Communication in Smoking Cessation and Self-management : a study at Nurse-led COPD-clinics in Primary Health CareÖsterlund Efraimsson, Eva January 2010 (has links)
ABSTRACTThe general aim of this thesis was to investigate behavioral change communication at nurse-led chronic obstructive pulmonary disease (COPD) clinics in primary health care, focusing on communication in self-management and smoking cessation for patients with COPD.Designs: Observational, prospective observational and experimental designs were used.Methods: To explore and describe the structure and content of self-management education and smoking cessation communication, consultations between patients (n=30) and nurses (n=7) were videotaped and analyzed with three instruments: Consulting Map (CM), the Motivational Interviewing Treatment Integrity (MITI) scale and the Client Language Assessment in Motivational Interviewing (CLAMI). To examine the effects of structured self-management education, patients with COPD (n=52) were randomized in an intervention and a control group. Patients’ quality of life (QoL), knowledge about COPD and smoking cessation were examined with a questionnaire on knowledge about COPD and smoking habits and with St. George’s Respiratory Questionnaire, addressing QoL. Results: The findings from the videotaped consultations showed that communication about the reasons for consultation mainly concerned medical and physical problems and (to a certain extent) patients´ perceptions. Two consultations ended with shared understanding, but none of the patients received an individual treatment-plan. In the smoking cessation communication the nurses did only to a small extent evoke patients’ reasons for change, fostered collaboration and supported patients’ autonomy. The nurses provided a lot of information (42%), asked closed (21%) rather than open questions (3%), made simpler (14%) rather than complex (2%) reflections and used MI non-adherent (16%) rather than MI-adherent (5%) behavior. Most of the patients’ utterances in the communication were neutral either toward or away from smoking cessation (59%), utterances about reason (desire, ability and need) were 40%, taking steps 1% and commitment to stop smoking 0%. The number of patients who stopped smoking, and patients’ knowledge about the disease and their QoL, was increased by structured self-management education and smoking cessation in collaboration between the patient, nurse and physician and, when necessary, a physiotherapist, a dietician, an occupational therapist and/or a medical social worker.Conclusion The communication at nurse-led COPD clinics rarely involved the patients in shared understanding and responsibility and concerned patients’ fears, worries and problems only to a limited extent. The results also showed that nurses had difficulties in attaining proficiency in behavioral change communication. Structured self-management education showed positive effects on patients’ perceived QoL, on the number of patients who quit smoking and on patients’ knowledge about COPD.

Vers un outil d'évaluation du langage préscolaire et détecteur de préalables pour l'éducation de base au Cameroun en cas de retard de langageTakam Taguemné, Aurélie 03 1900 (has links)
L’étude présente l’élaboration du premier outil d’évaluation normalisée du langage destiné aux enfants francophones d’âge préscolaire des pays africains qui se caractérisent par l’absence de services publics en orthophonie et la méconnaissance des retards de langage. En s’inspirant des techniques psychométriques langagières utilisées en Amérique du nord et en se basant sur les théories d’acquisition du langage, on a construit un outil appelé Batterie de tests de dépistage approfondi (BATEDA) qui compte onze tests répartis en trois sous-batteries évaluant respectivement l’intégrité des modalités de transmission du langage (BATEM), les composantes langagières (BATEL) et la mémoire verbale (TMV). A partir d’un échantillon de 159 enfants camerounais francophones, on a normalisé la batterie suivant la cote z et les rangs en centile. Les tests présentent une fiabilité inter-juge et intra-juge (p<0,05), une cohérence interne globale d’environ 96% (α de Cronbach=0,958) et une validité prédictive de la réussite scolaire des enfants (r=0,700). Les stratégies d’application de la BATEDA visent à favoriser la pratique du dépistage systématique et la prise en charge des retards de langage au Cameroun, en mettant à contribution les enseignants et les parents comme principaux intervenants. Les implications de cette étude sont de trois ordres : (1) une meilleure connaissance des difficultés langagières, la revalorisation des personnes atteintes et une attitude de vigilance sociale; (2) l’instauration de la pratique de dépistage précoce favorable à l’intervention; (3) une potentielle diminution du taux d’exclusion et d’abandon scolaire d’enfants présentant des retards langagiers à travers un enseignement ciblé en leur faveur. / The study aims at the building of the first normalized language assessment test intended for French-speaking preschoolers living in African countries where language delays are ignored and where there is a lack of public services in speech-language therapy. Being inspired by the format of standardized language tests, and based on language acquisition theories, we developed a tool named Batterie de tests de dépistage approfondi (BATEDA) which combine eleven tests organized in three batteries assessing respectively modalities (BATEM), language components (BATEL) and memory (TMV). From a sample of 159 Cameroonian French-speaking four-to-five-years old children, the tool has been normalized using standard scores technique (the z score and the percentile). Tests have an inter-scorer reliability, an internal consistency of 96% (α de Cronbach=0,958) and a predictive validity of the primary school success of children (r=0,700). The application strategies of BATEDA aim at encouraging the screening of language delays in order to take charge of these difficulties in Cameroon by implicating teachers and parents as main participants. Among the implications of this study, we have the better knowledge of difficulties related to language delays, the better consideration of children affected, the establishment of early screening and language assessment on behalf of intervention, and a potential decrease of the rate of school abandon and expulsion of children with language delays.

