The purpose of this study was to explore nurse educators‟ perception of Objective Structured Clinical Evaluation (OSCE) as summative assessment for students‟ clinical competence ,with the aim of recommending alternatives to the current situation. The rationale behind the study is that the researcher became aware of concerns raised by nurse educators regarding the appropriateness of OSCE as summative assessment of students clinical competence and differences of opinion on the matter. An explorative, descriptive, contextual, qualitative research design was chosen. The sample included nurse educators actively involved in OSCE as summative assessment of nursing students in the provincial nursing college for at least 5 years or more. Data was collected by means of two (2) focus group interview sessions with fourteen (14) participants. During data analysis it became clear that data saturation had not been reached and a third focus group interview was conducted with seven (7) participants from the original sample who were available. The focus of the interviews was to explore participants‟ perception of Objective Structured Clinical Evaluation as summative assessment for students‟ clinical competence. The data was independently analyzed by the researcher as well as an independent co-coder according to the principles for content analysis as described by Creswell (2008:251). Three (3) themes and nine (9) sub-themes emerged from the data collected from participants. The first theme described the value of OSCE as summative assessment method for students‟ clinical competence. Theme two described the obstacles in using OSCE as summative assessment for students‟ clinical competence. Theme three described the suggested alternatives. The collected data was discussed and integrated with relevant national and international literature to culminate in three (3) conclusive statements. Particularities of many childhood cancers and the adverses effect of the antineoplastic agents can change radically the oral environrnent and predisposes to the risk of oral complications. This study assessment clinically the oral health of 40 children on treatment for different types of malignant neoplasm with age range of O to 1S years old (Group I) and compared to 38 nonnal children in the same age range (Group lI). The results shown that nonnal patients had a gingival bleeding index (GBI) and caries experience minar than patients of Group lI, the visivel plaque index (VPI) was lightly higher in patients of Group 1. There was not difference statistically significant in the variables. Sixteen patients of Group I developed together 61 oral complications with predominance of mucositis, followed by spontaneous oral bleeding, candidiasis and xerostomy, that complication were most commons in patients with systemic neoplasm. Its was concluded that patients submitted to antineoplastic therapy with poor oral health had a higher risk to develop oral complications / O C?ncer na inf?ncia caracteriza-se pela predomin?ncia de neoplasias dos sistemas hematol?gicos e linf?tico,tumores do sistema nervoso central e v?rios tipos de sarcomas. Evaluation clinique, caractérisation mécanique et modélisation pour l'évolution de la conception d'un implant rachidien dynamique

L'objectif principal de tout dispositif médical implantable est d'améliorer l'état de santé du patient en lui assurant un risque minimum. Dans ce but, l'étude de l'implant rachidien B Dyn comporte plusieurs volets : - la réalisation d'un suivi clinique, - l’analyse et la proposition de solutions techniques (actions correctives), - la création d'un outil numérique pour des évolutions ultérieures (actions préventives).L’étude bibliographique initiale permet d'appréhender l'anatomie fonctionnelle du rachis lombaire, de comprendre les états pathologiques et leurs conséquences et enfin de faire un inventaire des techniques chirurgicales associées (résection osseuse, implantation de dispositifs...).Le suivi clinique d'une population de trente patients souligne ensuite les apports (somatiques et fonctionnels) du B Dyn dans sa conception première. Pour quelques cas, l'analyse des clichés radiographiques en position de flexion montre une détérioration naissante de l'anneau liée, probablement, à une surcharge accidentelle de l'implant. Ce constat impose une évolution de la conception de l'implant.Une analyse de la conception initiale et la caractérisation mécanique en traction, permettent de cibler les actions correctives à appliquer dans le cadre de cette évolution. La démarche mise en place s'appuie sur l'évaluation expérimentale pour sélectionner des solutions techniques satisfaisant aux critères fonctionnels ; elle conduit à une évolution du choix de matériau de l'anneau.Pour la réalisation d'évolutions ultérieures, un modèle éléments finis est créé. L’approche numérique se