Descreve-se características demográficas, clínicas e de saúde, identificando as necessidades de saúde, sistematizando os cuidados prestados à saúde e construindo um fluxograma dos acompanhamentos clínicos desta comunidade. Caracteriza-se como um estudo de intervenção, com enfoque quantitativo. Trata-se de uma comunidade de 13 religiosos jesuítas, com idades variadas e atividades laborativas diversificadas. O gerenciamento das ações de enfermagem implantadas centraliza-se no Instituto São José, Casa de Saúde dos jesuítas da BRM em São Leopoldo/RS. A idade variou de 33 a 82 anos, quanto à procedência, a maioria são descendentes do estado do Rio Grande do Sul. Na escolaridade, aparece o ensino superior com maior evidência. Em relação à categoria religiosa, tiveram-se Padres, Irmãos e Escolásticos, com ocupações diversificadas, de acordo com a categoria religiosa e missão recebida. O fato de serem proprietários da instituição, faz com que a jornada de trabalho seja máxima estabelecida por lei. Outro ponto pesquisado foi o lazer, esse aparece entre todos de forma variada. Nos dados clínicos, no que se refere ao Índice Massa Corporal (IMC) e a Mini Avaliação Nutricional (MAN), foram encontrados jesuítas, abaixo do peso, risco de desnutrição, desnutridos, com sobrepeso e obesos. Também apareceu o hábito de fumar e de ingerir álcool. . Poucos realizaram a prática de atividade física. Os diagnósticos clínicos que mais apareceram foi de HAS e DM2. A maioria não tem médico assistente e não realizam exames médicos periodicamente. Nenhum dos pesquisados possuem o esquema vacinal de forma completa e atualizada. No uso das redes de apoio em saúde, à procura ocorre entre os postos de saúde, farmácias, centro de exames diagnósticos e radiológicos e clinica de fisioterapia. As medicações são gerenciadas pelos próprios sujeitos pesquisados e prescritas de acordo com os diagnósticos clínicos. Na auto percepção em saúde, a avaliação ficou entre “boa” e “muito boa”, o que leva a crer, que todos tem uma percepção positiva de sua saúde. A espiritualidade dos jesuítas, demonstrou que, há uma relação intima entre o transcendente e o imanente. De acordo com as situações de saúde dos Jesuítas, enumeraram-se diagnósticos de enfermagem e intervenções de acordo com a taxonomia da Classificação Internacional das Práticas de Enfermagem em Saúde Coletiva (CIPESC), permitindo elaborar um plano de cuidados de enfermagem para a comunidade João Paulo II e outro de caráter individual. Esse plano de cuidados deve ser acompanhado periodicamente pelo enfermeiro, através de um fluxograma. Outras situações encontradas como, não realizar exames médicos periódicos; não possuir médico assistente; não usufruir de redes de apoio, não ter orientações sobre espiritualidade e o uso de medicações, não são contemplados com os diagnósticos e intervenções da CIPESC. Espera-se, com essas intervenções, facilitar a elaboração de um instrumento para uma boa gestão em saúde dos Jesuítas das demais residências da Província BRM. / The present study aimed to the implementation of management of nursing care in the community John Paul II the Province of Southern Brazil (BRM), located in Florianópolis / SC as pilot plan study. Describes the demographic, clinical and health by identifying health needs, streamlining the health care and building a flowchart of clinical accompaniments of this community. Characterized as an intervention study with quantitative approach. It is a community of 13 Jesuits, of varying ages and diverse work activities. The management of nursing actions implemented focuses on the Institute of St. Joseph, Jesuit House Health of BRM in São Leopoldo / RS. The age ranged from 33 to 82 years, on the merits, the majority are descendants of the state of Rio Grande do Sul The members have higher education appears more clearly. Regarding the religious category, had to priests, brothers and scholastics, with diverse occupations, according to the religious category and mission received. The fact that they are owners of the institution, makes the workday is maximum established by law. Another point was researched leisure, this appears among all so varied. Clinical data in relation to Body Mass Index (BMI) and the Mini Nutritional Assessment (MNA), were found jesuits, underweight, risk of malnutrition, undernourished, overweight and obese. Also appeared in the habit of smoking and drinking alcohol. . Few realized the physical activity. Clinical diagnoses that appeared was more hypertension and T2DM. Most have no doctor and do not conduct medical examinations periodically. None of the respondents have the vaccination schedule so complete and updated. The use of support networks in health, looking occurs between health centers, pharmacies, examination center and radiological diagnostics and physiotherapy clinic. Medications are managed by the subjects researched and prescribed according to clinical diagnoses. In self-perception in health assessment was between "good" and "very good", which leads us to believe that everyone has a positive perception of their health. The spirituality of the Jesuits, has shown that there is an intimate relationship between the transcendent and the imminent. According to the health situation of the Jesuits, were listed nursing diagnoses and interventions according to the taxonomy of the International Classification of Nursing Practice in Public Health (CIPESC), allowing a plan of nursing care to the community John Paul II and another individual character. This care plan must be monitored periodically by a nurse, via a flowchart. Other conditions as found, do not perform periodic medical examinations; has no doctor, do not take advantage of support networks, not guidelines on spirituality and the use of medications, are not included with the diagnoses and interventions CIPESC. It is expected, with these interventions, facilitate the development of an instrument for good health management Jesuit Province of the other residences BRM.

