Reliability and Validity of Body Composition and Bone Mineral Density Measurements by DXAZack, Melissa Kareen 18 April 2002 (has links)
Dual energy X-ray absorptiometry (DXA) has been well established in both clinical and research settings for measurement of bone mineral density (BMD), and is becoming more widely utilized for assessment of body composition. Reliability and validity are essential factors in both applications of this technique; however, neither have been confirmed for the QDR-4500A DXA at Virginia Tech. Therefore, measurements of the whole body (WB), lumbar spine (LS), total proximal femur (TPF) and total forearm (TF) were made in a group of young-adult males and females at two time-points, 5-7 days apart. Significant differences were not found in BMD (g/cm2) at these body sites with repeated measurements by DXA. Furthermore, measures of percent body fat (%BF), lean body mass (LBM), and fat mass (FM) by DXA were reliable. Validity of %BF by DXA was assessed from comparison to single-frequency bioelectrical impedance analysis (BIA). Significant differences were not found in measures of %BF by DXA and BIA. A second study investigated the reliability and validity of the QDR-4500A DXA in measurements of distal tibia (DT) BMD. Significant differences were not found between repeated measurements. Validity was established by a significant correlation between WB BMD and DT BMD. A third study examined the influence of navel jewelry on the accuracy of LS DXA measurements. Repeated measurements with a spine phantom revealed that both a navel ring and a barbell produced significantly greater measures of LS BMD compared to the spine phantom alone. Manual correction of navel jewelry did not eliminate BMD inaccuracies. Data from these studies confirmed that the QDR-4500A DXA at Virginia Tech was a reliable and valid device in measurement of WB, LS, TPF, TF and DT BMD, as well as %BF, LBM, and FM. In addition, effects of navel jewelry on LS BMD have been recognized. Further studies investigating the reliability and validity of DT BMD measures as well as effects of different types, gauges, and shapes of body jewelry on BMD measures in human subjects are warranted. / Master of Science

Untersuchungen zur Bestimmung der Körperzusammensetzung wachsender Ziegenlämmer in vivo mit Hilfe der D2O- VerdünnungsmethodeTreitel, Ulla 05 March 2009 (has links)
Die Anwendbarkeit der Verdünnungsmethode mit Hilfe des Markers Deuteriumoxid (D2O) zur in vivo-Erfassung der sich ändernden Körperzusammensetzung wurde an 53 Ziegenlämmern der Rasse Bunte Deutsche Edelziege im Gewichtsabschnitt von 4 kg bis 20 kg LM überprüft. Bei einer Lebendmasse von 4, 8, 12, 16 bzw. 20 kg wurde den Tieren nach einer 13- bis 18stündigen Nüchterung D2O injiziert und nach weiteren vier bis fünf Stunden Blutproben gezogen. Bei jeweils einem Teil der Tiere erfolgte anschließend die Schlachtung und die chemische Analyse der Körper zur Bestimmung des Ganzkörperwassergehaltes, der Chymusmasse und der Leerkörperzusammensetzung. Die Messungen der D2O-Konzentrationen in den Blutproben und die chemische Analyse der Ganzkörper ergaben eine Überschätzung des Ganzkörperwassergehaltes durch die Verdünnungsmethode um im Mittel 3,99 %. Zwischen dem chemisch bestimmten und dem geschätzten Wert besteht eine enge lineare Beziehung. Die Schätzung des Inhaltes des Gastro-Intestinaltraktes (Chymus) dient zur Schätzung der Leerkörpermasse. Dessen Variabilität lässt sich im Wesentlichen durch die Lebendmasse erklären. Die chemische Analyse der Tierkörper zeigte, dass die Zusammensetzung des fettfreien Leerkörpers im hohen Maß von der Leerkörpermasse abhängt. Anhand von allometrischen Regressionsgleichungen wurden die Bestandteile der fettfreien Leerkörpermasse geschätzt und daraus die Zusammensetzung des fetthaltigen Leerkörpers ermittelt. Der Vergleich der mittels chemischer Analyse bestimmten bzw. der Verdünnungsmethode geschätzten Leerkörperzusammensetzung ergab eine hohe Übereinstimmung beim Wasser-, Rohprotein- und Rohaschegehalt. Der Rohfettgehalt wies dagegen größere Abweichungen auf. Als Ursache wurde die Streuung der geschätzten Chymusmasse bzw. Chymuswassermasse aufgedeckt. Das Fazit ist, dass sich die D2O-Verdünnungsmethode zur in vivo-Bestimmung der sich ändernden Körperzusammensetzung von Ziegenlämmern im Gewichtsabschnitt von 4 kg bis 20 kg LM eignet. / On young goats (breed Bunte Deutsche Edelziege) with a live weight range of 4 kg up to 20 kg the suitability of the dilution method using the marker deuterated water (D2O) was checked in order to measure the changes of body composition during growth on living animals (in vivo). At a live weight