Yes / Fundus-controlled perimetry (FCP, also called 'microperimetry') allows for spatially-resolved mapping of visual sensitivity and measurement of fixation stability, both in clinical practice as well as research. The accurate spatial characterization of visual function enabled by FCP can provide insightful information about disease severity and progression not reflected by best-corrected visual acuity in a large range of disorders. This is especially important for monitoring of retinal diseases that initially spare the central retina in earlier disease stages. Improved intra- and inter-session retest-variability through fundus-tracking and precise point-wise follow-up examinations even in patients with unstable fixation represent key advantages of these technique. The design of disease-specific test patterns and protocols reduces the burden of extensive and time-consuming FCP testing, permitting a more meaningful and focused application. Recent developments also allow for photoreceptor-specific testing through implementation of dark-adapted chromatic and photopic testing. A detailed understanding of the variety of available devices and test settings is a key prerequisite for the design and optimization of FCP protocols in future natural history studies and clinical trials. Accordingly, this review describes the theoretical and technical background of FCP, its prior application in clinical and research settings, data that qualify the application of FCP as an outcome measure in clinical trials as well as ongoing and future developments.

Rehabilitation after hip fracture:comparison of physical, geriatric and conventional treatmentLahtinen, A. (Antti) 30 April 2019 (has links)
Abstract
Hip fracture causes substantial burden for individual and society, with increased mortality and loss of function. The purpose of this study was to (1) examine the effects of specialized (physical and geriatric) rehabilitation on home-dwelling hip fracture patients 50 years or older on recovery one year after the fracture, (2) to evaluate the costs and cost-effectiveness of specialized rehabilitation modalities, (3) to study the recovery after hip fracture between home-dwelling male and female patients and (4) to determine recommendations for hip fracture rehabilitation concerning the general rehabilitation practices in Finland.
A total of 538 consecutively, independently living patients with non-pathological hip fracture treated in Oulu University hospital, were randomized to one of the three rehabilitation modalities: privately-based rehabilitation unit (physical rehabilitation), geriatric department (geriatric rehabilitation) and healthcare centre hospital (control group). Patients were evaluated on admission, at 4 and 12 months for social status, residential status, walking ability, use of walking aids, pain in the hip, activities of daily living and mortality. Costs were evaluated by recording the use of healthcare service and the prices were obtained from Diagnosis Related Group (DRG) price list for the hospital and from a publication of the National Research and Development Centre for Welfare and Health.
Mortality was lower in the physical rehabilitation group 4 and 12 months after the fracture compared to geriatric and the control rehabilitation group. Physical and geriatric rehabilitation improved the ability of independent living after 4 months, but this effect could not be seen after 12 months. The rehabilitation costs were higher in the physical rehabilitation than in the control group, but the total healthcare-related costs one year after the fracture were lower in the physical rehabilitation group than in the control. Male and female patients recovered similarly after hip fracture. Age, poor functional status before the fracture and high ASA-score increased the mortality risk.
This thesis suggests that intensive mobilization and rehabilitation is a recommended practice after the hip fracture, resulting in better functional recovery, survival and lower economic costs compared to routine treatment. Poor recovery was predicted not by sex, but by prefracture function and morbidity. / Tiivistelmä
Lonkkamurtuma on yksi merkittävimmistä toimintakyvyn laskua ja kuolleisuutta aiheuttavista vammoista. Tämän väitöskirjatutkimuksen tarkoituksena oli (1) tutkia tehostetun (fysikaalisen ja geriatrisen) kuntoutuksen vaikutusta itsenäisesti asuvien, vähintään 50-vuotiaiden lonkkamurtumapotilaiden kuntoutumiseen murtumaa seuraavan vuoden aikana, (2) arvioida lonkkamurtuman hoidon kustannuksia ja kustannusvaikuttavuutta tehostetussa kuntoutusyksikössä, (3) tutkia lonkkamurtuman kuntoutumisen eroja mies- ja naispotilaiden välillä sekä (4) tarkentaa lonkkamurtuman hoitoketjun yleisiä suosituksia Suomessa.
