Riskfaktorer och omvårdnad för psykisk ohälsa bland ungdomar

Background: Young people are constantly exposed to risk factors that may affect mental health. But through knowledge and proper nursing, several risk factors can be eliminated which will improve mental health. Aim: The purpose of the study is to highlight risk factors and nursing measures for mental illness among young people. The aim was also to investigate the chosen articles' data analysis methodology. Method: This descriptive literature study was founded on 16 scientific articles. The literature was analyzed and reviewed to distinguish themes. In addition to this, the data analysis methodology was examined. Results: Several risk factors showed an increased risk of mental health among young people. Risk factors was most apparent amongst youth whose families had poor socioeconomic status, who witnessed or been subjected to violence and exclusion. Overall, a higher proportion of young female reported mental illness. In nursing care for young people who suffered from mental illness, there was a great need for support. The literature showed the importance of the nurse ability to spend enough time and built up a trustworthy relationship with the patient. Though motivation and education by the nurse, the awareness increased, and feeling of stigmatization decreased in the population of young people with mental illness. Conclusions: This literature study highlights several risk factors regarding mental illness among young people, the most significant were the impact of the family, socioeconomic conditions, postponed or witnessed violence, being a female and stress. The literature study also addresses nursing in mental health among young people, highlighting the importance of providing support, listening, being available, working to build a trustful relationship, interacting with parents and mental healthcare expertise. This knowledge enables nurses to identify mental illness among early-stage adolescents and to institute nursing measures on time. / Bakgrund: Ungdomar utsätts ständigt för riskfaktorer som kan påverka den psykiska ohälsan. Men genom kunskap och rätt omvårdnad kan flera riskfaktorer elimineras och det psykiska måendet förbättras. Syfte: Syftet med denna studie var att belysa riskfaktorer och omvårdnadsåtgärder vid psykisk ohälsa bland ungdomar samt granska de valda artiklarnas dataanalysmetod. Metod: Denna beskrivande litteraturstudie grundades på 16 vetenskapliga artiklar som analyserats och granskats för att urskilja teman samt vilken dataanalysmetod som använts. Huvudresultat: Flera olika riskfaktorer visade ge ökad risk för psykisk ohälsa bland ungdomar. Det som stod ut mest var ungdomar vars familjer hade sämre socioekonomiska förhållanden, ungdomar som bevittnat eller blivit utsatt för våld samt utanförskap. Överlag led fler andel tjejer av psykisk ohälsa. I omvårdnaden kring ungdomar som drabbats av psykisk ohälsa sågs ett stort behov av stöd. Det var också viktigt att sjuksköterskan tog sig tid och byggde upp ett förtroendefullt förhållande gentemot ungdomen. Genom att sjuksköterskan motiverade och utbildade ungdomar med psykisk ohälsa ökade medvetenheten och stigmatiseringen minskade. Slutsatser: Denna litteraturstudie belyste flera riskfaktorer för psykisk ohälsa bland ungdomar, vanligaste var familjens inverkan, socioekonomiska förhållanden, att utsättas eller bevittna våld, att vara tjej och stress. Litteraturstudien lyfte även omvårdnadsåtgärder vid psykisk ohälsa bland ungdomar där de belyste vikten av att ge stöd, lyssna, vara tillgänglig, arbeta för att bygga en förtroendefull relation samt samverka med föräldrar och andra professioner. Denna kunskap möjliggör för sjuksköterskor att identifiera psykisk ohälsa bland ungdomar i tidigt stadie och sätta in omvårdnadsåtgärder i tid.

The thesis deals with the problem of discrimination and stigmatization of people with a psychotic illness, its ethical as well as legal dimension and the specificity of social work with this target group. The thesis consists of five chapters. In chapter I. I briefly define health and illness in general and the specifics of mental health and mental disorders. In chapter II. I describe psychotic illness in general i.e. formation, symptoms and manifestation, treatment and I also shortly introduce particular types of psychotic illnesses. In chapter III. I specialize in the application of ethics in social work and in ethical and legal problems in psychiatry. Chapter IV. deals with the specifics of discrimination and stigmatization of people with a psychotic illness. I present the most frequent areas in which discrimination occurs, consequences which are caused by discrimination and stigmatization, activities leading to their moderation and legal standards that are supposed to aim at the prevention of discrimination and ensure the protection of rights for mentally ill people. I ethically reflect these issues. Chapter IV. is concerned with the specifics of social work with people suffering from a psychotic illness. I describe how to work with such a target group, I introduce methods of social work helping clients with a psychotic illness to function socially, to integrate into the society and reduce the impact of stigmatization and discrimination.