A experiência do câncer peniano e seus tratamentos na perspectiva dos adoecidos / The experience of penile cancer and treatment from the perspective of patients

O câncer de pênis é uma doença de baixa incidência e sua manifestação está associada a fatores intrínsecos e extrínsecos, e sua principal forma de tratamento é a penectomia. Devido à doença ocorrer no órgão genital masculino busquei conhecer como os homens adoecidos lidam com suas masculinidades na situação do adoecimento. Dessa forma, propus a tese de que a cultura influencia na forma como os homens sobreviventes do câncer de pênis lidam com suas masculinidades. Elaborei o objetivo de interpretar os significados atribuídos à experiência com o câncer de pênis e seus tratamentos pelos homens adoecidos. O referencial teórico adotado foi o da antropologia médica e das masculinidades, pois ambas consideram a cultura como um elemento importante no mundo social. Para conhecer a cultura do outro é necessário vivenciá-la de perto, por este motivo optei por desenvolver a etnografia narrativa. Esse método propõe ao pesquisador o uso de técnicas como a observação participante, múltiplas entrevistas, uso do diário de campo, análise temática e a apresentação dos resultados na estrutura de narrativas que contenham a estória das vivências dos participantes. Ressalto que a narrativa tem como principal objetivo mostrar as transformações vividas pelos depoentes, considerando os aspectos comuns e divergentes dos entrevistados. Foram selecionados 18 homens com o diagnóstico de câncer de pênis no ambulatório de uro- oncologia de um hospital do interior paulista. As características sociais e clínicas dos entrevistados são semelhantes aos dados nacionais e internacionais em relação à baixa incidência da doença, faixa etária e os tratamentos empregados. Diante do processo de análise foi possível construir oito sínteses temáticas: O câncer de pênis e seu itinerário; O corpo após a doença e seus tratamentos; O câncer e suas repercussões sociais: o trabalho, o lazer e as relações sociais; Por trás do visível: a confidencialidade da doença; A sexualidade pós- tratamento; Um homem inteiro ou meio homem: a ressignificação das masculinidades; Recursos de apoio social; O hoje e as expectativas para o futuro. Por meio das sínteses narrativas é possível conhecer os sentidos que os homens empregaram ao adoecimento e como a doença afetou sua masculinidade. Estes achados permitiram alcançar o significado de ser homem sobrevivente do câncer de pênis e seus tratamentos, que pode ser compreendido pela expressão \"ser meio homem\". Desde o início do diagnóstico até a vida no pós- tratamento, os homens negociam suas masculinidades com o mundo social, e percebem que algumas características adquiridas durante o adoecimento os tornam diferente dos demais homens, tais como se assumir doente, a extirpação total ou parcial do pênis, redução da força física, impossibilidade de trabalhar e alterações na prática sexual. Comprovo, então, que a cultura tem influência na forma que os homens lidam com suas masculinidades, uma vez que os adoecidos constantemente revisitam seus padrões culturais de masculinidade para obedecê- lo ou ressignificá-lo. Os enfermeiros podem utilizar os achados deste estudo para refletir sua prática de cuidado aos homens com câncer e para desenvolverem outras pesquisas que o complementem / Penile cancer is a disease of low incidence, and its manifestation is associated with intrinsic and extrinsic factors, and its main form of treatment is the penectomy. Due to the fact that the disease occurs in the male genital organ, I sought to know how the ill men deal with their masculinities in the disease situation. Thus, I proposed the thesis that culture influences in the way as the penile cancer survivors deal with their masculinities. The aim of this study is to interpret the meanings attributed to the experience with penile cancer and its treatments by the ill men. The theoretical framework adopted was medical anthropology and masculinities, since both considered culture as an important element in the social world. In order to understand the culture of the other people is necessary to experience it closely, for this reason