O objetivo deste estudo foi avaliar, pela manometria anorretal e pelo escore de Agachan, a eficácia do biofeedback com apnéia pós expiração forçada em portadores de constipação secundária a discinesia do assoalho pélvico. Foram estudados 20 pacientes, 19 do sexo feminino, com idade média de 52,15 (27- 67) anos. O diagnóstico foi realizado por meio de manometria anorretal e empregouse para avaliação clínica o escore de Agachan com valor mínimo igual a 10. As variáveis estudadas foram: pressão anal média de repouso, pressão anal média e área sob a curva ao esforço de evacuação com e sem apnéia pós expiração forçada, e o escore de Agachan, obtidos no pré e pós tratamento. A análise do escore de Agachan evidenciou diferença significativa (p<0,001) entre as avaliações pré e pós tratamento. Os valores de pressão média obtidos ao esforço evacuatório com e sem apnéia também foram estatisticamente diferentes (p<0,05), bem como a área sob a curva (p<0,05). A comparação entre pré e pós tratamento ao esforço de evacuação sem apnéia evidenciou diferença estatística para a área sob a curva. A comparação dos valores pressóricos dos diferentes canais mostrou diferenças significantes apenas nos canais dois e três após o tratamento e para a área sob a curva nos canais dois, três e seis (p<0,05). Ao esforço de evacuação com apnéia observou-se diferença estatística nos valores pressóricos dos canais três e seis, e na área sob a curva do canal três. O biofeedback com técnica de apnéia pós expiração forçada mostrou-se eficaz no tratamento da constipação secundária à discinesia do assoalho pélvico potencializando o relaxamento dos músculos durante o esforço evacuatório / Abstract: Pelvic floor dyssinergia is the main cause of functional constipation. It is a common disorder caused by contraction or insufficient relaxation of the pelvic floor muscles and can be treated by behavioral therapy with biofeedback. The aim of this study was to evaluate by anorectal manometry and Agachan score clinical evaluation, the effectiveness of biofeedback treatment with apnea after forced expiration in patients with constipation secondary to pelvic floor dyssynergia. Twenty patients, being 19 female with mean age of 52.15 (27-67) years were evaluated. The diagnosis was made by anorectal manometry and score of Agachan employed for clinical evaluation. Pelvic floor dyssinergia diagnosis was made with anal pressure increase during the evacuation effort and a minimum score of 10 was considered for eligible patients. The variables studied were mean resting anal pressure, mean anal pressure during straining with and without apnea after forced expiration, the area under the curve of the latter two situations, and Agachan's score before and after treatment. Biofeedback training resulted in reduced Agachan's score (p <0.001). The mean anal pressure during straining as well the area under the curve were both lower with apnea after forced expiration (p <0.05). The comparison between pre and post treatment with straining without apnea, the area under the curve was statistically lower (p< 0.05). The analysis by channels showed that the anal pressure during straining was reduced on channels two and three and on channels two, three and six with the analysis of the area under the curve (p < 0,05). With forced apnea there was statistical difference in anal pressure on channels three and six and the area under the curve on the channel three. Avaliação do tratamento de fissuras anais com a isossorbida tópica a 1% / ASSESSMENT OF TREATMENT OF ANAL FISSURES ISOSORBIDE WITH A TOPICAL 1%.

