Thesis (MNutr (Interdisciplinary Health Sciences. Human Nutrition))--University of Stellenbosch, 2006. / INTRODUCTION: The objective of the study was to determine the prevalence of
certain known risk factors for intra-uterine growth restriction (IUGR) in women who
gave premature birth to growth-restricted infants at a large regional hospital
(Kalafong) in the Gauteng province of South Africa and to investigate the possible
associations between the presence of various risk factors and the severity of growth
restriction found in these infants.
METHOD: The study was designed as cross-sectional, descriptive and observational.
The subjects included singleton growth-restricted premature infants (n=80), without
congenital abnormalities and their mothers (n=80). Anthropometric data [weight,
height, mid-upper arm circumference (MUAC) and triceps skinfold thickness (TSF)]
were collected from these mothers three to four days post-partum. Infant birth weights
were recorded at birth, while the lengths and head circumferences were recorded
within 2 days post-partum. Additional information, such as birth spacing, maternal
age, smoking habits and alcohol use, was collected by personal interview and blood
pressure data and HIV status was obtained from medical records. Data capturing and
descriptive statistics were done using Microsoft Excel and comparative analytical
statistics were performed with the Statistical Package for the Social Sciences (SPSS),
version 12.0.
RESULTS: The study demonstrated a high prevalence (69%) of infants born with a
birth weight <3rd percentile. In the sample, 81% of the mothers were aged 17-34 years
and most (93%) had their children 18 months or longer apart. Malnutrition prevalence
was moderate. In 58% of the mothers the BMI was normal (18.5-24.9 kg/m2) and in
47% the upper arm muscle area (UAMA) was between the 10th-85th percentile. Grade
III overweight occurred in 3% and TSF ≤5th percentile occurred in 35% of the
mothers. About half (51%) of the mothers in the sample population had hypertension
during the second trimester of pregnancy. Smoking and alcohol use during pregnancy
was rare (1% and 6% respectively) and the prevalence of HIV infection in the mothers
was 26%. The prevalence (16%) of Grade II overweight among the mothers of
symmetric growth-restricted (SGR) infants was higher than among the mothers of asymmetric growth-restricted (AGR) infants (7%). Of the hypertensive mothers, 55%
had infants with SGR compared to 45% with AGR (p=0.47). Although rare, smoking
occurred only in mothers with AGR infants (3%). No significant differences were
found between the smoking and non-smoking group (p=0.21). Although the use of
alcohol was more prevalent at 6% in mothers with SGA infants and 7% in mothers
with AGR infants, no significant associations were found (p=0.95). Although not
significant (p=0.76), there was a higher prevalence of HIV infection in mothers with
SGR infants at 29%, compared to 23% of mothers of AGR infants.
CONCLUSION: Although further studies are needed before intervention strategies
can be planned and implemented, the findings of this study suggest that apart from the
usual factors (maternal age and nutritional status, smoking and alcohol use during
pregnancy and birth spacing) that may influence intra-uterine growth, hypertension
may contribute greatly to IUGR in this study population.

