A dual therapy of off-pump temporary left ventricular extracorporeal device and amniotic stem cell for cardiogenic shock

Background: Temporary mechanical circulatory support device without sternotomy has been highly advocated for severe cardiogenic shock patient but little is known when coupled with amniotic stem cell therapy. Case presentation: This case reports the first dual therapy of temporary left ventricular extracorporeal device CentriMag with distal banding technique and human amniotic stem cell injection for treating a severe refractory cardiogenic shock of an 68-year-old female patient. A minimally-invasive off-pump LVAD was established by draining from the left ventricle and returning to the right axillary artery with distal arterial banding to prevent right upper extremity hyperperfusion. Amniotic stem cells were injected intramyocardially at the left ventricular apex, lateral wall, inferior wall, and right subclavian vein. Conclusion: The concomitant use of the temporary minimally-invasive off-pump CentriMag placement and stem cell therapy not only provided an alternative to cardiopulmonary bypass and full-median sternotomy procedures but may have also synergistically enhanced myocardial reperfusion and regeneration.

Myocardial and cerebral preservation during off-pump coronary artery surgery

Abstract Interest in off-pump coronary surgery and ischaemic preconditioning has been increasing. The aim of this study was to evaluate surrogate indicators of haemodynamic, myocardial, and cerebral outcome during off-pump surgery and preconditioning. Haemodynamics and myocardial preservation were monitored in a pilot study of twelve patients undergoing off-pump coronary surgery. Indicators of myocardial metabolism and tissue injury as well as cerebral damage were evaluated in a randomized study of thirty-three patients undergoing on-pump (11) or off-pump surgery with (11) or without (11) preceding myocardial ischaemic preconditioning for five minutes followed by reperfusion for five minutes. The pilot study showed minimal haemodynamic changes and myocardial derangements during off-pump surgery as evaluated intraoperatively based on transcardiac differences of ATP degradation products and lactate and postoperatively based on MB mass of creatine kinase and troponin T. In the following studies, myocardial ischaemic metabolism was evaluated intraoperatively by measuring transcardiac differences of ATP degradation products, lactate, and pH, which increased significantly from the baseline values in all study groups. However, the maximum values of lactate and pH were significantly higher in the cardiopulmonary bypass group (p = 0.02 and p = 0.007, respectively). There were no statistical differences between the preconditioning and non-preconditioning groups. Myocardial tissue injury was evaluated by postoperative leakage of MB mass of creatine kinase and troponin I. Their peak values were significantly higher (p < 0.001 and p = 0.008) after cardiopulmonary bypass (15.1 μg/l and 13.8 μg/l) than after off-pump surgery without preconditioning (6.3 μg/l and 5.2 μg/l). The respective values were 14.8 μg/l and 7.4 μg/l after preconditioning, and there were no statistically significant differences between the off-pump groups with and without preconditioning. Cerebral damage was evaluated based on the intra- and postoperative serum concentrations of neuron-specific enolase, which were corrected with respect to haemolysis. The corrected values were significantly higher after on-pump than off-pump surgery (p = 0.003 and p = 0.005). In conclusion, multi-vessel off-pump coronary artery surgery is a haemodynamically feasible procedure offering better myocardial preservation compared to on-pump surgery. Ischaemic preconditioning of the myocardium does not seem to improve myocardial preservation in off-pump surgery. The slightly lower levels of neuron-specific enolase also suggest less cerebral damage.