Chronic anal fissures are benign, painful, found deep ulcers in the anal canal. Affecting young adults and occur especially by the trauma of hard stools, the sphincter hypertonia and poor vascularization in the anterior and posterior regions of the anus. Its main symptom is pain evacuation. Surgical treatment is most effective with high cure rates, but with significant adverse effects such as later anal incontinence. The option of conservative treatment is growing, has managed to decrease temporary the pressure at rest (PREP) and increased blood flow to the anal canal, healing large number of lesions without muscle damage. In order to evaluate the treatment of chronic anal fissure with isosorbide (ISO) 1% topical has conducted a clinical trial, double-blind study in patients enrolled in the service of coloproctology Federal University of Sergipe, in the period of one year. We studied 24 patients, distributed as follows: 14 in group 1 cream isosorbide, and 10 in group 2 placebo. We evaluated the behavior of the PREP, pain relief and degree of wound healing with and without ISO. The results showed that the patients ages ranged from 21 to 69 years, the cleft affects more women and the predominant symptom was pain. Constipation was observed in 58.3%. As for pain, lower intensity was obtained in group 2, but without significance. The healing after 60 days was similar in both groups (50%). The profile of the average PREP 30 and 60 days, there was a decrease in the standard in both groups, but without statistical significance. It was observed that patients cured were those with the greatest reduction in PREP. It was concluded that the ISO did not alter the response pattern gauge, however there was significant clinical improvement in both groups. The healing rate was similar in both groups. / As fissuras anais crônicas são úlceras benignas, dolorosas, profundas encontradas no canal anal. Acometem adultos jovens e ocorrem especialmente pelo trauma das fezes endurecidas, pela hipertonia esfincteriana e pela pobre vascularização nas regiões anterior e posterior do ânus. Seu principal sintoma é a dor evacuatória. O tratamento cirúrgico é o mais efetivo, com altos índices de cura, porém com efeitos adversos importantes como incontinência anal tardia. A adoção da terapêutica conservadora vem crescendo, tem conseguido o decréscimo transitório da pressão de repouso (PREP) e o aumento do fluxo sanguíneo do canal anal, cicatrizando grande número de lesões, sem dano muscular. Com o objetivo de avaliar o tratamento de fissuras anais crônicas com isossorbida (ISO) a 1% tópica, foi realizado um ensaio clínico, duplo-cego em pacientes atendidos no serviço de coloproctologia da Universidade Federal de Sergipe, no período de um ano. Foram estudados 24 pacientes, assim distribuídos: 14 no grupo 1 - creme com isossorbida, e 10 no grupo 2 - placebo. Avaliaram-se comportamento da PREP, melhora da dor e grau de cicatrização das feridas com e sem ISO. Os resultados mostraram que a idade dos pacientes variou de 21 a 69 anos, a fissura acometeu mais as mulheres e o sintoma predominante foi a dor. Constipação foi observada em 58,3%. Quanto à dor, obteve-se menor intensidade no grupo 2, mas sem significância. A cicatrização ao fim de 60 dias foi igual nos dois grupos (50%). Quanto ao perfil das médias de PREP com 30 e 60 dias, houve uma queda no padrão em ambos os grupos, porém sem significância estatística. Observou-se que os pacientes curados foram aqueles com maior redução de PREP. Concluiu-se que a ISO 7 não modificou o padrão de resposta manométrica, no entanto houve melhora clínica importante nos dois grupos. A taxa de cicatrização foi igual em ambos os grupos.

Resultados da avaliação clínica e manométrica anorretal em obesos com indicação de cirurgia bariátrica comparados a indivíduos não obesos / Results of clinical and anorectal manometric evaluation in obese patients referred to bariatric surgery compared to non obese

INTRODUÇÂO: Alguns fatores como paridade, cirurgias pélvicas e hipoestrogenismo têm uma relação direta e bem estabelecida com disfunções da musculatura do assoalho pélvico. Outros fatores como o aumento da pressão intra-abdominal, tal como o que ocorre na obesidade, também podem se associar às disfunções do assoalho pélvico. No entanto, distúrbios da defecação e da continência fecal não são muito bem estudados nesse grupo de pacientes. Assim, a manometria anorretal pode avaliar as disfunções do assoalho pélvico nesse grupo. Logo, o objetivo do presente estudo é comparar resultados clínicos e manométricos em obesos graus II e III, com indicação de cirurgia bariátrica, com pessoas não obesas. MÉTODOS: Estudo caso-controle entre pacientes obesos graus II e III, com indicação de cirurgia bariátrica, e pessoas não obesas, sem sintomas anorretais, pareados por idade e sexo. O número de 26 pacientes em cada grupo foi previamente calculado, através de análise estatística. Os critérios de inclusão foram sexo masculino, mulheres nulíparas e ausência de cirurgia abdominal e anorretal prévias. O grupo de pessoas não obesas se compôs de pessoas sem sintomas de constipação intestinal, ou de distúrbios do assoalho pélvico, acrescido aos mesmos