Tornar-se pai e mãe no contexto da reprodução assistida

O tornar-se pai e mãe e constituir uma família são fenômenos importantes para o estabelecimento de uma identidade social adulta, contribuindo para a realização pessoal do indivíduo. Quando esse projeto parental não pode ser realizado em decorrência de infertilidade no casal, é possível recorrer às técnicas de reprodução assistida, como uma tentativa de concretização desse projeto. Entretanto, o desgaste físico e emocional decorrente desses procedimentos pode conferir à gestação e à parentalidade um caráter específico, com repercussões ainda pouco conhecidas. Por meio deste estudo longitudinal, com três casais que engravidaram por técnicas de reprodução assistida, buscou-se conhecer o processo de tornar-se pai e mãe no contexto da reprodução assistida, com medidas coletadas no terceiro trimestre de gestação e aos três meses do bebê. Os resultados apontaram que a vivência da gestação nesse contexto caracteriza-se pelo constante medo de perder o bebê, mesmo no último trimestre gestacional, sendo que as gestantes deste estudo apresentaram sentimentos de incapacidade de levar a gestação a termo. A prematuridade tornou real a possibilidade de perda do bebê, tornando a vivência dessa etapa um desafio. Superado esse período, o medo da perda do bebê diminuiu, dando espaço para o estabelecimento da crença de que esses casais são capazes de garantir a sobrevivência de seus bebês. / Becoming a father or a mother and building a family are important phenomena that contribute for the establishment of an adult identity as well as for a personal accomplishment. When the dream of building a family can not be fulfilled couples may be helped by assisted reproduction techniques, although the physical and psychological stress produced by these techniques is not well-known yet. This longitudinal and qualitative research aims at knowing how transition to parenthood occurs in the context of assisted reproduction techniques. Three couples who got pregnant through assisted reproduction techniques were interviewed in two moments: during the last trimester of pregnancy and when the baby is three months old. The results show that the participants of this research lived pregnancy freightened by the possibility of loosing their baby. When the baby was born these feelings turned into a possibility due to they were pre-term babies. Afterwards, these feelings decreased while the belief of being capable of keeping the baby alive emerged.

O desenvolvimento da relação do casal durante a transição para a parentalidade no contexto da reprodução assistida : um estudo longitudinal da gestação ao primeiro ano de vida do bebê

Este trabalho investigou o desenvolvimento da relação coparental e conjugal durante o processo de transição para a parentalidade, no contexto da reprodução assistida. Realizou-se um estudo de caso coletivo longitudinal. Três casais responderam a entrevistas individuais semiestruturadas em três momentos: (a) a gestação (b) após o terceiro mês de vida do bebê e (c) após o primeiro ano do bebê. Os dados foram submetidos a uma análise narrativa, que enfocou tanto as particularidades de cada casal, como suas semelhanças. Sugere-se a existência de um processo de diferenciação crescente da relação do casal, que passa a englobar coparentalidade e conjugalidade. Propõe-se que, no contexto da reprodução assistida, considere-se o período do tratamento como marco inicial da relação coparental. Destaca-se a necessidade de considerar a história do casal, sua experiência da infertilidade e do tratamento, suas relações com a família de origem e sua configuração familiar na compreensão desse processo. / This study investigated the development of the marital and the coparental relationships during the transition to parenthood, in couples who conceived trough assisted reproduction techniques. A collective longitudinal case study was carried out. Three couples answered individual semi-structured interviews during three moments: (a) pregnancy, (b) after baby’s third month, and (c) after baby’s first year. The data were submitted to a narrative analysis, which focused both the particularities of each couple and the similarities between them. It has been identified a process in which couples’ relationships became more differentiated and divided into marital and coparental relationships. It is proposed that in couples who conceive through assisted reproduction, the treatment should be considered as a milestone of coparenthood beginning. It is also highlighted the need to consider the couple’s history, their infertility and treatment experience, the relationships with their families of origin, and their family form to understand this relationship development process.

