A inscrição psicológica do sintoma: uma aproximação psicoanalítica a psicossomática

No. of bitstreams: 1 000029435.pdf: 2722916 bytes, checksum: 5666edc297c2ca8fa6b3cf399dc7dfdd (MD5) / Familiar discourses comprise within themselves the social myths, feelings and unconscious fantasies connected with them, which remain secret and powerful by their historlcity and which are inscribed in the raw material: the neroly-born, who must be molded through the process of identification, through libidinal investments and through a registration of the discourse of impulses (triebe) and their representatives. Through a psychoanalytical approach of psycosomatic symptoms, we have studied the early beginnings of the constitution of the subject of the unconscious, by resorting to the structuring of narcissism and of the unconscious, which is built up as a language in linking significants by contiguity or similarity, by displacements and condensations, i.e., building itself up in a chain as metonimies and metaphors organized in a discourse. Narcisism would be that which structures the corporal ego, the body being the biological support erotogenized on basis of the maternal discourse whose wish anticipates a gestalt of the image, organizing and giving contour to the infant’s sphinctered and anarchical body. In the narcissistic structure are articulated the impulses (triebe), the erotogenic zones, the wishes, and the primary repression, fixing the representations which will from the corporeal ego. The symptom emerging in the acturality of the body would appear as the outcome of a disorganization in the articulation of the significants of the maternal wish, which provide for the internal laws of the discourse, thus hindering the symbolic inscription of the meaningfulness of the limits and frontiers of the infant’s erotogenized body, whose instinctual representative (Triebreprãsentanz) expresses itself as a demand for signification. / Os discursos familiares encerram, no seu bojo, mitos sociais, sentimentos e fantasias inconscientes a eles conectados, que permaneceram secretos e poderosos através de sua historicidade e que vão se inscrever na matéria-prima pura: o recém-nascido, que deve ser moldado mediante o processo de identificação, investimentos libidinais e registro do discurso das pulsões e seus representantes. A partir de uma abordagem psicanalítica dos sintomas psicossomáticos, estudamos os primórdios da constituição do sujeito do inconsciente, recorrendo à estruturação do narcisismo e do inconsciente, construindo-se como uma linguagem ao encadear os significantes por contiguidade ou semelhança, respectivamente, por deslocamentos e condensações, isto é, construindo-se numa cadeia como metonímias e metáforas que se organizam num discurso. O narcisismo seria a causa do corpo. Este último, suporte biológico, que se torna erógeno a partir do discurso materno cujo desejo antecipa uma gestalt à imagem, organizando e delimitando o corpo despedaçado e anárquico do bebê. Como estrutura narcísica relacionamos a articulação das pulsões, das zonas erógenas, do desejo, do recalcamento fixando os representantes que irão constituir um Ego corporal. O sintoma no real do corpo apareceria como efeito de uma desorganização na articulação dos significantes do desejo materno como legalidade de um discurso, dificultando a inscrição simbólica da significação dos limites e fronteiras do corpo erógeno do bebê, cujo representante da pulsão evidencia-se como demanda de significação.

