Previous issue date: 2007-10-19 / This study focuses on questions raised in the course of clinical observation involving patients with dementia, more particularly, the ones with Alzheimer s Disease (AD). It aims at discussing, with particular emphasis, the symptomatic speech manifestations of the common events referred to in literature as anomia or empty speech speech, although grammatically well formed, does not properly convey meaning neither does it establish reference. I departure from a critical review of the literature of the fields of Medicine and Speech Therapy.
The assessment of such literature indicated that - despite the variability concerning both the description of speech symptoms in AD and its causes - all studies eventually mention a loss of external reference in characterizing the patients speech. Such common trait between otherwise differing studies is interpreted as being the result of the theoretical assumption of language as nomenclature. It is argued that in such a perspective reduces language to a code which can, as so, be approached through descriptive apparatuses (based either on grammar, pragmatics or semantics). This view on language is associated with the notions of language and sign, sustained by Classical Philosophy, as well as with the notion of psychological subject. It is pointed out that the process of displacement of such notions to clinical fields involves a significant loss of conceptual strength.
In this dissertation, I examine different approaches to the speech of AD patients. In order to do so, I assume the theoretical perspective on language which was inaugurated by Saussure a perspective that promoted a discontinuity in the path traditionally followed in the reflections on language (and the sign) in western literature. From this point of view, language is not a nomenclature. Such a statement guided the interpretation of the speech of AD patients presented in this study. As a result of this theoretical approach, it is argued, that the so-called empty speech can be finally recognized as being full of truth the truth of a singular relationship between a speaking-subject and language / Este trabalho aborda uma questão suscitada pelo meu encontro com falas de sujeitos com demência, ou seja, com manifestações sintomáticas que, na literatura sobre o assunto, são designadas como anomia - diminuição de vocabulário funcional e como fala vazia - falas que, embora articuladas e coesas, são esvaziadas de sentido, carente de referências. Parto de um levantamento bibliográfico não exaustivo, porém extenso, de trabalhos na área da demência.
Explorei com maior detalhe estudos referentes à Doença de Alzheimer (DA), desenvolvidos nos campos clínicos na Medicina e na Fonoaudiologia. Pude observar que, apesar das diferenças quanto ao tipo de explicação sobre suas causas e quanto ao modo de descrição dos sintomas na fala, em todas as pesquisas sobre a DA a perturbação lingüística é caracterizada em termos de perda da referência externa. Argumento que, em todos eles, a linguagem é implicitamente assumida como uma nomenclatura. Em outras palavras, ela é código, pode ser abordada a partir de aparatos categoriais (gramaticais, pragmáticos ou semânticos) e tem, declaradamente, as funções representativa e designativa. Trata-se de uma visão fortemente ligada às concepções de linguagem e de signo que movimentaram a Filosofia Clássica, que deixam em saliência a concepção de sujeito psicológico. Essa perspectiva sobre a linguagem se estende, com perdas conceituais, sem dúvida, para os campos clínicos.
Nesta tese, discuto tais abordagens (na verdade redutíveis a uma). Tomo o partido de Saussure porque dou reconhecimento à enunciação da ordem própria da língua, um corte em relação ao pensamento ocidental sobre a linguagem e sobre o signo. Segundo Milner (1978), Saussure não é mais um na história das idéias lingüísticas e filosóficas sobre a linguagem: ele é UM. Seu pensamento é exigente e oferece resistência. Assumo, com Saussure que a língua não é nomenclatura e procuro retirar conseqüências teóricas e empíricas para a escuta e explicação de sujeitos com demência. Procuro fazer valer as leis de referência interna da linguagem que deslocam o signo para o lugar de efeito de suas operações. Com base neste solo teórico, encontro na ditas falas vazias , falas plenas de uma verdade sobre a relação profunda e indissolúvel do sujeito com a linguagem