of 4, 8, 12, 16 and 20 kg the animals received an intrave¬nous dose of D2O after a fastening time of 13 to 18 hours. Blood samples were taken after a waiting period of 4 to 5 hours. In connection with the last blood sample in each period a part of the animals were slaughtered to determine the total body water content, the digesta mass and the empty body composition by chemical analysis. The determination of the marker concentration in the blood samples and the chemical analysis of the total bodies showed that the dilution method overestimated total body water content by 3,99 %. There is a close linear relationship between the estimated and the chemically determined content of total body water. The estimation of the digesta mass serves for the estimation of the empty body mass. Its variation mainly depends on the live weight. The chemical analysis of the animal bodies revealed that the composition of the fat-free empty body is mainly dependent on the empty body mass. Using allometric equations the components of the fat-free empty body mass were estimated and thus the composition of the empty body could be calculated. The comparison of the chemically determined components with the estimated components of the empty body showed a high agreement concerning the content of water, protein and ash. However estimation of the fat content is less accurate. The reason is the variation of the estimated digesta mass and the digesta water mass. It can be concluded that the dilution technique using deuterated water is a practicable method to determine in vivo the changing body composition of growing kids with a live weight range of 4 kg up to 20 kg.

Objectives: The aim of this thesis is to compare values of gained percentage of total body fat by descriptive method such as bioelectrical impedance analysis and skinfolds measuring in comparison with "gold standard method" DEXA and to evaluate the usability of each method of body composition measuring by five groups of women, devided according to BMI level (in range 20-47 kg/m2 ). Title: Evaluation of body composition analysis using anthropometric measurements and methods based on bioimpedance Aims: The main goal of this study is to analyze correlations of the data from bioimpedance with direct anthropometric measurements using statistical data analysis taking into consideration that the latter data is recognized as a gold standard. Methods: The sample of 42 men and 20 women was evaluated using standard anthropometric techniques. In addition, the dimensions, which are necessary to establish the weight of segments in upper and lower arms. The obtained data was firstly evaluated using a universal software ANTROPO. The both samples were concurrently investigated using the bioimpedance methods and the relationships between the aforementioned data and data from InBody was evaluated by three independent statistical methods such as substantive significance (Cohen's d), two-parameter linear regression and robust Passing- Bablok regression. Results: The results obtained enable to conclude that (with the exception of body height and BMI) the remaining parameters do not sufficiently correlate when using anthropometric measurements and data from InBody. Introdução: O nascimento de recém-nascidos pré-termo tardios (RNPT-T), geralmente tratados como \"quase termo\", vem aumentando nas últimas décadas. Vários fatores são atríbuídos à sua etiologia e contribuem para aumentar as taxas de morbimortalidade perinatal e infantil. Atualmente se dispõe de poucos estudos sobre esses recém-nascidos, especialmente sobre o seu crescimento. Objetivos: Analisar o crescimento de RNPT-T nos seis primeiros meses de idade corrigida. Métodos: Estudo observacional, analítico de uma coorte de 64 RNPT-T, coletados nos quatro maiores hospitais de Cuiabá, entre janeiro e setembro de 2013 e acompanhados até 6 meses de idade corrigida. Os dados foram coletados ao nascer, às 40 semanas, 3 e 6 meses de idade corrigida. Foram realizadas análises de correlação entre os parâmetros antropométricos perímetro braquial (PB) e prega cutânea tricipital (PCT) em função dos índices antropométricos PB/PC (perímetro cefálico), peso/comprimento (P/C) e Índice de Massa Corpórea (IMC) e realizada regressão linear múltipla. Resultados: Ao nascimento, 81,3% foram classificados como adequados para a idade gestacional, com a média de peso de 2343,80g ± 430,50. Todos os recém-nascidos apresentaram crescimento contínuo nos seis meses de idade corrigida para os valores de peso, comprimento e perímetro cefálico (PC). Já os valores médios de PCT, de PB/PC e de IMC não mostraram diferença aos 6 meses de idade corrigida. Todos