Prevalência e extensão da perda de inserção clínica entre trabalhadores adultos das áreas rural e urbana / Prevalence and extention of clinical attachment loss between adult workers of rural and urban areas

Introduction: Clinical attachment loss is a consequence due to periodontal disease (PD) and is characterized by irreversible damage to dental support tissues, and, if not treated, might lead to dental loss. The clinical parameter currently considered as the most adequate to measure the amount of bone loss on dental sites is the clinical attachment level (CAL), obtained by the sum of probing depth (PD) and gingival margin level (GM). This parameter is still little explored in epidemiologic studies, especially in underdeveloped countries. A wide variation on both prevalence and extension of clinical attachment loss is verified in different populations, which can be explained by different exposures to risk factors. Despite Brazil's periodontal profile having shown significant improvement in the last years, the disparity between rural and urban populations is still notorious. Add to this fact the lack of epidemiologic research that portray the heterogeneity of each population group. To know the prevalence and extension of clinical attachment loss and specific risk factors is essential to build effective preventive measures to lower both occurrence and severity of PD. Objective: This study aims to compare CAL dimension (prevalence and extension) among adult workers from rural and urban areas, as well as describe the study sample and determine CAL profiles. Materials and methods: This is a transversal observational study, that clinically evaluates subjects of urban and rural areas. Data collection was conducted through form application, periodontal screening and full mouth clinical exam station. Results: 331 volunteers were examined, 113 from urban area (UA) and 218 from rural area (RA), the average age in years of those examined in the UA was of 36,1±6,2, while in the RA was of 42,8±6,6. The average number of teeth present in those in the UA was of 23,8±4,2 and of 20,7±3,4 in the RA. Clinical dental mobility was detected in 1,3±5,8 per cent on teeth in the UA volunteers, and in 3,8±10,1 in those in RA. The clinically visible plaque ratio was detected in 31,1±23,1 per cent of dental features in UA volunteers, and in 61,4±25,2 per cent in volunteers from RA. Bleeding on probing was detected in 22,5±15,7 per cent on screened dental sites in the volunteers from UA, and 49,5±24,2 per cent on volunteers from RA. The percentage of CAL prevalence and extension, stratified by age, gender, per capita income, educational level and smoking habits present in the RA were higher or equal when compared to those present in the UA. Conclusion: The results demonstrate high CAL prevalence and extension in the general sample, with the observation that the levels were significantly higher in those who live in the rural area when compared to those of the urban area. The highest CAL prevalence and extension were associated with the rural area, with emphasis in young adults, even when stratified by: age, gender, per capita income, educational level and smoking habits. / Introdução: A perda de inserção clínica é uma sequela da doença periodontal (DP) sendo caracterizada por lesão irreversível aos tecidos de suporte dental, quando não tratada pode evoluir para a perda dentária. O parâmetro clínico atualmente considerado como mais adequado para medir a quantidade de perda óssea nos sítios dentários é o nível de inserção clínica (NIC), obtido pela soma da profundidade de sondagem (PS) com alteração de margem gengival (MG). Parâmetro este, ainda pouco explorado nos estudos epidemiológicos principalmente nos países subdesenvolvidos. Uma ampla variação na prevalência e extensão da perda de inserção clínica é verificada em diferentes populações, isto é explicado por diferentes exposições à fatores de risco. Apesar do perfil periodontal no Brasil ter apresentado significante melhora nos últimos anos, ainda é notória a disparidade entre populações rurais e urbanas. Soma-se a este fato a falta de levantamentos epidemiológicos que retratem a heterogeneidade de cada grupo populacional. O conhecimento da prevalência e de extensão da perda de inserção clínica e de fatores de risco específicos são essenciais para construção de ações preventivas efetivas na diminuição da ocorrência e severidade da DP. Objetivo: O estudo objetiva comparar as medidas de NIC (prevalência e extensão) entre os trabalhadores adultos jovens das áreas rural e urbana, descrever a amostra do estudo e determinar perfis de NIC. Materiais e métodos: O estudo é observacional do tipo transversal, com avaliação clínica de voluntários da área urbana e rural. A coleta de dados foi feita através da aplicação de formulário, exame de triagem periodontal e estação de exame clínico periodontal de boca completa. Resultados: 331 voluntários foram examinados, 113 da área urbana (AU) e 218 da área rural (AR), a média de idade dos examinados da AU foi de 36,1±6,2, enquanto que na AR foi de 42,8±6,6. A média de número de dentes presentes foi 23,8±4,2 na AU e 20,7±3,4 para AR. Mobilidade dentária clínica foi verificada em 1,3±5,8 por cento dos dentes nos voluntários da AU e em 3,8±10,1 nos voluntários da AR. O índice de placa visível clinicamente foi detectado em 31,1±23,1 por cento das faces dentárias nos voluntários da AU enquanto que foi verificada 61,4±25,2 por cento das faces dentárias nos voluntários da AR. O sangramento à sondagem foi detectado em 22,5±15,7 por cento dos sítios sondados nos voluntários da AU e em 49,5±24,2 por cento dos sítios sondados nos voluntários da AR. Os percentuais de prevalência e extensão de NIC estratificados por faixa etária, gênero, renda per capita, nível educacional e tabagismo da AR foram maiores ou iguais quando comparados aos da AU. Conclusão: Os resultados demonstram alta prevalência e extensão de NIC na amostra geral, sendo que nos moradores das áreas rurais estes níveis foram significativamente maiores se comparados aos urbanos. A maior prevalência e extensão de NIC foi associada a área rural, com mais ênfase em adultos jovens, mesmo quando estratificado por: faixa etária, gênero, renda per capita, nível educacional e tabagismo. / Lagarto, SE