já estabelecidos no grupo de obesos. Realizou-se a manometria anorretal com a técnica estacionária e com cateter de oito canais radiais. RESULTADOS: A média de idade no grupo de obesos foi de 44,8 anos (±12,48 desvio padrão) e de 44,1(±11,78) anos no grupo de não obesos (p=0,829). A média de índice de massa corpórea foi de 48,79 (±8,53) no grupo de obesos e 25,08 (±2,84) nas pessoas não obesas (p=0). A incidência de sintomas de incontinência anal no grupo de obesos foi de 65,4% (17 pacientes). Através da manometria anorretal verificou-se uma redução significativa das pressões de contração voluntária no grupo de obesos (155,55 mmHg e 210,06 mmHg, p=0,004) e uma tendência de redução das pressões de repouso nesse grupo (63,66 mmHg e 74,06 mmHg, p=0,051), em comparação com o dos não obesos. A sensibilidade e a capacidade retal mostraram-se similares entre os grupos de obesos e não obesos. Não se verificou diferença estatisticamente significativa nos resultados da manometria anorretal entre obesos com e sem sintomas de incontinência anal. Além disso, a idade também não demonstrou relação com a incontinência anal nos pacientes obesos. A consistência das fezes, que poderia ser um viés para incontinência anal, foi similar entre os grupos (p=0,953). CONCLUSÃO: Nos pacientes obesos graus II e III, com indicação de cirurgia bariátrica, em relação aos não obesos, as pressões de contração anal voluntária são significativamente menores, com uma tendência de redução das pressões de repouso. Além disso, nesse grupo de pacientes, a prevalência de incontinência anal de qualquer tipo é elevada, independente da idade, do sexo e do índice de massas corpórea - o que não se conhecia previamente / Some factors such as parity, pelvic surgeries and hypoestrogenism are well established to have a direct relation to dysfunction of pelvic floor muscles in women. Other factors related with higher intra-abdominal pressure such as morbid obesity and constipation to be also related with pelvic floor dysfunction. Morbid obesity is configured nowadays as a public health problem due to its increasing incidence. However defecation disturbance and fecal continence are not very well studied in this group of patients. Anorectal manometry could objectively represent anorectal dysfunction in this group of patients. Therefore, the objective of the present study is to compare manometric and clinical results of morbid obese and non obese patients. A case-matched study between morbid obese patients, elective to bariatric surgery, and non obese patients without anorectal complaints was conducted. The groups were paired by age and gender. The number of patients in each group of 26 patients was previously calculated with a power analysis. Inclusion criteria was male sex, nuliparous women, absence of abdominal and anorectal surgeries. Non obese group was comprised by patients without any symptoms of constipation or pelvic floor dysfunction. Anorectal manometry was performed with an eight radial channels catheter water perfused and stationary technique. The mean age was 44.8 years (±12.48) in the morbid obese group and 44.1 years (±11.78) in the non obese group (p=0,829). The mean body mass index was 48.79 (±8.53) in the morbid obese group and 25.08 (±2.84) in the non obese group (p=0). The incidence of any degree of fecal incontinence in the morbid obese group was 65.4% (17 patients); besides a significant reduction of mean squeeze pressure (155.55mmHg vs. 210.06mmHg, p=0.004) and a tendency of reduction of mean rest pressures (63.66mmHg vs. 74.06mmHg, p=0.051) in comparison to non obese. The rectal sensibility and maximum capacity were similar comparing morbid obese and non obese patients. There was no significant difference when comparing manometric results of obese patients with and without symptoms of fecal incontinence. Also, older patients did not have relation to fecal incontinence. Fecal consistency that could be a bias for fecal incontinence was similar for the groups studied. Anal squeeze pressures are significantly lower in morbid obese patients and there is a tendency of reduction of rest pressure in this group of patients in comparison to the non obese population. Furthermore, in this group of patients, the prevalence of anal incontinence of any type is high, independently of age, sex and body mass index- what was not known previously

Função motora do esôfago em pacientes com doença do refluxo gastroesofágico / Esophageal motor function in patients with gastro-esophageal reflux disease

