesa An Object and Action Naming Battery (Bateria de Nomeação de Objetos e Verbos - BNOV). Houve diferenças estatisticamente significativas entre os grupos de afásicos no número de acertos para substantivos e verbos (p<0,001). O número de acertos para a nomeação de substantivos é maior do que para verbos no GNF enquanto o número de acertos para a nomeação de verbos é maior no GF. Os sujeitos do GNF apresentaram como tipo de resposta desviante mais frequente a \"complementação por gesto\", seguido dos tipos xiii \"hiperônimo\" e \"componente\" na nomeação de substantivos e \"erros de associação\", \"co-hipônimo\", e \"substantivo relacionado\", na nomeação de verbos. Os sujeitos do GF apresentaram como tipo de resposta desviante mais frequente o \"neologismo\", seguido de \"parafasia fonêmica\" e \"circunlóquios\" para a nomeação de substantivos e \"neologismo\", \"erros fonológicos\" e \"circunlóquios\" para verbos. / Naming is one of the most important functions in language processing. The naming of different semantic and grammatical categories diverge in their lexical properties and possess distinct neuroanatomical substrates. This study aims at characterizing the naming alterations and types of errors of aphasic individuals in a verbs and nouns naming. The sample was composed of subjects divided into two groups: GC and GP. The GC consisted of 95 subjects with no language impairment, with the mean age of 56.7 years (15.4) and schooling of 9.3 years (4.4). The GP comprised 33 aphasic patients, subdivided into non fluent (GNF) and fluent (GF). In the GNF, 17 subjects with the mean age of 65.5 years (14) and schooling of 8.2 years (6.4) were evaluated. In the GF, 16 subjects with the mean age of 60.5 years (16.7) and 11.5 years of schooling (7.6) were evaluated. The test used was the English battery An Object and Action Naming Battery. The most frequent type of error presented by the GF was \"neologism\", followed by \"phonemic paraphasia\" and \"circumlocutions\" in the naming of nouns and \"neologism\", \"phonological errors\" and \"circumlocutions\" for verbs.

manifestation which occurs in some severe cases of aphasia. It can be
characterized, according to medical and phonoaudiological literature, as
repetitive and crystallized speech segments. Such insistent enigmatic
productions tend to be unintelligible or uninterpretable by the other. The
jargonaphasic speech requires a clinical approach less normative. It requires an
accurate therapeutic listening to what is unique in its surprising occurrence. The
final clinical aim should be to open the possibility of giving "voice and position"
in language to the aphasic subject (FONSECA, 2002). In the clinical literature,
few studies have been devoted to the so called jargon speech and very few
have attempted to reflect upon its implications for speech therapy. In fact, it is
emphasized that this symptomatology stands as a "barrier" for therapy. The
present study discussed jargonaphasic speech from a specific linguistic theory,
i.e., European structuralism, an attempted to offer a solid interpretation for the
subject-language relationship in jargonaphasic clinical cases.
Interactionism (DE LEMOS, 1992, 2002) and Language Clinic (LIER-DeVITTO,
2000) were the essential theoretical sources. The state of art in jargonaphasia
field was discussed and also particular issues, such as the vagueness of
definitions and the descriptive and classificatory biases, which are also strong in
the area, were dealt with. An important critical step was given towards
traditional therapeutic approaches and an alternative view offered to the speech
therapy setting. An expressive amount of clinical segments were discussed to
sustain theoretical assumptions assumed in this thesis, which suspends
grammatical descriptions in favor of an explanatory trend which involves the
functioning of language (SAUSSURE, 1916, JAKOBSON, 1960) and the
hypothesis of the unconscious (FREUD, 1900) / O jargão é uma manifestação linguística presente em alguns casos de afasia,
que se caracteriza por segmentos restritos de fala cristalizada e repetitiva,
segundo a literatura médica e fonoaudiológica. Nesses casos, o que o paciente
diz são fragmentos ininteligíveis ou pedaços de fala que insistem em seus
enunciados. A fala jargonafásica indaga um clínico de linguagem que pretende,
como direção de tratamento, dar vez e voz ao afásico (FONSECA, 2002). Na
literatura clínica, são raros os trabalhos que têm se dedicado à esta
manifestação de fala com jargão e poucos se detido na tentativa de pensar a
respeito de suas implicações no tratamento fonoaudiológico. Na verdade,
destaca-se que esta sintomatologia grave é uma barreira , uma
impossibilidade para a terapêutica. Sendo assim, este trabalho problematiza a
fala jargonafásica a partir de uma teorização linguística particular, que tem no
cerne a interpretação da relação sujeito-linguagem (língua e fala), como
proposto pelo Interacionismo (DE LEMOS, 1992, 2002 e outros) e a Clínica de
Linguagem (LIER-DeVITTO, desde 2000). Apresento o estado da arte na
jargonafasia, com discussão de questões sobre as descrições e classificações,
que dão suporte a diferentes abordagens fonoaudiológicas. Com a Clínica de
Linguagem, e com base nos materiais clínicos, destaco a importância de um
refinamento teórico-clínico necessário para a construção de uma escuta, que
tome o lugar do apoio a métodos científicos de descrição e classificação.
