Rouquette, Alexandra 09 1900 (has links)
Recently, subjective measurements have increasingly been used in epidemiology, alongside the growing will to integrate individuals’ point of view on their health in studies on diseases or health interventions. Psychometrics includes statistical methods used to develop questionnaires and to analyze questionnaire data. This doctoral dissertation aimed to explore methodological issues raised by the use of psychometric techniques in epidemiology. Three empirical studies are presented and cover 1 / the validation stage of a questionnaire: the objective was to develop, using simulated data, a tool to determine sample size for internal validity studies on psychiatric scale; 2 / the mathematical properties of the subjective measurement: the objective was to compare the performances of the minimal clinically important difference of a questionnaire, assessed on data from a cohort study, computed using the classical test theory (CTT) framework or the item response theory framework (IRT); 3 / its use in a longitudinal design: the objective was to compare, using simulated data, the performances of a statistical method aimed to analyze the longitudinal course of a subjective phenomenon measured using the CTT or IRT framework, especially when some of the available items used for its measurement differ at each time of data collection. Finally, directed acyclic graphs were used to discuss the results from these three studies and the concept of information bias when subjective measurements are used in epidemiology.

Adaptação e validação de escalas de resiliência para o contexto cultural brasileiro: escala de resiliência disposicional e escala de Connor-Davidson / Cross-cultural adaptation and validation of resilience scales for Brazil: dispositional resilience scale and Connor-Davidson resilience scale

Solano, João Paulo Consentino 02 June 2016 (has links)
A more resilient person is the one that exhibits greater abilities to adapt under stress, despite the burden of difficulties and of an unfavorable context. The Dispositional Resilience Scale (DRS-15) and the Connor-Davidson Resilience Scale (CD-RISC) are two scales to measure individual resilience, both of which have had psychometrics evaluated by researchers from the US, Africa, Europe and Asia. OBJECTIVE: To verify the reliability and validity of culturally adapted Brazilian Portuguese versions of the DRS-15 and CD-RISC. METHODS: The following stepwise methodology was used: translation / back translation / cultural adaptation / reliability study / validation study. Cultural adaptation was performed by an expert committee of epidemiologist, linguists, psychiatrist and pain specialists. Comprehension of the culturally adapted versions was tested through 65 interviews with adult patients from the pre-anesthetic consultation ambulatory and general ambulatory for anxiety disorders of Hospital das Clínicas of FMUSP. Back-translations of the culturally adapted versions were fully approved by the authors of the original scales. Validation studies were carried out by concurrent application of both the adapted versions of resilience scales, the Brazilian Stress Symptoms Inventory for Adults (ISSL), the Self-report Questionnaire (SRQ), the Sheehan Disability Scale (SDS) and the Chronic Pain Grade (CPG-Br) to 575 participants (outpatients and companions) from the same population. Test-retest reliability was studied by means of a second interview with 123 subjects, which took place between 7 and 14 days after the first one. RESULTS: Subjects of the validation phase were mostly women (74%), with an average of 44 years of age (18-93) and 10 years of formal schooling. There was a predominance of socioeconomic levels B or C (93%) on an A to E scale. Exploratory factor analyses resulted in a three-factor for the DRS and a four-factor solution for the CD-RISC. Alpha coefficients of 0.71 for the DRS and 0.93 for the CD-RISC indicated better internal consistency for the latter. Temporal stability was regarded as excellent, with intra-class correlation coefficients of 0.81 and 0.86 for the DRS and CD-RISC, respectively. Correlation coefficient between the two scales was 0.52. Significant negative correlations were observed between the scores of both resilience scales and five out of six dimensions of the ISSL, and so as between the resilience scales scores and those of the SRQ and SDS (p < 0.001). No correlation was observed between the resilience scales and the CPG-Br. The CD-RISC was more competent than DRS to depict such correlations. Both scales were able to discriminate differences in resilience scores of non-psychiatric and psychiatric patients, the latter presenting with lower scores. The group of borderline patients significantly presented with lower resilience scores in comparison with those of the post-traumatic stress disorder patients. CONCLUSION: Good reliability and validity were demonstrated with the Brazilian Portuguese versions of the DRS and CD-RISC as tested on a sample of adult ambulatory patients and their adult companions at Hospital das Clínicas, São Paulo

Qualité de vie dans le trouble d'usage d'alcool : une mesure de l'effet thérapeutique du point de vue des patients / Quality of life in alcohol use disorder : a patient-centered assessment of efficacy