In the speech therapy clinic, patients complaints usually involve difficulties related to talking. However, problems related to reading and writing can be quite surprising and demonstrate that the main issue of aphasic patients is related to the speech process. Reading and writing are, like talking, subjective activities and, as such, a rich area for therapeutic approaches. The objectives of this study are to (1) review the literature and comment on the different clinical presentations of aphasia in relation to talking, reading and writing; (2) present and discuss the effect of this context in the Speech Therapy Clinic; (3) compare these findings with the ones that the Research Group Aquisição, patologias e clínica de linguagem support (Group Coordinated by Prof. Maria Francisca Lier-De Vitto no LAEL/PUC-SP); (4) discuss clinical issues related to the implementation of activities that involve not only talking but also reading and writing. For this purpose, I present a report of four case studies. It is important to remark that many other researchers and scientists have also evidenced and described many interesting findings in this group of patients, and this was the motivation of this study. Nevertheless, reports are much more restricted to the observations of signs and clinical presentations, with little appraisal of the relation with reading and writing, other symptoms and therapeutic approaches. In conclusion, impasses and clinical handering of aphasic patients demand more discussion and attention in the literature / Afásicos, via de regra, apresentam perturbações na fala em função de lesão cerebral. Na clínica fonoaudiológica de linguagem, a queixa dos pacientes costuma restringir-se às dificuldades relacionadas à fala. Porém, a heterogeneidade e imprevisibilidade das perturbações sobre a relação entre fala-leitura-escrita são surpreendentes nas afasias e nos permitem afirmar que o problema de afásicos é, sem dúvida, com a linguagem. Leitura e escrita são, como a fala, espaços de realização subjetiva o que, conseqüentemente, aponta para sua relevância como recurso terapêutico, permanecendo, muitas vezes, como espaço(s) exclusivo(s) de manifestação lingüística e subjetiva do paciente e, portanto, como condições privilegiadas na situação clínica. Sendo assim, este trabalho objetiva (1) situar e comentar os diferentes modos sintomáticos de relação fala-leitura-escrita instaurados em quadros afásicos, conforme abordados na literatura neurológica e neuropsicológica; (2) apresentar e discutir os efeitos desse raciocínio na Clínica Fonoaudiológica; (3) procurar discernir esse tipo de raciocínio do que embasa o Grupo de Pesquisa Aquisição, patologias e clínica de linguagem coordenado pela Profa. Dra. Maria Francisca Lier-DeVitto no LAEL/PUC-SP; (4) discutir questões clínicas relacionadas à implementação de procedimentos que envolvem, além da fala, também leitura e escrita. Para isso, apresento quatro relatos de casos. De minha leitura crítica vale destacar que os pesquisadores (afasiologistas e fonoaudiólogos) investigados notaram e registraram os mesmos acontecimentos surpreendentes e misteriosos na fala/leitura/escrita que me impulsionaram nesta pesquisa. Porém, esta observação permitiu apenas que eles observassem sinais da doença (compondo o quadro nosográfico das afasias) mas não refletissem sobre a relação afásico-leitura/escrita, nem sobre os efeitos clínicos dos atendimentos e/ou manifestações sintomáticas. Impasse e manejos clínicos no atendimento de afásicos não encontram, portanto, espaço para discussão fora da Clínica de Linguagem

Previous issue date: 2009-06-01 / In post-modernity, the psychoanalytical practice has been facing the challenge of dealing with changes in subjectivity and new psychopathological disorders. In this scenario, it comes the Borderline patient, bringing up challenges and opportunities in psychoanalysis that are gaining more relevance in the clinical and scientific discussions of the country, requiring analysts dedication to reflect on how to handle this sort of patiens within the clinical practice.
Given this reality and in search for a more clinically effective approach, the objective of this survey was to deepen the current insights on the Borderline Personality disorder, with the intention of undertanding specific aspects of transference and countertransference demand that are experienced in the psychoanalysis process. This work was accomplished through a qualitative approach based on the clinical method of psychoanalysis, following the theoretical framework of Donald Winnicott.
Having reflected on how to manage a borderline case, this survey outlines specific issues concerning transference and countertransference. It was observed that the problem lies at the confluence of the existential suffering of the patient and the countertransferencial suffering of the analyst, that requires professional sensitivity so as to ensure an appropriate approach aligned with the patient needs, indeed considering its own limits. In sum, the analysis process should be oriented to provide the patient a more integrated self / Na pós-modernidade, a clínica psicanalítica vem se deparando com o desafio de lidar com mudanças na subjetividade e novas formações psicopatológicas. Neste cenário, está o paciente borderline, cujos desafios e possibilidades no atendimento vêm ganhando cada vez mais espaço nos meios clínicos e científicos do país e exigindo dos analistas tempo para refletir sobre como manejá-los clinicamente.
Diante desta realidade e em busca de uma clínica mais efetiva, o objetivo deste trabalho foi aprofundar o estudo acerca do Transtorno de Personalidade Borderline, para compreender aspectos específicos da demanda transferencial e da contratransferência experimentados nesse atendimento. Tratou-se de um trabalho de abordagem qualitativa, baseado no método clínico da psicanálise, seguindo o referencial teórico winnicottiano.