I chose to develop the narrative ethnography. This method proposes to the researcher the use of techniques such as participant observation, mult iple interviews, the use of field diary, thematic analysis and the presentation of results in the structure of narratives that contain the story of participants\' experiences. I emphasize that the narrative has as main objective to show the transformations lived by the deponents, considering the common and divergent aspects of the interviewees. We selected 18 men with the diagnosis of penile cancer in the uro-oncology clinic of a hospital in the interior of São Paulo. The social and clinical characteristics of the interviewees are similar to the national and international data regarding the low incidence of the disease, age group and the treatments used. From the process of analysis it was possible to construct eight thematic syntheses: Penile cancer and its itinerary; The body after disease and its treatments; Cancer and its social repercussions: work, leisure and social relations; Behind the visible: the confidentiality of the disease; The sexuality post-treatment; The whole man or half man: resignification of masculinities; Social support resources; The today and expectations for the future. Through narrative syntheses it was possible to comprehend the meaning attributed by the patients, what they assigned to illness and how that disease affected their masculinity. These findings allowed us to achieve the meaning of being a man penile cancer survivor and its treatments, which can be understood by the expression \"being half man\". From the onset of diagnosis up to life post-treatment, men negotiate their masculinities with the social world, and realize that some characteristics acquired during illness make them different from other men, such as, to assume themselves as sickness, the fact that they have total or partial penile extirpation, reduction of physical strength, impossibility of working and changes in sexual practice. I then show that culture influences the way men deal with their masculinities, since the ill men are constantly revisiting their cultural patterns of masculinity to obey or re-signify it. Nurses can use the findings of this study to reflect their practice of caring for men with cancer and to develop further researches that complements this one

A Cross-National Analysis of the Human Papillomavirus, Sexually Transmitted Infections, and Sexual Behavior among Men

There is a paucity of research on the risk for sexually transmitted infections (STIs) and sexual behavior among general populations of men. Research with male populations predominantly has focused on those subgroups considered to be at high risk of disease transmission, such as gay and bisexual men, injection drug users, and adolescents/young adults. Considerably fewer studies have examined factors among men, in general, and heterosexual men, specifically. Therefore, I conducted analyses with a cross-national sample of adult, sexually active men in Brazil, Mexico, and the United States to investigate sexual behaviors and risk factors associated with the human papillomavirus (HPV) and other STIs. The research questions were: 1) How does sexual risk differ among men residing in Brazil, Mexico, and the US by age cohort?; 2) Do men's sexual behaviors change after being tested for HPV and other STIs?; and 3) Do men's sexual behaviors change after being informed of diagnosis with HPV and other STIs? These research questions were explored through a quantitative assessment of secondary data collected through a risk factor questionnaire administered using computer assisted self-interviewing. The study findings underscore the need for public health interventions to address STI risk and transmission among men across the lifespan. Additionally, this study revealed the potential of STI testing as an effective strategy to reduce sexual risk-taking among men. While this research identifies key issues of importance in improving men's sexual health, additional research is needed to provide an enhanced contextual understanding of socio-cultural, interpersonal, and community level factors that affect sexual behaviors and decision-making among men.