Here, we propose a multidisciplinary analysis, examining factors used in the classic issue of dealing with the concept of coronelismo, under the academic perspective and also in the common sense of the plan. And points as genuine training tool of rural sociability provincial family based structures, in particular the institute of god parenthood, which serves as a basic element of the cultural model and its features. In an anthropological and historical approach, the paper examines the process of formation of nationality, prepared by identifying the elements considered "desirable" and hence the rejection of many others. These features, presented as anachronisms and obstacles to the "modernization" of the country, promoted the formation of an exclusive national discourse, inducing consider as subordinate the dominant traditional values in the rural environment. Thus, there has been a discriminatory environment in qualitative terms, alienating the people of the interior and helping to prevent, in practical terms, their integration into the environment of imagined community that builds the Brazilian national fabric / A presente tese de Doutorado tem como objetivo analisar, sob uma perspectiva inovadora, a dinâmica das relações sociais em um ambiente típico do interior rural brasileiro, visando em sentido mais amplo compreender os mecanismos que atuam sobre a sociabilidade e sobre o comportamento político dessa população. Neste sentido, propõe uma análise multidisciplinar, examinando fatores clássicos utilizados no trato da questão como o conceito de coronelismo, sob a ótica acadêmica e também no plano do senso comum. E aponta como genuíno instrumento de formação da sociabilidade rural interiorana as estruturas de base familiar, em especial o instituto do compadrio, que serve como elemento básico de estruturação do modelo cultural e suas particularidades. Em uma abordagem antropológica e histórica, o trabalho examina o processo de formação da nacionalidade, elaborado por meio da identificação de elementos considerados desejáveis e, consequentemente, na rejeição de outros tantos. Coparenting across the transition to parenthood : qualitative evidence from South-Brazilian families

Coparenting emerges across the transition to parenthood and refers to the way individuals coordinate, support each other in their parental roles and share responsibility in childrearing. Despite the increase in research on coparenting, relatively few studies have focused on non- North American or non-European families, which has hindered practice and policy targeting diverse countries. Likewise, qualitative research on coparenting is relatively rare, yet critical to shed light on details and complexities not well captured by other methods, including insights into sociocultural factors linked to coparenting in distinct contexts. Moreover, a qualitative longitudinal approach is particularly well suited to examine important life course transitions and turning points, such as the transition to parenthood. To address these gaps, we investigated coparenting across the transition to parenthood in South-Brazilian families, using a qualitative, longitudinal, multiple case study. Semi-structured, face-to-face interviews were conducted with 12 first-time mother and fathers (six nuclear families altogether), at 6, 12, and 18 months postpartum (36 interviews altogether). In two families only the father was employed, with the mother caring for the child; in two families both parents were employed, and they hired a nanny to care for the child in their home; and, in two families both parents were employed, and the child started attending daycare at the end of maternity leave. Through the two articles that comprise the current doctoral dissertation, we explored three components of Feinberg’s (2003) coparenting framework: division of labor (how parents divide childcare tasks and household chores, as well as their satisfaction with this division); agreement/disagreement (e.g., regarding children’s emotional needs and discipline); and, support/undermining (appreciation and cooperation, or criticism and competition). The first article is focused on the division of labor, whereas the second is focused on agreement/disagreement and support/undermining. Deductive thematic analysis revealed similarities and singularities between families. As presented in the first article, we found weaker sharing of household chores over time, against greater sharing of childcare tasks during the first few days postpartum, followed by a downward tendency in the fathers’ contributions during the first few months postpartum. This more unequal division of labor remained stable over time only for families who had chosen maternal care, changing after the end of maternity leave for families who had chosen nanny care and daycare. Parental satisfaction regarding the division of labor remained relatively high over time only for families who had chosen nanny care, which suggests that counting on a domestic worker since the beginning of the transition to parenthood contributed to preventing parental feelings of overload or unfairness across the transition to parenthood. Findings were discussed in the 10 light of the role that instrumental and social support, as well as the prevailing Brazilian gender norms, may play in the division of labor for new parents. With respect to the second article, our findings showed that agreement among parents remained relatively stable during the first year, whereas disagreements concerning discipline demanded more parental negotiation as infants advanced toward toddlerhood. Support and undermining coexisted in the same families, although mothers and fathers expressed undermining differently. Aspects of the ecological context, such as family of origin, instrumental and social support, as well as labor market, also appeared to influence coparenting agreement/disagreement and support/ undermining. Following each article, we presented strengths, limitations, suggestions for future research, as well as implications for practice (e.g., counseling) and policy (e.g., childcare arrangements and parental leave).