Abordo a possibilidade de apreender o funcionamento linguístico particular que
comanda cada fala afásica e seus efeitos no sujeito. Esse trabalho perpassa,
ainda, uma reflexão sobre a escuta e seus efeitos na fala afásica

In the speech therapy clinic, patients complaints usually involve difficulties related to talking. However, problems related to reading and writing can be quite surprising and demonstrate that the main issue of aphasic patients is related to the speech process. Reading and writing are, like talking, subjective activities and, as such, a rich area for therapeutic approaches. The objectives of this study are to (1) review the literature and comment on the different clinical presentations of aphasia in relation to talking, reading and writing; (2) present and discuss the effect of this context in the Speech Therapy Clinic; (3) compare these findings with the ones that the Research Group Aquisição, patologias e clínica de linguagem support (Group Coordinated by Prof. Maria Francisca Lier-De Vitto no LAEL/PUC-SP); (4) discuss clinical issues related to the implementation of activities that involve not only talking but also reading and writing. For this purpose, I present a report of four case studies. It is important to remark that many other researchers and scientists have also evidenced and described many interesting findings in this group of patients, and this was the motivation of this study. Nevertheless, reports are much more restricted to the observations of signs and clinical presentations, with little appraisal of the relation with reading and writing, other symptoms and therapeutic approaches. In conclusion, impasses and clinical handering of aphasic patients demand more discussion and attention in the literature / Afásicos, via de regra, apresentam perturbações na fala em função de lesão cerebral. Na clínica fonoaudiológica de linguagem, a queixa dos pacientes costuma restringir-se às dificuldades relacionadas à fala. Porém, a heterogeneidade e imprevisibilidade das perturbações sobre a relação entre fala-leitura-escrita são surpreendentes nas afasias e nos permitem afirmar que o problema de afásicos é, sem dúvida, com a linguagem. Leitura e escrita são, como a fala, espaços de realização subjetiva o que, conseqüentemente, aponta para sua relevância como recurso terapêutico, permanecendo, muitas vezes, como espaço(s) exclusivo(s) de manifestação lingüística e subjetiva do paciente e, portanto, como condições privilegiadas na situação clínica. Sendo assim, este trabalho objetiva (1) situar e comentar os diferentes modos sintomáticos de relação fala-leitura-escrita instaurados em quadros afásicos, conforme abordados na literatura neurológica e neuropsicológica; (2) apresentar e discutir os efeitos desse raciocínio na Clínica Fonoaudiológica; (3) procurar discernir esse tipo de raciocínio do que embasa o Grupo de Pesquisa Aquisição, patologias e clínica de linguagem coordenado pela Profa. Dra. Maria Francisca Lier-DeVitto no LAEL/PUC-SP; (4) discutir questões clínicas relacionadas à implementação de procedimentos que envolvem, além da fala, também leitura e escrita. Para isso, apresento quatro relatos de casos. De minha leitura crítica vale destacar que os pesquisadores (afasiologistas e fonoaudiólogos) investigados notaram e registraram os mesmos acontecimentos surpreendentes e misteriosos na fala/leitura/escrita que me impulsionaram nesta pesquisa. Porém, esta observação permitiu apenas que eles observassem sinais da doença (compondo o quadro nosográfico das afasias) mas não refletissem sobre a relação afásico-leitura/escrita, nem sobre os efeitos clínicos dos atendimentos e/ou manifestações sintomáticas. Impasse e manejos clínicos no atendimento de afásicos não encontram, portanto, espaço para discussão fora da Clínica de Linguagem

Previous issue date: 2007-01-26 / Aphasia is a disorder that affects the language in its receptive and expressive, syntactic, semantic and morphologic aspects. The most common cause is the
