Det vårdande samtalet utgör en betydande del av omvårdnaden inom psykiatrisk vård och är således psykiatrisjuksköterskans främsta redskap att använda sig av. Ett vårdande samtal beskrivs som ett samtal som lindrar lidande och där hälsoprocesser kan uppstå. Efter litteraturgenomgång i ämnet visade det sig att endast ett fåtal studier i ämnet är gjorda vilket innebär att det föreligger en betydande kunskapsbrist i ämnet. Författarnas förhoppning är att följande studie bidrar med värdefull kvalitativ kunskap ur ett patientperspektiv med avsikt att förbättra omvårdnaden inom den psykiatriska kontexten, samt medverkar till att det vårdande samtalet framledes etableras som en evidensbaserad vårdaktivitet. Syftet med studien är att beskriva hur patienter med förstämningssjukdom erfar det vårdande samtalet med en psykiatrisjuksköterska. En kvalitativ livsvärldsansats användes som metod och nio livsvärldsintervjuer genomfördes vilka ligger till grund för analysarbetet. Innebördsanalys valdes som analysmetod för att söka djupare innebörd av fenomenet det vårdande samtalet. Resultat presenteras utifrån fem innebördsteman: där personligt engagemang bejakar livsvärlden, där utrymmet skapar en atmosfär av lugn och ro, där livsvärlden berörs och berör, där det osynliga synliggörs samt att ändra sitt tänkesätt. Diskussionen förs i två perspektiv där patienternas upplevelser utgör det ena och psykiatrisjuksköterskans förhållningssätt det andra. Slutsatsen av studien visar att det vårdande samtalet utgör en viktig del i den vårdande relationen, samt att den genererar såväl gemenskap, delaktighet och ett lärande. Studien visar också att psykiatrisjuksköterskans etiska förhållningssätt är framträdande och utgör en stor betydelse för patienter med förstämningssjukdom på så sätt att ett caritativt förhållningssätt påvisas i det vårdande samtalet. Förslag på kliniska implikationer lämnas och flera intressanta forskningsområden presenteras. / Program: Specialistsjuksköterskeutbildning med inriktning mot psykiatrisk vård

Este trabalho tem como objetivo problematizar as práticas do assistente social a partir da minha experiência profissional nas áreas da saúde mental e assistência social. Tais práticas circulam principalmente entre os direitos e a filantropia, que são analisados nesse trabalho como relações de poder e como mecanismos de controle da vida. No primeiro capítulo, problematizo minha experiência na área da saúde mental, principalmente no que tange ao trabalho com famílias. Tal experiência propiciou diversos questionamentos acerca do papel que vem sendo atribuído às famílias pelas políticas públicas. No segundo capítulo, coloco em análise minhas experiências em um Centro de Referência de Assistência Social (CRAS) localizado em Angra dos Reis. Nesse momento da dissertação, problematizo as práticas do assistente social, como a elaboração de relatórios, as visitas domiciliares, o fornecimento de cestas básicas, a atuação no Programa Bolsa Família. No terceiro capítulo discorro acerca dos direitos e da filantropia na política de assistência social e sobre a participação política nesse contexto. Neste capítulo trago referências conceituais e históricas, no sentido de problematizar as práticas do assistente social na política de assistência. / This work aims at problematizing some practices of the social assistant, from my professional experience in the areas of mental health and social assistance. Those practices circulate mainly between the rights and philanthropy, which are analyzers, since they indicate power relationships and mechanisms of control of life. In the first chapter, I problematize my experience in the area of mental health, mainly in what concerns to the work with families. Such experience has provided many questionings about the roll that has been attributed to families by public policies. In the second chapter I put in analyses my experiences in a Reference Center of Social Assistance (CRAS) localized in Angra dos Reis. In this moment of thie thesis, I problematize some of the social assistant practices, as the reports that it has to do, the home visits, the distribution of baskets of food, its action in the Family Grant. Chapter three discusses the rights and the philanthropy in the social assistance policy, as well as the political participation in this context. In the chapter I bring out conceptual and historical references to problematize the practices of the social assistant in the assistance policy.