O desenvolvimento tecnológico contemporâneo permite a detecção precoce de malformações fetais, tais como a anencefalia, que inviabilizam a sobrevivência do bebê. Tal constatação implica imediata revelação diagnóstica aos pais, que deverão decidir pela continuidade ou interrupção da gravidez. O presente trabalho tem como objetivo investigar a experiência emocional de casais que se deparam com o diagnóstico de anencefalia fetal, tendo em vista trazer subsídios para um melhor atendimento psicológico. A investigação organizou-se como pesquisa qualitativa, com método psicanalítico, estruturada ao redor de procedimentos investigativos de acesso, registro, interpretação e interlocuções reflexivas sobre atendimentos clínicos. A partir de sessões com casais parentais, realizadas ao longo de oito anos, foram elaboradas duas narrativas transferenciais ficcionais, que preservam elementos essenciais da dramática em pauta. Uma das narrativas aborda uma situação de opção por interrupção gestacional, enquanto a outra focaliza uma decisão de continuidade. Os procedimentos interpretativos permitiram a \"criação/encontro\" dos seguintes campos de sentido afetivo-emocional ou inconscientes relativos: É um pesadelo?, Quem ou o quê está aí?, É preciso decidir. O quadro geral aponta que grande parte do trabalho clínico tem lugar num campo bastante singular, É preciso decidir, que se define pela urgência de tomada de decisão relativa à eventual interrupção de processos vitais. O campo É um pesadelo? aponta para o fato deste tipo de revelação diagnóstica, derivada do uso de uma tecnologia, que detecta problemas que não estão sendo vivenciados como sinais ou sintomas físicos, gerar muito frequentemente reações dissociativas, cujo manejo torna-se, assim, clinicamente indispensável. O outro campo, Quem ou o quê está aí?, assume uma posição de centralidade, nesta clínica, na medida em que porta consigo uma interrogação radical acerca do estatuto ontológico do feto, vivido como um bebê ou como um não-bebê. Uma compreensão sensível e atenta acerca dos diferentes modos como cada casal habita este campo parece fundamental para a provisão de um cuidado psicoterapêutico / The contemporary technological development allows the early detection of fetal malformations, such as anencephaly, which makes the babys survival unfeasible. This assumption leads to the immediate revelation of the diagnosis to the parents who will decide either to continue or interrupt the pregnancy. This paper focuses on the investigation of the emotional experience that couples have when facing a fetal anencephaly diagnosis, bringing instruments for a better psychological care. The investigation process was organized as a qualitative research, through psychoanalytic approach, based on the investigation procedures of access, register, interpretation and reflexive interlocution on clinical care sessions. From the sessions with couples, during eight years, two transferential fictional narratives, that preserve essential elements of drama at stake, were created. One of the narratives approaches a situation of choice for pregnancy interruption, while the other aims at the decision to keep it. The interpretative procedures allowed the \"creation / finding\" of the following fields of affective-emotional sense or relative unconscious: Is it a nightmare?, Who or what is there?, We have to make a decision. The big picture shows that great part of the clinical work takes place in a singular field, We have to make a decision, defined by the sense of urgency around the decision about the eventual interruption of vital processes. The field Is it a nightmare? leads to the fact that this kind of diagnosis revelation, derived from the technology which detects problems that were not being lived as physical signals or symptoms, frequently generates dissociative reactions, making the clinical care mandatory. The other field Who or what is there? has a central role, in this clinic, as soon as it contains an extreme question around the fetuss ontological statute, being a baby or a non-baby. A sensible and attentive comprehension of the different ways in which couples deal with this field is essential for a psychotherapeutic care

A parentalidade em famílias homossexuais com filhos: um estudo fenomenológico da vivência de gays e lésbicas / The parenthood in homosexual families with children: a phenomenological research of gays and lesbians experiences