Tutkimuksessa seurattiin 538 lonkkamurtumapotilasta, jotka hoidettiin kirurgisesti Oulun yliopistollisessa sairaalassa. Valintakriteereihin kuului vähintään 50 vuoden ikä sekä kyky itsenäiseen asumiseen joko omassa kodissa tai kodinomaisessa ympäristössä ennen murtumaa. Lonkkaleikkauksen jälkeen potilaat satunnaistettiin yhteen kolmesta kuntoutusryhmästä: fysikaaliseen kuntoutusryhmään (Oulun Diakonissalaitos), geriatriseen kuntoutusryhmään (Oulun kaupunginsairaalan kuntoutusosasto) sekä kontrolliryhmään (terveyskeskussairaalassa tapahtuva kuntoutus). Seuranta-aika oli yksi vuosi. Potilaat haastateltiin ennen kuntoutusta, sekä neljän ja 12 kuukauden kuluttua murtumasta, joiden yhteydessä aineisto kerättiin koskien yleistä terveydentilaa, toiminta- ja kävelykykyä, asumismuotoa ja kuolleisuutta. Kustannukset arvioitiin terveyspalveluiden käytöstä ja näiden hintoina käytettiin sairaalan yksikköhintoja (DRG) ja sosiaali- ja terveysalan tutkimus- ja kehittämiskeskuksen (STAKES) yksikköhintoja.
Fysikaalinen kuntoutus vähensi merkittävästi potilaiden kuolleisuutta neljän ja 12 kuukauden seurannoissa verrattuna geriatriseen kuntoutukseen sekä kontrolliryhmään. Lisäksi fysikaalinen ja geriatrinen kuntoutus lisäsivät lyhytaikaisesti potilaiden kykyä itsenäiseen asumiseen verrattuna kontrolliryhmään. Taloudellisten vaikutusten osalta fysikaalinen kuntoutusjakso oli huomattavasti tavanomaista kuntoutusta kalliimpi, mutta fysikaalisen kuntoutusryhmän kokonaiskustannukset vuoden aikana olivat pienemmät kuin kontrolliryhmän potilailla. Sukupuolella ei ollut vaikutusta potilaiden toimintakykyyn, laitostumiseen tai kuolleisuusriskiin murtuman jälkeen. Merkittäviksi kuolleisuutta ennustaviksi tekijöiksi osoittautuivat ikä, toimintakyky ennen murtumaa sekä leikkauskelpoisuusluokitus.
Tulokset puoltavat tehostetun, erikoistuneessa kuntoutusyksikössä suoritetun hoidon käyttöä lonkkamurtumapotilailla sekä terveydellisten että taloudellisten syiden osalta.

Cellular Reactions and Behavioral Changes in Focal and Diffuse Traumatic Brain Injury : A Study in the Rat and MouseEkmark Lewén, Sara January 2013 (has links)
Traumatic brain injury (TBI) is a severe condition and a major cause of death and disability. There is no pharmacological treatment available in clinical practice today and knowledge of brain injury mechanisms is of importance for development of neuroprotective drugs. The aims of the thesis were to get a better understanding of astrocyte reactions and immune responses, as well as behavioral changes after focal unilateral cortical contusion injury and diffuse bilateral central fluid percussion injury in rats and mice. In the focal injury models, the astrocyte reactions were generally restricted to the ipsilateral hemisphere. After diffuse TBI, vimentin and glial fibrillary acidic protein (GFAP) positive reactive astrocytes were bilaterally expressed in brain regions even distant from the injury site, including regions where axonal injury was seen. Early after diffuse TBI, there was a robust immune response, including activation of macrophages/microglia (Mac-2+) and infiltration of neutrophils (GR-1+) and T-cells (CD3+). In order to measure functional outcome, the recently established Multivariate Concentric Square Field™ (MCSF) test for complex behaviors, including risk taking and explorative strategies was used. The Morris water maze (MWM) was applied for testing learning and memory. The MCSF test revealed alterations in risk taking, risk assessment and exploratory behavior, in the mice subjected to focal injury whereas mice subjected to the diffuse injury showed a deviant stereotyped behavior. After focal injury mice showed a decreased ability to adapt to the arena in the second trial, when tested repeatedly in the MCSF test. Mice subjected to diffuse injury had an impaired memory but not learning, in the MWM test. Post-injury treatment with the anti-inflammatory anti-interleukin-1β (IgG2 a/k) antibody showed a positive effect on functional outcome in the diffuse injury model. Altogether, the results demonstrate that focal and diffuse TBI models produce differences in cellular reactions and behavioral outcome and that the immune response plays a key role in the pathology after brain injury.