Esta é a hipótese deste trabalho. Portanto, o objeto desta pesquisa trata-se da semiologia médica ocidental. O objetivo principal será rastrear este processo de radicalização, que se inicia com maior ênfase a partir da década de setenta e tenta se consolidar nos anos noventa, sob a denominação de Medicina Baseada em Evidência (MBE). Como objetivo secundário, pretendo traçar aqui um caminho que reinterprete as condições de evidência a partir da prática médica, do sutil a ser desvendado na relação médico-paciente por meio da perspicácia. Metodologicamente, trata-se de um trabalho teórico baseado na concepção de dialética de Maurice MERLEAU-PONTY, que identifica como primordial, no processo de construção de conhecimento, a elimitação clara do campo de presença do sujeito que está à procura deste conhecimento, e, a partir de sua experiência perceptiva, fenomenológica, ir construindo suas categorias, suas contradições, suas mediações. Portanto, será a partir de minha experiência como pessoa e como médica que estarei articulando meu processo de reflexão. Não será apenas minha prática como médica que estará articulando as categorias analíticas deste trabalho. Fez-se necessário um mergulho no ideal de prática semiológica prescrito nos livros de Semiologia básica mais utilizados nas escolas médicas, em especial do Brasil, nas últimas três décadas, até se chegar à produção teórica mais recente da MBE, - que não só aponta o ideal semiológico como o de toda prática médica, que deveria ,contrar se na evidência, produzida nos centros de excelência e documentada periodicamente na literatura médica Aí, encontra-se exatamente o que estou denominando de cilada. Ou seja, ao ancorar-se na evidência referida na literatura médica, a prática aprofunda o abismo, que a tecnologia já impunha, entre o médico e o paciente. Tomo como conclusão deste trabalho a necessidade de se encontrarem caminhos para se livrar de tal cilada. Caminhos que desvendem o sutil e o evidente desta prática / Abstract: The hypothesis treated in this paper is that modem Westem medicine has built itself a trap in the recent radicalization of its own basic semiological pre- suppositions, in the sense of evidence recorded in the medicalliterature. This trap can be overcome on1y through serious reflection on these pre-suppositions. The subject of the study, therefore, is Westem medical semiology. Its main objective is to scrutinize this process of radicalization, which became more intense as ofthe 1970s and made attempts at consolidation in the 1990s, under the name of Medicine Based on Evidence (MBE). A secondary objective is, based on medical practice, to outline a reinterpretation of conditions which would allow more perspicacious detection of evidence by touching upon more subtle aspects underlying the physician-patient relationship. Methodologically, this is a theoretical paper based on Maurice MERLEAUPONTY's conception of dialectic. In the process of constructing knowledge, this author holds that the clear delimitation of. the field of presence of the subject which is seeking said knowledge is primordial. Based its perceptive, phenomenological experience, the subject builds up its categories, contradictions and mediations. I have organized my process of reflection and analysis, therefore, as based on my experience as a person and as a physician, but not solely on this experience. The ana1ytic categories brought up here will also be based on an analysis of the ideal of semiological practice, as prescribed in the books on Basic Semiology most often employed in medical sch901s, especially in Brazil, over the last three decades, inc1uding the more recent theoretical production of MBE, which propounds' the semiological ideal for all of medical practice, to be centered on evidence produced in centers of excellence and periodically documented in the medicalliterature. It is precisely this that I call a trapo By anchoring itself on evidence referred to in the medicalliterature, contemporary medical practice" hâs widened the gap between doctor and patient which technology had already imposed. The conciusion of this paper is that there is need to encounter ways to get out of this trap, paths which will recover all subjectivity of medical practice and anchor in it the sense of evidence. The interest for this object of investigation came out of the volume of diagnosed people who are also medicated and led into specialized mental health and educational services where I practice as a psychologist and in which I daily ask myself: are the times we are living in producing the mental disorders or these are something inherent in human being? That is the context in which I built an allegory using the cinematographic language of Eisenstein, along with a theoretical and methodological support of Foucault to think about the realities that we have been producing. Through the language of cinema, I approach the processes and meanings that create a scenery that resembles the dynamics of our everyday lives. Therefore