The use of repertory grids to explore nursing staff's construal of adult service users admitted to a psychiatric inpatient ward

Acute inpatient mental health nursing staff provide mental health care for individuals when they are most vulnerable and unwell. The therapeutic relationship can facilitate positive changes and recovery for individuals. Therefore, understanding nursing staff’s attitudes is paramount. In this thesis, the attitudes of nursing staff towards those experiencing mental health difficulties was explored. In the systematic review, the attitudes of European nursing staff supporting those experiencing severe mental health difficulties were synthesised and evaluated. A total of 14 cross-sectional studies met the inclusion criteria. The review identified that the role of personal experiences of mental health difficulties through friends or family members was shown to elicit more positive attitudes. Overall attitudes amongst nursing staff were varied, and factors that influenced these attitudes were less clear and consistent. The limitation of using cross-sectional questionnaires to explore attitudes was also discussed. Moreover, recommendations regarding further research priorities as well as clinical implications were identified. Nursing staff’s attitudes towards adults who were either ‘informal’ voluntary clients or those who had been admitted under the Mental Health Act (1983) to an inpatient ward were explored in the empirical paper. The attitudes of nursing staff have the potential to impact on the development of therapeutic relationships and therefore upon treatment outcomes. Repertory grid interviews were completed with twelve nursing staff. All staff made critical judgements about some of their clients; however, staff who used more dimensions to construe clients made less clear distinctions between clients and non-clients. The findings highlight the need for support mechanisms that enable staff to formulate clients’ difficulties and explore the complexity of interactions. The implications of these results are discussed, as well as future research directions. The final paper consists of a critical reflection of the research and the research process. This includes an evaluation of the decision making processes and discussion of the strengths and weaknesses of this research.

O estudo busca, ainda, identificar estratégias para a superação de limites vivenciados pela família do cliente com T.M, bem como a repercussão desse tipo específico de cuidado em sua reintegração social. Para tanto, estabelece diálogo com pesquisadores da área de saúde mental, tomando, como ponto de partida a conceituação do T.M enquanto um fenômeno complexo que demanda cuidados específicos. Nesta perspectiva, aborda também o cotidiano da área de enfermagem no que diz respeito à assistência desse tipo de usuário nas mais variadas instâncias de tratamento, especificamente nos novos modelos de assistência propostos pela Reforma Psiquiátrica. Evidencia estudos sobre o comportamento do familiar ante a problemática do T.M, a partir de discursos que se entrecruzam na construção de estratégias de cuidado prestado a esses sujeitos. Esses estudos tornaram possível o desenvolvimento de um raciocínio acerca da reintegração social do cliente com T.M, a partir do momento em que ele é compreendido como alguém que necessita de cuidado integral e que é privilegiado em sua subjetividade e nas relações sociais que estabelece. / This study addresses the care of family and professional nursing related to the subject with a mental disorder in the process of deinstitutionalization of psychiatric illness. As a general objective, seeks to understand the contribution of family and nursing in the treatment of mental disorders compared to alternative venues of care, recommended by the psychiatric reform in Brazil. In order to do this, it talks about the reform and the theoretical foundations that support it, from public health policies aimed at T.M. The study aims also to identify strategies for overcoming the limitations experienced by the family of the patient with T.M, as well as the repercussions of this specific type of care in their rehabilitation. To do so, establishes dialogue with researchers in the field of mental health, taking as a starting point, the conceptualization of T.M as a complex phenomenon that requires specific care. In this perspective, also addresses everyday nursing with regard to the assistance of this type of user in various instances of treatment, specifically in the new models of care proposed by the Psychiatric Reform. I highlight this researches a way to bring out studies on the behavior of the family before the issue of T.M, from discourses that intersect in the construction strategies of care provided to these subjects. These studies have made possible the development of reasoning about the social reintegration of patients with T.M, from the moment it is perceived as someone who needs comprehensive care that is privileged in its subjectivity and social relations that are established.

Mental Illness and Perceived Social Support upon Reentry: An Analysis of Inmates in Arizona

abstract: Mentally ill offenders continue to contribute to mass-incarceration within the United States. The cost, both social and economic, of housing a large number of mentally ill inmates in our prison system has reached a breaking point. The need for empirically founded correctional research, with an emphasis on individuals who suffer from a mental illness, is crucial to reducing the number of incarcerated individuals in the United States. The current study analyzes whether mentally ill inmates reported statistically significant differences in levels of perceived reentry social support, when compared to their non-mentally ill counterparts. The current study utilized data from the APVP. The APVP contained a sample of 231 individuals, 121 female and 110 male, from two Arizona Department of Corrections facilities. The majority of respondents were white (44.58%), medium security (40.26%), non-married (77.49%), and had a mean age of 36.04 years (SD=11.74). The current study conducted both bivariate and multivariate analyses to determine whether mentally ill inmates perceived differences in the reentry social support available to them as compared to non-mentally ill inmates. Further multivariate analyses were conducted to determine whether there were any significant differences the key independent variable and the dependent variables across gender. Mentally ill female inmates reported significantly lower rates of perceived reentry social support in a number of emotional support factors. The Effects of an Internet-Based Self-Compassion Writing Intervention for Adults with Mental Illness