Da paternidade responsável à paternidade participativa? representações de paternidade na Política Nacional de Atenção Integral à Saúde do Homem (PNAISH)

Nesta dissertação, problematizo as representações de paternidade veiculadas, atualizadas e (re)produzidas na Política Nacional de Atenção Integral à Saúde do Homem (PNAISH), política pública atual do governo federal brasileiro que tem como objetivo promover a melhoria da saúde de homens, facilitando o acesso aos serviços de assistência integral à saúde. O estudo inscreve-se no referencial teórico-metodológico dos Estudos de Gênero e dos Estudos Culturais, situando-se no campo da Saúde Coletiva. Utilizo a pesquisa documental para reunir o conjunto de materiais examinados na perspectiva da análise cultural, a fim de descrever e analisar os ensinamentos da política aos homens-pais, operando com os conceitos de gênero, representação, cultura, poder, política pública, saúde e saúde coletiva. Analiso como os homens passam a ser convocados a participar e integrar as rotinas de cuidado com os/as filhos/as e como os profissionais de saúde, no âmbito do SUS, buscando-se promover e/ou incluir a participação dos homens-pais nos espaços de saúde. A PNAISH atua como um artefato cultural que incide sobre representações de paternidade, envolvendo-se com a nomeação, a classificação e a socialização dos homens-pais em meio às disputas travadas desde a construção, implantação e implementação de tal política. Discuto outros significados atribuídos à paternidade no âmbito da PNAISH, indicando um deslizamento analítico que permite uma provável ampliação de uma “Paternidade Responsável” para uma “Paternidade Participativa”. Desse modo, a investigação realizada permite-me argumentar que o direcionamento dado aos homens-pais, por meio dos materiais da política, busca consolidar a representação de um pai participativo, que necessita integrar-se às rotinas cotidianas dos/as filhos/as, compartilhando responsabilidades e assumindo atribuições, a fim de posicioná-lo como um sujeito integrante do processo de cuidado. Assim, os sentidos atribuídos à participação dos homens-pais vão sendo reconstruídos em redes de significados que reafirmam, atualizam e deslocam representações de paternidade vigentes no Brasil contemporâneo. / In this essay I problematize the paternity representations transmitted, updated and (re) produced in the National Policy of Integral Attention to Men's Health - PNAISH, current public policy of the Brazilian federal government that aims to encourage improvements in the health of the male population, facilitating access to comprehensive health care services. The study is part of the referential theoretical methodological of Gender Studies and Cultural Studies, situated in the field of Collective Health. I use the documentary research to gather all materials examined from the perspective of cultural analysis in order to describe and analyze the lessons of politics men parents, operating with the concepts of gender, representation, culture, power, public policy, health and collective health. I analyze how men come to be called to participate and integrate care routines with the children and how health professionals, within the "SUS", seek to promote and / or include the participation of men parents in health space. The PNAISH acts as a cultural artifact that focuses on parenting representations, involving the nomination, classification and socialization of men parents among the disputes waged since its construction, deployment and implementation. I discuss other meanings attributed to paternity within the PNAISH, indicating an analytical slip that allows a likely extension of a "Responsible Parenthood" to a "Participatory Parenthood." Thus, the investigation allows me to argue that the direction given to men parents, through political materials, seeks to strengthen the representation of a participatory father, who needs to integrate the daily routines of the children, sharing responsibilities and assuming powers in order to position him as an integrant member of the care process. Therefore, the meanings attributed to the participation of men parents are being reconstructed in networks of meanings that reaffirm, update and move paternity representations prevailing in contemporary Brazil.

O interesse em estudar sobre promoção de saúde em pessoas em situação de pobreza foi a mola propulsora para a realização deste estudo, além disso, o interesse crescente da academia em realizar pesquisas com o universo masculino também estimulou o despertar pelo tema. Para a pesquisa, foi abordado, em especial, às condições de saúde dos homens, particularmente os vivendo na pobreza com o intuito de entender como estes homens enfrentam a condição de estar doente e utilizando a cadeira de rodas como meio de locomoção. Foi utilizado como modelo a Classificação Internacional de Funcionalidade Incapacidades e Saúde (CIF), na segunda parte, que trata do ambiente, visto ser a CIF o modelo de classificação em que se pode vislumbrar alguma mudança no campo social. A metodologia correspondeu a um estudo qualitativo com abordagem do tipo etnográfica utilizando a metodologia da história de vida. As técnicas utilizadas foram: observação direta e entrevistas semiestruturada. Os resultados obtidos demonstraram que a presença do capital social e cultural impacta na condição de saúde da população de baixa renda assim como a presença de barreiras de acessibilidade na mobilidade de pessoas que utilizam a cadeira de rodas como meio de locomoção. Neste estudo, mesmo com uma amostra reduzida, pode-se perceber o quanto o Estado está ausente das localidades periféricas, que o programa de proteção e recuperação da saúde não acontece nas comunidades carentes e o quanto são exacerbados os laços e vínculos entre os seus moradores na perspectiva de seguridade e coesão social, para assim poderem transpor todas as adversidades comuns a moradias em uma comunidade periférica. / The importance of approaching the social policies of the population with a little access to basics property makes the possibility of a less unfair society, mainly on the health item, come true. The interest on the study about the promotion of health to poor people was the driving force to the execution of this study, furthermore, the academy‟s growing interest about make researches with the male universe stimulated also the attention to the issue. To the research, was addressed, especially, the conditions of men‟s health, particularly those who live on a poor situation with the intention to understand how they can face the condition of being sick and using a wheelchair as the method of locomotion. As model was used the International Classification of Functioning Disability and Health (ICF), in the second part, that talks about the ambient, as the ICF is the classification model in which one is possible to glimpse a change in the social field. The methodology corresponded to a qualitative study with the ethnographic approach using the methodology of the life story. To get the result for the research were used the following techniques: direct observation and semi-structured interview. The results archived show that the social capital and cultural has an impact on the health condition of the low-income population as well as the presence of accessibility barriers on the mobility of the people who use wheelchair as method of locomotion. In this study, even with a reduced sample, can be perceived how the State is not present on the peripheral locations, which the health protection and recovery program does not happen on the poor communities and how exacerbated the ties and linkages between the neighbors are on the perspective of singularity and social cohesion, and then they can transpose all of the common adversities to houses in a poor community.