O objetivo deste estudo foi estimar prevalência dos transtornos mentais comuns (TMC) e bebedores problemas (BP) e também a utilização de serviços, a partir de estudo seccional, populacional com amostragem aleatória. Incluiu-se sujeitos com 16 anos e mais, avaliando-se aspectos sócio-demográficos, uso de álcool, saúde mental e utilização de serviços. A prevalência de TMC foi 21,7%, de BP foi 4,4%. Na ultima quinzena 14,6% procuraram serviço de saúde. As prevalências observadas confirmam achados da literatura: TMC e BP são importantes problemas de saúde pública, com implicações para a organização de serviços e formação de recursos humanos / The aim of this cross-sectional study was to estimate the prevalence of common mental disorders (CMD) and problem drinking (PD), as well as health services use in an unselected populational sample. Subjects over 16 years were included and aspects related to socio-demographic characteristics, alcohol use, mental health and services use were studied. The prevalence of CMD was 21.7%, of PD 4.4%. In the two weeks preceeding the interview, 14.6% had used a health service. The results are in accordance with the literature: CMD and PD are important health problems, with implications for health services organization and formation of health professionals

Making nutritious food more accessible in an urban society : The potential of community creation and citizen engagement in food-related social innovation projects

Individual food choices have a considerable impact on public health and the environment. The incidence of nutrition-related diseases has been continuously increasing during the last decades. Diets higher in plant-based foods have shown to be healthier and more environmentally sustainable.Many urban social innovation projects based on citizen engagement and community building are able to influence food consumption habits. However, there is a large variety of projects, which target specific population groups and focus on different sustainability issues. To understand how social innovation projects located in Germany, and especially Berlin, use citizen engagement and community creation to encourage nutritious and sustainable food choices, the research team conducted a qualitative research study consisting of semi-structured interviews and participant observations. The results were compared and validated using different social change, social innovation, network and leadership theories. When people with various backgrounds come together over a shared purpose and interact with each other, they can influence each other’s opinions and behaviors. A physical community setting was shown to be efficient to create awareness about sustainable and nutritious food choices. In order to include all population groups, citizens need to feel heard and understood, and have to be directly engaged in the decision-making process. A need for a leadership figure or a democratic leadership group is highlighted, and a combination of ‘transformational leadership’ and ‘authentic leadership’ styles proved to be the most effective. Urban food social innovation projects tend to cooperate with each other, as well as with other leaders and organizations, influencing them and serving as role models. These findings can help social innovation projects that implement citizen engagement and community building practices to efficiently promote nutritious and plant-based food choices among an urban population. Since this study is based on a limited number of organizations in Berlin and other regions of Germany, further research in other urban and/or rural environments is needed.

Geographical Location and Stage of Breast Cancer Diagnosis: A Systematic Review of the Literature

Objective: To examine systematically the literature on the effect of geographical location variation on breast cancer stage at diagnosis, race/ethnicity, and socioeconomic status. Methods. Eight electronic databases were searched using combination of key words. Of the 312 articles retrieved from the search, 36 studies from 12 countries were considered eligible for inclusion. Results. This review identified 17 (47%) of 36 studies in which breast cancer patients residing in geographically remote/rural areas had more late-stage diagnosis than urban women. Ten (28%) studies reported higher proportions of women diagnosed with breast cancer resided in urban than rural counties. Nine (25%) studies reported no statistically significant association between place of residence and stage at diagnosis for breast cancer patients residing in rural and urban areas. Conclusions. Cancer patients residing in rural and disadvantaged areas were more likely to be diagnosed with distant breast metastasis. Efforts to reduce these inequalities and subsequent mortality are needed.