Impact of Teacher Demographic, Knowledge, and Instructional Variables on Children's Language DevelopmentEllis, Donna 01 January 2011 (has links)
The purpose of the present study was to determine whether a set of teacher demographic, knowledge, and instructional variables is related to preschool children’s literacy development. Specifically, the study investigated how these teacher variables impact children’s language development scores on the four subscales of the Preschool Language Assessment Instrument, Second Edition (PLAI2) and the four subscales of the Test of Language Development – Primary, Fourth Edition (TOLD-P:4). There were two major research questions in the study: (a) Will the predictor set of CLASS emotional support, CLASS classroom organization, CLASS instructional support, level of education, years teaching pre-kindergarten, and answers on a teacher knowledge questionnaire (TKQ) correlate with the TOLD-P:4 language assessment subscales of relational vocabulary, syntactic understanding, sentence imitation, and morphological completion? (b) Will the predictor set of CLASS emotional support, CLASS classroom organization, CLASS instructional support, level of education, years teaching pre-kindergarten, and answers on a TKQ correlate with the PLAI2 language assessment subscales of matching, selective analysis, reordering, and reasoning? Results indicated no noteworthy correlations between the predictor variable set and the subtests of the TOLD-P:4; hence, the variable relationships posited in research question 1 were not supported by the data. Results for research question 2 indicated support for the variable relationships posited. Specifically, canonical correlation yielded two roots of noteworthy size (Rc 2 values = .19 and .09 for roots 1 and 2, respectively). Canonical structure coefficients indicated positive correlation between the teacher predictor variables of education, experience, knowledge, and the CLASS domain of emotional support with students’ scores on the PLAI2. At the same time, the amount of teaching experience that teachers had in the childcare industry was found to be negatively correlated to PLAI2 subscale scores. Findings are discussed relative to the literature on professional development.

The German Proficiency Exam at Brigham Young University: A Validation StudyStarr, Tina Grahovac 05 July 2008 (has links)
In order to continuously improve teaching and learning of a language program, it is a crucial part of program evaluation to assure that its assessment instruments have a beneficial influence on the teaching and learning procedures. For that reason, evidence was gathered to investigate the validity of test scores of the German Proficiency Exam (GPE) used by the German Section of the Germanic and Slavic Languages Department at Brigham Young University. The GPE consists of seven exam components: Listening comprehension, reading, writing, speaking, grammar, vocabulary, and strong verbs. The GPE component scores of 179 students were used to conduct the analysis for this study. In order to estimate the reliability of the test scores, Cronbach's Alpha was calculated for the listening comprehension exam, the reading exam, the grammar exam, the strong verbs exam, and the vocabulary exam. In addition, the analysis included overall descriptive statistics, item facility and item discrimination, distractor analysis, ANOVA, and a post-hoc Tukey's pairwise comparison. The results of the Cronbach's alpha indicated relatively high reliability of scores of all the exam components except the listening component. The item and distractor analysis of the strong verbs and vocabulary exam revealed that the scoring procedures need to be revised so that the scores reflect a student's true knowledge. The descriptive statistics of the exams showed a limited usage of the scoring range and suggest defining the scoring procedures and training the scorers. Further, it was suggested to define general language construct and the specific construct of each language skill on the basis of which proficiency levels can be developed. Using the results of the data analysis. various suggestions were given to improve the validity of scores of the GPE.

Grammatical classification ("tagging") of words in language samples is a component of syntactic analysis for both clinical and research purposes. Previous studies have shown that probability-based software can be used to tag samples from adults and typically-developing children with high (about 95%) accuracy. The present study found that similar accuracy can be obtained in tagging samples from school-aged children with and without language impairment if the software uses tri-gram rather than bi-gram probabilities and large corpora are used to obtain probability information to train the tagging software.