Ao refletir sobre o manejo do borderline, foram sublinhadas demandas específicas assentadas nas questões da transferência e da contratransferência. Observou-se que a problemática reside na confluência do sofrimento existencial deste paciente com o sofrimento contratransferencial do analista, exigindo do profissional sensibilidade para a realização de um manejo ajustado às necessidades de cada paciente, considerando também seus próprios limites. Desta forma, a análise funcionaria como um caminho para a construção de um self mais integrado

Previous issue date: 2009-06-05 / A research about anorexia and bulimia that had its start in the cyberspace and
overflowed to the real world, this is the true abstract of what the present paper
represents. The research started through a website known as Orkut, where I
searched for online communities of anorexic and bulimic girls, and also where I
invited them to take part in my research, through an interview using MSN. When I
interviewed the young women with eating disorders, I tried to keep as my focus the
relationship between the etiology of eating disorders and the family relations. As the
research went on, I collected a wide variety of interviews, both of anorexics and
bulimics. While I was researching on the internet, I enrolled in an institution that
offers psychoanalytic treatment to the youth that suffers from anorexia and bulimia,
called CEPPAN, Centro de Estudos e Pesquisas em Psicanálise da Anorexia e
Bulimia. Due to CEPPAN and the internet, I was able to achieve clinical experience
in cases of anorexia and bulimia. Therefore, my research ended with data that
blended clinical experience with a careful statistical analysis of the interviews
collected using the internet. The research was successful in confirming some of the
major psychoanalytical hypothesis about eating disorders and to offer a new finding,
which is the high prevalence of anorexia and bulimia among those which I called the
first daughter of the couple, that is, the first woman to be born, regardless of the
presence or absence of brothers / Uma pesquisa sobre anorexia e bulimia, que teve início no mundo virtual e acabou
transbordando para o real constitui o verdadeiro resumo do que a presente
dissertação de mestrado apresenta. A pesquisa teve início através do site de
relacionamento Orkut, onde busquei comunidades virtuais de anoréxicas e
bulímicas, e as convidei a participar de uma entrevista pelo MSN. Ao entrevistar as
jovens com transtornos alimentares, tentei manter o foco na relação entre a etiologia
dos transtornos alimentares e as relações familiares. Aos poucos, coletei um amplo
número de entrevistas, tanto de anoréxicas quanto de bulímicas. Ao mesmo tempo
em que pesquisava pela internet, comecei a freqüentar uma instituição que oferece
tratamento psicanalítico para jovens com anorexia e bulimia, o CEPPAN, Centro de
Estudos e Pesquisas em Psicanálise da Anorexia e Bulimia. Através do CEPPAN e
também da internet acabei tendo experiência clínica com casos de anorexia e
bulimia. Portanto, o resultado foi uma pesquisa que misturou experiência clínica com
uma análise estatística cuidadosa dos dados coletados através da internet. A
pesquisa teve sucesso ao confirmar algumas das principais hipóteses da psicanálise
sobre os transtornos alimentares e propor um novo dado, que é a alta prevalência de
anorexia e bulimia entre o que chamei de primeira filha do casal, isto é, a primeira
mulher que nasce, independentemente da presença ou não de irmãos

Speech Therapy of totally laryngectomized patients who use a tracheoesophageal
prosthesis. Methods: The participants of the study were three speech therapists with
experience in the field of laryngectomy rehabilitation, three head and neck surgeons and
three patients using a vocal prosthesis for a minimum period of one year. The subjects
were divided into three separate groups, according to their role in the study: Speech
therapists (GF), head and neck surgeons (GM) and patients (GU). The speech
therapists were submitted to an open interview, in which they assessed their clinical
practice with such patients, in detailed manner. After reading the material collected from
these interviews, a questionnaire was developed so that the surgeons could consider
their expectations towards speech therapy as well as their ideas on clinical success.
Likewise, a third questionnaire was developed, targeting the patients views on speech
therapy, their recollections on the subject and their references to specific clinical
strategies. After all interviews and questionnaires were collected, the material was
thoroughly read several times, and the content was organized in different categories
which derived from the contents of subjects speech. Speech order was maintained
while organizing the categories. Thus, the data analysis was done considering each
group separately, since not all categories were the same for the three groups. However,
a comparison was made for all groups, with regards to those categories that matched in
all three. Results: The time span of therapy was considered small by GF. The most
common techniques described by the speech therapists were those aiming stoma
occlusion, coordination between occlusion, breathing and speech, speech intelligibility
and pitch and loudness modulations. For this group, the main objective of speech
therapy is to ensure that the vocal prosthesis is the main way of alaryngeal
communication, if the patient so desires. For GM, the main role of speech therapy is to
provide vocal expression resources so that the patient can achieve communicative
independence. They also feel that speech therapy should be a space where the patients
learn to master their prosthesis resources and learn how to care for it effectively. As for
GU, the main gain related to speech therapy was improvement in quality of life, due to
the re-gaining of speech and voice, leading to greater social interaction. All viewed their
time with the speech therapist as a positive one, and do not necessarily consider a short
time-span as a main advantage of the tracheoesophageal prosthesis. As a result of the
combined analysis, the principles of speech therapy with these patients are: to insure
the patients adjustment to their prosthesis, to present them with specific speech
techniques and to promote conditions where the patient is free to express himself and
thus expand the abilities acquired in therapy to his daily life activities. Conclusions:
Therefore, the main goal of speech therapy is a combination of the aspects pointed out
by members of the three groups; that is, to provide the patient with independence to
handle and use his prosthesis not only in a daily manner, but with mastery of all its
resources and expression possibilities, but to promote as well a greater social
interaction of these people with their peers, hence improving their overall quality of life / Objetivo: Analisar os princípios e métodos presentes na terapia fonoaudiológica de
pacientes laringectomizados totais usuários de prótese traqueoesofágica, do ponto
de vista de fonoaudiólogos, médicos e pacientes. Métodos: Os participantes da
pesquisa foram três fonoaudiólogas que atuam na área de reabilitação de
laringectomizados totais (GF), três médicos cirurgiões de cabeça e pescoço (GM) e
três usuários de prótese traqueoesofágica (GU). Os fonoaudiólogos responderam a
uma entrevista contendo uma pergunta de caráter aberto, sobre a sua prática na
reabilitação desses pacientes. A partir da leitura do material coletado, elaborou-se
um questionário para GM, sobre suas expectativas quanto ao processo terapêutico
fonoaudiológico, e outro para GU, que versava sobre técnicas específicas,
expectativas e lembranças do processo de terapia fonoaudiológica. Após a leitura
do material, foram elaborados mapas de associação de idéias, e as falas dos
entrevistados foram alocadas em categorias correspondentes. Dessa forma a
análise foi realizada de forma individual para cada grupo. Em um segundo
momento, as falas dos entrevistados dos três grupos foram comparadas entre si,
com base nas categorias elaboradas. Resultados: O trabalho com os usuários de
prótese traqueoesofágica é descrito pelas fonoaudiólogas como rápido. A atuação
no período pré-operatório consiste de orientações acerca da cirurgia e suas
implicações e sobre os métodos alternativos de comunicação disponíveis. Após a
cirurgia, a terapia fonoaudiológica consiste, predominantemente, de técnicas
específicas para oclusão do estoma, coordenação entre expiração e fonação,
fluência de fala, tempo máximo fonatório, e variação de altura e intensidade vocal.