Embora os Pankararu sejam originários da região do submédio São Francisco, no Estado de Pernambuco, depois de um intenso processo de migração, muitos deles passaram a residir na Favela do Real Parque, em São Paulo. Este estudo se concentra nos Pankararu do Real Parque. Este trabalho descreve partes da cosmologia Pankararu e suas formas tradicionais de cura, na busca de compreender a integração dessas práticas com o atendimento biomédico do Município de São Paulo. Descrevo então como ocorre essa integração, assinalando os conflitos e os processos de “tradução” e ressignificação de seus elementos rituais para o contexto urbano. O crescente número de etnias que vivem na metrópole suscita indagações quanto à transformação e readaptação de seus discursos. Esta etnografia pretende explicitar como é realizado esse movimento e as formas de afirmação identitária e de conquistas políticas, principalmente, no campo da saúde. / This essay seeks to approach the Pankararu concepts of healing and their relations with Biomedicine. Although Pankararu originate from the region of São Francisco submid, in the State of Pernambuco, after an intense process of migration, many ofthem took up residence in the shantytown of Real Parque, in São Paulo city. This study focuses on the Pankararu of Real Parque. This paper describes parts of the Pankararu cosmology and their traditional ways ofhealing, seeking to understand the integration of these practices with biomedical care in São Paulo city. I describe,then, how this integration occurs, pointing out the conflictsand the processes of "translation" and their redefinition of ritual elements to the urban context. The growing number of ethnic groups living in the metropolis raises questions aboutthe transformation and upgrading of their speech es. Environnement alimentaire local et son association avec les habitudes alimentaires de personnes âgées

Contexte : Un accès adéquat aux aliments sains dans les environnements résidentiels peut contribuer aux saines habitudes alimentaires. Un tel accès est d’autant plus important pour les personnes âgées, où les changements associés au vieillissement peuvent accentuer leur dépendance aux ressources disponibles dans le voisinage. Cependant, cette relation n’a pas encore été établie chez les aînés. Objectifs : La présente thèse vise à quantifier les associations entre l’environnement alimentaire local et les habitudes alimentaires de personnes âgées vivant à domicile en milieu urbain. La thèse s’est insérée dans un projet plus large qui a apparié les données provenant d’une cohorte d’aînés québécois vivant dans la région métropolitaine montréalaise avec des données provenant d’un système d’information géographique. Trois études répondent aux objectifs spécifiques suivants : (1) développer des indices relatifs de mixité alimentaire pour qualifier l’offre d’aliments sains dans les magasins d’alimentation et l’offre de restaurants situés dans les quartiers faisant partie du territoire à l’étude et en examiner la validité; (2) quantifier les associations entre la disponibilité relative de magasins d’alimentation et de restaurants près du domicile et les habitudes alimentaires des aînés; (3) examiner l’influence des connaissances subjectives en nutrition dans la relation entre l’environnement alimentaire près du domicile et les habitudes alimentaires chez les hommes et les femmes âgés. Méthodes : Le devis consiste en une analyse secondaire de données transversales provenant de trois sources : les données du cycle 1 pour 848 participants de l’Étude longitudinale québécoise « La nutrition comme déterminant d’un vieillissement réussi » (2003-2008), le Recensement de 2001 de Statistique Canada et un registre privé de commerces et services (2005), ces derniers regroupés dans un système d’information géographique nommé Mégaphone. Des analyses bivariées non paramétriques ont été appliquées pour répondre à l’objectif 1. Les associations entre l’exposition aux commerces alimentaires dans le voisinage et les habitudes alimentaires (objectif 2), ainsi que l’influence des connaissances subjectives en nutrition dans cette relation (objectif 3), ont été vérifiées au moyen d’analyses de régression linéaires. Résultats : Les analyses ont révélé trois résultats importants. Premièrement, l’utilisation d’indices relatifs pour caractériser l’offre alimentaire s’avère pertinente pour l’étude des habitudes alimentaires, plus particulièrement pour l’offre de restaurants-minute. Deuxièmement, l’omniprésence d’aspects défavorables dans l’environnement, caractérisé par une offre relativement plus élevée de restaurants-minute, semble nuire davantage aux saines habitudes alimentaires que la présence d’opportunités d’achats d’aliments sains dans les magasins d’alimentation. Troisièmement, un environnement alimentaire plus favorable aux saines habitudes pourrait réduire les écarts quant à la qualité de l’alimentation chez les femmes ayant de plus faibles connaissances subjectives en nutrition par rapport aux femmes mieux informées. Conclusion : Ces résultats mettent en relief la complexité des liens entre l’environnement local et l’alimentation. The thesis is part of a larger project involving the linkage of data from a cohort of Québec seniors living in the Montréal metropolitan area and data from a geographic information system. Three studies addressed the following specific objectives: (1) to develop relative indices of local-area food sources outlets to qualify stores potentially selling healthful foods and supply of restaurants in neighborhoods that were part of the study area, (2) to quantify associations between the relative availability of food stores and restaurants in residential area and dietary patterns of members of the cohort, (3) to examine the influence of subjective nutrition knowledge in the relationship between the residential food environment and dietary patterns among older men and women. Methods: Cross-sectional analysis of data from three different sources was performed: (1) person-level data on 848 participants from cycle 1 of the Québec Longitudinal Study on Nutrition and Successful Aging , (2) 2001 Census data from Statistics Canada and (3) data from private businesses and services registry (2005), these two gathered in a geographic information system called Megaphone. Nonparametric bivariate analyses were applied to address objective 1. Associations between exposure to residential-area food sources and dietary patterns (objective 2), as well as moderating effect of nutrition knowledge (objective 3), were tested using linear regression analyses. Results: Analyses revealed three important results. First, the use of relative indices to characterize availability of local-area food sources is relevant to the study of dietary patterns, particularly regarding the supply of fast food restaurants. Second, the ubiquity of unfavorable aspects in the food environment, characterized by relatively higher fast food restaurants offer seem more detrimental to healthful eating habits that the presence of opportunities to buy healthful foods in food stores. Third, a residential food environment more favorable to healthful dietary patterns could reduce disparities in diet quality between women with low nutrition knowledge compared to women more knowledgeable. Conclusion: These results highlight the complex links between local environment and diet. If findings can be replicated in future research, population-based strategies to address an imbalance between accessibility to healthful food sources relative to unhealthful food sources, would be promising.