os coeficientes de correlação (r) dos parâmetros PCT e PB em relação aos índices antropométricos apresentaram significância estatística, sendo os valores mais altos para PB versus PB/PC e P/C e menores para PB versus IMC. No modelo final da regressão linear múltipla da PCT, o PB/PC contribuiu significantemente para predizer o depósito de gordura ao nascimento, às 40 semanas e aos 3 meses de idade corrigida (33,8%%, 46,07% e 18,08% respectivamente); aos 6 meses o melhor preditor foi a razão P/C (10,45%). O melhor preditor para o PB foi o PB/PC, que permaneceu no modelo com valores de 73,71%% em todos os tempos avaliados. Conclusão: Os parâmetros peso, comprimento, perímetro cefálico e perímetro braquial e a relação P/C aumentaram nos quatro tempos de aferição, enquanto o PCT e as relações PB/PC e IMC não se modificaram do primeiro para o segundo trimestre e o PB/PC foi o melhor preditor do depósito de gordura subcutânea tricipital (PCT) e de massa gorda e muscular (PB) / Introduction: The birth of newborns late preterm (RNPT -T), usually treated as \"near term\", has been increasing in recent decades. Several factors are attributed to its etiology and contribute to increased rates of perinatal and infant morbidity and mortality. Currently there are few studies of these newborns, especially about their growth. Objectives: To analyze the growth pattern of late preterm infants from birth to six months of corrected age. Methods: This cohort study included 64 RNPT-T, in four major hospitals in Cuiabá, between January and September 2013 and followed until 6 months corrected age. Data were collected at birth, at 40 weeks, 3 and 6 months corrected age. Correlation analyzes between anthropometric parameters mid- arm circumference (MAC) and triceps skinfold (TS) on the basis of anthropometric indices MAC / HC (head circumference), weight / length (W / L) and body mass index (BMI) were performed multiple linear regression. Results: At birth, 81.3 % were classified as appropriate for gestational age, with the average weight of 2343.80 ± 430.50 g. All newborns had a continuous growth in the six months corrected age for the values of weight, length and head circumference (HC). The average value of TS values of MAC / HC and BMI showed no difference at 6 months of corrected age. All correlation coefficients (r) of the TS and MAC parameters in relation to anthropometric indices showed statistically significant, with the highest values for MAC versus MAC / HC and W / L and lower for MAC versus BMI. At the end of the TS model of multiple linear regression, the MAC / HC contributed significantly to predict the deposition of fat at birth, at 40 weeks and at 3 months\' corrected age (33.8 %, 46.07 % and 18.08 % respectively); at 6 months was the best predictor of the ratio W/ L (10.45%). The best predictor for the MAC was the MAC / HC, which remained in the model with values of 73.71 % at all evaluated times. Foi comparada a massa óssea e a composição corporal dos hemicorpos dominante e não dominante de um grupo (A) de 16 pessoas com paralisia cerebral com hemiplegia espástica, e de um grupo (B) de 29 voluntários normais por meio da mensuração da massa corporal, estatura e densitometria óssea do corpo total com composição corporal. Foi encontrada diferença estatística significante no conteúdo mineral ósseo dos membros superiores do grupo A e dos membros superiores e inferiores, tronco e total do grupo B; na massa magra dos membros inferiores do grupo A e dos membros superiores e inferiores, tronco e total do grupo B; na massa adiposa dos membros inferiores do grupo A e dos membros superiores e inferiores, tronco e total do grupo B; e no conteúdo mineral ósseo dos membros superiores e inferiores, e total e na massa magra dos membros superiores e inferiores entre os hemicorpos não dominantes dos grupos A e B. O tecido ósseo é extremamente complexo que, juntamente com a cartilagem, constitui o sistema esquelético. Tanto os ossos quanto a cartilagem são compostos por tecido metabolicamente ativo com duas funções básicas para o organismo, uma mecânica e outra bioquímica. O impacto do déficit calórico e da perda de peso pode reduzir a massa óssea e mudar a composição corpórea. Na pancreatite crônica alcoólica o paciente relata ingestão alcoólica por longo período, além da referência do alto consumo de cigarros e de uma alimentação deficiente. Os objetivos do presente estudo foram avaliar a frequência da doença osteometabólica, os hábitos alimentares, a frequência de deficiência de vitamina D assim como, se os achados de massa corpórea por densitometria de corpo total se relacionam à deficiência de massa óssea, em indivíduos portadores de pancreatite crônica de etiologia alcoólica. Foram avaliados três grupos de pacientes do sexo masculino com pancreatite crônica alcoólica. Foram divididos de acordo com o resultado da densitometria óssea: 5 pacientes no grupo da osteoporose, 26 no grupo da osteopenia e 8 no grupo normal. Todos os pacientes foram submetidos ao registro alimentar de três dias, mensuração de peso, altura, cintura e quadril, Índice de Massa Corpórea (IMC) e exames laboratoriais. A composição corpórea foi avaliada pela densitometria óssea por raios X de dupla energia (DXA) e por bioimpedância elétrica. 