Marital Status as a Proxy Measure of Social Support and its Influence on Health Status and Depression Rates

Diabetes disproportionately affects minority populations. Social support, and more specifically marriage, has been found to buffer the negative effects of diabetes and depression. Data collected from African Americans with type 2 diabetes in Atlanta and NHANES data were compared to examine whether marriage affects health status and mental health. Approximately, 1742 African Americans aged 18-80 were included in this study. Chi square analysis revealed that married men had lower rates of depression (15.9% vs. 24.7%) compared to unmarried men (p < 0.05), but the same effect was not found in women. The findings show that marriage was not associated with HbA1c, but was associated with rates of depression. The lack of association of HbA1c with marriage may be because marriage may not be the best proxy of social support in the African American community. Future research should focus on alternative forms of social support such as cohabitation, extend family, and friend.

Psychosocial status and mental health in adolescents before and after bariatric surgery

Objective: As long-term results of conservative treatment for obesity are discouraging, bariatric surgery is becoming a treatment option for extremely obese adolescents. However, mental and behavioral problems need to be respected when treating this vulnerable target group. Methods: A detailed systematic literature review on pre- and post-operative depressive, anxiety and eating disorder symptoms of adolescent patients was performed in PsychINFO, PubMed and Medline electronic databases. Results: Twelve studies met the inclusion criteria. Although strength of evidence was limited, results suggested that pre-operatively a third of adolescents suffered from moderate to severe depressive disorder symptoms and a quarter from anxiety disorder symptoms, while a substantial number showed eating disorder symptoms. Post-operatively, levels of depressive disorder symptoms significantly improved. Original articles on outcomes of eating and anxiety disorder symptoms after weight loss surgery were not found. Conclusions: Further attention is needed on consistent clinical assessment of mental health disturbances and their consecutive treatment in adolescents. Future research should also focus on psychological and psychosocial predictors of weight loss after bariatric surgery.

Le rôle de l’infirmière de première ligne quant aux troubles mentaux courants dans un contexte interprofessionnel : une étude de cas multiples

Problématique : Au Québec, les troubles anxieux et dépressifs touchent près d’une personne sur cinq au cours de leur vie. Elles consultent majoritairement dans les services de première ligne (SPL) qui sont souvent dispensés en collaboration interprofessionnelle, mais qui ne semblent pas répondre efficacement à leurs besoins. Le rôle optimal (RO) infirmier permet d’intervenir lors du continuum de services. Un écart entre les rôles réels (RR) et le RO des infirmières semble s’observer dans les SPL. But : Décrire et analyser le RR de l’infirmière lors de la prestation des soins aux personnes atteintes de troubles dépressifs et anxieux dans les équipes multiprofessionnelles de première ligne. Méthodologie : 15 professionnels de la santé (infirmières [n=9], autres [n=6]) dans deux milieux de SPL ont participé à cette étude de cas multiples qualitative. Résultats : Trois catégories de facteurs influencent l’écart entre les RR et le RO des infirmières, soit les facteurs liés : 1) à l’équipe, 2) à la clientèle et 3) aux infirmières. La formation du RO des infirmières influence également l’écart. L’approche typiquement holistique et non stigmatisante des infirmières ainsi que la réduction de l’écart entre les RR et le RO semblent optimiser la prise en charge des TMC dans les SPL. Conclusion : Cette recherche met en lumière les facteurs pour réduire l’écart entre les RR et le RO. La formation des professionnels et l’organisation du travail dans le milieu clinique devrait mettre l’accent sur la collaboration interprofessionnelle, maximisant ainsi la pratique des professionnels selon leurs RO. / Problematic: In Québec, anxiety and depressive disorders affect nearly one in five people in their lives. They mainly consult in primary care services which are often provided in interprofessional collaboration but does not seem to succeed to effectively meet their needs. The optimal role allows nurses to intervene throughout the care plan. A difference between the nurse’s actual and optimal roles seems to be observed in the primary care services. Purpose: To describe and analyze the nurse’s actual roles in primary care multiprofessional teams during the delivery of care to people with depressive and anxiety disorders. Methods: 15 healthcare professionals (nurses [n=9], others [n=6]) in two primary care service environments participated in this qualitative multiple case study. Results: Three categories of factors influence the difference between the nurses’ actual and optimal roles: 1) the team, 2) the customer and 3) the nurses. Nurse’s training on the optimal role also influences the gap. The typical nurse’s holistic approach and the nurse’s actual role approaching the optimal role seem to contribute to primary care services in mental health. Conclusion: University training for health professionals and the organization of work in the clinical setting should focus on interprofessional collaboration, maximizing the contribution of optimal role. This research highlights the factors to reduce the gap between the nurse’s actual and optimal roles.