Predictors of immediate outcome after coronary artery bypass surgery

Abstract The identification of risk factors for major adverse events after coronary artery bypass surgery is of main importance as it allows outcome prediction, facilitates preoperative patient selection and improves the quality of care. In the present clinical studies we have evaluated the impact of preoperative angiographic severity of a coronary artery disease and preoperative C-reactive protein (CRP) on the immediate outcome after coronary artery bypass surgery. We have reviewed the results of off-pump (OPCAB) versus conventional on-pump coronary artery bypass surgery (CCAB) in high risk patients. We have evaluated the impact of postoperative pulmonary artery blood temperature on the immediate outcome as well. In addition, we have investigated the incidence, timing and outcome of an atrial fibrillation (AF) related stroke after surgery. The multivariate analysis showed that among 2233 patients, the overall coronary angiographic score was predictive of postoperative death (p = 0.03; OR 1.027, 95% CI: 1.003–1.052) and of a low cardiac output syndrome (p = 0.04; OR 1.172, 95% CI: 1.010–1.218). The poor status of the proximal segment of the left circumflex coronary artery, the diagonal branches and the left obtuse marginal artery were most closely associated with adverse postoperative outcome. Patients (114/764) with a preoperative serum concentration of CRP ≥ 1.0 mg/dL had a higher risk of overall postoperative death (5.3% vs. 1.1%, p = 0.001), cardiac death (4.4% vs. 0.8%, p = 0.002), a low cardiac output syndrome (8.8% vs. 3.7%, p = 0.01). Among 179 high risk patients with an additive EuroSCORE6, the 30-day postoperative death and stroke rates were 7.5% and 6.0% in the OPCAB group, and 5.4% (p = 0.75) and 8.0% (p = 0.77) in the CCAB group, respectively. No significant differences were observed in other major outcome end-points between these non-randomised groups either. High pulmonary artery blood temperature on admission to the ICU among 1639 patients was significantly associated with an increased risk of overall postoperative death (p = 0.002), cardiac death (p = 0.03), and a low cardiac output syndrome (p < 0.0001), and was significantly correlated with prolonged length of the ICU stay (r = 0.095; p < 0.0001), and postoperative bleeding (ρ = –0.091; p = 0.001). Among 2,630 patients who underwent coronary artery bypass grafting (CABG), 52 (2.0%) experienced a postoperative stroke. Twelve out of these 52 patients (23.1%) died postoperatively. The ischemic cerebral event occurred after a mean of 3.7 days (0–33). In 19 patients (36.5%), atrial fibrillation preceded the occurrence of neurological complication. The angiographic severity of the coronary artery disease and the preoperative serum concentration of CRP predict postoperative outcome after a CABG operation. OPCAB can be performed safely in high-risk patients with results as satisfactory as those achieved with CCAB. CABG patients with a high pulmonary artery blood temperature on admission to the ICU seem to have a higher risk of postoperative adverse events. Atrial fibrillation occurring after coronary artery bypass grafting is a major determinant of a postoperative stroke.

Avaliação hemodinâmica durante a revascularização do miocárdio sem utilização de circulação extracorpórea / Hemodynamic evaluation during off-pump coronary artery bypass surgery