substitue ainsi à l’approche expérimentale contraignante et coûteuse. La caractérisation préalable des élastomères est nécessaire à l'obtention de données matériaux pour élaborer ce modèle. Les résultats des premières simulations d'un essai de traction sont comparés aux données expérimentales dans la perspective de la validation du modèle.A ce stade, l'étude du B Dyn propose une première solution d'évolution de l'implant et un outil numérique pour l'analyse future de solutions techniques. / The main focus of any implantable medical device is to improve the health of the patient by providing minimum risk. For this purpose, the study of the B Dyn spinal implant comprises several constituents: - The carrying out of a clinical follow up, - The analysis and choice of technical solutions (corrective actions) - The creation of a digital tool for further development (preventive actions).The initial bibliographical study enables to comprehend the functional anatomy of the lumbar spine, to understand the pathological states and their consequences and finally to list the associated surgical techniques (osseous resection, implantation of devices…).The clinical follow-up of a population of thirty patients then underlines the contributions (somatic and functional) of the B Dyn in its first design. For a few cases, the analysis of radiographs in flexion shows an incipient deterioration in the ring probably related to an accidental overloading of the implant. This observation requires an evolution in the design of the implant.An analysis of the initial design and the mechanical characterization in traction, allow targeting the corrective actions to be applied in the context of this evolution. The developed approach is based on the experimental evaluation in order to select technical solutions that would satisfy the functional criteria; this leads to an evolution of the choice of the ring material.To conduct subsequent developments, a finite element model is created. Thus the digital approach replaces the restrictive and expensive experimental approach. The preliminary characterization of elastomers is necessary to obtain materials data to work out this model. The results of the first simulations of a tensile test are compared to experimental data in the perspective of the model validation.At this stage, the B Dyn study provides a first solution of implant evolution and a numerical tool for the future analysis of technical solutions.

Processes used by nursing faculty when working with underperforming students in the clinical area: a theoretical model derived from grounded theory

Indiana University-Purdue University Indianapolis (IUPUI) / Clinical nursing faculty members often work with students who underperform in the clinical area. Underperforming students are those who exhibit deficits in nursing knowledge, the application of nursing knowledge, psychomotor skills, motivation, and/or interpersonal skills. The outcomes of faculty work with underperforming students have implications for patient safety and the nursing workforce, yet little is known about how faculty work with underperforming students. The purpose of this project was to develop a theoretical framework that describes how clinical faculty work with underperforming students in the clinical area. Twenty-eight nursing faculty who had worked with underperforming nursing students during clinical rotations were interviewed and invited to tell stories about working with these students. Their narratives were analyzed using constant comparison analysis, and a theoretical framework was developed. The framework included three stages that unfolded as faculty worked with underperforming students over time. The first stage, Being Present, was the process by which faculty came to know students were underperforming. They did this by noticing red flags, taking extra time with students, working side-by-side with students, and connecting with students "where they were at." The second stage, Setting a New Course, was the process by which faculty attempted to provide remedial experiences to improve the performance of those students determined to be underperforming. The participants did this by beginning a new course of instruction for the students, bringing in new people to help the students, and creating new learning experiences for them. This process could result in students turning it [their performance] around, making it through [the clinical rotation], or not making it. The final stage, Being Objective, was the process by which participants made negative progression decisions. They did this by relying on objective indices, documenting problematic student behaviors, and obtaining validation for their decisions.

Evolução da vítima com trauma cranioencefálico na sala de emergência / Evolution of the victim with traumatic brain injury in the emergency room.