Relations sociales et troubles dépressifs chez les personnes âgées au Québec : interactions avec le genre et la région de résidence

Chez les personnes âgées, la dépression est un problème important en santé publique, à cause de sa prévalence élevée et de son association avec les incapacités fonctionnelles, la mortalité et l’utilisation des services. La plupart des études ont montré que le manque de relations sociales était associé à la dépression, mais les résultats ne sont pas clairs. Au Québec et au Canada, on possède peu de données sur la prévalence de la dépression chez les personnes âgées et de son association avec les relations sociales. Peu d’études ont examiné le rôle des relations sociales sur l’utilisation des services de santé par les personnes âgées déprimées. Le but de cette recherche était d’examiner le rôle des relations sociales dans la présence de la dépression et dans la consultation chez un professionnel de la santé des personnes âgées déprimées, au Québec. Plus spécifiquement, ce travail visait à : 1) examiner les associations entre les relations sociales et les troubles dépressifs selon la région de résidence; 2) examiner les associations différentielles des relations sociales sur la dépression des femmes et des hommes âgés; 3) examiner le rôle des relations sociales dans la consultation auprès d’un professionnel de la santé des personnes âgées déprimées. Pour répondre à ces objectifs, nous avons utilisé les données de l’enquête ESA (Enquête sur la Santé des Aînés), réalisée en 2005 -2006 auprès d’un échantillon de 2670 personnes âgées résidant à domicile au Québec, qui nous ont permis de rédiger trois articles. Les troubles dépressifs (incluant la dépression majeure et mineure) ont été mesurés, selon les critères du DSM-IV, en excluant le critère de l’altération du fonctionnement social, professionnel ou dans d’autres domaines importants, à l’aide du questionnaire ESA développé par l’équipe de recherche. Les relations sociales ont été mesurées à l’aide de cinq variables : (1) le réseau social; (2) l’intégration sociale; (3) le soutien social, (4) la perception d’utilité auprès des proches et (5) la présence de relations conflictuelles avec le conjoint, les enfants, les frères et sœurs et les amis. Des modèles de régression logistique multiple ont été ajustés aux données pour estimer les rapports de cote et leur intervalle de confiance à 95 %. Nos résultats ont montré des prévalences de dépression plus élevées chez les personnes qui résident dans les régions rurales et urbaines, comparées à celles qui résident dans la région métropolitaine de Montréal. La pratique du bénévolat, le soutien social et les relations non conflictuelles avec le conjoint sont associés à une faible prévalence de dépression, indépendamment du type de résidence. Comparés aux hommes, les femmes ont une prévalence de dépression plus élevée. L’absence de confident est associée à une prévalence de dépression élevée, tant chez les hommes que chez les femmes. La probabilité de dépression est plus élevée chez les hommes veufs et chez ceux qui ne pratiquent pas d’activités de bénévolat, comparativement à ceux qui sont mariés et font du bénévolat. Chez les femmes, aucune association significative n’a été observée entre le statut marital, le bénévolat et la dépression. Cependant, la présence de relations conflictuelles avec le conjoint est associée avec la dépression, seulement chez les femmes. Les relations avec les enfants, les frères et sœurs et les amis ne sont pas associées avec la dépression dans cette population de personnes âgées du Quebec. En ce qui concerne la consultation chez un professionnel de la santé, nos résultats ont révélé que presque la moitié des personnes âgées dépressives n’ont pas consulté un professionnel de la santé, pour leurs symptômes de dépression, au cours des 12 derniers mois. Par ailleurs, notre étude a montré que les personnes âgées qui disposent de tous les types de soutien (confident, émotionnel et instrumental) consultent plus pour leurs symptômes de dépression que ceux qui ont moins de soutien. Comparativement aux hommes mariés, les femmes mariées consultent plus les professionnels de la santé, ce qui laisse supposer que le réseau de proches (épouse