Swedish Sign Language Skills Training and Assessment / Utbildning och bedömning av svensk teckenspråksförmågaPotrus, Dani January 2017 (has links)
Sign language is used widely around the world as a first language for those that are unable to use spoken language and by groups of people that have a disability which precludes them from using spoken language (such as a hearing impairment). The importance of effective learning of sign language and its applications in modern computer science has grown widely in the modern aged society and research around sign language recognition has sprouted in many different directions, some examples using hidden markov models (HMMs) to train models to recognize different sign language patterns (Swedish sign language, American sign language, Korean sign language, German sign language and so on). This thesis project researches the assessment and skill efficiency of using a simple video game to learn Swedish sign language for children in the ages within the range of 10 to 11 with no learning disorders, or any health disorders. During the experimental testing, 38 children are divided into two equally sized groups of 19 where each group plays a sign language video game. The context of the video game is the same for both groups, where both listened to a 3D avatar speak to them using both spoken language and sign language. The first group played the game and answered questions given to them by using sign language, whereas the other group answered questions given to them by clicking on an alternative on the video game screen. A week after the children have played the video game, the sign language skills that they have acquired from playing the video game are assessed by simple questions where they are asked to provide some of the signs that they saw during the duration of the video game. The main hypothesis of the project is that the group of children that answered by signing outperforms the other group, in both remembering the signs and executing them correctly. A statistical null hypothesis test is performed on this hypothesis, in which the main hypothesis is confirmed. Lastly, discussions for future research within sign language assessment using video games is described in the final chapter of the thesis. / Teckenspråk används i stor grad runt om i världen som ett modersmål för dom som inte kan använda vardagligt talsspråk och utav grupper av personer som har en funktionsnedsättning (t.ex. en hörselskada). Betydelsen av effektivt lärande av teckenspråk och dess tillämpningar i modern datavetenskap har ökat i stor utsträckning i det moderna samhället, och forskning kring teckenspråklig igenkänning har spirat i många olika riktningar, ett exempel är med hjälp av statistika modeller såsom dolda markovmodeller (eng. Hidden markov models) för att träna modeller för att känna igen olika teckenspråksmönster (bland dessa ingår Svenskt teckenspråk, Amerikanskt teckenspråk, Koreanskt teckenspråk, Tyskt teckenspråk med flera). Denna rapport undersöker bedömningen och skickligheten av att använda ett enkelt teckenspråksspel som har utvecklats för att lära ut enkla Svenska teckenspråksmönster för barn i åldrarna 10 till 11 års ålder som inte har några inlärningssjukdomar eller några problem med allmän hälsa. Under projektets experiment delas 38 barn upp i två lika stora grupper om 19 i vardera grupp, där varje grupp kommer att få spela ett teckenspråksspel. Sammanhanget för spelet är detsamma för båda grupperna, där de får höra och se en tredimensionell figur (eng. 3D Avatar) tala till dom med både talsspråk och teckenspråk. En vecka efter att barnen har utfört experimentet med teckenspråksspelet bedöms deras teckenspråkliga färdigheter som de har fått från spelet genom att de ombeds återuppge några av de tecknena som de såg under spelets varaktighet. Rapportens hypotes är att de barn som tillhör gruppen som fick ge teckenspråk som svar till frågorna som ställdes överträffar den andra gruppen, genom att både komma ihåg tecknena och återuppge dom på korrekt sätt. En statistisk hypotesprövning utförs på denna hypotes, där denna i sin tur bekräftas. Slutligen beskrivs det i rapportens sista kapitel om framtida forskning inom teckenspråksbedömning med tv spel och deras effektivitet.