Barriers & Motivators to Physical Activity in Older Mexican American Men

abstract: The purpose of this phenomenological study was to explore the cultural, social, environmental, and gender factors that may influence physical activity (PA) in older Mexican American (MA) men living in Tucson, Arizona. The Mexican origin population is the fastest growing Hispanic subgroup in our nation, increasing from 20.6 million in the year 2000 to 31.8 million in 2010. Arizona has the sixth largest Hispanic population in the United States and the Mexican origin population accounts for 91% of Arizona's Hispanics. Despite the fast growing Mexican population, there are a limited number of studies that examine MAs and PA. There are even fewer interventions created to foster PA among older (≥65 years old) MA men. Fourteen individual interviews were conducted with older MA men living in Tucson, Arizona. Data was collected, organized, and analyzed according to the methodologies of Clark Moustakas and the Social Ecology Model for Health Promotion framework. Six themes emerged which reflected the older MA male's perception of health, masculinity, and physical activity: a) Retirement promotes self-care behaviors, b) Women, health care providers, and the Internet are important in promoting health, c) Aging changes physical activity, d) I take care of myself, e) Physical activity is a personal choice and lifestyle, and f) I learn and make adjustments as needed. Themes were used to create textural and structural descriptions of their experiences. Descriptions were formed into the essence of the phenomenon. The results of this study increase our understanding of health, masculinity, and physical activity in older MA men. This research will inform the development of an evidence-based PA intervention to promote cardiovascular (CV) health in older MA men that may be implemented in a variety of community-based settings. / Dissertation/Thesis / Doctoral Dissertation Nursing 2015

Assim, os sentidos atribuídos à participação dos homens-pais vão sendo reconstruídos em redes de significados que reafirmam, atualizam e deslocam representações de paternidade vigentes no Brasil contemporâneo. / In this essay I problematize the paternity representations transmitted, updated and (re) produced in the National Policy of Integral Attention to Men's Health - PNAISH, current public policy of the Brazilian federal government that aims to encourage improvements in the health of the male population, facilitating access to comprehensive health care services. The study is part of the referential theoretical methodological of Gender Studies and Cultural Studies, situated in the field of Collective Health. I use the documentary research to gather all materials examined from the perspective of cultural analysis in order to describe and analyze the lessons of politics men parents, operating with the concepts of gender, representation, culture, power, public policy, health and collective health. I analyze how men come to be called to participate and integrate care routines with the children and how health professionals, within the "SUS", seek to promote and / or include the participation of men parents in health space. The PNAISH acts as a cultural artifact that focuses on parenting representations, involving the nomination, classification and socialization of men parents among the disputes waged since its construction, deployment and implementation. I discuss other meanings attributed to paternity within the PNAISH, indicating an analytical slip that allows a likely extension of a "Responsible Parenthood" to a "Participatory Parenthood." Thus, the investigation allows me to argue that the direction given to men parents, through political materials, seeks to strengthen the representation of a participatory father, who needs to integrate the daily routines of the children, sharing responsibilities and assuming powers in order to position him as an integrant member of the care process. Therefore, the meanings attributed to the participation of men parents are being reconstructed in networks of meanings that reaffirm, update and move paternity representations prevailing in contemporary Brazil.