et enfants) semble agir comme un substitut en réduisant la fréquence de consultation chez les hommes. Vu la rareté des études canadiennes sur la prévalence de la dépression chez les personnes âgées et les facteurs psychosociaux qui y sont associés, les résultats de ce travail seront utiles pour les cliniciens et pour les responsables des politiques à l’échelle nationale, provinciale et locale. Ils pourront guider des interventions spécifiques, selon la région de résidence et pour les hommes et les femmes âgées, dans le domaine de la santé mentale. / Depression is an important public health problem among the elderly population due to its high prevalence and its association with disability, mortality and use of health services. Most studies have shown that lack of social networks is associated with depression but results are not clear. In Quebec and in Canada, little evidence exists on the prevalence of depression in the elderly population and on its associations with social relationships. Few studies have examined the associations between social relationships and health services utilisation among the depressed elderly patients. The aim of this work was to examine the role of social relationships on the presence of depression and on the consultation with a health professional by the elderly patient with depression, in Québec. More specifically, this work aimed at examining if: 1) the associations between social relationships and depression varied in rural and urban areas; 2) the associations between social relationships and depression were different in men and in women and 3) assessing the role of social relationships in the consultation of a health professional by the elderly patients with depression. To attain these objectives we have written three articles using data from the ESA survey, conducted in 2005-2006 on a representative sample of 2670 community dwelling people over 65 in Quebec. Depressive disorders (including major and minor depression) were measured by the DSM-IV criteria using the ESA questionnaire developed by the research team which excludes the criteria on limitations of social functioning. Social relationships were measured by aspects of social networks (marital status and diversity of ties), engagement in community social activities (religious attendance, frequenting social centers, and volunteerism), social support and perception of usefulness and presence of conflict in the relationships with spouse, children, siblings and friends. Multiple logistic regressions were adjusted to estimate odds ratios and their 95 % confidence intervals. Prevalence of depression was higher among those living in urban and rural areas of Quebec compared with metropolitan Montreal. Volunteerism, social support and not having conflict with the spouse were associated with lower frequency of depression independently of the area of residence. Women had higher prevalence of depression compared with men. Lack of a confident was associated with higher frequency of depression both in men and in women. Men who were widows and those who were not involved in volunteer work had higher odds of depression than men in other marital situations and men who were volunteering. Marital status and volunteer work were not associated with depression in women. However, conflict in marriage was associated with depression in women. Relationships with children, siblings and friends are not associated with depression in this population of elderly people from Quebec. Concerning consultation with a health professional for symptoms of depression, we have shown that approximately half of those depressed elderly, both among men and among women, have not consulted for their symptoms in the last 12 months. Consultation is associated with high levels of support, but the immediate network (spouse and siblings) seems to act as a substitute for depressed elderly men, reducing their frequency of consultation. Given the scarcity of Canadian studies on depression prevalence among the elderly population, these results on depression and its associated factors provide important information for clinicians and decision makers for planning and targeting of services by area of residence and for both men and women.