In an increasingly interconnected world where English has become the lingua franca of business, culture, entertainment, and academia, learners of English as a second language (L2) have been steadily growing. This has contributed to an increasing demand for automatic spoken language assessment systems for formal settings and practice situations in Computer-Assisted Language Learning. One common misunderstanding about automated assessment is the assumption that machines should replicate the human process of assessment. Instead, computers are programmed to identify, extract, and quantify features in learners' productions, which are subsequently combined and weighted in a multidimensional space to predict a proficiency level or grade. In this regard, transferring human assessment knowledge and skills into an automatic system is a challenging task since this operation should take into account the complexity and the specificities of the proficiency construct. This PhD thesis presents research conducted on methods and techniques for the automatic assessment and feedback of L2 spoken English, mainly focusing on the application of deep learning approaches. In addition to overall proficiency grades, the main forms of feedback explored in this thesis are feedback on grammatical accuracy and assessment related to particular aspects of proficiency (e.g., grammar, pronunciation, rhythm, fluency, etc.). The first study explores the use of written data and the impact of features extracted through grammatical error detection on proficiency assessment, while the second illustrates a pipeline which starts from disfluency detection and removal, passes through grammatical error correction, and ends with proficiency assessment. Grammar, as well as rhythm, pronunciation, and lexical and semantic aspects, is also considered in the third study, which investigates whether it is possible to use systems targeting specific facets of proficiency analytically when only holistic scores are available. Finally, in the last two studies, we investigate the use of self-supervised learning speech representations for both holistic and analytic proficiency assessment. While aiming at enhancing the performance of state-of-the-art automatic systems, the present work pays particular attention to the validity and interpretability of assessment both holistically and analytically and intends to pave the way to a more profound and insightful knowledge and understanding of automatic systems for speaking assessment and feedback.

Sprachproduktionstest zu narrativen Kompetenzen in Deutscher Gebärdensprache (NaKom DGS) - eine TestadaptionKolbe, Vera 09 February 2023 (has links)
Diese Erwerbsstudie beschreibt die Adaption eines Testverfahrens aus der Britischen Gebärdensprache von Herman et al. (2004) in Deutsche Gebärdensprache (DGS). NaKom DGS ist ein Testverfahren das narrative und grammatische Kompetenzen in Kindererzählungen analysiert. NaKom DGS elizitiert Erzählungen mittels eines kurzen sprachfreien Stimulus-Videos und ist für Kinder im Alter von 4-11 Jahren validiert. In einer Querschnittsstudie wurde das Testverfahren mit 97 Kindern deutschlandweit durchgeführt, um das Testverfahren zu validieren. Die Referenzwerte für NaKom DGS basieren auf den Erzählungen von 72 Kindern mit Zugang zu DGS ab Geburt durch taube in DGS kommunizierende Eltern.
Durch diese Studie wurden neue Erkenntnisse zum Spracherwerb von DGS gewonnen. Die gefundenen Erwerbsverläufe bieten Wissenschaftler_innen in vielen der untersuchten Strukturen erste Anhaltspunkte für den Spracherwerb in DGS, mit denen zukünftige Forschungsergebnisse verglichen werden können.
Ausgewertet werden als narrative Kompetenzen Strukturelemente nach dem globalen Strukturmodell von Labov and Waletzky (1973), sowie Erzählinhalt und Reihenfolge der Erzählung. Als grammatische Kompetenzen werden in Anlehnung an Johnston (2016) auf kleiner satzähnlicher Ebene Verbmodifikationen analysiert: Modifikationen der Art und Weise, direktionale Modifikationen, aspektuelle Modifikationen und abbildende Verben. Auf Textebene, d.h. in Bezug zur gesamten Erzählung, wird konstruierte Aktion untersucht.
Die Referenzwerte von NaKom DGS werden cross-linguistisch mit den Ergebnissen des Grundlagentests, sowie einer weiteren Adaption in Amerikanische Gebärdensprache (Enns et al. 2019) verglichen und dadurch zusätzlich bestätigt. / This acquisition study describes the test adaptation process of the British Sign Language (BSL) Productive Skills test from Herman et al. (2004) to German Sign Language (DGS), resulting in the new assessment tool „Sprachproduktionstest zu narrativen Kompetenzen in Deutscher Gebärdensprache“ (NaKom DGS). NaKom DGS analyzes narrative and grammatical competences in children´s narrations, that are elicited via a short language free videoclip. In a nationwide cross-sectional study NaKom DGS was validated with data from 97 signing children, 4-11 years old. The standards for the test are derived from the results of 72 native signing children, that acquire DGS from Deaf DGS signing parents.
This study provides new insights in language acquisition in DGS. Up to now research in DGS acquisition is sparse, therefore the results of NaKom DGS are the first insights in many of the analyzed areas. The results provide scholars with first insights and stepping stones for future research.
This study focuses on narrative competences as the global structural elements from the model of Labov and Waletzky (1973), as well as narrative content and narrative sequence. Grammatic competences specifically modifications of verbs are analyzed on the level of small clause like units, following Johnston (2016): indicating directional modifications, depicting verbs, aspect and manner. On text level constructed action is analyzed across the whole narration.
Additionally the newly developed standards for NaKom DGS are compared to the standards from the BSL test as well as another adaptation to American Sign Language (Enns et al. 2019). This cross-linguistic comparison supports the validity of the German test results.