Objective: A malignant profile of early brain ischemia has been demonstrated in the Diffusion and Perfusion Imaging Evaluation for Understanding Stroke Evolution (DEFUSE) trial. Patients with a malignant profile had a low chance for an independent functional outcome despite thrombolysis within 3–6 h. We sought to determine whether CT angiography (CTA) could identify a malignant imaging profile within 3 h from symptom onset. Methods: We studied consecutive patients (04/02–09/07) with anterior circulation stroke who received CTA before intravenous thrombolysis within 3 h. We assessed the Alberta Stroke Program Early CT Score (ASPECTS) on CTA source images (CTASI). Intracranial thrombus burden on CTA was assessed with a novel 10-point clot burden score (CBS). We analyzed percentages independent (modified Rankin Scale score ≤2) and fatal outcome at 3 months and parenchymal hematoma rates across categorized combined CTASI-ASPECTS + CBS score groups where 20 is best and 0 is worst. Results: We identified 114 patients (median age 73 years [interquartile range 61–80], onset-to-tPA time 129 min [95–152]). Among 24 patients (21%) with extensive hypoattenuation on CTASI and extensive thrombus burden (combined score ≤10), only 4% (1/24) were functionally independent whereas mortality was 50% (12/24). In contrast, 57% (51/90) of patients with less affected scores (combined score 11–20) were functionally independent and mortality was 10% (9/90; p < 0.001). Parenchymal hematoma rates were 30% (7/23) vs. 8% (7/88), respectively (p = 0.008). Abstract
The purposes of the present research were (1) to analyse and characterize the hip fractures treated at Oulu University Hospital during a one-year period using the special forms of the Standardized Audit of Hip Fractures in Europe (SAHFE) and to evaluate their value for quality control, (2) to compare gamma nail (GN) and dynamic hip screw (DHS) fixation for the treatment of trochanteric hip fractures, focusing especially on the functional aspects, (3) to compare the short-term outcome of gamma nail (GN) and dynamic hip screw (DHS) fixation for the treatment of subtrochanteric hip fractures, and (4) to examine the rate and reliability of the classification of basicervical hip fractures and the outcome of the operative methods used for their treatment.
Oulu University Hospital joined the Swedish Hip Fracture Project (Rikshöft), aimed at developing the quality control of hip fracture treatment, in 1989, and this later evolved into a project called the Standardized Audit of Hip Fractures in Europe (SAHFE), funded by the European Commission. Registration of hip fractures on the SAHFE forms was common practise in Oulu from 1st September 1997 until the end of December 2003. SAHFE data collection forms were used in all four studies belonging to this thesis.
There were 238 hip fracture patients during the one-year period of registration at Oulu University Hospital. The intracapsular / extracapsular fracture rate (60/40) and the female/male rate (80/20) seemed to be similar to those reported in the recent Finnish Health Care Register data. The most frequent method for treating cervical fractures was Austin-Moore hemiarthroplasty (68%) and that for trochanteric and subtrochanteric fractures GN fixation (86%). The SAHFE forms proved to be easy to use and practicable for evaluating the quality of hip fracture treatment.
In a matched-pair study the short-term outcomes of the treatment of trochanteric fractures (after 4 months) were slightly better in the DHS group than in the GN group with respect to walking ability and mortality. The difference in mortality was at least partly due to the higher number of complications requiring re-operations associated with GN fixation.
In the treatment of subtrochanteric hip fractures, there were four intraoperative complications (9.3%) in the GN group but none in the DHS group. On the other hand, postoperative complications were more common in the DHS group (20% vs. 2%). It is significant that all these complications in the DHS group occurred in Seinsheimer type IIIA fractures. It is concluded that, despite the perioperative problems associated with gamma nailing, this technique may be preferable to DHS fixation for specific fracture types with medial cortical comminutation, such as Seinsheimer type IIIA.