INTRODUÇÃO: A cirurgia de revascularização miocárdica sem utilização de circulação extracorpórea (CEC) tem sido cada vez mais utilizada, especialmente após a introdução de dispositivos estabilizadores da parede cardíaca. Entretanto, a técnica pode causar alterações hemodinâmicas durante a realização das anastomoses coronárias. OBJETIVOS: Analisar as alterações hemodinâmicas decorrentes das mudanças de posição do coração para abordar as artérias coronárias sem CEC e comparar os monitores de débito cardíaco semi-contínuo e de ecodoppler transesofágico quanto à precisão das medidas hemodinâmicas. MATERIAL E MÉTODOS: Foram selecionados aleatoriamente 20 pacientes adultos com idade inferior a 80 anos, candidatos a cirurgia eletiva de revascularização miocárdica sem utilização de circulação extracorpórea. A avaliação hemodinâmica incluiu a utilização de ecodopppler com transdutor esofágico e de cateter de artéria pulmonar com filamento térmico. A coleta de dados foi realizada: 1 - após a indução da anestesia, antes do início da revascularização propriamente dita, 2 - durante a realização das anastomoses distais, logo após o posicionamento e estabilização do coração e 3 - após cinco minutos do início da anastomose. Os dados hemodinâmicos foram analisados por análise de variância de duplo fator com repetição, complementada por teste de Newman-Keuls. O nível de significância considerado foi de 5%. Os valores de débito cardíaco foram comparados segundo método proposto por Bland e Altman, analisando a correlação intraclasses, diferenças médias e intervalos de confiança de 95%. RESULTADOS: Alterações hemodinâmicas significativas foram detectadas para o aumento de pressão de oclusão de artéria pulmonar (de 17,7 ± 6,1 para 19,2 ± 6,5 mmHg - p<0,001 e para 19,4 ± 5,8 mmHg - p<0,001) e pressão venosa central (de 13,9 ± 5,4 para 14,9 ± 5,9 mmHg - p=0,007 e para 15,1 ± 6,0 mmHg - p=0,006), além de diminuição do débito cardíaco obtido por termodiluição intermitente (de 4,70 ± 1,43 para 4,23 ± 1,22 L/min - p<0,001 e para 4,26 ± 1,27 L/min - p<0,001). Houve interação grupo-tempo estatisticamente significativa no débito cardíaco por Doppler esofágico, que apresentou redução no grupo lateral de 4,08 ± 1,99 para 2,84 ± 1,81 L/min (p=0,02) e para 2,86 ± 1,73 L/min (p=0,02), e no fluxo sanguíneo aórtico, que diminuiu de 2,85 ± 1,39 para 1,99 ± 1,26 L/min (p=0,02) e para 2,00 ± 1,21 L/min (p=0,02). As medidas de débito cardíaco intermitente, semicontínuo e por Doppler esofágico apresentaram diferenças médias e intervalos de confiança de 95% acima de limites aceitáveis clinicamente. CONCLUSÕES: Houve deterioração hemodinâmica significativa durante a revascularização miocárdica sem CEC. Pelo Doppler esofágico, o débito cardíaco apresentou redução detectada apenas na parede lateral. As diferenças nos valores de débito cardíaco foram muito amplas para considerar os métodos concordantes, em quaisquer das condições hemodinâmicas estudadas. / INTRODUCTION: Coronary artery bypass graft (CABG) surgeries have been performed increasingly without cardiopulmonary bypass (off-pump CABG), specially with introduction of cardiac wall stabilizing devices. However, hemodynamic changes can occur during coronary anastomosis. OBJECTIVES: To study hemodynamic alterations caused when cardiac position is changed to operate coronary arteries and to compare continuous cardiac output and esophageal Doppler monitor regardig accuracy of hemodynamic measurements. MATERIALS AND METHODS: Twenty adult patients under age of 80 undergoing elective off-pump CABG were enrolled. Hemodynamic evaluation was performed with esophageal echodoppler and continuous thermodilution pulmonary artery catheter. Data were collected 1 - after induction of anesthesia, before revascularization, 2 - during distal anastomosis, right after heart positioning and stabilization, and 3 - five minutes following the beginning of anastomosis. Repeated measures two-way ANOVA with post hoc Newman-Keuls tests were used to analyse hemodynamic data and level of significance was set at 0.05. Cardiac output values were compared using the method proposed by Bland and Altman, and included analysis of correlation, mean differences and 95% confidence intervals. RESULTS: Significant hemodynamic alterations were detected during revascularization of coronary arteries as elevation of pulmonary artery occlusion pressure (from 17.7 ± 6.1 to 19.2 ± 6.5 mmHg - P <0.001, and to 19.4 ± 5.8 mmHg - P <0.001) and of central venous pressures (from 13.9 ± 5.4 to 14.9 ± 5.9 mmHg - P =0.007, and to 15.1 ± 6.0 mmHg - P =0.006), and as reduction of intermittent cardiac output (from 4.70 ± 1.43 to 4.23 ± 1.22 l/min - P <0.001, and to 4.26 ± 1.27 l/min - P <0.001). Statistically significant group-time interaction was observed in esophageal Doppler cardiac output, that decreased in the lateral wall from 4.08 ± 1.99 to 2.84 ± 1.81 l/min (P =0.02) and to 2.86 ± 1.73 l/min (P =0.02), and in aortic blood flow, that decreased from 2.85 ± 1.39 to 1.99 ± 1.26 l/min (P =0,02) and to 2.00 ± 1.21 l/min (P =0.02). Intermittent, STAT-mode or esophageal Doppler cardiac output mean differences and 95% confidence intervals were beyond clinically acceptable limits. CONCLUSIONS: There was significant hemodynamic deterioration during off-pump CABG. On the esophageal Doppler monitor, cardiac output decrease was detected only in the lateral wall. Differences in cardiac output measurements were too wide to say methods agreed, in all hemodynamic conditions studied.

Oclusion temporaire des artères coronaires par un gel thermo-réversible : LeGoo®. De l'expérimentation à l'utilisation humaine / Temporary occlusions of coronaries arteries by thermoreversible gel : leGoo.To experimental to human case