cerebrovascular accident (CVA). The authors of studies about the clinical-topographical correlations of the aphasias have been presenting variable results regarding the cerebral location of the language. The objective of this study was to establish the clinicaltopographical
correlations of the fluent and non-fluent aphasias, observing the contributions of Spect scanning to map the affectec areas in the aphasias. A total of 29 patients with
aphasia due to first episode of CVA comprised the study. They were submitted to language evaluation by means of selected proofs of the Boston Diagnosis Aphasias Examination
(BDAE). Spect scanning was used to evaluate perfusion alterations in the whole encephalon , and it was analyzed by a nuclear doctor and a radiologist, in order to supply
fidelity to the location data. After clinical evaluation, it was observed that 62% of the patients presented non-fluent aphasia and 38% fluent aphasia. It was not possible to classify 17% of the sample according to the classic types of aphasia. The analysis of the main components based on covariances allowed to relate the non-fluent aphasias mainly to the left and right frontal regions , left temporal region, presence of contralateral cerebelar
diasquise to the left, subcortical regions , among others. The fluent aphasias are related to the inferior and superior left parietal regions . These data have corroborated for the concept of neural net in which several cortical , subcortical and cerebelar regions are involved to
perform a linguistic task. In this way, this study showed that other cerebral regions participate in the language process as a neural circuit. Cerebral Spect provided different xi contributions from other image methods, adding important information on the participation of subcortical and cerebelar areas in the language process. / A afasia é uma desordem que afeta a linguagem nos seus aspectos receptivo e expressivo, sintático, semântico e morfológico. Sua causa mais comum é o acidente vascular cerebral (AVC). Os autores de estudos sobre as correlações clínico-topográficas das afasias apresentam resultados variáveis quanto à localização cerebral da linguagem. O
objetivo desse trabalho foi estabelecer as correlações clínico-topográficas das afasias fluentes e não fluentes, verificando as contribuições do Spect cerebral para mapear as áreas
comprometidas nas afasias. Foram sujeitos desse estudo 29 pacientes com afasia decorrente de primeiro episódio de AVC, os quais foram submetidos à avaliação de linguagem por
meio de provas selecionadas do Teste de Boston para o diagnóstico das afasias (BDAE). O Spect cerebral foi utilizado para avaliar alterações perfusionais em todo o encéfalo e foi
analisado por médico nuclear e radiologista, a fim de fornecer fidelidade aos dados de localização. Após avaliação clínica, observou-se que 62% dos pacientes apresentaram afasia não fluente e 38% afasia fluente. Não foi possível classificar 17% da amostra dentro
dos tipos clássicos de afasia. A análise dos componentes principais com base em
covariâncias permitiu relacionar as afasias não fluentes principalmente às regiões frontal
esquerda e direita, temporal esquerda, presença de diásquise cerebelar contralateral à
esquerda, regiões subcorticais, entre outras. As afasias fluentes estão relacionadas às
regiões parietal inferior e superior esquerda. Esses dados corroboram para o conceito de
rede neural nas quais estão envolvidas várias regiões corticais, subcorticais e cerebelares
para a execução de uma tarefa lingüística. Dessa forma, este estudo mostrou que outras
regiões cerebrais participam do processamento da linguagem como um circuito neural. O
Spect cerebral ofereceu contribuições diferentes das obtidas por outros métodos de imagem

e acrescentou informações importantes sobre a participação de regiões subcorticais e
cerebelares no processamento da linguagem.