O caso é considerado bem sucedido quando o paciente utiliza a prótese como
principal forma de comunicação. Para os médicos, a função da terapia
fonoaudiológica é dar ao sujeito autonomia de comunicação, por meio de recursos
expressivos e domínio do uso da prótese. Para os usuários, o contato com a
fonoaudióloga foi positivo e os principais ganhos foram em qualidade de vida, pela
re-integração social e retomada da fala. Assim, os princípios da atuação
fonoaudiológica foram: mediar a adaptação do paciente à PTE, oferecer técnicas
para a fonação e propiciar condições para que ele se expresse. Considerações
Finais: Observa-se que a função do fonoaudiólogo é a combinação entre as
expectativas dos três grupos: propiciar ao paciente um domínio da prótese por meio
da expressão, autonomia no cuidado com a mesma, fazendo com que esse seja
seu principal recurso de comunicação. Dessa forma, a melhora da qualidade de
vida, pela reintegração na sociedade passa a ser o principal objetivo da atuação
com laringectomizados

researches, especially those which investigate issues related to the singing
voice. Aim: This study related a choral group s vocal symptoms and their
probable causes, according to gender and vocal classification. Methods: The
subjects of this study were 143 choral singers (58,7% female and 41,3%
male) from a University in the City of São Paulo. Subjects age varied
between 18 and 45 years, and subjects had different vocal classifications.
The participants answered a questionnaire that was adapted from the one
proposed by GUERRA (2005), which included, questions which enabled the
characterization of the group, questions that evaluated the presence of vocal
symptoms, and their likely causes, in the singers opinions. The data was
analyzed considering numeric values and percentage, of the following: most
frequent vocal symptoms and related causes according to gender (male x
female); most frequent symptoms and related causes in sopranos and altos;
most frequent symptoms and related causes in tenors and bases; overall
symptoms and their related causes; and most frequent symptoms in spoken
voice, singing voice, and both. In order to perform the statistical analysis, the
SPSS (Statistical Package for Social Sciences) version 13.0 was used, and
the level of significance was established at 5% (0.050). In addition, the Mann-
Whitney; Test was used to analyze possible differences between gender and
vocal classification, and Spearman s correlation test was used to verify the
relationship between vocal symptoms and their possible causes. Results:
From the subjects amongst the male gender, 25% did not mention any of the
listed symptoms, as well as 22.9% of the female subjects did not mention any
of the above mentioned symptoms. The symptoms that were most mentioned
were phlegm/secretion, (44,6%, both for male and female), followed by
hoarseness (22,9% for female and 26,8% for male) and dry cough (20,5% for
female and 14.3% for male). When comparing gender, there was significant
difference (p=0,015) in favor of male subjects, with regards to weak voice.
With regards to vocal classification, dry throat and dry mouth, differentiate
altos and sopranos. Amongst the causes, that were most mentioned by the
group, as those which could possibly explain the occurrence of the referred
symptoms, were: flu (50,8%), intense use of the singing voice (42,4%) and
allergies (41,6%). Conclusions: The vocal symptoms which were most
frequently mentioned were phlegm/secretion, hoarseness and wet cough.
Hoarseness was the symptom most mentioned in spoken voice, while
phlegm/secretion was the most common in singing voice, or both. When corelating
with gender, the symptom of weak voice was present in favor of male
subjects, and when co-relating vocal classification, the symptoms of dry
throat and dry mouth were most common amongst the altos. The subjects
showed previous knowledge when relating symptoms to possible causes.