Introdução: A diminuição do tônus basal e da extensão do esfíncter inferior do esôfago são considerados como principais mecanismos responsável pela ocorrência de refluxo gastroesofágico. Um adequado clareamento esofágico depende da presença de peristaltismo primário e secundário efetivos. Ainda há dúvidas se o achado de alterações do peristaltismo esofágico em pacientes com doença do refluxo gastroesofágico é uma anormalidade primária ou surge como consequência da agressão causada pelo refluxo. Objetivo: avaliar as alterações motoras esofágicas do esfíncter inferior do esôfago e do corpo esofágico em diferentes formas da doença do refluxo gastroesofágico. Métodos: foram selecionados 268 prontuários de pacientes encaminhados para avaliação motora do esôfago através de manometria como parte da investigação diagnóstica da doença do refluxo gastroesofágico e foram distribuídos em quatro grupos: SE: 33 pacientes sem esofagite ao estudo endoscópico; EE: 92 pacientes que apresentavam esofagite erosiva (classificação de Los Angeles); BC: 101 pacientes que apresentavam esôfago de Barrett curto (< 3 cm) e BL: 42 pacientes que apresentavam esôfago de Barrett longo (> 3 cm). Resultados: O grupo SE apresentou um tamanho médio do esfíncter inferior do esôfago maior quando comparado aos grupos EE, BC e BL, estes foram semelhantes quando comparados entre si. Considerando esfíncter curto quando seu tamanho total encontrava-se menor do que 2 cm, os grupos EE, BC e BL foram semelhantes quando comparados entre si. Quanto à média de pressão do esfíncter, observamos que o grupo SE apresentou valor médio maior em relação aos grupos EE, BC e BL, estes foram semelhantes quando comparados entre si. Observou-se que os grupos EE e BL foram semelhantes e apresentaram maior percentual de hipotonia acentuada do esfíncter inferior do esôfago quando comparados ao grupo BC. Os grupos EE, BC e BL apresentaram amplitude de contração no segmento distal, significativamente inferiores quando comparados ao grupo SE; os grupos BC e BL foram semelhantes quando comparados entre si. Os grupos EE, BC e BL foram semelhantes em relação ao percentual de hipocontratilidade acentuada do segmento distal do corpo esofágico. Em relação à motilidade esofágica, observou-se que não houve diferença entre os grupos EE, BC e BL, o grupo SE não apresentou esta alteração. Conclusões: Os doentes com sintomas típicos de refluxo gastroesofágico, mas sem esofagite ao estudo endoscópico, não apresentaram comprometimento da função motora esofágica. Aqueles com esofagite de refluxo e esôfago de Barrett curto tiveram comprometimento da função motora esofágica, intermediárias entre os pacientes sem esofagite e com esôfago de Barrett longo. As alterações mais intensas na motilidade esofágica e esfíncter inferior do esôfago foram mais observadas no grupo com esôfago de Barrett longo. Estes fatos indicam que as alterações motoras do esôfago surgem como conseqüência do comprometimento da mucosa esofágica por RGE. / Introduction: A more extensive damage to the system of refluxate contention and to the esophageal clearance are thought to be associated to the increased occurrence of esophageal inflammation. Objective: This study aimed to assess the esophageal motor alterations of the lower esophageal sphincter and esophageal body, in the various forms of gastro-esophageal reflux disease. Methods: two hundred and sixty eigth patients were selected and split into four groups: NE: 33 patients who had presented with typical complaints of gastroesophageal reflux, albeit with no esophagitis on endoscopy; EE: 92 patients who had erosive esophagitis (Los Angeles classification); SBE: 101 patients who had short Barretts esophagus (< 3 cm); and LBE: 42 patients who had long Barretts esophagus (> 3 cm). All the patients underwent esophageal manometry with an 8-channel computerized system and a low compliance pneumo-hydraulic perfusion pump. Results: The manometric evaluation of the esophagus detected that the mean lower esophageal sphincter length in group NE was longer in comparison with the other groups (EE, SBE and LBE), which were all similar among themselves. Taking lower esophageal sphincter to be shortened with a total length equal or shorter than 2 cm, the groups EE, SBE and LBE were similar when compared one to another. This abnormality was not detected in group NE. Lower esophageal sphincter pressure, as assessed by the mean respiratory pressure, showed the group NE the highest mean value, while no difference was found between groups EE, SBE and LBE. Percentages of patients showing marked lower esophageal sphincter hypotonia (<6 mmHg) showed the groups EE and LBE had higher percentages of hypotonia as compared with group SBE. Comparison between groups EE and LBE in this respect yielded no difference. As to the mean amplitude of contraction of the distal segment swallowing complex showed that the groups EE, SBE and LBE had significantly lower amplitudes when compared with group NE; groups SBE and LBE were similar. The percentage of marked hypocontractility of the distal segment of the esophageal body (< 30 mmHg) was similar among groups EE, SBE and LBE. In relation to the esophageal motility, no difference could be detected among groups EE, SBE and LBE. Conclusions: patients who had presented with typical complaints of gastroesophageal reflux disease, albeit with no esophagitis on endoscopy didnt have alterations of esophageal motor function. The groups who had erosive esophagitis and short Barretts esophagus had intermediary alterations of esophageal function; the group long Barretts esophagus showed lower mean value and higher percentage of marked hypotonia of LES and the highest percentage of marked hypocontractility and alteration in esophageal periltalsis. These findings sugest also that esophageal abnormalities are secondary to esophageal mucosa damage.