Luquiens, Amandine 10 November 2015 (has links)
Context : The evolution of practices in alcohol use disorder field, following the model of patient-centered care, lets revisit the concept of remission for a better consideration of the patient's point of view, participating predominantly in the definition of therapeutic objectives and in their assessment. Yet few of these subjective criteria of the evaluation of patient outcomes have actually included the patient in their development. Until recently, quality of life instruments the most used among patients with alcohol use disorder were generic ones, and their development and particularly generation of items did not involve patients, but rather experts and were based on existing literature. Methods : we conducted a systematic literature review identifying randomized clinical trials testing an intervention in alcohol-dependent patients, and measuring its effectiveness on quality of life. We have listed the dimensions explored in the existing instruments. It has been shown that these instruments did not optimally explore the impact of alcohol dependence on quality of life in this population. On this, we have developed a scale specific for patients with alcohol use disorder, following the recommendations of the Food and Drug Administration (FDA) on the development of "Patient-Reported Outcomes", namely involving patients extensively. We used the method of focus groups conducted in parallel in France and Great Britain with 38 patients with alcohol use disorder. These groups allowed the generation of a pool of items, secondarily tested by individual cognitive interviews with 31 patients, resulting in the final scale "Alcohol Quality of Life Scale" (AQoLS). We then validated the AQoLS scale on a sample of 285 French patients with alcoho use disorder, recruited from randomly selected centers in France. Results: The scale has 34 items developed, distributed in seven dimensions: social relationships, activities, living conditions, looking after self, negative emotions, sleep and loss of control. The validation study confirmed six of the seven dimensions, with a slightly different distribution of the items. The "looking after self" dimension disappeared in favor of a new dimension of self-esteem. Control and self-esteem dimensions are innovative in a health-related quality of life instrument in adults. The scale showed good psychometric properties, with good internal consistency (Cronbach = 0.96), and a moderate correlation with sub-scores of the SF-36 and EQ-5D. A qualitative complementary work could illustrate the links between the different affected areas, including the proximity in patients’ perception between self-esteem, loss of control and cognitive impairment. Conclusion : The move towards a model of patient-centered care allows considering a different approach of remission and to build and validate a measure of health-related quality of life close to the concerns of patients with a alcohol use disorder, AQoLS scale. Two innovative dimensions were attached to this concept: loss of control from the patient's perspective, and self-esteem.

However, parental satisfaction is also considered an important indicator. Satisfaction surveys are already carried out frequently, but the questionnaires are not evaluated in relation to validity and reliability. In the Netherlands, the Empowerment of Parents in the Intensive Care 30 (EMPATHIC-30) questionnaire was developed to assess parental satisfaction in UTIO. In Brazil, validated questionnaires were not found for this purpose, so our aim is to translate, culturally adapt and validate the content of EMPATHIC-30 to evaluate parents' satisfaction in Brazilian PICUs. Methods: : the translation and cultural adaptation was carried out according to the protocol of the translation and cultural adaptation group of the International Society for Pharmacoeconomic Research - ISPOR 2005. The content validation was carried out by a committee of experts. After that, the mean and standard deviation (SD) for all items were calculated, and the content validity coefficient (CVC) for each item and for the instrument as a whole. Conclusion: EMPATHIC-30 was translated and adapted culturally to the Brazilian population. Validation demonstrated a total above-average CVC, confirming the validity of the instrument content.

Satisfação do paciente hospitalizado com sua privacidade física: construção e validação de um instrumento de medida / The satisfaction of the hospitalized patient with his/her physical privacy: construction and validation of a measurement instrument.