Amongst these, the most commonly mentioned were flu, intensive voice
usage and allergy. Intense singing and spoken voice usage, respiratory
infections, allergies and digestive issues were specifically related to
hoarseness / Introdução: Na Fonoaudiologia, são poucas as pesquisas epidemiológicas,
principalmente as que investigam as questões com a voz cantada. Objetivo:
Este estudo correlacionou os sintomas vocais e suas possíveis causas, de
acordo com o sexo e a classificação vocal, em um grupo de coralistas da
Cidade de São Paulo. Método: Participaram 143 coralistas (58,7% mulheres
e 41,3% homens) de uma universidade da Cidade de São Paulo com idade
entre 18 e 45 anos e de classificação vocal diversa. Os participantes
responderam a um questionário adaptado da proposta de GUERRA (2005),
que incluiu, além de questões de caracterização do grupo, as que tinham
como proposta levantar a presença de sintomas vocais e as possíveis
causas na opinião dos coralistas. Os dados foram analisados considerando
o levantamento numérico e percentual, conforme a ocorrência de: sintomas
e causas que mais aparecem segundo o sexo (homens x mulheres);
sintomas e causas mais freqüentes em sopranos e contraltos; sintomas e
causas mais freqüentes em tenores e baixos; sintomas em geral e causas; e
sintomas vocais mais freqüentes na voz falada, cantada e em ambas. Para a
análise estatística foi utilizado o programa SPSS (Statistical Package for
Social Sciences) versão 13.0. Adotou-se o nível de significância de 5%
(0,050), para a aplicação dos testes estatísticos deste estudo. Para a análise
das possíveis diferenças entre o sexo e classificação vocal foi utilizado o
Teste de Mann-Whitney; e a análise de correlação de Spearman foi utilizada
com o intuito de verificar o nível de relacionamento entre sintomas vocais e
possíveis causas. Resultados: Dentre os homens, 25% referiram não sentir
nenhum dos sintomas mencionados, enquanto que 22,9% das mulheres não
mencionaram nenhum deles. Os sintomas mais referidos foram
pigarro/secreção (44,6%, tanto para mulheres quanto para homens), seguido
de rouquidão (22,9% para as mulheres e 26,8% para os homens) e tosse
seca (20,5% para o sexo feminino e 14,3% para o sexo masculino). Na
comparação referente ao sexo, houve diferença significante (p=0,015) a
favor dos homens no que se refere à presença da voz fraca. Na comparação
referente à classificação vocal, garganta seca e boca seca, diferenciaram-se
contraltos de sopranos. Dentre as causas, as mais mencionadas pelo grupo,
como possíveis para explicar os sintomas referidos, foram: gripe (50,8%),
uso intenso de voz cantada (42,4%) e quadro de alergia (41,6%).
Conclusão: Os sintomas vocais mais mencionados foram pigarro/secreção,
rouquidão e tosse com secreção. O sintoma de rouquidão foi o mais
mencionado como presente na voz falada, enquanto que pigarro/secreção,
na voz cantada ou em ambas as situações. Na correlação com a variável
sexo, o sintoma de voz fraca esteve presente a favor dos homens; e na
correlação com a classificação vocal, os sintomas de garganta e boca secas
estiveram mais presentes entre as contraltos. Os participantes
demonstraram conhecimento ao relacionar os sintomas assinalados e suas
possíveis causas. Dentre estas, as mais citadas foram gripe, uso intensivo
da voz e alergia. Especificamente relacionadas ao sintoma de rouquidão
foram citadas as causas de uso intenso da voz (falada e cantada), infecção
respiratória, alergia e problemas digestivos

Previous issue date: 2007-08-28 / The early diagnosis of auditory deficiencies is one of the decisive factors for the better development of the language functions of a deficient child. It is during childhood that the neurological maturation occurs, thus favoring the development of the basic perceptive abilities, as well as of language itself. Given the importance the early auditory deficiency diagnosis, programmes of universal neonatal hearing screening have been proposed world-wide. With the aim of implementing a neonatal hearing screening programme that has an efficient protocol, this study describes the findings of a hearing screening programme conducted with 140 babies born at the Hospital Evangélico de Belo Horizonte in the period between April and June 2005, utilizing two forms of scheduling: hearing screening conducted 15 days after hospital release and hearing screening conducted immediately after hospital release. The methodology utilized in the study includes: anamnesis of the prenatal, neonatal and postnatal conducted through interviews with the mothers, hearing screening using Transient Evoked Otoacoustic Emissions, Distortion Product Otoacoustic Emissions and research of the Eye-Blink Reflex (EBR). Of the 228 (100%) babies born in this period at the hospital, of which 138 (65,7%) were born in April and 72 (34,3%) in June, 210 (92,1%) have been recruited to have the hearing screening test administered. In the April group hearing screening was scheduled for 15 days after hospital release and in the June group hearing screening was scheduled immediately after hospital