Condições de vida, acesso e utilização dos serviços de saúde no município de Camaquã, RS : contribuições ao estudo dos determinantes sociais de saúde

O presente estudo insere-se no projeto de pesquisa denominado “Determinantes sociais e interfaces com a mobilidade de usuários: análise dos fluxos e utilização de serviços de saúde” e trata-se de um estudo sobre os determinantes sociais do acesso e utilização dos serviços de saúde no município de Camaquã/RS. Tem como objetivo caracterizar as condições de vida da população em estudo, identificar e analisar as diferenças no acesso e utilização dos serviços de saúde entre os diferentes grupos sociais do Município e a influência das redes de apoio e do apoio social na utilização desses serviços. Trata-se de um estudo epidemiológico de base populacional, com desenho seccional. Foi composta uma amostra probabilística de 256 famílias, totalizando 860 indivíduos. Os dados foram coletados entre abril e julho de 2008 por inquérito domiciliar, por meio de um instrumento contendo questões demográficas, de condições de vida, sobre as redes de apoio e o apoio social, morbidades referidas, acesso e utilização dos serviços de saúde. A estatística descritiva e a análise bivariada foram empregadas na avaliação do acesso e utilização dos serviços de saúde, utilizando-se o teste quiquadrado, para avaliar a significância estatística nos cruzamentos estabelecidos com variáveis demográficas e de condições de vida. Utilizou-se ainda, regressão logística para investigar a magnitude da associação entre as diversas dimensões de apoio social e as variáveis: procurou serviços de saúde e restrição de atividades habituais. De modo geral, este estudo revelou que os moradores de Camaquã têm acesso aos serviços de saúde da rede municipal quando procuram por atendimento, observando-se que as diferenças na utilização dos serviços de saúde são reflexo das diferenças de comportamento dos indivíduos frente às doenças, onde o ato de procurar atendimento passa pelas escolhas individuais de cada usuário, indo além das características de organização da oferta ou disponibilidade de serviços. Nesse contexto, o estudo das redes de apoio e do apoio social aparece como instrumento capaz de dar voz à complexidade de fatores que interferem no modo de vida e saúde dos indivíduos, sugerindo a importância desses como determinantes sociais do acesso e utilização dos serviços de saúde. Evidencia-se, portanto, a necessidade de se aprofundar o conhecimento sobre as dinâmicas sociais decorrentes das diferenças entre os seres humanos e suas implicações no acesso e utilização dos serviços de saúde, sendo necessário para isto uma visão ampliada sobre o tema, que permita a elaboração de políticas de saúde adequadas ao contexto socioeconômico, político e cultural local. / The present study is inserted in a research project named “Social determinants and interfaces of users’ mobility: analysis of fluxes and the use of health services”. It is a study on social determinants of the access and use of health services in the city of Camaquã-RS. Its main objective is to characterize the life conditions of the mentioned population, to identify and analyze the access and use of health services among different social groups in Camaquã, as well as the influence of social support nets in the use of such services. It is an epidemiological study of population basis with sectional design. It was set a probabilistic sample of 256 families, totalizing 860 subjects. Data were collect between April and July 2008 by postal survey by the means of an instrument containing demographic and living conditions issues, about the networks of social support, referred morbidity, access and the use of health services. Descriptive statistics and bivariate analysis were applied in the assessment of the access and use of the health services, using qui-squared test in order to assess the statistic significance in crossing demographic variables and living conditions. It was also used logistic regression to investigate the magnitude of the association between the various dimensions of social support and