In die studie word daar ook verwys na verwante kwessies, soos aanraking en grense in die terapeutiese verhouding, terapie as 'n a-sosiale konteks, terapie as 'n paradoksale situasie en die neutraliteit van die terapeut, wat aansluit by die tema van hierdie studie. Hierdie temas word geintegreer tydens die bespreking van die gevallestudie. / An individual case study is used in this study, to discuss the complementarity between distance and intimacy in the therapeutic relationship. The focus is on recursive and selfreflective processes between the therapist and the client in order to explain the way in which intimate and distant interactions were coconstructed. The therapeutic process was discussed in the context of ecosystemic and social constructionist epistemologies. Research was done according to the principles of the naturalistic research paradigm and overlaps between this research paradigm and epistemologies that were discussed are brought forward through the discussion of the case study. There are also references in this study to related issues, such as touching and boundaries in the therapeutic relationship, therapy as an a-social and paradoxical context, and the neutrality of the therapist, which connect with the theme of this study. Inledning: ISTDP-metoden har en växande evidensbas. Metoden kräver högre grad av aktivitet och direkthet från terapeutens sida jämfört med klassiskt psykodynamiskt förhållningssätt. Syftet med denna studie är att fördjupa kunskapen om psykoterapeutstuderandes upplevelser av att lära sig och arbeta med ISTDP parallellt med andra utbildningsinslag. Frågeställningar: Fokuserar på psykoterapeutstuderandes upplevelser av lära och integrera ISTDP-metoden med andra psykodynamiska arbetssätt. Metod: Kvalitativ intervjustudie med psykoterapeutstuderande som utbildats i ISTDP under psykoterapeututbildningen. Resultat: Upplevelserna av ISTDP-momentet skiljer sig mellan intervjupersonerna, som beskriver en ambivalens: dels uppskattning över att ha lärt sig metoden, dels utmaningen i att vara aktiv, uppmärksam och samtidigt tänka ut lämpliga interventioner. Initial osäkerhet avtog med ökad erfarenhet och gav större mod och trygghet i terapeutrollen. Metoden upplevs underlätta tydlighet, fokus och ångestreglering i patientarbetet. ISTDP-momentet gavs stort utrymme vilket begränsade utrymmet för andra ämnen. Diskussion: Denna studie visar på svårigheter förknippade med att lära sig en metod där arbetssättet skiljer sig från övriga utbildningsmoment. Studien belyser vikten av balans mellan utmaning och trygghet i lärsituationen med hänsyn taget till studenternas tidigare kunskaper och begränsade erfarenhet. Faktorer som gynnar respektive hämmar lärandet diskuteras. / Introduction: ISTDP, a method with a growing evidence base, requires a greater degree of activity and directness from the therapist than the classical psychodynamic approach. The purpose of this study is to gain knowledge of psychotherapy students' experience of learning ISTDP in parallel with other educational features. Questions: Focus on psychotherapy students' experience of learning ISTDP and integrating it with other ways to work in psychodynamic therapy. Results: Interviewees describe ambivalence: on one hand an appreciation of having learned the method, on the other the challenges of being active, attentive, and finding appropriate interventions. Growing experience led to greater courage and sense of security in the therapist role. The method was perceived to facilitate clarity, focus, and regulation of anxiety in patient work. Discussion: This study demonstrates the difficulties involved when learning a method where working ways differ from those taught in other courses. The study underlines the importance of a balance between challenge and security in the learning situation, considering the students' previous knowledge and limited. Factors that promote and impede learning are discussed.

Digital art therapy and trauma : a psycho-educational perspective

This study explores the possible use of digital media as an effective psychotherapeutic aid in art psychotherapy, specifically in addressing the effects of trauma experienced by adolescents. A literature review provides evidence on what exactly can be understood under the concepts 'art psychotherapy' and 'digital media'. The literature study further investigates trauma, its treatment and how art psychotherapy can be used as a psychotherapeutic tool with adolescents who have been exposed to trauma. An empirical study including six participants, chosen through a specific sampling process, and whose background information regulates the appropriate methods of intervention demonstrates the practicality of digital media in art psychotherapy. Data gathered from pre-assessment activities, art psychotherapy sessions and post-assessment activities are analysed, interpreted, and reduced through a systematic process. The empirical findings are then presented in a detailed, concise manner From the empirical investigation, it is determined that digital media can be used as a successful tool in art psychotherapy, specifically with adolescents who have experienced trauma. The researcher gained experience in the implementation of digital media in art psychotherapy and was able to provide valuable information about this practice, specifically for professionals interested in the field of art psychotherapy. Mathematical model of the dynamics of psychotherapy