abstract: Multiple studies have found that writing with self-compassion about a difficult event helps promote mental health and improve affect in college students and non-clinical populations (Johnson & O'Brien, 2013; Leary et al, 2007; Shapira & Mongrain, 2010). This study investigated whether a self-compassion writing intervention would lead to increases in self-compassion and proactive coping and reductions in depression and physical symptoms in a sample of individuals with different types of mental illness. This study also looked more broadly at the feasibility of conducting an online randomized trial on individuals with mental illness, including psychotic disorders, on Amazon MTurk. Individuals with schizophrenia, schizoaffective disorder, bipolar disorder and/or depression on Amazon MTurk were recruited and randomly assigned to either a (1) treatment condition in which participants wrote with self-compassion or a (2) neutral condition in which participants wrote about how they spent their time. Participants were asked to write for 20 minutes each day for three consecutive days. Outcome measures were administered at baseline, after the three-day intervention, and one month later. Computerized linguistic analysis (LIWC; Pennebaker et al., 2015) was also used to analyze participants' writing to determine if the intervention had the intended effect. Both the treatment and control groups showed significant improvements in self-compassion, proactive coping, general mental health and physical health following the intervention and both groups showed significant improvements in self-compassion, proactive coping and general mental health between the post-test and 1-month follow-up. In addition, the self-compassion writing group's positive affect improved significantly more than the control group after the wave 1 writing intervention and the control group's negative affect improved significantly more than the self-compassion writing group after the wave 2 writing intervention. Overall, the results suggest both self-compassion writing and writing about how one spends one's time may be beneficial for individuals with mental illness with different needs. Moreover, it was found Amazon MTurk may not be a reliable platform for recruiting individuals with psychotic disorders, and that the prevalence of individuals with any mental illness on MTurk may be equal or greater than the prevalence of any mental illness in the general population. / Dissertation/Thesis / Masters Thesis Social Work 2018

The aim of this study was to describe and analyze how the municipality's elderly care, primary care and volunteer organizations work and cooperate with each other and elderly caretaker regarding mental illness, with the purpose to pay attention to, manage and prevent mental illness for the elderly.   In order to achieve the purpose of this study a qualitative research was used. The empirical material was collected through four qualitative interviews, which were held with two volunteer organizations and one doctor and one nurse from the primary care. Two vignette studies  were held with elderly-care workers. One study conducted in a small municipality and the other in a bigger municipality, both in Kalmar county.  The study results were analyzed using the following theories: stigma and social exclusion, the power of diagnosis, social psychiatric field and cooperation. The results of this study show that the municipality and county council do work and cooperate to prevent and address mental illness among the elderly. However, there may be a better and more interaction with psychiatry in order to further and more effective manage the problems. Those who were interviewed feel that through continuity, motivation and collaboration they work to prevent and detect mental illness, but that competence is not always enough when it comes to how mental illness shall be managed when it emerges. Our conclusion is that by addressing the question of resources at national level and increase available resources to actors dealing with the mental health of elderly people, better cooperation can be created, leading to improved care for the elderly.