Fotografia cervical digital para rastreamento de câncer de colo uterino e suas lesões precursorasHillmann, Elise de Castro January 2010 (has links)
Background: O câncer de colo uterino é um grande problema de saúde pública no Brasil e no mundo. Atinge cerca de meio milhão de mulheres, sendo que 50 % destas morrem. Os métodos visuais, alternativos á citologia e viáveis para países em desenvolvimento, têm sido avaliados mais intensamente na última década. Objetivos: Avaliar o desempenho da Fotografia Cervical Digital (FCD) na detecção do câncer do colo de útero e de suas lesões precursoras. Métodos: 176 mulheres foram avaliadas pelos métodos de Inspeção Visual com ácido acético (VIA), Inspeção Visual com lugol (VILI), Fotografia Cervical Digital com ácido acético (FCDA) e Fotografia Cervical Digital com lugol (FCDL). Destas, 36 foram classificadas pela histologia. Resultados: A concordância entre avaliadores observou valores de Kappa: Fotografia Cervical Digital com ácido acético, K=0,441, e Fotografia Cervical Digital com lugol, K=0,533. A concordância entre as inspeções a olho nu e as fotografias cervicais digitais após a utilização de ácido acético no colo uterino, K=0,559, e a concordância após a utilização da solução de lugol, K=0,507, considerada como concordância moderada em ambos os casos. Das 36 pacientes com avaliações histológicas, 20 dos 25 casos positivos foram corretamente avaliados tanto pela FCDA, como pela FCDL, demonstrando resultados similares aos descritos anteriormente. Conclusão: A FCD é um método promissor para o rastreamento do câncer de colo de útero e suas lesões precursoras, para países em desenvolvimento. / Background: Uterine cervix cancer is a major public health problem in Brazil and in the world. It affects half a million women with a death rate of 50%. The visual, alternative methods to cytology, considered viable for developing countries, are being assessed more intensively in the last decade. Objective: To evaluate the performance of Cervical Digital Photography (CDP) in detecting cervical cancer and its precursory lesions. Methods: A total of 176 women were evaluated by the following methods: Visual Inspection with Acetic Acid (VIA), Visual Inspection with Lugol’s Iodine (VILI), Cervical Digital Photography with Acetic Acid (CDPA) and Cervical Digital Photography with Lugol’s Iodine (CDPL). Among these, 36 were classified by histology. Results: The correlation between evaluators observed Kappa values: Cervical Digital Photography with acetic acid, K = 0.441, and Cervical Digital Photography with Lugol's Iodine, K = 0.533. The correlation between the naked eye and cervical digital photography inspection after using acetic acid in the uterine cervix, K = 0.559, and the correlation after using Lugol's Iodine solution, K = 0.507, were regarded as moderate in both cases. Of the 36 patients with histological evaluation, 20 of the 25 positive cases were correctly assessed both by the CDPA as well as by CDPL, showing similar results than those described previously. Conclusion: CDP is considered a more promising method for screening the uterine cervix cancer and its precursory lesions in developing countries.

Previous issue date: 2007 / O câncer de colo de útero é o mais comum entre as mulheres no mundo. No
Brasil, estima-se que seja a terceira neoplasia maligna encontrada entre
mulheres. Pode ser prevenido se for detectado precocemente, há
necessidade de sua conscientização por meio da educação em saúde, que
pode auxiliar as mulheres a terem maior autonomia sobre seu corpo e
saúde. Com o propósito de contribuir para a melhoria na promoção da
saúde de mulher, este trabalho teve o objetivo de construir e validar
material educativo com orientações passo a passo sobre a importância e
realização do exame de Papanicolaou para usuárias de serviços públicos de
saúde da cidade de São Paulo/Brasil. Trata-se de pesquisa descritiva,
exploratória e observacional que se propôs construir um folder com
orientações para prevenção do câncer cérvico-uterino para mulheres, tendo
como referencial teórico a educação de adultos. Sua construção foi realizada
com as mulheres, utilizando-se linguagem e desenhos de fácil compreensão.
Foi avaliado por oito profissionais especialistas na área. Realizadas as
modificações sugeridas, ele foi reproduzido e validado por 96 usuárias de
unidades básicas de saúde da cidade de São Paulo. Os resultados da
validação revelam que as mulheres consideraram por unanimidade o texto
interessante com frases de fácil compreensão, ilustrações simples que
facilitaram seu entendimento. A grande maioria teve o interesse em ler o
folder até o final que as fez repensar sobre sua saúde. As mulheres
expressaram a boa qualidade do material, verbalizando o interesse em levá-
lo para casa. Concluiu-se que o material educativo construído e validado
poderá colaborar na produção do conhecimento das mulheres que
freqüentam unidades de saúde e contribuir para o recrutamento da
população para realização desse exame simples e indispensável para
prevenção do câncer cérvico-uterino com o objetivo de contribuir para
diminuição dos índices de morbimortalidade feminino por câncer. / The cancer of the cervix of the uterus is the most common among the women
in the world. In Brazil, it is supposed to be the third malignant neoplasia
found among women. It can be prevented if detected precociously. It is
necessary to aware people through health education so that it can help
women to be more autonomous concerning their body and health. With the
purpose of contributing to improvement in women health promotion, this
paper had the aim of building and validating educational material with step
by step orientations about the importance and accomplishment of
Papanicolaou exam to public health service users of São Paulo city in Brazil.