This is a novel attempt to produce a rigorous mathematical model of a complex system. The complex system under study is the relationship between therapists and their clients. The success of psychotherapy depends on the nature of the relationship between a therapist and a client. We use dynamical systems theory to model the dynamics of the emotional interaction between a therapist and client. We determine how the therapeutic endpoint and the dynamics of getting there depend on the parameters of the model. ... We describe the emotional state of both the therapist and client with coupled, first order, nonlinear ordinary differential equations (ODE's). The rate of change of the emotional state of the therapist and client is proportional to their previous state, their uninfluenced state when alone, and an influence function which depends on the state of the other person. We formulated influence functions based on the research literature on psychotherapy and the therapeutic alliance. We then determined the critical points from the intersection of the nullclines and used a numerical ODE solver (Matlab ODE113) to compute the trajectories from different initial conditions. ... It is intended to present and discuss the analysis, as a possibility of psychic rebirth. More specifically, the proposition was to analyse the process that involves this phenomenon, in the dialectical movement to idealize and "de-idealize", considering the experience through the Oedipal situation and depressive position, with the report and discussion of two clinical cases. This dissertation was guided by the psychoanalytical method. The psychoanalytic theoretical framework underpinned this study and its analysis, with Melanie Klein, Winnicott and Balint as main authors. In both cases under analysis, it was possible to follow the experience of psychic rebirth. In this program, patients were able to retrieve their experience with the good object and then proceeded and progressed towards the elaboration of the depressive position and Oedipal situation. In the journey of analysis, the patient's need was considered, in the sense of regressing to both dual and idealized relationships with the analyst object; lasting in that more dependent performance, for the time necessary, to meet his/her needs; and thereby ensure a secure and safe environment in which the good object could be retrieved and then trust it. Thus, the relations with the analyst object and, consequently, with the other objects, went from ideal to more integrated and, therefore, the patients at issue could acquire more resources before the task of living and existing / A presente tese tem como objeto de estudo o processo de renascimento psíquico vivido em análise. Pretende-se apresentar e discutir a análise, como possibilidade de renascimento psíquico. Mais especificamente, a proposta foi analisar o processo que envolve esse fenômeno, no movimento dialético de idealizar e desidealizar, considerando a passagem pela situação edípica e posição depressiva, com o relato e discussão de dois casos clínicos. Esta tese se orientou pelo método psicanalítico. O referencial teórico que fundamentou este estudo e sua análise foi psicanalítico, tendo Melanie Klein, Winnicott e Balint como autores basilares. Em ambos os casos objeto de análise, foi possível acompanhar a experiência do renascimento psíquico. Nesse movimento, os pacientes puderam recuperar a experiência com o bom objeto e, então, prosseguiram e progrediram, no sentido da elaboração da posição depressiva e situação edípica. Nesse percurso vivido em análise, considerou-se a necessidade do paciente, no sentido de regredir para relações duais e idealizadas com o objeto analista; permanecer nesse funcionamento mais dependente, pelo tempo necessário, para atender às suas necessidades; e, desse modo, garantir um ambiente seguro em que pudesse resgatar o bom objeto e, então, confiar nele. Assim, as relações com o objeto analista e, consequentemente, com os demais objetos, passaram de ideais para mais integradas e, desse modo, os pacientes em questão puderam adquirir mais recursos diante da tarefa de viver e existir