Late-Life Depressive Symptoms: An International Study

Objectives. Evaluate differences in depressive symptoms, compare sociodemographic and health-related variables associated with depressive symptoms and report level of impact of depressive symptoms on daily activities. Methods. Cross-sectional study using a self-administered questionnaire and Patient Health Questionnaire-9 (PHQ-9) diagnostic survey on 1115 patients aged 60–93 years who attended a primary care clinic in Korea, Russia or USA. Results. At least mild depression (PHQ-9 score of ≥5) occurred in 28% of Koreans, 65% of Russian and 27% of US participants. Russians scored more depressed on all PHQ-9 items (P < 0.01) and more suicidal thoughts (P < 0.001), while Koreans had less feelings of worthlessness (P < 0.001). Depression predictors included poorer self-rated health [odds ratio (OR) 2.47, 95% confidence interval (CI) 1.84–3.33, P < 0.0001], chronic diseases (OR 1.34, CI 1.21–1.48, P < 0.0001), female gender (OR 1.56, CI 1.15–2.12, P = 0.0046) and religious attendance (OR 0.88, CI 0.79–0.97, P = 0.0099) for all subjects. Being employed was protective in Korea (OR 0.41, CI 0.21–0.77, P = 0.0061) and being married (OR 0.42, CI 0.27–0.66, P = 0.0002) and of older age (OR 0.95, CI 0.93–0.98, P = 0.0006) protective in US participants. Vascular disease was associated with depressive symptoms in Russia (OR 3.47, CI 1.23–9.80, P = 0.0187). In regression analyses stratified by country for a given level of depressive symptoms, the Russian sample had less impact on daily activities (Russia R2 = 0.107 versus Korea R2 = 0.211 and US R2 = 0.419) P = 0.029. Conclusions. Depressive symptoms were more common in Russia than in Korea and USA but had less impact on daily functioning. Cultural or environmental factors may account for this finding.

The waxing and waning of mental disorders: Evaluating the stability of syndromes of mental disorders in the population

This article examines the stability of symptoms, syndromes, and diagnoses of specific anxiety and depressive disorders, as well as diagnostic shifts from one syndrome to another over time. Using retrospective and longitudinal prospective data from the baseline and first follow-up investigation (19.7 months later) of the Early Developmental Stages of Psychopathology Study (EDSP), we focus on establishing stability measures for early stages of mental disorders in a community sample of adolescents aged 14 to 17 years at baseline. The results are as follows: (1) Although only about 30% developed a full-blown DSM-IV disorder, psychopathological syndromes are widespread in adolescents: 15% of the population aged 14 to 17 at baseline were not affected by at least some clinically relevant symptoms of mental disorders either throughout their previous life or throughout the follow-up period. (2) The likelihood of staying free of symptoms and threshold disorders during follow-up was highest among subjects who were completely well at baseline. The probability of a positive outcome decreased as a function of severity of baseline diagnostic status. (3) There was a considerable degree of fluctuation not only in the diagnostic status and severity of specific disorders, but also in terms of complete remissions and shifts from one syndrome and disorder to another. (4) Anxiety disorders, overall, slightly differ with regard to the persistence and stability of the diagnostic status from depressive disorders. (5) However, there were remarkable differences between specific types of anxiety and depressive disorders. Consistent with other longitudinal epidemiological studies in the general population, this study finds that the syndromes and diagnoses of mental disorders have a strong tendency to wax and wane over time in this age group.