subjects and searches the construction of therapeutic processes that can maximize the use of
discursive potentialities; even within the suffering traces implied in aphasia and the
personal and social changes usually resultant from it. This research is divided into three
parts: studies of aphasia, discursive genres and the occurrence of those genres among
aphasic subjects in a specific group. This is a longitudinal, descriptive study, which was
carried out at Centro de Convivência de Afásicos (Aphasics Living Centre - CCA) - IEL,
University of Campinas: a group of aphasics and non-aphasics built inside a discursive
environment where the language (practised here) is established within its various
occurrences. The data used in this research were built at CCA and come from weekly notes
and VHS recordings, orthographically transcribed and analysed on a qualitative basis,
included in the Neurolinguistic Data Bank (CNPq: 521773/95-4). The analysis of the data
shows how the subjects make use of discursive genres to reconstruct the suffered
language after aphasia and, this way, produce meanings through oral verbal expressions as
well as non-verbal expressions, by sharing the dialogical relationship practised in this
environment. It was possible to conclude that the use of discursive genres helps subjects
with aphasia, when this disorder does not hinder the individual from using the genres and
circulating within them in dialogical relationships. Thus, the use of genres can be assumed
as a focus of therapeutic strategies, as long as the therapist is based on a discursively
oriented language conception and sees the clinical procedures as a process of acceptance,
hearing and technical-scientific support for the patient to reflect upon/ re-elaborate his
suffering and the language changes resultant from aphasia / Este trabalho tem por objetivo analisar a ocorrência dos gêneros discursivos entre
sujeitos afásicos, buscando pensar na construção de processos terapêuticos que maximizem
o uso de potencialidades discursivas, ainda que em meio a marcas de sofrimento implicadas
com a afasia e com as mudanças pessoais e sociais que costuma acarretar. Esta pesquisa
está dividida em três partes: os estudos da afasia, os gêneros discursivos e a ocorrência
destes gêneros entre os sujeitos afásicos de um determinado grupo. Trata-se de um estudo
longitudinal e descritivo, realizado no Centro de Convivência de Afásicos (IEL-Unicamp),
um grupo de afásicos e não afásicos construído em um ambiente discursivo onde a
linguagem (que aqui se pratica) se estabelece, em suas variadas formas de acontecimento.
Os dados foram aí construídos por meio de registros semanais e de gravações em fitas
VHS, transcritos ortograficamente e analisados qualitativamente, inseridos no Banco de
Dados em Neurolingüística (CNPq: 521773/95-4). A análise dos dados mostra como os
sujeitos recorrem aos gêneros discursivos para reconstruir a linguagem sofrida após a
afasia e, assim, produzir significados em expressões verbais oralizadas e não-verbais,
partilhando da relação dialógica que se pratica nesse ambiente. Pôde-se concluir que o uso
do gênero discursivo auxilia o sujeito com seqüelas afásicas, quando estas não são
impeditivos ao uso de diferentes gêneros e à circulação por eles nas relações dialógicas.