This study was developed in a palliative care setting. Its purpose is to reflect on psychoanalytic care of children in PC, their families, the health team and on the psychoanalyst. With psychoanalysis as a theoretical, technical and ethical reference, a three case study was developed which provided the opportunity to observe how psychoanalytic care occurs in this special condition. The traumatic potential present in the situational dimension tends to cause changes in the Self of the patients and their relatives, who present a psychic functioning and suffering, similar to the psychopathological dimension, such as narcissistic-identity disorders. Through this process, children and their families lose, even temporarily, some important Ego functions to face the circumstances, such as the capacity for mediation, organization, synthesis and integration of the elements present in the situational dimension. This requires that the analyst assume the deficient Ego functions until the people involved gradually rescue the dormant skills. It is as a mediator that the analyst can be a transformational object and enables the psychoanalytic work, the construction of meaning and the elaboration of experience. It is predominantly a more active care with an additional amount of implied presence, although the analyst has to keep, dialectically, the reserved presence which is essential for self-restraint, self-care and to avoid intrusion caused by the excess of implication / Cuidados paliativos (CP) são uma prática nova no mundo e no Brasil e ainda mais recente com crianças. É neste pano de fundo que se desenvolveu este estudo. Seu objetivo é refletir sobre os cuidados psicanalíticos destinados a crianças em CP e sua família, à equipe de saúde e ao próprio psicanalista. Tendo a psicanálise como referencial teórico, técnico e ético, foi desenvolvido um estudo de três casos, que permitiu observar como o cuidado psicanalítico ocorre nesta condição especial. O potencial traumático presente na dimensão situacional tende a provocar alterações no Eu dos pacientes e seus familiares, que passam a apresentar um funcionamento psíquico e um sofrimento semelhante ao da dimensão psicopatológica, isto é, aos transtornos narcísico-identitários. Nestes momentos, as crianças e suas famílias perdem, ainda que temporariamente, algumas funções egoicas importantes para o enfrentamento das circunstâncias, tais como a capacidade de mediação, de organização, síntese e integração dos elementos presentes na dimensão situacional. Este quadro requer que o analista assuma as funções egoicas deficientes naquele momento até que as pessoas resgatem gradativamente as habilidades perdidas. É como mediador que o analista pode ser um objeto transformacional e possibilitar o trabalho psíquico e a construção de sentido e de elaboração da experiência. Tratase, predominantemente, de um cuidado ativo com um quantum a mais de presença implicada, embora o analista tenha que manter dialeticamente a presença reservada, essencial para a autocontinência e o autocuidado e para evitar a intrusão provocada pelo excesso de implicação

Self-Disclosure by Mexican-American Women: The Effects of Acculturation and Language of Therapy

The present study proposed to investigate the effects of level of acculturation and of language of the therapy interview on self-disclosure by Mexican-American women. It was predicted that self-disclosure would be affected by both level of acculturation and by the language of the initial therapy interview. The principal implication of this finding is that for the first-generation Mexican-American woman, that is, a woman who has not acculturated to the mainstream society, the language in which therapy is conducted constitutes a significant factor in predicting whether she is likely to self disclose and thereby benefit from the therapy. The findings of this study suggest that less acculturated Mexican-American women would be more likely to utilize mental health services if they are available in Spanish.