It is a descriptive, exploratory and observational research with the purpose
of developing a folder with orientation about the cancer of the uterine cervix
to women, having as theoretical reference the adults' education. Its
production was done with women, using language and drawings easily
understood. It was analyzed by eight specialists in the area. After the proper
changes, the folder was printed and validated by 96 users of the basic health
units of São Paulo city. The results of acceptance reveal that all the women
considered the text interesting with easy understandable sentences and
simple illustration which helped their comprehension. The majority was
interested in reading the text until the end which made them to rethink their
health. The women expressed the good quality of the material when they
said they were interested in taking it home. It was concluded that the
educational material produced and validated will be able to help in the
knowledge production of women that usually go to health public bases and
contribute to the population recruiting and accomplishment of this simple
and indispensable exam to uterine cervical cancer that must help a lot to

A progressão para as lesões invasoras está associada à imunidade do hospedeiro, dentre as quais, a interferência no processo de apoptose celular. Entre os vários genes associados à morte celular, o gene FAS parece ser um elemento importante. O objetivo deste estudo foi avaliar se há uma associação do polimorfismo da região promotora -670 do gene FAS com a NIC 3 e o carcinoma invasivo do colo do útero. Métodos: A avaliação da presença do polimorfismo do gene FAS foi realizada através da Técnica de PCR e RFLP em amostras de sangue para o grupo controle (225 amostras) e de tecido cervical para o grupo de casos, com NIC 3 (75 casos) e carcinoma invasor (214 casos). A análise estatística foi realizada através do cálculo do Teste Exato de Fisher, Kruskal-Wallis, Mann-Whitney e Qui-Quadrado, sendo considerado o valor de significância de 5% ou p<0,05. Resultados: O genótipo heterozigoto (AG) do gene FAS -670 foi significativamente mais frequente no grupo de mulheres com NIC 3, com um risco estimado de 3 vezes (OR=3,0; 95% IC: 1,29-7,16), quando ajustado a idade. Não houve diferença, porém, tanto no grupo controle quanto nas mulheres com carcinoma cervical. Nenhum genótipo esteve associado a um maior risco de NIC 3 ou câncer quando comparamos idade e etnia. Nas mulheres com câncer, os genótipos foram semelhantes nos diferentes tipos histológicos (epidermóide e adenocarcinoma) e grau de diferenciação tumoral. Avaliando a distribuição alélica (A e G), não observamos diferença na frequência dos alelos nos grupos estudados. Conclusão: O polimorfismo da região promotora do gene FAS -670 esteve associado a um risco maior de lesão intraepitelial de alto grau (NIC 3), mas não para o câncer invasor do colo do útero, quando comparados ao grupo controle. / Objective: The human papillomavirus (HPV) is the etiologic agent of cervical cancer, but alone it is incapable of inducing oncogenesis. Rather, progression to invasive lesions is associated with host immunity and interference in the process of cellular apoptosis. Among the several genes involved in cell death, the FAS gene appears to be an important factor. The aim of this study was to evaluate whether there is an association between polymorphisms of the FAS -670 gene promoter region and preinvasive and invasive lesions of the cervix. Methods: The FAS gene was evaluated for the presence of polymorphisms using polymerase chain reaction and restriction fragment length polymorphism techniques in 225 blood samples for the control group, as well as cervical tissue from patients with cervical intraepithelial neoplasia grade 3 (CIN 3; 75 cases) or invasive carcinoma (214 cases). Results: The heterozygous genotype (AG) of the FAS -670 gene promoter region was significantly more frequent in women with CIN 3, with an estimated risk of three times (OR=3.0; 95% CI: 1.29 to 7.16). No difference, however, was observed in the control group and women with cervical cancer. In women with cancer, the genotypes were similar in the different histological types and degree of tumor differentiation. Assessing allelic distribution (A or G), we observed no difference in frequency of allele in studied groups. de 2007 referidas pela rede básica de saúde para colposcopia, obtendo o diagnóstico final
através dos laudos histopatológicos das peças de conização.