Sendo assim, o uso dos gêneros pode ser assumido como eixo de estratégias terapêuticas,
desde que o fonoaudiólogo se paute por uma concepção de linguagem discursivamente
orientada, bem como entenda a clínica como processo de acolhimento, escuta e suporte
técnico-científico à elaboração, pelo paciente, do sofrimento e das alterações de linguagem
decorrentes da afasia

Previous issue date: 2011-12-02 / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / I have always wondered about the reception of aphasic patients in speech therapy clinic: about whether (and how) the establishment of aphasia that undoubtedly weakens the human existence, would involve the specifics if the subject were experiencing the entry in the old age. The aphasia is "talk in suffering that hurts the subject" (FONSECA, 1995, 2002). The subjective drama that is imposed to the aphasic is related to the difficulty of sustaining itself as a speaker facing each other. To that extent, the aphasia can lead to isolation and to the weakening of social ties. The issue that involves the discussion about the weakening of old age is current in the reflection of Gerontology, although the dominant trend is to approach it through the body. In view of my clinical experience, I recognized the need to discuss this concept in a perspective that takes into account the notion of "body inhabited". Clarify, then, the objectives that guided the implementation of this master s thesis: I) to investigate the link between aphasia and old age with the theme underlying the debate a reflection on the polarity of health/ disease in the aging process; II) To discuss the specificity of the notion of weakness when what is at stake is the incidence of aphasia in old age; III) To raise points relating to the necessary convergence between clinical and nonclinical interventions in the care of elderly; IV) To explore the concept of clinical in Social Gerontology. To do this, I realized (1) a literature review of studies that deal with the concepts of health, illness and fragility both in the field where they were forged (medicine) and in Gerontology. Depending on the issue problem that motivate this study, was also concerned; (2) a thorough reading of theoretical clinical perspective of aphasia enacted in research groups (CNPq) "Acquisition, pathologies and language Clinic" (coordinated by profas. Dras. Maria Francisca Lier-DeVitto and Lucia Arantes, LAEL-PUCSP) and "The weakening of old age and the Clinical Practice in the field of Gerontology" (coordinated by profa. Dra. Suzana Carielo da Fonseca). With this specific training, I revisited the serving Lady d. speech therapies that several times made me wonder how much particulars on the aging process and elaborations on it might be involved in the demands and expectations that she told me. Inquiry that included the need for clarification about the nature of the clinical management of something that indeed exceeded the request of D. to get rid of the symptoms in language. That is to say that if changes have been operated by the crossover effect of clinical care with the effects of participation of Ms. D. in social inclusion programs of the treatment Center to aphasics (Caaf- DERDIC/PUCSP), they don t imply the reversibility of the symptomatic condition. All these issues were discussed from the link of the review of relevant literature to the subject (Chapters I, II and III with the case study , as a methodological option. At the end, I take this research endeavor the possibility of to grasp specificities that particularize the clinical practice in the field of gerontology / Sempre me questionei sobre o acolhimento dos pacientes afásicos na clínica fonoaudiológica: se (e como) a instauração da afasia que, indubitavelmente, fragiliza a existência humana, implicaria especificidades caso o sujeito estivesse experimentando a entrada na velhice. A afasia é fala em sofrimento que faz sofrer o sujeito (FONSECA, 1995, 2002). O drama subjetivo que se impõe ao afásico está relacionado com a dificuldade de sustentar-se como falante frente ao outro. Nessa medida, a afasia pode levar ao isolamento e à fragilização do laço social. A questão que envolve a discussão sobre a fragilização da velhice é atual na reflexão da Gerontologia, embora a tendência dominante seja de abordá-la pela via do organismo. Tendo em vista minha experiência clínica, reconheci a necessidade debater esse conceito numa perspectiva que leve em conta a noção de corpo habitado . Esclareço, então, os objetivos que orientaram a realização desta dissertação de mestrado: I) investigar a articulação entre a afasia e velhice, tendo como eixo articulador do debate uma reflexão sobre a polaridade saúde/doença no processo de envelhecimento; II) discutir a especificidade da noção de fragilidade, quando o que está em causa é a incidência de afasia na velhice; III) levantar pontos relativos à necessária convergência entre intervenções clínicas e não clínicas no atendimento de idosos frágeis; IV) explorar o conceito de clínica no âmbito da Gerontologia Social. Para tal, realizei (1) uma revisão bibliográfica de trabalhos que problematizam os conceitos de saúde, doença e fragilidade, tanto no campo em que eles foram forjados (a Medicina), quanto na Gerontologia. Em função da questão-problema que