Nesta dissertação, problematizo as representações de paternidade veiculadas, atualizadas e (re)produzidas na Política Nacional de Atenção Integral à Saúde do Homem (PNAISH), política pública atual do governo federal brasileiro que tem como objetivo promover a melhoria da saúde de homens, facilitando o acesso aos serviços de assistência integral à saúde. O estudo inscreve-se no referencial teórico-metodológico dos Estudos de Gênero e dos Estudos Culturais, situando-se no campo da Saúde Coletiva. Utilizo a pesquisa documental para reunir o conjunto de materiais examinados na perspectiva da análise cultural, a fim de descrever e analisar os ensinamentos da política aos homens-pais, operando com os conceitos de gênero, representação, cultura, poder, política pública, saúde e saúde coletiva. Analiso como os homens passam a ser convocados a participar e integrar as rotinas de cuidado com os/as filhos/as e como os profissionais de saúde, no âmbito do SUS, buscando-se promover e/ou incluir a participação dos homens-pais nos espaços de saúde. A PNAISH atua como um artefato cultural que incide sobre representações de paternidade, envolvendo-se com a nomeação, a classificação e a socialização dos homens-pais em meio às disputas travadas desde a construção, implantação e implementação de tal política. Discuto outros significados atribuídos à paternidade no âmbito da PNAISH, indicando um deslizamento analítico que permite uma provável ampliação de uma “Paternidade Responsável” para uma “Paternidade Participativa”. Desse modo, a investigação realizada permite-me argumentar que o direcionamento dado aos homens-pais, por meio dos materiais da política, busca consolidar a representação de um pai participativo, que necessita integrar-se às rotinas cotidianas dos/as filhos/as, compartilhando responsabilidades e assumindo atribuições, a fim de posicioná-lo como um sujeito integrante do processo de cuidado. A Política Nacional de Atenção Integral à Saúde do Homem existe desde 2008, o que torna necessárias as discussões e reflexões sobre sua implementação. Entre as neoplasias, o câncer de próstata é o tipo mais predominante na população masculina, merecendo destaque nas discussões. Por esse motivo foi traçado o Itinerário Terapêutico de pacientes encaminhados ao Centro Atenção à Saúde do Homem na Policlínica Piquet Carneiro, após a realização da biópsia de próstata, com resultado positivo. Os sujeitos da pesquisa foram os homens para aproximar a realidade de saúde dessa população no país, tanto no que diz respeito a compreensão do processo saúde – doença, desigualdades de gêneros e as formas de organização do Sistema de Saúde. Trazendo como objetivos: Avaliar o perfil da clientela que busca atenção ao serviço de Assistência à Saúde do Homem; Discutir as facilidades e dificuldades do cesso; Demonstrar as consequências da Desigualdade de Gênero; Contribuir para o aperfeiçoamento da gestão e do cuidado à esta população. Na 1ª etapa foram analisados 142 prontuários, traçado as características dos pacientes no ano de 2014 e após foram feitas entrevistas com sete pacientes e traçado o itinerário terapêutico. Do total de 142 prontuários, cinco tiveram que ser excluídos por falta de dados para avaliação, desses 137, 61 pacientes o resultado foi positivo para Câncer correspondendo a um total de 44,5% e 76 pacientes tiveram o resultado negativo, correspondendo a 55,5% da amostra. O resultado da pesquisa mostrou que os pacientes ficam em média um ano e seis meses para ter acesso ao diagnóstico de câncer de próstata e um mês, após receber o diagnóstico, para ter acesso ao tratamento. Para auxiliar na analise de dados usamos analise de conteúdo e foram criadas três categorias: Desigualdade de gênero no Acesso a Saúde, Dificuldade de acesso ao Diagnóstico de Câncer de próstata e Facilidade de Acesso ao Tratamento. Por fim, destacou-se que as dificuldades de acesso encontradas pela população masculina não podem ser atribuídas de modo unilateral as suas questões sócios culturais uma vez que as políticas públicas também não atentaram para as demandas e necessidades dessa população / This study refers to the bioethical reflection regarding the access problem and inequality of genders specifically in the masculine population. Taking into consideration that there is the National Policy of Whole Assistence to Men’s Health since 2008, it is recente, which makes necessary the discussion and reflections about the implementation of this Policy. Among the neoplasias, prostate câncer is the most predominant in the male population, deserving to highlight its discussion. For this reason it was tracked the Therapeutical Itinerary of patients forwarded to the Center Assistance of Men’s Health in Policlínica Piquet Carneiro, after the biopsy of prostate with positive result. The subjects of the research were men to approach the reality of health in this population in the country, both in terms of comprehension in the procedure health–disease, inequalities of genders and ways of organization of the Health System. Presenting as objective: Evaluate the profile of the clients that search for the attention of the service