Efeitos da prostatectomia perineal sobre a continência anal: estudo clínico e manométrico / Effect of perineal prostatectomy on anal continence: a clinical and manometric study

Introdução: A prostatectomia perineal tem sido proposta como um procedimento seguro e pouco invasivo, sem comprometer os princípios oncológicos. No entanto, este acesso tem sido questionado sobre o risco de promover a incontinência anal. Objetivo: avaliar os efeitos do acesso para a prostatectomia perineal no mecanismo de continência anal. Métodos: Trinta e um pacientes com indicação cirúrgica de prostatectomia perineal foram avaliados entre agosto de 2008 e maio de 2009. Dados do pré e pós operatório (8 meses): estadiamento do câncer de próstata, a avaliação clínica (Índice de incontinência anal da Cleveland Clinic - CCISS), índice de qualidade de vida na incontinência anal (FIQL) e manometria anorretal. Os parâmetros médios manométricos foram: pressão de repouso (RP / mmHg), pressão de contração voluntária máxima (MSP / mmHg), zona de alta pressão (ZAP / cm), índice de fadiga do esfíncter (SFI / min.), índice de assimetria esfincteriana (SAI /%), limiar de sensibilidade retal (RST / ml) e volume retal máximo tolerado (MTRV / ml). Resultados: Foi concluída a avaliação em vinte e três pacientes, com média de idade de 65 (54-72) anos. Pré-operatório: o peso médio da próstata foi de 34,5 (24-54) gramas, Gleason intervalo de valor da pontuação 06/07. Os valores médios pré e pós-operatório da CCISS foram 0,9 ± 1,9 e 0,7 ± 1,2 (p> 0,05) e não houve uma mudança significativa no valor FIQL. Os valores médios pré e pós operatório de parâmetros manométricos foram, respectivamente: RP: 64 ± 23 e 65 ± 17, SP: 130 ± 41 e 117 ± 40, ZAP: 3,0 ± 0,9 e 2,7 ± 0,8, SFI: 3,0 ± 2,1 ± 11 e 5.4, RST: 76 ± 25 e 71 ± 35, MTRV 157 ± 48 e 156 ± 56, e SAI: 22,4 ± 9 e 14,4 ± 5, sendo o SAI o único parâmetro com mudança estatisticamente significativa (p: 0, 003). Conclusão: O acesso perineal para prostatectomia não afetou os parâmetros de continência anal. Houve, no entanto melhora na simetria esfincteriana / Introduction: Perineal prostatectomy has been proposed as a less invasive and a safer procedure, without compromise of oncological principles. However, this access has been questioned about the risk of promoting anal incontinence. Purpose: this study aimed to evaluate the effects of perineal access for prostatectomy in continence mechanism. Methods: Thirty one patients with surgical indication for perineal prostatectomy were evaluated between August 2008 and May 2009. Preoperative and postoperative (8 months) data included: prostate cancer staging, clinical evaluation (Cleveland Clinic anal incontinence score system - CCISS), Fecal incontinence quality of life score (FIQL) and anal manometry. Mean manometric parameter were: resting pressure (RP/mmHg), maximal squeeze pressure (MSP/mmHg), high pressure zone (HPZ/cm), sphincter fatigue index (SFI/min), sphincter asymmetry index (SAI/%), rectal sensory threshold (RST/ml) and maximum tolerated rectal volume (MTRV/ml). Results: Twenty three patients, mean age 65 (54-72) years, completed evaluation. Preoperative: prostate weight was 34.5 (24-54) grams, Gleason score value range 6 /7. Mean pre and postoperative values of CCISS were 0.9±1.9 and 0.7±1.2 (p>0.05) and there was not a significant change in FIQLS value. The mean preoperative and postoperative values of manometric parameters were, respectively: RP: 64±23 and 65±17, SP: 130±41 and 117±40, HPZ: 3.0±0.9 and 2.7±0.8, SFI: 3.0±11and 2.1 ±5.4, RST: 76±25 and 71±35, MTRV 157±48 and 156±56, and SAI: 22.4±9 and 14.4±5. Significant statistics change only in the SAI (p=0,003). Conclusion: The perineal prostatectomy did not affect anal continence parameters

Pacientes e métodos: Trinta e um homens voluntários com idade média de 51,8 ± 4,9 anos foram submetidos a manometria anorretal e avaliação do índice de massa corporal e do nível de atividade física, este pelo Questionário Internacional de Atividade física. Na manometria anorretal com utilização de sonda com balão e um único canal de medição pressórica, foram avaliados os valores de pressão retal e pressão de repouso e de contração máxima do esfíncter anal externo. Resultados: Na manometria, tiveram distribuição normal os valores de pressão retal com média de 44,4 ± 13,8 cm/H2O e os valores de força de contração voluntária máxima com média de 164,9 ± 32,4 cm/H2O. Pacientes com nível de atividade física alta tiveram menor força de contração máxima (155,4 ±26,6) comparativamente com pacientes com baixa atividade física (193 ±40,2) (p=0,039). Houve associação positiva entre pressão retal e pressão de repouso (p=0,018) e tendência à associação positiva entre pressão retal e IMC (p=0,08), pressão de repouso e IMC (p=0,093) e pressão retal e contração voluntária máxima (p=0,098). Conclusão: Quanto maior o nível de atividade física do paciente menor a força de contração voluntária máxima do assoalho pélvico. Esta parece aumentar de maneira compensatória com o aumento da pressão retal, que tende a ser maior nos pacientes com maior IMC. / Aims: Indirectly evaluate the pelvic floor strength through anorectal manometry and evaluate the association between pelvic floor strength and physical activity, body mass index (BMI) and rectal pressure in healthy men over the age of 45. Methods: thirty-one male volunteers with a mean age of 51.8 ± 4.9 years were subject to anorectal manometry and evaluation of BMI and level of physical activity through the International Physical Activity Questionnaire. In the manometry a probe with balloon and a single pressure channel was used to measure rectal pressure and external anal sphincter pressure at rest and maximum contraction. Results: Manometry, showed a normal distribution of values of rectal pressure with a mean of 44.4 ± 13.8 cm/H2O and the values of maximum contraction pressure with a mean of 164.9 ± 32.4 cm/H2O . Patients with high levels of physical activity had a lower maximum contraction pressure (155.4 ± 26.6) compared with patients with low physical activity (193 ± 40.2) (p = 0039). There was a positive association between rectal pressure and pressure at rest (p = 0018) and tendency toward positive association between rectal pressure and BMI (p = 0.08), the pressure at rest and BMI (p = 0093) and rectal pressure and maximum contraction (p = 0098). Conclusions: The higher the level of physical activity the lower the strength of maximum contraction of the pelvic floor. This seems to be a compensatory increase with increasing rectal pressure, which tends to be higher in patients with higher BMI.

As miopatias mitocondriais formam um grupo de desordens clinicamente heterogêneas que podem afetar múltiplos sistemas além do músculo esquelético. A oftalmoplegia externa crônica progressiva (CPEO) é um tipo de miopatia mitocondrial que tem como características alterações nos movimentos oculares, ptose, podendo ter acometimento da musculatura facial, além de atrofia muscular de membros. A fatigabilidade precoce pode ser a queixa principal e claramente desproporcional ao grau de fraqueza e atrofia muscular detectada. A disfagia na doença é uma manifestação descrita por muitos autores, porém pouco estudada ou caracterizada. O presente estudo teve como objetivo avaliar a deglutição de pacientes com miopatia mitocondrial do tipo CPEO através de avaliação clínica, manométrica e videofluoroscópica. Para tanto, foram selecionados 14 pacientes com diagnóstico de miopatia mitocondrial do tipo CPEO, em acompanhamento no Ambulatório de Doenças Neuromusculares do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo, independente de apresentarem queixas ou não quanto à alimentação. A idade variou de 15 a 62 anos, com idade média de 35,3 anos sendo 5 (35,7%) do gênero masculino e 9 (64,3%) do feminino. O grupo controle foi formado por 16 indivíduos saudáveis (sem doenças neurológicas ou queixas quanto à alimentação) com idade variando de 21 a 44 anos, idade média de 27,5 anos, sendo 6 (37,5%) do gênero masculino e 10 (62,5%) do feminino. Na avaliação clínica, além da anamnese, foi realizada avaliação estrutural e funcional da deglutição. Para isso foram utilizadas as consistências pastosa, líquida e sólida, em volume livre, conforme hábito do paciente. Na avaliação manométrica, as medidas foram feitas durante a deglutição líquida, para avaliar as pressões intraluminares no corpo do esôfago. Foram realizadas 10 deglutições com intervalo de 30 segundos entre elas e após cinco minutos de repouso mais 10 deglutições com intervalo de 10 segundos entre elas. Foram consideradas as medidas da pressão intraluminal, sua duração, área sob a curva e tempos parciais e totais de deslocamento da onda. Na avaliação videofluoroscópica utilizaram-se as dietas pastosa, líquida e sólida, sendo as duas primeiras em volume controlado de 5ml. Para todas as consistências foram realizadas três ofertas, sendo que para o pastoso e sólido esse processo foi repetido. Além da dinâmica da deglutição na fase orofaríngea foram