motivou a realização deste estudo, também esteve em causa (2) uma leitura aprofundada da perspectiva teórico-clínica da afasia, instaurada nos Grupos de Pesquisa (CNPq) Aquisição, Patologias e Clínica de Linguagem (coordenado pelas profas. Dras. Maria Francisca Lier-DeVitto e Lúcia Arantes, no LAEL-PUCSP) e A Fragilização da Velhice e o Exercício Clínico no Campo da Gerontologia (coordenado pela profa. Dra. Suzana Carielo da Fonseca). Com essa formação específica, revisitei o atendimento fonoaudiológico da senhora D. que, por diversas vezes, fez me questionar o quanto particularidades relativas ao seu processo de envelhecimento e elaborações relativas a ele poderiam estar em causa nas demandas e expectativas que ela me dirigia. Indagação que incluía a necessidade de esclarecimento acerca da natureza do manejo clínico de algo que, a rigor, excedia o pedido de D. de se livrar do sintoma na linguagem. Vale dizer que se mudanças foram operadas por efeito do entrecruzamento do atendimento clínico com os efeitos da participação da senhora D. em programas de inclusão social do Centro de Atendimento a Afásicos (CAAf - DERDIC/PUCSP), elas não implicaram reversibilidade da condição sintomática. Todas essas questões foram abordadas a partir da articulação da revisão bibliográfica pertinente ao tema (Capítulos I, II e III) com o estudo de caso , como opção metodológica. Ao final, retiro deste empreendimento de pesquisa a possibilidade de apreender especificidades que particularizam o exercício clínico no campo da Gerontologia

Previous issue date: 2017-05-26 / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES / The subject matters dealt with in this study are clinical in their nature since it was stimulated by three different patients’ abandonment of treatment, which led me to reflect on the problem of patient's commitment to therapy. It is well known that such a commitment is strictly linked to the relationship between the complaint – demand related notions. Concerning the “complaint” it is expected that patient offers reasons for his suffering when he tries to say "what he has". In the Language Clinic, suffering has to do with (is caused) a symptom in speech, i.e., to an unbearable pain related to speech. The “demand”, in its turn, is the request made to the other therapist, to whom the patient supposes knowledge about his symptomatic condition. In the present work, I approach themes related to the aphasic symptom; to case study presentations in the clinical literature (medical, psychoanalytical) and, of course, to complaint and demand in the Language clinic with aphasic patients. I bring Freud (1891) to oppose his notion of symptom in aphasia to the medical viewpoint on the subject - a deep difference that is of great relevance for the Language Clinic. Fonseca (1995) pioneered the criticism concerning the organicist discourse on aphasia which supports the idea that symptoms in speech are directly caused by the cerebral damage. She postulated that, for a language therapist, aphasia is basically a language problem that affects the aphasic speaker. The author proposes clinical procedures consistent with that statement. Following this trend of thought, I considered important to discuss the question: "what is a clinical case". In other words, I tried to demonstrate how case studies are approached in the medical field and in the psychoanalytic field and presented a brief overview of how "cases" (clinical materials) have been dealt and written in the Language Clinic with aphasics. Finally, I present and discuss segments of therapies conducted by. They were the start-point for the present study / As questões que impulsionaram este trabalho nasceram de abandonos de tratamento, que me levaram a refletir a respeito da problemática do compromisso do paciente com a terapia, que se estabelece e se discute na esfera da relação queixa – demanda. Na primeira, o paciente oferece razões para o seu sofrimento, ao procurar dizer “o que tem”. Na Clínica de Linguagem, o mal-estar liga-se a um problema na fala, a um incômodo insuperável com ela. A demanda, por sua vez, é o pedido feito ao outro-terapeuta, a quem o paciente supõe um saber sobre o que se passa com ele. Neste trabalho, abordo temas referentes ao sintoma, caso e, por certo, queixa e demanda na Clínica de Linguagem com afásicos. Trago Freud (1891) para contrapor sua noção de sintoma nas afasias com a perspectiva médica, gesto que é de grande importância para a Clínica de Linguagem. Fonseca (1995) foi pioneira na sustentação da crítica à adesão do fonoaudiólogo ao discurso médico da causalidade lesão-sintoma. Ela postula que, para um clínico de linguagem, a afasia é um problema na linguagem que afeta o sujeito e propõe procedimentos clínicos consistentes com esta afirmação. Na medida em que fiquei sob efeito de atendimentos, considerei importante discutir questões relacionadas a “o que é um caso clínico”. Ou seja, procurei mostrar como o estudo de caso é visto no campo médico e no campo psicanalítico. Comentei, ainda, o modo como “casos” (materiais de atendimentos clínicos) têm sido trabalhados e escritos na Clínica de Linguagem com afásicos. Apresento e discuto, por fim, segmentos dos atendimentos que conduzi e que me interrogaram, dando margem ao delineamento do assunto desta dissertação