Altogether 108 of the 1624 hip fractures were initially classified by the surgeons as basicervical fractures, but after a careful second look only 30 fulfilled all the criteria. The definitive rate of basicervical fractures was thus 1.8%. Treatment of basicervical fractures as trochanteric fractures proved superior to their treatment as cervical fractures, resulting in lower re-operation rates.
In conclusions; this thesis suggests that SAHFE forms are very useful for evaluating the quality of hip fracture treatment. Both GN fixation and DHS fixation are effective methods for the treatment of trochanteric hip fractures in elderly patients; in less comminuted fractures, the DHS method is the preferred method of treatment whereas GN fixation is alternative treatment for more comminuted fractures. GN fixation is preferable for the subtrochanteric fratures. Basicervical fractures shoud be regarded clinically as extracapsular fractures and managed in a similar manner to trochanteric fractures.

Depressive and anxious symptomatology in relation to a primary brain tumor:prospective study of neurosurgical patients in Northern FinlandMainio, A. (Arja) 03 May 2005 (has links)
Abstract
The findings on depression and anxiety among brain tumor patients have so far been based on case series and case samples. In Finland, psychiatric research in relation to psychiatric symptoms among patients with different types of brain tumors is lacking.
The study population of this thesis consisted of 101 patients (39 males and 62 females) aged between 20 and 82 years with a solitary primary brain tumor treated surgically at the Oulu Clinic for Neurosurgery, Oulu University Hospital between February 1990 and March 1992. The major histological subgroup consisted of gliomas (40%), and the rest were meningiomas (33%), acoustic neurinomas (13%), pituitary adenomas (8%) and other types (6%).
The psychiatric symptoms of the patients were assessed at three time points, namely before tumor operation as well as at three months and at one year after operation by two valid measurement instruments, the Beck Depression Inventory and the Crown Crisp Experiential Index. In addition, the patients' functional state was evaluated by the Karnofsky Performance Scale and their quality of life according to Sintonen 15 D.
Prevalence of at least mild depression before tumor operation was 30% for males and 38% for females. The mean depressive scores decreased significantly for up to one-year during follow-up for both males and females, but they remained notably high in all patients. Decreased functional status (KPS under 70) in the patients was significantly associated with high depressive scores at all measurement points. The decrease in the mean depressive scores was significant among patients with an anterior tumor and those with a pituitary adenoma.
Five-year survival of the brain tumor patients was found to be mainly associated with the histology of the tumor. Survival time in months (SD) of the patients with high-grade (III–IV) gliomas was shown to be 22.5 (21.4), while it was 50.2 (19.9) for the patients with low-grade (I–II) gliomas, and 58.2 (9.4) for the rest of the patients. Depression among low-grade glioma patients was significantly associated with worse survival at five years follow-up.
The level of anxiety was shown to be significantly higher among patients with a primary brain tumor in the right hemisphere compared to the anxiety scores among patients with left hemispheric tumors. A significant increase was found in the level of obsessionality over time in the female patients with a brain tumor in the left anterior location of the brain at three months after operation.
The level of quality of life (QOL) was significantly worse among female brain tumor patients compared to males. Depressive females had significantly lower quality of life compared to that of non-depressive females up to one-year follow-up after surgical operation of the tumor.
Depression, anxiety and obsessive-compulsive symptoms have to be recognized and be treated by psychotherapy and pharmacotherapy as soon as possible at every unit where brain tumor patients are followed and encountered.