Introdução: A frequência de vítimas de traumatismo cranioencefálico (TCE) nos serviços de emergência vem aumentando no mundo e conhecer a evolução dessas vítimas nas primeiras horas após lesão é um caminho importante para padronizar sua avaliação e alcançar melhores resultados no seu tratamento. Objetivos: Analisar, durante permanência na sala de emergência (SE), a evolução das vítimas que apresentaram TCE contuso como lesão principal e identificar, entre as variáveis demográficas, os indicadores de gravidade e as características do atendimento pré e intra-hospitalar, os fatores associados ao tempo de permanência na SE dessas vítimas. Método: Estudo do tipo corte prospectivo, com dados coletados na admissão da vítima de TCE na SE e 2, 4 e 6 horas após, ou até a transferência para tratamento definitivo em unidades específicas, ou óbito. Participaram desse estudo vítimas de TCE contuso como lesão principal, com idade 15 anos, admitidas em serviço de emergência referência para neurocirurgia, até 1 hora após trauma. A evolução das vítimas foi descrita pelas diferenças na pontuação Rapid Emergency Medicine Score (REMS) entre as avaliações na admissão na SE e 2, 4 e 6 horas após. Análises descritivas foram utilizadas para caracterização dos casos e descrever a evolução das vítimas. Testes estatísticos foram aplicados para verificar associação entre tempo de permanência na SE e idade, sexo, gravidade e características do atendimento pré e intra-hospitalar. Resultados: A casuística foi composta de 46 vítimas, 84,7% do sexo masculino, idade média de 34,7 anos (dp=15,1), 63% envolvidas em acidentes de trânsito. Na admissão na SE, o REMS médio dessas vítimas foi de 4,0 (dp=2,5), as médias do Injury Severity Score (ISS) e New Injury Severity Score (NISS) foram de 11,8 (dp=7,7) e 17,2 (dp=12,7), respectivamente. A Escala de Coma de Glasgow(ECGl) indicou 54,4% de casos de TCE grave e o Maximum Abbreviated Injury Scale/ região cabeça (MAIS/cabeça) apresentou média de 2,7 (dp=1,1). Entre a admissão na SE e 2 horas após, foram observadas mudanças desfavoráveis em 35,1% das vítimas e favoráveis em 27%; entre 2 e 4 horas, a evolução desfavorável foi constatada em 13,6% e favorável em 27,3%; entre 4 e 6 horas, constatou-se piora em 42,8% dos casos e melhora em 28,6%. Em média, as vítimas que tiveram piora apresentaram diferença de 2,8 (dp=2,3) no REMS entre admissão e 2 horas, de 2 (dp=0) entre 2 e 4 horas e de 2,3 (dp=0,5) entre 4 e 6 horas. Houve diferença estatisticamente significativa no tempo de permanência na SE somente em relação à variável uso de suporte hemodinâmico nesse serviço. Conclusão: A melhora de cerca de mais de ¼ das vítimas de TCE contuso ocorreu em todos os períodos de avaliação, porém a frequência dos casos de evolução desfavorável foi maior do que favorável entre admissão e 2 horas e ainda mais elevada após permanência de 4 horas na SE. O maior tempo de permanência na SE esteve relacionado com uso de suporte hemodinâmico nesse serviço. / Introduction: The frequency of traumatic brain injury (TBI) in emergency services is increasing worldwide and knowing the evolution of these victims in the first hours after injury is an important way to standardize their evaluation and achieve better treatment outcomes. Objectives: To analyze the evolution of the victims whose main lesion was blunt TBI and to identify, among demographic variables, severity indicators and characteristics of pre and in-hospital care, as well as factors associated with length of stay in the emergency room (ER) of these victims. Method: A prospective study, with data collected at the admission of the TBI victim at ER and 2, 4 and 6 hours after, or until transfer to definitive treatment in specific units, or death. Participants of this study presented blunt TBI as the main lesion, age 15 years and were admitted at ER for neurosurgery up to 1 hour after trauma. The evolution of the victims was described by the differences in the Rapid Emergency Medicine Score (REMS) among the assessments on admission at ER and 2, 4 and 6 hours after. Descriptive analyzes were used to characterize the cases and describe the evolution of the victims. Statistical tests were applied to verify the association between length of stay in ER and age, sex, severity and characteristics of pre and in-hospital care. Results: The casuistry consisted of 46 victims, 84.7% were male, mean age was 34.7 years (SD=15.1), and 63% were involved in traffic accidents. On admission at ER, the mean REMS of these victims was 4.0 (SD=2.5), the mean Injury Severity Score (ISS) and New Injury Severity