Atualmente, presenciamos múltiplos tipos de família, como: famílias nucleares, monoparentais, reconstituídas, com filhos biológicos e/ou adotivos, dentre as quais encontramos famílias homossexuais. Há uma escassez de trabalhos sobre essas famílias e percebe-se a forte presença de preconceito e discriminação nos mais diversos segmentos e contextos sociais, em especial, no que diz respeito às questões homossexuais e de gênero e de como isso influenciaria na educação das crianças. Esse estudo visa compreender como homossexuais entre 20 e 55 anos, vivenciam a paternidade, a maternidade e/ou parentalidade e que significados lhe atribuem. A fenomenologia ancorada à filosofia do diálogo de Buber foi o referencial teórico-metodológico adotado nesta pesquisa para alcançarmos o objetivo proposto. Foram entrevistados seis homens e nove mulheres homossexuais com filho(a)(s) biológicos e/ou adotivos que formaram famílias monoparentais, adotivas, reconstituídas ou nucleares. Os resultados apontam um maior preparo psíquico e socioeconômico para a chegada de uma criança, em especial quando o desejo de ter filhos ocorre após a tomada de consciência da homossexualidade e/ou formação do vínculo conjugal homossexual. As funções parentais são exercidas pelos(a)s colaboradore(a)s os com nuances da relação intersubjetiva EU-TU. Foram relatadas situações de preconceito quanto ao exercício da parentalidade e/ou à expressão da homossexualidade, nas famílias de origem, no trabalho e entre os amigos, os quais puderam ser diminuídos por intermédio da convivência e conhecimento das situações vivenciadas. Alguns do(as)s colaboredore(a)s deixaram entrever uma homofobia internalizada, principalmente em relação à sua própria homossexualidade, que os aproxima das palavras princípio EU-ISSO. O modelo heterocêntrico de família é recorrente nos discursos assim como a falta de referenciais de famílias homossexuais. / At present, we are witnessing multiple family forms such as nuclear families, single parent families, and reconstructed families with biological and/or adopted children. Among these we find homosexual families. There is a shortage of works about those families. We strongly perceive the presence of prejudice and discrimination concerning homosexual questions with respect to more diverse segments and social contexts, especially about the homosexuality issue and how it would influence in the children?s education. Our goal is to understand how homosexuals between 20 and 55 years experience fatherhood, motherhood, and/or kinship and what meanings they attribute to these. The phenomenology anchored in the philosophy of of Buber\'s dialogue was the theoretical-methodology referential referred to and adopted in this research to reach the proposed goal. Six men and nine homosexual women, with biological and/or adopted children, who formed single parent, adopted, rebuilt or nuclear families were interviewed. The results aim towards a greater socio-economic and psychological preparation for the arrival of an infant, especially when the wish to have a child occurs after consciousness of the homosexuality and/or the formation of the conjugal homosexual link. The parental function is exercised by both, with nuances from the intersubjective relationship of ME-YOU. Situations of prejudice, especially as regards the sexual papers examined for this research are also related, as well as the expression of homosexuality, in the families of origin, at work among friends, in which could be decreased through the experience and knowing of the situations experienced. Some of the collaborators showed during the study an internalized homophobia, mainly in relation to his/her own homosexuality, which approach them to the principle ME-IT. The heterocentric model of family appears in the speech as well as the lack of homosexual families references.

Um estudo sobre o adiamento da maternidade em mulheres contemporâneas / A study on the postponement of motherhood in contemporary women