of Assistance to Men’s Health; Debate the advantages and difficulties of access; Demonstrate the consequences of gender inequality; Contribute to the improvement of the management and care to this population. In the 1st stage, 142 medical records were analyzed, tracing the profile of patients in 2014 and after interviews were made with seven patients and stroke the therapeutic itinerary. Of the total of 142 records, 5 had to be excluded for lack of data for evaluation, these 137, 61 patients the result was positive for cancer corresponding to a total of 44.5% and 76 patients had negative result, corresponding to 44,5% of the sample. The survey results showed that patients stay on average one year and six months to gain access to the diagnosis, and 1 month after receiving the diagnosis , to have access to treatment. Based on the analysis we were created three (3) major categories : Gender Disparities in Access to Health Lack of access to cancer diagnosis prostate and Treatment Access Facility. Finally, it was highlighted that the difficulties of access encountered by the male population should not be attributed unilaterally their socio cultural issues as the public policies also understood not the demands and needs of this population / Este es el estudio de la reflexión bioética al problema del acceso y la desigualdad de género específicamente la población masculina. Toma encuenta que La Política Nacional de Atención Integral a la Salud de los Hombres ha existido, no Brazil, desde 2008 es nuevo, lo que hace que las discusiones y reflexiones necesarias sobre la aplicación de esta política. Entre los tipos de cáncer, el cáncer de próstata es el tipo más frecuente en la población masculina, mereciendo más destacado de los debates. Por esta razón, fuelaruta Terapia Ruta de los pacientes remitidos al centro de Atención a La Salud de los Hombres Policlínico Piquet Carneiro, después de realizar la biopsia de próstata, con resultados positivos. Los sujetos de La investigación fueron los hombres de acercarse a la realidad de la salud de esta poblaciónen el país, tanto en lo que se refiere a la comprensión de la salud - enfermedad, la desigualdad de géneros y formas de la organización del sistema de salud que traen los siguientes objetivos: Evaluar primerel perfil de los clientes que busca nun servicio de Asistencia para La atención a los hombres de la Salud; Comenta las ventajas y dificultades de acceso; Demostrar las consecuencias de la desigualdad de género; Contribuir a la mejora de la gestión y La atención a esta población. En la primera fase se analizaron 142 historias clínicas, trazando el perfil de los pacientes en 2014 y fueron hechos después de entrevistas consiete pacientes y acariciar elitinerario terapéutico. Del total de 142 registros, 5 tuvieron que ser excluidos por falta de datos para La evaluación, estos 137, 61 pacientes el resultado fue positivo para el cáncer que corresponde a un total de 44,5% y 76 pacientes tuvieron resultado negativo, lo que corresponde a 44,5% de La muestra. Los resultados del estudio mostraron que los pacientes permanecen enpromediounaño y seis meses para tener acceso al diagnóstico, y 1 mes después de recibir el diagnóstico , para tener acceso al tratamiento. Con base en el análisis que se crearon tres ( 3 ) categorías principales: las disparidades de género en el acceso a la salud La falta de acceso a la próstata diagnóstico de cáncer y de las instalaciones de Acceso al Tratamiento . Por último, se destacó que las dificultades de acceso encontrados por la población masculina no deben atribuirse de manera unilateral sus problemas socioculturales como las políticas públicas también entendieron no las demandas y necesidades de esta población

A experiência do câncer peniano e seus tratamentos na perspectiva dos adoecidos / The experience of penile cancer and treatment from the perspective of patients

O câncer de pênis é uma doença de baixa incidência e sua manifestação está associada a fatores intrínsecos e extrínsecos, e sua principal forma de tratamento é a penectomia. Devido à doença ocorrer no órgão genital masculino busquei conhecer como os homens adoecidos lidam com suas masculinidades na situação do adoecimento. Dessa forma, propus a tese de que a cultura influencia na forma como os homens sobreviventes do câncer de pênis lidam com suas masculinidades. Elaborei o objetivo de interpretar os significados atribuídos à experiência com o câncer de pênis e seus tratamentos pelos homens adoecidos. O referencial teórico adotado foi o da antropologia médica e das masculinidades, pois ambas consideram a cultura como um elemento importante no mundo social. Para conhecer a cultura do outro é necessário vivenciá-la de perto, por este motivo optei por desenvolver a etnografia narrativa. Esse método propõe ao pesquisador o uso de técnicas como a observação participante, múltiplas entrevistas, uso do diário de campo, análise temática e a apresentação dos resultados na estrutura de narrativas que contenham a estória das vivências dos participantes. Ressalto que a narrativa tem como principal objetivo mostrar as transformações vividas pelos