I created three sequences in which I perform a critical analysis of the speeches of the professionals that work in the areas of psychology and education, as well as a genealogical of the DSM discourse, taking in consideration the history of psychiatry. Esta criação, no entanto, depende de um esforço por parte do médico para não se deixar capturar pelo discurso da Medicina, que o transforma em normatividade e o paciente quase sempre em doença. O objetivo da pesquisa constitui a partir da experiência prática do autor e, de uma reflexão teórica sobre o objeto de trabalho do médico (o corpo vibrátil, subjetivo) encontrar elementos que possam subsidiar a reflexão autopoiética para cada médico repensar em seu contexto a sua forma de criar uma semiologia do vínculo. Do ponto de vista teórico, parte-se da Teoria da Biologia do Conhecimento, de Humberto Maturana e Francisco J.. Varela com suas categorias de autopoiese, acoplamento estrutural e competência ética, aproximando-as da filosofia de Gilles Deleuze e Félix Guattari e das concepções da Etica de Baruch Espinosa. Metodologicamente, trata-se de uma investigação qualitativa centrada na experiência de Primeira pessoa, a partir das propostas de Francisco Verela e Jonathan Shear. Para estes autores, as metodologias de Primeira pessoa não só aceitam a experiência como um domínio a ser explorado tanto em nível filosófico como científico, assim como a consideram marca concreta de nossa existência em ato e ponto privilegiado para transformação das intervenções e práticas profissionais. Os resultados são apresentados através do relato vivo na experiência de Primeira pessoa do autor em contato com seus pacientes, com comentários de interlocutores que possibilitaram a sua posição em Segunda pessoa e através da criação de conceitos que permitem uma reflexão em posição de Terceira pessoa a respeito do objeto de investigação: a relação e suas possibilidades de vínculo / Abstract: The present thesis is based on the hypothesis that it is possible to create a semiology of the bond existing between doctor and patient, a semiology that will allow one and the other to avoid de-subjectivation in that relationship.The establishment of such a semiology, however, depends on an effort by the physician to resist being taken in by medical discourse, which tends to transform doctors into normativity and patients into disease or disorder. The study is based on practical experience and on a discussion of the doctor's object of work, the vibratile subjective body. The study's objective, on the other hand, is to pinpoint aspects that may provide more solid grounds for a self-poietic discussion by each individual physician concerning a semiology of the bond. Theoretically, the study is based on the Knowledge of Biology Theory, by HumbertoMaturana and Francisco J. Varela. These authors postulate the categories of self-poiesis,structural coupling, and ethical competence,approximating them to the philosophy of Gilles Deleuze and Felix Guattari and to conceptions found in Baruch Spinosa's Ethics. Methodologically, the study is a qualitative investigation focused on First- person experience and based on the proposals of Francisco Varela and Jonathan Shear, for whom First-person methodologies not only accept experience as a domain to be explored in both philosophical and scientific terms, but also consider it a concrete aspect of our existence in act and an exceptional starting point for modifying professional interventionsand practices. A variabilidade da frequência cardíaca (VFC) vem sendo utilizada para realizar a monitorização e prognóstico de mortalidade, contudo será que índices de VFC podem ser utilizados para predizer sinais e sintomas em indivíduos que frequentam PRC? Se sim, quais os índices que melhor se correlacionam com a possibilidade de sinais e sintomas nesses programas? Será possível, estabelecer um ponto de corte desses índices que possa melhor prever as chances do aparecimento de sinais e sintomas nesses programas? Objetivo: Analisar a correlação entre os índices da VFC mensurados em repouso com a presença de sinais/sintomas em participantes de um PRC e determinar pontos de corte dos índices lineares da VFC, utilizando a curva ROC, para o surgimento de sinais e sintomas. Materiais e métodos: Foram analisados dados de 48 pacientes atendidos em um PRC. Foram realizadas avaliações em repouso da modulação autonômica e posteriormente, estes indivíduos foram acompanhados por um período de três meses durante as sessões do PRC para avaliação de sinais e sintomas. (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Cardiovascular rehabilitation programs (CRP) may promote control of cardiovascular risk factors and reduce the incidence of new coronary events, but physical exercise may be associated with the risk of signs and symptoms arising during its development. Heart rate variability (HRV) has been used to perform mortality monitoring and prognosis, but can HRV rates be used to predict intercurrences in