Prognostic Factors in First-Episode Schizophrenia : Five-year Outcome of Symptoms, Function and ObesityBodén, Robert January 2010 (has links)
Our knowledge of prognostic factors and optimal treatment organisation in schizophrenia is incomplete. The disparity of outcome measures used has been a major obstacle for research. Increasing evidence has shown that schizophrenia is associated with increased cardiovascular mortality, development of obesity and autonomic nervous system imbalance. Assertive community treatment (ACT) has been suggested as a promising direction for organising treatment services for first-episode schizophrenia, but its long-term effect has not been evaluated. One aim of the present thesis was to investigate prognostic factors for 5-year symptomatic and functional outcome and obesity development. A further aim was to evaluate a recently proposed definition of remission and examine the long-term effects of introducing a modified ACT programme (mACT). Thus, we performed a follow-up study of all consecutive first-episode psychosis patients in Uppsala County, Sweden during 1995-2000 (n=144). In the first study we investigated the changes in a broad 5-year outcome of symptoms and function among patients presenting first time ever to psychiatric health care during 3 years before and during 3 years after the implementation of mACT. This change in the psychiatric service, however, was not followed by any long-term clinical benefits. In the second study, we examined the association between remission of eight core schizophrenia symptoms and functional outcome. Remission was strongly associated with having good function and having a higher self-rated satisfaction with life. In the third study, we explored a set of biochemical markers as predictors of weight gain and development of obesity. Haemoglobin, red blood cell count, hematocrit, γ-glutamyltransferase and creatinine were associated with the development of obesity in first-episode schizophrenia. In the fourth and final study, we tested electrocardiographic measures of autonomic imbalance as predictors of symptomatic remission. Higher heart rate and high ST and T-wave amplitudes were related to symptomatic remission, indicating that cardiac autonomic imbalance at baseline may have a prognostic value in first-episode schizophrenia.

knee proprioception. However, there are still few studies that had evaluate the characteristics
of proprioception acuity in the early period of rehabilitation after the ACL-reconstructed
surgery, over all when it was associated to functional outcomes. Thus, the objective of this
study was to evaluate knee proprioception and subjective knee function in subjects who had
an ACL injury, before and after (3 months) the reconstruction surgery with the middle third
of patellar tendon autograft; and in control subjects. Ten volunteers who had unilateral ACL
injury (26.9±6.8 years) reconstructed group - and fifteen health volunteers (22.1±3.0 years)
control group performed a proprioceptive evaluation (joint position sense - JPS during
active and passive movements, threshold for detection of passive movement TDPM) on a
Biodex Multi-Joint System III Pro isokinetic dynamometer, at 2º/s and at two target-angles
(30º and 60º of knee flexion). The variables analyzed were the mean of angular displacement
(TDPM) and the mean of absolute error (JPS) in relation to each target-angle. The subjective
knee function was evaluated by Lysholm scale. The analyses of variance showed subjective
functional deficit in involved limb, compared to the uninvolved limb, as well as in relation to
the control group, in both test moments (p<0.01). Although no significant, the results showed
a tendency for higher TDPM in the reconstructed group (p=0.06). There was no significant
difference in the mean of absolute error between the groups (p>0.05), for any modality of
JPS, target-angles or test moments. The TDPM showed strong correlation with the subjective
knee functional score (just in the involved limb). Under the experimental conditions utilized,
the subjects who had an anterior cruciate ligament injury didn t show proprioceptive deficits
in relation to the control group, although of the subjective functional limitations presented. / Diversos estudos têm evidenciado o importante papel do ligamento cruzado anterior (LCA)
na propriocepção do joelho. No entanto, ainda há poucos estudos que tenham investigado as
características da acuidade proprioceptiva no período inicial de reabilitação após a
reconstrução cirúrgica do LCA, sobretudo quando associada a aspectos funcionais. Diante do
exposto, foi objetivo do presente estudo avaliar a sensibilidade proprioceptiva e a função
subjetiva do joelho em indivíduos com lesão do LCA, antes e após (3 meses) a reconstrução
cirúrgica pelo terço médio do tendão patelar autógeno; e em indivíduos controle. Dez
voluntários com lesão unilateral do LCA (26,9±6,8 anos) grupo reconstruído - e quinze
clinicamente sadios (22,1±3,0 anos) - grupo controle - realizaram uma avaliação
proprioceptiva (percepção da posição articular PPA - durante os movimentos ativo e
passivo, limiar de detecção do movimento passivo LDMP) no dinamômetro isocinético
Biodex Multi-Joint System III Pro, a 2º/s e em dois ângulos-alvo (30º e 60º de flexão do
joelho). As variáveis estudadas foram o deslocamento angular médio (LDMP) e o erro
absoluto (PPA) em relação a cada ângulo-alvo. A função subjetiva do joelho foi avaliada por
meio da Escala de Lysholm. A análise de variância indicou déficit funcional subjetivo no
membro envolvido, comparado ao membro contralateral, bem como em relação ao grupo
controle, nos dois períodos de teste (p<0,01). Embora não significativo, os resultados
indicaram tendência de maior LDMP para o grupo reconstruído (p=0,06). Não houve
diferença significativa entre os grupos quanto à média de erro absoluto (p>0,05), para
qualquer uma das modalidades de PPA, ângulos-alvo ou períodos considerados. O LDMP
apresentou forte correlação com o escore funcional do joelho (apenas para o membro
envolvido). Nas condições experimentais utilizadas, os indivíduos com lesão do LCA não
apresentaram déficits proprioceptivos significativos em relação ao grupo controle, apesar das
limitações funcionais verificadas subjetivamente.