A partir de observações de campo e de entrevistas com profissionais e usuários destes serviços é possível afirmar que as oficinas artísticas, apesar de serem espaços de experiências muito significativas para seus participantes, não acompanharam as transformações ocorridas no universo artístico e nas metodologias de ensino da arte. De maneira geral, os coordenadores destas oficinas ainda trabalham (sem necessariamente estarem conscientes disto) com um ideário de arte ancorado... / The present work discusses how the contemporary conceptions of art/education can contribute to the field of the Brazilian Psychiatric Reform, starting from the theoretical reference of the so-called “philosophers of the difference” – specially Foucault, Deleuze and Guattari (all influenced by Nietzsche’s ideas) – and the idea of a bricolage method (which among other characteristics, allows a composition with other methodological perspectives). Intends, therefore, think the artistic workshops of public mental health through the eyes of Art / Contemporary Education. In order to reflect on the range and limits of a work on art/education in the mental health institutions, and also to make a historical study of the links between madness and its conceptions of treatment, and between art and its teaching in a archeological perspective, I researched four mental health units (two CAPSs and two CECCOs) in the city of São Paulo. I tried to map out which are the discourses and practices regarding the artistic activities (focusing on the workshops which work with visual arts) which circulate in the mentioned institutions, understanding that they represent the social imaginary which surrounds the work developed there. From the field observations and interviews with professionals and users of those services, it is possible to say that in general the artistic workshops, though they are very significant spaces of experience for its participants, do not follow the transformations occurred in the artistic universe and in the methodologies of art teaching. In general, the coordinators of those workshops still work (without necessarily being conscious of that) with an idea of art anchored in the classical representations, and making use of a art teaching methodology linked to the freeexpression of the modernists, wrongly interpreted as... Usually triggered by the family, the interdiction process aims to restrict the civil living acts of a person diagnosed with “mental illness” that characterizes the person as “incapable” from the medical and juridical point of view. With the interdiction sentenced by the judge, a guardian is indicated to be responsible for the goods and the life management of the person that was interdicted, who from this moment on has all his actions considered null for undetermined period. Besides, the interdiction has been used to attest the incapacity of the person to work in cases when the guardian requires a governmental assistance named “Benefício de Prestação Continuada” at the National Institute of Social Security (INSS from the Portuguese abbreviation), on behalf of the interdicted person. The intention, from detailing ethnographic fragments, is to promote the debate on the different meanings of the individual behaviors between the institutions, as well as the relationships between the different knowledges and actions, sometimes conflicting, promoted by this process. The intention is also to embrace moral and ethical questions present in legal, medical, and social speeches about behaviors framed as “symptoms” and “diseases” on the psychiatric knowledge and, because of the medical expertise, disabling from the juridical point of view, as well as its effects in the life of interdicted individuals and their families. / Essa dissertação tem o intuito de analisar os atuais mecanismos de interdição de pessoas com diagnóstico de “doença mental” a partir da compreensão sobre as relações entre família, justiça e psiquiatria tecidas durante o processo legal. Geralmente acionado pela família, o processo de interdição busca estabelecer restrições ao exercício dos atos da vida civil de um indivíduo diagnosticado com uma “doença mental” que o caracterize como “incapaz” do ponto de vista médico e jurídico. Com a interdição sentenciada pelo juiz, um curador é nomeado para responsabilizar-se pela administração dos bens e da vida do sujeito interditado passando este a ter todos seus atos considerados legalmente nulos por tempo indeterminado. Além disso, a interdição tem sido um recurso utilizado como comprovação da incapacidade do indivíduo para o trabalho em casos onde o curador requer o “Benefício de Prestação Continuada” junto ao Instituto Nacional de Seguro Social (INSS), em nome da pessoa interditada. Pretende-se então, a partir da exposição de trechos etnográficos, suscitar a discussão sobre a negociação de significados entre as instituições envolvidas assim como as relações entre os diferentes saberes e práticas, certas vezes conflitantes, ocasionadas por tal processo. O intuito é também contemplar questões morais e éticas presentes nos discursos legais, médicos e sociais sobre os comportamentos categorizados como “sintomas” e “doenças” pelo saber psiquiátrico e, em razão da avaliação da perícia médica, incapacitantes do ponto de vista jurídico, assim como os seus efeitos na vida dos indivíduos interditados e suas famílias.

La thèse porte sur la maladie mentale dans les écritures dramatiques contemporaines d’expression française ainsi que son incidence sur la mise en scène. Le théâtre s’est toujours intéressé à la maladie mentale, et pour certains chercheurs l’art dramatique va même trouver son origine dans celle-ci, à travers des rituels consacrés à la transe et aux rêves. Après un rappel des grands « fous » qui peuplent l’histoire du théâtre européen, que ce soit dans les tragédies grecques, le théâtre shakespearien ou le théâtre classique (nous verrons ainsi que la « folie » sur scène est un thème qui a traversé les siècles), nous nous focaliserons sur une période d’ « ultra-contemporanéité » : en effet, l’essentiel de la thèse portera sur des pièces ne remontant pas au-delà du début des années quatre-vingt. Cela nous permettra de voir que malgré la démocratisation de la psychanalyse, de la psychiatrie et de la psychologie, toutes ces disciplines restent fascinantes et un peu mystérieuses, et que les auteurs aiment à manipuler l’ambigüité entre maladie mentale et originalité du comportement. / The thesis is about mental disorders in French contemporary drama as well as their representations on stage. Theatre has always been concerned by mental diseases and for some researchers origins of the dramatic art have to be found in rituals and dreams. After a recall of the most famous “fools” presents in the history of European drama (Greek tragedies, Shakespeare’s plays or classic French drama) – we will see that “madness” is a long-lasting theme. We will focus on contemporary period: in fact, this thesis studies plays which were written since the beginning of the eighties. We could see that, despite the democratization of psychoanalyse, psychiatry and psychology, they all remain mysterious and fascinating. Playwrights like to use ambiguities about mental disorders and original behaviour.