release. Of the 210 (100%) babies recruited, 140 (66,6%) attended triage, of which 70 (50,7%) belonged to the 15-day group and 70 (97,2%) to the Release group. Of the 70 (100%) evaluated babies in the 15-day group, 6 (8,5%) have presented indicators of risk of auditory deficiency; and of the 70 (100%) evaluated babies in the Release group, 6 (8,5%) have also presented indicators of risk auditory deficiency. Of all the babies evaluated, 140 (100%) presented EBR. In the research of both the transient evoked and distortion product otoacoustic emissions, 2 babies (2,9%) of the 15-day group have failed the evaluation, and in the Release group 22 (31,4%) have also failed. The findings of this study have demonstrated that the failure rate of the 15-day group in the otoacoustic emission test is lower when compared to the rate of the Release Group, however the consent to the test went higher in this group, given that the ease of having the test performed at the moment of release does not require the mother to return to the hospital / O diagnóstico precoce da deficiência auditiva é um dos fatores decisivos para o melhor desenvolvimento da linguagem da criança com deficiência. É na infância que ocorre a maturação neurológica, favorecendo o desenvolvimento das habilidades perceptivas básicas, bem como a linguagem. Tendo em vista a importância do diagnóstico precoce da deficiência auditiva, programas de triagem auditiva neonatal universal têm sido propostos mundialmente. Com o objetivo de implantar um programa de triagem auditiva neonatal com um protocolo eficiente, este estudo descreve os achados de um programa de triagem auditiva, realizado em 140 bebês nascidos no Hospital Evangélico de Belo Horizonte no período de Abril e Junho de 2005, em duas formas de agendamento: triagem auditiva realizada 15 dias após a alta hospitalar e triagem auditiva realizada imediatamente após a alta hospitalar. A metodologia utilizada no estudo incluiu: anamnese dos períodos pré-natal, neonatal e pós- natal, realizada por meio de entrevistas com as mães; triagem auditiva realizada com Emissões Otoacústicas Transientes, Emissões Otoacústicas Produto de Distorção e pesquisa do Reflexo Cócleo Palpebral (RCP). Dos 228 (100%) bebês que nasceram neste período no hospital, foram recrutados para realizar a triagem auditiva 210 (92,1%), sendo 138 (65,7%) nascidos em abril e 72 (34,3%) nascidos em junho. No grupo abril a triagem auditiva foi marcada para 15 dias após a alta hospitalar e no grupo junho a triagem auditiva foi marcada imediatamente após a alta hospitalar. Dos 210 (100%) bebês recrutados compareceram para triagem 140 (66,6%), sendo 70 bebês (50,7%) do grupo 15 dias e 70 bebês (97,2%) do grupo alta. Dos 70 (100%) bebês avaliados no grupo 15 dias, 6 (8,5%) apresentaram indicadores de risco para deficiência auditiva; e dos 70 (100%) bebês avaliados no grupo alta, 6 (8,5%) apresentaram também indicadores de risco para deficiência auditiva. De todos os bebês avaliados, 140 (100%) apresentaram RCP. Na pesquisa das emissões otoacústicas, tanto transiente quanto produto de distorção, no grupo 15 dias 2 bebês (2,9%) falharam e no grupo alta 22 (31,4%) também falharam. Os achados deste estudo demonstraram que no grupo 15 dias o índice de falha no exame de emissões otoacústicas é menor quando comparado ao grupo alta; porém, a adesão ao exame foi maior neste grupo, pois a facilidade em realizar o exame no momento da alta não exige que a mãe retorne ao hospital

Previous issue date: 2007-10-09 / Introduction: The Newborn Hearing Screening Program have been responsible for
anticipating the sensorineural and conductive hearing loss diagnoses for the last few
years. This study refers to the experience of a hearing screening starting before the
hospital discharge in the Neonatal Intensive Care Unit (NICU) and also to the
orientation to the Well-baby Nursery screening after the discharge. Aims: Describe a
Newborn Hearing Screening Program in a maternity hospital of the privative network in
Salvador, from 2000 to 2006, considering the historical context, the process of
implementation, the changes through the years, as well as the hearing findings and the
relation with the quality criteria proposed by the Joint Committee on Infant Hearing
2000.Method and Material: This is a retrospective, cross-sectional study. 4997
newborns and infants were screened through the use of Echocheck, the otoacoustic
emissions equipment and through the observation of the cochlear-palpebral reflex. The
results were divided in four groups: AI = NICU and screening in the pre-discharge; AII
= NICU and screening in the post-discharge; BI = Well-baby Nursery and screening in
the pre-discharge; BII = Well-baby Nursery and screening in the post-discharge.
Results: A description of the evolution and implementation process of the hearing
screening from 2000 to 2006 was done; in relation to the audiological findings, the
hearing screening results described were the ones from 2001 to 2006. There was an
assessment of 38,9% of the newborns and infants in 2001, and 79,5% in 2006.