individuals who attend PRC? If so, which indexes best correlate with the possibility of intercurrences in these programs? Is it possible to establish a cutoff point for these indices that can better predict the chances of the occurrence of intercurrences in these programs? Objective: To analyze the correlation between the HRV indices measured at rest with the presence of signs/symptoms in participants of a PRC and to determine cutoff points of HRV linear indices, using the ROC curve, for the occurrence of intercurrences. Materials and methods: Data from 48 patients seen in a CRP were analyzed. Resting evaluations of autonomic modulation were performed and, later, these individuals were followed up for a period of three months during the PRC sessions to assess intercurrences. Sujeitos e Métodos: Neste estudo de corte transversal foram incluídas 199 mulheres com massa anexial (67 com tumores malignos e 132 com tumores benignos) e 150 mulheres saudáveis. Todas as mulheres com massa anexial, atendidas no hospital do Departamento de Obstetrícia e Ginecologia da Faculdade de Medicina da UNICAMP, foram convidadas a participar do estudo. Um grupo-controle, de mulheres saudáveis atendidas nos ambulatórios de menopausa e planejamento familiar no mesmo hospital, foi selecionado. Após uma explicação sobre os métodos e objetivo da pesquisa, todas as mulheres responderam o questionário com relação aos sintomas específicos. Foram coletados dados sobre a idade e índice de massa corpórea e sangue periférico para quantificação da mesotelina, o CA125 e a HE4. Foi usado o algoritmo de particionamento recursivo baseado no modelo de regressão linear para verificar a contribuição da idade e de cada marcador sérico no diagnóstico de tumores malignos. Foram comparadas as áreas sob as curvas (AUCs) obtidas através das curvas ROC (Receiver Operator Characteristics) de cada marcador sérico e índice ROMA, para diferenciar mulheres com tumores malignos. Foi calculada a proporção de mulheres com cada um dos 22 sintomas específicos nos grupos com tumores malignos de ovário, tumores benignos e mulheres saudáveis. O sintoma foi considerado positivo quando ocorria mais que 12 vezes ao mês e por até um ano. A proporção de sintomas foi comparada utilizando teste de qui-quadrado ou teste exato de Fischer, quando apropriado. Os 16 sintomas específicos aplicáveis a toda a coorte e para o qual a periodicidade foi verificada foram submetidos à análise pelo Método de Ward para agrupamento hierárquico. Os agrupamentos de sintomas e sintomas isolados identificados foram: abdômen (abdômen inchado e/ou aumento do volume abdominal); dor (dor pélvica, costas e/ou abdominal); pernas inchadas; digestão (estômago cheio e/ou náusea /vômito); alimentação (dificuldade para comer e/ou empachada); sente alguma massa abdominal; diversos (fadiga e/ou dificuldade para respirar); bexiga (urgência em urinar e/ou urinar frequentemente). Foi avaliada a proporção de mulheres com cada agrupamento de sintomas ou sintomas isolados em mulheres com tumores malignos, tumores benignos e saudáveis, através do teste qui-quadrado para tendências. Utilizou-se um algoritmo de particionamento recursivo para verificar a contribuição da idade da mulher, de cada agrupamento de sintomas ou sintomas isolados, estado menopausal, perda de peso e marcadores séricos no diagnóstico de tumores malignos. Resultados: O CA125 foi o marcador sérico com maior capacidade para discriminar mulheres com tumores malignos (p<0,001). Entre as mulheres com tumores benignos e CA125 positivo, a HE4 foi positiva em apenas um caso e a mesotelina foi positiva em outro. Em mulheres com CA125 negativo, a idade, a mesotelina e a HE4 não contribuíram para a diferenciação entre mulheres com tumores malignos, tumores benignos e saudáveis. Em contrapartida, em mulheres com CA125 positivo, a HE4 contribuiu significantemente para detecção de mulheres com tumores malignos (p<0,01). A AUC da mesotelina foi menor que das AUC dos outros marcadores. O ROMA e o CA125 apresentarm melhores AUCs do que o HE4. A proporção de mulheres com cada um dos agrupamentos de sintomas ou sintomas isolados foi significativamente maior em mulheres com tumores malignos, quando comparadas àquelas com tumores benignos e, destas, comparadas com as mulheres saudáveis (p tendência em todas as comparações <0,01). Após a análise multivarida, as associações mais significativas para detecção de tumores malignos de ovário foram as do agrupamento abdômen (p<0,001), expressão do CA125 (p<0,001), agrupamento dor (p=0,01) e perda de peso (p=0,03). Conclusões: Em mulheres com CA125 negativo, a mesotelina e HE4 não contribuíram para detecção do carcinoma de ovário. Entretanto, em mulheres com CA125 positivo, a HE4 contribuiu para diferenciar aquelas com tumores malignos. Em mulheres com tumores malignos de ovário, os sintomas específicos, abdômen e dor foram significantemente mais frequentes. Podem ser utilizados em associação ao CA125 na diferenciação de tumores malignos em mulheres com