Score (NISS) were 11.8 (SD=7.7) and 17.2 (SD=12.7), respectively. Glasgow Coma Scale (GCS) indicated 54.4% of cases of severe TBI and the Maximum Abbreviated Injury Score/ head region (MAIS/head) showed an average of 2.7 (SD=1.1). From admission at ER until 2 hours later, unfavorable changes were observed in 35.1% of the victims and favorable changes were seen in 27%; between 2 and 4 hours, the unfavorable evolution was observed in 13.6% and favorable evolution was noted in 27.3%; between 4 and 6 hours, it was found worsening in 42.8% of the cases and improvement in 28.6%. On average, patients who experienced worsening had a difference of 2.8 (SD=2.3) in REMS from admission to 2 hours, 2 (SD=0) between 2 and 4 hours and 2.3 (SD=0.5) between 4 and 6 hours. There was a statistically significant difference in the lenght of stay in ER only in relation to the variable use of hemodynamic support in this service. Conclusion: The improvement of more than ¼ of the victims of blunt TBI occurred in all the evaluation periods, but the frequency of unfavorable cases was higher than favorable between admission and 2 hours and even higher after 4 hours in ER. The longer time in ER was related to the use of hemodynamic support in this service.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Este é um estudo transversal feito com pacientes mulheres que compareceram ao Setor de Endoscopia Urológica e Urodinâmica do Serviço de Urologia do Hospital Universitário Pedro Ernesto HUPE entre dezembro de 2009 e dezembro de 2012, para a realização de estudo urodinâmico, com encaminhamento médico e agendamento prévio para investigação de queixa de incontinência urinária. O estudo foi realizado nas pacientes do sexo feminino, com idade entre 23 e 86 anos e com queixa de incontinência urinária nao complicada. Os dados utilizados nesse estudo têm três origens: (1) a avaliação primária formada pelo conjunto dos questionário de perda por esforço e ou urgência e International Consultation on Incontinence Questionnaire Short Form (ICIQ-SF) e história padronizada; (2) avaliação médica, realizada pelo médico residente; e (3) avaliação urodinâmica, resultado do estudo urodinâmico conduzido por médico residente, com supervisão e laudos feitos por um dos professores do serviço. O objetivo do trabalho foi analisar se o uso de métodos mais simples poderia diagnosticar incontinência urinária não complicada sem a necessidade de realizar a avaliação urodinâmica. Os nossos achados mostraram que entre a avaliação primária e a médica há elevada sensibilidade e especificidade além de forte concordância. O estudo urodinâmico tem menor probabilidade de fazer o diagnostico de IUM e maior frequencia de falso negativo. Os nossos achados fortalecem a indicação de uma abordagem primária antes de intervenções mais invasivas e dispendiosas como a avaliação urodinâmica. A realização de uma avaliação simplificada pode fornecer informações suficientes para começar um tratamento medicamentoso e fisioterapêutico. / This cross-sectional study of female patients who attended the Department of Urology and Urodynamic of the Endoscopy Department of Urology, Pedro Ernesto University Hospital HUPE/UERJ between December 2009 and December 2012, for performing urodynamic study with medical referral and scheduling prior to investigation of stress urinary incontinence. The study was conducted in female patients, aged between 23 and 86 years with complaints of urinary uncomplicated incontinence. The data used in this study have three origins: (1) the primary assessment formed by the set of questionnaire and loss effort or urgency and International Consultation on Incontinence Questionnaire - Short Form (ICIQ-SF) and standardized history; (2) medical evaluation performed by a resident physician; and (3) urodynamic evaluation, results of urodynamic study conducted by resident physician, with supervision and reports made by one of the Professors of the service. The objective was to analyze whether the use of simpler methods could diagnose uncomplicated urinary incontinence without the need for urodynamic evaluation. Our findings showed that between primary medical evaluation and there is high sensitivity and specificity in addition to strong agreement. The urodynamic study is less likely to make the diagnosis of MUI and higher frequency of false negative. Our findings strengthen the indication of a primary approach before more invasive and costly interventions such as urodynamic evaluation. The realization of a simplified assessment can provide enough to get a drug and physical therapy information.