ObjectifsL’objectif de ce travail a été d’évaluer la faisabilité d’utiliser le gel LeGoo pour occlure temporairement les artères coronaires afin de réaliser des pontages coronariens à cœur battant initialement chez le porc puis chez l’humain. Les objectifs secondaires ont été d’étudier l’impact de l’injection du gel sur l’endothélium et le myocarde.MéthodesLa première étude a eu pour but d’évaluer la possibilité de réaliser chez le cochon des pontages coronariens sur l’artère interventriculaire antérieure ou l’artère coronaire droite avec une artère thoracique interne, et d’évaluer la fonction endothéliale des artères coronariennes au niveau de la zone d’injection du gel.La deuxième étude a analysé chez le porc la fonction endothéliale des artères thoraciques internes au niveau de la zone d’occlusion par du gel LeGoo chez le porc.Après l’obtention du marquage CE, la troisième étude a permis de prouver la faisabilité de l’utilisation de LeGoo dans la réalisation de pontages coronariens à cœur battant chez l’humain et d’évaluer l’impact myocardique de cette utilisation.La dernière étude était une étude randomisée, multicentrique, internationale, comparant l’utilisation du gel pour la réalisation des anastomoses, versus les occlusions conventionnelles par élastiques péri-coronaires. RésultatsPour la première étude, 14 cochons ont été pontés à cœur battant et sacrifiés à la 3ème h (n=8) ou au 3ème jour (n=6). L’étude de la réactivité endothéliale n’a pas mis en évidence de différence entre les artères coronaires occluses par le gel et l’artère coronaire circonflexe qui servait de témoin. L’endothélium ne présentait pas de lésions histologiques en microscopie. Les mêmes résultats ont été obtenus dans la deuxième étude, sur les 9 artères thoraciques internes qui avaient été occluses durant 15 minutes avec le gel LeGoo.Les premières utilisations humaines ont permis de réaliser 99 anastomoses chez 50 patients. Le taux de satisfaction sur la qualité de l’hémostase obtenue a été de 91%. Un patient a présenté une arythmie, qui a nécessité de dissoudre le gel et de mettre en place un shunt intra-coronaire. Un autre patient a présenté un infarctus du myocarde à J1 lié à une sténose sur l’artère thoracique interne en amont de l’anastomose, nécessitant une réfection du pontage. Les autres complications étaient identiques aux séries de la littérature.Au cours de l’étude randomisée, les temps d’anastomoses étaient plus courts dans le groupe LeGoo que dans le groupe élastiques (12,8 ± 4,7 min vs 15,4 ± 6 min, p<0,001). Les écarts étaient de 2 minutes environ pour les territoires antérieurs et de 4 minutes pour les territoires latéraux et postérieurs. Le taux de satisfaction pour l’hémostase était de 88% avec le gel alors qu’il était de 60% avec les élastiques (p<0,0001). Les autres critères péri- opératoire étaient statistiquement identiques. Conclusions L’utilisation du gel LeGoo (Poloxamer 407) permet de réaliser des anastomoses en diminuant les temps d’occlusion et de manière sécuritaire sans altérer les fonctions de l’endothélium coronarien. / ObjectifsThe objective of this work is to assess the feasibility of using the LeGoo gel to occlude the coronary arteries temporarily, to facilitate the performance of coronary bypass (off-pump technique), initially in pigs then in human. A Secondary objective was to evaluate the impact of the injection on the endothelium and myocardium.MethodesThe first study evaluated in pigs the feasibility of using LeGoo, while performing coronary bypass on the left anterior descending or on the right coronary artery and to evaluate the endothelial function of coronary arteries occluded by the gel.The second study analyzed endothelial function of internal thoracic arteries, in the area occluded by the gel.After obtaining the CE mark, a third study demonstrated the feasibility of the utilisation of LeGoo for temporary coronary artery occlusion during the performance of beating heart coronary bypass surgery in human.The last study was a randomized, multicenter and international clinical study that compared the gel with conventional vessel loop during the performance of off-pump coronary artery bypass anastomoses.ResultsIn the first study 14 pigs received coronary bypass and were sacrificed after 3 hours (n=8) or 3 days (n=6). The study of endothelial reactivity did not reveal any difference between the coronary arteries occluded by the gel and the circumflex coronary artery, which served as a control. The endothelium showed no histological damage. Identical observations were made on the 9 internal thoracic arteries, used in the second study and occluded during 15 minutes by a LeGoo plug.In the first human use, 99 anatomoses were performed in 50 patients. The level of satisfaction on the quality of haemostasis was 91%. One patient presented arrhythmia which required the introduction of an intracoronary shunt after rapid dissolution of the gel. Another patient had a myocardial infarct on day 1 associated with stenosis on the internal thoracic artery upstream of the anastomosis, requiring a refection of this bypass (new off pump procedure). Other complications were identical to what is reported in the literature.In the randomised study, anastomoses times were shorter in LeGoo group than with vessel loops (12,8 ± 4,7 min vs 15,4 ± 6 min, p<0,001). The differences were 2 minutes for anterior bypass and 4 minutes for the lateral and posterior territories.The satisfaction rate of haemostasis was 88% with the gel against it was 60.7% with the vessel loop (p<0,0001). Other peri-operative measurements were statistically identical between the 2 groups.ConclusionsLeGoo (a poloxamer based formulation) can be used safely for the performance of coronary anastomoses. A Comparison of Patients Undergoing On- vs. Off-Pump Coronary Artery Bypass Surgery Managed with a Fast-Track Protocol