According to the pre-discharge service analyses, the group AI/BI had from 8,1% to
11,2% of fails; in the post-discharge service analyses, the group AII/BII maintained the
fail index in 2,6%. As final results of the AI/BI group, from the total of 1739 screenings,
there were 21 confirmed cases of hearing alterations, being 20 conductive(6,1:1000) and
4 sensorineural (1,2:1000). Conclusion: The data indicates that the service before the
discharge to the NICU and a gradual extension to the Well-baby Nursery is workable,
ever since the protocol is continuously adjustable. Although the AI/BI group was the
third part of the sample it represented a higher prevalence of hearing alterations. These
show the importance of the hearing screening before the hospital discharge, so a higher
number of congenital hearing problems can be assessed / Introdução: Nos últimos anos, os programas de Triagem Auditiva Neonatal (TAN) têm
sido responsáveis por antecipar o diagnóstico das deficiências auditivas sensorioneurais
e condutivas, quando ambas estão presentes ao nascimento. O presente estudo relata
uma experiência de triagem auditiva iniciada antes da alta hospitalar na UTI Neonatal e
de orientação para triagem no Berçário Comum após a alta. Com o passar dos anos, o
programa estendeu-se para atendimento também no Berçário Comum. Objetivos:
Descrever um programa de Triagem Auditiva Neonatal em uma maternidade da rede
privada de Salvador, no período de 2000 a 2006, no que se refere ao seu contexto
histórico, ao processo de implantação e às modificações ocorridas ao longo dos anos;
também são discutidos os achados audiológicos e a relação com os critérios de
qualidade propostos pelo Joint Committee on Infant Hearing, de 2000. Material e
método: trata-se de um estudo retrospectivo, de caráter transversal. Foram triados 4997
neonatos e lactentes com emissões otoacústicas, equipamento Echocheck, e observação
do reflexo cócleo-palpebral. Os sujeitos foram divididos em quatro grupos: AI= UTI
Neonatal e triagem na pré-alta; AII= UTI Neonatal e triagem na pós-alta; BI= Berçário
Comum e triagem na pré-alta; BII= Berçário Comum e triagem na pós-alta. Resultados:
Foi descrito o processo de evolução e implantação da triagem auditiva no período de
2000 a 2006. Quanto aos achados audiológicos, foram descritos os resultados da triagem
auditiva de 2001 a 2006. Houve cobertura em 38,9% dos neonatos e lactentes em 2001,
chegando a 79,5% em 2006. Na análise dos atendimentos na pré-alta, o grupo AI/BI
apresentou índices de falha de 8,1% a 11,2%; na análise de atendimentos na pós-alta, no
grupo AII/BII, os índices de falha mantiveram-se em 2,6%. Como resultados finais, no
grupo AI/BI, do total de 1739 triagens, houve 21 casos confirmados de alterações
auditivas, sendo 17 condutivas (9,8:1.000) e quatro sensorioneurais (2,3 :1000). No
grupo AII/BII, do total de 3256 triagens, houve 24 casos confirmados de alterações
auditivas, sendo 20 condutivas (6,1:1000) e quatro sensorioneurais (1,2 :1000).
Conclusão: os dados indicaram que o atendimento antes da alta para UTI Neonatal e
sua extensão gradual para o Berçário Comum são viáveis, desde que o protocolo seja
continuamente ajustado. Embora representasse a terça parte da amostra, o grupo AI/BI
apresentou uma prevalência maior de alterações auditivas, evidenciando a importância
da TAN antes da alta hospitalar para o diagnóstico precoce de alterações auditivas
congênitas

of vocal disturbances and work psychosocial factors related to the
psychological demand and control over the work of public school teachers in
the city of Salvador. Method: a Transversal epidemiological study. A
standardized questionary was used for the collectin of information, as well as
a vocal assessment on 461 fundamental, secondary and Early Childhood
Education teachers in 24 schools in the municipality of Salvador. Results:
The prevalence of vocal disturbance of 56.6% among the teachers studied.
The study in high demand (low control and high demand) concentrated the
highest frequencies of the faculty with moderate and severe vocal
disturbance. The characteristics most frequently indicated by the teachers
concerning the psycological demand in work situations were: long
concentration periods, depence on the work of others which lead to the
slowdown of the overall work and exposition to conflincting demands. The
most frequentenly referred characteristics of control over work were: The
possbility of taking decisons and opine at the workplace, autonomy on the
fulfillment of tasks, be creative and boast a high level of abilities. It was
perceived that the factors that associate both posively and statiscally
significant to the moderate and severe vocal disturbance were: be a educator
for more than 20 years old, be between 51 and 56 years old, teach on a
double shift basis, namely Early Childhood or fundamental I and fundamental
II or secondary education. Conclusions: The results show that the vocal wellbeing
is influenced by the educator s working conditions / Este estudo teve como objetivo avaliar a associação entre a presença de
distúrbio vocal e os fatores psicossociais do trabalho, relacionados à
demanda psicológica e controle sobre o trabalho em professores da rede
pública de Salvador. Método: foi realizado um estudo epidemiológico
transversal. Para coleta de informações foi utilizado questionário
padronizado, bem como realizada avaliação vocal, por meio de escala
GRBAS, em 461 professores do ensino fundamental, médio e educação
infantil de 24 escolas, do município de Salvador. Resultado: A prevalência
de distúrbio vocal foi de 56,6% entre os professores estudados. O trabalho
em alta exigência (baixo controle e alta demanda) concentrou as
freqüências mais elevadas de docentes com distúrbio vocal moderado e
severo. As características mais frequentemente referidas pelos professores
sobre a demanda psicológica nas situações de trabalho foram: períodos
longos de concentração, esperar pelo trabalho de outras pessoas deixa mais
lento o ritmo de trabalho e exposição à demandas conflitantes. As
características, do controle sobre o trabalho, mais frequentemente referidas
pelos professores foram: a possibilidade de tomar decisões e opinar dentro
do próprio trabalho, autonomia para realização de tarefas, ser criativo e ter
alto nível de habilidades.Conclusão: No grupo pesquisado foi constatado
que os fatores que se associaram positiva e estatisticamente significante ao
distúrbio vocal moderado e severo foram: ter mais de 20 anos de ocupação
como docente, ter de 51 a 65 anos de idade, lecionar em dupla jornada, a
saber, ensino infantil ou nível fundamental I; e fundamental II ou médio

voice/throat symptoms and self-referred aspects related to stress under
elementary and secondary public school teachers Justification: The vocal
disorders affect each and every working class, however, they assume vital
importance and are highly incapacitating to the ones who use their voices as a
work tool, such as teachers. Stress is present on a regular daily basis of
scholastic work due to various social factors as well as modern work relations.