depoentes, considerando os aspectos comuns e divergentes dos entrevistados. Foram selecionados 18 homens com o diagnóstico de câncer de pênis no ambulatório de uro- oncologia de um hospital do interior paulista. As características sociais e clínicas dos entrevistados são semelhantes aos dados nacionais e internacionais em relação à baixa incidência da doença, faixa etária e os tratamentos empregados. Diante do processo de análise foi possível construir oito sínteses temáticas: O câncer de pênis e seu itinerário; O corpo após a doença e seus tratamentos; O câncer e suas repercussões sociais: o trabalho, o lazer e as relações sociais; Por trás do visível: a confidencialidade da doença; A sexualidade pós- tratamento; Um homem inteiro ou meio homem: a ressignificação das masculinidades; Recursos de apoio social; O hoje e as expectativas para o futuro. Por meio das sínteses narrativas é possível conhecer os sentidos que os homens empregaram ao adoecimento e como a doença afetou sua masculinidade. Estes achados permitiram alcançar o significado de ser homem sobrevivente do câncer de pênis e seus tratamentos, que pode ser compreendido pela expressão \"ser meio homem\". Desde o início do diagnóstico até a vida no pós- tratamento, os homens negociam suas masculinidades com o mundo social, e percebem que algumas características adquiridas durante o adoecimento os tornam diferente dos demais homens, tais como se assumir doente, a extirpação total ou parcial do pênis, redução da força física, impossibilidade de trabalhar e alterações na prática sexual. Comprovo, então, que a cultura tem influência na forma que os homens lidam com suas masculinidades, uma vez que os adoecidos constantemente revisitam seus padrões culturais de masculinidade para obedecê- lo ou ressignificá-lo. Os enfermeiros podem utilizar os achados deste estudo para refletir sua prática de cuidado aos homens com câncer e para desenvolverem outras pesquisas que o complementem / Penile cancer is a disease of low incidence, and its manifestation is associated with intrinsic and extrinsic factors, and its main form of treatment is the penectomy. Due to the fact that the disease occurs in the male genital organ, I sought to know how the ill men deal with their masculinities in the disease situation. Thus, I proposed the thesis that culture influences in the way as the penile cancer survivors deal with their masculinities. The aim of this study is to interpret the meanings attributed to the experience with penile cancer and its treatments by the ill men. The theoretical framework adopted was medical anthropology and masculinities, since both considered culture as an important element in the social world. In order to understand the culture of the other people is necessary to experience it closely, for this reason I chose to develop the narrative ethnography. This method proposes to the researcher the use of techniques such as participant observation, mult iple interviews, the use of field diary, thematic analysis and the presentation of results in the structure of narratives that contain the story of participants\' experiences. I emphasize that the narrative has as main objective to show the transformations lived by the deponents, considering the common and divergent aspects of the interviewees. We selected 18 men with the diagnosis of penile cancer in the uro-oncology clinic of a hospital in the interior of São Paulo. The social and clinical characteristics of the interviewees are similar to the national and international data regarding the low incidence of the disease, age group and the treatments used. From the process of analysis it was possible to construct eight thematic syntheses: Penile cancer and its itinerary; The body after disease and its treatments; Cancer and its social repercussions: work, leisure and social relations; Behind the visible: the confidentiality of the disease; The sexuality post-treatment; The whole man or half man: resignification of masculinities; Social support resources; The today and expectations for the future. Through narrative syntheses it was possible to comprehend the meaning attributed by the patients, what they assigned to illness and how that disease affected their masculinity. These findings allowed us to achieve the meaning of being a man penile cancer survivor and its treatments, which can be understood by the expression \"being half man\". From the onset of diagnosis up to life post-treatment, men negotiate their masculinities with the social world, and realize that some characteristics acquired during illness make them different from other men, such as, to assume themselves as sickness, the fact that they have total or partial penile extirpation, reduction of physical strength, impossibility of working and changes in sexual practice. I then show that culture influences the way men deal with their masculinities, since the ill men are constantly revisiting their cultural patterns of masculinity to obey or re-signify it. Nurses can use the findings of this study to reflect their practice of caring for men with cancer and to develop further researches that complements this one