The purpose of this study was to compare patients who underwent on- vs. off-pump coronary artery bypass surgery managed with a fast-track protocol. Between September 2012 and December 2018, n = 3505 coronary artery bypass surgeries were managed with a fast-track protocol in our specialized post-anesthesia care unit. Propensity score matching was applied and resulted in two equal groups of n = 926. There was no significant difference in ventilation time (on-pump 75 (55–120) min vs. off-pump 80 (55–120) min, p = 0.973). We found no statistically significant difference in primary fast-track failure in on-pump (8.2% (76)) vs. off-pump (6% (56)) groups (p = 0.702). The secondary fast-track failure rate was comparable (on-pump 12.9% (110) vs. off-pump 12.3% (107), p = 0.702). There were no significant differences between groups in regard to the post-anesthesia care unit, the intermediate care unit, and the hospital length of stay. Postoperative outcome and complications were also comparable, except for a statistically significant difference in PACU postoperative blood loss in on-pump (234 mL) vs. off-pump (323 mL, p < 0.0001) and red blood cell transfusion (11%) and (5%, p < 0.001), respectively. Our results suggest that on- and off-pump coronary artery bypass surgery in fast-track settings are comparable in terms of ventilation time, fast-track failure rate, and postoperative complications rate.

Alterações hemostáticas e clínicas em cirurgias de revascularização miocárdica com e sem circulação extracorpórea: estudo prospectivo randomizado / Hemostatic changes and clinical sequelae after on-pump compared with off-pump coronary artery bypass surgery: a prospective randomized study

Introdução: a revascularização miocárdica (RM) sem circulação extracorpórea (CEC) tem sido associada a menores complicações quando comparadas à com CEC. Objetivos: determinar os efeitos da CEC em marcadores de hemostasia, fibrinólise, inflamação e correlacionar com eventos clínicos. Método: os pacientes foram incluídos de forma prospectiva e randomizada para cirurgia de RM com (n=41) ou sem CEC (n=51). As concentrações de proteína C reativa (PCR), fibrinogênio, dímero-D e inibidor do ativador do plasminogênio tipo 1 (PAI1) foram quantificadas antes e após (1 e 24 horas) a RM. As técnicas cirúrgicas e anestésicas foram padronizadas para ambos os grupos. Eventos clínicos foram avaliados durante a hospitalização inicial e após 1 ano de seguimento. Resultados: as concentrações de PAI1 e dímeros-d foram maiores quando comparados os valores pré-operatórios com os de 1 e 24 h, após a RM em ambos os grupos, porém as concentrações de PAI1 aumentadas estenderam-se por 24 h após a RM com CEC (p<0,01). A concentração de PCR teve um aumento de pequena magnitude imediatamente após a cirurgia em ambos os grupos e aumentou de modo similar 24h após a RM (p<0,01). A RM com CEC foi associada com maior perda sanguínea durante a cirurgia e mais sangramento pós-operatório (p<0,01). A incidência de todas as outras complicações foi similar nos dois grupos. Conclusão: a RM com CEC apresentou evidências bioquímicas de um estado pró-trombótico precoce após a cirurgia, porém, sem evidências no aumento no número de eventos trombóticos. O estado pró-trombótico pode ser consequência do circuito extracorpóreo, resposta compensatória ao sangramento, ou a ambos em pacientes submetidos à cirurgia com CEC. / Objective: To delineate the effects of extracorporeal bypass on biomarkers of hemostasis, fibrinolysis, and inflammation and clinical sequelae. Methods: Patients were assigned prospectively and randomly to either on-pump (n=41) or off-pump (n=51) coronary bypass surgery. The concentrations of C-reactive protein (CRP), fibrinogen, D-dimer, and plasminogen activator inhibitor type 1 (PAI-1) in blood were quantified before and after (1 hour and 24 hours) surgery. Similar surgical and anesthetic procedures were used for both groups. Clinical events were assessed during initial hospitalization and at the end of 1 year. Results: The concentrations of PAI-1 and d-dimer were greater compared with preoperative values 1 hour and 24 hours after surgery in both groups, but their concentrations increased to a greater extent 24 hours after surgery in the on-pump group (p<0.01). The concentration of CRP did not change appreciably immediately after surgery in either group but increased in a parallel fashion 24 hours after either on-pump or off-pump surgery (p<0.01). Bypass surgery in the on-pump group was associated with greater blood loss during surgery and more bleeding after surgery (p0.01). The incidence of all other complications was similar in the 2 groups. Conclusion: On-pump surgery was associated with biochemical evidence of a pro-thrombotic state early after surgery but no greater incidence of thrombotic events. The pro-thrombotic state may have been a consequence of extracorporeal bypass, compensation in response to more bleeding, or both in patients undergoing on-pump surgery.