In the Central nervous system-SNC is one of his more severe manifestations. One of the frames of Lupus neuropsychiatric - LES-NP, is a Demyelinating Syndrome - SD. SD in rare cases, may be the first manifestation of LES, resembling multiple sclerosis - MS in their symptoms. MS is autoimmune disease with involvement of SNC with components, degenerative and inflammatory, which can produce autoantibodies and general symptoms that make your final diagnosis. The disease occurs more in young women than in men, typically evolve with outbreaks and remissions. In order to contribute to the diagnosis of these two illnesses, we proposed a comparative analysis, considering clinical, laboratory, and image aspects. As there is a lack of definition of SD, aims to contribute to the definition and establishment of standards of compromise of the SNC by SD of LES-NP, comparing patients with LES-NP and MS (the prototype of CNS demyelinating disease), attended in rheumatology and neurology of the HC/FM/UFG, Goiania. GO, from 2009 to 2010, according to the demographic aspects (sex and age), clinical (time of illness, functional systems and EDSS), laboratory (Autoantibodies and liquid Cerebroespinhal - LCR), image (Conventional magnetic resonance - RM) and Neuropsychological - NPS (cognitive and emotional). It is one epidemiological, analytical and comparative cross-sectional study of LES-NP and MS, which was approved by the CEP/HC/UFG. Were used in the survey questionnaires: an analysis of demographic and personal and family background; other symptoms; Neurological exams and NPS; Analysis: image by RM, laboratory of autoantibodies and LCR; and symptoms of depression and anxiety by Beck inventory (BAI and BDI). The results demonstrated an increased frequency in patients of age; time of disease; Visual symptoms; changes of sensitivity; of coordination; of the March; retention and urinary incontinence; changes of speech and swallowing; depressive and psychotic symptoms in family and personal history; WMHs in parietal and frontal lobes, occipitais, corpus callosum, periventricular, justacortical, brain stem; the cerebellum; spinal cord and the Barkhof and Tintoré criteria - CBT. Other findings in the RM were: infarcts; Vasculitis; menigioma and thrombosis in patients with LES-NP. The involvement of SNC in LES was measured by clinical symptomatology, neurological examination, EDSS, image findings and neuropsychological tests. No Sistema Nervoso Central - SNC é uma de suas manifestações mais graves. Um dos quadros neurológicos do Lúpus Neuropsiquiátrico - LES-NP, é a Síndrome Desmielinizante - SD. Em raros casos, SD pode ser a primeira manifestação do LES, se assemelhando a Esclerose Múltipla - EM em sua sintomatologia. EM é doença autoimune com envolvimento do SNC com componentes, inflamatório e degenerativo, que pode apresentar auto-anticorpos e sintomas gerais que dificultam seu diagnóstico final. As doenças ocorrerem mais em mulheres jovens que em homens, evoluem tipicamente com surtos e remissões. Com o intuito de contribuir para o diagnóstico dessas duas enfermidades, propusemos uma análise comparativa, considerando aspectos clínicos, laboratoriais e de imagem. Como há uma carência de definição da SD, pretende-se contribuir com a definição e o estabelecimento de padrões de comprometimento do SNC pela SD do LES-NP, comparando os pacientes com LES-NP e EM (o protótipo da doença desmielinizante), atendidos no serviço de reumatologia e neurologia do HC/FM/UFG, Goiânia/GO, de 2009 a 2010, segundo os aspectos demográficos (sexo e idade), clínicos (tempo de doença, sistemas funcionais e EDSS), laboratoriais (Autoanticorpos e Líquido Cerebroespinhal - LCR), de imagem (Ressonância Magnética Convencional - RM) e Neuropsicológicos - NPS (cognitivo e emocional). É um estudo epidemiológico, analítico, transversal, comparativo de LES-NP e EM, que foi aprovado pelo CEP/HC/UFG. Foram utilizados na pesquisa análise de questionários: um demográfico e de antecedentes pessoais e familiares; outro de sintomas; exames: neurológico e de NPS; avaliações: imagem por RM e laboratorial de auto-anticorpos e LCR; e sintomas de depressão e ansiedade pelo inventário de Beck (BAI e BDI). Os resultados demonstraram uma frequência aumentada nos pacientes de EM, em idade; tempo de doença; sintomas visuais; alterações de sensibilidade; da coordenação; da marcha; retenção e incontinência urinária; alterações da fala e deglutição; sintomas depressivos e psicóticos na história pessoal e familiar; alterações focais da substância branca (WMHs) nos lobos frontais, parietais e occipitais, corpo caloso, periventricular, justacortical, tronco encefálico; cerebelo; medula espinhal e os critérios de Barkhof e Tintoré - CBT. Outros achados na RM foram: infartos cerebrais; vasculites; meningioma e trombose nos pacientes com LES-NP. O envolvimento do SNC no LES foi mensurado pela sintomatologia clínica, exame neurológico, EDSS, achados de imagem e testes neuropsicológicos. RM foi preditiva em relacionar carga lesional em regiões cerebrais tipicamente envolvidas na EM. A aplicação dos CBT nos pacientes com SD do LES-NP, associado à avaliação qualitativa das WMHs pode definir as diferenças de padrão de envolvimento neurológico da lesão da SD do LES-NP, daquelas da EM. A dosagem de autoanticorpos foi preditiva para o diagnóstico de LES, especialmente o anti DNA, o anti-ENA e o LAC, os quais foram exclusivos no LES-NP. O exame clínico e neurológico, da investigação laboratorial de auto-anticorpos antinucleares e da imagem por RM oferece segurança no diagnóstico das duas enfermidades. Mostram que há aspectos do envolvimento da SD no LES que necessitam de novas avaliações, utilizando os marcadores de lesão neuronal, mais específicos e com estudos de imunogenética comparados com a EM, o protótipo da doença desmielinizante do SNC.