analisados os tempos de fase oral (TFO), depuração faríngea (DF), trânsito faríngeo (TF), trânsito pela transição faringoesofágica (TTFE) e tempo de movimentação hióidea (TMH). Em anamnese observamos que 9 (64,3%) pacientes tinham queixas quanto a alimentação mas apenas 7 (50%) apresentaram alterações na avaliação clínica, sendo essas alterações principalmente para as consistências pastosa (57,1%) e sólida (100%). As principais alterações foram fase oral prolongada, mastigação inadequada, deglutições múltiplas e fadiga. Na avaliação manométrica foi observado redução da motilidade esofágica sendo essa principalmente no esôfago proximal, com amplitude, duração e área sob a curva reduzidas. Quando comparado o desempenho do esôfago nos diferentes intervalos de deglutição, não houve relação com a presença da doença, pois a diferença foi significante tanto para os pacientes como para os controles. Na videofluoroscopia da deglutição, assim como na avaliação clínica, as principais alterações encontradas foram para as consistências pastosa e sólida, sendo elas tanto em fase oral como faríngea. Apesar disso não foi encontrado nenhum episódio de aspiração laringotraqueal. Quanto aos tempos de deglutição, apenas o TFO foi significantemente maior nos pacientes, sendo menor nos outros parâmetros como TF e TTFE. Ao compararmos as primeiras com as últimas deglutições para as consistências pastosa e sólida, verificamos que os pacientes apresentaram um aumento dos tempos de DF, TF, TTFE e TMH para a consistência sólida, sendo significante apenas para TF. O estudo permitiu concluir que pacientes com miopatia mitocondrial do tipo CPEO apresentam dificuldades de deglutição, com alterações orofaríngeas e esofágicas, sendo maiores para as consistências pastosa e sólida. / Mitochondrial myopathies constitute a group of clinically heterogenous disorders which can affect multiple systems besides the skeletal muscle. The chronic progressive external ophthalmoplegia (CPEO) is a type of mitochondrial myopathy charactherized by eye movements alterations, ptosis, which may attack the facial musculature, as well as atrophy the muscles of members. The early fatigability may be the main complaint and clearly disproportional to the degree of weakness and muscular atrophy detected. The dysphagia in the disease is a manifestation described by several authors, however very little studied and characterized. The present study aimed at evaluating the swallowing of patients with chronic progressive external ophthalmoplegia (CPEO) mitochondrial myopathy, followed at the Neuromuscular Diseases Clinic at Hospital das Clínicas of Faculdade de Medicina de Ribeirão Preto of Universidade de São Paulo, independently whether or not they presented complaints about alimentation. The age varied from 15 to 62 years old, with average age of 35,3, which were 5 (35,7%) males and 9 (64,3%) females. The control group was constituted by 16 healthy individuals (without neurological diseases or complaints about alimentation) with age varying from 21 to 44 years old, average age of 27,5, which were 6 (7,5%) males and 10 (62,5%) females. In the clinical evaluation, anamnesis, structural and functional assessment of swallowing were carried out. According to the patients habits, pasty, liquid and solid consistency were used. In the manometric evaluation, the measurements were done during liquid swallowing, to assess the intraluminal pressure in the esophagus. It was performed 10 swallowings with intervals of 30 seconds among them, and after five minutes of rest, 10 more swallowings with intervals of 10 seconds among them. The intraluminal pressures measurement, its duration, area under curve and wave displacements partial and total time were considered. In the videofluoroscopic evaluation, pasty, liquid and solid diets were used, in which the first two with a controled volume of 5ml. For all the consistencies were realized three offer, and for the solid and pasty, these process were repeated. Besides the swallowing dynamic in the oropharyngeal phase, the oral phase time (OPT), the pharyngeal depuration (PD), pharyngeal transit (PT), pharyngoesophageal transition transit (PTT), hyoid movement time (HMT) were analysed. In the anamnesis, we observed that 9 (64,3%) patients had complaints about alimentation, but only 7 (50%) presented alterations in the clinical evaluation, and these alterations were mainly for pasty (57,1%) and solid (100%)consistencies. The main alterations were extended oral phase, inadequate