Perioperative bleeding and use of blood products in coronary artery bypass grafting

Abstract Coronary artery disease (CAD) is the leading cause of death in developed countries. In patients with complex CAD, coronary artery bypass grafting (CABG) remains the preferred treatment as it can provide long-lasting results. However, CABG carries a significant risk of excessive perioperative bleeding and other complications, which may deteriorate the prognosis. Transfusion of blood products is generally used to compensate blood loss. However, both bleeding and blood transfusions have been shown to be associated with an adverse outcome. This cohort study aimed to clarify the impact of perioperative bleeding and the use of different blood products in the development of perioperative complications in 2,764 patients undergoing isolated CABG. The universal definition of perioperative bleeding classification (UDPB) was employed to stratify the severity of bleeding. Additionally, the impact of storage time of transfused red blood cells (RBCs) on the outcome was investigated. Increased UDPB classes, particularly classes 3 and 4, were associated with significantly poorer immediate and late outcome. RBC transfusion in patients who underwent elective off-pump CABG was independently associated with increased troponin I release indicating myocardial injury. Prolonged storage duration of transfused RBCs did not affect immediate and late outcome of patients with moderate bleeding. The most remarkable risk factors for stroke after off-pump CABG were any degree of atherosclerosis of the ascending aorta as well as transfusion of platelets and/or solvent/detergent-treated plasma. The UDPB classification appears to be a promising research tool to stratify the severity of perioperative bleeding and to assess its prognostic impact after coronary surgery. Prevention of major bleeding that leads to blood transfusion may protect from myocardial injury and stroke and possibly result in better early and late outcomes. Patients with a diseased ascending aorta could be considered at high risk of stroke because of their risk of generalized atherosclerosis. In case of mildly diseased aorta, the “no-touch” aorta policy should be considered with the intention of preventing postoperative stroke. / Tiivistelmä Sepelvaltimotauti on yleisin kuolinsyy kehittyneissä maissa. Potilailla, joilla on vaikea monen suonen tai vasemman sepelvaltimon päärungon tauti, sepelvaltimoiden ohitusleikkaus on edelleen paras hoitovaihtoehto, koska sillä pystytään saavuttamaan pitkäkestoisia tuloksia. Kuitenkin ohitusleikkaukseen liittyy suuri riski leikkauksen aikaiselle tai jälkeiselle verenvuodolle ja muille haittatapahtumille, jotka osaltaan huonontavat potilaan ennustetta. Vuodon hoitona käytetään yleisesti verensiirtoa. Kuitenkin on osoitettu, että sekä verenvuoto että verituotteiden anto lisäävät riskiä komplikaatioille. Merkittävimmät riskitekijät ilman sydän-keuhkokonetta tehdyn leikkauksen jälkeiselle aivoinfarktille olivat minkä tahansa asteinen nousevan aortan ateroskleroosi sekä verihiutaleiden ja/tai jääplasman anto. UDPB-luokitus vaikuttaa lupaavalta tutkimustyökalulta verenvuodon vaikeusasteen luokitteluun. Lisäksi sitä voidaan käyttää vuodon ennusteellisen vaikutuksen arvioimiseen ohitusleikkauksen jälkeen. Runsaan verenvuodon ja siihen liittyvän verensiirron ehkäiseminen saattaa suojata potilasta sydänlihasvauriolta ja aivoinfarktilta ja mahdollisesti johtaa parempaan lyhyen ja pitkän aikavälin ennusteeseen. Potilaita, joilla on nousevan aortan ateroskleroosi, voisi pitää suuressa aivoinfarktiriskissä yleisen ateroskleroosiriskin vuoksi. Potilailla, joilla on lieväkin nousevan aortan ateroskleroosi, tulisi harkita aortan jättämistä pihdittämättä aivoinfarktin ehkäisemiseksi.