Resultados: Foram incluídas 65 pacientes na situação descrita e encontrado percentual de
33,8% (IC95% 23,1- 45,9%) de HSIL e 4,6% (IC95% 1,1- 12,0%) de câncer confirmados
histologicamente. Os demais casos apresentaram lesão intra-epitelial escamosa de baixo
grau (LSIL - 26,1%, IC95% 16,5- 37,8%), displasia glandular (1,5%, IC95% 0,07- 7,35%)e ausência de doença (33,8%, IC95% 23,1- 45,9%). Não foi encontrada diferença
estatisticamente significativa na razão de prevalência de HSIL e câncer em grupos de
mulheres segundo estratos de idade abaixo e acima de 45 anos e entre mulheres
soropositivas e soronegativas para HIV.
Conclusão: A prevalência de HSIL ou câncer encontrada não parece suficiente para
defender a conduta de encaminhá-las de imediato para conização a fim de investigar o canal
cervical. Porém, são necessários ensaios clínicos randomizados para que possamos saber
qual a conduta mais adequada na situação em estudo. / Introduction: Cervix cancer is a serious public health problem in Brazil. Some of the
recommendations for controlling the disease in the country are based on the opinion of
specialists. In patients with unsatisfactory colposcopy with no visible lesion and cervical
cytology suggesting high-grade squammous intraepithelial lesion (HSIL), the
recommendation is to repeat cervical cytology after three months. In the Cervical
Pathology unit at Instituto Fernandes Figueira/ FIOCRUZ, these patients are directly
subjected to conization in order to confirm diagnosis and to excise the lesions found, if
HSIL is verified.
Objectives: To assess prevalence of HSIL and cancer in patients with unsatisfactory
colposcopy with no visible lesion and first cervical cytology suggestive of HSIL, and to
identify an age group or other groups with higher prevalence in order to contribute to the
discussion about a more effective clinical approach that will also prevent patients from
abandoning follow-up before full diagnosis and treatment.
Method: Cross-sectional study with data from the Cervical Pathology unit database on
patients treated between December 1989 and April 2007 who were referred to the unit for
colposcopy by the primary healthcare network. Final diagnosis was obtained through
histopathological examination of conization specimens.
Results: Of the 65 patients studied, 33,8% (CI95% 23,1- 45,9%) had HSIL, and 4,6%
(CI95% 1,1- 12,0%) had cancer confirmed by histological examination. The other patients
were diagnosed as having low-grade squammous intraepithelial lesion (LSIL - 26,1%,
CI95% 16,5- 37,8%), glandular dysplasia (1,5%, CI95% 0,07- 7,35%), and absence of
disease (33,8%, CI95% 23,1- 45,9%). This study did not find statistically significant difference of the prevalence ratio of HSIL and cancer between age groups of more than 45
and less than 45 years of age, and between women tested positive or negative for HIV.
Conclusion: The prevalence of cancer and HSIL found in this study does not seem to be
enough to justify direct referral for conization to investigate the cervical canal.
Nevertheless, randomized clinical trials are necessary to determine an approach that
would be more adequate in this situation.