mastication, multiple swallowings and fatigue. In the manometric evaluation, the esophageal motility reduction was observed, which was mainly in the proximal esophagus, with amplitude and area under curve reduced. When compared the esophagus development in the different intervals of swallowing, no relation to presence of the disease was found, because the difference was significant for both patients and control group. In the swallowing videofluoroscopy, as well as the clinical evaluation, the main alterations were found in the pasty and solid consistencies, in both oral and pharyngeal phases. Despite that, no laryngeal - tracheal aspiration episode was found. Regarding the swallowings times, only oral phase time (OPT) was actually longer in patients, which was shorter in other parameters such as pharyngeal transit (PT) and pharyngoesophageal transition transit (PTT). When we compared the first swallowings with the last ones for pasty and solid consistencies, the patients presented a increase that time PD; PT; PTT and HMT for solid consistencies with significant value only for PT. The study allowed us to conclude that patients with chronic progressive external ophthalmoplegia (CPEO) mitochondrial myopathy present swallowing dificulties, with oropharyngeal and esophageal alterations, which were greater for pasty and solid consistences.

Associação da motilidade esofágica ineficaz com a exposição ácida elevada no esôfago distal / Association of pathological acid exposure in the distal esophagus with inefficient esophageal motility

Objetivos: Avaliar a associação entre a dismotilidade esofágica, caracterizada como Motilidade Esofágica Ineficaz (MEI), com a presença de refluxo ácido patológico avaliado pela pH-metria de 24 horas, controlando por Esfíncter Esofágico Inferior (EEI) estruturalmente defeituoso, Hérnia Hiatal (HH) e Esofagite, em pacientes em investigação de Doença do Refluxo Gastroesofágico. Métodos: Foram estudados 311 pacientes referenciados para investigação de DRGE em laboratório de motilidade esofágica. Os pacientes foram submetidos à Endoscopia Digestiva Alta (EDA), Manometria Esofágica, pHmetria Esofágica de 24 horas e a uma entrevista sobre os sintomas clínicos apresentados. Foram comparados os grupos de pH-metria negativa com o de pH-metria positiva quanto à presença dos fatores de risco – MEI, EEI defeituoso, HH e Esofagite. A associação entre MEI e pH-metria positiva foi primeiramente avaliada através de análise univariada e, posteriormente, através de análise de regressão logística (multivariada). Resultados: Do total de 311 pacientes estudados, 208 preencheram os critérios de inclusão. A idade média foi 47 anos, com 88 pacientes apresentando pH-metria normal e 120 pH-metria positiva. Após a análise univariada, foi observado que a ocorrência de MEI, EEI defeituoso e HH foi significativamente maior no grupo de pH-metria positiva. Após análise de regressão logística, a ocorrência de MEI e EEI defeituoso permaneceram significativamente maior no grupo de pH-metria positiva. Conclusões: MEI está associada à presença de refluxo ácido anormal, avaliado através de pH-metria esofágica de 24 horas, independentemente da presença de EEI defeituoso, HH ou Esofagite. / Objectives: To assess the association between esophageal dysmotility, characterized as inefficient esophageal motility (IEM), and the presence of pathological acid reflux due to a structurally defective lower esophageal sphincter (LES), hiatus hernia (HH), or esophagitis in patients suspected of having gastroesophageal Reflux reflux disease (GERD). The following risk factors of patients in the negative pH-metry group were compared to those in the positive pH-metry group: IEM, defective LES, HH, and esophagitis. The association between IEM and positive pH-metry results was first assessed by means of univariate analysis and later determined with logistic regression analysis (multivariate). Results: Of the total 311 patients studied, 208 met the inclusion criteria (mean age 47 years); 88 had normal pH-metry reslults and 120 had positive pH-metry results. Univariate analysis revealed that the occurrence of IEM, defective LES, and HH was significantly greater in the positive pH-metry group. Following logistic regression analysis, the occurrence of IEM remained significantly greater in the positive pH-metry group. Conclusions: IEM is associated with the presence of abnormal acid reflux, as assessed by 24-hour esophageal pH-metry, regardless of the presence of defective LES, HH, or esophagitis.