79% dos pacientes do sexo masculino com pancreatite crônica alcoólica tiveram densidade mineral óssea comprometida. Os pacientes que tinham vitamina D prescrita foram excluídos porém nos nossos resultados a maioria dos pacientes apresentavam níveis normais da vitamina. Em relação ao tabagismos, dos pacientes fumavam. Os pacientes com maior comprometimento ósseo eram mais magros,contudo, não houve diferença entre os pacientes de acordo com o IMC. Os pacientes classificados pelo DXA como normais eram mais jovens do que os pacientes com osteopenia e osteoporose. Em síntese, a osteoporose e osteopenia são fontes subvalorizadas de morbidade em pacientes com pancreatites crônicas sendo necessárias diretrizes de gestão de saúde óssea neste grupo de pacientes / The bone tissue is extremely complex, along with cartilage constitutes the skeletal system. We evaluated three groups of male patients with chronic pancreatitis alcoholic. They were according to the results of bone densitometry. 5 in osteoporosis group, 26 in the osteopenia group and 8 in the normal group. All patients underwent three-day food record, measurements of weight, height, waist and hip, body mass index (BMI) and laboratory tests. The body composition was evaluated by densitometry by dual energy X-ray absorptiometry (DXA) and electrical bioimpedance. 79% of male patients with alcoholic chronic pancreatitis had compromised bone mineral density. Patients were prescribed vitamin D were excluded however results in the majority of patients had normal levels of the vitamin. Half of all patients smoking. Patients with higher bone involvement were thinner, there was no difference between patients according to BMI. Patients classified as normal by DXA were younger than patients with osteopenia and osteoporosis. In summary, osteoporosis and osteopenia are undervalued sources of morbidity in patients with chronic pancreatitis and necessary health management guidelines bone in this group of patients

Caracterização do estado nutricional de pacientes com síndrome de Noonan e síndromes Noonan-like / Characterization of the nutritional status of patients with Noonan syndrome and Noonan-like syndromesKanno, Fernanda Marchetto da Silva 03 December 2015 (has links)
INTRODUÇÃO: as RASopatias, que englobam a síndrome de Noonan e as síndromes relacionadas à síndrome de Noonan (Noonan-like), são doenças monogênicas de herança autossômica dominante causadas por mutações em genes pertencentes a mesma via de sinalização (RAS-MAPK). Caracterizam-se por um envolvimento multissistêmico, no qual a baixa estatura é um sinal cardinal. Os aspectos nutricionais são mais estudados nas síndromes relacionadas à síndrome de Noonan, nas quais uma dificuldade alimentar nos primeiros meses de vida é mais frequente e acentuada, requerendo uma intervenção terapêutica. OBJETIVOS: avaliar o estado nutricional de indivíduos com RASopatias, o consumo de energia e macronutrientes de acordo com a idade e gênero, comparar os dados antropométricos e o consumo alimentar com os dados populacionais e estabelecer uma associação entre o genótipo e o fenótipo dos indivíduos. MÉTODOS: o estado nutricional de 62 indivíduos com Rasopatias, confirmado por estudo molecular, com mediana de idade de 12 anos, foi avaliado a partir da realização de medidas antropométricas (peso, estatura, circunferência do braço e prega tricipital), da análise do consumo alimentar habitual e do nível de atividade física. RESULTADOS: Observou-se um déficit de crescimento de origem pós-natal em 48% dos indivíduos e, na classificação pelo índice de massa corpórea, aproximadamente 80% deles foram classificados como eutróficos ou com baixo peso/ magreza acentuada. Os parâmetros de composição corporal mostraram um comprometimento na circunferência muscular do braço e, nas classificações pela área gordurosa do braço e área muscular do braço, 43,5% apresentavam baixa reserva de gordura e 69% eram desnutridos ou abaixo da média, respectivamente. Ao associar o genótipo ao fenótipo dos indivíduos estudados, os genes SHOC2, RAF1 