O cavalo Crioulo é originário de raças de cavalos espanhóis trazidos há quatro séculos e reproduziu-se livremente no sul do continente da América.A seleção natural que ocorreu durante séculos fez o cavalo Crioulo ser conhecido por sua rusticidade e resistência, o que pode ter influenciado positivamente a fertilidade e longevidade desta raça, mas informações sobre fertilidade e performance reprodutiva da égua Crioula sob manejo natural ou controlado ainda são escassas. Os objetivos do presente estudo foram: (1) elucidar a dinâmica folicular das éguas Crioulas; (2) caracterizar o desenvolvimento e a regressão do corpo lúteo; e (3)estudar as mudanças de tônus e de ecotextura uterina ultrassonográfica durante dois ciclos estrais consecutivos. No experimento, 26 intervalos interovulatórios (IIO) foram estudados em 13 éguas. O ciclo estral natural foi caracterizado por: (i) o comprimento do IIO foi de 21 dias; (ii) os folículos dominante e subordinado emergem juntos no dia 5 e a taxa de crescimento é lenta até o momento do divergência; (iii) o divergência folicular ocorreu quando o folículo dominante atingiu o diâmetro de 22mm; (iv) o diâmetro pré-ovulatório foi de 43 mm; (v) o corpo lúteo atingiu o tamanho máximo 1,5 dias depois da ovulação e no dia 15 já não pode ser mais visualizado por ultrassonografia transretal; e (vi) a forte correlação de repetibilidade demonstrou que o tônus uterino e cervical podem ser usados para determinar a fase do ciclo estral. Os resultados apresentados neste trabalho possuem grande importância para melhor compreensão das características reprodutivas naturais do ciclo estral da égua Crioula. / The Criollo horse originates from breeds of spanish horses that were brought four centuries ago and reproduced freely in the southern part of the American continent. The natural selection that occurred over the centuries made the Criollo horse known for its roughness and resistance, which may have influenced positively the fertility and longevity of this breed, but information regarding fertility and reproductive performance of Criollo breed mare under natural or controlled management conditions is scarce. The objectives of the present study were to: (1) elucidate the follicular dynamics of Criollo breed mares; (2) characterize growth and regression of the corpus luteum; and (3) study the tone and ultrasonographic uterine echotexture changes during two consecutive estrous cycles. In this trial, 26 interovulatory intervals (IOI) were evaluated in 13 mares. The spontaneous estrous cycles were characterized by: (i) the length of the IOI was 21 days; (ii) dominant and subordinate follicles emerge together at day 5 and their growth rate were slower until the deviation moment; (iii) deviation occurred when the dominant follicle has achieved 22 mm; (iv) diameter of the preovulatory follicle was 43 mm; (v) corpus luteum has achieved its maximum diameter 1.5 days after ovulation and at day 15 it can no longer be visualized by transrectal ultrasonography; and (vi) the strong correlation for repeatability demonstrated that the uterine and cervical tonus can be used to determine the phase of the estrous cycle. The results presented are of fundamental importance for a better understanding of the reproductive physiological patterns of the estrous cycle in the Criollo mare.

Alterações citológicas em esfregaços cervicais de pacientes com Lúpus Eritematoso Sistêmico em terapia imunossupressora / Citological alterations in pap smears of patients with SLE and immunosuppressionMario Lucio Cordeiro Araujo Junior 18 September 2009 (has links)
O Lúpus eritematoso sistêmico (LES) é uma doença auto-imune, mais prevalente em mulheres, com marcante aumento da sobrevida nos últimos anos, principalmente relacionado ao avanço do ratamento com terapia imunossupressora / citotóxica. Este estudo visa identificar a prevalência de alterações em esfregaços cervicais nesse grupo de pacientes. Participaram do estudo 80 pacientes do grupo com LES e 101 pacientes do grupo controle. Observou-se que a prevalência de alterações no grupo de paciente com LES foi de 31,3% contra 5,0% do grupo controle (p = 0,000). Não foram encontradas associações estatisticamente significativas entre a presença de alteração no esfregaço cervical e as variáveis: história de DST, sexarca com idade ≤ 17 anos, parceiros múltiplos (≥ 4), história de HPV prévio, uso de ACO, tabagismo, estrato social ou com droga imunossupressora específica. Encontrou-se associação estatisticamente significativa apenas para a variável grupo, tendo paciente pertencente ao grupo LES chance 8,2 vezes maior (IC 95% = 2,7 24,9) de ter alterações no esfregaço cervical que uma paciente do grupo controle. Tais dados demonstram que pacientes com LES em terapia imunossupressora apresentam mais alterações em exames preventivos ginecológicos, merecendo atenção especial quanto à prevenção do câncer de colo do útero. / Systemic Lupus Erythematosus (SLE) is an autoimmune disease with higher prevalence over women. There has been a significant increase in survival time over the years resulting from the improvement of citotoxic and immunosuppressant therapy. This study aims at identifying the prevalence of altered results for cervical smears in this group of patients. 80 patients of the SLE group and 101 patients of the control group were part of the study. The prevalence of alterations in the SLE group was 31.3%, and 5% in the control group (p=0.000). No statistically significant association was found between the presence of alterations in pap smears and the variables STD history, first sexual intercourse at age ≤ 17, multiple partners (≥ 4), previous history of HPV, use of oral contraceptives, tobacco smoking, social class and cancer presence in the family. Statistically significant association was found only in the group variable; patients in the SLE group had a 8.2 higher chance (IC 95% = 2.7 24.9) of showing alterations in the pap smears than those in the control group. These numbers show that SLE patients in immunosuppressant therapy have more alterations in pap smear exams, deserving special attention with regards to cervical cancer prevention.