A possibilidade do controle feminino sobre a reprodução, de escolha ou não pela maternidade, do número de filhos e de quando tê-los, se coloca como um fenômeno que vem se consolidando ao longo do tempo, acompanhando as diversas transformações na família, no casamento, na maternidade e na paternidade. Cresce, nesse contexto, o número de mulheres que postergam a maternidade para idades cada vez mais avançadas. Tal fenômeno vem muitas vezes carregado de idealizações e de desconhecimento, o que pode gerar a ilusão de um controle que não necessariamente equivale às vivências reais. O objetivo geral desta tese é o de investigar o fenômeno do adiamento da maternidade, na sociedade atual, com base nas vivências de mulheres de dois diferentes grupos que optaram por ter o primeiro filho depois dos trinta e cinco anos: aquelas que conseguiram e aquelas que não, seja porque ainda estivessem tentando, seja porque houvessem desistido. Como objetivo específico, pretende-se pensar a influência dos fatores conjugalidade, carreira profissional e história de vida, nas famílias de origem na determinação desse tipo de escolha. Para tanto, foram realizadas, a partir de uma metodologia clínico-qualitativa, a aplicação das pranchas 1, 2, 7MF, 10 e 16 do TAT, além de uma entrevista semidirigida, com oito mulheres quatro pertencentes a cada grupo com idades entre trinta e seis e quarenta e um anos, em um relacionamento estável, com mais de oito anos de escolaridade, habitantes da região metropolitana de São Paulo. Os dados foram analisados de acordo com o referencial psicanalítico, levando-se em conta, também, aspectos históricos e psicossociais que influenciam o fenômeno. Os resultados são apresentados de acordo com as seguintes categorias de análise: a experiência do adiamento da maternidade; a maternidade, como experiência para aquelas que se tornaram mães, e como experiência projetada, para aquelas que não se tornaram mães; conjugalidade; carreira profissional; relação com a própria mãe e conjugalidade dos pais; e a questão do poder. Conclui-se que o fenômeno do adiamento da maternidade, na sociedade contemporânea, mostra-se complexo e imerso em uma multiplicidade de significados. Em meio às especificidades de cada caso, algumas aproximações puderam ser feitas entre as experiências das mulheres dos dois grupos, especialmente no que diz respeito à valorização e idealização da conjugalidade, percebida inclusive como mais importante que o desenvolvimento de uma carreira profissional, bem como a possibilidade de integração dos aspectos que concernem ao poder em relação ao corpo e ao controle da reprodução por meio dos binômios: onipotência/impotência e elementos femininos/masculinos puros na mulher contemporânea, de um ponto de vista winnicottiano. As divergências mais marcantes entre os dois grupos se referem sobretudo à relação estabelecida com as próprias mães, além da conjugalidade dos pais. A postergação da maternidade, fenômeno tão atual e com potencial para mobilizar tantas angústias e sofrimentos, pode ser vivenciada pelas mulheres contemporâneas de maneira mais ou menos saudável ou criativa, com uma maior ou menor capacidade de integração da ambivalência, a depender dos recursos psíquicos individuais e do legado geracional / The womans option to control her fertility, to choose motherhood or not, to choose the number of children and the time to have them, is a phenomenon that has been establishing itself over time, following diverse transformations in the family and the marriage, as well as in motherhood and fatherhood. In this context, there has been an increase in the number of women who delay childbirth until later in life. Many times, such phenomenon comes loaded with idealization and lack of knowledge, which may generate an illusion of control that does not necessarily equate with the real experience. The general objective of this thesis is to investigate the phenomenon of delayed motherhood in current society from the perspective of two different groups of women who delayed childbirth to later than thirty-five years of age: those who had already succeeded and those who hadnt yet and either gave up or kept trying. The specific objective is to reflect on the influence of the following factors in determining the choice of delayed childbirth: marital situation, professional career and history of families of origin. For this purpose, it was used a clinical-qualitative methodology that included the application of pictures 1, 2, 7GF, 10 and 16 of the TAT (Thematic apperception test) and of semi-directed interviews with eight women (four from each group) with ages ranging from thirty-six to forty-one years, who were in a steady relationship, had more than eight years of schooling, and resided in the metropolitan region of São Paulo. The data are analyzed in accordance to the psychoanalytical referential, taking into account historical and psychosocial aspects that influence the phenomenon. The results are presented in accordance to the following categories of analysis: the experience of delayed childbirth; the experience of motherhood for those who became mothers and the projected experience for those who did not become mothers; marital life; professional career; relationship with their own mother; their parents marital life; and the power issue. The conclusion is that the phenomenon of delayed motherhood in contemporary society reveals itself as complex and immersed in a multiplicity of meanings. Among the specificities of each case, some proximity between the experiences of the women of the two different groups could be established particularly with regard to the attributed value and idealization of the marital relationship, perceived as more important than the development of a professional career; and with regard to the integration of the power aspects that refer to the body and the control of fertility, by means of the binomials: omnipotence/impotence and pure masculine/feminine elements in the contemporary woman, from a Winnicottian point of view. The remarkable divergences between the two groups referred especially to the relationship established with their own mothers and the marital life of their parents. The delayed motherhood, a current phenomenon with potential to stir much distress and suffering, can be experienced deeply by the contemporary women in a more or less healthy or creative way, to a greater or lesser degree of integration of their capacity for sustaining ambivalence, depending on the individual psychic resources and his/hers generational legacy