massa anexial / Abstract: Objective: To evaluate the accuracy of mesothelin, CA125, HE4 and ROMA index in the differentiation of Brazilian women with ovarian malignant tumors from those with benign tumors or healthy women; and to evaluate whether the prevalence of specific self-reported symptoms can be used in association to the expression of serum markers for the preoperative differentiation of ovarian malignant tumors. Study Design: For this cross sectional study, 199 women with adnexal mass (67 with malignant tumors and 132 with benign tumors) and 150 healthy women were included. All women with adnexal masses, attending the hospital of the Department of Obstetrics and Gynecology of the Unicamp School of Medicine were invited to participate in the study. A control group of healthy women attending menopause and family planning clinics at the same hospital were selected. After an explanation about the study research methods and purpose all women answered a survey regarding specific symptoms. There were also collected data on age and body mass index. Peripheral blood was collected for serum measurements of mesotelina, CA125 and HE4. A recursive partitioning algorithm, based on a linear regression model was used to confirm the contribution of age and each of the serum markers to the diagnosis of malignant tumors. Comparison of Area Under the Curve (AUC) obtained through Receiver Operator Characteristics (ROC) curves for each of the serum markers and ROMA index were used to differentiating women with malignant tumors. We next calculated the proportion of women with each of the 22 specific symptoms in the groups of women with ovarian malignant tumors, benign tumors and healthy women. We considered a symptom positive if it occurred more than 12 times per month and for less than one year. The proportions were pairwise compared using chi-square or the Fisher exact test where appropriate. The 16 specific symptoms which applied to the entire cohort and for which the periodicity had been ascertained were further subjected to the Ward's Hierarchical Clustering Method. Clusters of symptoms and isolated symptoms were: abdomen (abdominal bloating and/or increased abdomen size); pain (pelvic, back and/or abdominal pain); leg swelling; digestion (indigestion and/or nauseas /vomiting); eating (unable to eat normally and/or feeling full quickly); able to feel abdominal mass; miscellaneous (fatigue and/or difficulty breathing); bladder (urinary urgency and/or frequent urination). We evaluated the trend in proportion of women with each cluster of symptoms in the groups of women with malignant tumors, benign tumors and healthy women using the chi-squared test for trend in proportions. Another recursive partitioning algorithm was used to confirm the contribution of patient age, clusters of symptoms, menopausal status, weight loss and the serum markers to the diagnosis of malignant tumors Results: CA125 was the serum marker that had the greatest capacity to discriminate women with malignant tumors (p<0.001). Among the women with benign tumors and positive CA125, HE4 was positive in only one case and mesothelin in another case. In women with negative CA125 neither age nor mesothelin nor HE4 contributed any further to the differentiation between women with malignant, tumors benign tumors and healthy women. In contrast, for women with positive CA125, HE4 contributed significantly to the detection of women with malignant tumors (p<0.01). The AUC for mesothelin was smaller than that for all the other curves, and ROMA and CA125 had better AUC than HE4. The proportion of women with each of the clusters of symptoms and isolated symptoms decreased significantly from the group of women with malignant tumors to that with benign tumors and from this group to the healthy women (p for trends in all comparisons= <0.01). After a multivariate analysis the association that contributed the most to the detection of malignant ovarian tumors was that of the abdomen cluster (p<0.001), CA125 expression (p<0.001), pain cluster (p=0.01) and weight loss (p=0.03). Conclusion: In women with negative CA125 neither mesothelin nor HE4 contributed to detect ovarian carcinoma. HE4 was helpful to differentiate malignant tumors when CA125 is positive. Avaliação da toxicidade do inseticida novaluron em Bombyx mori (Lepidoptera Bombycidae) /