In this context, where vocal disorders (materialized in voice symptoms and in
laryngoscopic images) combine with the daily reality of teachers who face
stress often enough in their jobs, this current research has been conducted.
Theoretical and methodological aspects: The research was based on an
observational transversal prospective study. Data was collected and analyzed
taking into account the evaluation of 58 teachers in two elementary and
secondary municipal schools in Sorocaba/SP, which fulfilled the proposed
criteria of inclusion and exclusion. The teachers were submitted to: (1) self
assessment questionnaire on organizational questions, including questions
about inference from stress and violence in the school environment and from
voice/throat symptoms, called "Conditions of the Teacher s Vocal Production
CPV-P", which examined organizational issues, including the inference with the
stress and violence in the school environment and voice and throat symptoms;
(2) Larynx examination by image, consisting of a video-nasal laryngoscopy,
registered in digital media for further analysis and classification of existing
alterations, by professional doctors in the area of otolaryngology. Statistical
analysis of data were employed: the Pearson s Correlation, ANOVA-Analysis of
Variance Test and the test of Association Chi-Square were used. Results:
From the sample, 70.68% of subjects referred to, at least, one voice symptom.
The ones which highest prevalence were: hoarseness, weak voice and flaws in
the voice. At least one throat symptom was referred by 91.40% of subjects and
the most prevalent were: phlegm, effort and fatigue to talk. During the image
evaluation 91.37% of subjects showed at least one kind of disorder: evidence of
laryngopharyngeal reflux in 68.97%, supra-glottic constriction in 65.52%, vocal
cleft in 27.56%, and mass lesion in 32.75%. The results showed tendency of
teachers with higher inference to stress the greater number of symptoms
present in the throat, this does not occur with the vocal symptoms. There was
no correlation between level of stress and laryngoscopy changes / Objetivo: Analisar e relacionar a ocorrência de alterações laríngeas, sintomas
de voz/garganta e aspectos autorreferidos relacionados ao estresse em
professores do ensino fundamental e médio da rede de ensino público.
Justificativa: Os distúrbios que atingem a voz afetam todas as profissões, mas
assumem importância vital e são altamente incapacitantes para aqueles que
usam sua voz como ferramenta laborativa, como é o caso dos professores. O
estresse está presente na rotina diária docente em função de vários fatores
sociais e das relações do trabalho moderno. Dentro desse contexto, no qual se
cruzam os distúrbios da voz (materializados nos sintomas vocais e nas
imagens laringoscópicas) e a realidade diária do estresse, cada vez mais
presente na profissão de professor, foi idealizada esta pesquisa. Aspectos
teórico-metodológicos: A pesquisa consistiu em um estudo prospectivo
transversal observacional. Foram analisados os dados obtidos a partir da
avaliação de população de 58 professores de duas escolas de ensino
fundamental e médio da rede Municipal de Ensino do Município de
Sorocaba/SP, que preencheram os critérios de inclusão e exclusão propostos.
Os professores foram submetidos a: (1) questionário de autoavaliação,
denominado Condições de Produção Vocal do Professor CPV-P , em que
foram analisadas questões organizacionais, incluindo-se as com inferência ao
estresse e à violência no ambiente escolar e sintomas vocais e de garganta; (2)
exame da laringe por imagem, consistindo em uma vídeonasolaringoscopia,
registrada em mídia digital, para posterior análise e classificação das
alterações presentes, por profissionais médicos da área de
Otorrinolaringologia. Analisaram-se as relações entre o nível de estresse
inferido pelas respostas dos questionários com os sintomas referidos e os
achados laringoscópicos. Na análise estatística dos dados foram empregados:
a Correlação de Pearson, o teste ANOVA Analysis of Variance e o teste de
associação Qui-Quadrado. RESULTADOS: Da amostra, 70,68% dos sujeitos
referiram apresentar pelo menos um sintoma de voz. Os de maior ocorrência
foram: rouquidão, voz fraca e falhas na voz. Pelo menos um sintoma de
garganta foi referido por 91,40% dos sujeitos, sendo os de maior ocorrência:
pigarro, esforço ao falar e cansaço ao falar. Nas avaliações das imagens,
91,37% dos sujeitos apresentaram pelo menos um tipo de alteração: sinais de
refluxo laringofaríngeo, em 68,97%; constrição supraglótica, em 65,52%; fenda,
em 27,56% e lesão de massa, em 32,75%. A população estudada não
apresentou resultados que comprovassem intensidade elevada de inferência
ao estresse. Os resultados indicaram tendência dos professores com maior
inferência ao estresse a apresentar maior número de sintomas de garganta, o
mesmo não ocorrendo com os sintomas vocais. Não se encontrou correlação
entre nível de estresse e alterações laringoscópicas