The clinical care in the Brasilian Unified Health System (SUS) was the source of the production of fertile clinical narratives for this research. These fragments have allowed me to discuss cases that constitute both the clinical method as the research method. The clinical method guides the practices of this research to the construction of narratives about the clinical cases, besides allow a clinical reflection of this material and the phenomenon under analysis. This work is focused on the subjective position of clinical experience in the obscure that underlies the analytic encounter. Such research requires put the question about the preservation of margins, borders and boundaries on the territories, continuously marked and remarked. These lines show up in the field and in the work of mental health nowadays and they are very similar to the demarcations and categories in force in the past. Users of mental health system seen as marginal from point of view of the system of rights, are still massively kept within the walls of official confinement of the insanity, banished from society, but now outside of the walls. When we find In the SUS so striking separation of bodies, spaces of distinction and demarcation of places, still in force, it is evident the urgency to inquire about the function of this clinical position, its foundations and the basic assumptions which maintains frequently separate the suffering of pathos from the conditions to establish otherness / Este estudo registra o curso de vinte e cinco anos de trabalho na saúde pública da cidade de São Paulo. O atendimento clínico no Sistema Único de Saúde (SUS) foi se constituindo como um material para a produção de narrativas clínicas férteis para esta pesquisa. Estes fragmentos me permitiram discutir casos que constituem tanto o método clínico, como método de pesquisa. O método clínico orienta as práticas de pesquisa para a construção de narrativas sobre casos atendidos, permite a reflexão clínica deste material e do fenômeno em análise. Este trabalho está focado na posição subjetiva da experiência clínica no obscuro que subjaz ao encontro analítico. Essa investigação requer se indagar sobre a preservação das margens, bordas e fronteiras nos territórios sucessivamente marcados e remarcados. Hoje em dia, essas linhas aparecem no campo e no trabalho de saúde mental muito semelhantes ao das demarcações e categorias vigentes em outros períodos do passado. Os usuários de saúde mental, vistos como marginais do ponto de vista do sistema de direitos, ainda são maciçamente mantidos dentro das paredes de confinamento oficial da insanidade, banidos da sociedade, mas agora do lado de fora das paredes. Quando encontramos no SUS separação tão marcante dos corpos, espaços de distinção e demarcação de lugares, ainda em vigor, é evidente a urgência em saber mais sobre a função desta situação clínica, seus fundamentos e pressupostos básicos, que frequentemente mantêm apartados o sofrimento do pathos das condições que constituem a alteridade

O presente trabalho toma como reflexão a saúde mental com crianças e adolescentes, estabelecendo como ponto de partida experiências vividas pelo autor como psicólogo desta área. Usa a memória e o biográfico como lugar potente para a reflexão historiográfica e um campo de composições entre forças passadas e o presente. Seguindo os rastros da Reforma Psiquiátrica brasileira e seu cenário dos anos 70 e 80, a tese mostra em alguns fragmentos históricos, o nascimento de uma psiquiatrização e psicologização da infância, ao mesmo tempo em que trata infância e loucura como objetos históricos a partir da presença de Michel Foucault e Philippe Àries no cenário da historiografia. Realça o fato de apesar de crianças e jovens terem ficado longe da agenda da Reforma Psiquiátrica brasileira, isto não significou ausência de movimentações sociais nos anos 70/80, trazendo outras cenas de militância para este público. No decorrer do estudo, discute a saúde mental e sua interface com a justiça e a educação, bem como mostra o avanço das teses cerebrais sobre a criança e os adolescentes. Ao final, o autor discute e estabelece uma reflexão dos espaços de intervenção criados no campo da saúde mental infanto-juvenil, a partir das noções de território, dispositivo, rizoma, clínica e política. / This paper takes as a reflection on mental health with children and adolescents, establishing as a starting point experiences of the author as a psychologist in the area. Uses memory and biography as potent place for historiography reflection and as a field of compositions between past and present. Following the tracks of the Brazilian Psychiatric Reform and its setting of 70 and 80 years, the thesis shows some historical fragments, the birth of a psychiatrization and psychologizing of childhood, at the same time it describes childhood and madness as historical objects from the presence Michel Foucault and Philippe Aries on the stage of history. Highlights the fact that although children and adolescent have stayed away from the agenda of the Brazilian Psychiatric Reform, this did not mean the absence of social movements in the years 70/90, bringing other scenes of activism for this audience. During the study, discusses mental health and its interface with the justice and education, and shows the progress of cerebral thesis on children and adolescents. Finally, the author discusses and provides a reflection of the spots where created in the field of mental health for children and youth, from the notions of territory, device, rhizome, practice and politics.