service variables such as: sought for health service and normal activities restriction. In general terms, this study revealed that the citizens of Camaquã have access to health services from the municipal network when they look for it. It was also observed that the differences in the use of health services are the reflection of inequalities in the subjects’ behavior concerning the diseases, where the act of looking for the service pass through each user’s choices going beyond the characteristics of services’ supply or availability. In that context, the study of support nets and of social support appears as an instrument able to give voice to the complexity of factors which interfere in the life style and health of the individuals, suggesting their importance as social determinants of the access and use of health services. It was observed, though, the need of deepening the social dynamic knowledge coming from the differences between human beings and their implications for access and use of the health services, being necessary to do that an expanded vision on the theme, which allows the elaboration of health policies appropriate for each socioeconomic, political and cultural local context. / El presente estudio se insiere en el proyecto de pesquisa denominado “Determinantes sociales e interfaces con la movilidad de usuarios: análisis de los flujos y utilización de servicios de salud” y se trata de un estudio sobre los determinantes sociales del acceso y utilización de los servicios de salud en el municipio de Camaquã/RS. Tiene como objetivo caracterizar las condiciones de vida de la populación en estudio, identificar y analizar el acceso y la utilización de los servicios de salud entre los diferentes grupos sociales del Municipio y la influencia de las redes de apoyo y del apoyo social en la utilización de esos servicios. Se trata de un estudio epidemiológico descriptivo de base de populación, con diseño seccional, que utilizó colecta y análisis cuantitativo de los datos. Fue compuesta una amuestra probabilística de 256 familias, totalizando 860 individuos. Los datos fueron colectados entre abril y julio de 2008 por averiguación domiciliar, por medio de un instrumento conteniendo cuestiones demográficas, de condiciones de vida, sobre las redes de apoyo y el apoyo social, morbideces referidas, acceso y utilización de los servicios de salud. La estadística descriptiva y el análisis bivariada fueron empleadas en la evaluación del acceso y utilización de los servicios de salud, utilizándose el test quicuadrado, para evaluar la significancia estadística en los cruces establecidos con variables demográficas y de condiciones de vida. Se utilizó todavía, regresión logística para investigar la magnitud de la asociación entre las diversas dimensiones de apoyo social y las variables: procuró servicios de salud y restricción de actividades habituales. De modo general, este estudio reveló que los habitantes de Camaquã tienen acceso a los servicios de salud de la red municipal cuando buscan por atendimiento, observándose que las diferencias en la utilización de los servicios de salud son reflejo de las diferencias de comportamiento de los individuos frente a las enfermedades, donde el acto de buscar atendimiento pasa por las elecciones individuales de cada usuario, yendo más allá de las características de organización de la oferta o disponibilidad de servicios. En ese contexto, el estudio de las redes de apoyo y de apoyo social aparece como instrumento capaz de dar voz a la complexidad de factores que interfieren en el modo de vida y salud de los individuos, sugiriendo la importancia de ésos como determinantes sociales de acceso y utilización de los servicios de salud. Se evidencia, por lo tanto, la necesidad de examinar el conocimiento sobre las dinámicas sociales provenientes de las diferencias entre los seres humanos y sus implicaciones en el acceso y utilización de los servicios de salud, siendo necesario para esto una visión ampliada sobre el tema, que permita la elaboración de políticas de salud adecuadas al contexto socioeconómico, político y cultural local.