No. of bitstreams: 0 Previous issue date: 2018-03-23 / O expressivo crescimento do número de terapeutas cognitivo-comportamentais contrasta com a reduzida preocupação sobre os processos de formação mais efetivos e sobre as melhores estratégias de avaliação de competência de terapeutas iniciantes e já experientes. Assim, foram conduzidos três estudos sobre formação e competência em terapias cognitivo-comportamentais. O primeiro teve como objetivo descrever o estado da arte sobre a formação de terapeutas cognitivo-comportamentais. Os temas das publicações, as estratégias formativas e as medidas de competência empregadas em artigos indexados pela PsycNET e publicados entre 2011 e 2016 foram analisados. Observou-se que a produção científica analisada (N=24) pode ser considerada reduzida e que os artigos tratam de cinco temas: Modalidades de Formação, Preparação para Atuação em Quadros Específicos, Preparação para Atuação com Clientes Específicos, Desenvolvimento Pessoal do Terapeuta e Preparação para o Domínio de Técnicas ou Modelos Específicos. As estratégias de formação que têm sido mais utilizadas são formação expandida e supervisão. A avaliação de competência comumente ocorre pelo exame da prática do terapeuta. Como não foram encontradas medidas válidas e fidedignas para esse fim no Brasil e dada a necessidade de se definir como e quais competências serão desenvolvidas durante as formações, o objetivo do segundo estudo foi apresentar o processo de tradução e adaptação da Cognitive Therapy Scale Revised (CTS-R) e a análise de suas propriedades psicométricas. Após obter uma versão brasileira da CTS-R com base em diretrizes internacionais de tradução e adaptação de medidas, terapeutas experientes realizaram avaliações de sessões de terapia gravadas em vídeo com o instrumento e estagiários foram avaliados durante a supervisão clínica por seus supervisores e também se autoavaliaram. Foram obtidas evidências de validade segundo a estrutura interna e evidências de validade baseada nas relações com variáveis externas, assim como estimativas de fidedignidade a partir de consistência interna e de confiabilidade interexaminadores. Como essa medida mostrou-se adequada, foi utilizada para avaliar o desenvolvimento de competências de terapeutas-estagiários. Objetivou-se também analisar a aplicabilidade desse instrumento para autoavaliação. Houve aumento significativo de competência ao longo do treinamento e, tanto na autoavaliação quanto na avaliação por supervisores, os estagiários tenderam a ser considerados competentes desde a primeira coleta. Logo, a versão brasileira da CTS-R parece uma medida válida e fidedigna também para autoavaliação por terapeutas iniciantes. Além de analisar as limitações desta pesquisa, especialmente aquelas referentes à validade externa, são discutidas, como considerações finais da dissertação, as possíveis contribuições dos estudos para o avanço da formação de terapeutas cognitivo-comportamentais no Brasil, ressaltando que ela deve ser baseada em evidência. / The expressive increase in the number of cognitive-behavioral therapists contrasts with the reduced concern about the most effective training processes and the best competency assessment strategies of beginning and experienced therapists. Thus, three studies were conducted on training and competence in cognitive-behavioral therapy. The first one aimed to describe the state of the art on cognitive-behavioral therapists training. The topics of publications, training strategies and measures of competence employed in articles indexed by PsycNET and published between 2011 and 2016 were analyzed. It was observed that the analyzed scientific production (N = 24) can be considered reduced and that the articles deal with five themes: Training Modalities, Training to work in Specific Clinical Conditions, Training to work with Specific Clients, Personal Development of the Therapist and Training for the Domain of Techniques or Specific Models. The training strategies that have been most used are expanded training and supervision. Competency assessment commonly occurs by examining the therapist's practice. Since no valid and reliable measures were found for this purpose in Brazil and given the need to define how and which competences will be developed during the training, the objective of the second study was to present the process of translation and adaptation of the Cognitive Therapy Scale Revised (CTS) -R) and the analysis of their psychometric properties. After obtaining a Brazilian version of the CTS-R based on international guidelines for translation and adaptation of measures, experienced therapists conducted evaluations of video-recorded therapy sessions with the instrument and trainees were evaluated during clinical supervision by their supervisors and also self-assessed. Evidence of validity was obtained according to internal structure and evidence of validity based on relations with external variables, as well as reliability estimates based on internal consistency and inter-rater reliability. As this measure proved to be adequate, it was used to evaluate the development of competences of trainee therapists. The objective of this study was to analyze the applicability of this instrument for self-evaluation. There was a significant increase in competency throughout the training and, in both self-evaluation and evaluation by supervisors, the trainees tended to be considered competent from the first collection. Therefore, the Brazilian version of the CTS-R seems a valid and reliable measure also for self-assessment by beginning therapists. Besides the analysis of the limitations of this research, especially those related to external validity, the possible contributions of the studies to the advance of the training of cognitive-behavioral therapists in Brazil are discussed as final considerations of the dissertation, emphasizing that it should be based on evidence.

Theoretical Orientation, Style, and Compatibility as Factors in Spouse Cotherapy

Cotherapy has been advocated as an effective treatment mode, especially with groups, couples, and families. The relationship between the cotherapists has been identified as an important determinant in the success of this method. This relationship has been compared to the marital relationship between spouses, and the marriages of therapists have been viewed as offering advantages for cotherapy. Since not all therapists who are married to each other work as cotherapists, the purpose of this study was to investigate whether or not spouse cotherapists differ from other therapists who are also married to each other but who do not work together on a regular basis as cotherapists. The five dimensions measured for all subject couples include frequency of differing theoretical orientation, similarity of self-reported behavior in therapy, compatibility of needs for inclusion, compatibility of needs for control, and compatibility of needs for affection. Subjects for this study were 6 5 married couples in which both spouses were psychotherapists. The couples were divided into two groups according to whether or not they worked together as cotherapists on a regular basis. The group of spouse cotherapists included 29 couples who reported regular cotherapy together. The group of therapist couples included 37 couples who reported no regular cotherapy with their spouses. All subject couples were provided a packet of materials which included a letter explaining the general purpose of the study, two copies of the Therapist Personal Data Form, two copies of the Self-Description of Therapist's Behavior, two copies of the Fundamental Interpersonal Relations Orientation-Behavior, and a stamped envelope addressed to the experimenter for the return of the materials.