Pupulim, Jussara Simone Lenzi 26 February 2009 (has links)
The study is of a methodological type using the psychometric approach which deals with the theory of the development of measurement instruments. The methodological apparatus is based on empirical and analytical theoretical proceedings, to the development of a psychometric instrument (PASQUALI, 1998), and is grounded in two privacy theories (WESTIN, 1970; ALTMAN, 1975), on the territoriality theory (ARDREY, 1966) and on the proxemic theory (HALL, 1986). The theoretical proceedings entailed the definition of the physical privacy construct in hospital context, especially the exposure and body manipulation, drawing on the theoretical apparatus used, the related literature and a study with thirty-four patients admitted in hospitals in Maringá-PR, with qualitative approach, using the content analysis (BARDIN, 2000). This study allowed to describe the perception of the hospitalized patient as regards general privacy and physical privacy, identifying three empirical categories concerning general privacy, namely dignity and respect, personal and territorial space and autonomy, and as regards physical privacy the same categories were extracted, adding to the previous ones the category of intimacy and touch. Such categories composed the four domains of the instrument under construction. The instrument was subjected to semantic and conceptual analyses by a panel of judges composed by seven connoisseur, followed by a pretest with the target population, resulting in an instrument with 22 items, in which the domain dignity and respect had eight items, intimacy and touch and personal and territorial space had five items each, and the domain autonomy had four items. The empirical proceedings entailed the application of the constructed instrument, Satisfaction Scale of the Hospitalized Patient with his/her Physical Privacy (ESPH-PF), to a sample of 254 patients admitted in the hospital chain of Maringá-PR between November 2007 and February 2008. The analytical proceedings entailed the internal consistency analysis (-Cronbach total of 0,68 variation between 0,35 and 0,60 in the domains) and the construct validity by means of factorial analysis, extracting six factors (57,6% of total variation) and opting to maintain the four preestablished ones (47,7% of explained variation). The convergent validity of the construct utilizing the correlation product-moment of Pearson (p < 0,01) and the application of Feeling Scale before the Invasion of Territorial and Personal Space of the Hospitalized Patient (EMSFIETEP) contemplated the domain personal and territorial space of ESPH-PF and confirmed the discriminating power of all 22 items of the instrument. The discriminating validity of the construct employing the test t of student (p < 5005) did not find significant statistical difference, except for extreme groups regarding education. The psychometric analysis shows that the ESPH-PF can be applied to measure the patients satisfaction with his/her physical privacy, once the level of satisfaction implicitly reveals his/her privacy preferences and necessities, and also indirectly reflects the performance of health professionals as regards protection and maintenance of hospital privacy.

Hand functioning in children with cerebral palsy / Le fonctionnement de la main chez les enfants infirmes moteurs d'origine cérébrale

Arnould, Carlyne 13 February 2006 (has links)
The purpose of the present work was to study hand impairments and manual ability in children with cerebral palsy (CP) as well as to clarify their relationship. Appraising the degree of hand impairments requires normative data to differentiate the real dysfunctions of CP children from the normal difficulties according to their age, sex, or handedness. As there is no normative data for gross manual and fine finger dexterity, a first experiment focused on the normal development of manipulative functions was conducted to establish these norms. A second experiment was carried out to develop and validate through the Rasch model a measure of manual ability in children with CP since such a measure was not yet available. The invariance of the ABILHAND-Kids questionnaire was also tested across relevant demographic and clinical subgroups of CP children. Finally, a third experiment was performed to quantify the hand impairments in children with CP and to investigate their relationship with manual ability as measured with the ABILHAND-Kids questionnaire. Hand motor impairments, markedly more prevalent than hand sensory impairments, were moderately correlated with manual ability measures and predicted 58% of their variance. Consequently, manual ability cannot simply be inferred from hand impairments and should be measured and treated per se.

Hand motor impairments, markedly more prevalent than hand sensory impairments, were moderately correlated with manual ability measures and predicted 58% of their variance. Consequently, manual ability cannot simply be inferred from hand impairments and should be measured and treated per se.

An investigation into the criterion related validity of the Prejudiced Attitudes Towards Aboriginals Scale

Nesdole, Robert Shawn Andrew 21 September 2009
The topic of racism and prejudice is a very sensitive issue for many Canadians. This is primarily due to Canada's cherished image as a tolerant society leads even the most progressive people to adopt the belief that racism is comprised of only overt acts. However, what is not acknowledged by this old-fashioned view of prejudice is that the nature and expression of prejudice has evolved into a more covert form of prejudice known as modern prejudice (McConahay, 1986). This is particularly important because there is a well documented history of prejudice and discrimination of Aboriginal peoples in Canada, and without a proper understanding of the changing face of prejudice in Canada it is difficult to determine if there in fact has been a reduction in prejudiced attitudes towards Aboriginal peoples in Canada.
This study examines the criterion-related validity of the Prejudice Attitudes Towards Aboriginals Scale (PATAS) (Morrison, 2007) a 25 item (11 old-fashioned prejudice items and 14 modern prejudice items) newly developed measure of prejudicial attitudes towards Aboriginals developed using the theory of Modern Prejudice (McConahay, 1986). Four hundred and five undergraduate students completed a questionnaire including the PATAS, Right-wing Authoritarianism scale (RWA scale) (Altemeyer, 2007), Social Dominance Orientation scale (SDO5) (Pratto, Sidanius, Stallworth, and Malle, 1994), Motivation to Control Prejudice Reactions Scale (MCPRS) (Dunton and Fazio, 1997), and Form C of the Marlowe Crowne Social Desirability Scale (Renolds, 1982).
The results of this investigation revealed evidence attesting to the criterion-related validity of the PATAS. Participants who scored high on the PATAS also had high scores on the RWA scale, SDO5 scale, and tended to have a conservative political orientation. However, contrary to expectations, religious self schema was not found to be associated with PATAS scores. Practical applications and limitations of these findings are discussed as are possible directions for future research.