Os miomas uterinos (MU) são considerados os tumores mais comumente encontrados no sistema reprodutor feminino. Este estudo buscou investigar associação entre o ganho de peso na idade adulta e a ocorrência de miomas. Esta relação foi explorada em estudos internacionais na maior parte americanos. No Brasil nenhum estudo havia abordado este tema até o término desta dissertação. Para investigar como a associação entre o ganho de peso na idade adulta afeta o risco de miomas, foi conduzido um estudo transversal referente aos dados da linha
de base (1999-2001) do estudo longitudinal Pró-Saúde onde 2594 servidores técnico-administrativos de uma universidade pública do Rio de Janeiro são participantes, entre eles foram selecionadas 1560 mulheres com diagnóstico médico autorrelatado de MU e que tiveram seus pesos aos vinte anos de idade autorrelatados em questionário autopreenchível cujas idades variaram entre 22 a 67 anos. Foi avaliada a mudança de peso desde os 20 anos até o momento da entrada no estudo. As prevalências de miomas encontradas neste grupo foram23.3%
(IC95% - 21,2; 25,4), situando-se em posição intermediária quando comparada as prevalências encontradas nos estudos norte-americanos. A maior prevalência de ganho de peso foi encontrada no 4 quintil cujos valores variaram entre ganho maior que 0,7 Kg/ano e menor que 1,1 Kg/ano. A faixa etária com maiores prevalências de diagnóstico médico auto-relatado de miomas ficou compreendida entre 45 a 54 anos (39,1%).Os valores de peso aos 20 anos informados pelas respondentes variaram entre 30,0 e 96,0 kg. O IMC aos 20 anos de idade se situou está dentro da categoria de normalidade entre a maior parte das participantes. Os resultados não sugerem associação entre o ganho de peso na idade adulta e miomas nesta população de estudo / The uterine fibroids (MU) are considered the most common tumors found in the female reproductive system. This study investigated the association between weight gain in adulthood and the occurrence of fibroids. This relationship was explored in international studies mostly Americans. In Brazil, no studies had
addressed this issue until the end of this dissertation. To investigate how the association between weight gain in ddulthood affects the risk of fibroids, a crosssectional study was conducted for the data from the baseline (1999-2001)
longitudinal study of the Pro-Saúde where2594 technical and administrative staff of a public university in Rio de Janeiro are participating, among them were selected in 1560 women with self-reported medical diagnosis of MU and had their weights at the age of twenty self-reported in self-administered questionnaire whose ages ranged from 22-67 years. Weight change since 20 years was evaluated until the entry into
the study. The prevalence of fibroids found in this group were 23.3% (95% CI - 21.2, 25.4), standing at an intermediate position compared the prevalence found in American studies. The highest prevalence of weight gain was found in the 4th quintile values ranged between gain greater than 0.7 kg / year and less than 1.1 kg / year. The age group with highest prevalence of self-reported medical diagnosis of fibroids was between 45 and 54 (39.1%). The weight at age 20 reported by respondents ranged between 30.0 and 96.0 kg. The BMI at 20 years old stood in the category is normal among most participants. The results suggest no association between weight
gain in adulthood and fibroids in this study population.