Orientador: Daniela Carvalho dos Santos / Resumo: O bicho-da-seda, Bombyx mori (Lepidoptera: Bombycidae), é o inseto de maior importância econômica na produção de seda. A lagarta se alimenta de folhas de amoreira e é altamente sensível a agrotóxicos, assim o uso destes em culturas agrícolas circunvizinhas às plantações de amoreira pode afetar o desenvolvimento de B. mori, acarretando em desequilíbrio nas suas funções metabólicas e, consequentemente, comprometendo a produção de casulos. Entre estes agrotóxicos, destaca-se o Novaluron, inseticida inibidor da síntese de quitina nos insetos e empregado no controle de insetos pragas de culturas agrícolas próximas as plantações de amoreira. Assim, investigamos os efeitos tóxicos de Novaluron no desenvolvimento de B. mori. Lagartas de B. mori, foram separadas em dois grupos experimentais: grupo controle (GC) e grupo tratamento (GT: tratado com 0, 15 mL/L de Novaluron). Após ecdise do 2° para o 3° instar, lagartas do GT foram alimentadas por 24 horas com folhas de amoreira tratadas com o inseticida. Paralelamente foi realizada uma nova exposição, porém em lagartas que realizavam a ecdise do 4° para o 5° instar. Lagartas, pupas e adultos de B. mori foram anestesiadas e segmentos do intestino médio, glândula da seda e órgãos reprodutores retirados e processados convencionalmente para técnicas de microscopias de luz, eletrônica e imunohistoquímica. The larvae feeds on mulberry leaves and is highly sensitive to agrochemicals, thus the use of these in agricultural crops surrounding the mulberry plantations can affect the development of B. mori, causing an imbalance in its metabolic functions and, consequently, compromising the production of cocoons. Among these agrochemicals, stands out the Novaluron, an insecticide inhibitor of the synthesis of chitin in insects and used in the control of insect pests of crops near mulberry plantations. Thus, we investigated the lethal and sublethal effects of Novaluron on the development of B. mori. Larvae were selected into two experimental groups: control group (CG) and treatment group (TG: treated with 0, 15 mL/L Novaluron). After ecdysis from the 2nd to the 3rd instar, the TG larvae were fed for 24 hours with mulberry leaves treated with the insecticide. Estudo retrospectivo de casos de leishmaniose visceral canina atendidos em um hospital veterinário de uma área endêmica para a doença /

Orientador: Mary Marcondes / Resumo: Os objetivos do presente estudo foram verificar, em uma população de cães com leishmaniose visceral, atendida em um hospital veterinário de área endêmica para a doença no Brasil, as principais alterações clínicas observadas, os métodos utilizados para confirmar o diagnóstico, o tempo decorrido entre o início dos sintomas e o diagnóstico e a utilização de métodos de prevenção da doença. Os prontuários dos cães atendidos durante o período do estudo foram avaliados afim de selecionar àqueles que possuíam diagnóstico da doença. Na população estudada machos e cães de raças definidas apresentaram uma maior predisposição ao desenvolvimento da doença. Dentre as alterações clínicas mais frequentemente observadas destacam-se linfoadenomegalia, lesões dermatológicas, alterações gastrointestinais, anorexia ou hiporexia, perda de peso e alterações oftálmicas. Mais de 80% dos cães apresentava hiperproteinemia. Em 59,52% dos animais o diagnóstico foi confirmado apenas por meio de exame parasitológico direto, sendo em 56,34% por citologia de punção biopsia aspirativa de linfonodos. O tempo de evolução da doença no momento da primeira consulta no hospital veterinário variou de uma semana a mais de um ano, com 42,86% dos animais apresentando sintomas entre um e 12 meses. Cerca de metade dos cães já havia recebido algum tipo de tratamento para os sintomas, sem diagnóstico de leishmaniose visceral canina. (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: The aim of the present study was to verify, in a population of dogs with visceral leishmaniosis referred to a veterinary teaching hospital in an area endemic for the disease in Brazil, the main clinical signs observed, the time elapsed between onset of symptoms and diagnosis, the methods used to confirm the diagnosis, and the use of preventive methods. The medical records of the dogs attented during the study period were evaluated in order to select those who had a diagnosis of the disease.In the studied population, males and pure breed dogs showed a greater predisposition to the development of the disease. Among the most frequently observed clinical signs are lymphadenomegaly, anorexia or hyporexia, weight loss, dermatological, gastrointestinal and ophthalmic alterations. More than 80% of the dogs had hyperproteinemia. In 59.52% of the animals the diagnosis was confirmed only by means of direct parasitological examination, and in 56.34% by cytology of lymph nodes. Symptoms lasted from one week to more than one year at the moment of the consultation at the veterinary hospital, with 42.86% of the dogs presenting symptoms between one and 12 months. Objetivos: Avaliar a prevalência, variabilidade e repercussões dos sintomas em tabagistas, ex- tabagistas e pacientes com DPOC. Métodos: Foram avaliados 108 indivíduos de ambos os sexos com idade ≥ 40 anos (tabagista e ex-tabagistas: 32; DPOC leve/moderado: 36; DPOC grave/muito grave: 40) provenientes dos ambulatórios de Pneumologia do Hospital das Clínicas da Faculdade de Medicina de Botucatu. Todos foram submetidos à avaliação clínica, responderam a questionário elaborado para o estudo para avaliação dos sintomas, a variabilidade dos mesmos durante o dia e semana durante períodos de estabilidade da doença.