e KRAS eram os que apresentavam os valores médios mais baixos de estatura e os indivíduos com mutações no gene SHOC2, os menores valores de mediana de índice de massa corpórea Já os indivíduos com mutações no gene BRAF tiveram os maiores valores desta mediana. Houve uma diferença estatisticamente significante no escore-Z da dobra cutânea tricipital e na área gordurosa do braço entre BRAF e SHOC2 e no escore-Z da dobra cutânea tricipital entre RAF1 e SHOC2. O consumo energético e de macronutrientes dos indivíduos foi semelhante à referência de ingestão diária e ao da população brasileira e no nível de atividade física, 65,4% eram sedentários e irregularmente ativos. Encontrou-se somente um indivíduo com dificuldade importante na alimentação, necessitando de gastrostomia. CONCLUSÕES: A baixa estatura é frequente e mais pronunciada nos indivíduos com mutação nos genes SHOC2, RAF1 e KRAS. Diferindo da tendência da população mundial a apresentar uma alta prevalência de obesidade, a grande maioria dos indivíduos com RASopatias apresenta-se eutrófica ou com baixo peso/magreza. Na composição corpórea, há um acometimento do tecido muscular em todos os indivíduos estudados. A diminuição destas medidas não é explicada por uma baixa ingesta alimentar e nem por um aumento nas atividades físicas do grupo. É possível que os fatores genéticos, por um aumento da sinalização da via RAS-MAPK, confiram um perfil mais magro a estes indivíduos, por acometimento não apenas da gordura, mas especialmente da musculatura / INTRODUCTION: RASopathies that include Noonan syndrome and Noonan related disorders (Noonan-like) are monogenic conditions with autosomal dominant inheritance caused by mutations in genes belonging to the same signaling pathway (RAS-MAPK). They are characterized by a multisystemic involvement in which short stature is a cardinal feature. The nutritional aspects are more frequently described in Noonan related disorders, in which feeding difficulties in the first months of life are more frequent and severe, requiring therapeutic intervention. OBJECTIVES: To evaluate the nutritional aspects of individuals with RASopathies, the energy and macronutrients consumption, according to age and sex; to compare the anthropometric data and dietary intake with population references and to establish a genotype-phenotype correlation. METHODS: The nutritional status of 62 individuals with Rasopathies confirmed by DNA analysis, with a median age of 12 years was evaluated by performing anthropometric measurements (weight, height, arm circumference and triceps skinfold thickness), analysis of habitual food intake and physical activity level. RESULTS: There was growth deficiency of postnatal origin in 48% of subjects and in the classification by body mass index, approximately 80% of them were classified as eutrophic or underweight/thinness. The parameters of body composition showed a decreased upper arm muscle circumference and in the classification by the upper arm fat area and upper arm muscle area, 43.5% had low fat reserves and 69% were malnourished or below average, respectively. Genotype-phenotype correlation showed that individuals with mutations in SHOC2, RAF1 and KRAS were those presenting the lowest mean values of stature and individuals with mutations in SHOC2, the lower median values of bone mass index. On the other hand, individuals with mutations in the BRAF had the highest median values. There was a statistically significant difference of the triceps skinfold thickness and upper arm fat area between Z-scores between BRAF and SHOC2 and of the triceps skinfold thickness Z-score between RAF1 and SHOC2. The energy and macronutrient intake of the subjects were similar to the dietary intake reference and the Brazilian population and in the level of physical activity, 65.4% were sedentary and irregularly active. It was found only one individual with significant difficulty in feeding, requiring gastrostomy. CONCLUSIONS: Short stature is common and more pronounced in individuals with mutations in SHOC2, RAF1 and KRAS. Opposed to the trend of the world\'s population of an increased prevalence of obesity, the majority of individuals with RASopathies present as eutrophic or underweight/ thinness. In body composition, there is an involvement of muscle tissue in all studied subjects. The decrease of these measures in the group is explained neither by a lower food intake nor by an increase in physical activity. It is possible that genetic factors, leading to hyperactivation of the RAS/ MAPK signaling, confer a slimmer profile to these individuals, mostly by the muscle involvement and not only by a decrease in fat reserve