Mobilisation in the acute stages of stroke is a complex subject where the nurse must considerseveral confounding variables. There is an insufficient state of evidence around when andhow the first mobilisation should occur after stroke, and there is often uncertainty in thedecision making. The aim was to describe how early mobilisation in the acute stages of stroke affects thepatient’s level of functioning. The method used was a general literature review with an integrated analysis. Results: 17 articles analysing the effect of early mobilisation on nursing-related outcomemeasures, published between 2015–2019, were included in the literature review. Sample,interventions and outcome measures varied between the different articles. Three of the articlesshowed results suggesting that early mobilisation could be negative for the patient’sfunctional outcome after stroke. Among the remaining articles there was an even distributionof positive effects of early mobilisation and results not showing any effect on functionaloutcome. The results of this literature review imply that early mobilisation to some extentmay contribute to improved basic functions as in managing the toilet, dressing and also moreinstrumental features as cooking and driving. Early mobilisation appears to be a safe nursingintervention in most cases, as long as the patient is considered medically stable. The conclusion to be drawn from this literature review is that nurses, through their mainresponsibility for nursing care can influence the patient’s functional outcome through the decisions made regarding the first mobilisation in patients hit by acute stroke. / Mobilisering i akutskedet efter stroke är ett komplext ämne där sjuksköterskan måste ta ställning till ett flertal samverkande faktorer. Evidensläget kring när och hur den första mobiliseringen bör ske efter stroke är otillräckligt, och ofta uppstår en osäkerhet i beslutsfattandet. Syftet var att beskriva hur tidig mobilisering i akutskedet efter stroke påverkar patientens funktionsnivå. Metoden som användes var en allmän litteraturöversikt med integrerad analys. Resultat: 17 artiklar som analyserat effekten av tidig mobilisering på omvårdnadsrelaterade utfallsmått, publicerade från 2015–2019, inkluderades i litteraturöversikten. Urval, interventioner och utfallsmått varierade mellan de olika artiklarna. Tre av artiklarna visade på resultat som talar för att tidig mobilisering skulle kunna vara negativt för patientens funktionsutfall efter stroke. Bland övriga artiklar sågs en jämn fördelning bland positiv effekt av tidig mobilisering och resultat som inte visat någon påverkan på funktionsutfallet. Resultatet i denna litteraturöversikt tyder på att tidig mobilisering i viss mån kan bidra till förbättrade basala funktioner såsom att klara av toalettbesök, på- och avklädning och även mer instrumentella funktioner som att laga mat och köra bil. Tidig mobilisering ter sig vara en säker omvårdnadsåtgärd i de flesta fall, så länge patienten bedöms som medicinskt stabil. Slutsatsen som kan dras av litteraturöversikten är att sjuksköterskan genom sitt huvudansvar för omvårdnad kan påverka patientens funktionsutfall genom de beslut som fattas kring den första mobiliseringen av patienter som drabbats av akut stroke. Ytterligare forskning avseende tidpunkt och mobiliseringens intensitet behövs för att kunna avgöra när och hur den första mobiliseringen skall påbörjas efter att en patient drabbats av stroke.