Adolescent Religion and Parenthood Outcomes in Young Adulthood

A multitude of research exists examining the relationship between religion and early marriage, yet little research has focused on the relationship between religion and early childbearing. Even less has examined the influence of adolescent religion on early parenthood. Using data from the National Study of Youth and Religion, I examined the relationship between religion in adolescence and parenthood outcomes in early adulthood. I focus on how religiosity in adolescence shapes whether an individual is more or less likely to be sexually active, become pregnant, and/or have and keep a child. Results suggest that those who are religious in adolescence are less likely to have children early because of the postponement of sexual debut.

Manitoban Consensual Non-monogamous Couples' Conciliation of Their Parenting Role and Their Sexual Lifestyle During the Transition to Parenthood

Background: Consensual non-monogamous couples (CNMCs) are viewed less favourably than their heteromononormative counterparts by the general population and by healthcare providers. Research indicates that they are less likely to seek health care and are at greater risk for STIs and HIV. This stigma and judgment perceived by CNMCs can be even further compounded when these couples choose to have a child. No study to date has looked at consensual non-monogamous parenting couples (CNMPCs) during the transition to parenthood. The aim of the present study was to explore Manitoban CNMCs’ perceptions of the conciliation between their parenting role and their sexual lifestyle during the transition to parenthood. Methodology: This mixed methods descriptive, exploratory study used a triangulation design-convergence model. Six participants identifying as CNMCs during the transition to parenthood were interviewed using a semi-structured interview guide as well as completing an online questionnaire. Results: The participants in this sample experienced challenges in regard to their transition to parenthood as many other parents do, yet this transition was more harmonious for some participants compared to others. Consensual non-monogamy (CNM) was a sexual lifestyle chosen either before or during this transition. However, the lifestyle did stop during conception and pregnancy, and was resumed several months after childbirth. Relationship breakdown may occur, but not necessarily associated with CNM. The conciliation of parenting and sexual roles is facilitated when communication and intimacy are present between partners. Participants emphasized the importance of family before their chosen sexual lifestyle. The relationship with health care providers is critical for participants of CNM as it impacts how they seek health care or disclose their lifestyle. This is more important during the transition to parenthood as more challenges can be present. Three phases that CNMCs pass through were also identified, contemplation, acting and incorporation, to integrate CNM as a lifestyle. Discussion: These findings permitted a closer look at the conciliation of the parenting role and the sexual lifestyle of CNMPCs during their transition to parenthood. These findings demonstrated not only how CNMPCs were similar and different from participants in other studies, but also highlighted how they were uniquely distinct. This distinction appears to stem from a certain resilience gained from the three phases of the incorporation of CNM as a lifestyle that appears to buffer these couples in situations of stress. A new proposed model, CNMPCs’ Model of Resilience during the Incorporation of CNM as a Lifestyle While Parenting, is suggested. There is a need for more psychosexual education for perinatal nurses in regard to sexuality minorities such as CNMPCs in the context of the transition to parenthood.

Planned Parenthood and Fertility, Illegitimacy, and Abortion Rates in the State of Utah

The purpose of this study was not to judge the rightness or wrongness of Planned Parenthood. It is an empirical question to ask what influence, if any, Planned Parenthood has had on reproductive behavior in the State of Utah. This was accomplished by gathering data from each county in the State of Utah from 1970 to 1979. The data gathered were for the five dependent variables of General Fertility, General Abortion, General Illegitimacy, Teen Fertility and Teen Illegitimacy. A mean rate for each county and variable was computed. Each county was also weighed per population so as to give counties with large populations more weight than counties with small populations. A comparison of the rates in counties with Planned Parenthood and counties without Planned Parenthood was then made. It was noted that counties with Planned Parenthood had higher rates for each variable except General Fertility which was lower than counties without Planned Parenthood. But the differences were not of a significant level. In most cases the differences already existed before Planned Parenthood was established.
